Değerli meslektaşlarım, Bahara merhaba dediğimiz bu günlerde 2. sayımız yine ilgi çekici konular ile dolu. İkinci sayımıza “İntraoperatif tramadol ve ketaminin tonsillektomi uygulanan hastalarda ameliyat sonrası ağrı üzerine etkilerinin karşılaştırılması” isimli klinik çalışma ile başlıyoruz. Keyifle okuyacağınız bu çalışmayı ve onu takip eden “Spinal anesthesia for elective cesarean section is associated with shorter hospital stay compared to general anesthesia”, “Sağlıklı gönüllülerde cinsiyet ve çalışma şartlarının ağrı eşik değeri üzerine etkisi’”, “Intracranial hypotension is a rare cause of orthostatic headache: a review of the etiology, treatment and prognosis of 13 cases” ve “Primer dismenore ağrılı hastalarda diklofenak sodyum ve parasetamol tedavi etkinliklerinin Vizüel Analog Skala kullanılarak karşılaştırılması” isimli çalışmaların hepimizin klinik pratiğimize katkı sağlayacağını düşünüyorum.

Oldukça seyrek görülen olgu sunumlarının irdelenerek tartışıldığı olgu sunumları ise yine dikkat çekici.

Dernek olarak düzenlediğimiz bölgesel toplantılar tüm hız ve başarıları ile devam etmekte. Şu ana kadar vermiş olduğunuz destek için derneğimiz adına teşekkür eder gelecekteki toplantılarda sizleri aramızda görmeyi dilerim.

YAZARLARA BİLGİ İLGİ ALANI VE AMACI Bu dergi Türk Algoloji (Ağrı) Derneği’nin süreli yayın organıdır. Üç ayda bir yayımlanır.

Ağrının yapısı, mekanizmaları ve tedavisi ile ilgili özgün araştırmalar yayınlanır. Multidisipliner bir yaklaşım ile ağrı ile ilgili temel ve klinik bilimlerde yapılan araştırmaların geniş kitlelere yayılması için bir forum oluşturulmaya çalışılır.

YAZI TİPLERİ Derginin kategori başlıkları şunlardır: 1. Derleme 2. Deneysel / Klinik Çalışma 3. Olgu Sunumu 4. Editöre Mektup GENEL AÇIKLAMALAR 1. Dergi Türkçe ve İngilizce olarak yayınlanır.

2. Editoryal Kurul tarafından uygun görülen metinler yayımlanır. Editoryal Kurul, yayın kurallarına uymayan metinleri yayımlamamaya ve düzeltilmek üzere yazarına geri göndermeye yetkilidir. “Derleme” kategorisindeki yazılar, Editoryal Kurul tarafından belirlenen yazarlardan yapılan istek üzerine kabul edilmektedir. Editoryal Kurul tarafından istenmedikçe bu kategoriler için yazı gönderilmemelidir. Ancak bu konuda editöre öneride bulunulabilir. Tüm yazılarda editöryel değerlendirme ve düzeltmeye başvurulur; gerektiğinde, yazarlardan bazı soruları yanıtlanması ve eksikleri tamamlanması istenebilir. Dergide yayınlanmasına karar verilen yazılar “manuscript editing” sürecine alınır; bu aşamada tüm bilgilerin doğruluğu için ayrıntılı kontrol ve denetimden geçirilir; yayın öncesi şekline getirilerek yazarların kontrolüne ve onayına sunulur.

3. Dergi, uluslararası tıbbi dergi editörleri kurulunca hazırlanan “Biyomedikal dergilere teslim edilecek metinlerde aranan ortak özelliklerin” 3. baskısındaki (1983) kurallara uygun olarak hazırlanmamış yayın metinlerini kabul etmez.

Metinler teslim edilmeden once bu kuralların yayınlandığı British Medical Journal 1988;296:401-5 veya Annuals of Internal Medicine 1988;108:258-65’e bakılmalıdır.

4. Dergiye yazı gönderilmesi sadece internet üzerinden yapılmaktadır.

Bunun için resmi web sitesi olan www.agridergisi.org adresindeki ilgili basamaklar takip edilmelidir.

5. Yayınlanması istenen metnin dayandığı çalışma, daha önce başka yerde yayınlanmış veya yayınlanmak üzere teslim edilmiş veya kabul edilmiş olmamalıdır. Özet biçiminde yayınlanmış bir ön bildirinin bitmiş haline yer verilebilir.

6. Dergide yayınlanan yazılar için telif hakkı ödenmez. Bu nedenle başvuru mektubunda telif hakkının dergiye bırakılacağı açıklanmalı ve metnin tüm yazarlarca okunduğunu ve onaylandığını belirten bir ifade bulunmalıdır.

7. Yayımlanmış şekil vb. gereçlerin yerinde basılabilmesi, tanınabilecek kişilerin resimlerinin kullanılabilmesi ve katkılarından dolayı kişilerin adlarının belirtilebilmesi için alınmış izinler posta ile ulaştırılmalıdır.

8. Yazarlar teslim ettikleri her şeyin birer kopyasını saklamalıdır.

9. Teslim edilmiş bir metnin tümünün veya bir bölümünün başka bir yerde yayımlanması söz konusu olursa Editoryal Kurul’a bilgi verilmesi zorunludur.

10. Araştırmalar için, ilgili kurumun bağlı bulunduğu etik komitenin onayının alınmış olması şarttır. Bu durum “gereç ve yöntem” bölümünde belirtilmelidir.

11. Yayımlanan yazıların sorumluluğu yazarlarınındır.

12. Yazarlara tıpkıbasım gönderilmeyecektir.

YAZIM KURALLARI 1. Yazı kategorilerine göre kelime sınırlamaları (özetler ve kaynak listesi hariç) aşağıda belirtildiği gibidir: Derleme: 5000 kelime; Deneysel / Klinik Çalışma: 3000 kelime; Olgu Sunumu: 1500 kelime.

2. Her kategoride yazıya 250’şer kelimeyi aşmayacak Türkçe ve İngilizce özet eklenmelidir.

3. Her kategoride yazı için en fazla 6 adet anahtar sözcük, Türkçe ve İngilizce olarak belirlenmelidir.

4. Yazının metin ve grafik dosyaları birbirinden ayrı olarak hazırlanmalı, şekiller metin belgesinin içerisinde yer almamalıdır. Dosyalar uygun şekilde isimlendirilmelidir (örn.: metin.doc, sekil1A.jpg, sekil3.tif, vb.).

5. Deneysel / Klinik Çalışmalar şu şekilde düzenlenmiş olmalıdır: “Giriş”, “Gereç ve Yöntem”, “Bulgular”, “Tartışma” ve isteğe bağlı olarak “Teşekkür”.

6. Tablolar, grafikler ve şekiller ayrı düzenlenmeli, sıra numarası verilmeli (Tablo 1, 2 gibi), başlık eklenmeli ve metnin ilgili yerinde “Tablo 1” ve “Tablo 2” gibi belirtilmelidir.

7. Kaynaklar, metin içindeki kullanım sırasına göre rakamlarla gösterilerek düzenlenmelidir. Yazı içinde geçen kaynak numaraları köşeli parantezle ve küçültülmeden belirtilmelidir. Kaynak listesinde yalnızca yayınlanmış ya da yayınlanması kabul edilmiş çalışmalar yer almalıdır. Kaynak bildirme “Uniform Requirements for Manuscripts Submitted to Biomedical Journals” (www.icmje.org) adlı kılavuzun en son güncellenmiş şekline (Şubat 2006) uymalıdır. Dergi adları Index Medicus’a uygun şekilde kısaltılmalıdır. Altı ya da daha az sayıda olduğunda tüm yazar adları verilmeli, daha çok yazar durumunda altıncı yazarın arkasından “et al.” ya da “ve ark.” eklenmelidir. Kaynakların dizilme şekli ve noktalamalar aşağıdaki örneklere uygun olmalıdır.

Özet

Amaç: Çocukluk çağında sıklıkla uygulanan tonsillektomi ameliyatlarında intraoperatif ketamin ve tramadol uygulamasının ameliyat sonrası ağrı ve analjezik ihtiyacı üzerine etkilerini karşılaştırmayı amaçladık.

Gereç ve Yöntem: Çalışmaya tonsillektomi ameliyatı planlanan, 5-15 yaş arası, ASA I-II sınıfında 90 hasta alındı. Standart genel anestezi sonrası olgular üç gruba ayrıldı. Kanama kontrolü sırasında ketamin grubuna 0.5 mg.kg-1 ketamin, tramadol grubuna 2 mg.kg-1 tramadol ve kontrol grubuna ise SF i.v. olarak uygulandı. Anestezi ve ameliyat süreleri ile derlenme odasında kalış zamanları kaydedildi. Ameliyat sonrası ağrı değerlendirmesinde NRS ve CHEOPS skalaları kullanıldı. NRS 3’den, CHEOPS 8’den büyük olduğunda; ilk 6 saatte rektal, sonrasında oral 15 mg.kg-1 parasetamol uygulandı. İlk analjezik zamanı, uygulanan toplam doz ve komplikasyonlar kaydedildi.

Bulgular: Olguların demografik verileri, anestezi ve ameliyat süreleri ile derlenme odasında kalış süreleri istatiksel olarak benzerdi (p>0.05). Kontrol grubunda, tramadol ve ketamin grubuna göre ek analjezik gerektiren hasta sayısı anlamlı olarak yüksekti (p<0.05). Tramadol ve ketamin grupları arasında ise fark bulunamadı (p>0.05). Ek analjezik verilme zamanı kontrol grubunda tramadol ve ketamin gruplarına göre anlamlı olarak kısa bulunurken (p<0.05), tramadol ve ketamin gruplarında ek analjezik verilme süreleri benzer olarak hesaplandı (p>0.05). Ek analjezik olarak kullanılan parasetamol dozu kontrol grubunda tramadol ve ketamin grubuna göre anlamlı olarak fazla (p<0.05) iken, tramadol ve ketamin grubunda ise benzerdi (p>0.05).

Tramadol ve ketamin gruplarında kontrol grubuna göre bulantı-kusma anlamlı olarak yüksek bulundu (p<0.05).

Sonuç: Pediatrik dönem tonsillektomi ameliyatlarında 2 mg.kg-1 tramadol ve 0.5 mg.kg-1 ketamin uygulaması ile etkili ameliyat sonrası ağrı sağaltımı sağlanmıştır.

Summary

Objectives: We aimed to compare the effects of ketamine and tramadol administered intraoperatively on postoperative pain and analgesic demand in children undergoing tonsillectomy operations.

Methods: Ninety children undergoing tonsillectomy, aging 5-15 years old, with ASA I – II , were included in this study. Cases were arranged in three groups and given standardized general anesthesia. After coagulation, ketamine (0.5 mg.kg-1) was administered intravenously (iv) in the ketamine group, and tramadol iv (2 mg.kg-1) was given in the tramadol group and the control group received a physiologic iv. The duration of anesthesia, duration of operation, and length of stay in the PACU were recorded. NRS and CHEOPS scales were used to assess postoperative pain. Paracetamol (15 mg.kg-1) was given rectally in the first 6 hours (hrs) and orally over the subsequent 6 hrs, if NRS greater than 3 and CHEOPS greater than 8 were observed. All data were recorded concerning the initial and total dosage of analgesic and the presence of complications within 24 hrs.

Results: Demographic data, duration of anesthesia, duration of operation, and duration of stay in the PACU were similar between groups (p>0.05). The number of patients requiring additive analgesic was higher in the control group. No differences were found between the tramadol and ketamine groups (p>0.05). Additional analgesic was given earlier in the control group (p<0.05), but the need for additional analgesic was similar in tramadol and ketamine groups (p>0.05). Paracetamol dosage was significantly higher in the control group (p<0.05), but similar between the tramadol and ketamine groups (p>0.05). The frequency of nausea and vomiting was found to be significantly higher in tramadol and ketamine groups compared to the control group (p<0.05).

Conclusion: Postoperative pain was effectively managed using 2 mg.kg-1 tramadol and 0.5 mg.kg-1 ketamine in pediatric tonsillectomies.

Giriş Tonsillektomi, kulak burun boğaz ameliyathanesinde en sık yapılan ameliyatlardan biridir. Tekrarlayan tonsillit, peritonsiller apse ve obstrüktif hipertrofi en sık rastlanan tonsillektomi endikasyonlarıdır. Tonsillektomi sonrasında ağrı, kanama, tonsil lojunda enfeksiyon, kulak ağrısı, yutma güçlüğü, ses ve konuşma bozukluğu sıklıkla görülen yakınmalardır.[1] Ameliyat sonrası ağrı, cerrahi travma ile başlayıp doku iyileşmesi ile sona eren akut bir ağrı şeklidir.[2] Şiddeti kişisel farklılıklar gösterse de ameliyat sonrası ağrı korku ve anksiyete kaynağıdır. Yeterince tedavi edilmemiş ameliyat sonrası ağrı özellikle çocukluk çağında anlamlı morbiditeye yol açar.[3] Çocukluk çağında sıklıkla uygulanan ameliyatlardan olan tonsillektomi sonrası, erken ameliyat sonrası dönemde hastaların çoğunda ciddi ağrı olmakta ve analjeziye ihtiyaç duyulmaktadır.

Ketamin, fensiklidin grubu nonbarbitürat bir intravenöz anesteziktir. Güçlü analjezik ve amnestik etkiye sahip olan tek intravenöz anestezik maddedir.

Analjezik etkisi daha ziyade somatik ağrılar üzerine dominanttır. Tramadol HCI ise, yapıca kodeine benzeyen fenilsikloheksanol türevi, akut ve kronik ağrı tedavisine uygun, merkezi etkili, sentetik bir analjeziktir.

Bizim bu çalışmadaki amacımız, tonsillektomi veya adenotonsillektomi ameliyatlarında intraoperatif olarak verilen ketamin ve tramadolün ameliyat sonrası analjezik ihtiyacına etkilerini karşılaştırmaktır.

Gereç ve Yöntem Çalışmaya, etik kurul onayı ve çalışma hakkında bilgilendirilen ailelerin yazılı onamı alındıktan sonra, elektif tonsillektomi ameliyat yapılması planlanan 5-15 yaş arası, ASA (American Society of Anesthesiologists) I-II sınıfında toplam 90 hasta alındı.

ASA sınıfı II’den büyük olanlar, 5-15 yaş dışındakiler, çalışmada kullanılacak ilaçlara allerjisi olduğu bilinenler, epilepsi ve kanama bozukluğu olanlar, karaciğer, böbrek, kalp ve akciğer hastalığı olanlar ile aileleri çalışmaya izin vermeyen çocuklar çalışma dışında tutuldular.

Ameliyat odasına gelmeden yaklaşık bir saat önce tüm hastalara oral 0.5 mg.kg-1 midazolam (Demizolam, Dem Medikal, Almanya) ile premedikasyon yapıldı. Ameliyat odasına alınan hastalara, standart I ve II derivasyonlarda elektrokardiyografi (EKG), kalp atım hızı (KAH), non-invaziv sistolik (SAB), diyastolik (DAB), ortalama (OAB) arteriyel kan basıncı ve periferik oksijen satürasyonu (SpO2) monitörizasyonu uygulandı (Dräger infinity kappa, Dräger Medical GmbH Lübeck, Almanya). Anestezi öncesi kaydedilen değerler bazal değerler olarak kabul edildi. Bazal değerin ± %20 sapması normal sınırlar olarak değerlendirildi. Bazal değerlerin ± %20’sinden daha büyük ve daha düşük değerler hipotansiyon, hipertansiyon, bradikardi, taşikardi olarak kabul edildi. Bu ölçümler, entübasyondan sonraki 1. dakikada ve daha sonraki her 5 dakikada bir ameliyat bitimine kadar tekrarlandı. Standart şeklinde monitörize edilen olguların anestezi indüksiyonunda 2 mg.kg-1 propofol (Propofol, Fresenius, Avusturya) iv olarak veya eğer çocuk damar yolunu kabul etmediyse %8 sevofluran (Sevorane Likid, Abbott, İngiltere) ve oksijen içinde, %70 nitröz oksit (%30:70 O2 / N2O) inhalasyonla uygulandı. Damar yolu açıldıktan sonra remifentanil (Ultiva, GlaxoSmithKline, İtalya), 1 µg.kg-1 yükleme dozundan sonra 0.15 μg.kg-1.dk-1 dozunda iv infüzyon ile idame edildi. 1.5 mg.kg-1 süksinilkolin (Lysthenon, Fako, Türkiye) iv olarak uygulanması ile yeterli kas gevşemesi oluşmasını takiben hastalar, uygun büyüklükteki oral RAE (Ring-Adair-Elwyn) tüpü ile entübe edilerek mekanik ventilatöre bağlandı (Dräger Primus, Dräger Medical GmbH Lübeck, Almanya). Entübasyon sonrası olguların end-tidal karbondioksit basıncı (ETCO2) monitörize edildi (Dräger infinity kappa, Dräger Medical GmbH Lübeck, Almanya). Anestezi idamesi %0.51 sevofluran, %30:70 O2/N2O ve 0.15 μg.kg-1.dk-1 iv remifentanil infüzyonu ile sürdürüldü. Sevofluran konsantrasyonu OAB ve KAH bazal değerlerinin ± %20 sınırları içinde tutacak şekilde titre edildi. Hastalar, 10 ml.kg-1 tidal volüm ve ETCO2 değerini 30-35 mmHg aralığında tutacak solunum sayısı ile ventile edildiler. Anestezi indüksiyonu ile anestezik ilaçların kesilmesi arasında geçen zaman “anestezi süresi” ve tonsillektomi pozisyonundan kanama kontrolü bitimine kadar geçen zaman ise “ameliyat süresi” olarak kabul edildi ve bu süreler kaydedildi.

