Yayın İlkeleri

1. Türkiye Acil Tıp Dergisi, Türk Tıp Dizini koşullarına uygun olarak bir yıl içindeki toplam araştırma makalesi sayısı 15’den az olmayacak ve toplam makale sayısı (araştırma makalesi, olgu sunusu, editöre mektup, derleme) en az %50’sini oluşturacak şekilde yılda 4 kez yayımlanır. Her sayıda en az 4 araştırma makalesi yayınlanır. Dergimiz araştırma makalesi dışında, editör kurulunun görüşü dahilinde olgu sunum, olgu serisi, acil tıpta görsel tanı, editöre mektup, kısa rapor, derleme ve kanıta dayalı acil tıp kategorisinde yazılar yayınlar. Derlemeler editörün daveti üzerine hazırlanır.

2. Derginin elektronik arşiv sisteminde tüm hakem kararları, başvuru yazılarının imzalı örnekleri ve düzeltme yazıları en az beş yıl süreyle saklanır.

3. Türkiye Acil Tip Dergisi’ne gönderilen yazılar ilk olarak editör tarafından değerlendirilir. Editör her yazıyı değerlendirmeye alınıp alınmaması konusunda yazıyı gözden geçirir, gerekli görürse yazıya editör yardımcısı atar. Editör ve yazıya atanan editör yardımcısı en az iki hakem veya bir hakem ve bir editör yardımcısına yazıyı değerlendirmek üzere gönderir. Eğer her iki editör de yazının bilimsel değerliliğinin ve orijinalliğinin bulunmadığı ve acil tıp konusuna ve dergi okuyucu kitlesine hitap etmediğini düşünüyorsa yazıyı hakem kuruluna göndermeden direkt red edebilir.

4. Yazıların değerlendirilme sürecinde yazarların başvurularına yazının hakemlerce değerlendirme altına alındığına dair ilk yanıt için 14 gün, hakemlerin ilk değerlendirmesi için 21 gün, ayrıntılı değerlendirme sonucunun yazarlara bildirilmesi için 28 günlük süreye uyum amaçlanır. Dar kapsamlı düzeltme gereken yazılar için yazara 10 gün, geniş kapsamlı düzeltme gereken yazılarda yazarlara 20 günlük süre tanınır. Yazının basım için kabul edilmesinden sonraki 30 gün içerisinde basımevi editöründen yazarlara baskı öncesi son düzeltme önerileri gönderilir.

5. Editör yardımcısı metodolojik bir sorunu değerlendirme safhasında “araştırma metodolojisi editörlerine” danışabilir. Böyle bir danışma yapılmasa bile eğer yazı hakemler tarafından kabul edilmişse kabul yazısı gönderilmeden önce editörün isteği üzerine araştırma metodolojisi editörünün onayına sunulabilir.

6. Türkiye Acil Tıp Dergisi’ne gönderilen ve içerisinde ingilizce bölümler bulunan tüm yazılar, derginin dil editörü tarafından, basım öncesinde değerlendirilir.

7. Türkiye Acil Tıp Dergisi’nde yayınlanacak yazıların biyomedikal dergilerin ortak ihtiyaçlarını karşılaması ve Helsinki deklarasyonuna uyması aranır.

8. Dergide yayımlanan makaleler, içindekiler sayfasında ve makale başlık sayfalarında türlerine göre (araştırma, olgu sunusu, derleme gibi) sınıflandırılarak basılır.

9. Dergi basımında asitsiz kâğıt kullanılır.

10. Reklâmlara makale içinde yer verilmez.

11. Türkiye Acil Tıp Dergisi editörü veya editörleri Türkiye Acil Tıp Derneği Yönetim Kurulu tarafından, her yıl Ocak ayında belirlenir. Dergi yayın kurulunu editörler, editör yardımcıları, araştırma metodolojisi editörü ve dil editörü oluşturur.

12. Türkiye Acil Tıp Dergisi’nde yayımlanan tüm yazıların yayın hakkı Türkiye Acil Tıp Derneği’ne aittir. Bu dergiden kaynak gösterilmeden alıntı yapılamaz ve derginin hiçbir bölümü izinsiz çoğaltılamaz. Dergide yayımlanan yazılardaki görüşler, tümüyle yazarlarına aittir.

Publishing with the Tr J Emerg Med

1. The Turkish Journal of Emergency Medicine (Tr J Emerg Med) is published four times per year. The total number of original research articles is 15 per year and research articles (including original research, case studies, letters to the editor and reviews) constitute at least 50% of the published material. Every issue published will contain a minimum of 4 research articles. Apart from the research articles, Tr J Emerg Med also publishes articles in the categories of case studies, case series, visual diagnoses in emergency medicine, letters to the editor, brief reports, reviews and evidence based emergency medicine in consultation with the editorial board. Reviews are presented upon invitation from the editor.

2. All reviewer comments, signed copies of manuscripts and corrections will be kept in digital format in the journal archives for a minimum period of 5 years.

3. The submitted manuscripts are first reviewed by the journal’s editor who determines whether the manuscript deserves further evaluation or not. For submissions that are granted further evaluation, the editor assigns the manuscript to one of the assistant editors.

The editor and the assistant editor then forwards the manuscript to two reviewers or one reviewer and a member of the scientific board for evaluation. If both the editor and the assistant editor determines the manuscript is not scientifically valuable or not an original work, or if it does not relate to emergency medicine or does not address the journal’s target audience, then they reject the manuscript directly without forwarding it to the reviewers.

4. The goal of the Tr J Emerg Med is to notify the authors with the acceptance of their submission for peer review within 14 days, peer review period of 21 days and final evaluation and notification of 28 days from the receipt of the manuscript. The authors are given 10 days for minor revisions and 20 days for major revisions. The final page layout is provided to the authors within 30

days of the acceptance of the manuscript for publication, for final review and proof.

5. The assistant editor may consult the research methodology editor to clarify any problems in the statistical design and evaluation of the study during the peer review process. Even if such consultation is not sought during the review process, it can be implemented upon request of the editor in chief prior to the final acceptance of the manuscript.

6. All manuscripts containing material written in English will be evaluated by the language editor before the manuscripts are considered for publication.

7. Manuscripts submitted to the Tr J Emerg Med are expected to conform with the Helsinki Declaration and meet the common requirements of the biomedical journals.

8. Articles are listed on the content page and are published in appropriate sections (original research, case report, review, etc.).

9. The journal is printed on acid-free paper.

10. Advertisements are not allowed within articles.

11. The editor(s) of the Tr J Emerg Med are elected by the Board of the Emergency Medicine Association of Turkey once a year in January. The Tr J Emerg Med board consists of editor(s), assistant editors, a research methodology editor and a language editor.

12. All material published in the Tr J Emerg Med are the property of the Emergency Medicine Association of Turkey. This material may not be referred without citation nor may it be copied in any format. Authors are responsible for all statements made in their articles.

Yazarlara Bilgi Türkiye Acil Tıp Dergisi, Türkiye Acil Tıp Derneğinin resmi bilimsel yayın organıdır, ulusal ve uluslararası makaleleri yayımlayan, ön değerlendirmeli bir dergidir. Yayın hayatına 2000 yılında başlayan ve bu konuda ülkemizde yayınlanan ilk dergi olan Türkiye Acil Tıp Dergisi, Türk Tıp Dizini, EBSCOHost, Index Copernicus, DOAJ, Gale/Cengage Learning, SCOPUS, EMBASE ve Türkiye Atıf Dizini’nde indekslenmektedir. Türkiye Acil Tıp Dergisi acil tıp ve acil sağlık hizmetleri konusunda bilimsel içerikli araştırma makaleleri, olgu sunumu, olgu serisi, acil tıpta görsel tanı, derleme, kısa rapor, kanıta dayalı acil tıp makaleleri, tıbbi düşünceleri ve ilgili bilimsel duyuruları yayınlar. Dergi içeriğindeki temel bölümler acil tıp sistemleri, akademik acil tıp, acil tıp eğitimi, acil servis yönetimi, afet tıbbı, çevresel aciller, travma, resusitasyon, analjezi, pediatrik aciller, tüm tıbbi aciller, hastane öncesi bakım, toksikoloji, acil hemşireliği, sağlık politikaları, etik, yönetim, görüntüleme ve prosedürlerdir.

Türkiye Acil Tıp Dergisi’nde yayınlanacak yazılarda öncelikle biyomedikal dergilerin ortak ihtiyaçlarını karşılaması ve Helsinki Deklarasyonu’na uyum aranmaktadır. Bu konu hakkındaki bilgiler aşağıdaki makaleden elde edilebilir.

KATEGORİLER Araştırma Makaleleri: Acil tıp konusunda yapılmış temel veya klinik araştırma makaleleridir. Türkçe ve İngilizce özet gereklidir. Giriş, gereç ve yöntem, bulgular, tartışma, kısıtlılıklar ve sonuç bölümleri içermelidir. En fazla 4000 kelime, altı tablo veya resim içerir. Tek merkezli çalışmalar için yazar sayısı en fazla sekiz kişi ile sınırlıdır. Çalışmaların yayımlanabilmesi için etik kurul onayı gereklidir. İlaç çalışmaları için ise merkez etik kurul onayı gereklidir.

Olgu Sunumları: Acil tıpta nadiren karşılaşılan, eğitimsel yönü olan, hekimlere mesaj veren klinik vakaların veya komplikasyonların sunumudur.

Bu bölüme yayın için gönderilen yazılarda daha önce bilimsel literatürde bildirilmemiş klinik durumları, bilinen bir hastalığın bildirilmemiş klinik yansımaları veya komplikasyonlarını, bilinen tedavilerin bilinmeyen yan etkilerini veya yeni araştırmaları tetikleyebilecek bilimsel mesajlar içermesi gibi özellikler aranmaktadır. Türkçe ve İngilizce özet gereklidir. Giriş, olgu sunumu ve tartışma bölümlerini içerir. Makalenin uzunluğu 1500 kelime ile sınırlıdır. On beş veya daha az kaynak, iki tablo veya resim içermelidir. Yazar sayısı en fazla beş kişi ile sınırlıdır.

Olgu Serileri: Acil tıpta nadiren karşılaşılan eğitimsel yönü olan klinik vakaların veya komplikasyonların sunumudur. Yediden fazla olgunun sunulması gereklidir. Türkçe ve İngilizce özet gereklidir. Giriş, olgu sunumu ve tartışma bölümlerini içerir. Makalenin uzunluğu 2500 kelime ile sınırlıdır. On beş veya daha az kaynak, üç tablo veya şekil içermelidir. Yazar sayısı en fazla altı kişi ile sınırlıdır. Özet Raporlar: Ön çalışma verileri ve bulguları, daha ileri araştırmaları gerektiren küçük olgu sayılı araştırmalardır. Türkçe ve ingilizce özet gereklidir.

Giriş, metod, bulgular, tartışma, kısıtlılıklar ve sonuç bölümlerini içermelidir.

En fazla 4000 kelime, dört tablo veya resim içerir. Tek merkezli çalışmalarda yazar sayısı en fazla 6 kişi ile sınırlıdır. Etik kurul onayı gereklidir. İlaç çalışmaları için ise merkez etik kurul onayı gereklidir.

Konseptler: Acil tıp ile ilgili ve acil tıp bilimini geliştirmeye yönelik klinik veya klinik olmayan konulardaki yazılardır. Türkçe ve İngilizce özet gereklidir. Makalenin uzunluğu 4000 kelime ile yazar sayısı ise en fazla 3 kişi ile sınırlıdır. İngilizce ve Türkçe özet içermelidir.

Derleme Yazıları: Acil tıp konusundaki güncel ulusal ve uluslar arası literatürleri içeren geniş inceleme yazılarıdır. Türkiye Acil Tıp Dergisi davetli derleme makaleleri yayımlamaktadır. Davetli olmayan derleme başvuruları öncesinde editör ile iletişime geçilmelidir. En fazla 4000 kelime olmalıdır. Kaynak sayısı konusunda sınırlama yoktur. Yazar sayısı en fazla 2 kişi ile sınırlıdır.

Kanıta Dayalı Acil Tıp (Evidence-Based Emergency Medicine): Klinik ve tıbbi uygulamalara yönelik sorulara yanıt verebilen makalelerdir. Makale; klinik senaryo, soru (veya sorular), en iyi kanıtın araştırılması, en iyi kanıtın seçilmesi, kanıtın analiz edilmesi ve kanıtın uygulanması bölümlerini içermelidir. En fazla 4000 kelime olmalıdır. Türkçe ve İngilizce özet gereklidir.

Yazarlar kullandıkları makalelerin kopyalarını da ekte editöre göndermelidir. Yazar sayısı en fazla 4 kişi ile sınırlıdır.

Acil Tıpta Görsel Tanı: Görsel materyali eğitici ve ilgi çekici olan, kısa olgu sunumlarıdır. Yazı 2 bölümden oluşur. İlk bölümde olgu kısaca özetlenir ve görsel materyal sunulur. Bu bölümde görsel materyalin tanısı okurla paylaşılmaz. İkinci bölümde tanı başlık olarak verilir, olgunun yönetimi ve görsel materyalin özellikleri kısaca tartışılır. En fazla 500 kelime ve 5 kaynak olmalıdır. Yazar sayısı en fazla 2 kişi ile sınırlıdır. Özet gerekli değildir.

Editöre Mektup: Acil tıp ile ilgili konulardaki görüşler, çözüm önerileri, Türkiye Acil Tıp Dergisinde veya diğer dergilerde yayımlanan makaleler hakkında yorumları içeren yazılardır. En fazla 1000 kelime ve 5 kaynak ile sınırlıdır. Tek yazarlı makalelerdir. Özet gerekli değildir.

MAKALE BAŞVURUSU Türkiye Acil Tıp Dergisi’ne makale başvuruları çevrimiçi olarak kabul edilmektedir. Türkiye Acil Tıp Dergisi’nin web sayfasında “çevrim içi makale gönder” sekmesini kullanarak makale başvuru yapılabilir.

MAKALE HAZIRLAMA DETAYLARI Türkçe ve İngilizce Özet: Orijinal makaleler, kanıta dayalı acil tıp ve özet raporlar için 250 kelime ile sınırlandırılmış özet gereklidir. Orijinal makaleler ve özet raporlar için amaç, gereç ve yöntem, bulgular ve sonuç olmak üzere 4 bölümden oluşmalıdır. Olgu sunumları için Türkçe ve İngilizce özet 150 kelimeyi aşmamalıdır.

Araştırma Makalelerinin İçeriği: Araştırma makaleleri aşağıdaki bölümleri içermelidir; Giriş: Üç paragraf halinde yazılmalıdır. Yazarlara Bilgi Gereç ve Yöntem: Araştırma makalelerinin en önemli bölümlerinden birisi olan metod, olabildiğince detaylı yazılmalıdır. Çalışmanın yöntemi, çalışma örneklemi, analiz yöntemleri, kullanılan ticari istatistik program, ölçme ve değerlendirme yöntemlerinin detayları (örneğin biyokimyasal testler için cihaz ve test kiti markası) açıkça belirtilmelidir. Çalışmaya alma ve dışlama kriterleri maddelenmelidir. Anket çalışmalarında, anketin kim tarafından nasıl doldurulduğu belirtilmelidir.

Bulgular: Çalışma popülasyonun demografik özellikleri, çalışma hipotezini sınayan ana ve yan sonuçlar önem sırasına göre verilmelidir. Bu bölümde sonuçlar hakkında yorum yapılmasından ve literatür bilgilerinin tartışılmasından kaçınılmalıdır. Sonuçların grafikler, ortalama, ortanca, dağılım ve %95 güven aralığı ile sunulmasına özen gösterilmelidir.

Tartışma: Çalışmanın ana ve yan sonuçları kısaca belirtilir ve literatürdeki benzer örnekler ile karşılaştırılır. Yoğun ve çalışmanın konusu dışındaki gereksiz ansiklopedik bilgi paylaşımından kaçınılmalıdır.

Kısıtlılıklar: Çalışmanın kısıtlayıcı faktörleri, tartışma bölümünün sonunda “kısıtlılıklar” alt başlığı ile mutlaka belirtilmelidir.

Sonuç: Çalışmanızdan ortaya çıkan veriler ışında, dergi okurları için uygun ve net bir sonuç çıkarılmalıdır. Bir cümle ile çalışma sonucunun, mevcut klinik uygulamalar üzerine olası etkileri belirtilmelidir. Çalışma verilerinin desteklemediği çıkarımlardan uzak durulmalıdır.

Genel yazımda dikkat edilecek noktalar İstatistiksel testler: Çalışmalar istatistik alanında deneyimli kişilerin kontrolünde analiz edilmelidir.

Değerler: Kullanılan madde, ilaç, laboratuvar sonuçları değerlerinde genel standartlara uyulmalıdır. Laboratuar sonuçlarının normal aralıkları belirtilmelidir. İlaçlar: Jenerik isimler kullanılmalıdır. İlaçların dozu, uygulama yolu belirtilmelidir.

Türkçe kullanımı: Türkçe terminolojinin kullanılmasına (“Hospitalizasyon” yerine “hastaneye yatış”, suicid yerine “özkıyım” kullanımı gibi) ve Türkçe yazım kurallarına uyulmasına özen gösterilmelidir.

