WHAT’S NEW IN 2026
To provide competitive and meaningful benefits for employees, KUB benchmarks annually against other utility and non-utility companies. As part of this review, KUB evaluates its benefit plans to see how they are being used, how much they cost, how expenses are shared, and to look for ways to save money while also improving plans. Employees and their dependents help KUB continue to offer great benefits by actively participating in cost savings when utilizing in-network providers, generic prescriptions, surgery centers instead of hospitals, and other cost-conscious choices. Beginning in 2026, KUB is offering the following plan enhancements with minimal cost-share adjustments:
Medical: If an emergency room (ER) visit results in admission to the hospital, the ER copay will be waived. For those ER visits that do not result in admission to the hospital, the ER copay will increase slightly from $150 to $200 per visit. Please see BlueCross BlueShield (BCBS) Smart Consumer Information on page 25 for details on appropriate resources for various medical conditions.
Pharmacy: To allow for more options in filling your prescriptions, KUB will now cover 90-day prescriptions at both mail-order and retail pharmacies. This new option will add a third payment category for each drug tier: Retail 30, Retail 90, and Mail-Order. Please see page 7 for more details.
Dental:
● Dependent Child Eligibility Requirements – New criteria for dependents will remove the marriage, education, and financial requirements while also increasing the coverage age until the dependent’s 26th birthday. This change mirrors current dependent eligibility for Medical and Life.
○ Please Note – If your dependent child was previously dropped from Dental and/or Vision coverage and is now eligible, you must re-enroll them during Open Enrollment if you want their coverage reinstated.
● Preventive – Exams, cleanings, and X-rays are covered at 100% and do not apply toward the individual Annual Maximum Benefit.
● Annual Maximum Benefit – The Annual Maximum Benefit for each covered individual will increase from $1,500 per year to $2,000 per year.
● New Premiums – The monthly premium for both Employee Only and Family will increase by $3. New monthly premiums will be:
○ Employee Only - $3
○ Family - $51
Vision: New criteria for dependent children will remove the marriage, education, and financial requirements while also increasing the coverage age until the dependent’s 26th birthday. This change mirrors current dependent eligibility for Medical and Life.
Financial Wellness: KUB is excited to renew its partnership with Dave Ramsey’s SmartDollar. This benefit is fully paid by KUB and available to all employees - full-time, part-time, and students. Please see page 22 for additional details.

Open Enrollment Options
You can make the following changes during open enrollment, with elections effective January 1, 2026:
● Enroll, terminate, or change coverage for you or your dependents in the Medical, Dental, Vision, and Supplemental Life and Accidental Death & Dismemberment (AD&D) plans.
● Update your beneficiaries in the Life and AD&D, 401(k), and Pension benefits. Employees hired prior to 2011 are eligible for the Pension. Employees hired on or after Jan. 1, 2011, are eligible for the enhanced 401(k) benefit.
● If you do not submit changes, your benefit elections will remain the same for the next plan year, except for the Health Care and Dependent Care Flexible Spending Accounts (FSA). If you do not elect an FSA amount, the Health Care and Dependent Care FSAs will default to zero.
You must re-elect the Health Care and Dependent Care Flexible Spending Accounts each year if you wish to participate. If you do not elect an amount, the Health Care and Dependent Care FSAs will default to zero.

The choices you make during annual enrollment or as a new hire remain in place during the plan year (Jan. 1–Dec. 31). Teladoc is the telehealth provider through BCBST offering free 24/7/365 physician access for common medical conditions such as:

Teladoc is convenient if you are unable to see a doctor during normal business hours, want to avoid germs in a waiting room, or when you are on vacation, even out of state. Teladoc calls can be made via phone or video. The best part is Teladoc has a $0 copay!
You can also use Teladoc for mental health issues, dermatology, and nutrition.
Mental Health
Mental health offerings are driven by your preferences. You choose a therapist or psychiatrist based on their bios and specialties, then schedule a phone or video appointment. Appointments are available seven days a week, and this service is 100% confidential.
Dermatology
Dermatology care is handled through Teladoc by photo submission. Instead of a phone call, take a picture of your skin condition and start an online review with a board-certified dermatologist. A custom treatment plan will be sent to you via your Teladoc Health messaging center within 24 hours.
Examples of conditions that can be treated through Teladoc Dermatology are rashes, severe sunburn, poison ivy, insect stings, skin infections, moles or skin spots, dermatitis, dry skin, acne, eczema, psoriasis, and rosacea.
If you have a personal or family history of skin cancer, it is vital to see your current dermatologist if something concerning develops, like a new mole. Minor conditions listed above can go through Teladoc.