İntraoperatif tramadol ve ketaminin tonsillektomi uygulanan hastalarda ameliyat sonrası ağrı üzerine etkileri

Olgular geliş sırasına göre üç gruba ayrıldı. Tonsiller çıkarılıp kanama kontrolü yapıldığı sırada ketamin grubuna (Grup K, n=30) 0.5 mg.kg-1 ketamin (Ketalar 10 ml flk, 50 mg.ml-1, Pfizer, Türkiye), tramadol grubuna (Grup T, n=30) 2 mg.kg-1 tramadol (Contramal 2 ml amp, 50 mg.ml-1, Abdi İbrahim, Türkiye) toplam hacim 4 ml olacak şekilde serum fizyolojik (%0.9 NaCl=SF) ile sulandırılarak ve kontrol grubuna (Grup S, n=30) ise yalnızca 4 ml SF iv olarak uygulandı. Ameliyat bitip kanama kontrolü sağlandıktan sonra anestezik ilaçlar kesildi, spontan solunumu yeterli olan hastalar ekstübe edilerek derlenme odasına gönderildi. Derlenme odasında hastalara sedasyon değerlendirilmesi için Wilson Sedasyon Skoru[4] (1: Tamamen uyanık, oryante; 2: Uykuya eğilimli; 3: Gözler kapalı, sözlü komutla açılıyor; 4: Gözler kapalı, orta şiddette fiziksel uyarıyla açılıyor; 5: Gözler kapalı, orta şiddette fiziksel uyarıya yanıt yok) uygulandı. Kas gücü, solunumu ve oksijen satürasyonu yeterli olan hastaların sedasyon skoru 2 ve altında olduğunda derlenme odasından servise transferine izin verildi.

Her hasta için derlenme odasına alınma ve taburcu edilme zamanı kaydedildi.

Aventis, Türkiye) ile 6. saatten sonra oral 15 mg.kg-1 parasetamol (Tamol pediyatrik şurup, 120 mg/5 ml, Sandoz, Türkiye) ile ağrı sağaltımı yapıldı. Her olgu için ek analjeziğe ihtiyaç duyduğu ilk zaman ve toplam ek analjezik dozu kaydedildi.

Verilerin istatistiksel analizinde “Statistical Package for Social Sciences (SPSS) for Windows Release 13.0”(SPSS Inc, Chicago, IL ) programı kullanıldı.

Veriler hasta sayısı, ortalama±standart sapma (SS) olarak sunuldu. Katagorik değişkenlerin arasındaki ilişki ki-kare testi ile analiz edildi. Gruplar arası karşılaştırmada parametrik değişkenler için OneWay ANOVA, parametrik olmayan değişkenler için Kruskal-Wallis testi uygulandı. Anlamlılık tespit edilen parametrelere Post-Hoc; ikili karşılaştırma testleri yapıldı. P<0.05 anlamlılık düzeyi olarak kabul edildi.

Bulgular İstatistiksel olarak değerlendirildiğinde çalışmaya alınan hastaların yaş, kilo, anestezi ve cerrahi süreleri benzerdi (p>0.05). İndüksiyon için iv propofol uygulanan hasta sayısı her üç grupta da benzerdi (p>0.05) (Tablo 1).

Olgulara ameliyat sonrası 1, 5, 10, 15, 20, 30 ve 45. dakikalar ile 1, 2, 6 ve 24. saatlerde ağrı değerlendirmesi için Numeric Rating Scale (NRS) ve Children’s Hospital Eastern Ontario Pain Scale (CHEOPS) uygulandı.[5] Yine bu süre içerisinde yan etkiler (bulantı-kusma, ağız kuruluğu, baş ağrısı, terleme, ajitasyon, halüsinasyon, aşırı sekresyon, allerjik reaksiyon ve diğer komplikasyonlar) kaydedildi.

Üç grupta hastaların ameliyat sonrası dönem ek analjezik gereksinimi, ek analjezik ilaca ilk kez ihtiyaç duyulan zaman, ek analjezik ihtiyacı olan hastalarda kullanılan parasetamol dozu karşılaştırıldığında kontrol grubu tramadol grubuna ve ketamin grubuna göre anlamlı olarak yüksek iken derlenme odasından çıkma süreleri gruplar arasında benzer olarak bulundu. Tramadol ve ketamin grupları arasında ise istatiksel olarak anlamlı bir fark bulunamadı (p>0.05) (Tablo 2).

Yan etkiler yönünden ise gruplar arasında fark vardı (p<0.0.5) (Tablo 3).

Tartışma Ameliyat sonrası dönemde ağrı sıkça rastlanan, hasta ve sağlık personelinin keyfini kaçıran bir komplikasyondur. Ağrı stres cevabın aktivasyonuna neden olarak kardiyovasküler sistem başta olmak üzere tüm organ sistemleri, metabolizma ve immün sistem üzerinde belirgin değişikliklere neden olur.

Daha önce cerrahi girişim uygulanan ve eski ağrı deneyimi olan çocuklar, sonraki dönemlerde hekimler tarafından yapılan en küçük işlemlerde bile şiddetli ağrı ve anksiyete hissederler. Çocuklarda ameliyat sonrası ağrı varlığı, çocuğun yanı sıra aile ve hastane personeli için de önemli bir sorundur.[6] Tonsillektomi ve adenotonsillektomi ameliyatları kısa süren küçük ameliyatlar gibi kabul edilmekle birlikte bu ameliyatlardan sonra ağrı yaygın olarak karşılaşılan bir sorundur.[7] İngiltere’de pediatrik anestezistler tonsillektomi ameliyatlarından sonra hastaların 50

%68’inde morfin ve meperidin analjezisine ihtiyaç duyduklarını belirtmişlerdir.[8] Adenotonsillektomi sonrası ağrı çoğu zaman oldukça şiddetlidir ve opioid analjezik kullanımını gerektirebilir. Opioidler her ne kadar ağrı için bir çözüm olarak görülseler de solunum depresyonu, sedasyon, bulantı ve kusma başta olmak üzere ciddi yan etkilere sahiptir. Adenotonsillektomi ameliyatlarından sonra ağrının giderilmesinde sıkça kullanılan diğer bir ajan ise nonsteroid anti-enflamatuvar (NSAİ) ajanlardır. Ancak bu ajanların kullanımı da ameliyat sonrası kanamaya neden olabilir.[9] Güvenli ve etkili bir analjezi yöntemi planlanırken, bu hastalarda ameliyat sonrası dönemde havayolu obstrüksiyonu ve solunum depresyonu gibi ciddi yan etkilerin gelişebileceği unutulmamalıdır. Adenotonsillektomi ameliyatlarından sonra yeterli analjezi sağlamak aşırı sedasyon, havayolu açıklığının korunamaması, solunum depresyonu riski taşırken, analjezinin yetersiz kalması çocukta stres, ağlama atakları ve sonuçta kanama ile birlikte olabilmekteNİSAN - APRIL 2013

dir.[10] Tonsillektomi ameliyatları sonrasında oksijen desatürasyonundan entübasyon ve ventilasyon desteği gerektiren solunum yetmezliğine kadar giden ve hastaların %30’unda gözlenen obstrüktif uyku apnesi sendromu gelişebilir.[11] Opioidlerin yan etkilerinden kaçınılması amaçlanarak yapılan çalışmalardan sonra, parasetamol cerrahi sonrası dönemde ağrı sağaltımında yaygın olarak kullanıma girmiştir. Parasetamol verilen hastaların çoğunda yeterli analjezi sağlanamadığı gösterilmiştir.[10] Parasetamol cerrahi hastalarda premedikan olarak kullanılmakta ve iyi bir NSAİ olarak kabul edilmektedir, bununla birlikte parasetamol verilen hastaların çoğu ek analjezik ihtiyacı duymaktadır. Çoğu anestezist, özellikle Kuzey Amerika’da görev yapanlar, ameliyat sonrası kanama nedeni ile parasetamol kullanımına sıcak bakmamaktadırlar. Bu gelişmelerin üzerine özellikle pediyatrik hastalarda yeni ve etkin analjezik arayışları hızlanmıştır.[10] Cerrahi sonrası ağrı sağaltımı ile ilgili olarak yapılan çalışmalarda genel anestezi ve bölgesel anestezi uygulamaları arasında ameliyat sonrası dönemdeki analjezik ihtiyacı yönünden fark olduğuna dikkat çekilmiştir. Genel anestezinin, spinal ve lokal anestezinin aksine ameliyat sahasından kalkan nosiseptif uyarıların spinal korda iletimini önlemediği belirtilmiş ve ağrının iletiminde N-metil-D-aspartat (NMDA) reseptörlerinin önemi vurgulanmıştır.[12] İnsanlardaki çeşitli çalışmalarda NMDA blokerlerinin elektriksel, termal ve diğer bazı ağrı çeşitlerinde analjezik etkisinin olduğu gösterilmiştir.[13] Cerrahi travma, spinal kordun dorsalinde yerleşmiş olan NMDA reseptörlerinin aktivasyonuna benzer şekilde santral sinir sisteminde (SSS) değişikliklere neden olmaktadır. NMDA reseptörlerinin aktivasyonu ameliyat sonrası ağrının oluşmasında önemli yer tuttuğu düşüncesinden yola çıkarak NMDA reseptör blokerlerinin ağrı sağaltımı amacı ile kullanılması planlanmış ve NMDA reseptörlerinin blokajının SSS’nin uyarılmasını engelleyeceğinden dolayı ameliyat sonrası ağrının önlenebilmesi için NMDA reseptör blokerlerinin kullanımı yaygınlaşmıştır.

Klinikte en yaygın kullanılan NMDA blokerleri ketamin ve dekstromethorfandır.[14] Ketaminin küçük dozları özellikle çocuklarda ameliyat sonrası analjezi sağlamak için kullanılmaktadır.[10] NİSAN - APRIL 2013

N-metil-D-aspartat reseptör antagonistlerinin ameliyat olacak hastalara veriliş zamanı ile ilgili çelişkiler mevcuttur. Çalışmaların bir kısmında cerrahi stimulus öncesi verilmesi gerektiği vurgulanmaktadır. Preemptif analjezi SSS’yi uyaran ve ağrıya neden olan uyarılar başlamadan önce hastaya bu uyarıların SSS’ye ulaşmasını engelleyen analjezik ajanların verilmesi veya buna yönelik girişimlerin yapılmasıdır.[15] Preemptif analjezinin amaçlarından birincisi SSS’ye ağrılı stimulusların ulaşmasını engellemek, ikincisi ise ameliyattan sonraki dönemde hastanın daha az analjezik ihtiyacı duymasını sağlamaktır.[1517] Çalışmamızda ketamini ameliyat bitimine yakın 0.5 mg.kg-1 dozda iv olarak uyguladık. Yapılan bazı çalışmalarda ise preemptif analjezik uygulaması ile analjeziğin ameliyat esnasında veya sonrasında uygulanması arasında fark olmadığı belirtilmiştir.[14,18] DA Conceição ve ark.[18] ve Butkovic ve ark.nın[14] yaptığı çalışmaların her ikisinde de benzer sonuca ulaşılmış ve insizyon öncesi ve sonrası dönemde aynı doz ketamin uygulanmış, analjezi düzeyi ve analjezik ihtiyacı açısından fark olmadığı bildirilmiştir. Çalışmamızda vermeyi planladığımız analjezik ilaçları ameliyat sonrası dönemde daha uzun süre etkili olmasını amaçlayarak ameliyat bitiminde vermeyi tercih ettik. Çalışmamızda ameliyat sonrası analjezi için kullanmayı planladığımız ilaçlardan biri ketamin idi. Ketamin NMDA antagonistidir ve hayvan çalışmalarında NMDA reseptör antagonistlerinin sistemik ve spinal olarak verilmesinin etkileri araştırılmış ve her iki yoldan cerrahi sonrası ve kronik ağrıyı önledikleri gözlenmiştir.[19,20] Ketaminin subanestezik dozlarda kullanıldığında belirgin yan etkileri olmadan etkin ameliyat sonrası analjezi sağladığı kabul edilmektedir. Önerilen subanestezik doz 0.15-0.5 mg.kg-1’dir. Bununla birlikte doz konusu tartışmalıdır ve çeşitli çalışmalarda farklı sonuçlara ulaşılmıştır. Butkovic ve ark.[14] inguinal bölge cerrahisi geçirecek pediyatrik yaş grubu hastalarda 0.3 mg.kg-1 ketamini, insizyon öncesi ve sonrasında vermişler ve her iki grupta ameliyat sonrası ağrı ve analjezik ihtiyacı değerlerinin kontrol grubu ile aynı olduğunu bildirmişlerdir.

Bu çalışmanın aksine 0.5 mg.kg-1 dozda verilen ketamin ile yapılan çalışmalarda etkin ameliyat sonrası 51

analjezi sağlandığı bildirilmiştir.[10,18,21] Bu nedenle ameliyat sonrası analjezi için önerilen subanestezik ketamin dozu 0.15-0.5 arasında olmasına rağmen çalışmamızda 0.5 mg.kg-1 gibi önerilen dozun üst sınırında bir doz seçtik.

Adenotonsillektomi ameliyatlarından sonra opioidler etkin analjezi sağlamaları nedeni ile uzun yıllar başarı ile kullanılmışlardır.[10] Fakat yan etkileri nedeni ile anesteziologlar arasında ameliyat sonrası ağrıda kullanımları soru işaretlerine neden olmuştur. Opioidlerden en sık kullanılan morfindir. Tonsil ve adenoid ameliyatları sonrasında ağrı sağaltımı sağlamak için de morfin kullanılmaktadır. Bununla birlikte yan etkileri ile ciddi sıkıntı yaratacağı görüşü yaygındır. Özellikle opioidlerin bulantı-kusmaya neden olması bu tür ameliyatlarda sıkıntıyı artırmaktadır.[21] Adenotonsillektomi ameliyatlarından sonra kan yutulması ve orofaringeal irritasyon nedeni ile bulantı ve kusma riski artmıştır.[22-24] Morfin kullanımı bu riski daha fazla artırır. Sutherland ve ark.[25] ameliyat sonrası morfin kullanımı ile tonsillektomi sonrası %71 gibi çok yüksek bir oranda kusma bildirmişlerdir.

Tramadol morfine alternatif olarak gösterilen zayıf bir opioid reseptör agonistidir. Aynı zamanda nörotransmitter reuptake inhibitörüdür. Tramadol opioid reseptörlerine zayıf afinite gösterir fakat santral monoaminerjik yolaklara direkt etkilidir. Etkisi meperidine benzer. Tramadol 1 yaş üzeri çocuklarda ameliyat sonrası dönemde ağrı sağaltımında başarı ile kullanılır, fakat yan etkileri diğer opioidlere benzer. Çalışma kapsamına aldığımız pediyatrik yaş grubu hastalar 1 yaş üzerinde olduğu için çalışma grubumuzun birinde tramadol kullandık. Çocuklarda tramadolün önerilen dozu 1-2 mg.kg-1’dir ve bu dozun günde 3 veya 4 defa ihtiyaca göre yinelenebileceği bildirilmiştir.[26] Daha yüksek dozların çocuklarda yan etki insidansında artışa neden olmadan analjezik etkinliği artırdığına dair bir kanıt yoktur.[7] Bu nedenle çalışmamızda tramadol grubunda ameliyat sonrası ağrı sağaltımı amacıyla 2 mg.kg-1 tramadol kullandık.

Tramadolün ameliyat sonrası dönemde ağrı sağaltımındaki etkinliği tartışmalıdır. Macarone ve ark.[27] adenotonsillektomi uygulanan çocuklarda yaptıkları çalışmada 2 mg.kg-1 tramadol kullanmışlar ve ame52

liyat sonrası ağrı sağaltımında etkili bulmuşlardır.

Bununla birlikte Umuroğlu ve ark.nın[21] yaptığı çalışmada 1.5 mg.kg-1 tramadolün etkin analjezi sağlamadığı gösterilmiştir, ilginç olan aynı çalışmada 0.5 mg.kg-1 dozda verilen ketamin tramadole benzer şekilde ameliyat sonrası analjezi sağaltımında morfin ile karşılaştırıldığında etkili bulunmamıştır.