Kaynaklar: Kaynaklar çift aralıkla yazılmalı ve makalenin sonunda yer almalıdır. Kaynakları makale içinde kullanım sırasına göre numara verilmeli, alfabetik sıralama yapılmamalıdır. “Abstract” olarak faydalanılmış makaleler için parantez içinde “abstract” yazılmalıdır. İstatistik Danışmanı: İstatistiksel analiz içeren tüm makaleler istatistik konusunda deneyimli bir uzmana danışılmış olmalıdır. Instructions for Authors Tr J Emerg Med is the official publication of the Emergency Medicine Association of Turkey. It is a peer-reviewed journal that publishes national and international articles. Founded in 2000, it is the first journal of its kind in Turkey and is indexed in the Turkish Medical Index, EBSCO Host, Index Copernicus, DOAJ, Gale/Cengage Learning, SCOPUS, EMBASE and Turkiye Citation Index. Tr J Emerg Med publishes articles relevant to emergency medicine and emergency medical services such as; scientific research, case reports, case series, visual diagnoses, brief reports, evidence based emergency medicine articles, opinions and relevant scientific announcements.

The main sections of the journal include emergency medicine systems, academic emergency medicine, emergency medicine education, emergency department management, disaster medicine, environmental emergencies, trauma, resuscitation, analgesia, pediatric emergencies, medical emergencies, pre-hospital medicine, toxicology, emergency nursing, health policy, ethics, management, imaging and procedures.

The articles published in the Tr J Emerg Med are expected to conform with the Helsinki Declaration and meet the common requirements of biomedical journals. CATEGORIES Research Articles: Original studies of basic or clinical investigations in emergency medicine. Turkish and English abstracts are required. Articles must include introduction, material and method, results, discussion, limitations and conclusion sections. The maximum number of words is 4,000 with a total of six tables or figures are allowed. For single centre studies the number of authors is limited to eight. The approval from the Institutional Review Board (IRB) is required prior to publication. Pharmeceutical studies require approval from the Regional Ethics Board prior to publication.

Case Reports: Brief descriptions of clinical cases or the complications that are seldom encountered in emergency medicine practice and have an educational value. Consideration will be given to articles presenting clinical conditions, clinical manifestations or complications previously undocumented in the existing literature and unreported side of adverse effects of the known treatment regimes or scientific findings that may trigger further research on the topic. Turkish and English abstracts are required. Case reports must include introduction, case presentation and discussion sections. They must be limited to 1,500 words, contain 15 references or less and two tables or figures. A maximum of five authors for a case study will be permitted.

Case Series: Brief descriptions of clinical cases or the complications that are seldom encountered in emergency medicine practice and have educational value. Case series must include introduction, case presentation and discussion sections. They must be limited to 2,500 words, contain 15 references or less and three tables or figures. A maximum of six authors for a case series will be permitted.

Brief Reports: Reports involving a small number of cases that require further investigation. Preliminary data and results are shared. Turkish and English abstracts are required. Reports must include introduction, methods, results, discussion, limitations and conclusion sections. They are limited to 4,000 words and four tables or figures. For single centre studies he number of authors are limited to six. Approval from the Institutional Review Board (IRB) is required prior to publication. Pharmeceutical studies require approval from the Regional Ethics Board approval prior to publication.

Concepts: Clinical or non-clinical articles related to the field of emergency medicine and detailing improvements to emergency medicine practice.

Turkish and English abstracts are required. The manuscripts must not exceed 4,000 words and limited three authors per article.

Review Articles: Comprehensive articles reviewing national and international literature related to current emergency medicine practice. Generally Tr J Emerg Med publishes invited review articles. Other authors should contact the editor prior to submission of review articles. Manuscripts must be limited to 4,000 words and a maximum two authors. There is no limit to the number of references.

Evidence-Based Emergency Medicine: Articles seeking to detail clinical and medical practices should present a clinical scenario followed by the research question(s), followed by a selection of the best available evidence, analysis of the evidence and the application of the evidence. Turkish and English abstracts are required. The manuscript must be limited to 4,000 words and a maximum of four authors. The authors should also submit copies of the articles proposed as supporting evidence.

Images in Emergency Medicine: Short case reviews with interesting and educative visual material. The case study is to be presented in two parts.

In the first part, the case is summarized and the image is presented. In the second part, the diagnosis is provided in the heading, followed by a discussion of the management of the case and the specifications of the images.

The review should consist of a maximum of 500 words and 5 references are allowed. The article should be prepared by no more than two authors.

There is no need for abstract.

Letter to the Editor: Opinions, comments and suggestions made concerning articles published in Tr J Emerg Med or other journals. Letters should contain a maximum of 1,000 words and 5 references are allowed for these single author submissions. No abstract is required.

SUBMITTING MANUSCRIPTS Tr J Emerg Med accepts online manuscript submission. Users should go to the journal’s web site (http://www.journalagent.com/tatd/) and create an account before submitting their manuscripts.

REQUIRED SUBMISSION DOCUMENTS Cover Letter: The author(s) should present the title, type and category of the article, and whether the submitted work had previously been presented in a scientific meeting. In addition, the full name of the corresponding author and his/her contact information including the address, phone number, fax number and email address should be provided at the bottom of the cover letter.

Title Page: On the title page, the title of the article, and the names of the authors’, including their academic titles and institutions should be listed in order. In addition, the running title and the name of the corresponding author along with his/her contact information should be provided.

For the Blind Initial Review: The names of the authors’, and any identifying information including the academic titles, institutions and addresses must be omitted. Manuscripts submitted with any information pertaining to the author(s) will be rejected.

MANUSCRIPT PREPARATION Turkish and English Abstracts: Turkish and English abstracts containing a maximum of 250 words are required for original research articles, evidence based emergency medicine and brief reports. The abstracts for original research articles and brief reports must contain four sections including the aim, material and method, results and conclusion. For a case report of medical care the Turkish and English abstracts should not exceed 150 words.

Instructions for Authors Key Words: Key words must be chosen carefully from PubMed MeSH (www. nlm.nih.gov) websites.

Sections of Original Research Articles: Original research articles should contain the following sections: Introduction: A three-paragraph structure should be used. Background information on study subject (1st paragraph), context and the implications of the study (2nd paragraph) and the hypotheses and the goals of the study (3rd paragraph).

Material and Method: The method section, is one of the most important sections in original research articles, and should contain sufficient detail.

The investigation method, study sample, analyses performed, commercial statistical programs used, details of measurement and evaluation (e.g.: make and model of biochemical test devices and kits) should all be clearly stated. There should be a list of the inclusion exclusion criteria. In survey studies, information concerning who implemented the survey and how it was performed should be specified.

Results: The demographic properties of the study population, the main and secondary results of the hypothesis testings must be provided. Commenting on the results and discussing the literature findings should be avoided in this section. The results should be presented with graphs, mean, median and standard deviation values as well as a 95% confidence interval.

Discussion: The main and secondary results of the study should briefly presented and compared with similar findings in the literature. Providing intensive and encylopedical information should be avoided in this section.

Limitations: The limitations of the study should be mentioned in a separate paragraph subtitled as the “Limitations” in the end of the discussion.

Conclusion: A clear conclusion should be made in the light of the results of the study. The potential effects of the results of the study on the current clinical applications should be stated in a single sentence. Inferences that are not supported by the study results should be avoided.

Points to be considered for general writing Statistical Analysis: All studies should be analysed in consultation with those experienced in statistical analysis.

Units of Measure: Standard units of measure should be used when presenting the substances used, drugs and laboratory values. Normal limits should be provided for the laboratory values.

Drugs: Generic names for drugs should be used. Doses and routes for the drugs should be stated.

Use of Turkish/English: Proper use of Turkish/English terminology and grammar should be emplolyed.

References: References should be written double spaced at the end of the article. They should be numbered in the order they appear in the text, and not listed alphabetically. The references that are used in the “Abstract” section should be stated as “(abstract)”. The names of the first three authors should be included in a given reference followed by “et al”. The authors are responsible for the accuracy of the references.

Obesity, Pepper Pills and Acute Coronary Syndrome

Dear Editor, Obesity is a growing public health problem and ‘Obesity Prevention and Control Program’ is one of the current policies of Ministry of Health. Promoting physical activity and good nutritional habits has paramount importance in obesity prevention. In some instances, use of some medications under control of health care professionals may be necessary. However, despite the warnings of the authorities, non-prescribed herbal weight control products and food supplements are being extensively consumed by over-weight population.

Contrary to common belief of the users, herbal medicines and food supplements are complicated by some problems such as lack of scientific evidence of safety and efficacy, lack of regularity oversight, lack of quality control, lack of knowledge about interactions with other drugs and also public misinformation.[1] Randomized controlled trials are the best way to define efficacy and safety of a medication. Most herbal products do not possess controlled trials which are not requested by consumers or health care practitioners, and not required by regulatory agencies. Compared to prescribed drugs, oversight and restrictions on production of herbal drugs and food supplements are limited. Additionally, contamination with heavy metals, adulteration with pharmaceuticals, and prohibited animal and plant ingredients are repeatedly found in herbal products. In this context, here, I presented an acute coronary syndrome case with angiographically normal coronary arteries possibly related to use of pepper pills which were taken for weight losing purpose.

A 34-year-old overweight male patient (body mass index: 29.8 kg/m2) was admitted to our emergency department with squeezing chest pain which emerged two days ago. He claimed that chest pain became more severe in the last six hours and lasted more than 25 minutes. He had no dyspnea or dyspepsia. He was an ex-smoker and quitted smoking seven months ago. No additional conventional risk factors of coronary artery disease were present. He claimed that currently he did not use any prescribed or illicit drugs other than pepper pills for weight losing. He was taking those pills

once-a-day for three days and chest pain began after ingestion of the pills. On physical examination, no murmur was heard and lungs were clear. Electrocardiography revealed nonspecific ST-T changes. On transthoracic echocardiography, there was no significant wall motion abnormality, sign of pericarditis or aortic dissection. However, blood tests revealed high troponin values. Since chest pain and positive troponin result were compatible with acute coronary syndrome, anti-ischemic, anti-thrombotic treatment was initiated. In the 18th hour of hospitalization, coronary angiography was performed which revealed normal coronary arteries. Troponin levels showed relevant rise and fall in the follow-up. Troponin level was still slightly high when he was discharged in the third day of hospitalization. Electrocardiogram did not change significantly during the follow up.

Possible prolonged coronary vasospasm was considered as mechanism of cardiac injury. Apparently pepper pills were the possible cause of coronary event. Patient was advised to cease using pepper pills. After the cessation of aforementioned pills, chest pain did not recur during hospitalization and after discharge. He was symptom free in the first month follow-up visit.

Pepper pills and its well-known active ingredient capsaicin are used in the obesity treatment for its thermogenic effect by stimulating catecholamin secretion. Besides, topical capsaicin preparations are widely used in the pain management and capsaicin was also studied for its potential use in cancer therapy.[2] Capsaicin is highly irritant molecule and ingestion of large amounts may cause gastrointestinal disturbances.

Also capsaicin intake may cause cardiac complications. In a study, pretreatment with capsaicin was found to be related to more extensive myocardial infarct size in animals with experimentally occluded coronary arteries.[3] Recently, two acute coronary syndrome cases related to capsaicin containing pepper pills have been reported by Turkish authors.[4,5] In both cases, patients were relatively young for coronary accident and had no conventional cardiovascular risk factor. In clinical practice, prescribed drugs are questioned during examinations but non-prescribed herbal drugs are usually ignored. Thus, awareness of community and health care providers about these kinds of drugs is important to avoid undesired consequences.

Sevkli ve Ön Tanılı Hastalara Dikkat Etmeli!

Gebe Uterus Rüptürü Olgusu Değerli Editör, Bu yazımızda bir olgu üzerinden sevk sırasında düşünülen ön tanılara göre hareket edilmemesinin gerekliliği ve uterus rüptürünün önemini vurgulamayı amaçladık. Bilindiği gibi acil servislere hasta kabulu ya ilk başvuru ya da sevkli şekilde olmaktadır. Sevkli ve ön tanılı hastalar hekimi yanlış yönlendirebilmektedir. Bu durum tanıda, tedavide gecikmelere ve istenmeyen sonuçlara neden olabilmektedir. Maalesef sağlıkçı olarak en büyük ve en önemli eksiğimiz yetersiz anamnez ve fizik muayene, ‘hastalık yok hasta var’ kaidesinin unutuluşu ve inceleme hekimi olma yönünde eğilimlerimiz. Bu önemli gereklilikler eksik kalınca olumsuz sonuçlar ortaya çıkabilmektedir. Özellikle adli olaylarda, hekim sorumlulukları ve hataları daha ön plana çıkmaktadır. Klinikte az rastlanan durumlarda hekim ayırıcı tanıda her türlü olasılığı düşünüp hastayı bir bütün olarak görüp, ilk doktoru olarak değerlendirerek gerekli anamnez, fizik muayene ve sonrasında tetkik incelemelerde bulunmalıdır. Bu konuda özellikle acil doktorları en az görülebilecek hastalıkları bile ayırıcı tanıda düşünerek ona göre yönlendirme yapmalıdır. Evet öncelikle düşünmek gerekli! Düşünülmez ise unutulmuş ve atlanmış demektir. Yapılan çalışmalarda eksik anamnez, hastalarla dil gibi problemlerle yetersiz iletişim ve ön tanılarla hareket sonucu oluşabilecek yanlış tedavi ve uygulamaların önemi vurgulanmaktadır.[1,2] Gebelikte meydana gelebilecek arrest nedenleri anneye ve fetüse bağlı olarak gelişebilir. Gebelikte ani ölümün 30000 doğumda bir olduğu ve medikal olarak en sık nedeninin akciğer embolisi olduğu belirtilmektedir.[3] Uterus rüptürü şok ayırıcı tanısında ve ani ölüm nedenleri arasında nadir görülen bir durumdur. Klinikte altta yatan nedene göre nedbeli ve nedbesiz rüptür olarak gruplara ayrılmıştır. Bunlardan ise en sık nedbeli uterus rüptürünün olduğu belirtilmiştir. Yapılan çalışmalarda nedbesiz uterus rüptürünün ise genellikle doğum sırasında veya geç gebelik döneminde olduğu, 8000 ile 15.000 doğumda bir, ortalama %0.009 oranında olduğu bildirilmiştir.[4] Bir başka çalışmada ise bu oran 2100 doğumda 1 olarak belirtilmiştir.[5] Nedbeli uterus rüptürünün ise ge-

nellikle geçirilmiş sezeryan öyküsü olan hastalarda olduğu ve %0.6 oranında görüldüğü belirtilmektedir.[2] Kırk yaşında 26 haftalık gebe olan hasta dış merkezden ani solunumsal ve kardiyak arrest, pulmoner emboli ve miyokart enfarktüsü ön tanıları, başarılı resüsitasyon sonrası yoğun bakım ihtiyacı nedeniyle sevk edildi. Hastanın sekiz saat önce başlayan kendini kötü hissetme, nefes darlığı, baygınlık, aşırı terleme şikâyetleri olduğu öğrenildi. Bu şikâyetlerle 112 tarafından evinde entübasyon uygulandığı, dış merkeze götürüldüğü, acil servisde kardiyopulmoner arrest geliştiği ve fetüsün ölü olduğu bildirildi. Hastanın ikinci gebeliği ve sezeryan öyküsü mevcuttu.

Hasta acil servise getirildiğinde genel durumu kötü, Glaskow Koma Skalası 3, entübe, pupiller dilate, ışık refleksi yoktu. Spontan solunumu olmayan hastada tansiyon arteryel 50/30 mmHg, nabız 50/dk, yüzeyel EKG’de sinüs ritmi mevcuttu. Hastaya dopamin infüzyonu başlanmıştı. Yapılan kan tahlillerinde anormal olarak beyaz küre 60.6.10^3/µ (4.0010.00), hemoglobin 5.2 g/dl (11-15), hemotokrit %17.3 (37.047.0), D-Dimer >10000 ng/ml (0-500), glukoz 265 mg/dl (70100), AST 138.00 U/L (5-34), ALT 80.00 U/L (0-55), potasyum 5.7 mmol/L (3.5-5.1) tespit edildi.

Hastaya pulmoner emboli ve miyokart enfarktüsü olabileceği düşünülerek, kadın doğum, kardiyoloji ve göğüs hastalıklara konsultasyonu istendi. Resüsitasyon odasında yatak başı ekokardiyografi (EKO) yapıldı. EKO’da kardiyak patoloji düşünülmedi. Çekilen göğüs tomografisi normal olarak değerlendirildi. Yatak başı batın ultrasonografisinde karaciğer ve dalak çevresinde yaygın serbest sıvı ve uterus içinde fetal kalp atımları negatif olan tek fetüs saptandı. Acil ameliyata alınan hastada eski sezeryan hattından yırtık tespit edildi.

Ameliyat sırasında arrest olan, resüsitasyona cevap vermeyen hasta hayatını kaybetti.

Sonuç olarak, hastanın her zaman ilk doktoru olunmalıdır.

Dış merkezden yapılan ön tanılar düşünülerek yaşanacak ikilemler sonrasında hastalar yönlendirilmemelidir. Sezer-

yan öykülü, gebe, hipovolemik şok ve ani ölümde çok ender görülen uterus rüptürü akla getirilmelidir. Hastalarda uterus rüptüründe veya karın içi olaylarda beklenen klasik karın ağrısı, bulantı, kusma gibi şikâyetlerin olmayabileceği akıldan çıkarılmamalıdır.

