CLM has been serving the Greater Derry region for 57 years providing the most comprehensive array of mental health and substance use services of any other provider in the region Annually, our organization proudly serves more than 6,000 children, adolescents, adults and seniors Collaborating with them on their path to resiliency, recovery and greater well-being

We continue to experience soaring demand for mental health services here and across the nation as was anticipated as we came out of the pandemic This places particular pressure on our ability to meet need while balancing the real strain of workforce recruitment and retention in a highly competitive healthcare environment. We met this challenge with continual and persistent improvements with Access to Care and our ability to connect people to services in as timely a manner as possible. Additionally, we continued the use of Telehealth services as an option even as we returned to more in person care. Finally, increased Access To Care coupled with the increase in volume of services and clients served has also resulted in consistent year over year financial growth and viability

One of the most significant statewide changes to our system of care was the launch of the NH Rapid Response 24/7 mobile crisis outreach program in January 2022 We are proud to share that our leadership and Acute Care Services team embraced and adopted the Mobile Crisis Response model at the beginning of the pandemic This truly prepared us well for the statewide launch which included the addition of Peer Specialists as part of the team. Additionally, our efforts to enhance partnership with law enforcement throughout our region with a focus on safety has resulted in the strongest collaboration in our organization’s history. Outreach focused on explaining the changes in crisis service availability and the need for close collaboration given the weight on law enforcement to handle and intervene on a multitude of issues such as domestic violence, addiction, mental illness, homelessness, and other concerns

Across all programs and services, we continue with our vision of a wholeperson approach to care to prevent premature death of people who experience mental illness and substance use disorders. Perhaps the most notable example is what began several years ago with a successful partnership with Lamprey Health Care to provide primary medical care onsite at CLM. This past year we made significant strides in further developing our Integrated Care Program and the scope of services provided

Jean’sPlace,asix-bedsupportivehousingprogramforadultsinneedofsupportfortheirmental healthcare,welcomeditsfirstguestinSpringof2022.LocatedinSalem,thegoaloftheprogram istoreducetheneedforhospitalizationandpeoplebacktotheirlivesinthecommunityJean’s Placeprovidedatimelimited,supportedapartmentprogramthatpromoteswellnessandrecovery skillstoempowergueststoremaininthecommunitywiththegoalofsecuringpermanentand stablehousing.

WholePersonMentalHealthandPrimaryMedicalCare

OverthepastseveralyearsourIntegratedCareProgramhascontinuedtoexpandthescopeof servicesprovided.Formanyindividualslivingwithmentalillness,accesstoprimarymedicalcareis limitedandoften“notagoodfit”inhelpingaddresstheirmentalandphysicalhealthcareneeds. CLM’s Integrated Care Program is focused on improving health outcomes as well as life expectancyforthosewithmentalillness InpartnershipwithLampreyHealthCare,theHealthy TogetherprogramonsiteatCLMDerrygivesclientsaconvenient,one-stopplaceforwholeperson care.HealthyTogether,markeditsfourthanniversarythisyear,servingadultsandalsoexpanding to include adolescents ages 12 and older Additionally, the Integrated Care Program has incorporatedcasemanagement,communitysupportformedicalneeds,behavioralhealthvisiting nurseservices,integratedillnessmanagementandrecovery,andNEWYou(Nutrition,Exerciseand Wellness)wellnessandcoachingsupport.

OurTMSprograminoperationfor13yearsnow,hasreachedthebenchmarkofbeingoneofthe highestvolumeNeuroStartreatmentpracticesintheregion WhenfirstofferedatCLM,TMSwas aninnovative,non-medicationtherapyforthetreatmentofmajordepressioninadults.Recently, TMS treatment has been FDA-cleared for the treatment of adults with obsessive-compulsive disorderaswellasadolescentdepressionofferingnewhopeforthenearly1in5USadolescents experiencingamajordepressiveepisode