Daha yüksek tramadol dozu kullanarak van den Berg ve ark.[28] adenotonsillektomi geçirecek hastalarda ameliyat sonrası ağrı sağaltımı ile ilgili bir makale yayınlamışlardır. Bu çalışmada indüksiyon sırasında 3 mg.kg-1 tramadol uygulanmış fakat 1.5 mg.kg-1 petidin ve 0.3 mg.kg-1 nalbufin ile 3 mg.kg-1 tramadolün karşılaştırıldığı bu çalışmada tramadol ameliyat sonrası ağrı sağaltımında etkili bulunmamıştır. Bu tartışmalı sonuçlara Engelhardt ve ark. nın[29] çalışması farklı bir bakış açısı getirmiştir. Bu çalışmada araştırmacılar indüksiyon döneminde tek doz verdikleri morfin ve iki farklı doz tramadolü karşılaştırmışlar ve 0.1 mg.kg-1 morfin, 1 ve 2 mg.kg-1 dozlarda verilen tramadolü ameliyat sonrası ağrı sağaltımında etkili bulmuşlardır. Bu ve benzeri çalışmalar ışığında pediyatrik yaş grubu hastalarda ameliyat sonrası ağrı sağaltımında ketamin ve tramadolün etkinliğinin tartışmalı olduğu söylenebilir. Çalışmamızda 0.5 mg.kg-1 ketamin ve 2 mg.kg-1 tramadolü kontrol grubu ile karşılaştırdığımızda ameliyat sonrası ağrı sağaltımında etkili bulduk.

Ameliyat sonrası dönemde ağrı sağaltımı sağlamak amacı ile kullanılan ilaçların etkinliğinin belirlenmesinde verilen ek analjezik miktarı ve ilk dozun verilmesine kadar geçen süre önemli belirteçlerdir.

[7] Çalışmamızda ameliyat sonrası analjezi için herhangi bir analjezik verilmeyen kontrol grubunda ek analjezik ihtiyacı duyulan hasta sayısı tramadol ve ketamin uygulanan hastalardan belirgin olarak daha fazla idi. Tramadol ve ketamin uygulanan gruplar birbiri ile karşılaştırıldığında ek analjezik verilen hasta sayısı ve verilen ek analjezik miktarı benzer olarak saptandı.

Tramadol kullanımı yan etkiler yönünden araştırıldığında diğer opioidlerle benzer yan etkilere sahip olduğu gösterilmiştir. Bu yan etkiler kullanılan doza bağlıdır, yüksek dozlarda kullanıldığında yan etki sıklığında artış olmaktadır.[30] Opioid kullanımı sırasında en sık rastlanan yan etkiler bulantı ve kusmadır. bulantı kusma insidansı bildirilmiştir.[31] Tramadol kullanımı ile birlikte aynı diğer opioidlerde olduğu gibi yüksek oranda bulantı kusma insidansı bildirilmekle birlikte,[32] tramadol kullanımı sırasında bulantı kusma görülme sıklığının morfin kullanımına göre çok daha az olduğunu bildiren çalışmalar da mevcuttur.[29] Yayınlar arasındaki bu fark kullanılan dozlar arası farkla açıklanmıştır. Aynı zamanda verilme zamanı ile bulantı kusma sıklığı arasında doğrudan ilişki olduğu ve tramadol intraoperatif dönemde uygulandığında bulantı kusma sıklığının ameliyat sonrası uygulamaya göre daha az olduğu görülmüştür.[33] Morfin, ketamin ve tramadolün ameliyat sonrası ağrı sağaltımındaki rolünü karşılaştıran bir çalışmada ketamin grubunda bulantı kusma insidansının morfin ve tramadol grubuna göre daha fazla olduğu gösterilmiştir.[21] Çalışmamızda bulantı ve kusma görülme sıklığı ketamin ve tramadol grubunda benzer ve kontrol grubuna göre anlamlı olarak daha yüksek bulunmuştur.

Ameliyat sonrası ağrı sağaltımında opioidlerin ve ketaminin kullanımı sırasında uzamış sedasyon ve solunum depresyonu ile karşılaşılabilmektedir. Bu yan etkilerin görülme sıklığı diğer yan etkilerde olduğu gibi kullanılan dozla birlikte artmaktadır. Çalışmamızda grupların hiçbirinde uzamış sedasyon ve solunum depresyonu ile karşılaşılmamıştır.

Bu durum kullanılan dozların yüksek olmamasına bağlanmıştır.

Ketamin kullanımı ile birlikte görülen diğer yan etkiler çift görme, kabuslar, sayıklama, halüsinasyonlar, sekresyon artışıdır.[34] Çalışmamızda bu yan etkilere ketamin verilen grupta oldukça nadir olarak rastlandı. Ketamin verilen grupta sadece bir hastada sayıklama ve halüsinasyona rastlanmış olup tedaviye gerek kalmadı. Bir hastada çift görme şikayeti gözlendi, 40 dakika kadar devam ettikten sonra düzeldi. Sekresyon artışı görülen 2 hastada beraberinde öksürük ve hastalardan birinde ise bronkospazm gözlendi. Bu hastalardan ilkinde bulgular hafif seyretti ve ameliyat sonrası 2. saatte kendiliğinden düzeldi. Diğer hastada ise steroit uygulanmasına ihtiyaç duyuldu. Ketamin kullanımı sırasında tanımlanan yan etkilerin erişkinlerde çocuklardan daha sık görüldüğü önceki çalışmalarda kanıtlanmıştır.[10,34,35] NİSAN - APRIL 2013

Sonuç olarak tonsillektomi veya adenotonsillektomi uygulanan pediyatrik yaş grubu hastalarda 2 mg.kg-1 tramadol ve 0.5 mg.kg-1 ketamin uygulaması ile etkin ameliyat sonrası ağrı sağaltımı sağlanmıştır. Bununla birlikte ketamin uygulanan grupta yan etki görülme sıklığı daha fazla olduğu için tramadolün pediyatrik yaş grubunda daha güvenli bir alternatif olabileceği kanısındayız.

Summary

Objectives: This prospective study aims to compare maternal and neonatal effects of spinal and general anesthesia for elective cesarean section.

Methods: Term parturients receiving routine spinal (Group SA, n=95) or general (Group GA, n=93) anesthesia and standard postoperative analgesia for elective cesarean section were included in this study. Operation time, incision-hysterotomy (TS-H) and hysterotomy-umbilical cord clamping (TH-U) intervals, oxytocine requirement, intraoperative fluids, ephedrine requirement, incidence of hypotension, time to first analgesic requirement (Tanalg), pethidine consumption, adverse events, time to first breastfeeding, oral food intake (TOI), flatulence (TF), defecation (TD), mobilization, and postoperative hospital stay were compared between the groups. Newborn Apgar scores, umbilical venous blood gas analysis, incidence of hypoglycemia, nutritional support, phototherapy and ventilatory support were also analyzed.

Results: Spinal anesthesia was associated with longer TS-H and TH-U durations, lower oxytocine requirements, higher incidence of hypotension, increased ephedrine and fluid consumption, and delayed Tanalg. Furthermore, TOI, TF, TD and postoperative hospital stay was shorter in patients given spinal anesthesia when compared with patients given general anesthesia (48h vs. 52 h, respectively; p<0.01). No difference in postoperative analgesic consumption and neonatal outcomes, except 1st min Apgar scores and umbilical blood gas analysis, was detected.

Conclusion: Spinal anesthesia, when compared to general anesthesia shortens postoperative hospital stay with early return of gastrointestinal functions in elective cesarean section.

Amaç: Bu prospektif çalışmada elektif sezaryen ameliyatlarında spinal ve genel anestezinin anne ve yenidoğan üzerine etkilerinin kıyaslanması amaçlanmıştır.

Gereç ve Yöntem: Elektif sezaryen ameliyatı için rutin spinal (Grup SA, n=95) veya genel anestezi (Grup GA, n=93) ile standart postoperatif analjezi uygulanan miyadında gebeler çalışmaya alınmıştır. Ameliyat süresi, cilt insizyonu-histerotomi (TS-H) ve histerotomi-umbilikal kordona klemp konması arası geçen süre (TH-U), oksitosin gereksinimi, intraoperatif sıvı, efedrin gereksinimi, hipotansiyon gelişen hasta sayısı, ilk analjezik gereksinimine dek geçen süre (Tanalg), petidin tüketimi, yan etkiler, ilk emzirme, annenin oral gıda alımı (TOI), gaz çıkarma (TF), defekasyon (TD), mobilizasyon ve hastanede kalış süresi gruplar arasında kıyaslanmıştır.

Yenidoğanın Apgar skorları, umblikal venöz kan gazı, hastanede kaldığı süre boyunca hipoglisemi gelişimi, ek gıda, fototerapi ve solunum desteği gereksinimi karşılaştırılmıştır.

Bulgular: Spinal anestezide genel anesteziye oranla daha uzun TS-H, TH-U süreleri gözlenmiş, oksitosin gereksinimi azalmış, hipotansiyon insidansı, efedrin ve sıvı gereksiniminde artış saptanmış, Tanalg süresi uzamıştır. Ayrıca spinal anestezi sonrası TOI, TF, TD ve hastanede kalış süresinin genel anesteziye oranla (sırasıyla 48 ve 52 saat, p<0.01) kısaldığı saptanmıştır. Postoperatif analjezik tüketimi ve 1. dak Apgar skoru ile umbilikal kan gazı sonuçları dışında neonatal veriler açısından gruplar arasında fark bulunmamıştır.

Sonuç: Elektif sezaryen ameliyatlarında spinal anestezi genel anesteziye oranla daha hızlı gastrointestinal derlenmeyi sağlamakta, hastanede kalış süresini kısaltmaktadır.

Introduction Neuraxial anesthesia is the preferred method in cesarean section as general anesthesia is associated with airway related adverse outcome, aspiration risk, intraoperative awareness and increased uterine atony leading to higher blood loss.[1] General anesthesia is performed in cases of contraindication to neuraxial anesthesia, failure of neuraxial technique or patient request for elective cesarean section.

[2] The favorable effects of neuraxial anesthesia on newborns has been demonstrated previously,[3,4] yet there is limited evidence on the effect of anesthetic techniques for maternal outcomes such as length of postoperative hospital stay and return of gastrointestinal functions.[5] This prospective study aims to compare the effects of spinal and general anesthesia on mother and neonate with length of hospital stay as primary outcome.

Materials and Methods Following approval by the Institutional Clinical Research Ethics Committee and patients’ informed consents, parturients undergoing elective cesarean section were included in this prospective study. Patients with gestation weeks <36 weeks, body mass index (BMI) ≥35 kg/m2, ASA status ≥ III, preeclampsia, multiple pregnancy, Rhesus immunization, fetal compromise or anomaly and patients in need of emergency operation have not been enrolled.

Following preoperative anesthetic evaluation, patients were divided into two groups: spinal anesthesia (Group SA) and general anesthesia (Group GA).

Noninvasive blood pressure, ECG and SpO2 were monitored and data were recorded prior to anesthesia induction and thereafter at 3 min intervals. All patients received 500 mL of lactated Ringer solution.

Spinal anesthesia was performed in sitting position at L3-4 or L4-5 interspinous levels with 25G spinal needle (Quincke tip, Braun). Fentanyl 20 µg combined with hyperbaric bupivacaine 8-10 mg were injected intrathecally to achieve a sensorial block at T4 level. General anesthesia was induced after preoxygenation with thiopental 5-7 mg/kg, succinylcholine 1 mg/kg. Following orotracheal intubation, patients were ventilated to achieve an ETCO2 of 32-35 mmHg. Anesthesia was maintained with 1.5% sevoflurane in oxygen. After delivery, fentanyl 56

2 µg/kg, midazolam 0.03 mg/kg and rocuronium 0.15 mg/kg were administered intravenously and sevoflurane was continued at 1% in 50% oxygen50% N2O mixture. Anesthesia was discontinued at the end of surgery and patients were extubated with reversal of muscle relaxation.

All patients were positioned supine with left lateral uterine displacement during operation. Following delivery, they received iv ampicillin/sulbactam 1 gr, ranitidine 20 mg and oxytocine 25 IU infused over 30 minutes. If uterine tone assessed by surgical palpation was inadequate, supplemental oxytocine was utilized. In case of hypotension, defined as decrease in systolic blood pressure (SAP) of ≥30% of baseline or a value of SAP <100 mmHg, rate of iv fluid infusion was increased. If hypotension persisted in the next consecutive measurement, an ephedrine bolus of 5 mg was administered. Heart rate <60 bpm was planned to be treated with atropine.

Postoperative pain was evaluated using verbal rating scale (VRS; 0=no pain...10=worst possible pain).

Analgesia was started when patients complained of a pain score ≥4 with 10-15 mg iv pethidine bolus at 7 min intervals at postoperative care unit (PACU).

Maintenance analgesia in the ward was achieved with im diclofenac 75 mg bi-daily and iv patient controlled analgesia (PCA) with pethidine. PCA pump was programmed to deliver pethidine 0.05 mg/kg/h basal infusion, 0.1 mg/kg PCA bolus with 7 min lock-out. Patients were advised preoperatively for free oral intake and breast feeding following surgery as soon as possible.

Maternal age, BMI, gestation week, operation duration, time from skin incision to hysterotomy (TS-H) and hysterotomy to umbilical cord clamping (TH-U), oxytocine consumption were documented. Intraoperative fluid, number of patients with ephedrine requirement, ephedrine consumption and number of patients with intraoperative hypotension were recorded. Time to first analgesic requirement (Tanalg), pethidine consumption at PACU, pethidine-PCA consumption at ward and VRS scores as well as side effects like postoperative nausea, vomiting (PONV), pruritus and postdural puncture headache were also documented. bilization (TM) and postoperative hospital stay were noted. Fetal gestation age, newborn height, weight, umbilical venous blood gas analysis, 1st and 5th min Apgar scores, presence of hypoglycemia, need for phototherapy, nutritional and ventilatory support were recorded.

Statistical analysis In our institution, patients can be discharged at daytime between 8:00 am - 4:00 pm. Patients ready to be discharged after 4:00 pm have to wait a maximum of 16 hours. To determine a difference of 16 hours in actual hospital stay between the groups with a standard deviation of 32 hours (alpha 0.05, beta 0.1), we calculated a sample size of 85 patients per group. For possible dropouts, we collected the data of first 100 consented patients in each group.

Data are presented as mean±SD, median [minmax] or number (percentage). Student’s t-test and Mann-Whitney U-test were used for parametric and

non-parametric quantitative data respectively. Chisquare or Fisher’s exact test was utilized for comparing distributions of categorical data. A p value <0.05 was accepted as statistically significant.

Results Patient flow through the study is shown in Figure 1. Patients’ demographics, operation data and oxytocine consumption are given in Table 1. More patients in Group GA required oxytocine supplementation (≥26 IU) than those in Group SA (52 vs. 31 respectively, p=0.001). The number of hypotensive patients, total amount of fluid given and ephedrine requirement was higher in Group SA when compared to Group GA (Table 2). Ephedrine in addition to fluids was given to two hypotensive patients in Group GA secondary to rapid oxytocine infusion because of uterine atony. The number of hypotensive episodes observed were 0[0-7] in Group SA and

Table 3 presents intraoperative course of SAP and heart rate values during the first 15 minutes as the shortest operation duration was 17 minutes. SAP during the first nine minutes, at the end of operation and PACU entry were significantly higher in Group GA compared to Group SA. Likewise heart rate values were significantly higher at 3rd minutes and at the end of the operation in Group GA. Maternal postoperative data are demonstrated at Table 4. Although Tanalg was shorter in Group GA with higher pethidine consumption at PACU, no statistical difference was noted in pethidine consumptions 58

at ward between the groups. Postoperative pain scores during the first hour were significantly higher in Group GA than Group SA and similar thereafter (Figure 2). TBF was equal in both groups. TOI, TF, TD was significantly shorter in Group SA. One patient in Group GA had paralytic ileus with first oral in-

0[0-2] in Group GA (p<0.001). Intraoperative nausea and/or vomiting were observed in 21 patients (22.1%) and intraoperative pruritus was seen in 37 patients (38.9%) in Group SA. None of the parturients had postdural puncture headache or complained of intraoperative recall postoperatively.

with the exception of three neonates (two in Group SA and one in Group GA) (p=1). The first neonate in Group SA with a pH value of 7.13 was delivered from a mother with hypotension, and the second one with a pH value of 7.16 had a TH-U interval of 197 seconds. First minute Apgar scores of both babies were nine. Neonate in Group GA with pH of 7.11 was small for gestational age with birth weight of 2370 g. There was no statistical difference between the groups in terms of neonates with hypoglycemia or requiring nutritional support. Also no differences were observed in requirements for respiratory support and phototherapy.

take five days after surgery and she left the hospital at postoperative 7th day. When length of stay was classified into two categories (<49 hours and ≥49 hours), 49 patients (55.7%) in Group SA stayed less than 49 hours, whereas only 29 patients (31.6%) in Group GA stayed less than 49 hours.

Neonatal data are given at Table 5. The number of neonates with 1st min Apgar score <7 was three vs. zero in group GA and SA respectively (p=0.12).

Discussion According to the results of this prospective study, spinal anesthesia for elective cesarean section is associated with a shorter length of postoperative hospital stay. It also enables early oral intake and recovery of gastrointestinal functions with lower oxytocine consumption, prolonged interval to first analgesic requirement. On the other hand, general anesthesia offers shorter delivery intervals with stable hemodynamics, less fluid and ephedrine requirements.

Neonatal outcome was similar between the groups except 1st min Apgar score and umbilical blood gas results.