You can download the Teladoc App to your smartphone and register as a new user or go to the website –www.teladoc.com and click “Register Now.” You will need your legal name, date of birth, email address, and current ZIP code.
PRESCRIPTION DRUG COVERAGE
KUB’s Pharmacy coverage is through Express Scripts, a stand-alone pharmacy administrator. You can go to any Express Scripts network pharmacy and receive prescriptions at reduced rates. You may also use Smart90 or the mail-order prescription plan to get a 90-day supply of maintenance drugs, often at a lower cost than continuous 30-day refills at a retail pharmacy.

Specialty medications are drugs to treat complex conditions such as cancer, growth hormone deficiency, multiple sclerosis, and rheumatoid arthritis.
All specialty prescriptions have the option to go through Specialty Precision Network. Specialty Precision Network is a network of pharmacies that provides personalized care from trained pharmacists who specialize in managing your condition. Using a pharmacy in the Specialty Precision Network is required to gain access to steeper discounts up to a $0 copay. Specialty prescriptions filled through a pharmacy outside of the Specialty Precision Network are subject to coinsurance. GLP-1 Medications

How the Plan Works

FILING FOR REIMBURSEMENT
The Dependent Care Account
● Reimbursement claims for eligible daycare services can be easily submitted directly through the HealthEquity member portal at www.healthequity.com or through the HealthEquity mobile app.
● Documentation includes itemized bills or receipts, cancelled checks, or signed statements that include the provider’s name, dependents cared for, the amount charged, and the dates of service.
● You’ll receive reimbursement from HealthEquity on the next payment period via direct deposit or paper check ($2 fee for paper checks).
The Health Care Account
If you are shopping at a store with an Inventory Information Approval System (IIAS), which automatically recognizes FSAeligible items:
● Take all of your items to the register, including prescriptions, over-the-counter items, vision products, and any other purchases you may have.
● Present your FSA debit card; if required to select debit or credit, select credit, since no PIN is provided with this card.
● The system will approve the transaction for eligible items and deduct the amount from your elected contribution balance.
● Provide another form of payment for any items that are not eligible.
● Save your receipts. You may be required to submit receipts later to HealthEquity to verify that your purchase was eligible according to IRS rules. You can also upload your receipts through the HealthEquity mobile app to link your debit cards transactions and documentation.
● If you are shopping at a store without an IIAS, pay for your purchase with another type of payment and then submit your eligible expenses for reimbursement from your FSA by submitting a claim. To submit a claim, log into your account via the website or mobile app.
• Save your receipts, or upload them into the HealthEquity mobile app, in case HealthEquity needs them to verify purchase eligibility.