The Continuum of Collaborative Care is a specialized treatment model and person-centered approachtoworkingwithindividualswithco-occurringMentalHealthneedsandIntellectualand DevelopmentalDisabilities,AcquiredBrainInjuriesand/orAutism Themodelwasdevelopedina collaborativeeffortbytheCenterforLifeManagementandCommunityCrossroads.In2021,The model was deemed by the NH Bureau of Behavioral Health and Department of Development Disabilitiesasabest-practicetreatmentmodelthatisbeingimplementedatcommunitymental healthcentersanddevelopmentaldisabilitiesagenciesthroughouttheNHsystemofcare

ImprovedAccesstoCare

Mentalhealthandaddictionstreatmentorganizationsmustgearuptomeettheincreaseddemand forservicesknowingit’snecessarytoavoidwaitlistsandoffertimelyappointmentswhenpeople needthem.Thatdividebetweenmeetingdemandforserviceandaveryrealstrainoncapacityisa challenging one Much attention has been focused on improving operational efficiencies and redesigning our intake process for more timely access to care The Access to Care team was expanded to allow for more live calls, process flows were streamlined to identify and reduce barriers to access. All callers are assessed for their level of acuity which allows for immediate scheduling of critical care intakes and reduced wait times for regularly scheduled intake appointments We’ve seen that quick access to care reduces the rate of missed appointments, improvescontinuityandmorequicklyengagesclientsintheirtreatmentandrecovery.

WorkforceDevelopment,RecruitmentandRetention

CLM, like community mental health centers across the state, faces workforce challenges as demandforservicesstrainsasystemthatisalreadyhighlycompetitiveforstaffing.CLMhasa deeply rooted philosophy that cultivates a positive work culture and highly values our “people power” CLM’s Human Resources team led the way in implementing assertive recruitment and retentionstrategies TheseeffortshaveresultedinCLMfillingvacanciesinashorteramountof timeandminimizingturnoverthatcangreatlyimpactcontinuityofserviceprovision.

Crane Charitable Fund Donates $5,000

The Crane Charitable Fund awarded the Center for Life Management Foundation a $5,000 grant to enhance and implement Applied Behavior Analysis (ABA)and provide play-based therapy tools. that will be used to reinforce positive through ABA and improved outcomes for clients diagnosed with autism spectrum disorder.

CLM’s 16 th Annual Concert and Auction raised was a raise the roof, sold-out event held at Tupelo Music Hall in Derry. Thanks to the support of our concert goers and generous sponsors, the event raised $##,## to support those in need of mental health services who are un- or underinsured as well as to help eliminate barriers to successful mental health outcomes.

Likemanyrecentgraduates,gettingintotheworkforceinameaningfulwaycanbea challenge Aftergraduatingwitha bachelor’sdegreeindigitalforensicsfrom ChamplainCollegeinVermont,ColinCochranehitaroughpatchashetriedtofigure outwhatthenextstepwouldbeforhimHeacknowledgesthatthiswasaperiodof timethatcreatedsomefrictionbetweenhimandhisparents.Hehadsuccessfully completedhisdegree,butwasstillstrugglingtofindhisway

Someone suggested to Colin’s family he might get some needed help from services with the Center for Life Management Colin was 25 when he first started at CLM He began working with a treatment team that included a mental health clinician, Supported Employment Specialist (SES) and, for a time, a Wellness Coach. Initially diagnosed with anxiety and depression, he was also struggling with symptoms along the autism spectrum that made it harder for him to interact socially with others. Though his symptoms were not severe, they made it harder for him to function successfully without a specific and more repetitive routine

Colin started working with his Supported Employment specialist to determine what his work interests were and what type of employment where he could be productive and successful According to Colin, “This was probably the most helpful service I’ve had at CLM. I got assistance in building a resume and they worked with me to practice my interviewing skills They helped me do those things and got me connected with the local Hannaford store”

Colin interviewed and got a job working 30 hours per week at the bakery “People are nice there and I really enjoy the work environment I’m really proud that I just marked my second anniversary on the job,” says Colin. He also successfully completed the goal of getting his driver’s license. In addition to working with his SES, Colin also worked with a Wellness Coach at CLM who helped him develop a workout schedule and helped him get comfortable going to the gym “At first, I started out going once a week with my Wellness Coach, now I go every day on my own to use the equipment and run on the tread mill.”