Surgery and anesthetic technique employed has been shown to effect postoperative outcomes, specifically effecting the length of hospital stay.[6] Rapid recovery after cesarean section should not only aim for an early return to normal daily life but also for the motherâ&#x20AC;&#x2122;s bonding and nursing of the newborn.

Spinal anesthesia has been shown to be superior to general anesthesia in previous studies in terms of maternal mortality and morbidity due to lack of airway instrumentation, avoidance of regurgitation and intraoperative awareness.[7-11] However evidence on the effect of neuraxial anesthesia on hospital stay and discharge is missing.

General anesthesia due to its quick induction is preferred in obstetrics when urgent induction of surgery and delivery of the fetus is needed. This feature of general anesthesia is also observed in our study of elective cesarean section patients with shorter TS-H and TH-U intervals. One contributing factor for these short intervals in general anesthesia is the use of muscle relaxants and volatile anesthetics that can decrease abdominal muscle tone and facilitate delivery. The other is probably due to faster surgical dissection when neonatal depressive effects of general anesthetics are considered. Furthermore, when T4 sensorial level is reached in spinal anesthesia, abdominal muscle tone could still be higher than general anesthesia, as motor block routinely tested in spinal anesthesia is only for lower extremities.

Studies about the effects of different anesthesia techniques on neonatal outcome do not include detailed data like skin incision-delivery or uterine incisiondelivery times. Kamat et al. studied the effect of 60

induction-delivery and uterine-delivery on neonatal Apgar scores and found shorter induction-delivery and uterine incision-delivery intervals in general anesthesia compared to spinal anesthesia (57 vs 68 sec respectively).[12] On the other hand Tonni et al. compared the effects of general and neuraxial anesthesia on neonatal status and reported longer uterine incision-delivery times in general anesthesia than spinal anesthesia group (59 vs 45 sec respectively) without statistical difference.[13] However both authors failed to comment about these findings.

Uterine relaxing effects of volatile anesthetics is previously reported in literature.[14] General anesthetic effect on uterine tonus is dose-dependent and reversible. In our study, although sevoflurane dose is decreased immediately after delivery with N2O, midazolam and fentanyl supplementation, Group GA had increased oxytocine requirements compared to Group SA. When hemodynamic changes are compared, the finding that hypotension is more frequent in Group SA is not surprising. Conventional crystalloid preloading prior to regional anesthesia is no longer recommended due to lack of efficacy.[15,16] As the need for intravascular volume expansion starts with sympathetic blockade of spinal anesthesia, coloading is to be more beneficial to decrease vasoconstrictor requirement.[17] Parturients in this study were cohydrated with the start of spinal anesthesia as reflected in increased fluid consumption in Group SA. However, we still encountered hypotension which was treated with ephedrine boli. Hypotension can lead to maternal discomfort due to nausea, vomiting, light headedness and most important placental hypoperfusion and fetal compromise. Ephedrine can also lead to neonatal acidosis.[18] Yet, our treatment of hypotension seems to be effective both for the parturient and newborn as reflected by similar neonatal Apgar scores and umbilical blood pH values in both groups.

The time to first postoperative analgesic requirement in Group SA was longer as sensorial block duration overextends surgical operation time. This also led to decreased pethidine consumption at PACU. However after the sensorial block recovery of the single-shot spinal anesthesia, patients required simiNÄ°SAN - APRIL 2013

lar analgesic amounts as reflected by PCA-pethidine delivery. To our knowledge there is no study in the literature comparing spinal and general anesthesia in terms of first analgesic requirement time. Kessous et al., who described meperidine as a rescue in severe pain, reported higher number of meperidine requiring patients in the first 24 h following general anesthesia compared to spinal anesthesia for cesarean section.[19] One interesting result is that there was no difference between the groups in terms of breastfeeding.

This may be due to the time needed by the neonatology team to examine the newborn as well as staff inadequacies resulting in a delayed meeting of the baby with the mother. Sener et al. compared epidural with general anesthesia for cesarean section and reported similar first breast feeding time for epidural anesthesia whereas prolonged duration for first breast feeding following general anesthesia (107.4 vs 228.07 min respectively).[20] One of the most important findings of this study is the early recovery of gastrointestinal functions following spinal anesthesia. One important reason for early return of flatulence and defecation in Group SA is the sympathetic blockade. Sympathetic flow is the dominating inhibitory control for gastrointestinal system. When sympathetic flow is blocked and unopposed parasympathetic stimulation remains, motility in stomach, small bowel and proximal colon is increased.[21,22] Another reason may be late oral intake observed in Group GA. This late intake may be due to residual sedative effects of general anesthetics. There is no study comparing the effects of different anesthesia techniques for cesarean section on gastrointestinal function. However a recent meta-analysis of studies focus on early oral intake which may promote gastrointestinal recovery.[23] Ambulation, postoperative opioid consumption and PONV may further affect bowel recovery; however TM interval, pethidine consumption and PONV incidence were similar between the groups in this study.

Primary outcome of this study, length of postoperative hospital stay, was shorter in Group SA when compared to Group GA. There are only two studies from the same authors looking at the effect of NİSAN - APRIL 2013

anesthesia type on hospital stay.[24,25] Our finding are similar to Fassoulaki et al. who reported postoperative hospital stay between years 2002-2005. They showed a progressive decrease during this period of time and especially after neuraxial (combined spinal-epidural or epidural) anesthesia when compared to general anesthesia.[24] However, compared to our results (median 2 days) they reported a longer hospital stay (median 4 days) following neuraxial anesthesia, even in the final year of the study period.

This may depend on the late oral intake allowed only after the removal of urinary and epidural catheters.

It may also be attributed to the differences in institutional obstetric team policy. The large volume of obstetric patients in our institution accelerates bed turnover. According our obstetric team, uncomplicated patients following cesarean section with recovered bowel function, tolerance of oral intake, lack of micturition problems and ability to take care of the baby can be discharged.

In terms of neonatal outcome, the lower 1st min Apgar scores in Group GA can be the result of neonatal depression by general anesthesia.[1] Also when neonates with Apgar scores <7 are examined, three neonates in Group GA had corresponding TH-U intervals>180 seconds which may be responsible for this result.[26] However, the small difference in 1st min Apgar scores between the groups is clinically insignificant as the 5th minute scores are similar.

Kavak et al.[27] reported similar 1st and 5th minute Apgar scores, whereas Tonni et al.[13] and Mancuso et al.[4] observed more depressed newborns in general compared to spinal anesthesia. Neonates of mechanically ventilated mothers in Group GA had also higher PCO2 values. Pregnant patients have physiologically higher respiratory frequencies during spontaneous breathing. We ventilated our patients receiving general anesthesia to keep ETCO2 between 32-35 mmHg and they had possibly higher PCO2 levels compared to spontaneous breathing patients under spinal anesthesia. As the removal of neonatal PCO2 occurs via maternal lungs, these neonates had also slightly higher CO2 values. The lower pH values of neonates in Group GA are the result of slightly higher PCO2. Hodgson and Wauchob also reported slightly lower pH values of newborns with TH-U less than three minutes in general anesthesia.[28] The higher PO2 values of the neonates in Group GA 61

is expected due to positive pressure ventilation of the mothers with 100% O2 until delivery similar to Ochiai et al.[29] Early outcomes of neonates in terms of respiratory and nutritional support, incidence of hypoglycemia and need for phototherapy were similar between the groups. Tonni et al. found similar incidence of hypoglycemia in spinal and general anesthesia groups too.[13] Ozcakir et al. observed no differences between epidural and general anesthesia in terms of phototherapy needs.[30] The lack of other neuraxial techniques can be seen as a limitation of the study. But we aimed to compare the two routine anesthesia methods, namely general and spinal anesthesia of our daily practice. We also think that spinal anesthesia was more comparable with general anesthesia rather than a neuraxial technique with catheter, for it was applied as single shot and did not include long-acting opioids.

The use of high inspired oxygen concentration until delivery can be seen as a drawback of the study. We administered 100% oxygen to increase fetal oxygenation during this period as hysterotomy causes an interruption in oxygen delivery to the fetus. This may help to overcome a decrease of fetal oxygen reserve in unexpected prolonged hysterotomy to delivery periods. Pregnancy leads to a decrease in minimum alveolar concentration of volatile anesthetics up to 40% (60% MAC of sevoflurane ≈ 1.2%).[31,32] As N2O is omitted we used a higher inspiratory sevoflurane fraction to avoid intraoperative awareness.

The reported incidence of awareness in cesarean section is 0.1-0.3%, therefore this study is underpowered to comment about awareness. However we did not encounter recall in the postoperative period.

One other limitation is that umbilical venous instead of arterial blood gas analysis was obtained. Although umbilical venous sampling is easier, arterial samples would better reflect fetal acid-base status.

Fast-tracking in elective cesarean section is not well studied in terms of anesthetic technique as most western countries employ regional anesthesia.

[33] However this is not the case in Turkey, where a recent survey of obstetric anesthesia practice has shown relatively low ratio of regional anesthesia (36.1%) for cesarean section when compared with 62

western countries.[34] Other than unavailability of obstetric anesthetic care, patients’ choice and obstetric team preference may play a role in these low numbers. Demonstrating the beneficial effect of spinal anesthesia on hospital stay can further convince both the parturient to choose and the obstetric team to promote neuraxial anesthesia.

In conclusion spinal anesthesia when compared to general anesthesia for elective cesarean section allows faster discharge from the hospital. Fast recovery and return of the mother to the family offers social benefits as well as quick turnover of hospital beds in heavily occupied clinics.

Gereç ve Yöntem: Yirmi-kırk yaş arası toplam 262 sağlıklı gönüllü üzerinde painmatcher cihazı kullanılarak duysal algılama seviyesi ve ağrı eşik değeri ölçümleri yapıldı. Değerlendirme aşamasında olgular 4 gruba ayrıldı. Grup I kadın sağlık personeli (KS) (kadın doktor/hemşire), Grup II erkek sağlık personeli (ES) (erkek doktor), Grup III kadın temizlik personeli (KT) ve Grup IV erkek temizlik personeli (ET) olarak gruplandırıldı.

Bulgular: Duysal seviye ölçümlerindeki değerler erkek ve kadın temizlik personelinde kadın sağlık personelinden anlamlı derecede yüksek bulundu (p<0.05). Erkek sağlık personeli ve kadın temizlik personeli arasında ağrı duyusal düzeyi benzerdi (p>0.05).

Ağrı eşik değer ölçümleri erkek temizlik personelinde, sağlık personeline oranla yüksek saptandı (p<0.05). Kadın temizlik personeli ile erkek sağlık personelinin ağrı eşik değeri benzerdi (p>0.05). Her grupta bakılan ağrı eşik değeri ile duysal seviye korelasyonu KS ve ET gruplarında önemsizken, ES ve KT’de önemli idi (p<0.05).

Sonuç: Duysal seviye ve ağrı eşik değer seviyeleri cinsiyet farkından çok çalışma koşullarından etkilenmektedir. Bedenen çalışanlarda eşik değerlerin daha yüksek olduğu sonucuna varıldı.

Summary

Objectives: The aim of this study was to evaluate the sensorial/affective dimension pain levels of patients using an electrostimulation method and to investigate the effects of gender and working conditions on pain thresholds.

Methods: The sensorial dimension of pain and pain threshold levels of 262 healthy volunteers, aged between 20 and 40 years, were assessed using a Painmatcher machine. Patients were divided into four groups during the assessment period: Group I female medical staff (KS) (female doctors/nurses), Group II male medical staff (ES) (male doctors), Group III female sanitation workers (KT), and Group IV male sanitation workers (ET).

Results: The sensorial dimension of pain was significantly higher in male and female sanitation workers than among female medical staff (p<0.05). Sensorial dimension of pain were similar between male medical staff and female sanitation workers (p>0.05). Pain threshold levels were significantly higher in male sanitation workers than male medical staff (p<0.05). Female sanitation workers and female medical staff had similar pain threshold levels (p>0.05). The correlation between pain threshold levels and sensorial dimension of pain was not significant in Groups KS and ET, however this correlation was significant in groups ES and KT (p<0.05).

Conclusion: The sensorial dimension of pain and pain threshold levels are more strongly associated with working conditions than gender.

The threshold level of pain was high in individuals with physically demanding jobs.

Sağlıklı gönüllülerde cinsiyet ve çalışma şartlarının ağrı eşik değeri üzerine etkisi

Giriş Ağrı, vücudun belli bir bölgesinden kaynaklanan, doku harabiyetine bağlı olan veya olmayan, kişinin geçmişteki deneyimleri ile ilgili, hoş olmayan emosyonel bir duyumdur; davranış şeklidir.[1] Ağrı, her zaman subjektiftir ve kişinin sosyal yaşamının da etkin olduğu ağrı eşiğine parelel ifade edilir. Ağrının ölçümü ve değerlendirmesinde; görsel veya sözel skalalar, basınç testleri, hipo veya hiperaljezik kuteneal değerlendirmelere olanak sağlayan uyarıcevap grafikleri gibi yöntemler kullanılmaktadır.[2] Son zamanlarda geliştirilen ve elektrostimülasyon uyarısının algılanmasını rakamsal değerlere çevirerek ağrı eşiği ölçümü yapan cihazlar çalışmalarda yer almaktadır.[3] Ağrı eşiği, ağrı algısı oluşturan minumum stimulus değeri olarak tanımlanmaktadır. Ağrı eşiğini kişinin geçmişteki deneyimleri, sosyokültürel düzeyi ve cinsiyeti etkileyebilir. Ağrı iletim yolaklarındaki santral ve periferik disfonksiyonlar, enflamatuvar değişimler, inici inhibitör sistemin kaybı ve santral sensitizasyon gibi nedenler ağrı eşik değerini düşürürken; nonsteroid antienflamatuvar ilaç ve opioid analjezik kullanımı, elektrostimülasyon, sıcak ve soğuk uygulama gibi lokal fizik tedavi yöntemleri ise ağrı eşiğini geçici olarak yükseltebilir. Ağrılı stimulus için, ısı, basınç, elektrostimülasyon ve kas iskemisi oluşturulması yöntemleri kullanılabilir. Literatürde, elektrostimülasyon yöntemi ile olguların duysal ve ağrı algılama seviyelerini değerlendirmek ve sayısal değerlerle ifade etmek için geliştirilen “painmatcher” cihazı çeşitli çalışmalarda kullanılmıştır.[3-5] Painmatcher cihazı, verilen elektrostimülasyonun olgu tarafından ilk algılandığı değer olan “duysal algılama seviyesi” ile ağrı oluşturduğu düzeyin sayısal değeri olan “ağrı eşik değerinin” izlenmesine olanak vermektedir.

Bu çalışmada painmatcher cihazı kullanılarak sağlıklı gönüllülerde duysal algılama seviyesi ve ağrı eşiğine cinsiyet ve çalışma şartlarının etkisini araştırmayı amaçladık.

onamları alınarak çalışmaya dahil edildi. Olguların çalışmaya alınmasında esas kriter tam sağlıklı olmaları oldu. Herhangi bir hastalık nedeniyle kronik ağrılı olgular, son bir hafta içinde analjezik kullanım öyküsü olanlar ile sol el işaret ve başparmağında ölçümü etkileyecek lokal değişimi olan olgular çalışmaya dahil edilmedi.

Olguların duysal seviye ve ağrı eşik değerleri Painmatcher® cihazı ile değerlendirildi. PainMatcher cihazı; bir elin işaret ve başparmağı arasında sıkıştırılarak aktive edilen bir çift elektrot, sabit akım üreten bir devre, parmak basısıyla aktivasyonu devam ettiği sürece kademeli olarak artan elektriksel stimulus ve bunu kontrol eden bir mikro işlem biriminden oluşmuştur (Şekil 1). 10 Hz frekans, 15 mA amplitüd ve dikdörtgen şeklindeki monofazik elektriksel stimulusun dalga genişliği sıfırdan başlayarak 4 mikrosaniye artışlarla toplam 99 adımda maksimum 396 mikrosaniye düzeyine ulaşabilmektedir. Cihaz üç basamaklı (duysal seviye, ağrı eşiği ve deneyim sahibi olduğu en şiddetli ağrı düzeyi) ölçüm yapabilmekte ve elektrotlar üzerindeki parmaklar gevşetildiğinde her basamaktaki ölçüm skoru otomatik olarak kayıt edilmektedir. Kayıt edilmiş skorlar cihaz üzerinde tek tek kontrol edilebilmekte veya toplu olarak dokümante edilebilmektedir. Yöntemin standardizasyonu için, ölçüm yönteminin olgulara anlatılması ve ölçümün yapılması aynı kişi tarafından gerçekleştirildi. Tüm olguların eşik değer ölçümleri, yapılacak işlemin ayrıntılı anlatımını takiben oturur pozisyonda, sol el parmakları kullanılarak ve tekrarlayan ölçümlere izin verilmeden yapıldı. Painmatcher cihazı üzerinde bulunan elektrotlar sol elin baş parmak ve işaret parmağı arasında sıkıştırılarak aktive edildi (Şekil 1). Olgunun akımı ilk hissettiği anda elektrotları gevşeterek oluşturduğu rakamsal değer kişinin “duysal algılama seviyesi”, ikinci adımda ağrı duyduğu anda elektrotları gevşetmesi ile elde edilen değer ise “ağrı eşiği” olarak

kaydedildi. Ölçüm verileri hem formlara işlenerek hem de cihaz hafızasına alınarak kayıt altına alındı.