Karboksihemoglobin Seviyeleri ve Zehirlenme Şiddeti Arasındaki İlişki Değerli Editör, Derginizin 2013-1. sayısındaki “Karbonmonoksit Zehirlenmesinde Karboksihemoglobin Düzeyi, Zehirlenmenin Şiddeti ve Mini Mental Durum Testi Skalası Arasındaki İlişki”[1] başlıklı Sayın Genç ve Aygün tarafından yazılan makaleyi ilgi ile okuduk. Deneyimlerini bizlerle paylaştıkları için teşekkür ederiz. Yazarların çalışmaya dahil olma kriterlerinde karboksihemoglobin (COHb) seviyesini belirtmeyerek bir hata yaptıkları kanaatindeyiz ve bu noktaya temas etmek istiyoruz.

Yazarlar çalışmalarında 51 karbonmonoksit zehirlenmesi olgusunu incelemişler ve karboksihemoglobin seviyesi normalden yüksek olan olguları çalışmaya dahil ettiklerini bildirmişlerdir. Çalışmada COHb düzeyi %7.58 olan bir hasta da yer almış ve zehirlenme şiddetinin ağır olduğu grupta değerlendirilmiştir. Karboksihemoglobin seviyeleri, birçok çalışmada ve pek çok merkezde %5 ve %10 gibi farklı referans değerlerinin üzerinde yüksek kabul edilmektedir.[2,3] Sigara içenlerde COHb seviyeleri %5-9 olarak sıklıkla bulunurken, sağlıklı bireylerdeki COHb seviyelerindeki %4.4’lük bir değişimin maksimum oksijen tüketimini sınırlayarak çalışma kapasitesini

azalttığı da bilinmektedir.[3] Bu sebeplerden dolayı yapılan çalışmada COHb seviyelerinin alt sınırının da belirtilmesi, COHb düzeyi, zehirlenmenin şiddeti ve mini mental durum testi skalası arasındaki ilişkinin araştırıldığı bir makalede kritik öneme sahiptir.

Yazarın yanıtı Sayın Editör, Sayın Eyi ve Karslıoğlu’nun “Karbonmonoksit Zehirlenmesinde Karboksihemoglobin Düzeyi, Zehirlenmenin Şiddeti ve Mini Mental Durum Testi Skalası Arasındaki İlişki” başlıklı makalemize, sayın editörün aracılığıyla yaptığı katkıyı okuduk. Değerli görüş ve katkısı için teşekkür ederiz.

Kandaki karbonmonoksitin (CO) eliminasyonu, solunum havasındaki oksijen konsantrasyonu ya da atmosfer basıncı artırılarak hızlandırılabilir. Oda havasında ortalama 4-5 saat olan karboksihemoglobin (COHB) yarılanma ömrü, %100 oksijen tedavisi ile ortalama 60 dakikaya, hiperbarik oksijen tedavisi ile ise ortalama 20 dakikaya inmektedir. Ayrıca oksijen verildiğinde kanda çözünen oksijen miktarı artmakta ve CO yarılanma ömrü azalmaktadır.[1-3] COHB düzeyi ile zehirlenmenin şiddeti arasındaki ilişkinin varlığı tartışmalı olmaya devam etmektedir. Kan COHB düzeyi ile zehirlenmenin şiddeti arasında tam bir korelasyon olduğunu belirten yayınların yanında, bu ilişkinin sadece hafif şiddetteki zehirlenmelerde mevcut olduğunu belirten yayınlar da mevcuttur.[1,4-7] Biz çalışmamızda herhangi bir CO kaynağına maruz kalan, COHB değeri normal sınırların üzerinde olan (COHB düzeyi >%5) zehirlenmenin ilk 24 saati içerisinde başvuran, 18 yaş üzerindeki hastaları çalışmaya dahil ettik. Bu olgularında çoğu kez CO kaynağından temasın kesilmesi ile hastanın bir acil kliniği başvurusu arasındaki süre ve bu süre içindeki almış olduğun oksijen tedavisi kan COHB miktarını azaltmaktadır. Bundan ötürü olgular klinik belirti ve bulgularına göre ağır zehirlenme grubunda yer almasına rağmen kan

COHB değerleri düşük olabilir. Her ne kadar bu olguların sigara içip içmediğinin değerlendirilmemesi bu çalışmada kısıtlayıcı bir faktör olarak değerlendirilebilse de biz çalışmamızda kan COHB değerlerinin alt sınırının belirtilmemiş olması kritik öneme sahip olmadığını ve çalışmamız bütünlüğünü bozmadığını düşünmekteyiz.

Baldırda Ani Gelişen Ağrı ve Şişlik Boğaçhan TOKATMAN, Özkan KÖSE Antalya Eğitim ve Araştırma Hastanesi, Ortopedi ve Travmatoloji Kliniği, Antalya

Kırk beş yaşında erkek hasta, baldırda ağrı, yanma ve topallama şikayetleri ile acil servisimize başvurdu. Hastanın şikayetleri iki saat önce merdivenden çıkarken aniden başlamıştı ve kısa süre içinde baldırın iç kısmında şişme gelişmişti. Fizik muayenede hastanın baldır proksimal medial kısmında ekimoz, palpasyonla hassasiyet ve ağrısı mevcuttu (Şekil 1). Diz

ekstansiyondayken ayak bileği zorlu dorsifleksiyon sırasında baldırda ağrısı artıyordu (Pozitif Homan bulgusu). Özgeçmişinde bilinen bir hastalığı yoktu, 20 paket-yıl sigara kullanımı öyküsü mevcuttu. Direk radyografisi normal hasta, MRG ile değerlendirildi (Şekil 2).

SUMMARY

ÖZET

Objectives The goal of this study was to compare lecture and small-group discussion methods for educating medical students in emergency department.

Amaç Bu çalışmanın amacı acil serviste tıp öğrencilerinin eğitiminde ders verme ve küçük gruplu tartışma yöntemlerini karşılaştırmaktı.

Methods Over a 2-year period, 155 fourth to fifth-year medical students in the emergency department were given educational sessions on basic life support (BLS) and adult advanced Cardiovascular Life Support (ACLS). These sessions were given by a single instructor in a traditional lecture format, in a small-group discussion, or in a combination of these two methods. Prior to the instructional sessions, students anonymously completed a 20-question multiple-choice examination on the covered topics. At the completion of sessions, students repeated the same multiple-choice examination.

They also completed an evaluation form on the instructional format, using a 5-point Likert scale, ranging from 1 (strongly agree) to 5 (strongly disagree).

Gereç ve Yöntem İki yıl içinde toplam 155 dördüncü ve beşinci sınıf tıp öğrencisi için acil serviste temel yaşam desteği (TYD) ve erişkinde ileri kardiyovasküler yaşam desteği (İKYD) üzerine eğitsel toplantılar düzenlenmiştir. Bu toplantılarda tek bir eğitmen geleneksel ders formatı, küçük gruplu tartışma veya bu iki yöntemin kombinasyonunu uygulamıştır. Eğitsel toplantılardan önce öğrenciler adlarını yazmadan kapsanan konular hakkında 20 soruluk çoktan seçmeli bir sınavdan geçmiştir. Dersler tamamlandığında öğrenciler tekrar aynı çoktan seçmeli sınavdan geçmiştir. Öğrenciler ayrıca, 1 (kesinlikle olumlu) ila 5 (kesinlikle olumsuz ) arası puanlama yapan Likert ölçeğini kullanarak bir eğitimi değerlendirme formunu doldurmuştur.

Results There was a significantly higher level of enjoyment in the combination group (lecture plus discussion), although there were no differences in the test scores immediately after the instructional sessions.

Bulgular Kombinasyon grubundan (ders artı tartışma) anlamlı derecede daha çok hoşlanılmasına rağmen eğitsel oturumlardan hemen sonra test puanlarında hiçbir farklılık yoktu.

Conclusions Medical students learning about BLS and ACLS preferred a combination of lecture and small-group discussions over traditional lectures or discussion groups. However, this preference did not lead to improved test scores on these subjects.

Sonuç TYD ve İKYD konularını öğrenen tıp öğrencileri geleneksel ders ve tartışma grupları yerine ders ve küçük gruplu tartışmaların kombinasyonunu tercih etmiştir. Ancak bu tercih bu konularda test puanlarının iyileşmesine yol açmamıştır.

Introduction One important step in educational design is choosing a teaching method. Traditional lectures often focus on memorization rather than the comprehension and use of information. Lecturing is one of the primitive and maybe the oldest method of teaching, and it is currently the most conventional educational technique. In this approach, students silently and passively receive information.[1] However, active participation and cooperation of students often leads to better, more effective and permanent learning. Because of this, educational experts emphasize the use of modern comprehensive methods,[2] which help students better understand and remember information.[3] In various studies, the conventional lecturing method has failed to transfer concepts to students as effectively as active learning approaches such as small group tasks and discussions. Therefore, it has been suggested that it is better to use the conventional lectures only for transferring data to students.[4-7] In contrast to these studies, others have reported that both teaching methods have equal impact on learning.

Lectures continue to be the most used education method since they save time and resources that are used in more active learning techniques.[8-10] Botelho and O’Donnell’s research has shown that teaching via small group discussions creates an active learning environment for all students, including those with all levels of intelligence and mental ability. They suggested that teaching in a group discussion format with two-sided data exchange is most efficacious for students. They have also emphasized that students prefer learning in active lectures with an expert faculty who predominates the educational discussion.

[11] Hiill’s comparison between lecturing and discussion showed that students’ mental abilities and skills are higher in group discussions and that this approach is useful for remembering information.[12] Roche has also reported that by combining lectures with group tasks, the students’ attitudes towards the subject would become better more quickly.[13] These two techniques have been compared in various medical educational groups. For example, dentistry students showed that the combined methods led to increased student satisfaction and inner motivation, which led to higher educational quality.[14] In nursing students, the combined teaching method improved their communication skills in the clinical environment.[15] Medical students need to learn the skills of problem solving and decision making in addition to receiving data and knowledge. Since cardiopulmonary resuscitation is a lifesaving maneuver that can save the patient from definite death, it is of high importance for medical staff. Therefore, we studied the effects of different teaching methods for Ba-

sic life support and Adult advanced cardiovascular life support in medical students.

Materials and Methods This study was performed with 155 of fourth or fifth year medical students. Due to the clinical importance of being familiar with primary and advanced resuscitation techniques for cardio-pulmonary arrest patients based on American Heart Association latest guidelines, we chose this topic for teaching. Students were randomly divided into three groups: traditional lecture, group discussion, and lecture plus group discussion. For all three groups, we performed 3 educational sessions of 50 minutes each. Based on their teaching type, we gave the students the performance protocol for each group, which included the list of lessons to be learned, a step by step manual of teaching performance, and goals of the class.

The traditional lecture session was carried out in a large classroom with plentiful light and audiovisual equipment including a white-board, video projector, and teaching slides.

In the beginning, the aim of the class was explained, and during the session we tried to increase the students’ attention by asking questions, giving examples, and making conclusions and outlines. There were 3 topics to discuss, which took a total time of 3 hours (50 minutes for each topic and 10 minutes of questions and answers).

In the group discussion session, students were divided into 5 groups of 10, and they were guided by an instructor (a total of 5 mutual instructors) through scenarios covering the assigned subjects. All of the needed material was given to the students in advance. This session was performed in a 3 hour period as a whole. In the third group, first, a 30 minute lecture was given for each topic followed by a 20 minute group discussion guided by instructors in groups of 10. The content was similar in all 3 groups and we used uniform slides.

In all three sessions, we completed forms containing the students’ demographic data and characteristics, and then gave them a pre-test questionnaire with a 20 minute time limit. At the end of the program they completed a similar post-test as well as an evaluation form. The pre and post-test each contained 20 questions in multiple-choice format with the same value. The validity of the tests’ contents was confirmed by 5 respectful emergency medicine academic members of Tehran University of Medical Sciences. The quality of teaching was evaluated with a form added at the end of the post-test questionnaire, which contained five sentences with answers adjusted using a 5-point Likert scale (including: strongly agree, agree, no opinion, disagree, strongly disagree). These statements evaluated the way the class was handled (how, who, when, where).

of their medical education. Ninety six of them (61.9 percent) were female students.

Based on the number of students present in Rasoul Akram hospital during our research time, after coordination with university authorities, and after a 20 percent drop in participants , a total of 155 students (in their 4th or 5th year) were enrolled in the study and were randomly divided into three groups.

The pre-test mean score in the lecture group was 53.3±16.3 and after the lecture it was 89.5±5.8. In the group discussion session, the pre-test had a mean score of 45.0±13.1 and the post-test mean score of this group was 85.6±9.6. In the lecture and discussion group, the mean score of the pre and post-test were 47.8±14.6 and 89.8±5.9, respectively. Using a t-paired test, the differences between the pre and post-test scores for each group were significant (Table 1).

Data Analysis Method Comparison of the difference between the score of pre and post-tests was analyzed by a Paired t-test. For all three groups, we used variance analysis for quantitative variables and Chi squared tests for qualitative variables. A p value of less than 0.05 was accepted as significant. All statistical calculations were performed using the SPSS software program (version release 13.0, SPSS, Inc., Chicago, IL).

For comparing the effectiveness of three mentioned methods, a variable showing the difference between pre-test and post-test score was calculated. The mean increase of scores was 36.2±15.9 in the lecture group, was 40.6±13.0 for those in group discussion, and was 43.0±13.8 in the lecture plus group discussion group. According to an ANOVA test, these differences were not statistically significant (Table 2).

Results

Based on a 5-point Likert scale, participants evaluated the overall session, the session moderator, and the timing and place of that session. Using a Kruskal Wallis test, there was no significant difference between the groups. At the end of each session and right before the post-test, students were asked about how satisfied they were with their sessions.

lecture group was 87.9%, was 89% in the group discussion group, and was 94% in the lecture plus group discussion group. A one-way ANOVA test showed that differences between the groups were not statistically significant (Sig=0.04) (Table 3).

Discussion The results of this research have shown that all three educational methods were able to effectively increase the students’ knowledge on the topics. A comparison of pre-test and post-test scores of all three groups showed elevation in scores, but the differences in the average scores were not statistically significant. All three methods increased the students’ post-test scores, but there was not a significant difference between the three sessions. There might not have been a difference because there were limited sessions, there was a high load of academic content in proportion to the time and number of sessions, and because of the type of the educational content.

The outcome of our study confirmed the published study of Fischer and colleagues in 2004. In that study, the methods of lecture and small group discussion were compared, and it was reported that although the students significantly preferred the group discussion to the lecture, there was no significant difference in the post-test scores of the two groups.[16] In another study done by Hanze and Berger in 2007, there was no difference between cooperative and traditional teaching groups in reaching goals, but the cooperative group had a higher motivation,[17] which matches our present results.

significantly elevates student satisfaction with the teaching methods. This could be due to the students’ obligation to actively cooperate in the discussion, the enforcement of communication skills, and because of the facilitation of the learning process.

Limitations Problems we faced in performing this study included difficulty in planning student groups, having concurrent sessions, and determining the pre-guidance of group discussion before the session. Another limitation was the inability to measure the students’ clinical skills after each program.

Another limitation was a lack of proper space for having group discussion sessions.

In conclusion, the lecture plus group discussion teaching method was as effective and capable as the traditional lecture or group discussion methods in increasing the knowledge of students about basic and advanced life support in patients with cardiopulmonary arrest.

ÖZET

SUMMARY

Amaç Ülkemizde acil tıbbın akademik gelişimi ile acil tıp uzmanlarının yayın sayıları da artmaktadır. Tüm dünyada mortalite ve morbiditenin önde gelen nedenlerinden biri olan travma, acil tıbbın da en önemli konularından biridir. Bu çalışma, acil tıp akademisyenlerinin travmayla ilgili yaptıkları yayınların ve aldıkları atıfların değerlendirilmesini amaçlamaktadır.

Objectives As the academic development of emergency departments evolves in our country, publications from emergency physicians are also increasing.

Trauma, which is a leading cause of mortality and morbidity worldwide, is one of the most important topics in emergency medicine. This study aims to analyze publications and citations on trauma authored by academicians of emergency medicine.

Gereç ve Yöntem Acil tıp doçent ve profesörlerinin 1994-2012 yılları arasında travma üzerine yapmış oldukları yayınlar, PubMed veritabanı üzerinden incelendi. Akademik atıflar Google Scholar aracılığıyla değerlendirildi.

Yayınlar dergi adlarına, makale türlerine ve atıf sayılarına göre analiz edildi.

Methods A literature search regarding trauma was performed on publications and citations authored by professors and associate professors of emergency medicine between the dates of 1994 and 2012 using the PubMed database. Academic citations were evaluated using Google Scholar. Publications were analyzed according to journal name, type of article, and the number of citations.

Bulgular Travma konulu 179 yayın değerlendirildi, yayınların 133’ü (%74.3) orjinal araştırma iken, 46’sı (%25.7) olgu sunumuydu. Orjinal araştırmalar için atıf oranı %77.4’ken, olgu sunumları için %56.5 idi. Deneysel hayvan çalışmaları, travma konusundaki tüm yayınların %11.2’sini oluşturmaktaydı (n=20). Yıllar içinde travmayla ilgili yayın ve atıf sayılarında artış eğilimi izlendi.

Results A total of 179 publications about trauma were evaluated. One hundred thirty three (74.3%) of these were original research articles and 46 (25.7%) were case reports. Citation ratios were 77.4% for original contributions and 56.5% for case reports. Animal experimental research articles constitute 11.2% of all trauma publications (n=20). We observed that the number of publications and citations about trauma had been increasing within recent years.