Life Insurance Core Coverage
Basic life insurance offers important financial protection for families of employees, so KUB provides basic coverage at no cost to all eligible employees. Employees can purchase additional life insurance.
● Basic life insurance is two times your base pay. In the event of your death, your beneficiaries will receive two times your annual base pay.
● Your basic life insurance includes an equal amount of basic accidental death and dismemberment (AD&D) coverage, which pays additional benefits if you suffer a covered loss or die as the result of an accident.
● The amount of coverage for basic life insurance for employees age 65 and over is reduced by 35% of the two times annual base pay.
● Maximum coverage for basic life insurance is $1,000,000.
Supplemental Life Insurance Coverage
KUB offers both KUB-paid life insurance and the opportunity to purchase additional life insurance coverage for you and your family members if you need it.
● On initial employment, you can purchase supplemental life coverage for 1, 2, 3, 4, 5, 6, or 7 times your base salary. Initial enrollment does not require evidence of insurability (EOI) for levels 1-5. Levels 6 and 7 will require completion of an EOI application.
● You may increase one level per year without evidence of insurability. If increasing more than one level, you must complete an EOI application.
● Supplemental life insurance is not reduced at age 65.
● Maximum coverage for supplemental life insurance is $1,000,000.
● Premiums are based on your age and base wage per $1,000 of coverage. Your enrollment form will show your individual premium cost.
Dependent Life Insurance
Your spouse and children can be covered by life insurance. You may make separate choices for your spouse and children. KUB offers both KUB-paid Accidental Death and Dismemberment (AD&D) insurance and the opportunity to purchase additional AD&D insurance coverage for you and your family members. AD&D insurance pays additional benefits for a covered loss or death as the result of an accident.
Supplemental AD&D Insurance Choices
● You may purchase optional AD&D coverage for 1, 2, 3, 4, 5, 6, or 7 times your base salary.
● Evidence of insurability is not required for AD&D coverage.
● Premiums for additional coverage are $0.030 per $1,000 of coverage. Your enrollment form will show your individual premium cost.
Imputed Income
Under federal laws, you are taxed on the value of employer-provided life insurance over $50,000 and any dependent life insurance.
Because you purchase supplemental life insurance with pre-tax dollars, it is considered employer-provided by the IRS. If your basic life insurance plus your supplemental life insurance gives you coverage above $50,000, imputed income will be determined. Because you purchase dependent life insurance with pretax dollars, imputed income will also be determined.
Dependent AD&D Insurance
You can cover your spouse and children with AD&D insurance. You may make separate choices for your spouse and children. Additional Offerings for Disability and Terminal Illnesses

DISABILITY COVERAGE
Statistics show that many people, especially those who are younger, are more likely to need disability coverage than life insurance. Disability coverage is designed to provide continuing income if you become ill or injured and are unable to work. This coverage includes ShortTerm Disability (STD) and Long-Term Disability (LTD). KUB provides STD and LTD coverage to all full- and parttime employees at no cost after 90 days of employment.
Short-Term Disability
● STD provides income protection after you take five consecutive workdays off if you become disabled from a non-occupational illness or injury.
● The maximum benefit is 60% of your base salary, paid to you on a weekly basis for a maximum of 90 calendar days from the date of disability.
● Continue to accrue PTO and receive holiday pay.
Long-Term Disability
● First five days of leave must be PTO
● Disability income can be supplemented with PTO or SIB to bring income up to 100%
● PTO and SIB may be used to supplement for benefit premiums only
● If not supplementing at all, benefit premiums will be billed to the employee monthly
● LTD provides income protection after you have been disabled for more than 90 calendar days.
● LTD coverage provides you with total disability income of 60% of your monthly base wage, paid to you on a monthly basis.
● LTD works together with benefits available from other sources, such as Social Security, Workers’ Compensation, Veterans Administration disability, and pension benefits to replace 60% of your pay.
● You must apply for any and all disability income benefits that you may be eligible for. Once approved, you must advise KUB of all benefits you receive.
● All employees who qualify for a LTD benefit will receive a minimum of $50 per month for the first 24 months, regardless of any other benefits they receive.
● No longer accrue PTO or receive holiday pay.

Family & Medical Leave (FMLA)
Have you had to miss work for a personal illness or to care for a family member? Have you had a new addition to the family through birth or adoption? Do you have an immediate family member who has been called to active duty? You may be eligible, after one year of employment, for Family & Medical Leave (FMLA). PET INSURANCE
KUB offers pet insurance through MetLife, similar to health insurance for you and your family. Pet insurance is coverage for dogs, cats, and exotic pets that can help prepare you for unexpected vet costs.
What you need to know
MetLife Pet insurance provides accident and illness coverage. In the event your pet has an accident or becomes ill, your coverage will help cover that cost. Examples of what can be covered: accidental injuries, illnesses, exam fees, surgeries, medications, ultrasounds, hospital stays, and X-rays or diagnostic tests. Pre-existing conditions are excluded.
How it works
1. Sign up for the plan that fits your needs. Plans are customizable (see below).
2. Take your pet to a U.S. licensed vet, emergency clinic, or specialist and pay your bill. Be sure to ask for a copy of your pet’s SOAP (Subjective, Objective, Assessment, and Plan) notes for the visit.
3. Submit your invoice to MetLife for reimbursement, along with a copy of the SOAP notes. You can submit these items through the mobile app, online portal, email, fax, or mail.
4. After meeting your deductible, you will be reimbursed the percentage amount you elect for your plan, up to the maximum annual benefit. Once your claim is processed, you are reimbursed by check or direct deposit.