Ayrıca olguların yaş, cinsiyet ve çalışma şartları kaydedildi. Değerlendirme aşamasında olgular cinsiyet ve çalışma şartlarına göre 4 gruba ayrıldı. Grup I kadın sağlık personeli (KS) (bayan doktor/hemşire), Grup II erkek sağlık personeli (ES) (erkek doktor), Grup III kadın temizlik personeli (KT) ve Grup IV erkek temizlik personeli (ET) olarak gruplandırıldı.

takiben intereksiyonun önemli olması durumunda ikili karşılaştırmalarda variyans heterojenliği nedeniyle Tamhane çoklu karşılaştırma testi kullanıldı.

Tanımlayıcı istatistikler ortalama±standart sapma (SS) olarak ifade edildi. Duysal algılama seviyesi ile ağrı eşik değerleri arasında ilişkinin belirlenmesi amacı ile Pearson korelasyon katsayısı kullanıldı. İstatistiksel hesaplamalarında SPSS versiyon 13.01 paket programı kullanıldı.

İstatistiksel değerlendirme Çalışmanın örneklem büyüklüğü, %99 Güven aralığı, %95 power, 2.1 birim etki büyüklüğü, 3.9 birimlik standart sapma değerleri için hesaplandığında her cinsiyetten 126 hastanın yeterli olduğu belirlendi. Duyusal algılama seviyesi, ağrı eşiği, yaş değerleri verileri öncelikle Normal dağılışa uygunluk için Shapiro-Wilk testi ile kontrol edildi. Ağrı eşik değeri Normallik varsayımını karşılamadığı halde büyük örnek durumunda geçerli olan merkezi limit teoremi nedeniyle tüm sürekli verilerin istatistik analizinde değerlendirmeler parametrik yöntemlerle yapıldı. Cinsiyet ve meslek faktörleri esas alınarak iki yönlü variyans analizi uygulandı

Bulgular Gruplardaki olgu sayıları KS’de 74, ES’de 61, KT’de 64, ET’de 63 idi. Grupların yaş ortalaması sırasıyla KS’de 34.23±5.76, KT’de 30.25±7.07, ES’de 31.36±5.26, ET’de 32.17±6.80 idi ve aralarında anlamlı farklılık yoktur (p>0.05).

Grupların duysal seviyesi ortalamaları sırasıyla KS’de 3.95±1.16, ES 4.39±1.20, KT 4.73±2.0, ET 5.04±1.51 idi. Erkek sağlıkçıların duyu seviyeleri kadın temizlik personelinin değerlerine yakınken, kadın sağlıkçıların duyu seviyeleri gruplar arasında en düşük bulundu. Grup KS ile Grup KT ve ET’ye göre fark anlamlı idi (p<0.05) (Şekil 2).

Ağrı eşik değerleri sırasıyla Grup KS’de 8.39±5.09, Grup ES 7.74±4.43, Grup KT 9.17±5.18, Grup ET 11.49±7.68 idi. Erkek temizlik personeli ağrı eşiği sağlık personelinin ağrı eşik ortalamalarından önemli derecede farklı iken kadın temizlikçilerin ağrı eşik değerleri sağlık personeli ile benzerdir (p>0.05). ET ile KS ve ES arasındaki fark anlamlı idi (p<0.05) (Şekil 3). Her grup içerisinde duysal seviye ve ağrı eşik değeri arasındaki ilişkinin saptanması amacıyla Pearson korelasyon katsayısı hesaplandı. Korelasyon katsayıları sırasıyla Grup KS’de r2=0.139, Grup ET’de r2=0.240 anlamlı değilken (p>0.05), Grup KT’de r2=0.395, Grup ES’de r2=0.467 anlamlı bulundu (p<0.05). Ancak bu anlamlılık gruplardaki olgu sayılarının fazlalığından kaynaklanmaktadır.

Belirtme katsayıları (r2) incelendiğinde en büyük %21 olmaktadır. Bu da ağrı eşik değerindeki değişkenliğin ancak %21 kadarının duyusal seviye ile izah edilebileceğini, bunun da çok düşük izah yüzdesi olduğu açıktır.

Tartışma Ağrı algılanması ve ifade edilmesi bireyler arası hatta aynı bireyde farklı zamanlarda değişen şekillerde tanımlanabilir. Subjektif olan ağrı duyusu, ölçülmesi zor ve hatta imkansız olarak vurgulanmaktadır. Klinik çalışmalarda, ağrının değerlendirilmesi, sıklıkla VAS (Visual Analogue Scale) ve NRS (Numeric Rating Scales) ile yapılmaktadır.

Alstergren ve Förström[5] çalışmalarında 40 gönüllü ve 28 akut oral ağrısı olan olguda elektrostimülasyon yöntemini (Painmatcher) kullanarak ağrı değerlendirmişlerdir. Elektrostimülasyon ile yapılan ağrı değerlendirmesinin, VAS skorlaması ile yorumladıklarında iki yöntemin sonuçlarının korelasyon gösterdiğini saptamışlardır. Käll ve ark.[6] ağrı eşiğinin değerlendirilmesi için yaptıkları çalışmada VAS ve painmatcher arasında korelasyon olduğunu rapor etmişlerdir. Bizim çalışmamızda sağlıklı gönüllüler üzerinde elektrostimülasyon yöntemi (Painmatcher) kullanılarak duysal algılama ve ağrı eşik düzeyleri saptanarak gruplar arası karşılaştırma yapıldı.

Sjölinger ve ark.nın[7] yaptığı çalışmada erkek öğrencilerin sensoryal seviye ve ağrı eşiğinin kız öğrencilerden yüksek olduğunu bildirmişlerdir. Käll ve ark.

lirgin olarak daha yüksek olduğu sonucu ile cinsiyet farkının ağrı eşik değerine etkisini bildirmişlerdir.

[6] Bizim çalışmamızda cinsiyet farkı değerlendirildiğinde, meslek gruplarını standardize ettiğimizde cinsiyet arasında fark olmadığı, fakat kadın sağlık çalışanları ile erkek temizlik personeli arasında anlamlı eşik değer farkı olduğu gözlendi. Bu farkın nedeni, cinsler arası fark gibi görünse de aslında meslek farkından kaynaklanan bir yansımanın sonucu olduğunu düşünmekteyiz.

Lundeberg ve ark.,[3] kronik nosiseptif veya nörojenik ağrılı 69 olguda ağrı değerlendirdiklerinde; painmatcherin VAS ve NRS kadar güvenli ve duyarlı olduğunu rapor etmişlerdir. Lund ve ark.[4] elektrokutanos stimülasyon üreten cihaz olarak tanımladıkları painmatcher cihazı ile sağlıklı gönüllülerde ve hastalarda duyu seviyesi ve ağrı eşiği ölçümlerini karşılaştırmışlardır. Sonuçta, sensoryal (duyu) seviyeyi sağlıklı gönüllüler ve hastalarda benzer (%94, %92), ağrı eşik değerlerini belirgin farklı (%49, %78) olarak bildirmişlerdir. Sensoryal seviye hastalarda gönüllülerden önemsiz derecede yüksek iken, ağrı eşik değeri belirgin olarak daha düşük saptanmıştır.

Dawson ve List[8] Ortadoğulular ve İsveçlilerin ağrı algılama ve eşik değerlerini karşılaştırmak için yaptıkları çalışmaya 16 kadın ve 16 erkek olmak üzere 32’şer olguyu dahil etmişlerdir. Painmatcher ile elektrik uyarısı kullanılarak yapılan ölçümlerde, kültürler arası fark olmadığı ancak cinsiyetler arası belirgin fark olduğunu, ayrıca İsveçlilerde cinsler arası ağrı eşik değeri farkının Ortadoğuluların cinsler arası farkından daha belirgin olduğu bildirmişlerdir. Çalışmamızın genel değerlendirmesinde; duysal seviye ölçümlerindeki değerler erkek ve kadın temizlik personelinde anlamlı derecede yüksek bulundu.

Ağrı eşik değer ölçümleri; erkek temizlik personelinde, diğer gruplara oranla, istatistiksel olarak anlamlı düzeyde yüksek saptandı. Grup içi analizlerde, ağrı eşik değerinin duysal seviye ile korelasyon gösterdiği gözlendi.

Sonuç olarak, duysal seviye ve ağrı eşik değer seviyelerinin cinsiyet farkından çok çalışma koşullarından etkilendiği ve bedenen çalışanlarda eşik değerlerin daha yüksek olduğu sonucuna vardık.

Summary

Objectives: The aim of this investigation is to examine the causes, clinical picture, treatment, and prognosis of spontaneous intracranial hypotension, a rare cause of orthostatic headache, among the cases presenting in our clinic.

Methods: Thirteen cases (5 males and 8 females), diagnosed with spontaneous intracranial hypotension in our clinic between January 1st, 2009 and October 30th, 2011, were included in this study. The presenting symptoms, treatment, findings on cranial magnetic resonance imaging, cerebrospinal fluid pressure measured at lumbar puncture (in available patients), and the healing period of the patients were recorded.

Results: Five patients with orthostatic headache and accompanying symptoms were treated with bed rest, increase in oral fluid intake, intravenous hydration and caffeine, and experienced a complete recovery. Complete recovery was observed in two patients (15.3%) within 10 days, in another two (15.3%) within 15 days and in one patient (7.6%) within 21 days. Headache and other clinical symptoms significantly regressed within 30 days in four patients (37.6%) who received similar treatment, but a mild headache persisted intermittently during follow-up in these individuals. As the headache had not resolved after 30 days, an epidural blood patch was applied in these four cases (37.6%) and the clinical picture completely improved within 10 to 15 days.

Conclusion: Spontaneous intracranial hypotension should primarily be suspected in cases complaining about postural headache and contrast-enhanced cranial imaging should be performed. The presence of cranial nerve paralysis and pyramidal tract signs should b considered. Conservative treatments should be considered initially, however if conservative treatments fail, epidural blood patches must be applied.

Özet

Amaç: Ortostatik başağrısının nadir bir deneni olan spontan intrakraniyal hipotansiyonun nedenleri, kliniği, tedavi ve prognozunu kliniğimizde izlenen olgular eşliğinde gözden geçirmek.

Gereç ve Yöntem: Ocak 2009-30 Ekim 2011 tarihleri arasında kliniğimizde tanı alan 5 erkek ve 8 kadın olmak üzere 13 tane spontan intrakraniyal hipotansiyon olgusu çalışmaya dahil edildi. Hastalar geliş semtomları, uygulanan tedavi, kraniyal MRG bulguları, uygun hastalarda yapınla BOS basınç değerleri ve klinik iyileşme süreleri değerlendirilerek incelendi.

Bulgular: Ortostatik baş ağrısının ve eşlik eden bulguların yatak istirahati, oral sıvı alımının artırılması, intravenöz hidrasyon ve kafein tedavisi uygulanan iki hastada (%15.3) 10 günde iki hastada (%15.3) 15 günde bir hastada (%7.6) 21 günde tamamen düzeldiği görüldü. Benzer tedavi uygulanan dört (%37.6) olguda başağrısı ve diğer klinik özelliklerinin 30 gün içerisinde belirgin olarak gerilediği ancak periyodik takiplerinde hafif şiddette baş ağrılarının aralıklı olarak devam ettiği gözlendi. 30 gün sonrasında ortostatik baş ağrısı gerilemeyen olgulara uygulanan epidural kan yaması sonrasında 10-15 gün içerisinde kliniklerinin tamamen düzeldiği görüldü.

Sonuç: Postural baş ağrısı tarifleyen olgularda öncelikle spontan intrakraniyal hipotansiyondan (SIH) mutlaka şüphe edilmeli ve kontrastlı kraniyal görüntüleme yapılmalıdır. Kraniyal sinir paralizileri ve piramidal traktus bulguları olabileceği unutulmamalıdır.

Epidural kan yamaları konservatif tedavinin yetersiz kaldığı olgulara uygulanmalıdır.

Introduction Spontaneous intracranial hypotension (SIH) is a rare condition with an annual incidence of 5/100000.

Female to male ratio is 2/1, with a peak incidence in the fourth decade of life. SIH syndrome is caused by the leakage of cerebrospinal fluid (CSF) within the spinal epidural space, resulting in low CSF volume.[1] It is defined as a CSF opening pressure lower than 60 mm H2O,[2] however the pathogenesis of CSF leakage remains unclear. Some authors reported that CSF leakage could be demonstrated on T2-weighted magnetic resonance (MR) myelography sequences.[3,4] Although clinical signs of SIH may exhibit variations, postural headache is the most characteristic feature. The diagnosis of idiopathic intracranial hypotension is made based on the criteria of International Classification of Headache Disorders (ICDH-2). According to the recent revision of ICDH-2, SIH is defined as a headache that begins or worsens within 15 minutes of moving to an upright position and relieves within 15 to 30 minutes of moving to supine position.[5] Diagnosis of SIH has been revolutionized by the advance in MRI techniques. Cranial MRI findings include pachymeningeal thickening and contrast uptake, subdural fluid collection, obstruction of venous outflow, dural thickening, pituitary hyperemia, and inferior displacement of the cerebellar tonsils and brain stem.[6,7] The aim of the present study is to present the presenting symptoms, clinical characteristics, etiology, treatment and prognosis of 13 patients (5 males and 8 females), who were diagnosed with intracranial hypotension and followed up in our clinic between 1 January 2009 and 30 October 2011.

Materials and Methods Demographic characteristics, presenting complaints, neurological examination findings, imaging findings and prognosis of 13 patients, who were diagnosed with intracranial hypotension according to clinical findings, findings on MRI and lumbar puncture (LP) and were followed up in our clinic for two and a half years between January 1st, 2009 and October 30th 2011, were retrospectively reviewed. In order to identify the etiology of intra70

cranial hypotension, blood biochemistry, complete blood count, erythrocyte sedimentation rate, prothrombin time, and Activated Partial Thromboplastin Time (aPTT) of the patients were obtained. Serological tests included rheumatoid factor, Protein C and S, antithrombin III, VDRL, ANA and TSH. CSF analysis was performed on 7 suitable patients, who did not develop intracranial hypotension after spinal anesthesia or LP. EEG recordings and anticardiolipin antibody values were also obtained. Imaging studies consisted of cranial CT or MRI, MR venography, and angiography. Statistical analysis was not done because of the small sample size and high probability of error.

Results Of the cases, 5 (38.4%) were male and 8 (61.53%) were female. The mean age of the patients was 40.1±19 years (range, 26-67 years); the mean age of the males was 46, whereas the mean age of the females was 36.5±16 years. The neurological symptoms and signs of the patients are presented in Table 1. Headache that worsened in the upright position and relieved in the supine position was the most common symptom observed in all patients (100%).

All the patients complained about throbbing headache that begins in the neck and spreads all over the head. Seven patients (53.8%) had a history of sudden onset headache triggered by coughing or straining. The symptoms and signs of the patients were; fever (n=1; 7.6%), tinnitus (n=7; 53.8), diplopia (n=5; 38.4%), blurred vision (n=8; 61.5%), facial anesthesia (n=6; 46.1%), iron deficiency anemia (n=7; 53.8%), and cranial nerve paralysis (n=2; 15.3%). Three patients (23%) had a history of neurological disease and four (30.7%) patients had a concomitant systemic disease. Vasculitis parameters were positive in two (15.3%) patients. Moreover, six (46.1%) patients had a history of spinal anesthesia and one patient (7.6%) had an additional neurological deficit. None of the patients had papilla edema.

Clinical characteristics of the patients, CSF pressure values taken at lumbar puncture, which was performed in available patients that did not develop SIH following spinal anesthesia, treatment methods and healing periods are summarized in Table 2. cal examinations of CSF, which were performed for differential diagnosis due to dural enhancement revealed no abnormal finding.

pendent of posture. Four patients (30.7%), who had persistent headache despite 30 days of symptomatic medical therapy, underwent epidural blood patches. Headache was completely relieved in these patients within 10 to 15 days after blood patch application. Cranial MRI of the 12th case revealed coronal T1-weighted MR image following the intravenous injection of gadolinium shows increase of diffuse pachymeningeal thickness and enhancement on both cerebral hemisfphere surfaces and tentorium. In addition, filling defects were observed in the dural venous sinuses due to thrombosis on the contrast-enhanced MR image (Figure 1a-c). Complete regression was observed on the control MRI of this patient, which was obtained one month later.

due to hyperaemia at cavernous sinuses and pituitary gland (Figure 6a, b). İn addition to other findings, contrast-enhanced sagittal T1-weighted MR image shows sagging at the upper brain stem and expansion of the dural venous sinuses (Figure 6c).

Discussion Clinical signs, typical MRI findings and treatment options of SIH have been clearly defined in the literature.[8] Decreased CSF pressure and pachymeningeal and dural thickening on brain MRI are detected in middle-aged subjects with orthostatic headache.