Sonuç Travmayla ilgili yayınlar hızla artmakta ve acil tıp uzmanları travma literatürünün gelişimine katkıda bulunmaktadırlar. Geçen 19 yıldaki yayınlar içinde, travmaya ilişkin birçok konu acil tıp akademisyenleri tarafından ele alınmıştır.

Conclusions The number of publications concerning trauma are growing incrementally and emergency physicians are participating in the development of trauma literature. In the past 19 years, many different topics about trauma have been published by academicians from emergency medicine.

Çalışmada 31 Aralık 2012 tarihine kadar acil tıp doçenti veya profesörü ünvanı almış olan hekimlerin isimleri üzerinden, travma konusundaki yayınları incelemeye alındı. İsim benzerliğine bağlı oluşabilecek hatalara karşı, sözkonusu yayınlardaki isim ve kurum bilgileri ayrıntılı olarak incelendi.

Yayınların yapıldığı dergilerin SCI (Science Citation Index) ve SCI-E (Science Citation Index-Expanded) indekslerine girip girmedikleri, Thomson Reuters-Master Journal List aracılığıyla incelendi.[7] Yayınlar hem orjinal çalışma ve olgu sunumu / editöre mektup gibi türler açısından, hem de insan çalışmaları ve deneysel çalışmalar gibi çalışma popülasyonu açısından ayrı ayrı değerlendirildi.

Çalışmada acil tıp doçenti veya profesörü ünvanı almış olan hekimlerin, travmayla ilgili yayınlarının ve aldıkları atıfların geriye dönük olarak incelenmesi amaçlandı. Doçentlik ve profesörlük ünvanı alan hekimler hem eğitim ve araştırma hastanelerinde, hem de üniversite hastanelerinde kullanılabilen ortak bir titre sahip oldukları için çalışma evreni olarak belirlendi. Tarama için PubMed veritabanı, ilk acil tıp asistanlık programının başladığı 1994 yılından başlamak kaydıyla, 2012 yılına kadar tarandı. Atıf taraması, atıflara yaygın erişim imkanı nedeniyle Google Scholar üzerinden gerçekleştirildi.

Gereç ve Yöntem

Çalışma için 68 acil tıp doçenti ile 11 acil tıp profesörünün travmayı konu eden yayınları incelendi. PubMed taraması sonucunda 79 akademisyene ait, 71 farklı dergide yayınlanmış 179 yayına ulaşıldı. Yayınların 133’ü (%74.3) orjinal araştırma kategorisinde, 46’sı (%25.7) ise olgu sunumu ve editöre mektup şeklindeydi (Şekil 1). Yayınlardaki yazar sayılarının ortanca değeri 5 (4-6) olarak saptandı.

Travma, acil tıp uzmanlarının oldukça sık karşılaştıkları ve eğitim müfredatında yeri olan ana konulardan biridir. Acil tıbbın uluslararası yayın üretimiyle ilgili yapılmış bazı araştırmalar mevcut olmakla birlikte, travma ile ilgili yapılmış yayınları değerlendiren bir çalışma bulunmamaktadır.[4-6] Bu çalışmanın amacı; Türkiye’deki acil tıp doçent ve profesörlerinin, geçen 19 yıl içinde travmayla ilgili olarak yaptıkları yayınların ve almış oldukları atıfların analizini yapmaktır.

Bulgular

1996

Ülkemizde 2000’li yılların başlarında 14 üniversite hastanesinde ve kısıtlı sayıda akademisyenin desteğiyle sürdürülmekte olan acil tıp asistanlık programları, günümüzde 43’ü üniversite hastanesi, 27’si eğitim ve araştırma hastanesi olmak üzere toplam 70 kurumda sürdürülmektedir.[3] Acil tıp uzmanı sayısındaki artışa paralel şekilde, acil tıp akademisyenlerinin sayısında da artış meydana gelmiştir. Buna bağlı olarak acil tıp alanında ulusal ve uluslararası indekslere giren dergilerdeki yayın sayılarında da yıllar içinde artış izlenmektedir.[4,5]

1995

Acil tıp Türkiye’de görece yeni bir uzmanlık dalıdır. İlk acil tıp uzmanlık eğitim programı, 1994 yılında Dokuz Eylül Üniversitesi Acil Tıp Anabilim Dalı’nda başlamış ve akademik birimlerin hızla gelişmeleriyle birlikte uzman ve akademisyen sayısında hızlı bir artış meydana gelmiştir.[1,2]

Veriler Windows için SPSS, versiyon 15.0’a girilerek analiz edildi. Verilerin normal dağılıma uygunluğu Kolmogorov Smirnov testiyle değerlendirildi. Niceliksel verilerin analizi için Mann-Whitney U-testi uygulandı; veriler ortanca ve %25-75’lik çeyreklik değerlerle ifade edildi. Kategorik verilerin analizi için Pearson ki-kare testi kullanıldı, değerler sayı ve yüzdelerle ifade edildi. p<0.05 değeri istatistiksel olarak anlamlı kabul edildi.

Çalışmaların 10’u (%5.6) SCI ve SCI-E indekslerinde olmayan hakemli dergilerde yayınlanmıştı. Geri kalan 169 çalışmanın 140’ı (%78.2) SCI-E tarafından taranan dergilerde, 29’u (%16.2) SCI tarafından taranan dergilerde yayınlanmıştı.

Yayınlanan 179 çalışmanın aldıkları toplam atıf sayısı 1378’di.

Çalışmaların aldıkları atıfların yıllara göre dağılımı Şekil 2’de sunuldu. Travma ile ilgili çalışmalardan 46 olgu sunumunun 26’sı atıf almışken (%56.5), 133 orjinal araştırmanın 103’ü (%77.4) atıf almıştı. Buna göre orjinal araştırmaların, olgu sunumlarına göre daha fazla atıf aldıkları izlendi (p=0.006). Çalışmaların 159’u (%88.8) insanlar üzerine yapılmışken, 20’si (%11.2) hayvan çalışması idi. la deneysel hayvan çalışmaları arasında, atıf alma durumları açısından istatistiksel anlamlı fark bulunmadı (p=0.455).

Travma konusundaki çalışmalar 71 farklı dergide yayınlanmış olmasına karşın, yayınların 95’i (%53.1) SCI ve SCI-E indekslerine giren beş dergide yapılmıştı. Sözkonusu çalışmaların beş dergiye göre dağılımları ve atıf sayıları Tablo 2’de sunuldu.

Tartışma Travma acil tıbbın yakın ilişkide olduğu alanlardan biridir.

1994 yılından itibaren akademik birimlerin sayısında görülen hızlı artış, bilimsel dergilerde yapılan yayın sayılarında artışı da beraberinde getirmiştir. Travma ile ilgili yayın sayılarında yıllar içinde görülen artışta, travma konusundaki bilgi ve beceri düzeyindeki artışın rolü olabilir.

2009 yılında yapılan 261 kişilik kesitsel bir çalışmada, acil tıp uzmanlık öğrencilerinin spesifik konulardaki yeterliliği, uzmanlık öğrencileri ve eğitmenler tarafından değerlendirilmiştir. Uzmanlık öğrencileri travma hastasına yaklaşım konusunda %75.8 oranda kendilerini yeterli hissetmekteyken, eğitmenlere göre asistanların bu alandaki yeterlik düzeyi %87.5’tir.[2] Sözkonusu çalışmanın sonuçlarına göre, travma tüm alt gruplar içinde yapılan değerlendirmede uzmanlık eğitimi sürecinde en yeterli hissedilen alanlardan biridir.

Literatürde Türkiye’deki acil tıbbın gelişimiyle paralel olarak, yayın sayılarını inceleyen bazı çalışmalar bulunmaktadır.[4-6] Bu çalışmalardaki temel sonuç yıllar içinde görülen yayın sayısı ve kalitesindeki artıştır. Bizim çalışmamızın sonuçlarına göre de, travma ile ilgili yayınlarda ve atıf sayılarında yıllar içinde artış izlenmektedir. Yayınların büyük kısmının orjinal araştırma kategorisinde olduğu görülmektedir.

atıf oranının %77.4 olması bu tezi desteklemektedir. SCI-E dergilerde yayınlanan makaleler dışında, ele alınan 29 makale (%16.2) ise görece yayın yapmanın daha zor olduğu SCI dergilerde yayınlanmıştır. Çalışmamızda travmayla ilgili orjinal araştırmalarda sıkça seçilen konular kafa travması, şok-sıvı resüsitasyonu ve ortopedik travmalardır. Acil hekimlerinin kafa travmaları ve ortopedik travmalarla acil serviste sıkça karşılaşmalarının bu sayının artışında rolü olabilir. Şok ve sıvı resusitasyonuyla ilgili çalışmaların ise sıklıkla deneysel hayvan çalışmaları olduğu göze çarpmaktadır.

Ersel ve arkadaşlarına göre 2010 yılına kadar acil tıp uzmanlarınca yapılan uluslararası yayınların 46’sı (%10.9) Ulusal Travma ve Acil Cerrahi Dergisi’nde yayınlanmıştır. Bu dergiyi %10.6 oranı ile American Journal of Emergency Medicine dergisi takip etmektedir.[5] Bizim çalışmamızda da benzer şekilde iki dergide yapılan yayınlar ilk sıralarda yer almaktadır. Her iki dergide yayınlanan yazıların aldıkları atıf oranları %60’ın üzerindedir.

Kısıtlılıklar Sadece acil tıp doçent ve profesörlerinin travmayla ilgili olarak yapmış oldukları çalışmaların incelenmiş olması ve yayınların sadece PubMed veritabanı üzerinden incelenmiş olması, araştırmanın önemli kısıtlılık noktalarını oluşturmaktadır.

Akademisyenlerin travmayla ilgili ana akım yayın eğilimlerinin saptanmasına yönelik olarak geliştirilmiş bu tarama stratejisi sonucunda, PubMed tarafından taranan dergilerde yayınlanmayan yazıların veya akademik ünvanı olmayan bazı acil tıp uzmanlarının yapmış oldukları çalışmaların kapsam dışında kalmış olması mümkündür.

Ersel ve ark.nın[5] yapmış oldukları, 2010 yılında yayınlanan ve acil tıbbın ilk 15 yılının değerlendirildiği çalışmada travma-ortopedi ile ilgili yayınların tüm yayınların %15.9’unu oluşturduğu ve toksikoloji-çevresel acillerle ilgili yayınlardan sonra en sık seçilen konu olduğu dikkati çekmektedir. Sözkonusu çalışmada travma konusunun sıkça seçilmesi, acil servise başvuran travma olgularının sayıca fazlalığına ve ilgili popülasyonun kolay tanımlanabilir olmasına bağlanmıştır.

Benzer şekilde Çınar ve ark.nın[6] yapmış oldukları PubMed veritabanının baz alan araştırmanın sonuçlarına göre acil tıp uzmanları tarafından 2010 yılına kadar yayınlanan 585 makalenin 86’sını (%16) travma ile ilgili yayınlar oluşturmaktadır. Bu sonuçlara göre travma, acil tıp uzmanlarınca sıklıkla tercih edilen çalışma konularından biridir.

Çalışmanın sonuçlarına göre akademik acil tıp birimlerinin sayısındaki artışla birlikte; acil tıp akademisyenlerinin travmayla ilgili yayın sayılarında, kalitelerinde ve makalelerin aldığı atıf sayılarında yıllar içinde artış izlenmektedir. Son yıllarda atıf sayılarında görülen azalma trendi, atıfların zamana bağlı aşamalı artma özelliği nedeniyle izlenmiş olabilir. Fakat bunun net olarak belirlenebilmesi için, ilerleyen zaman içinde atıf sayılarını tekrar inceleyen çalışmalara ihtiyaç vardır.

Travma ile ilgili farklı indekslerce taranan bilimsel dergi yelpazesinde genişleme olmasının, yayın sayılarına olumlu etki edeceği düşünülmektedir.

Bununla birlikte çalışmamızın sonuçlarına göre makalelerde sadece nicelik yönünden değil, nitelik yönünden de artış dikkati çekmektedir. Travmayla ilgili yayınlanan olgu sunumlarının yarısından fazlası atıf almışken, orjinal araştırmalardaki

SUMMARY

ÖZET

Objectives Sepsis-associated encephalopathy (SAE) is the most widespread metabolic encephalopathy, which continues to be clinically evaluated with the Glasgow Coma Scale (GCS). This study aimed to determine the clinical utility of serum tau protein as an effective and easily attainable biochemical marker of morbidity and mortality for patients with SAE.

Amaç Sepsisle ilişkili ensefalopati (SAE) en yaygın metabolik ensefalopati olup klinik olarak hâlâ Glasgow Koma Ölçeğiyle (GCS) değerlendirilmektedir. Bu çalışma SAE hastalarında morbidite ve mortalitenin etkili ve kolaylıkla test edilebilir biyokimyasal belirteci olarak tau proteinin klinik yararlılığını belirlemeyi amaçlamıştır.

Methods This prospective cohort study included 70 patients admitted to the emergency department with sepsis or septic shock, who were then diagnosed with SAE based on the GCS between February 2, 2009 and July 30, 2009.

Statistical analyses were performed to investigate the relationship between serum tau levels, the development of SAE and subsequent patient morbidity and mortality.

Gereç ve Yöntem Bu prospektif kohort çalışması acil servise sepsis veya septik şokla kabul edilip, daha sonra 2 Şubat ile 30 Temmuz 2009 arasında GCS’ye göre SAE tanısı konmuş 70 hastayı kapsamaktaydı. Serum tau düzeyleriyle SAE gelişmesi ve ardından hasta morbidite ve mortalitesi arasındaki ilişkiyi araştırma amacıyla istatistiksel analizler gerçekleştirildi.

Results Out of the total number of patients enrolled in the study, 50% received the diagnosis of SAE based on GCS upon admission. Of the patients admitted to the hospital for further observation, 2.9% developed SAE over the course of several days using GCS criteria. Even though tau levels were higher in patients with SAE, there was no statistically significant difference with tau levels in patients that did not develop SAE. In addition, there were no significant correlations between tau levels, sequential organ failure assessment (SOFA) score and patient mortality.

Bulgular Çalışmaya katılan tüm hastaların %50’si hasta kabulde GCS’ye dayanarak SAE tanısı almıştı. İleri değerlendirme için hastaneye kabul edilen hastaların %2.9’unda GCS ölçütlerine göre birkaç gün içinde SAE gelişmişti. Tau düzeyleri SAE hastalarında daha yüksek olmasına rağmen SAE gelişmeyen hastaların tau düzeyleri arasında istatistiksel açıdan anlamlı herhangi bir farklılık yoktu. Ayrıca, tau düzeyleriyle ardışık organ yetmezliği değerlendirme skoru (SOFA) ve hasta mortalitesi arasında anlamlı korelasyonlar yoktu.

Conclusions Serum tau levels do not reflect brain damage and encephalopathy for SAE patients. Moreover, this protein does not correlate with patient morbidity and mortality and the SOFA score. Thus, serum tau protein cannot be utilized as a reliable biochemical marker of SAE. Further research is needed to identify effective and easily-obtainable biomarkers to supplement the GCS in SAE diagnosis.

Sonuç Serum tau düzeyleri SAE hastaları için beyin hasarı ve ensefalopati durumunu yansıtmamaktadır. Ayrıca, bu protein hasta morbidite, mortalite ve SOFA skoruyla korelasyon göstermemektedir. O halde, serum Tau proteini SAE’nin güvenilir bir biyobelirteci olarak kullanılamaz. SAE tanısında GCS’yi takviye için etkili ve kolayca elde edilebilir biyobelirteçlerin tanımlanması amacıyla ileri araştırmalara gerek vardır.

Introduction Sepsis-associated encephalopathy (SAE) arises from a sepsis-associated systemic inflammatory response that leads to diffuse or multifocal cerebral dysfunction that is independent of structural, metabolic or direct infectious causes such as meningitis-encephalitis.[1] Encephalopathy due to sepsis develops in more than 50% of septic patients and is correlated with greater mortality.[2] Although the GCS is widely used in the analysis of cerebral dysfunction, it is frequently not reliable since most septic patients are sedated and on mechanical ventilation even before deteriorating neurologically. Electroencephalography, computed tomography and magnetic resonance imaging are utilized to evaluate brain dysfunction, but for these clinically tenuous patients such imaging is difficult to acquire especially if repeated imaging is indicated. Other considerations are the difficulties involved in safely transporting these patients to receive imaging and the risk of contrast-induced renal injury.[2] As such, easily obtainable and effective biochemical markers will be extremely useful for evaluating the morbidity and mortality of SAE. Levels of serum tau protein have been demonstrated to have diagnostic and prognostic importance in moderate to severe traumatic brain injury.[3] In our study we endeavored to determine whether serum tau protein levels reflect the extent of brain damage and the development of encephalopathy in the evaluation of SAE patients. We strived to achieve this by investigating if serum tau levels correlated with SAE patient morbidity and mortality and with Sequential Organ Failure Assessment (SOFA) scores. In all, we strived to further advance the search for an effective biochemical marker that might complement the GCS in diagnosing SAE.

