CÓMO USAR ESTE MANUAL Este manual ha sido escrito en inglés y español para que todos puedan tener acceso a los servicios de salud en el área central de Ohio. Tráigalo a sus citas para anotar todo lo referente a su consulta médica. Uselo para organizar la información médica sobre Ud. y su familia. Organizamos este manual alfabéticamente y por nivel de atención médica. La primera parte de cada sección es sobre Emergencias, para que Ud. sepa inmediatemente a quien llamar en caso de necesitar ayuda. Costo: $20 para un exámen completo de las ETS, $15 más para un Papanicolau, las pruebas del VIH son gratuitas (resultados están listos en approximadamente 10 días), $15 para una”Oraquick” o prueba rapida del VIH (se saca una gota de sangre del dedo), $50 para un análisis de sangre para herpes. HOW TO USE THIS MANUAL Algunos sitios de atención tienen horarios regulares y horarios para emergencias. Algunos servicios atienden a los pacientes sin cita, pero la mayoría requiren cita previa. Servicios: Los servicios en este libro son un resumen de lo que ofrece cada sitio de atención médica. This manual has been produced in both English and Spanish so everyone can access the health services in Central Ohio. Bring it with you to your appointments to write notes of health visits. Use it to help organize the medical information you already have for you and your family members. This manual is organized alphabetically by topic and level of service required. Emergency information is listed first under each topic so that you know immediately who to contact for help, followed by general services and then specialty services. The table of contents can help direct you to your topic of interest. For each listing, we provide the following information: hours of service, services provided, service costs, and whether or not an interpreter is available. Example: Columbus Public Health Sexual Health Clinic 240 Parsons Ave., 2nd Floor, Rm204A 645-7772 First come, first serve. Hours: M,Th,F 7:15AM - 3:30PM T 10:45AM - 7:00PM W 7:15AM - 12NOON Services: Pap smears, sexual transmitted disease (STD) testing, HIV testing and Counseling Cost: Medicare, Medicaid, Caresource accepted. Sliding fee-scale for all others. $15 additional cost for Pap smear. Both regular HIV tests (results in about 10 days) and “Oraquick” finger stick rapid HIV test (results in about 30min) are free. $50 for a herpes blood test. Treatment is free. No additional cost for consult. NOTE: Prices subject to change, depending on available funding. Interpreter: Available upon request. The doctor speaks Spanish.

Hours: Hours of operation are abbreviated as: Monday (M), Tuesday (T), Wednesday (W), Thursday (Th), Friday (F), Saturday (Sat), and Sunday (Sun) for the days of the week, followed by the open hours of service for the facility. On occasion, both the general business hours and emergency hours are posted. Some providers allow for walk-in visits, but most require appointments.

Interpreter: Many organizations provide an interpreter free of charge for their services, but it may be limited to specific days of the week or hours of the day. Some health providers have bilingual staff while others only have interpretive services on request. 6 DIRECTORIO DE SALUD / HEALTH DIRECTORY DATOS IMPORTANTES PARA PEDIR SUS CITAS Los sitios de consulta en salud esperan que Ud. traiga la siguiente información a cada cita para asegurarle una buena atención y determinar el costo de los servicios. Ud. debe guardar la siguiente información en un lugar seguro. De esta manera, usted estará preparado(a) para cualquier visita a los sitios de atención. Algunos sitios requiren información adicional que no está incluida en el siguiente listado. Favor preguntar a la persona que le da su cita la información que necesita. Para Ud. y miembros de su familia: Nombre completo (primero y segundo nombre y apellidos) o Fecha de nacimiento Identificación con foto (licencia para conducir, tarjeta de identificación, pasaporte, “Green Card”, Matrícula consular mexicana) Datos personales sobre alérgias a cualquier medicamento o comida Lista de vacunas y fechas de vacunación Número de seguro social, pasaporte o “green card,” (si está disponible)