PAID TIME OFF (PTO)
PTO that you accrue is available for you to use for vacation, sick, and personal time. Part-time employees working at least 20 hours, but less than 40 hours per week, earn prorated PTO based on their standard work hours.

Bereavement
KUB recognizes the emotional and personal impact of losing a loved one. To support employees during these challenging times, KUB offers bereavement leave to allow time for mourning, attending services, and handling family matters. Employees are entitled to up to twenty-four hours across three consecutive days of paid bereavement leave in the event of the death of an immediate family member. Employees should notify their supervisor or manager and the Compensation Team as soon as possible when bereavement leave is needed. PTO Donation Program
The PTO Donation Program allows employees to donate accrued PTO hours to fellow employees experiencing a personal or family medical emergency and who have exhausted all accrued PTO and SIB.

RETIREMENT BENEFITS
KUB’s Retirement Program offers competitive, secure, and flexible retirement benefits. Contact the Benefits Team at 865-558-2452 when you plan to retire. There are special rules that apply to your benefits when you retire.
● For employees hired prior to 1/1/2011: The Retirement Benefit consists of the Guaranteed Pension Benefit (GPB) and the 401(k) Plan with company match.
○ KUB pays the Pension, which is based on a number of employment factors.
○ You receive a match of 50% on the first 6% you contribute to the 401(k) Plan.
● For employees hired on or after 1/1/2011: The Retirement Benefit consists of the 401(k) Plan with company match and a non-elective contribution.
○ You receive a match of 50% on the first 6% you contribute to the 401(k) Plan.
○ KUB pays the non-elective contribution (regardless of individual contributions), which is based on your years of service:

Additional Post-Retirement Benefits
● For employees hired prior to 7/1/1999: KUB offers retiree medical coverage to employees who were hired prior to July 1, 1999, have at least 20 years of service, and meet the Rule of 80 (age plus years of service = 80)
● For employees hired on or after 7/1/1999: KUB also offers a $50,000 Retiree Health Reimbursement Arrangement (HRA) to employees who were hired after July 1, 1999, have at least 20 years of service, and meet the Rule of 80 (age plus years of service = 80)

If no beneficiary is designated upon death of the participant, benefits are paid to the participant’s estate. Your benefit could be paid to an incorrect beneficiary if you have not made changes since a marriage, divorce, birth, death, etc.

Changing Your 401(k) Contributions:
Step 1: On the “Employee Benefits” homepage, find “Accounts and Benefits” at the top of the page, and select “Manage contributions” under “Popular tasks.”
Step 2: On the “Contributions” tab, click once on “Contribution Amount” beside the first green arrow.
Step 3: You should now be on the “Contribution Amount per Pay Period” page. Under each section in the “Contribution Amount” box, update the current number to the contribution percentage of your choice in the “Desired Election” row. When finished, scroll to the bottom of this page, and click the “Change Contribution Amount” box.
Step 4: On the following page, review your changes, and click “Submit.”
Updating Your Beneficiaries:
Step 1: On the “Employee Benefits” home page, select the “Profile and Settings” icon in the upper right corner.
Step 2: On your “Profile” page, click on the “Beneficiaries” link.
Step 3: You should now be on “Your Beneficiaries” page. Click the gray “Manage” button to the right of the status or plan beneficiary you would like to change. Make changes as necessary.

You may enroll online through Employee Self Service in HRIS or use the paper enrollment form in your packet to select your options and desired coverage. We encourage you to enroll on Employee Self Service. It’s quick, easy, and helps to minimize potential errors from inputting data from a paper form into the electronic system. You cannot complete beneficiary changes online.
NOTE: New employees hired after Open Enrollment cannot enroll online. They must use the forms and instructions provided during orientation. They will have 31 days from their date of hire to enroll.