Materials and Methods Study subjects for this prospective cohort study were comprised of 70 ED admissions diagnosed with sepsis or septic shock between February 2, 2009 and July 30, 2009. This investigation examined the correlation between serum tau protein levels and the development, clinical course, and morbidity and mortality of SAE. Enrollment criteria Adult patients enrolled in the study received a diagnosis of sepsis or septic shock based on specific, commonly accepted clinical criteria.[4] Serum tau levels were measured at the time of admission or within 24 hours of receiving the diagnosis of sepsis or septic shock. After this initial level was taken three additional levels were measured daily over 72 hours. Patients were diagnosed with SAE based on low GCS

in the absence of clinical or laboratory evidence of direct brain infection. Statistical analysis was performed to determine if there is a correlation between SAE patient serum tau levels, clinical course, and mortality.

Exclusion criteria Patients with the following encephalopathies at presentation or during monitoring were excluded from the study: acute cerebral infarction, acute cerebral hemorrhage, status epilepticus, Alzheimer’s disease, hyperosmolar non-ketotic coma, diabetic ketoacidosis, recent head trauma, and direct central nervous system infection. Such cerebral pathologies were excluded because these conditions cause changes in consciousness that impede the accurate diagnosis of SAE. Data collection A standardized form detailing patient demographic information was completed for all subjects diagnosed with sepsis and septic shock. Upon admission or within 24 hours after receiving a diagnosis of sepsis or septic shock, a 5 mL blood sample was taken from patients to measure serum tau levels. Then three additional 5 mL blood samples were taken daily over the 72 hour period following admission or diagnosis with sepsis or septic shock. These blood samples were stored in biochemical tubes with separators and centrifuged and then transferred to Eppendorf tubes to avoid hemolysis.

Specimens were preserved at -20 degrees Fahrenheit for a maximum of 5 months until all samples were examined for serum tau levels simultaneously.

Daily GCSs and SOFA scores were obtained throughout the hospitalization to determine the patient’s clinical progression and severity of sepsis, respectively. The SOFA score is derived from 6 different scores that are determined by the level of impairment in respiration, coagulation, hepatic function, cardiovascular function, central nervous system function and renal function. SOFA scores range from 0 to 4, which indicate normal function to complete malfunction, respectively. The scores are tabulated by recording the lowest value for each of the 6 categories during the first 24 hours following the diagnosis of sepsis. Note that the GCS is used to quantify the SOFA score for the central nervous system.[5] Patients with a GCS lower than 15 in the absence of clinical or laboratory evidence of direct brain infection were diagnosed with SAE. Patients diagnosed with sepsis or septic shock were subdivided into two groups: SAE and non-SAE subjects. Patients were monitored throughout hospitalization until death or discharge. For patients that were discharged, the Glasgow Outcome Score (GOS) was measured to determine their neurological state at the end of the monitoring period. Serum tau levels were compared with SAE development and SAE patients’ morbidity and mortality.

Statistical analysis Statistical analysis was performed using Statistical Package for the Social Sciences (SPSS) 15.0 and MedCalc 10.0 statistical program. To determine whether the data was normally distributed, the data was analyzed using the KolmogorovSmirnov test. Data are represented as the mean±one standard deviation (SD). Student’s t-test was used to compare daily tau levels of the SAE and non-SAE groups with SOFA scores, indictors of prognosis, or GOS. Pearson correlation analysis was used for normally distributed parameters and the Spearman correlation analysis was used for data that did not follow a normal distribution. A p-value less than 0.05 was considered statistically significant.

Results From February 2, 2009 to July 30, 2009 there were 98 patients diagnosed with sepsis or septic shock that were admitted to the university emergency medicine department.

Of those patients 28 were removed from the study as they qualified for the exclusion criteria (Table 1). Study subjects’ demographics including age, gender, previous diseases, other clinical diagnoses, and blood culture results are summarized in Table 2. Patients that demonstrated a GCS lower than 15 were diagnosed with SAE. Of the 37 patients diagnosed with SAE, 94.6% were diagnosed upon admission and 5.4% were diagnosed later during their hospitalization.

Initial mean serum tau protein levels were obtained at admission from all 70 patients diagnosed with sepsis or septic shock, and these levels were organized according to whether the patients progressed to SAE or not (Table 3). Although tau levels were higher in patients that developed SAE, there was no statistically significant difference with patients that did not develop SAE. Also there was no significant correlation between GCS and serum tau protein levels (Table 4). Thirtysix patients died throughout the course of the study, and of

those patients 91.7% were diagnosed with SAE as opposed to 8.3% that died without the SAE diagnosis (p<0.0001).

Analysis of the relationship between SAE and mortality revealed a positive correlation (r=0.800, p<0.0001).

The mean SOFA score for SAE patients was 7.5±2.3, which was significantly higher as compared to the SOFA score for non-SAE patients (3.8±2.8, p<0.001). Initial SOFA score and SAE development were positively correlated (r=0.604, p<0.0001). Also there was a significant correlation between SAE development and subsequent mortality (r=0.487, p<0.0001). No significant correlation was observed between initial SOFA score and mean tau protein levels over the three day interval following SAE diagnosis (p=0.920; p=0.554; p=0.858, respectively).

For the 70 patients diagnosed with sepsis or septic shock upon admission, the GOS prognostic calculation is shown in Table 5. There was no correlation determined between GOS and mean serum tau protein levels over the 72 hour period after SAE diagnosis (p=0.43; p=0.31; p=0.19, respectively).

Mean serum tau levels organized according to mortality due to SAE are shown in Table 6. As you can see, there are no significant differences between mean serum tau levels among those who died with SAE and those who survived with SAE.

Discussion Tau protein is a potential biochemical marker that makes up a structural component of microtubules. This protein maintains the stability of axonal microtubules so to facilitate rapid axonal transport.[6] Neuron integrity is compromised following traumatic brain injury and tau escapes from the intracellular compartment into the CSF and the serum.[7] Zemlan et al.[7] reported that initial CSF C-tau levels were a relevant predictive marker for intracranial pressure. Interestingly, C-tau was particularly sensitive in determining a positive clinical outcome for patients with severe traumatic brain injury. To the best of our knowledge there is currently no literature regarding the correlation between serum tau levels and sepsis severity, SAE development and mortality due to SAE.

SAE is a reversible central nervous system dysfunction that is classically associated with sepsis. This condition gives rise to a spectrum of clinical presentations ranging from severe agitation to coma. In a study by Sprung et al.[8] septic encephalopathy developed in more than 50% of septic patients. Similarly, more than half of the patients in our study developed SAE. However, the mortality observed by Sprung et al. was greater for patients with SAE when compared to our investigation.[8] Seymour et al.[9] demonstrated that lower GCS scores are associated with higher SOFA scores. Jones et al.[10] showed that hospital mortality and SOFA score parallel each other for patients presenting with severe sepsis to the ED. Thus, SOFA scores provide valuable prognostic information regarding hospital survival. It was observed that SOFA scores upon arrival to the ED were significantly higher in patients that died as compared to their counterparts that survived. Likewise, a positive correlation was determined in our patients between SOFA score and SAE development and mortality.

Our study was the first to examine the relationship between SOFA score and tau protein levels. However, no significant correlation was found between them. Additionally, we strived to determine whether it was possible to use tau as a means to gauge a patient’s neurological condition at the conclusion of a patient’s hospitalization. This was performed by investigating whether tau protein levels are statistically correlated with GOS, but no such relationship was

established. Furthermore, no significant correlation was determined between survivors and non-survivors among patients developing SAE and serum tau protein levels. This suggests that serum tau levels cannot be used for prognostic purposes to reflect SAE patient mortality and neurological status among survivors.

markers were positively correlated with mortality.[2] Weigand et al.[12] reported that serum NSE levels possess high specificity and sensitivity in predicting mortality for severe sepsis in the intensive care unit. Overall, there are several possibilities for putative biomarkers that may serve as prognostic indicators for the clinical progression of SAE.

Easily obtainable and effective biochemical markers will be very useful in predicting the morbidity and mortality of SAE. Another potential biomarker is an astrocyte protein that is responsible for intracellular calcium homeostasis called S100B. It has been suggested that the presence of S-100B in the serum, urine or cerebrospinal fluid (CSF) indicates glial cell and blood-brain barrier damage. In particular, increased serum levels of this protein has been reported after cardiac arrest, traumatic brain injury, cardiac bypass surgery, perinatal asphyxia and other central nervous system injuries.[11]

Limitations

Dimopoulou et al. demonstrated that serum S-100B levels can predict the progression to brain death for children with brain injury.[11] An additional putative biochemical marker for SAE is neuron-specific enolase (NSE), which is a glycolytic enzyme localized in neurons. NSE has been utilized as a marker of neurological injury in trauma, cardiac arrest and other neurological disorders.[11] In a study of 170 adults with sepsis or septic shock, Nguyen et al. showed that there were high levels of both S-100B and NSE in 42% and 53% of the patients, respectively. This study also established that both

Because this was the first study investigating tau as a potential biomarker for the morbidity and mortality associated with SAE, there were several limitations inherent in our research design. Primarily, the investigation was conducted with a small sample size. More importantly, patients were triaged directly from the ED regardless of their preexisting health conditions that may have influenced the outcomes of this study. As such we recommend that larger multicenter studies be performed in other clinical contexts including intensive care units in order to draw a more generalizable conclusion regarding the association between serum tau and SAE. This study determined that serum tau levels do not reflect the extent of brain damage and the development of SAE. As such tau cannot be used as a supplementary biological indicator to the GCS in diagnosing SAE. Furthermore, there is no correlation between tau levels and SOFA score and SAE morbidity and mortality. Nevertheless, this was a prelimi-

nary study with several limitations and so the validity of our results must be further examined with larger multicenter cohort studies.

SUMMARY

ÖZET

Objectives The purpose of this study is to evaluate the necessity of computed tomography (CT) scans requested to examine the spine, extremity, or pelvis of orthopedic trauma patients in the emergency department.

Amaç Bu çalışmanın amacı acil ortopedik travma hastalarında omurga, pelvis ve ekstremitenin değerlendirilmesi için istenilen bilgisayarlı tomografinin (BT) gerekliliğini değerlendirmektir.

Methods We retrospectively screened the medical records of all patients who had a CT scan during their emergency department (ED) evaluation. All data were classified as either child (aged 0-14 years) or adult (aged >14 years).

Results Of the 32, 685 patients examined in the child and adult emergency trauma unit over one year, 1, 664 were recommended for an extremity, pelvis, or spine CT (7.02%). The mean age of the patients was 38.6 years (range 2-94 years).

Of these patients, 145 of the computed tomography scans (CTs) (80.1%) in the child group and 1, 108 CTs (74.7%) in adult group were negative.

Bulgular Çocuk ve erişkin acil travma bölümünde bir yılda muayene olan 32.685 hastadan 1.664 tanesinden ekstremite, pelvis veya vertebra BT (%7.02) istenmiştir. Hastaların yaş ortalaması 38.6 (2-94) idi. Bütün hastalar içinden çocuklarda çekilen BT’lerin 145’i (%80.1) ve erişkinlerin 1.108’i (%74.7) negatif olarak bulundu.

Conclusions The unnecessary use of CT in the emergency department to examine orthopedic trauma patients has drawn attention. Considering the risk of radiation to the patient, it is necessary to develop protocols to determine which emergency department patients should undergo computed tomography.

Introduction While computed tomography (CT) in addition to direct radiography is of great importance for the diagnosis and treatment of certain orthopedic patients in the emergency room, it is regarded as unnecessary for others.[1-3] The recent advancements in the quality of CT imaging and the ability to obtain CT sections at different planes have resulted in orthopedists demanding CT more often.[4,5] For example, a CT scan is a very useful imaging method in the evaluation of the posterior wall in acetabular fractures, the posterior elements, the loss of alignment and intra-canal fragments in spinal fractures, joint surfaces in intra-articular fractures, and especially the multi-planar epiphysiolisis in children.[5-7] The initial diagnosis, classification, and planning the treatment of spinal fractures in the Emergency Room (ER) is important.[8] Although pelvic fractures constitute only 3 % of all fractures, they are destructive traumas requiring careful attention in the ER. Since the mortality rate is high in pelvic traumas, their treatment and diagnosis should be aggressive.[9] For intra-articular fractures and infant fractures, the accurate diagnosis of the fracture is very important in order to accurately plan the treatment.[7] Though direct radiography can be used as an initial imaging method, CT is often used, especially in certain complicated locations. Now, CT is a reliable and safe method that can be used to examine spine traumas.[9] Yet, CT must be considered as a method to be used in addition to direct radiography. CT is indicated in instances especially when direct radiography is insufficient and findings are doubtful. In addition, even if there were no abnormal findings found with direct radiography, the presence of neurological deficiency, head trauma, persistent serious pain, and suspicion for serious injury indicate the need for CT.[1,6,10] Another benefit of CT is its capability to properly image the craniocervical, cervicothoracic, and posterior pelvic ring, which cannot be imaged via direct radiography.[9,10] The purpose of this study is to evaluate the necessity of CT scans requested for spine, extremity and pelvis examinations of emergency orthopedic trauma patients, and to determine the rates of positive and negative examinations.

Materials and Methods The records for the application of CT on children (aged 0-14 years) and adults (aged >14 years) visiting the emergency trauma section of the hospital over one year (1 January 20101 January 2011) were analyzed. During this analysis, we used emergency examination cards, the computer-based hospital registration system, and pre-CT direct radiographies and CT images taken from the computer archive (PACS). The analy-

ses were performed by one orthopedist and one radiologist. Patients that did not have direct radiographic images, a trauma history, or recorded examination findings were not included in the study. All patients who had CT scans due to their trauma history and their related direct radiographies were included in the study. The reasons for demanding a CT and the findings of the physical examination were recorded. The analyses that yielded no relevant findings from CT, those that contained irrelevant findings, and those analyses that did not yield more information than that obtained from the direct radiography of the relevant region and did not affect the method of treatment were considered negative CT. Those analyses which yielded findings consistent with the request, more information than that given by the direct radiography and therefore changed the method of treatment, and CTs that were requested to plan the surgical treatment were considered positive CT. Those fractures seen as normal in direct radiography but evaluated as occult fractures with CT were considered necessary CT. The CTs were grouped as either extremity, pelvic, cervical, thoracic or lumbar spine.

Iliac bone, sacrum, acetabulum and hip joint CTs were classified as pelvis CT. All data were also classified as either child (aged 0-14 years) or adult (aged >14 years).

The data were analyzed with the SPSS version 15.00 pack software program. Chi square and two sided analyses were used where appropriate, and the alpha value was accepted as 0.05.

Results The numbers of patients examined in the adult and child ERs of our hospital in 2010 were 183, 552 and 171, 450, respectively. Of the 32, 685 patients examined in the child and adult emergency trauma unit over one year, 1, 943 underwent an extremity, pelvis, or spine CT (5.9%). Both CT and plain radiographic imaging methods were used in 1, 664 patients. A total of 279 patients (14.3%), including those who had a CT scan but not a plain radiographic scan, and those whose examination findings were not recorded or no trauma history could be found were excluded from the study. The mean age of the adult patients was 42.06 years (15-94 years) and the mean age of the child patients was 10.2 years (2-14). Table 1 shows the distribution of the CTs according to anatomic locations. The number of occult fractures determined with CT scanning for adults and children, whose plain radiographic images were negative, were 102 and 12, respectively.

Out of the 493 extremity CTs requested for adults, a total of 176 (35.6%) were positive CT, with 37 detected to be occult.

Out of the 56 extremity CTs requested for children, a total of 18 (32.1%) were positive, with 2 detected to be occult. Out of the 169 pelvis CTs requested for adults, a total of 63 (37.2%)

were positive CT, with 14 detected to be occult. Out of the 6 pelvis CTs requested for children, 5 (83.3%) were positive, with 2 being occult positive.

Out of the 504 cervical CTs requested for adults, a total of 45 (8.9%) were positive CT, with 16 detected to be occult. Out of the 107 cervical CTs requested for children, 11 (10.2%) were positive, with 7 being occult. Out of the 44 thoracic spine CTs requested for adults, a total of 21 (47.7%) were positive CT, with 3 detected to be occult. Out of the 4 thoracic vertebra CTs requested for children, 2 (50%) were positive, with 1 being occult. Out of the 273 lumbar spine CTs requested for adults, a total of 70 (25.6%) were positive CT, with 32 detected to be occult. All of the 8 lumbar spine CTs requested for children were negative (Figure 1). The negativity among adults seemed to increase at older ages, and it was seen that CTs were requested especially for compression spine fractures.

1). The percentage of negative CTs in children was found to be significantly higher than that of adults (p<0.001). The percentage of negative CTs in spine locations in both the child and adult groups were significantly higher when compared to those of pelvic and extremity locations (p<0.001).

The percentage of negative cervical CT in both the child and adult groups was significantly higher than that of thoracic and lumbar CTs (p<0.001). We also observed that there was a significant difference in favor of children in using CT to detect occult fractures (p<0.004).

Discussion The use of CT has increased rapidly since its introduction to clinical medicine in the 1970s. Over the last two decades, CT use has increased 20 times in the USA.[11] In these examinations, the number of child patients varies between 6-11%.[12] Because of the rapid increase in CT use, cancer risk may become a public health problem in the future. According to the statistics, 0.4% of all cancer cases in the USA between 1991

and 1999 were caused by CT use.[13] This suggests that the benefits of a CT examination should be carefully considered before requesting one. However, CT examination should be used if it is medically necessary. Scans have to be carried out with the smallest dose required to obtain adequate information.[14] In our study, we retrospectively examined the 1, 664 CTs requested by the child and adult emergency room trauma department during 2010. In our country, there have not been any extensive epidemiological studies about the risk of cancer associated with CT. We hope that our study can inspire more of these studies in the future.

the risk of this radiation. The effects of radiation are not observed on the exposed individual, but rather on the further generations of that individual.[11,16,17] Therefore, it is essential to reduce the rates of preventable negative CT scans. In this study, we determined that the CT scans that gave negative results were mostly requested for old osteoporotic vertebra compression fractures, and for spine, hip and knee osteoarthritis patients.

