Dear Environmental Consults:

Effective December 1, 2017, the Los Angeles Fire Department (LAFD) CUPA Section will be transitioning to an electronic Soils Report submittal process. Soils Reports must be sent electronically to lafd.usttestnotify@lacity.org with "Soil Report" and the facility address indicated in the subject line of the email. We will no longer accept Hard Copy reports delivered in person or by mail, these reports will sent back. Please included the name, address and email address of the site’s responsible party and the name and email address of other person(s) that should receive a copy of the LAFD letter. A copy of the permit must be included with the report. If the report is a Business Initiated Soil Report, you will be contacted by our staff for payment and process of a Division 5 application. After the payment and application have been received, the report will be sent for review. Information regarding the electronic Soil Report process and Permit Application can be found at the following web address: lafd.org/fireprevention/cupa/documents-forms.
