Northwest Hospital – Patient Care Management Team Empowering Employees Gets Results The care management team at Northwest Hospital is comprised of nurses and social workers who improve patient care by coordinating care and providing resources.

“We wanted our case managers to feel more part of the decision-making process,” says Maria Manna, director of care management Patient Care Management Team at Northwest at Northwest. “So we created a scheduling committee that is responsible for organizing staff work hours, vacation and time- off requests.”

At LifeBridge Health, our mission is to improve the health of people in the communities we serve through compassionate, high-quality care. To do that, we need to work together and work efficiently.

But, as with any system, sometimes we can get bogged down in the process itself, and we don’t take the time to step back and ask why we do things the way we do.

So how can we work together to provide the best care possible to our patients and our communities?

On page eight of this edition of the Bridge, you can read more about our Operational Excellence (OPX) initiative, a systemwide approach to process improvement. Some LifeBridge Health teams have already begun to use OPX to make some impressive improvements that have benefitted patients as well as the staff themselves.

One of my favorite aspects of the OPX process is that it involves everyone. The best solutions come from the people who are actually doing the work, and OPX provides a framework for problem solving that gives you, our employees, the tools to make improvements in your area and across the system. It’s really very exciting.

One of the main goals of OPX is to create “a culture of continuous improvement,” which means that we are always striving to do better. Even if something is working well, it does not mean that it cannot be improved. OPX fosters creativity and innovation, and I can’t wait to see what we’ll be able to accomplish together.

And speaking of innovation, later this month, the Daily Record newspaper will honor LifeBridge Health with two recognitions in the annual Innovator of the Year awards. You can read more about these innovators on page 5.

These are just more examples of the great work that is taking place all across LifeBridge Health, thanks to you, our dedicated and hard-working employees. Thanks for all you do!

For our system to continue to grow and thrive it’s important for us to broaden our reach in the communities we serve; that’s why LifeBridge Health Partners is always looking for new opportunities to provide more comprehensive health care services to our patients.

One partner that is helping us with our goals is HomeCare Maryland LLC, a Medicare-certified home health agency that joined LifeBridge Health’s family five years ago. HomeCare Maryland sends experienced care providers – including physical and occupational therapists, skilled nursing professionals, dietitians, medical social workers and home health aides – to provide health services for people who are homebound.

Our shared values of providing high-quality compassionate health care and services have allowed LifeBridge Health to be a resource to HomeCare Maryland in patient care management, discharge planning and referrals. Through our collaboration, we have been able to identify patients who are at high risk for hospital readmission, and as a team we’ve helped to educate patients and their families on disease and medication management.

“By partnering with LifeBridge Health physicians and care management teams, we are better able to treat patients in the comfort of their own homes,” says Jonathan Binder, chief operating officer of HomeCare Maryland “and significantly reduce the frequency of potential avoidable re-hospitalizations."

Together, we’ve taken steps to become more efficient, and going forward our partnership with HomeCare Maryland will allow us to continue providing a full continuum of care for our patients.

The progress we’re making in my division is very exciting. We’re pulling together, removing barriers, opening clear channels of communication and working together. A lot of smart, invested and caring people are designing new ways for us to work to improve our systems and processes, and their enthusiastic response has been really inspiring.

For instance, the Medical Executive Committee will begin co-hosting town halls for clinical personnel from Sinai and Northwest hospitals. We expect that the conversations will lay the groundwork for creating new ways for us to collaborate, among both facilities, now and in the future.

Another area we’re collaborating on is in patient experience. As you know, our shared purpose is to provide high-quality patient care across the system, so we’ve created a new Patient Experience Governance Council to address the challenges we face and to inspire each other to find the solutions, together.

This coming together is our way of taking LifeBridge Health’s culture of teamwork and creating an army of intelligent, talented and creative problem solvers to challenge our traditional thinking and the status quo, giving employees a forum to express themselves and allowing one good idea to build upon another.

Those of you who know me know that I really enjoy collaborating, sharing ideas, engaging employees and finding solutions. After a hospital stay, patients and families often need additional care and resources — especially our aging population. Two programs that address this need, palliative care and hospice have grown tremendously over the past year, resulting in enhanced quality of life for patients, support for families, and decreased readmissions and hospital visits.

There’s some confusion about palliative and hospice care. The easiest way to remember the difference is that palliative care can occur at the same time as treatment for a chronic illness, while a patient is eligible for hospice care when he or she makes the decision to stop aggressive treatment and his or her physician believes the patient has a prognosis of six months or less to live.

Throughout LifeBridge Health, we offer palliative care services to support our patients and their families who are dealing with a serious illness, chronic disease or the frailty of advanced age.

Palliative care services include symptom management and comfort measures, educating the patient and family about the disease, treatment options and the importance of advanced care planning, and coordinating care and resources.

Carroll Hospice, a subsidiary of Carroll Hospital, and Seasons Hospice, a LifeBridge Health partner, provide skilled and personal end-of-life care tailored to individual needs and preferences. Services include care in the patient’s home, whether it’s a private residence, nursing home or assisted living facility or, when symptoms cannot be managed at home, at an inpatient facility. In Carroll County, we have Dove House, a beautiful inpatient facility that provides a home-like setting.

