Forms and Materials ALL APPLICANTS: Completed transfer Common Application (commonapp.org) or Coalition Application (coalitionforcollegeaccess.org) All official college transcript(s) including the JST (Joint Services Transcript) (if applicable) Note: Mid-term grades are required on a case-by-case basis.

College report to be completed by the Registrar Office or dean of your current/previous school (available through Common Application) Academic evaluation (formerly known as letter of recommendation) to be completed by an advisor, professor, or employer Official final high school transcript or GED score report SAT/ACT scores* *Only for students who have completed less than one year of full-time college coursework (less than 24 college credits after secondary school graduation) as of the application deadline. This is not required for students who have been out of high school for more than five years. Standardized test scores are not required for students who completed high school outside of the U.S. Portfolio materials** **Portfolios are required for music technology, studio art, and are encouraged for majors within the College of Arts, Media and Design, and should be submitted through our online portfolio submission portal, SlideRoom, which can be accessed through the Common Application. Please be aware that SlideRoom charges a $10 fee.