In CT examinations, patients receive more radiation doses compared to those from conventional X-ray examinations (Table 2).[15] For instance, in a lung X-ray, the organ dose is about 0.01-0.15 mGy, while it is 10-20 mGy for a thorax CT and is 1.5-10 mGy for a pelvis or spine CT. This amount is about 5 mGy on average for the pediatric age group.[11,12,1517] According to Berrington de Gonzalez et al., 29, 000 cases of radiation-related cancer were seen in 2007 because of 72 million CT scans. It has been reported that 0.4% of the cancer cases diagnosed between 1991 and 1999 in the USA were caused by CT use.[13] Considering that 1,253 (75.3%) of the total 1,664 scans made within a year had negative results, it is possible to suggest that the use of CT increases cancer risk.

A substantial amount of cervical spine injuries were detected in the cervical symptomatic cases following acute traumas (2-6%). It is possible to overlook some of the information in the radiological examination of the cervical region. The critical point is that there is often failure to take radiography with proper quality and the correct interpretation.[18,19] In 1995 Campbell et al. reported that 20% of thoracic and lumbar burst type fractures, including posterior column, were misdiagnosed as stable wedge type compression fractures, including only anterior column, with plain radiographs.[20] In our study, positive spine CT was determined in 149 patients (15.8%), and occult fractures were diagnosed in 59 of these patients, which led to a change in these patientsâ&#x20AC;&#x2122; treatment strategies. In 52 patients, surgical treatment was performed as a result of the CT scans. Due to this reason, emergency service doctors should be skeptical and cautious concerning spine trauma patients.

In 1995, Ron reported that the age of exposure to ionizing radiation strongly affects the estimated risk for thyroid cancer, with those younger than 15 years having the strongest association. Children are more radiosensitive than adults, and the thyroid is considered one of the most radiosensitive organs in the body. With the increased use of CT for the evaluation of spinal trauma, it is important to determine the quantity of ionizing radiation that patients are being exposed to and

Spinal injuries account for approximately 2-5% of all childhood traumas.[21,22] It has been often reported that CT use in pediatric patients offers only a small amount of improvement beyond plain radiography in the detection of fractures and wrong sequences despite the increase in the amount of radiation the patient is exposed to. Since there is usually a need for sedation during the CT examination of children,

the advantages the CT examination has for adults, such as short and effective use of time, do not apply.[17,23] In our study, 89.8% of patients who underwent CT for cervical trauma were evaluated normally. The percentage of negative CTs in children was found to be significantly higher than that of adults (p<0.001). Due to these differences between adult and pediatric patients, a trauma protocol should be designed specifically for pediatric patients.

In a research study involving radiologists and ER doctors in the USA, it was reported that the radiation dose caused by CT examinations was disregarded by 75% of both of these groups. Fifty three percent of radiologists, 91% of ER doctors, and 97% of patients questioned do not believe the fact that CT examinations increase cancer risk. In addition, 93% of 18 patients questioned said that the benefits and risks of CT were not explained to them before the procedure. Almost all patients stated they were not informed about the radiation dose. Despite the fact that the radiation dose of CT is much higher than that of other radiological methods, both doctors and patients disregard this fact.[24] Due to the high rates of negative findings from those undergoing CT that were obtained in our study, the attitudes of both doctors and patients concerning CT should be taken into consideration.

Other than one study regarding unnecessary CT use for the evaluation of the spine and pelvis in the ER, little research has been reported about unnecessary computerized tomography.[1,3] Daglar et al.[1] found examples of negative CT examinations (51.2%) in the evaluation of spine and pelvic regions in orthopedics clinics. Slovis et al.[3] found that one third of all CTs were unnecessary. Based on this information, they reported that about 20 million unnecessary CTs are requested in the USA each year. In the present study, negative CT was found in 80.2% of children and 74.8% of adults, which is remarkably higher than the results of the cited study.[1] Daglar et al. determined that the negativity in spine CT examinations is significantly higher than that for pelvic CT examinations, and a similar finding was also obtained in our study (p<0.001).

One limitation of our study is that it has a retrospective design. The second limitation is the fact that evaluations of the sensitivity and specificity of plain radiographs and CT examinations were not carried out. While extremity, pelvis, thoracic, and lumbar spine CT evaluations were carried out by an orthopedist, cervical vertebra evaluations were made by a radiologist. However, these experts cannot always perform the evaluations in the ER due to the high density of patients. A reason for the high negativity ratio in our study may be the high patient density of our hospital. This single center study exhibits the tendencies of our institution, and therefore, generalization of these to the entire universe may

not be accurate.

In conclusion, although CT has significant diagnostic medical benefits, it has a great risk of causing cancer due to exposure to radiation. Before using CT, a careful consideration should be made about its pros and cons. Also, other imaging methods should be considered.

As can be seen from our study, further retrospective and prospective studies should be done on the use of CT for trauma patients in order to establish guidelines of when to use CT and to prevent its superfluous utilization. Emergency service doctors should be adequately trained on CT and should consult the orthopedist first when needed.

ÖZET

SUMMARY

Amaç Çalışmamızda acil servise boğulma başvurularında demografik özellikleri, klinik ve biyokimyasal parametreleri kullanarak olguların yönetimini ve prognozunu etkileyecek faktörleri belirlemeyi amaçladık.

Objectives We investigated the demographic characteristics and the clinical and biochemical parameters for drowning admissions to the Emergency Department (ED) so to improve measures to prevent drowning and to determine the potential prognostic indicators for patient morbidity and mortality.

Gereç ve Yöntem Geriye dönük olarak yapılan bu çalışmada acil servise Ocak 2011 ile Aralık 2012 tarihleri arasında suda batma/boğulma şikayeti ile başvuran erişkin ve çocuk hastaların demografik özellikleri, başvuru tarihleri, Glasgow koma skorları (GKS), kan sodyum ve potasyum düzeyleri, kan gazı analizleri, boğulmanın gerçekleştiği suyun özelliği, ölüm, taburculuk, yatış kararları hasta dosyalarından kayıt altına alındı. Hastaların klinik durumları Szpilman’nın basit skorlama sistemi ile sınıflandırıldı. Sonuçlar istatistiksel olarak değerlendirildi.

Methods A retrospective chart review with statistical analysis was performed with patients admitted to the ED with submersion or drowning between January 2011 and December 2012. General demographic characteristics, reasons for admission, admission dates, features of the water in which the drowning occurred, and discharge were collected for each patient. Patient clinical status was classified via the Szpilman scoring system. Further biochemical and clinical parameters were also analyzed including Glasgow coma scores (GCS), serum sodium and potassium levels, and blood gases.

Bulgular Boğulma/suda batmalar en fazla erkeklerde (%61), 15 yaş altında (%56.1), sıcak havalarda (%87.8) ve tatlı sularda (%70.7) görüldü. Dört hasta hayatını kaybetti, ölen hastaların tümü 15 yaş altında idi. Tatlı ve/ veya tuzlu suda boğulma ve/veya batma, kan elektrolit düzeylerinde istatistiksel olarak anlamlı değişiklik oluşturmamıştı. Szpilman’ın skorlama sistemine göre yapılan klinik sınıflama ile birlikte, GKS ve kan pH düzeyleri prognostik açıdan istatistiksel olarak anlamlı idi.

Results Drowning or submersion was mostly observed in males (61%) that were <15 years of age (56.1%). Conditions in which drowning most often occurred were during hot weather (87.8%) and in fresh water (70.7%). Four patients in our sample died, and all of them were <15 years of age. When fresh water drowning was compared with salt water drowning, there were no statistically significant differences observed in patient blood electrolyte levels. In addition to the Szpilman scoring system, the GCS and blood pH level were statistically significant prognostic indicators of patient morbidity and mortality.

Sonuç Risk altındaki yaş gruplarının eğitim ve görsel/yazılı medya ile bilinçlendirilmesi, açık sularda yeterli güvenlik önlemlerinin alınması, özellikle yaz aylarında daha dikkatli olunması ölümleri azaltacaktır. Hastaların prognozunun belirlenmesinde GKS, fizik muayene bulguları ve kan pH’sı kullanılabilir. Morbidite/mortaliteyi etkileyecek tüm parametrelerin birlikte kullanılabileceği detaylı yeni bir skorlama sistemi geliştirilmesi için yeni çalışmalara ihtiyaç vardır.

Conclusions Effective measures to prevent drowning include raising awareness for at-risk age groups via visual and written educational media. Moreover, it is paramount to maintain adequate security measures around open waters especially during the summer months. Useful prognostic indicators for patients admitted for drowning are physical examination, GCS, and blood pH. Additional studies are needed to develop a more accurate scoring system to better gauge the morbidity and mortality of patients presenting with drowning or submersion.

Giriş

Yeni tanımlamaya göre boğulma immersiyon/submersiyon yolu ile havayolunun su ve likitlere maruz kalması sonucu ortaya çıkan respiratuvar bozukluktur.[1] İmmersiyon, hava yolu açıkta olup vücudun suda kalması, submersiyon ise hava yolunun da su altında kalmasıdır (suda batma). Hasta herhangi bir zamanda kurtarılırsa boğulma süreci durur ve buna ‘’ölümcül olmayan boğulma’’ denir. Boğulmaya bağlı herhangi bir zamanda ölüm olur ise ‘’ölümcül boğulma’’ denir. Suda batma durumunda solunumsal bir bulgu oluşmaz ise ‘’sudan kurtulmuş’’ denir ve bu boğulma değildir.[1,2] Boğulmada prognostik değeri olan bir diğer sınıflandırma ise ’’tanıklı ve tanıksız boğulma’’dır.[3] Bu tanımlamaların dışında boğulayazma, kuru ve ıslak boğulma gibi yanlış terimler kullanılmamalıdır.[1] Boğulmalar dünyada kasıtlı olmayan ölümlerin %0.7’sini oluşturmaktadır ve yılda yaklaşık 500.000 üzerinde ölüm demektir. Ölümlerin çoğu 15 yaş altı ve erkeklerdedir.[4] Boğulma için risk faktörleri erkek cinsiyet, <14 yaş, riskli davranışlar, düşük sosyo-ekonomik düzey, eğitim ve güvenlik kontrol eksikliği, alkol kullanımı, eşlik eden nörolojik veya kardiyak hastalıklardır.[5-8] Çalışmamızın amacı, acil servise boğulma başvurularında demografik özellikleri, klinik ve biyokimyasal parametreleri kullanarak olguların yönetimini ve prognozunu etkileyecek faktörleri güncel literatür ışığında belirlemektir.

Gereç ve Yöntem Çalışma geriye dönük planlandı ve etik kurul onayı alındı.

Yıllık hasta sayısı 200.000 olan acil servise 1 Ocak 2011 ile 31 Aralık 2012 tarihleri arasında suda batma/boğulma şikayeti ile başvuran erişkin ve çocuk hastalar çalışmaya alındı.

Hastane bilgi işlem sisteminin programından, International Code of Disease-10 (ICD-10) tanısı boğulma (W65-75, T71) olarak kaydedilmiş erişkin ve çocuk acile başvuran 51 hastanın kayıtları bulundu. Submersiyon ve immersiyon ile boğulma dışında yanlış tanı kodu almış yedi hasta çalışma dışı bırakıldı. Çalışmaya alınan 44 hastadan 41’inin verilerine ulaşıldı (Şekil 1). Hastaların demografik özellikleri, başvuru tarihleri, fizik muayene bulguları, Glasgow koma skorları (GKS), kan sodyum ve potasyum düzeyleri, kan gazı analizleri, boğulmanın gerçekleştiği suyun özelliği, ölüm, taburculuk, yatış kararları hasta dosyalarından kayıt edildi.

Hastaların klinik durumları Tablo 1’de gösterilen Szpilman’ın önerdiği sınıflama ile 0-6 arasında gruplandırıldı.[9] Sınıflandırmada hipotansiyon tanımı; dokuz yaş üstü için sistolik kan basıncı (SKB) <90 mmHg, dokuz yaş altı için 70 + (2 x Yaş) formülünden hesaplanan değerden daha düşük değer olarak

belirlendi. Her grup için GKS değerleri karşılaştırıldı. Hastaların sekelli ve/veya sekelsiz taburculuk kararları, hastanede yatış süreleri ve ölüm prognostik belirteç olarak değerlendirildi.

Verilerimiz “SPSS for Windows versiyon 16.0” (SPSS Inc. Chicago, IL, ABD) programına kayıt edilerek istatistiksel analiz yapıldı. İstatistiksel analizlerde ki-kare testi kullanıldı. %95 güvenilirlik aralığında (GA), p<0.05 olması anlamlı kabul edildi.

Bulgular Hastanemizin kayıt programından Ocak 2011-Aralık 2012 tarihleri arasında boğulma/suda batma olarak kayıtlı 41 hasta çalışmaya alındı. Hastaların 25’i (%61), 16’sı (%39) kadındı.

Dördü (%9.8) 0-4 yaş, 19’u (%46.3) 5-15 yaş ve 18’i (%43.9) 16-65 yaş grubundaydı. Yaş ortalamaları 16.3±10.3’tü. Hastalardan en küçüğü iki, en büyüğü ise 58 yaşında idi (Şekil 2).

Otuz altı (%87.8) hasta Mayıs-Ekim, beş (%12.2) hasta ise Kasım-Nisan ayları arasında başvurdu. Hastaların 29’u (%70.7) tatlı suda, 12’si (%29.3) tuzlu suda boğulma idi. Dört hastada (%9.7) ölüm gözlendi ve ölen hastalar acil servise ölü duhul olarak gelmişti. Ölenlerin üçü (%75) tatlı suda boğulma ve üçü (%75) erkekti. Ölümlerin tümü 15 yaş altındaydı. Çalışmaya alınan 41 hasta Spilzman’ın klinik sınıflamasına göre sınıflandırıldığında, 28’i grup 1, yedisi grup 2, biri grup 3, biri grup 4, dördü grup 6 sınıfında idi. Grup 1’de beş hasta

pH değerinde hafif bozulma (pH>7.1) veya sosyal endikasyon nedeniyle, 24 saat gözlem sonrası taburcu edildi. Grup 2, 24 saat gözlem sonrası taburcu edildi. Grup 3 ve 4, üç gün takip sonrası sekelsiz taburcu edildi. Grup 6’daki dört hasta dışında ölüm gözlenmedi. Sınıflandırmanın mortalite ve hastanede yatış süresinde etkisi istatistiksel olarak anlamlı bulundu (p<0.05). Çalışmaya alınan 41 hastanın GKS’leri karşılaştırıldığında; grup 1’in 15, grup 2’nin 13-15, grup 3’ün sekiz, grup 4’ün altı, grup 6’nın GKS’si üçtü. Çalışmamızda GKS hastanede yatış süresi, taburculuk kararı ve mortalitenin belirlenmesinde, istatistiksel olarak anlamlı bulundu (p<0.05).

Tatlı suda boğulan hastalarda GKS daha düşük bulundu (p<0.05). Ölü duhul olan dört hasta dışında 37 hastanın kan pH değeri karşılaştırıldığında, dokuzunda (%24) metabolik asidoz, üçünde (%8) respiratuvar asidoz, 25’inde (%67) pH normal bulundu. İki hastada pH<7.1 idi ve bu hastalar grup 3 ve 4 sınıfında idi. Anormal kan pH’sinin taburculuk kararı ve hastanede yatış süresine etkisi istatistiksel açıdan anlamlı bulundu (p<0.05). Suyun tatlı/tuzlu su olmasının hastaların kan pH’sinde etkisi bulunmadı (p>0.05; %95 GA). Ölü duhul olan dört hasta dışında 37 hastanın biyokimyasal parametrelerine bakıldığında; ikisinde (%5.4) sodyum değeri düşük (<130 mmol/l), 35’inde (%94.6) normal; dördünde (%10.8) potasyum değeri düşük (<3 mmol/l), 33’ünde (%89.2) normal bulundu. Laboratuvarımızdaki normal sodyum değeri 136-145 mmol/l, normal potasyum değeri 3.75.5 mmol/l’dir. Sodyum değeri düşük olan iki hasta tatlı suda boğulma idi. Sodyum ve potasyum düzeyleri prognostik belirteç açısından anlamlı değildi (p>0.05). Suyun tatlı/tuzlu su olmasının hastaların sodyum ve potasyum düzeyinde,

taburcu/yatış kararında, hastanede yatış süresinde etkisi bulunmadı (p>0.05; %95 GA).