By combining careful pain and symptom management with spiritual and emotional support, hospice enables patients to live out the remainder of their days in comfort and dignity, surrounded by those they love. For families, hospice provides extensive grief resources, both before and after the loss of a loved one.

The steady growth of these programs is evidence that they are being embraced by our community as we help our patients receive the right care at the right time in the right setting.

Congratulations to Alyson Scilipote, Northwest’s SuperStar for the month.

Alyson, a physical therapist assistant, is an exemplary employee who consistently meets the mark, and goes above and beyond willingly and without complaint.

Admired and respected by her co-workers, it is not uncommon to have patients and staff frequently comment on Alyson’s professionalism and upbeat personality. Her commitment to providing excellent, quality care and her selflessness is an inspiration to her team.

Blaine Jones II is a nurse technician on the high intensity care unit (HICU).

He is described by his patients and co-workers as “one of the best” because he consistently

committee are able to get back to employees within a couple of days. Since the committee took over in January, the call-out rate for sick time or vacation is zero. Also, team members are given the option to extend their weekends by taking Fridays or Mondays off if they wish.”

Committee Participation at Sinai “We did a variety of things to promote employee engagement,” says Diane Bongiovanni, director of patient care services for Sinai’s emergency department, critical care, behavioral health and respiratory departments. “We encouraged our more than 500 staff members to get actively involved in committee work at the unit level, and once we posted the list we made sure that each employee had signed up for at least one committee.”

Blaine is always willing to help; whether he’s providing quality care or speaking a kind word, he is humble and is always concerned with providing positive outcomes for his patients. Every day, he goes to great lengths to make sure that his patients are comfortable and happy.

Krista worked with Jed Rosen, M.D., chief medical information officer, to change Paragon’s e-documentation Krista McElwain, R.D. process to allow voice recognition to be used instead of telephone dictation.

Voice recognition allows health care providers to dictate their documentations directly into a computer.

Their voice is translated into text, which can be reviewed and saved instantly into a patient’s medical record.

At the time, these changes were geared toward inpatients. When the telephone dictation service was unavailable, Krista took it upon herself to modify Paragon’s e-documentation process to allow voice recognition for patients using outpatient nutrition services as well. By doing so, Krista created a huge cost savings for the hospital.

Brian White, executive vice president of LifeBridge Health responsible for Sinai and Northwest Hospitals, has been honored as an Innovator of the Year for 2017 by The Daily Record. He earned this prestigious recognition for his work to invigorate the Liberty Road Corridor in Randallstown, home to Northwest Hospital, through a series of tactical real estate and construction projects, and by strengthening business and community partnerships.

“Brian White and his team have taken an innovative approach to modernizing their facilities and constructing new ones closer to major travel corridors to meet the needs of a changing health care system and the people they serve,” writes Baltimore County Executive Kevin Kamenetz in his letter of recommendation. “The result is more streamlined, convenient health and wellness services on a vibrant, expanded campus that has revitalized aging buildings and brought even more jobs to the community.”

Cultural Training Makes a Difference to Patients

Jennie Yudin a critical care tech in Sinai’s emergency department, realized that her co-workers were not familiar with the customs and traditions of Orthodox Jewish patients.

She approached Diane Bongiovanni about providing cultural training for her co-workers and was assured she had her department’s full support.

“From my perspective as a religious Orthodox Jew, we have made such strides in accommodating members of our Orthodox community,” says Jennie. “In the past I had worked with Rabbi Mitchell Ackerson to arrange for transportation and prescription pickup services for our Jewish patients, but I realized that a lot of employees were not familiar with the Orthodox religious observances, especially during the Sabbath when patients cannot use their call buttons, turn on and off their room lights or even open the door. The cultural training has made a huge difference to our patients. Staff now know that certain patients can only eat kosher food or when the Sabbath is, and the patients know that our team is willing to help take care of them without jeopardizing their religious practices.” (continued on page 7)

Aziza Shad, M.D., Named 2017 Innovator of the Year The Daily Record has recognized Aziza Shad, M.D., the Ellen W.P. Wasserman chair of pediatrics, director of The Herman & Walter Samuelson Children’s Hospital at Sinai and director of pediatric hematology/oncology, as one of its 2017 Innovators of the Year.

Dr. Shad was acknowledged for taking a proactive, innovative approach to meet the psychological and social needs of young patients while providing cutting-edge medical care. Her philosophy is that kids need to be kids first, not just patients, as they battle life-threatening illnesses.

Task Force Improves Communication in Carroll’s ED

with her eight-member team by email but replaced their monthly staff meetings with Learning Circles, inspired by Levindale’s Eden Alternative program.

Recently, to address the area of respect, the department created a task force to improve processes within the ED that impact patient flow (i.e., throughput). The task force, which included ED associates and emergency medicine physician Vanessa Redd, discussed current issues in the ED, then designed and implemented an action plan.

Some of the changes included completing patients’ workups and having them return to the waiting room while their tests were being processed, and only allowing one support person per patient in the exam room to limit crowding.

