Northwest Hospital – Patient Care Management Team Empowering Employees Gets Results The care management team at Northwest Hospital is comprised of nurses and social workers who improve patient care by coordinating care and providing resources. “We wanted our case managers to feel more part of the decision-making process,” says Maria Manna, director of care management Patient Care Management Team at Northwest at Northwest. “So we created a scheduling committee that is responsible for organizing staff work hours, vacation and time- off requests.”