Step 1: Type HRPROD/ into your Internet browser using either Google Chrome or Mozilla Firefox (not Internet Explorer).
Step 2: Sign in using your Network ID (the initials in your ID must be upper case) and password. If you do not have a password, call the Help Desk at ext. 2001.
Step 3: Go to the Employee Quick Links section at the top left of the home page and click on the Benefits Enrollment link. Use the “select” button to begin your enrollment.
Step 4: The Enrollment Summary page displays the benefits options available for update. If you want to keep everything the same as last year, simply click on “I Have No Changes.”
Remember: Health Care and Dependent Care FSAs default to $0 unless you enter a new amount. If you do not want to contribute for the upcoming year, click OK.
Step 5: If you want to change one of your benefits, click the “Edit” button next to the benefit.

BlueHealth Rewards: BlueHealth Rewards is linked to your online BCBS account, and it allows you to track and earn points for things you already do, like maintaining healthy biometric measures and engaging in physical activity. Ergonomic Program: The industrial athletic trainer can provide specialized ergonomic evaluations and education to employees in all work groups. Contact Occupational Health at 865-558-2621 to learn more about the program and the opportunity for an individual ergonomic assessment.
Wellness Initiatives: KUB recognizes employees annually for their involvement with the Healthy Habits wellness program through monetary incentives. Contracted Occupational Health resources offer on-site biometric screenings, vaccines, mobile mammography, etc.
Mother’s Rooms: KUB offers four mother’s rooms across KUB campuses that provide a private space for breastfeeding mothers. Contact Occupational Health at 865-558-2621 to learn more or to gain access.

Employee Assistance Program (EAP)
EAP counseling is free and completely confidential. Employees and dependents are entitled to six counseling sessions per issue to help them deal with topics of a personal or psychological nature, including those listed below:

● Family & marital problems
● Healthy lifestyles
● Work/life balance
● Depression/anxiety
● Retirement preparation
● Child/elder/adult care issues

GLOSSARY OF TERMS
As you are reading through this guide, here are some terms that may be helpful to you:
Coinsurance: A percentage of each claim paid by the individual, after the deductible is met. For a 20% coinsurance clause, the individual pays for 20% of his/her claims, after the deductible has been met.
Co-Payment (Copay): A small, flat fee an individual pays for health care services. This is normally paid by the individual at the time of each office visit or filling of a prescription.
Deductible: An amount the individual will pay before the insurance begins to share the cost of claims each year. KUB’s deductible applies toward coinsurance services (such as hospital inpatient or outpatient visits). The deductible does not apply for copay visits or preventive services.
Evidence of Insurability (EOI): An application process in which you provide additional medical information for yourself or your spouse to the insurance carrier for review before approval can be determined.
Out-of-Pocket Maximum: A flat limit on what you will pay for health care in a plan year through your deductible, coinsurance payments, and copays. After the out-of-pocket maximum is met, the plan will pay 100% for an individual’s health care expenses (excluding premiums and prescriptions) for the remainder of the calendar year.
Generic Prescription: A medication whose active ingredients, safety, dosage, quality, and strength are identical to that of its brand-name counterpart.
Non-Preferred Brand Prescription: A medication that has been determined to have an alternative drug available that is clinically equivalent and less expensive.
Preferred Brand Prescription: A medication that has no clinically equivalent alternative.
Premium: The amount of money that KUB collects each month from employees and retirees, when applicable, to provide coverage described in the policy for a specified period of time.
Specialty Medications: Drugs that are used to treat complex conditions, such as cancer, growth hormone deficiency, multiple sclerosis, or rheumatoid arthritis.
Out-of-Pocket Maximum
When an individual covered under the medical plan reaches his or her individual OOP maximum, all covered expenses for that individual will be paid at 100%, even when the family OOP maximum may not have been reached yet. When the family OOP maximum is reached, the plan will pay 100% of all covered expenses regardless of whether each person covered has reached the individual OOP maximum. Example: Family of four with the KUB medical OOP maximums of $1,500 per individual and $3,000 per family.