Tartışma Boğulmalar kaza sonucu ölümler arasında beşinci sırada yer almaktadır. On beş yaş altı çocuklarda ise ikinci sırada kaza ile ölüm sebebidir.[10] Çalışmamızda 41olgunun dördünde ölüm gözlenmiştir ve tüm ölümler 15 yaş altındadır. Boğulmalarda yaş dağılımlarının ve yaşa göre mortalite oranlarının tanımlanması boğulmayı önleyecek tedbirlerin alınması açısından önemlidir. Yaşın boğulma için risk faktörü olduğunu söyleyen çalışmalar olduğu gibi, birçok çalışmada yaşın sonucu etkilemediği gösterilmiştir.[11,12] “Morbidity and Mortality Weekly Report”ta yayınlanan çalışmada 2005-2009 yılları arası ölümlü ve ölümlü olmayan boğulmalar en fazla 4 yaş altında bulunmuştur.[13] Quan ve ark.[8] ile Nasrullah ve ark. nın[14] yaptığı çalışmada en fazla ölüm dört yaş altında görülmüştür. Dünya Sağlık Örgütü’ne göre 5-14 yaş arasında en fazla ölüm gözlenmiştir.[4] İran’da yapılan bir çalışmada ölümlü boğulmaların tamamı beş yaş altında bulunmuştur.

[15] Çalışmamızda olguların %89.9’u 15 yaş üstü olduğu halde tüm ölümlü boğulmalar 15 yaş altında görülmüştür. Sonuçların coğrafi bölgelere göre değişiklik göstermesi dolayısıyla bölgesel verilerin ışığında boğulmaları önleyici programlar geliştirilmesi uygun olacaktır.

Yapılan çalışmalarda boğulma ve tüm yaş grupları için mortalite oranı erkeklerde daha yüksek bulunmuştur.[4,8,14-16] Bizim çalışmamızda da aynı sonuca ulaştık. Erkek bireylerin daha aktif olması, yüksek riskli aktiviteler yapması ve kültürel özellik nedeniyle açık alanlarda bulunma olasılıklarının daha yüksek olmasının, sonucu etkilediği söylenebilir.

Birçok çalışmada boğulmaların tatlı sularda daha fazla görüldüğü sonucuna ulaşılmıştır.[14,15,17,18] Deniz kıyısına yakın bölgede yapılan bir çalışmada ise tuzlu suya maruziyet ve ölüm daha yüksek bulunmuştur.[19] Çalışmamızda da tatlı suda boğulma sıklığı (Kanaletler, su drenaj alanları, baraj gölü gibi) daha fazladır. Dört ölüm olgusunun üçü de tatlı sudadır.

Bölgemizde kanalet ve drenaj suları, denize göre daha kolay ulaşılabilirdir. Tatlı suların yüzmeye elverişsiz alanlar olması ve yeterli güvenlik önlemlerinin alınmamış olması maruziyeti ve ölüm sıklığını artırmaktadır.

Bölgemizin ılıman iklim özelliği nedeniyle, başvuru tarihlerini Mayıs-Ekim ve Kasım-Nisan olarak sınıflandırdık. Başvurular en fazla sıcak aylar olan Mayıs-Ekim ayı arasında olmuştur (%87). Sonuç diğer çalışmalarla benzer bulunmuştur.[8,13,15,20] Sıcak havalarda dinlenme, serinleme ve eğlence amaçlı su kenarlarına gitme eğilimi boğulma riskini de beraberinde getirmektedir. Yaz aylarında, özelikle çocuklu ailelerin daha dikkatli olmaları; kanalet, su drenaj alanlarında ve göl kenarlarında tel örgüler gibi koruyucu önlemlerin alınması ile ma-

ruziyet ve ölümlerin azalacağını düşünmekteyiz.

Çıkar Çatışması

Tatlı/tuzlu suda boğulmanın, suyun aspirasyonuna bağlı, kan elektrolitlerinde değişiklikler yarattığı hipotezine yönelik Swann ve ark., Modell ve ark. ile Orlowski ve ark.nın yaptığı üç hayvan deneyinde venöz kan elektrolit düzeylerinde değişiklik saptanmamıştır.[21-23] Bizim çalışmamızda da tatlı/ tuzlu su maruziyetinin kan elektroit düzeylerinde (sodyum ve potasyum) değişiklik yapmadığı sonucuna ulaştık. Boğulma olgularında ortaya çıkan elektrolit düzey değişikliği, suyun kimyasal özelliğinin değil, boğulmanın patofizyolojisinde olan hipoksi veya anoksinin direkt etkisi sonucudur.[24]

Boğulmada prognozu belirlemeye çalışan birçok skorlama sistemi vardır, fakat hiçbiri tam güvenilir değildir.[2] Çalışmamızda mortalite’nin belirlenmesi amaçlı hastaları Szpilman‘nın yaptığı basit klinik skorlama sistemine göre sınıflandırdık.[9] Sınıflamaya göre mortalite oranı grup 1’de %0, grup 2’de %0.2, grup 3’te %5.2, grup 4’te %19.4, grup 5’te %44 ve grup 6’da %93 idi. Çalışmamızda, tüm ölüm olguları grup 6’da görüldü.

Bir çalışmada GKS 10 üstü olan tüm hastalar,[25] diğer bir çalışmada da GKS 5’in üstünde olan tüm hastalar yaşamış, beş’in altında olanlar ise ölmüştür.[26] Mosayebi ve ark.nın[15] yaptığı çalışmada GKS’si beşin üstünde olan tüm hastalar yaşamış, beşin altında olan hastaların %88.9’u ölmüştür. Çalışmamızda da benzer olarak yaşayan tüm hastaların GKS’si beş ve üzerinde, ölenlerin ise GKS’si üç idi. Çalışmamızda, klinik sınıflamanın ve GKS’nin prognozun belirlenmesinde kullanılabilir olduğu sonucuna vardık.

Hastalarda asidoz (pH<7.1) kötü prognoz ile ilişkilidir. Çalışmamızda yaşayan hastaların 35’inde (%95) pH düzeyi 7.2’inin üstündeydi. Prognozun belirlenmesinde kan pH’si da kullanılabilir parametreler arasındadır.

Çalışmamızın kısıtlayıcı faktörü geriye dönük olmasıdır. Acil servise başvuruda yanlış ICD-10 tanı kodu almış hastalara geriye dönük çalışma olduğu için ulaşamamış olabiliriz. Ölü duhul olan hastaların biyokimyasal parametreleri değerlendirilememiştir.

Suda boğulma önlenebilir kazalar arasındadır. Özellikle 15 yaş altındakiler risk altındadır. Eğitici okul programlarının düzenlenmesi, görsel ve yazılı medya kullanılarak özellikle çocuklu ailelerin bilinçlendirilmesi, açık sularda gerekli ve yeterli güvenlik önlemlerinin alınması ve yaz aylarında daha dikkatli olunması ölümler azaltacaktır. Çalışmamız sonucunda prognozun belirlenmesinde Szpilman’ın basit skorlama sistemi ile birlikte GKS ve kan pH’sinin kullanılabilir olduğu sonucuna vardık. Morbidite/mortaliteyi etkileyecek tüm parametrelerin birlikte kullanılabileceği detaylı yeni bir skorlama sistemi geliştirilmesi için yeni çalışmalara ihtiyaç vardır.

ÖZET

Heat stroke caused by electric blanket is rare and challenging clinical condition. Limited numbers of cases were reported; most of them were postmortem. In this paper, a 60-year-old man survived after electric blanket mediated heat stroke, was described. Also, diagnostic methods, clinical presentation and management were discussed briefly in the light of current literature.

Elektrikli battaniyenin neden olduğu ısı şoku sendromu nadir rastlanan bir klinik durumdur. Sınırlı sayıda olgu bildirilmiştir ve bunların çoğu ölüm sonrası bildirilerdir. Bu olgu sunumunda, elektrikli battaniyenin neden olduğu ısı şoku sonrası yaşamını sürdürebilen 60 yaşında bir erkek hasta bildirildi. Ayrıca, güncel literatür ışığında tanı metotları, klinik başvuru şekli ve hasta yönetimi tartışıldı.

Introduction

was 2120 ng/ml. He was hospitalized with the diagnosis of suspected acute pulmonary embolism. Immediate pulmonary computerized tomography angiography showed no signs of pulmonary embolism, also bilateral lower extremity venous color Doppler was normal. Cardiologic evaluation was entirely normal. Meanwhile additional laboratory tests were performed. They were shown in Table 1. Because of the diffuse muscle pain and increased muscle enzymes, he was consulted with neurology and rheumatology clinics. Electromyography was recommended and result was consistent with myopathy, thus muscle biopsy was performed. Parenteral steroid therapy was given to the patient because of suspected inflammatory myositis while awaiting the biopsy result. Muscle biopsy demonstrated that there was increased collagenized fibrous tissue in patches between muscle fibers and regenerative muscle fibers in some areas. No inflammatory infiltration was detected. Because of no definite clinical diagnosis despite detailed investigations, history of the patient was reexamined. He declared that he was slept on a very hot electric blanket before the onset of symptoms.

Electric blankets are widely used warming tools. Several accidents most commonly burning cases were reported due to inappropriate use. Besides, death cases were reported due to forgetting the electric blanket turn on and excessive heating of the individuals. Herein, we presented a rare surviving case of heat stroke because of electric blanket, which was initially misdiagnosed as pulmonary embolism.

Case Report A 60-year-old man, admitted to emergency department with dyspnea, chest pain, fatigue, nausea, and diffuse body pain. His blood pressure was 80/40 mmHg, pulse was 110/ min, respiratory rate was 25/min and axillary temperature was 37.8 °C (99.86 °F). Lung examination revealed bibasilar rales. Skin was tense, swollen and hyperemic. All of the limbs were painful. There was only mild tachycardia on 12-lead electrocardiogram. Arterial blood gas analyses showed compensated metabolic acidosis and hypoxemia. D-dimer level

After this interesting and important finding, the literature was reviewed and recognized that the major problems of our patient including renal insufficiency, hepatic enzyme elevation, rhabdomyolysis, coagulopathy, increased fibrin degrade products were all might be explained by “heat stroke” which is a form of heat-related illness. The patient was followed up in intensive care unit with supportive therapy for a week. After this therapy all of the disturbances and pain of the patient were relieved, all of the abnormal laboratory tests were returned to normal levels, patient was mobilized and skin findings were resolved. He was discharged after three days of follow up in step down unit. Follow up period for three months was eventful.

Discussion Heat related disorders vary among minor syndromes including heat cramps, heat syncope, heat exhaustion and serious life threatening conditions such as heat stroke.[1] Heat

stroke, emerges as a result of exposure to high temperature, is a disturbance characterized by tachycardia, tachypnea, normal or low blood pressure and elevation of body temperature over 40.6 °C and can result with complications including renal failure, liver failure, respiratory distress syndrome, rhabdomyolysis and disseminated intravascular coagulation.[2,3] Rare heat stroke cases due to electric blankets and rhabdomyolysis due to electric injury were reported.[4,5] In reported case, palpitation, chest pain, shortness of breath, diffuse muscle pain and cold sweating in association with hypoxemia and elevated D-dimer levels lead us suspicion of acute pulmonary embolism. However, normal results of pulmonary computerized tomography angiography and bilateral lower extremity venous color Doppler excluded the diagnosis of acute pulmonary embolism.

After the re-examination of patient history, this clinical situation was considered to be heat stroke despite subfebrile

fever of the patient. Because, it was stressed that if cooling procedures performed to the patient prior to arrival at the hospital, measured body temperature might be normal or subnormal.[2] Hypoxemia was attributed to inadequate respiration of the patient because of the effects of diffuse rhabdomyolysis to respiratory muscles. Fibrinolysis was considered to cause elevation of D-dimer levels. In a study of 16 cases exposed to heat stroke, from Bouchama et al. It was determined that coagulation and fibrinolysis activated and fibrin degrade products elevated in these patients.[6] Another problem of the patient was limitation of movement because of diffuse muscle pain. As a consequence of absence of inflammatory findings in muscle biopsy obtained from left deltoid muscle, normal rheumatologic tests, reversibility of symptoms, signs and laboratory findings, and healing without sequela excluded inflammatory muscle diseases and related this problem to rhabdomyolysis.

ted numbers of cases were reported involving heat stroke related to electric blanket. We presented a rare case in literature which survived after heat stroke due to electric blanket. There are no specific management methods; all of the therapies in literature were conservative and symptomatic.

Large, detailed investigations are needed to design optimal management methods. However, it is a useful warming tool especially in cold climates; electrical blanket should be used carefully for preventing undesirable events.

Almost all of the aforementioned complications of heat stroke including renal failure, liver failure, rhabdomyolysis, and elevations of fibrin degrade products and prolongation of prothrombin time and activated partial thromboplastin time were occurred in our patient. SUMMARY

ÖZET

Ectodermal dysplasia is a rare disease that present with: hypotrichosis, hypodontia, and typically absence of eccrine sweating. It consists of two basic forms; anhidrotic and hidrotic. In anhidrotic ectodermal dysplasia, the body’s thermoregulatory mechanism is impaired due to lack of sweat glands. These patients may therefore present to the emergency department with hyperthermia.

This report describes a patient with anhidrotic ectodermal dysplasia brought to the emergency department in an unconscious state and with a fever of 41°C. This rare disease should be kept in mind in patients presented to ED with an unconscious state due to hyperthermia. Typical clinical appearance is a useful clue for the diagnosis. Proper antipyretic therapy is sufficient for the treatment.

Ektodermal displazi, hipotrikozis (tüylerin yokluğu), anadonti (dişlerin yokluğu) ve tipik olarak ekrin ter bezlerinin yokluğu ile kendini gösteren nadir bir hastalıktır. İki temel form içerir, anhidrotik ve hidrotik. Anhidrotik ektodermel displazide (AED) ter bezleri olmadığından vücudun termoregulatuvar mekanizması bozuktur. Bu hastalar acil servise hipertermi ile başvurabilir. Bu yazıda, bilinç kapalılığı ve 41°C ateş nedeni ile acil servise getirilen anhidrotik ektodermal displazili bir hasta sunuldu. Bu nadir görülen hastalık hipertermi nedeni ile bilinci kapalı anhidrotik ektodermal displazili hastalarda akılda bulundurulmalıdır.

Tipik klinik görüntüsü tanı koymakta faydalıdır. Uygun antipiretik terapi tedavisi için yeterlidir.

Introduction

hair, missing or conical-shaped teeth, decreased sweat and mucous glands, hypoplastic skin, and heat intolerance with exercise or increased ambient temperature.[1] The greatest risk arises from their inability to sweat that result in episodes of severe hyperthermia. Mortality of ED is highest during the first year of life, but remains elevated throughout early childhood. Later in life, ambient temperatures of more than 25°C, exposure to the sun, and physical exercise may cause serious hyperthermia, often constraining AED patients in their professional careers and daily activities, especially in sports and traveling. Ectodermal dysplasia (ED) is a genetic disorder characterized by defective development of teeth, hair, nails, and eccrine sweat glands. The most prevalent form of ED is inherited as an X-linked condition; however, autosomal dominant and autosomal recessive forms of the disorder have been described. which may accelerate neuromuscular fatigue, reduce endurance, and performance or may even lead to dangerous conditions such as exertional heat stroke.[2] Herein, we described a patient with AED who admitted to our emergency department in an unconscious state with hyperthermia. We discussed the clinical characteristics and management of this rare disease.

Case Report A 20-year-old male patient was found in an unconscious state lying on the street and was brought to our emergency department. Upon admission, his pulse was 122/min, blood pressure was 90/60 mmHg, respiration rate 22/min, oxygen saturation %84 and body temperature 41°C. There were no signs of trauma, swelling or echymosis over his body. At systematic examination, pulmonary auscultation was normal and the heart was rhythmic and tachycardic. At neurological examination the patient retracted the extremities following painful stimulus (Glasgow Coma Scale: 10). There was no nuchal rigidity and reflexes were normal. He had very spare and fine hair, beard and moustache, with eyelashes so few as to be non-existent (Figure 1a). Teeth were entirely prosthetic.

The nasal root was collapsed, there was a pronounced supraorbital ridge and the ears were bowl-shaped and protruding (Figure 1b). The upper lip was protuberant. The skin was dry, hairless and smooth. Bedside blood glucose level was 130 mg/dl. Biochemical tests (blood urine nitrogen, creatinine, sodium, potassium, alanine aminotransferase, aspartate aminotransferase and thyroid function tests), electrolytes and hemogram were all normal. In order to exclude a possible intracranial pathology for unconsciousness we performed Cranial CT. Cranial CT revealed no remarkable pathology.

to disclose his unconscious state. Intravenous fluid replacement (1000mlt NaCl/h) was performed. External cold pack application was applied and intravenous Metamizole sodium (1 gr) was given to reduce the fever. The fever gradually began declining and consciousness began to return.

The patient regained full consciousness after approximately 4 hours. His past medical history revealed that he had anhidrotic ectodermal dysplasia and had walked around in the sun for some 3 h that day. The patient was aware of his disease and knew that he should not do exercise in hot weather but he never had a situation like this before in similar weather conditions. The patient was discharged in a healthy condition at the sixth hour.

Discussion Ectodermal dysplasia is a group of diseases presented with a characteristic triad of symptoms; hypotrichosis, anodontia and anhidrosis. It is seen in 1/100000 live births across all races. It mainly affects males (90% of the cases).[3] One hundred fifty-four types in 11 subgroups have been described.[4] Ectodermal dysplasia is frequently seen in two forms, anhidrotic and hidrotic. Anhidrotic ectodermal dysplasia is transmitted as an autosomal recessive trait, and is less common than the hidrotic form.[5] Characteristics of the syndrome were first described by Darwin.[5,6] The genetic and transmitted aspects and clinical characteristics were described

by Christ, Siemens and Touraine.[7] The main characteristics of this rare disease are sparse, fine and short hair eyebrows and eyelashes, soft, smooth and dry skin and sparse yellow hairs covering the entire body. In facial appearance the nasal root is depressed, there is a pronounced supraorbital ridge and dry, protuberant lips and prominent ear bowls. seen. Teeth may be totally or partially absent. Diagnosis is made on the basis of typical clinical findings.[8] Our patient’s appearance matched the majority of descriptions in the literature. Eczema, frequent infections and unexplained fever are other diagnostic clues.[9] Nijs et al.[10] reported repeating high fever in a nine-month-old male patient brought to the emergency department for that reason and described the approach they adopted. Patients presenting to the emergency department with different clinical conditions can easily be treated when their diseases are known. But if the disease is overlooked, this can give rise to poor outcomes because of the severe loss of time involved.

