Its purpose is to proactively recognize and remove probable failures and dangers that are unrecognizable. A multidisciplinary approach should be utilized for quality improvement.

AFT2 Accreditation Audit Task 3 Task 3-Patient Tracker Summary A.1. The admission assessment is the essential baseline assessment which commences the course of action of assessment, diagnosis, planning, intervention, and evaluation. A vital initial step in any patients care is the admission assessment.

Throughout this process, the patient is assessed by the nurse from head to toe in order to determine a baseline assessment. The initial assessment upon admission is the point of reference for all other assessments the patient will have during their hospital stay. This will provide the nursing staff a comparison of the patient’s condition so they can determine if there is a deviation in the patient’s condition.

A.2. The Corrective Action Plan will result after the discovery of a problem. We will set a goal to devise a user friendly form for the nursing staff. The corrective action plan should include: 1. 1. User friendly that utilizes an uncomplicated flow of information 2. 2. Display professionalism in the typesetting that is required 3. 3. Include a section that provides a summary for the team members can review.

Requirements for accreditation must be incorporated into the assessment so each component of that assessment carries more important components.

AFT2 Accreditation Audit Task 4 A.1.

The purpose in this periodic performance review, or PPR, is to establish if Nightingale Community Hospital is prepared for an accreditation audit. There are several standards that are outlined by the Joint Commission that the organization is

presently compliant with. The Emergency Department has proven its compliance in regards to their implemented plans on how emergencies are responded to. The Human Resources department has verified compliance by their presentation and their dedication in appropriately training staff and providing continuing education.

Nightingale Community Hospital has numerous standards that failed compliance.

For instance, in the standard involving the Environment of Care, the organization failed to prove that it could be a secure and the setting itself was functional. The Life Safety standard was not met due to its inadequate fire safety. The Leadership standard also was not met due to staffing insufficiencies.

There is a link between the amount of staff and their experience with the kind of care and how safe that care is being provided to patients. The organization has failed to respond to the need for more medical staff which in turn has led to the demise of the conditions of the working environment.

A vital step that can be taken to decrease the amount of falls is to devise a staffing plan that is efficient in emphasizing strengths and limitations of the nurses.

Developing a staff committee would be a great start in the development and implementation of a staffing plan.