CÓMO DISTINGUIR ENTRE EMERGENCIAS, “URGENT CARE,” Y CUIDADO BÁSICO Emergencias: Accidentes y enfermedades que necesitan atención inmediata y que ponen en peligro la vida de la persona, como dolor en el pecho, dificultad para respirar, hemorragia incontrolable, parálisis o debilidad de un lado del cuerpo, o dolor muy fuerte en el abdomen. LLAME AL '911' PARA OBTENER AYUDA INMEDIAMENTE. Usted será llevado a la sala de emergencias más cercana. “Urgent Care”: Para accidentes y enfermedades que necesitan atención médica el mismo día, pero no esta en riesgo la vida de la persona, como huesos rotos (fracturas) o infecciones en el oído. Algunos hospitales tienen los dos servicios, una clínica de “urgent care” y una sala de emergencia. Cuidado Básico: enfermedades no muy graves y consultas médicas para evitar enfermedades crónicas o agudas que necesitan atención continuada, no corre riesgo la vida de la persona, y no necesitan atención el mismo día. Todo el mundo debe tener su propio doctor (Médico Familiar) y recibir atención médica con cierta regularidad.

Se puede evitar visitas innecesarias a la sala de emergencias y “Urgent Care” asistiendo a los controles médicos regularmente. La atención en el servicio de Emergencias puede ser muy cara, aún si solicita y es elegible para Ayuda Financiera. El programa de Ayuda Hospitalaria -HCAP ( Hospital Care Assurance Program ) cubre los costos relacionados con el hospital. Las cuentas que corresponden a médicos privados no las cubre y pueden llegar a ser de cientos de dolares. Utilice el servicio de Emergencias solo para situaciones realmente graves, que pudieran ser cubiertas por AEMA (Alien Emergengy Medical Assistance).

IMPORTANT INFORMATION FOR APPOINTMENTS The health care provider will expect you to bring information with you to each appointment to ensure adequate care as well as determine the sliding fee scale (i.e., how much you pay for services). Below is a list that you should gather and keep in a safe place. This way you will be prepared for any health service visit. Some providers may require additional information not listed below. Please ask the person scheduling your appointment what information is required. For yourself and family members: Full Name (first, middle and last names) Date of Birth Photo identification (drivers license, ID card, passport, Green Card, Mexican identification) o Information about any allergies to medications or foods. List of vaccinations and dates received Social security number or passport or green card infor-

mation if available. DO NOT USE FALSE DOCUMENTS Insurance information (if available) Eyeglasses or contacts (for vision appointments) Residential information (some providers request copies of gas, water or electric bills, or proof of mortgage or rental receipt in the city of Columbus) Check stub from most recent paycheck to verify income.

HOW TO DISTINGUISH BETWEEN EMERGENCIES, “URGENT CARE,” AND PRIMARY CARE Emergency care: accidents and illness that need immediate attention and may be life threatening, such as chest pain, difficulty breathing, uncontrolled bleeding, one-sided paralysis or weakness, or severe abdominal pain. CALL '911' TO GET HELP IMMEDIATELY. You will be taken to the nearest emergency department. Urgent care: for accidents and illnesses that need attention that day, but are not life- threatening, such as brokenbones and ear infections. Some hospitals have both an urgent care and an emergency department. Primary care: regular illness and prevention check-ups for chronic or acute conditions that need on-going care, are not life-threatening, and do not require attention that day. Everybody should have a primary care physician and get regular medical attention.

Unnecessary trips to the ED and Urgent Care can be avoided with regular care Emergency Care can be quite expensive, even if you apply for and qualify for Financial Aid. H-CAP or Charity Care only pays for the hospital related costs. The physicians portion of the bill will not be covered and can be hundreds of dollars. Only use the Emergency Department for emergencies, for which AEMA may pay for your medical care.

Couple focuses on health, loses 147 pounds Like most Americans, Tom and Dorcas Winberg became careless with their eating habits. A slow but steady weight gain took place over the years, especially after they gave up smoking. Along with weight gain came medical problems associated with excess weight. The realization that they were “eating themselves sick” helped them decide to take responsibility for their health. Weighing over 300 pounds, Tom had high cholesterol and triglycerides, elevated liver enzymes, high blood pressure, and blood sugar that was creeping towards full-blown diabetes. Dorcas had elevated cholesterol and was diabetic. Despite using 106 units of insulin daily, she still had difficulty controlling her

To determine if you are eligible you will have an initial visit at the Contact Lens and Tear Film Lab at The College of Optometry. If enrolled, there are three visits total over a four week period.