The Monroe-Kellie hypothesis states that, the total intracranial volume (intracranial blood, CSF and brain tissue) must be constant in an intact cranium.

leaks in the inferior cervical and superior thoracic spine. Mechanical stress, meningeal diverticula and connective tissue diseases have been reported as the potential risk factors for the development of SIH. The annual incidence of SIH has been reported to be 5/100000, being more prevalent in women in the 4th decade of life.[8] In this case series, the mean age was 40.1±19 years and there was a slight female predominance. The disease has a wide clinical spectrum. Patients may present with orthostatic headache, nausea, vomiting, neck stiffness, and cranial nerve paralysis.[11] Although the abducens nerve is the mostly frequently involved cranial nerve in SIH, optic, oculomotor and trigeminal nerve paralysis may also be seen.[12] In the present study, all patients had orthostatic headache and two patients had cra(c)

nial nerve palsies (sixth cranial nerve involvement in one patient, and seventh cranial nerve involvement in the other).

Brain MRI shows diffuse pachymeningeal gadolinium enhancement, subdural fluid collection, and downward displacement of the brain. Both supratentorial and infratentorial pachymeninges, including cerebral and cerebellar convexities, and falx and tentorium are involved.[13] Brain MRI findings were consistent with SIH in all our cases.

Spontaneous intracranial hypotension may occur after lumbar puncture or spinal anesthesia. SIH may also develop at any time the dura mater is damaged, such as after craniotomy or spinal trauma.[14] CSF opening pressure is always low in cases with SIH. Six of the present cases had a history of spinal anesthesia, whereas one had a history of lumbar puncture. As the clinical symptoms and MRI findings were consistent with SIH, lumbar puncture was not performed in these patients, and, the diagnosis was based on clinical and radiological findings.

A headache that worsens in the afternoon is not a common condition. However, orthostatic headache, which occurs due to SIH following CSF leakage, worsens in the late morning or in the afternoon.[15] This phenomenon is called as the second half of the day headache.[16] This is corroborated by the intracranial hypotension signs on the cranial MRI of the cases with second half of the day headache secondary to CSF leakage. Moreover, orthostatic characteristics and MRI changes have been found to be associated with dizziness, tinnitus, aural fullness, neck stiffness, and horizontal diplopia.[17] In addition to orthostatic headache, eight of the present cases had tinnitus, nine had aural fullness and five had horizontal diplopia, whereas all of the cases had neck stiffness or discomfort.

CVT is one of the most important complications of SIH. It is a rare complication, occurring in only 2% of the cases. According to the Monroe-Kellie hypothesis, the decrease in intracranial blood volume is compensated by the dilatation of the cerebral veins.[10] Furthermore, CSF loss reduces the CSF absorption into the cerebral venous sinuses leading to an increase in blood viscosity in the cerebral comNİSAN - APRIL 2013

partment. One of the present cases had CVT associated with a similar mechanism.

There are several options for the treatment of SIH. Bed rest is of great importance and is adequate for the treatment of most cases. Increased oral fluid intake and oral caffeine may provide symptomatic relief. Caffeine is an adenosine receptor antagonist and produces an increase in CSF production secondary to an increase in cerebral blood flow. Sedatives and intravenous hydration therapy may also be applied.

Despite unsuccessful treatment with high dose steroids in the previous years, cases that responded to oral steroid therapy have been reported in the recent years.[18] In the present study, all cases benefited to varying degrees from bed rest, increased oral fluid intake, and concurrent intravenous hydration and caffeine therapy.

Epidural blood patch, epidural saline or dextran infusion are applied in persistent cases.[19,20] Epidural blood patch, using 10-15 ml of blood, is the main therapy. It is applied directly to the CSF leakage site if the leak is detected, or to the lumbar epidural region if the leak cannot be detected.[21,22] As CSF leaks usually occur in the thoracic region, high volume blood patches should be applied in the thoracolumbar region, and the patients should be placed in the trendelenburg position for 20-30 minutes and then should be turned to prone position for 2030 minutes; this positioning allows blood to travel over many spinal segments toward the site of the leak. Continuous lumbar epidural saline infusion is another treatment method.[23] Although the method is efficient at the beginning, recurrence is common.

[24] Surgical repair may be considered if conservative therapy proves unsuccessful. In the present case series, epidural blood patch was performed in cases with headaches that did not regress within 30 days despite treatment with analgesics, bed rest and hydration. None of the patients, who underwent blood patches, developed recurrence.

Some authors state that patients with SIH should be evaluated periodically, as headaches that are usually self-limiting and show a favorable response to conservative therapies within a few weeks may sometimes become chronic or recur.[25,26] In fact, mild orthostatic headache of four patients, who received 75

medical therapy alone, continue to occur intermittently. These patients do not have any radiological signs of recurrence or severe orthostatic headache, and they remain under follow up in our clinic.

Pathophysiological mechanisms underlying epidural lumbar blood patch may be associated with an increase in spinal epidural pressure that leads to a equilibrium in epidural venous pressure. This suggests that the main reason for intracranial hypotension is not the CSF leakage but that the negative epidural pressure within the spinal canal results in aspiration of CSF on dural surface and at the origin of spinal roots. Particularly activities like standing or walking, may contribute to the maintenance of negative pressure within the spinal canal via the epidural venous drainage towards the inferior vena cava.[9,27] Symptoms of headache did not recur in any of the present patients who underwent blood patches. Dural and pachymeningeal thickening signs detected on the cranial MRI were significantly regressed.

In conclusion, SIH is a less known syndrome and may usually be misdiagnosed despite its characteristic clinical picture and typical MRI findings.

SIH should be particularly suspected in cases that complain about postural headache and contrastenhanced brain MRI should performed. Epidural blood patches should be applied when conservative therapy remains inadequate. It should be kept in mind that headaches may become chronic or recur; therefore, these patients should be invited to periodic follow-ups. Moreover, in order to prevent chronic headache syndrome and secondary complications such as CVT and subdural hemorrhage, early diagnosis and treatment of SIH is of great importance.

Amaç: Bu çalışmada, acil servise primer dismenore nedeni ile başvuran hastaların ağrılarının giderilmesinde diklofenak sodyum ve parasetamol tedavi etkinliklerinin Vizüel Analog Skala (VAS) kullanılarak karşılaştırılması amaçlandı.

Gereç ve Yöntem: Çalışmaya primer dismenore (Grup 1, n=40) tanısı konmuş olan, parasetamol (1 gr intravenöz) tedavisi verilen ve (Grup 2, n=40) primer dismenore, diklofenak sodyum (75 mg intramüsküler) tedavisi alan kişiler alındı. Her iki grupta da 19-30 yaş arasındaki hastalar seçildi. Tüm gruplarda hastaların ağrı şiddeti VAS kullanılarak 0 (ağrı olmaması) ile 10 (dayanılmaz ağrı) arasında derecelendirildi. Grupların VAS’ ları karşılaştırıldı.

Bulgular: İki grup arasında yaş, OAB ve Nb değerleri açısından istatistiksel olarak fark yoktu. I. gruptaki bireylerin başvuru sırasındaki VAS değerleri II. gruptakilerden yüksek iken, onuncu ve otuzuncu dakikalardaki VAS değerleri II. gruptaki hastalardan daha düşüktü (p=0.00). I. ve II. grubun her birinin kendi içindeki başlangıçtaki, 10. ve 30. dakikalardaki VAS değerleri birbirlerinden anlamlı derecede farklıydı. 10. ve 30. dakikadaki VAS değerleri başlangıçtaki değerlere göre, 30. dakikadaki VAS değerleri 10. dakikaya göre anlamlı derecede düşüktü (p=0.00).

Sonuç: Çalışma sonucunda parasetamolün primer dismenore ağrısı tedavisinde diklofenak sodyum dan daha hızlı etki gösterdiği ve daha etkili olduğu söylenebilir.

Summary

Objectives: The aim of this study was to compare the efficiency of diclofenac sodium to paracetamol using a visual analog scale in the patients presenting to the emergency room with primary dysmenorrhea.

Methods: Group I (n=40) patients were diagnosed with primary dysmenorrhea and treated with paracetamol (1 gr intravenous) and Group II (n=40) patients were diagnosed with primary dysmenorrhea and treated with diclofenac sodium (75 mg intramuscular).

In both groups, patients were 19-30 years old. In all groups, the intensity of the pain was ranked from 0 (no pain) to 10 (intolerable) using VAS. The VAS scores were compared between treatment groups.

Results: Between two groups, there was no statistically significant difference in age, mean arterial pressure and pulse values. The initial VAS values of the first group were higher than that of 2nd group. Following treatment, in the 10th and 30th minutes, the VAS values were lower in Group I than Group II (p=0.00). The VAS values of each group were significantly different from each other on the 10th and 30th minutes. VAS values at the 10th and 30th minutes were lower compared to the initial values and the values in the 30th minute were lower relative to the 10th minute (p=0.00) in both treatment groups.

Conclusion: We can suggest that paracetamol is more efficient than diclofenac sodium in the treatment of primary dysmenorrhea.

Primer dismenore ağrılı hastalarda diklofenak sodyum ve parasetamol tedavi etkinlikleri ve VAS

Giriş Adet sırasında görülen ağrılı uterus kasılmaları olan dismenore, primer ve sekonder olmak üzere başlıca iki ana kategoride incelenir. Primer dismenorede organik bir patoloji tespit edilememesine rağmen, sekonder dismenorede pelvik patolojilere rastlanır.

Primer dismenore genç kadınlar arasında en sık görülen jinekolojik problemlerin başında gelir. Alt karın bölgesinde kramp tarzında ağrılar şeklinde kendini gösterir. Sıklıkla kot altından göbeğe veya karın alt kadranlarına doğru yayılabilen ağrılara, baş ağrısı, ateş, bulantı ve kusma eşlik edebilir. Ağrının şiddetine göre bazı insanlarda ruhsal yapıyı ve sosyal hayatı olumsuz etkileyebilir.[1,2] Nonsteroid antienflamatuvar ilaçlar (NSAİİ) primer dismenorenin tedavisinde kullanılan başlıca ilaçlar iken ağrının şiddetine ve direncine göre oral kontraseptifler, akupunktur ve laparaskopik cerrahi girişimler gibi alternatif tedaviler de kullanılmaktadır.[3] Parasetamol (Asetaminofen-Acetaminophen) dünyada en çok kullanılan analjeziklerden biridir. 1.000 mg kadar tedavi dozunda hem akut hem kronik durumlarda hafif ağrıdan orta ağrıya kadar çoğu ağrıda reçete edilen bir analjeziktir.[4,5] Parasetamol terapotik dozlarda hem opioidlerden hemde NSAİİ’den daha düşük yan etkiye (nefrotoksite, hepatotoksite, vs) sahiptir. Çoğunlukla oral ya da rektal yolla kullanılmasına rağmen son zamanlarda Avrupa ülkelerinde intravenöz yolla kullanım sıklığı giderek artmaktadır.[6,7] Bu çalışmada, acil servise primer dismenore nedeni ile başvuran hastaların ağrılarının giderilmesinde diklofenak sodyum ve parasetamol tedavi etkinliklerinin Vizüel Analog Skala (VAS) kullanılarak karşılaştırılması amaçlandı.

Gereç ve Yöntem Bu çalışma yerel etik kurul izni (2011- BADK -100) alındıktan sonra geriye dönük olarak acil servise başvuran primer dismenoreli hasta dosyaları taranarak gerçekleştirildi. Çalışmaya primer dismenore tanısı konmuş olup parasetamol (1 gr intravenöz) tedavisi (Grup 1, n=40) ve diklofenak sodyum (75 mg intramusküler) tedavisi alan (Grup 2, n=40), kişiler alındı.

Her iki grupta da 19-30 yaş arasındaki hastalar seçildi.

organik bir patoloji olmaması ile teyit edildi. Son 24 saat içinde NSAİİ ilaçları kullandığını ifade eden hastalar çalışma dışı bırakıldı. Hastaların başlangıçtaki, onuncu ve otuzuncu dakikalardaki VAS skorları, ortalama arter basıncı (OAB) ve nabız (Nb) değerleri ölçüldü. Tüm gruplarda hastaların ağrı şiddeti VAS kullanılarak, 0 (ağrı olmaması) ile 10 (dayanılmaz ağrı) arasında derecelendirildi. VAS günlük pratikte ağrı değerlendirmesi için kullanılan oldukça yaygın bir skala olup bu değerlendirmede 0 ile 10 arasında değişen ağrı ortalamaları verilmektedir. Buna göre “0” ağrının olmadığını belirtirken ortalama VAS değerinin 1-4 olması hafif ağrı, 5-6 olması orta şiddette ağrı, 7-10 olması şiddetli ağrıyı ifade etmektedir.

[8,9] Tedavi almadan önceki ve tedavi sonrası 10. dk, 30. dk VAS’a bakıldı. Sonuçlar “SPSS for Windows 15.0” programı kullanılarak değerlendirildi. İstatistiksel analiz Verilerin normal dağılıma uygunluğu KolmogorovSmirnov testi ile analiz edildi. İki grup arasındaki verilerin karşılaştırılmasında Independent Samples T-test ve Mann-Whitney U-testleri kullanıldı. Aynı grup içindeki tekrarlayan ölçümler Friedman iki yönlü varyans analizi ve Wilcoxon T-testi ile karşılaştırıldı. P<0.05 olan değerler istatistiksel açıdan anlamlı olarak kabul edildi.

Bulgular Olguların klinik özellikleri ve başlangıçtaki, 10. ve 30. dakikalardaki VAS skorları, OAB ve Nb değerleri Tablo 1’de gösterildi. İki grup arasında yaş, OAB ve Nb değerleri açısından istatistiksel olarak fark yoktu. Grup I’deki bireylerin başvuru sırasındaki VAS değerleri Grup II’dekilerden yüksek iken, onuncu ve otuzuncu dakikalardaki VAS değerleri Grup II’deki hastalardan daha düşüktü (p=0.00) (Şekil 1). Grup I ve II’nin her birinin kendi içindeki başlangıçtaki, 10. ve 30. dakikalardaki VAS değerleri birbirlerinden anlamlı derecede farklıydı. Her iki grupta 10. ve 30. dakikadaki VAS değerleri başlangıçtaki değerlere göre, 30. dakikadaki VAS değerleri 10. dakikaya göre anlamlı derecede düşüktü (p=0.00).

rafından yapılan tanımlamaya göre ağrı “vücudun herhangi bir yerinden kaynaklanan, gerçek ya da olası bir doku hasarı ile birlikte bulunan, insanın geçmişteki deneyimleriyle ilgili, duysal, afektif ve hoş olmayan bir duyu” olarak tanımlanmaktadır.

[10,11] Ağrı visseral ya da somatik alt gruplara ayrılan nosiseptif ağrı ya da perferik sinir sisteminde hasar ya da patolojik değişiklikler sonucunda anormal nöronal aktivite ile oluşan nöropatik ağrı şeklinde görülen olmak üzere alt gruplara ayrılmaktadır. hekimleri, kadın doğum uzmanları, acil tıp hekimlerinin pratiğinde oldukça sık karşılaşılan durumları oluşturmaktadır.

Bunlar içerisinde bulunan primer dismenore, prevelansı bazı bölgelerde %90’a kadar çıkabilen ve kadınlar arasında hayat kalitesini düşüren, okul ve iş hayatını olumsuz etkileyen en yaygın sebeplerden bir tanesi olarak bilinmektedir. Gerçekten de Unsal ve ark.nın[12] 623 kız öğrenci üzerinde yaptıkları bir araştırmada dismenorenin oldukça yaygın bir sağlık problemi olduğu ve üniversite öğrencileri arasında sağlıkla ilişkili hayat kalitesini negatif etkilediği belirtilmiştir.

Patogenez ve mekanizması günümüzde nispeten oldukça iyi ortaya konulmaya başlayan ağrının şiddetinin kesin ölçümünü göstermek sağlık mensuplarının en sık yüz yüze kaldığı zorluklardan birisidir.

[13] Ağrının ortaya konulmasında basit tanımlayıcı ölçek, görsel kıyaslama ölçeği ve sayısal ölçekler gibi subjektif olarak tanımlanan ve ağrıyı objektif hale getirerek etkili ağrı kontrolünü sağlamaya yönelik olarak uygulanan yöntemler bulunmaktadır. Bunlar içerisinde VAS sayısal olarak ölçülemeyen bazı değerleri sayısal hale çevirmek için kullanılır.[14] VAS olarak bilinen ölçeğin çeşitli ağrıların derecesini ortaya koymada kullanıldığı pek çok çalışmada gösterilmiştir. Bununla ilişkili olarak Lmoy’nin toplam 1387 dismenoreli kadın hastada yaptığı çalışmada hem VAS hem de sayısal ağrı skalasının dismenore ağrısının tespitinde kullanışlı bir yöntem olduğu NİSAN - APRIL 2013

ortaya konulmuştur.[13] Toplam 200 olgunun incelendiği diğer bir ileriye dönük randomize çalışmada ise dismenoreli hastalardaki ağrı şiddeti VAS’a göre 5.78±1.45 şiddetinde saptanmıştır.[15] Çalışmamızda ise primer dismenoreli hastalardaki başlangıç VAS değeri Grup I’de 8.6±1.3, Grup II’de 7.6±1.6 olarak tespit edildi. Bu değerler Jones ve ark.nın[8] değerlendirmesine göre şiddetli ağrıyı (710) ifade etmektedir.