Learning Circles at Levindale

Desiree Erdman is the manager of the guest relations department at Levindale. Building on feedback from the engagement survey, she decided to continue communicating

The Eden Alternative program aims to create an environment that encourages patients and residents to celebrate life and create community with elements such as animals, activities, friendships and Learning Circles. Desiree explains how Learning Circles differ from a traditional meeting in several ways. “Every Learning Circle begins with a question, and each person is offered the chance to share their feelings. They can also opt not to share as well. There are certain rules too.

If someone is speaking, no one can interrupt them or ask a question until the person is finished speaking. Even though our department is small it has been really beneficial because every voice is heard.”

Adrienne Grey has been a receptionist with Levindale for 13 years. “I really enjoy attending the Learning Circles because we learn a lot of new information,” says Adrienne. “If we have concerns we can address them and have a dialogue together. It feels great to know that any situations that come up, Desiree will address and solve them. I really appreciate her and my team, and I really enjoy working here.”

Operational Excellence: Giving You the Tools to Build a Better Workday We all face challenges at work. But what if there was a way to fix some of those frustrations, so you could be more productive and feel more satisfied?

LifeBridge Health has launched Operational Excellence (OPX), a systemwide process improvement management system that teaches employees strategies to make positive changes in their work areas – improving efficiency, increasing quality, decreasing costs and, just as importantly, reducing frustrations and stress. OPX gives our more than 10,000 employees tools to build a better workday and work smarter, not harder.

“We see the OPX program as creating more than 10,000 problem solvers by providing our employees the training and tools to improve their day-to-day work environments. Employees who have been through the program say they feel energized and empowered because they, themselves, are the ones creating the improvements,” explains Tressa Springmann, LifeBridge Health senior vice president and chief information officer.

One Step at a Time Launching the OPX approach at a large organization such as LifeBridge Health is a huge undertaking, so it will take time before teams at all facilities learn the process.

OPX debuted officially at Northwest, and employees there have used the system to make improvements ranging from co-pays in the emergency department to turnover time in operating rooms.

Operational Excellence Won Us Over “Operational Excellence got everyone on the same page,” says S. Muna Jeter, nurse manager for Northwest’s intensive care unit. “Some people were a bit skeptical at first, but when we started using the tools, we started seeing positive results that benefitted both patients S. Muna Jeter and employees, then everyone really got on board. It’s been exciting to see the transformation.”

New Telesitting Program for Patient Observation

As part of our ongoing patient safety initiatives, LifeBridge Health has launched a new program called Telesitting. Carroll Hospital was the first to go live with the new program, followed by Sinai and Northwest hospitals and Levindale.

A remote visual monitoring system, Telesitting uses audio and video technology to monitor multiple patients simultaneously from one location with the goal of reducing the risk of patient injury.

“We are pleased to be able to offer this technology to help keep our patients safe,” says Stephanie Reid, chief nursing officer and vice president of patient care services at Carroll Hospital. “We’re especially happy to host the monitoring station for all four facilities at our hospital.”

OPX: Talking the Talk Operational Excellence comes with its own language, with terms ranging from “kaizen” and “A3” to “Non-Value Added Activity.” While it can seem overwhelming at first, employees who’ve been through OPX training say it soon becomes second nature.

“Employees may have friends or family members who have worked with process improvement systems and may have heard of ‘LEAN’ or ‘Six Sigma.’ The LifeBridge Health OPX program uses similar tools and principles, which have been used by businesses in all types of industries around the world for process improvement,” says Ryan Gruver, LifeBridge Health OPX director.

The Interprofessional Education Model Curriculum, a new program launched recently at Northwest Hospital, is bringing together medical, pharmacy and physician assistant students, to promote collaboration and to teach the benefits of interprofessional practice.

“Northwest Hospital is uniquely positioned to implement this initiative,” says Yuliya Klopouh, M.H., Pharm.D, manager of clinical pharmacy services, “because the culture of collaboration is in the roots of patient care here.”

The interprofessional education model was introduced to the staff by the Clinical Pharmacy Services department with the intent of bringing multiple clinical disciplines together to enhance the high-quality patient-centered care already provided at the hospital. In the past, students were taught to provide care independently, without the input of other disciplines. This curriculum and the model will help students be better prepared for “real life” hospital environments when they begin to practice their clinical skills.

Teams at Carroll and Levindale have also started to use OPX, with Sinai coming on board this fall. Employees at some outpatient physician practices, as well as departments such as human resources, are also using the OPX process.

Remembering Sept. 11

Congratulations to Dr. Oscar Taube who was recently recognized by the American Academy of Pediatrics (AAP) with its National AAP Special Achievement Award.

Dr. Taube (seen here with members of AAP’s Maryland chapter) received this recognition “for distinguished service and dedication to the missions and goals of the academy.” The academy also recognized Dr. Taube as “a tireless champion for adolescents.”

In a recent competition, the team rated highest in the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) scores on room cleanliness for the past two quarters (October to December and January to March). Associates were presented with the Regional Patient Experience Winner’s Cup, which has been placed on display in the hospital lobby.