The mechanism that regulates body temperature is the thermoregulatory center in the hypothalamus. Ambient temperature changes can be controlled by this center without a change in body temperature. As the surrounding temperature rises, body temperature is reduced through vasodilatation in the skin capillaries and increased sweating. These patients, with no sweat glands, can develop hyperthermia in excessively hot weather. And hyperthermia can result in loss of consciousness. Our patient had a fever of 41 degrees.

Awareness of the typical facial and skin findings of the disease would have facilitated management of this patient brought to the emergency department.

There is no specific treatment for ectodermal dysplasia.

Patients are advised to avoid extreme heat and excessive activity. Patients whose temperatures rise must be kept in environments where the ambient temperature is reduced.

Air-conditioning, wet T-shirts and cold showers are simple measures for preventing fever developing.[11] We performed cold treatment with wet cloths in our case. Fever declined fell progressively and consciousness returned.

Conclusion Patients with ED may present to the emergency department with hyperthermia-associated loss of consciousness. Typical facial appearance and absence of teeth may help to identify this rare disease. Timely recognition of the condition may prevent unnecessary diagnostic work-up and provide faster treatment. ED physicians should keep this rare disease in

mind with high fever and unconscious state.

TANI: Tenisçi Baldırı Hastaya fizik muayene bulguları ve MRG değerlendirmesi sonucunda gastrokinemius kasının medial başının muskulotendinöz bileşke seviyesinden rüptürü, diğer adıyla “Tenisçi baldırı” tanısı kondu. MRG görüntülerinde gastrokinemius ve soleus kasları arasında kanama ve hematom oluşumu görülmektedir.

Hastaya istirahat, non-steroid antienflamatuvar ilaç ve buz uygulamasını içeren konservatif tedavi düzenlenerek taburcu edildi. On beş gün istirahat ardından ayak bileği ve diz egzersizlerine başlandı. Bir ay sonraki yapılan son kontrolünde hastanın hiçbir şikayeti kalmadı ve normal yaşantısına döndü.

ÖZET

SUMMARY

Türkiye’de özellikle Karadeniz Bölgesindeki dağların kuzeye bakan yamaçlarında doğal yayılış gösteren, halk arasında daha çok “Deli bal” olarak bilinen grayanotoksin, Na kanalları üzerinde toksik etki gösteren, Ericaceae familyası bitkilerinin çiçek nektarı ve polenlerinden üretilen ballarda bulunan doğal bir üründür. Deli bal zehirlenmesinde olgular çoğunlukla senkop, bradikardi, terleme, halsizlik, bulantı ve kusma semptomları ile başvurmaktadır. Daha ciddi olgularda AV blok ve asistoliye kadar ciddi kardiyak ritm bozuklukları olabilir. Acil servisimize 2010 yılı Ekim ayı boyunca, terleme, halsizlik, bulantı, kusma ve presenkop şikayetleri ile başvuran hastalar içinde EKG bulgusu bradikardi olarak tespit edilen 5 olgunun deli bal zehirlenmesine bağlı olduğu saptanmıştır. Bu yazıda halsizlik, bulantı, kusma ve presenkop şikayetleri ile acil servise başvuran ve EKG bulgusu bradikardi olan hastalarda ‘’Deli bal zehirlenmesi’’nin de akılda tutulmasını vurgulamak istedik.

Mad honey poisoning cases often appeal with syncope, bradycardia, sweating, weakness, nausea, vomiting like symptoms. In more serious cases, AV block and cardiac rhythm disorders serious enough to be asistoliye. Our Emergency Department during the month of October of 2010, sweating, weakness, nausea, vomiting, and bradycardia, electrocardiographic findings in patients presenting with symptoms of presyncope identified as Mad honey poisoning was found in 5 cases. In this our article we presented to the emergency department with complaints of weakness, nausea, vomiting and presyncope and electrocardiographic findings in patients with bradycardia wanted to emphasize to keep in mind in mad honey poisoning.

ponticum (mor çiçekli orman gülü) ve Rhododendron flavum (sarı çiçekli orman gülü) türleri bu toksini ihtiva ederler.

İlk kez MÖ 401 yılında, Atinalı tarihçi ve ordu komutanı olan Xenephon tarafından tanımlanan deli bal zehirlenmesine en sık Doğu Karadeniz bölgesinde rastlanır ve zehirlenmeye Ericaceae familyasına ait bitkilerden grayanotoksin içeren türler sebep olur.[1] Grayanotoksin, Na kanalları üzerinde toksik etki gösteren, Ericaceae familyası üyelerinden Rhododendron L. (orman gülü) gibi bazı bitki türlerinin çiçek nektarı ve polenlerinden üretilen ballarda bulunan doğal bir üründür.[2] Türkiye’de özellikle Karadeniz Bölgesindeki dağların kuzeye bakan yamaçlarında doğal yayılış gösteren Rhododendron

edilen hastaların anamnezleri irdelenmiş olup 5 olgunun deli bal zehirlenmesine bağlı olduğu tespit edilmiştir. Mevcut semptom ve bulgular ile gelen ve EKG bulgusunda bradikardi tespit edilen olgularda anamnezde deli bal yeme öyküsü sorgulanmasının gerekliliğini vurguladık.

Olgu Sunumu Acil servise 2010 yılı Ekim ayı boyunca başvuran 21700 hastanın 123’ünde terleme, halsizlik, bulantı, kusma, diplopi, presenkop ve elektrokardıyogramlarında bradikardi tespit edilmiştir. Bu vakaların 5’inde 1-2 saat içinde deli bal yeme öyküsü anamnezlerinden alınmıştır. Gelen hastaların 5’i de erkek ve yaşları sırasıyla 32, 44, 47, 52 ve 63 olup, daha öncesinde herhangi bir kardiyak rahatsızlıklarının olmadığı öğrenildi. İlaç kullanım öyküsü de olmayan hastaların, geldiklerinde tansiyon arteriyel (TA arteriyal), nabız, solunum sayısı, kan şekeri düzeyi, kardiyak panelleri, biyokimyasal tetkikleri ve EKG’leri alınmıştır. Gelen 2 hastada bal yeme sonrasında yaklaşık 45 dakika içinde baş dönmesi, terleme, halsizlik semptomları olmuştur. Diğer 3 hasta ise bulantı, kusma, presenkop, aşırı terleme şikayetleriyle başvurmuşlardır. Tüm hastalarda mevcut semptomların yanında EKG bulgularında bradikardi tespit edilmiş olup, sırası ile nabızları 37, 52, 33, 42 ve 40 idi. Göğüs ağrısı öyküsü olmayan hastaların TA arteriyal basınçları sırası ile 80/50, 95/50, 70/40, 85/50 ve 75/45 idi. Alınan kan tetkiklerinde kardiyak panellerden CK, CK-MB ve troponin hepsinde normal sınırlarda idi. Hastalar geldiklerinde monitörizasyon ve hidrasyona başlanmasıyla birlikte 1 mg atropin puşelendi. Hastalar sık aralıklarla çekilen EKG ile monitörize şekilde takibe alındı. Atropin sonrasında hastaların nabızları ve TA arteriyel basınçları 10 dakika içinde ivedi şekilde yükselmiş olup, 12-24 saat takibe devam edildi.

Hastalarda ek bir patoloji oluşmaması üzerine, önerilerle taburcu edildi.

Tartışma Türk halkı arasında “delibal” olarak bilinen ve Nepal, Brezilya, Japonya ve Türkiye’de de Karadeniz bölgesinde üretilen balda bulunan Rhododendron ponticum isimli nektarın şiddetli bradikardi ve hipotansiyon yaptığı bilinmektedir.[4] Zehirlen-

meye ait semptom ve bulgular grayanotoksin 1 ve 2’nin hücre membranındaki Na kanallarını bloke etmesine ve periferik vagal tonusu artırmasına bağlı olarak ortaya çıkar.[5,6] Hücre zarında sodyum geçirgenliğinin artmasıyla repolarizasyon inhibe olur ve hücre membranı depolarizasyon süresi uzar.

Sürekli depolarizasyon hali aksiyon potansiyelini azaltır ve sinus nodu disfonksiyonuna yol açar.[7] Toksik dozun ne kadar olduğu net olarak bilinmemesine rağmen daha önce yapılan çalışmalar bir çay kaşığı balın dahi intoksikasyon yapabileceğini göstermiştir.[8] Zehirlenmeye ait bulguların şiddeti doza bağlı olarak artar. Bu semptom ve bulguların hemen tamamı ilk 24 saat içinde normale döner.[4] Düşük dozlarla olan zehirlenmelerde kalp üzerine olan kolinerjik etkiler ön plandayken, yüksek dozlarda sürrenal medullada epinefrine bağlı etkileri ön plana çıkar.[9] En sık görülen etkileri %90 ile bradiaritmiler ve hipotansiyondur. Literatürde az görülse de asistoli, miyokart enfarktüsü, toksik hepatit gelişen ve ölümle sonuçlanan olgular da bildirilmiştir.[1,10,11] Özhan ve ark.[12] 2002 yılında kendi acil servislerine başvuran, bal intoksikasyonu düşündükleri 19 vakayla retrospektif bir çalışma yapmışlardır. Bu 19 vakanın 15’inde sinüs bradikardisi, 4 tanesinde ise atriyoventriküler blok tespit etmişlerdir.

Hastaların 30-180 mg arasında bal yedikleri tespit edilmiştir.

Hastaların hepsi 2-9 saat arasında 0.5 mg atropine cevap vermiştir. Yirmi dört saat içerisinde kan basınçları düzelmiştir.

Gündüz ve ark.[13] tarafından 2007 yılında bal yedikten sonra baş dönmesi ve senkop şikayetleri ile acil servise başvuran hastanın takibinde kardiyak arrest gelişip, ardından 0.5 mg atropine yanıt alınan daha sonra geçici pace implante edilen bir vaka bildirilmiştir. Gündüz ve ark.[14] 2008 yılında yaptıkları derleme çalişmasında on iki farklı olgu serisini incelemiş olup, %75’inde nonspesifik bradikardi veya sinüs bradikardisi tespit etmişlerdir.

Acil servise gelen hastalarda EKG bulgusu olarak bradikardi tespit edilmesi halinde hastaların anamnezi çok önemli bir yer tutmaktadır. Özellikle altta yatan herhangi bir yapısal kalp hastalığı olmayan, ilaç kullanım öyküsü bulunmayan (beta-blokerler, kalsiyum kanal blokerleri, antiaritmikler) sinüs bradikardili hastalarda anamnez esnasında bal yeme öyküsü mutlaka sorgulanmalıdır. bal zehirlenmesi hafif zehirlenme formu olarak algılansa da, özellikle fazla miktarda tüketilmesi, yaşlı hastalar ve beraberinde ilaç olarak sodyum kanal blokerleri kullanan hastalarda ölümlere yol açabilir.[15]

Sonuç Acil servise bulantı, kusma, halsizlik, presenkop şikayetleri ile başvuran ve EKG bulgusu bradikardi olan hastalarda ‘’Deli bal zehirlenmesi’’ olabilmektedir. ÖZET

SUMMARY

Uvula ödemi, özellikle küçük yaşlarda yaşamı tehdit edebilen nadir bir durumdur. Travma, alerjik nedenler, enfeksiyonlar gibi altta yatan patolojiler açısından araştırılması gerekir. Bu yazıda ip yutulması sonrası ipin uvula etrafına düğümlenmesinden sonra gelişen, uvula ödemi ile seyreden bir olgu sunuldu. 2.5 yaşında kız çocuk hasta acil servise ailesi tarafından huzursuzluk, yutkunma güçlüğü, beslenememe ve nefes darlığı şikayeti ile getirildi. Hastanın fizik muayenesinde uvulada ödem ve uvulanın distalinde düğümlenmiş ip görüldü. Hastaya iv steroit yapıldı ve acil olarak maske anestezisi altında düğümlenmiş ip kesilerek cıkartıldı. Belirti ve bulguları gerileyen hasta 12 saat sonrasında gerekli önerilerle taburcu edildi.

Uvula ödemi, mekanik travmalarla da oluşabilir ve ayırıcı tanısında ip düğümlenmesi de düşünülmelidir.

Uvula edema is a life threatening condition especially in children. Probable causes should be evaluated such as, trauma, allergic conditions and many infections. In this article ve reported a case that developed uvula edema after ingestion of a piece of rope and its incarcerating the uvula. Two and a half years old girl addmited to our emergency clinique by his familly with the complaints of irritability, difficulty in svaloving, dysphagia and shortness of breath. In physicial examination edema of the uvula and a piece of rope bound around the distal part of the uvula were noticed. IV steroid treatment started emergently and the rope has been cut out under the mask anethesia. Patient releiving right after the procedure has been discharged from the hospital 12 hours later. Edema of the uvula may also ocur due to mechanichal causes and incarceration by rope should be considered among the differential diagnosis.

Giriş

Ecballium elaterium (sinüzit tedavisinde halk arasında kullanılan acı hıyar, cırlatan, eşek hıyarı olarak bilinen bitki)[1,4-6] uygulaması, epiglottit, gastroduedonoskopi, bronkoskopi, orofarengeal cerrahi gibi üst solunum yollarını travmatize eden durumlar başlıca sebepleridir.[1-3]

Uvula ödemi, inhale veya yutulan allerjenler, enfeksiyon, travma ya da fiziksel nedenlere bağlı olarak gelişen yaşamı tehdit edebilen nadir bir durumdur.[1-3] Fonksiyonel ve anatomik malformasyonların uvula ödeminin gelişimini kolaylaştırdığı bildirilmiştir.[1,3] Literatürde bildirilen izole vakaların birçok etiyolojisi vardır. Olgu Sunumu

Bu amaçla steroit, antihistaminik, antibiyotik ve antienflamatuar ilaçlar kullanılabilir. Hava yolu sağlanamayan olgularda acil trakeotomi gerekebilir.[1]

2.5 yaşında kız çocuk hasta acil servise ailesi tarafından huzursuzluk, yutkunma güçlüğü, beslenememe ve nefes darlığı şikayeti ile getirildi. Bu şikayetlerinin yaklaşık 2 saat önce aniden başladığı, ailenin çocuğun ağız içine baktığında uvulada şişlik ve etrafında düğümlenmiş bir ip parçası gördüğü öğrenildi. Özgeçmişinde herhangi bir özellik yoktu. Hastanın fizik muayenesinde, orofarenks değerlendirmesinde uvulada ödem ve uvulanın distalinde düğümlenmiş ip görüldü (Şekil 1). Hastanın solunumunun güçleştiği, stridorunun olduğu, fonasyonunun bozulmuş, sesinin kabalaşmış olduğu gözlendi. Harici sistem muayenelerinde özellik yoktu. Kan basıncı ölçümü, tam kan sayımı, kan biyokimyası normal sınırlardaydı. Hastaya acil serviste intravenöz yoldan 20 mg steroit ve 1 ml (25 mg) feniramin maleat uygulandı. Hasta acil olarak ameliyata alınarak maske anestezisi altında uvula distalinde düğümlenmiş olan ip kesilerek çıkartıldı (Şekil 2). Ameliyat sonrası komplikasyon olmadı. Hastanın postoperatif saatlik periyodik takibinde uvula ödeminin gerilediği ve solunumunun düzeldiği izlendi. Postoperatif 4. saatte oral beslenmeye geçildi ve intolerans gelişmedi, 12. saatte bütün bulguları gerileyen hasta gerekli önerilerle taburcu edildi.

Tartışma Literatürde uvula ödeminin sebeplerini ve predisposan faktörleri inceleyen yayın sayısı azdır.[1] Uvula ödeminin en sık nedeni idiopatik (%50-55) olmakla birlikte, nedenleri arasında inhale ya da yutulan allerjenler, uyuşturucu, bazı ilaçlar, travma, enfeksiyon, tümörler, radyoterapi, bazı hastalıklar veya fiziksel ajanlar da sayılabilir.[1,3,4] Genel olarak uvula ödemi boğazda yabancı cisim hissi, boğaz ağrısı, ses kısıklığı, yutma güçlüğü ve konuşmada bozulma gibi semptomlar verir.[1-3] Uvula ödeminin tedavisinde öncelikli olarak hava yolunun sağlanması ve altta yatan sebebin uzaklaştırılması gerekir.[1,3]

Bu olguda uvula ödeminin nadir bir sebebi olarak literatürde daha önce rastlamadığımız uvula etrafına ip düğümlenmesini bildirdik.

Sonuç Sunduğumuz olguda olduğu gibi uvula ödemi mekanik travmalarla da oluşabilir. Bildiğimiz kadarıyla daha önce literatürde uvula etrafına ip düğümlenmesi nedeniyle gelişen uvula ödemi bildirilmemiştir. Uvula ödeminin ayırıcı tanısında düşünülmelidir.