Yapılan çalışmalarda primer dismenorenin nedenleri arasında artmış endometrial prostaglandin üretiminin de olduğu belirtilmektedir.[16] Bu maddelerin sekretuvar endometriyumda proliferatif endometriumdan daha fazla olduğu primer dismenoresi olan kadınlarda prostanoid sentezinin artmış olmasının uterusta yüksek amplitüdlü kontraksiyonalara neden olarak uterus tonusunu artırıp dismenoreye neden olduğu belirtilmektedir. Dismenorede COX enzim ve prostanoid sentez aktivitesinin yüksek olduğu ve bu durumdan dolayı tedavide COX enzim inhibitörü olan NSAİİ’nin kullanımının yaygınlığını artırmaktadır.

Bizim çalışmamızda parasetamol verilen gruptaki bireylerin başlangıç VAS değerleri diklofenak sodyum verilen gruptakilerden yüksek olmasına rağmen, tedaviye başladıktan sonra 10. ve 30. dakikalardaki VAS değerleri diklofenak sodyum verilenlerden daha düşük olarak bulundu.

Dismenore menstural siklusun bitmesine yakın progesteronun geri çekilmesinin sonucu gelişir. Bu geri çekilme prostaglandin F2 (PGF2) ve E2 (PGE2) sentezinde artmaya neden olur. Dolayısıyla prostaglandinler miyometriyumda kontraksiyonlara ve kramplara neden olur.[17,18] Parasetamol etki mekanizması tam olarak açık olmasa da yazarlar primer etkisini prostaglandin sentezini inhibe ederek gösterdiğini, bu inhibisyonunda analjezik etkiye yol açtığını ifade etmektedirler.[19,20] Bektas ve ark.[6] renal kolikli hastalarda parasetamol ile morfin grubu arasında VAS değerleri açısında fark olmadığını tespit etmişler ve renal kolik tedavisinde parasetamol tedavisinin güvenli bir şekilde kullanılabileceği kanısına varmışlardır.

dikloran alan gruba göre VAS değerlerinin daha hızlı ve etkili olarak düştüğü görüldü.

Günümüzde dismenore tedavisi NSAİİ odaklı gitmektedir.[21] Fakat bu ilaçların nefrotoksisite, hepatoksisite, gastrointestinal yan etki açısından parasetamol grubuna göre daha yüksek insidansa sahip olduğu bilinmektedir.[22] Dolayısıyla bizim çalışmamızda olduğu gibi yapılacak diğer çalışmalarda da daha az yan etkiye sahip parasetamolün etkinliği yüksek bulunursa, güvenlik marjı açısından tedavide ilk bu ilaçların kullanımı ön plana çıkacaktır.

Özet

Episakral lipoma, başlıca posteriyor iliyak krest bölgede oluşan küçük, hassas subkutan nodüllerdir. Akut ve kronik bel ağrısının önemli ve tedavi edilebilir bir nedeni olduğu gösterilmiştir. Episakral lipoma torakodorsal fasyadaki bir yırtık ve bunu takiben altta uzanan dorsal yağ tabakasının bir kısmının yırtık aracılığı ile fıtıklaşması sonucu oluşur. Yaygın bir klinik durumdur ve tanınması kolaydır. Ağrılı nodül varlığı ve anestezik solusyonla enjeksiyon sonrası ağrının kaybolması diyagnostiktir. İlaçla tedavi ve fizik tedavi faydalı olmayabilir. Episakral lipoma tedavisinde lokal anestezik (LA) ve steroit solüsyonu ile nodülün enjeksiyonu etkilidir. Burada posteriyor iliyak krest üzerinde ağrılı nodülü olan iki hasta tanımladık. Birinci hasta sol alt ekstremite yayılan şiddetli bel ağrısından yakınıyordu ve takiben disk ameliyatı geçirmişti. Diğeri ise büyük trokanterik ağrı sendromu olarak tedavi edilmişti. Her iki hastada görülen semptomlar LA ve steroit enjeksiyonu ile hafifledi. Episakral lipoma akut ve kronik bel ağrısının tanı ve ayırıcı tanısında daha sık akla getirilmelidir.

Summary

Episacral lipoma is a small, tender subcutaneous nodule primarily occurring over the posterior iliac crest. Episacral lipoma is a significant and treatable cause of acute and chronic low back pain. Episacral lipoma occurs as a result of tears in the thoracodorsal fascia and subsequent herniation of a portion of the underlying dorsal fat pad through the tear. This clinical entity is common, and recognition is simple. The presence of a painful nodule with disappearance of pain after injection with anaesthetic, is diagnostic. Medication and physical therapy may not be effective. Local injection of the nodule with a solution of anaesthetic and steroid is effective in treating the episacral lipoma. Here we describe 2 patients with painful nodules over the posterior iliac crest. One patient complained of severe lower back pain radiating to the left lower extremity and this patient subsequently underwent disc operation. The other patient had been treated for greater trochanteric pain syndrome. In both patients, symptoms appeared to be relieved by local injection of anaesthetic and steroid.

Episacral lipoma should be considered during diagnostic workup and in differential diagnosis of acute and chronic low back pain.

Giriş Bel ağrısına yol açan patofizyolojik mekanizmaların anlaşılmasına yönelik çalışmalarda sırtın fasiyal tabakasını oluşturan konnektif dokuya çok az dikkat edilmiş, torakodorsal fasyadaki yırtık, enflamasyon, fibrozis, yağ infiltrasyonu ve fıtıklaşması gibi patolojiler sadece sporadik olgu sunumları şeklinde kalmıştır.[1]

Episakral lipoma, torakodorsal fasyadaki bir yırtık sonucu fıtıklaşmış subfasiyal yağ dokusudur. Çeşitli çalışmalar ile episakral lipomanın akut ve kronik bel ağrısının önemli nedenlerinden biri olduğu ve ciddi bel ağrısına ve sakatlanmaya yol açabileceği gösterilmiştir.[2-4] Episakral lipomaya bağlı ağrının kalça ve bacağa re-

fere olması tipiktir. Bu bölgeler aynı zamanda lumbosakral strain, lomber spondiloz ve disk hastalıklarına ait refere ağrı bölgeleridir. Bu nedenle episakral lipomanın diğer bel ağrısı nedenleri ile birlikte bulunabileceği mutlaka akılda tutulmalıdır. Hafif disk lezyonu bulunan kişilerde episakral lipomanın varlığı önemli derecede ağrıya yol açabilir. Bu hastalara şiddetli ağrı nedeni ile gereksiz disk cerrahisi uygulandığı ve cerrahiden sonra ağrı şikayetinin geçmediği rapor edilmiştir.[4] Ayrıca büyük trokanterik ağrı sendromu (BTAS), abdominal ağrı gibi farklı refere ağrı paternlerine yol açan ve tanıda karışıklığa neden olan episakral lipoma olguları da bildirilmiştir.[5] Bu yazıda, episakral lipoma tanısında gecikilen iki farklı olgu ele alınarak bel ağrısı tanısında sıklıkla gözden kaçan fakat kolayca tedavi edilebilir bir patoloji olan episakral lipoma tanısına dikkat çekilmek istenmiştir.

Olgu Sunumu Olgu 1– Elli iki yaşında kadın hasta bel sol bacak ağrısı şikayeti ile kliniğimize yatırıldı. Öyküsünden hastanın 3 yıl önce aynı şikayetlerle lomber disk lezyonu tanısı ile ameliyat edildiği, ameliyattan sonra ağrısının kısmen azalmakla birlikte tam olarak geçmediği, bunun üzerine 10 ay önce yeniden ameliyat edildiği ancak ağrısının halen devam etmekte olduğu öğrenildi. Hastanın muayenesinde lomber bölgede ameliyat skarı mevcut, bel fleksiyonu eklem hareket açıklığı sonunda kısıtlı ve ağrılı, diğer bel hareketleri açık ve ağrılı idi. Düz bacak germe testi (DBKT) negatif, nörolojik muayene doğal idi. (a)

Lomber bölgenin dikkatli palpasyonla yapılan muayenesinde sol posterior iliyak krest yakınında yaklaşık 2 cm çapında, lastik kıvamında, kısmen hareketli, basınca karşı aşırı derecede hassas bir nodül tespit edildi. Nodülün palpasyonu ile lokal ağrı ve bacağa refere olan ağrı şiddetleniyor idi. Hastanın ağrısı Görsel Analog Skala (GAS) (0-10) ile 8 olarak tespit edildi. Bel ağrısına bağlı disabilite Roland Morris Disabilite Sorgulama Formu (0-24) ile 21 olarak hesaplandı. Hastanın lomber manyetik rezonans görüntülemesinde L5 vertebrada laminektomi defekti ve ameliyata bağlı granülasyon dokusu tespit edildi.

Hastaya bu muayene bulguları ile episakral lipoma ön tanısı ile nodül bölgesine 1 cc lokal anestezik (LA) enjekte edildi. Enjeksiyonu takiben ağrının belirgin ölçüde hafiflemesi ile episakral lipoma tanısı doğrulandı. Hastaya öncelikle yüzeyel ve derin sıcak, masaj ve analjezik akım uygulandı. Ağrının devam etmesi üzerine nodül içine lokal enjeksiyon uygulanılmasına karar verildi. Nodül 2 parmakla stabilize edilerek yeşil uçlu (21G çap 38mm) iğne ile nodül içine girilerek nodülün penetrasyonu sağlandı (Şekil 1a, b). İğne cilt altında tutulacak şekilde hafifçe geri çekilerek 4-10 farklı yerinden ardı sıra delinerek 2 cc prilokain, 20 mg metilprednizolon karışımı nodül içine infiltre edildi. İğnenin geri çekilmesinden sonra 30 sn basınç uygulandı. Enjeksiyonu izleyen 1. gün hastanın ağrısı GAS ile 2 olarak değerlendirildi. 2 idi ve Roland Morris Disabilite skorunda düzelme mevcuttu.

Olgu 2– Altmış yaşında kadın hasta şiddetli sol kalça ve lateral uyluk ağrısı şikayeti ile polikliniğimize baş vurdu. Hastanın 3-4 yıldır devam eden hafif şiddetteki kronik bel ağrısına iki ay önce sol büyük trokanter ve lateral uyluk ağrısı eklenmişti. Muayenede bel hareketleri açık hafif ağrılı, sol büyük trokanter palpasyonla aşırı derecede hassas, kalçanın dirence karşı abdüksiyonu ağrılı ve Patrick’in fabere testi pozitif idi. DBKT negatif, nörolojik muayene doğaldı.

Laboratuvar bulgularında patoloji yoktu. Hasta bu muayene bulguları ile Ege Rasmussen tarafından tanımlanan ölçütlere göre büyük trokanterik ağrı sendromu olarak değerlendirildi.[6] Hastanın büyük trokanter bölgesine LA ve steroit karışımı ile lokal enjeksiyon uygulandı. Enjeksiyonu takiben şikayetleri kısmen rahatlayan hasta bir ay sonra şikayetlerinin artması üzerine tekrar polikliniğimize başvurdu.

Hastanın muayenesinde büyük trokanterdeki hassasiyete ek olarak lomber bölgede derin palpasyonla 1-2 cm çapında ağrılı bir nodül tespit edildi. Nodül üzerine uygulanan basınç lokal ağrıya yol açarken büyük trokanter ve lateral uyluk bölgesindeki ağrıyı da şiddetlendiriyordu. Hastanın ağrısı GAS ile 7 olarak değerlendirildi. Hastaya episakral lipoma tanısı ile daha önce anlatılan teknikle intranodüler enjeksiyon uygulandı. Enjeksiyonu takiben hastanın ağrısı belirgin ölçüde azaldı. Birinci ay yapılan kontrolde ağrının GAS değeri 2 idi.

Tartışma Episakral lipoma, lumbosakral bölgede lokalize olan, derin subkutanöz yerleşimli, lastik kıvamında, sıklıkla mobil, yuvarlak veya oval şekilli yağ nodülleridir.[2-4] Bu nodüllerin orijinal tanımlaması ilk kez 1937’de Ries tarafından episakroiliak lipoma şeklinde yapılmıştır. Copeman ve Ackerman 1944’de bu nodüllere ilk kez kesin anatomik açıklama getirerek lumbar fasiyal yağ fıtıklaşması terimini kullanmışlardır.[7] Klinik pratikte Copeman nodülleri olarak isimlendirilen bu nodüller torakodorsal fasyadaki bir yırtık sonucu hernie olmuş subfasiyal yağ dokusudur ve akut ve kronik bel ağrısının önemli nedenlerinden biridir. Episakral lipoma genel popülasyonda yaygındır ve sıklıkla semptomsuzdur. Singewald NİSAN - APRIL 2013

bu nodüllerin toplumun %16’sında mevcut olduğunu ve bu bireylerin de sadece %10’unun semptomlu olduğunu ifade etmiştir.[8] Yağ fıtıklaşması sıklıkla bizim olgularımızda olduğu gibi posteriyor iliyak krestin hemen üzerinde sakrospinal kasın kenarı boyunca, sakroiliyak bölgede ise spina iliaka posterior superiorun yakınında oluşur.

Bu bölgelerde travmaya bağlı anormal gerilim sonucu veya fasyanın kalıtımsal zayıflığı ve kutanöz sinirlere ait nörovasküler foraminalar aracılığı ile subfasial yağ pedleri yüzeyel ve derin fasiya arasına fıtıklaşır ve sıkışarak inflame hale gelir.[4] Bizim her iki olgumuzda da ciddi bir travma öyküsü yok idi. Yağ lobülünün distansiyonu yeterli seviyeye ulaşırsa yağın kanlanması bozulabilir, ödem ve bazen hemoraji gelişir. Bu olaylar semptomları başlatır. Episakral lipomada ağrı mekanizması tümüyle anlaşılmış değildir.

Fıtıklaşan yağ lobülleri sapının strangülasyonu bel ağrısının nedeni olabilir. Yağ fıtıklaşmasının esnek olmayan sert fibröz kapsül içinde genişlemesi fibröz kapsülün gerilmesine yol açarak ağrı oluşturabilir.

Curtis’e göre ağrının nedeni intranodüler basıncın artmasıdır.[2,3,5,9] Episakral lipoma her iki cinste de görülebilmekle birlikte kadınlarda özellikle orta derecede obez kadınlarda yaklaşık üç misli daha sık görülür.[3,9] Bizim iki olgumuz da kadındı ve vücut kütle indeksleri >25 idi.

Episakral lipoma tek veya çok sayıda, genellikle 1-3 cm boyutlarındadır. Çoğunlukla tek taraflı bazen iki taraflı yerleşimli olabilir.[3,9] Bizim her iki olgumuzda da tek taraflı yerleşimli tek nodül mevcut idi.

Episakral lipomalı hastalar genellikle şiddetli ve bir tarafta daha belirgin olan bel ağrısı şikayeti ile hekime başvururlar. Sıklıkla kalça ve bacağa yayılan ağrı da mevcuttur. Episakral lipomanın benzer şikayetlere yol açan lomber spondiloz, disk lezyonu gibi diğer bel ağrısı nedenleri ile birlikte bulunabileceği akılda tutulmalıdır. Bu hastalara şiddetli ağrı nedeni ile bizim birinci olgumuzda olduğu gibi gereksiz disk cerrahisi uygulanabilir. Episakral lipomadan kaynaklanan refere ağrı sklerotomal yayılım gösterir. neden olan episakral lipoma olguları da bildirilmiştir.[5] Bizim ikinci olgumuzda da ana yakınma büyük trokanterik ağrı sendromunu düşündürdüğünden tanıda gecikilmiştir. Sakroiliyak bölgede oluşan episakral lipomaların üst 3 sakral sinirin dorsal ramilerinin oluşturduğu middle kluneal siniri sıkıştırarak pudental nöropatiye benzer bacak ve ayak ağrısını içeren şikayetlere de yol açabileceği bilinmelidir.[10] Episakral lipomalar, yerleşimleri benzer olsa da Travell tarafından tanımlanan tetik noktalardan ayrılmalıdır. Episakral lipomada kastaki gergin bant yerine sınırları belirli bir nodül söz konusudur.[4] Benzer anatomik lokalizasyonlarda görülen lipomlar (daha yüzeyel, palpasyonla daha yumuşak) ve sebase kistler (daha yüzeyel ve sirküler) ayırıcı tanıda göz önünde bulundurulmalıdır.[5] Ayrıca superiyor kluneal sinirin medial dalının orta hattın 7-8 cm lateralinde torakolomber fasya ve iliyak krest arasındaki fibroossöz tünelden geçerken basıya uğrayıp episakral lipomaya benzer şikayetlere yol açabileceği akılda tutulmalıdır.[11-14] Episakral lipoma tedavisi sıcak uygulama, masaj, elektrik stimülasyonu, maniplasyon, kuru iğneleme, LA steroit enjeksiyonları gibi uygulamaları içerir.[24,9] Bu tedavilerin yetersiz olduğu şiddetli olgularda yağ nodülünün cerrahi eksizyonu ve fasya tamirine başvurulur. Bizim olgularımızda çoklu delme tekniğiyle LA steroit karışımı ile intranodüler enjeksiyon uygulamaları etkili olmuştur, hastalarımızın uzun dönem takipleri halen devam etmektedir. Çoklu delme tekniği ile ağrıda hızlı azalma sağlanması ağrının intranodüler basınçtan kaynaklandığını doğrulayabilir. LA’nın yararlı etkisi substans P gibi irritan kimyasalların bölgeden temizlenmesine, lokal vazodilatasyon ile metabolitlerin uzaklaştırılmasına ve nöral feedback mekanizmanın kesilmesine bağlanabilir. Steroidin de analjezik ve antienflamatuvar etkileri enjeksiyon tedavisinin etkinliğini artırır.[5] Episakral lipoma oldukça yaygın bir problemdir.

Sıklıkla disk lezyonuna benzer semptomlara yol açar.

Hafif disk lezyonu bulunan kişilerde episakral lipomanın mevcudiyeti ağrıyı önemli derecede artırarak gereksiz cerrahiye neden olabilir. Ayrıca alışılmadık bölgelerde refere ağrıya yol açması tanıda karışıklık ve gecikmeyle sonuçlanabilir. Episakral lipoma akut ve kronik bel ağrısının ayırıcı tanısında mutlaka göz önünde bulundurulmalıdır.

Summary

Eagle syndrome is an aggregate of symptoms caused by an elongated styloid process, most frequently resulting in headache, facial pain, dysphagia and sensation of foreign body in throat. The proper diagnosis is not difficult with clinical history, physical examination and radiographic assessment if there is a sufficient degree of suspicion. The treatment is very effective. We report here a typical case of Eagle syndrome which was misdiagnosed as trigeminal neuralgia for many years and was treated with carbamazepine. We aim to point the place of Eagle syndrome in the differential diagnosis of facial pain. We also reemphasize the usefulness of the three-dimensional computed tomography in the diagnosis of Eagle syndrome. Even though Eagle syndrome is a rare condition, in cases of facial pain refractory to treatment or unexplained complaints of the head and neck region, it should be considered in the differential diagnosis as it has therapeutic consequences.

Özet

Eagle sendromu, elonge stiloid çıkıntının neden olduğu belirtiler topluluğudur. Eagle sendromunda başağrısı, yüz ağrısı, disfaji ve boğazda yabancı cisim varlığı hissi sık görülür. Öykü, fizik muayene ve görüntüleme bulgularıyla kolayca tanı konulabilir. Cerrahi tedavi etkindir. Olgu yüz ağrısı şikayeti nedeniyle trigeminal nevralji tanısı almış ve uzun yıllardır karbamazepin kullanmaktadır.

Düz kafa grafisi ve boyun bölgesinin üç boyutlu bilgisayarlı tomografisi tipik Eagle sendromu bulgularını göstermektedir. Bu olgu ile yüz ağrısı ayırıcı tanısında Eagle sendromunun yerine dikkat çekilmiş ve Eagle sendromu tanısında üç boyutlu bilgisayarlı tomografinin kullanımı üzerinde durulmuştur. Eagle sendromu, nadir bir durum olarak düşünülse de, özellikle cerrahi tedaviye iyi yanıtı nedeniyle baş ve boyun bölgesiyle ilgili açıklanamayan şikayetleri olan, tıbbi tedaviye dirençli hastalarda akla gelmelidir.

Introduction In 1937, the association of cervicofacial pain and elongated styloid process with calcification or ossification of the stylohyoid ligament was described by Eagle in a pair of case reports.[1] Eagle syndrome is characterized by facial and neck pain but may also manifest itself in a variety of symptoms such as dysphagia, tinnitus, otalgia, syncope and focal cerebral symptoms. The etiology and the pathophysiology of the syndrome are poorly understood. The clinical di-

agnosis is difficult and radiologic confirmation is required. The surgical treatment is highly effective.[1-5] Eagle syndrome is a rare and unusual cause of head and neck pain. It can be confused with oropharyngeal and maxillofacial disorders and particularly with cranial neuralgias but with appropriate clinical history and radiography, Eagle syndrome can easily be identified and treated.

Case Report The patient was a 70-year-old man with a 27-year history of constant right facial pain radiating into the anterior auricular area. Mouth opening and the anteflexion of the head precipitated episodes of more severe pain during several seconds to minutes. The pain spread also to the left side of the face for the last 6-7 months. He was diagnosed with trigeminal neuralgia at the beginning of the disease and was treated unsuccessfully with carbamazepine since then. His history was uneventful for any significant craniofacial trauma or surgery. Medical history was noncontributory. Physical examination revealed an exacerbation of the pain by palpation of the tonsillar fossa bilaterally. Neurological examination as well as brain and cervical magnetic resonance imaging and angiography were unremarkable. Radiographic examination demonstrated styloid processes extending down into the area of the tonsillar fossa bilaterally (Fig. 1a). Computed tomography (CT) of the neck with three-dimensional reconstruction showed elongation of the styloid processes that were 5.1 cm long on both sides (Fig. 1b-d). The findings were compatible with Eagle syndrome and the patient underwent surgical removal of the elongated styloid process.

Discussion The styloid process and the stylohyoid ligament are derived from Reichert’s cartilage, which arises from the second pharyngeal arch. Reichert’s cartilage is divided into tympano-hyal, stylo-hyal, cerato-hyal and hypo-hyal parts. The ossification of the stylo-hyal part and its fusion with the tympano-hyal part causes a long styloid process. Different theories such as post-traumatic reactive hyperplasia and metaplasia, anatomical variations, postmenopausal endocrine dysfunction and genetical transmission are proposed (a)

to explain this abnormal ossification but the exact cause of the Eagle syndrome is still not clear.[6,7] The normal length of the styloid process is estimated to be 2.5 cm by Eagle.[8] In further research, the average sizes varying from 2.6 to 4 cm are reported.

Generally, if the length of the styloid process is more than 3 cm, it is considered to be elongated.[9-10] The incidence of styloid process elongation is controversial. Eagle reported an incidence rate of 4%. Many authors reported a wide range of incidence rates, lower or much higher than Eagle’s, reaching a frequency as high as 28%. The incidence of the stylalgia is much lower than the incidence of elongated styloid process; only a small percentage (between 4% and 10.3%) of the patients with elongated styloid process are thought to actually be symptomatic. Styloid process elongation is usually bilateral, however, in most cases, the symptoms are typically unilateral and bilateral symptoms are quite rare.[11-13] Clinically two groups with different etiologies are identified. First or the classic type Eagle syndrome is typically seen in patients after tonsillectomy or minor trauma. The pain is often felt on the angle of the mandible and it may radiate to the ipsilateral ear.

The palpation of the ipsilateral tonsillar fossa may exacerbate the patient’s symptoms. The elongated styloid process can be palpated in the fossa. Frequent symptoms other than facial pain are dysphagia, tinnitus, the sensation of a foreign body in the throat and otalgia. The impingement of the cranial nerves V, VII, IX, X which pass in close proximity to the styloid process is thought to be the cause of the symptoms. The second type is attributed to impingement of the internal or external carotid artery by the styloid process deviating slightly from its normal direction. the artery may be painful because of the stimulation of the sympathetic nerve plexus accompanying the artery. Interruption of blood flow within the affected artery may cause syncope and focal cerebral symptoms including aphasia, blindness and weakness.[1-5,8] The diagnosis is based on the clinical signs, digital palpation of the styloid process in the tonsillar fossa, radiological findings and lidocaine infiltration test.

[14] The lidocaine infiltration test result is considered positive for Eagle syndrome if the patient’s symptoms subside with the administration of 1 ml 2% lidocaine to the area of the styloid process in the tonsillar fossa.

[15] Three-dimensional CT is considered to be the gold standard for the diagnosis of Eagle syndrome.

It allows the accurate measurement of the length of the styloid process, better depiction of the anatomy of the surrounding structures and visualization of the exact spatial orientation of the styloid processes.[16,17] The differential diagnosis includes migraine and other headaches, trigeminal, glossopharyngeal and other cranial neuralgias, ear nose and throat diseases such as tonsillitis and otitis, degenerative diseases causing neck pain, psychosomatic diseases and inflammatory and neoplastic processes in the oropharyngeal area.[14] Conservative treatment options are nonsteroidal anti-inflammatory medications, the injection of steroids and long-lasting anesthetics into the tonsillar fossa. Surgical excision of the elongated styloid process with transcervical or transpharyngeal approaches is often successful.[18] Our case was presented with a long history of cervicofacial pain centered on the angle of the mandible. The pain was referring to the ipsilateral ear and was exacerbating with mouth opening and sudden head movements. Hypersensitivity in the region of tonsillar fossa was observed on palpation.

Three-dimensional CT of the neck demonstrated bilateral elongated styloid processes consistent with Eagle syndrome. Although this classic form of Eagle syndrome is typically seen in patients after tonsillectomy, our patient did not present with any history of pharyngeal surgery. Bilateral elongation of the styloid processes despite unilateral symptoms is very frequent in cases of Eagle syndrome, but interestingly, in our patient’s case, symptoms were observed only on one side for many years and then occured on both sides within the last 6 to 7 months.

In our patient, the mandibular pain triggered with jaw movements led to the misdiagnosis of trigeminal neuralgia and the patient could not be treated properly for 27 years. Because of the wide range of clinical manifestations, Eagle syndrome may be confused with trigeminal neuralgia, like in our case, as well as with most disorders of the head and neck. It is a rare condition but is probably more common than generally considered and it should be kept in mind in the management of refractory cervicofacial pain.

Summary

Complex regional pain syndrome (CPRS) and Raynaud’s disease are disorders characterized by vasomotor disturbances associating with abnormal autonomic nervous system. We present a case of CRPS involving a history of recurrence and no initiating event. Raynaud’s disease accompanying CRPS was diagnosed clinically in the patient. We propose that a sympathetic dysfunction underlies the pathophysiologies of both disorders and may be responsible for the coexistence of these two distinct entities. Recurrence and unknown etiology of CRPS might account for temporary alterations in sympathetic function.

Özet

Kompleks bölgesel ağrı sendromu (KBAS) ve Raynaud hastalığı anormal otonom sinir sistemi ile ilişkili vazomotor bozuklukla kendini gösteren bir hastalıktır. Bu yazıda, herhangi bir başlatıcı olay olmadan gelişen ve tekrarlayıcı özelliği olan bir KBAS I olgusu sunuldu. Hastada ayrıca klinik olarak KBAS I’e eşlik eden Raynaud hastalığı tanısı da konuldu. Bu iki ayrı antitenin birlikteliğinden, her iki hastalığında patofizyolojisinde yer alan, altta yatan bir sempatik disfonsiyonun sorumlu olabileceği düşünülmüştür. KBAS I’in tekrarlaması ve herhangi bir etyolojik faktör olmadan ortaya çıkışı, sempatik disfonksiyonda meydana gelen geçici değişiklikler ile açıklanabilir.

Introduction Complex regional pain syndrome (CRPS) type I, formerly called reflex sympathetic dystrophy, is a clinical entity characterized by the presence of allodynia or hyperalgesia, edema, sweating changes, abnormal skin color and temperature, trophic changes of nails and hairs and often impairment of motor function in the affected extremity. The pain in CRPS is disproportionate to any inciting event. The history of an initiating noxious event such as trauma exists commonly. How-

ever, 5-10% of patients can have an obscure etiology.

[1] Raynaud’s phenomenon is an episodic vasospasm of peripheral arteries, triggered by exposure to cold and emotional stress.[2] This exaggerated vasospastic response causes the pallor of digits, followed by cyanosis and finally a hyperemia which may be lacking.[3] Autonomic nervous system disturbances contribute to mechanisms of both disorders which are represented by an altered mechanism of vasal motility.[4]

A case of recurrent complex regional pain syndrome accompanying Raynaud’s disease

We present a case of recurrent and idiopathic CRPS type I accompanying Raynaud’s disease. The coincidence of these two entities may be postulated to result from an individual disease of sympathetic nervous system which contributes to both pathophysiologies of disorders.

oratory findings, primary Raynaud’s phenomenon (Raynaud’s disease) accompanying CRPS type I was diagnosed. A three-week-rehabilitation program including range of motion and stretching exercises (30 minutes a day), contrast bath therapy (15 minutes a day) and TENS-therapy (20 minutes a day) was introduced. He responded well to physical therapy, his pain and swelling in the hand resulting from CRPS type I improved within three weeks. As the frequency of cyanosis of his fingers and toes reduced by avoiding cold, any pharmacological treatment was not considered for Raynaud’s phenomenon.

Case Report A 21-year-old male patient with pain, swelling and cyanosis of his right hand was admitted to our department. There was no history of trauma that might have triggered these complaints. He had suffered from similar complaints with the same hand one year earlier and it had improved spontaneously within three weeks. In physical examination, his right hand and fingers were edematous and cold, also had a cyanotic appearance (Figure 1a). He had allodinia and hyperlgesia in response to touch on the dorsum of his hand and fingers. The skin of his right hand was dry and the nails of his right fingers were dystrophic. These clinical findings were consistent with CPRS type I. It was also detected by physical examination that his left fingers and bilateral toes were cyanotic and cold (Figure 1b). The cyanosis of the fingers and the toes had recurred once or twice a month for a few years and had been exacerbated by cold, which were suggestive for Raynaud’s phenomenon. His plain radiograph, electromyography and doppler flowmetry, performed for differential diagnosis, were normal. His nailfold capillaroscopy and immunological tests of antinuclear antibody, erythrocyte sedimentation rate, C-reactive protein, performed to rule out possible underlying conditions, were normal. Discussion The IASP CRPS criteria include continuing pain disproportionate to any inciting event and four distinct diagnostic categories. The four categories are recognized as sensory findings including hyperesthesia, allodynia, hyperalgesia; vasomotor findings including temperature and/or skin color asymmetry; sudomotor findings including edema and/or sweating changes; motor/trophic findings including decreased range of motion, motor dysfunction (weakness, tremor, dystonia), trophic changes of hair, nail, skin. Clinical diagnostic decision rules are endorsed as the presence of signs observed by the physician on clinical examination in two or more of these categories and symptoms reported by the patient in at least three of four categories.[1] In our case, symptoms and signs in all four categories were positive. There were also informative tests and procedures that are not specific for the diagnosis of CRPS such as infrared thermography, quantitative sensory testing, sudomotor tests, bone scintipraphy and x-ray (spotty osteoporosis).[5] (b)

The pathophysiological mechanisms of CRPS are based on the posttraumatic regional inflammation, that sensitizes C-nociceptive fibers and leads to release of some vasoactive neuropeptides, and the sympathetic dysfunction.[5] Vasomotor and sudomotor changes in CRPS are the indicator of autonomic dysfunction. Deteriorated peripheral vasoconstrictor responses after sympathetic stimulation in CRPS I patients have recently been detected not only during acute and early phase, but also before the onset of the disease and even after years in a chronic state of the disease.[6,7] Accordingly it is theorized that the patients who develop CRPS I may have an underlying sympathetic nervous system abnormality which may recover to a certain extent during the course of the disease.[6] Abnormality in the sympathetic nervous system which controls thermoregulation contributes to the pathological pathways of Raynaud’s phenomenon as well.[2] In the study of Herrick et al. asymptomatic thermoregulatory abnormalities in noninvolved limbs of CRPS I patients were detected by defective response to cold challenge.[8] All these knowledge suggest the presence of similar sympathetic dysfunction in the pathophysiology of both Raynaud’s phenomenon and CRPS. In addition, sympathectomy was found beneficial in the treatment of both disorders.[4] In the light of this pathophysiology knowledges, it may be suggested that an autonomic disturbance in the patient account for the coexistence of these two entities. A likely individual defect in sympathetic nervous system may contribute to occur both primary Raynaud’s phenomenon and CRPS I in the patient who don’t have history of trauma or any initiating factor. CRPS I recurred two times apart one year in our case. Recurrence might be due to temporary alterations and relapsing in individual sympathetic dysfunction. There are two more reported cases of idiopathic and recurrent CRPS I which have also a migratory characteristic.[9,10] These were not discussed in terms of possible explanations for unidentified etiology and recurrence.

Primary goal of the treatment in CRPS is functional restoration. Specific exercise therapy is the most essential part of the treatment. Therapeutic cold and heat, TENS-therapy, contrast bath and occupa92

tional therapy are the others component of physical therapy. The pharmacological treatment includes nonsteroidal anti-inflammatory drugs, glucocorticoids, calcitonin, opioids, tricyclic antidepressants, membrane stabilizers (carbamazepin, gabapentin, lidocain, mexiletin), NMDA-receptor antagonists (ketamin), GABA-agonists (baclofen), adrenoceptor antagonists (clonidin, phentolamin) and topical capsaicin. Sympathetic ganglion block with local anesthetics and continuous conduction block of brachial or lumbar plexus form the regional anesthetic techniques. Neuromodulation with spinal stimulation and behavioral therapy are the remaining alternatives in the treatment. The physical therapy program we planned for our patient included exercise, contrast bath, TENS-therapy. The patient’s complaints relieved with this program.[5] To conclude, our case of recurrent and idiopathic CRPS I accompanying Raynaud’s disease is the first one reported in the literature. Recurrence and unknown etiology of CRPS I in our case may be explained by an underlying sympathetic nervous system abnormality which exacerbates temporarily.
