Treatment of cancer has progressed a long way forward in the last century. Surgery, radiotherapy and chemotherapy are the mainstay of treatments against cancer. Despite major successes in these fields, still some cancers, especially blood related cancers are difficult to treat. These will need more intense treatments like bone marrow transplants. These treatments are also used to cure other non-cancerous blood diseases like certain special types of anaemias, diseases where the bone marrow doesn’t work properly etc.
STEM CELL TRANSPLANT (or BONE MARROW TRANSPLANT or BMT)
Stem cell are mother cells of all cells in the body. During growth of a person, these stem cells slowly multiply and changes form to different types of cells of the body like nerves, heart cells, skin, nail, intestines etc. This property of stem cells is utilized in bone marrow transplantation where after BMT, a totally new set of blood cells form in the body. Joseph E. Murray and E. Donnall Thomas were awarded the Nobel Prize for medicine and physiology in 1990 for their discoveries concerning “organ and cell transplantation in the treatment of human disease”
Who benefits from BMT?
BMT currently is used predominantly for diseases of blood like blood cancers (eg. leukemias, multiple myeloma, lymphomas etc) different forms of anaemias (thalassemias, sickle cell anemias etc) , various diseases that affects the immune systems, for some solid tumors like Ewing’s sarcoma etc. For these diseases, when BMT is done, the stem cells once given to the patient will settle in the bone marrow and starts multiplying and changing into normal disease free blood cells, thus curing the disease.
What are the types of BMT?
BMT is broadly of two different types,
• Autologous (auto BMT)
• Allogeneic (allo BMT)
In auto BMT, the patients own stem cells are removed and stored and reinfused to the patient after giving very high doses of chemotherapy. Here, the treatment that cures the disease is the high dose chemotherapy which completely destroys the patients diseased bone marrow. The reinfused bone marrow is mainly intended to replenish the thus emptied marrow.
In allo BMT, like auto BMT, the patient receives high intensity chemotherapy which wipes out the diseased marrow but is replaced by stem cells from another donor.
How are donors selected for BMT
The donor for allo BMT is a crucial factor for the procedure’s success. The best persons who can donate stem cells for a patient are those who are similar with the patient for certain proteins in the blood called HLA. HLA group is another system of proteins on some blood cells, similar to the blood groups. Baruj Benacerraf, Jean Dausset and George Snell were awarded the Nobel Prize for medicine and physiology in 1980 for their works on HLA systems.
Matched sibling donor
Though there are many HLA types in humans, 5 or 6 are most important and needs to be perfect match in pairs between patient and donor So it is called a 10/10 match or a 12/12 match to be the best donor for a patient. Thus the best donor for any patient are the patient’s own siblings with at least 10/10 HLA match. The chances that two children of same parents will be match is around %25. So more the number siblings, higher is the chance that a matched sibling stem cell donor is available if needed. But with family size becoming smaller, a matched sibling is not always available for most patients. This is when alternate donor is needed.
How are unrelated donors selected?
If a sibling donor is not available then the next best option is to find another donor who is related or unrelated to the patient but had the perfect 10/10 match with the patient’s HLA system. There are international and national organisations (registries) which collect blood or saliva samples of voluntary donors and identify their HLA type and save the data electronically. Whenever a patient requests for a donor stem cell from these registries, the HLA pattern of patient is send to these registries for matching and searching. The worldwide registries contains details of around 3 crores of voluntary donors. The agencies makes a software search in these registries to see if the HLA pattern of the patient matches with any of the millions of donors.
The highest chance of getting a match is from donors of similar racial and ethnic background. So the chance of an Indian origin patient getting a match is highest from amongst Indian donors. Thus it is very important that more voluntary donors from India sign up for the registry to benefit our own patients. Once a match is established, the registry officer requests the donor for donation of stem cells.
What are the other donor sources?
Another alternate donor is called haplo or half match. The chances that a patient will be %50 similar to his parents or children (or even siblings if not %100 match) is very high. So potentially a parent or a child of patient can be a donor. Previously this was very risky since the chance of reaction between the patient and donor stem cell with only 5 out the required 10 HLA groups was very high. But newer techniques have reduced this risk very much that this is becoming almost as safe as a sibling transplant.
Are stem cell donations ethical?
Because the registries do not know details about the patients and there is no way by which the donor can come into contact with the patient, there is no issue of selling of stem cells from a donor. All stem cell donations till date are done voluntarily or from close relatives, making this process very ethical and transparent. Unlike various other transplantations allegations against BMT regarding organ selling are hardly ever heard.
How are stem cells collected?
Stem cell can be collected from various sources. Conventionally it is collected from the bone marrow. But currently most of the stem cell collections happen by a procedure very similar to blood donation, called Peripheral stem cell collection. Here, with the help of an injection, the stem cells which lives in the bone marrow are released into the veins and from there with the help of a machine, just like a blood donation, the stem cells alone are extracted out. The remainder of the blood is returned back to the donor. This is very convenient for the donor.

Attempts are continuously being done to find other sources of stem cells other than bone marrow and peripheral blood. Umbilical cord blood was found as a good source of stem cells for BMT, but is preferred more for children, since the amount of cord blood is only little. At the time of birth, the parents of the baby can decide to donate the blood left in the placenta and the umbilical cord of the baby How should we donate cord blood? Private cord blood banks or public banks?
Cord blood are currently stored in 2 different ways, as a public bank or a private bank. The public bank accepts cord blood from anyone who wishes to donate cord blood after delivery, at no cost to the donor. The donor doesn’t have rights over the cell but donates to a larger pool of stem cells. Any patient can request for search in this bank and if a match is found, they will pay for the processing, storing etc of the bank and other charges. This makes it socially more meaningful and financially more viable.
Alternately there are certain private cord blood storage bank facilities, which are slowly growing down in popularity. Here the donor donates the cord blood to the private bank and pays for its storage. Whenever the child needs the cord stem cells, they can use it. Unfortunately the chances that a child will need their own stem cell is very less and as the child grows up the amount becomes insufficient making this a very less viable system.
What are the numbers with regards to BMT?
On an average there are around 50-60,000 bone marrow transplantations happening world wide per year. Even though India has around 1/5th of the population of the world, only around 2,000 BMTs happen here per year. The main reasons why the numbers of transplant are low in India is primarily due to lack of donors and lack of availability of trained experts and equipped hospitals to perform these complex procedures. To date, there are only around 70 hospitals across the country where BMT is performed and most of them are concentrated in the Metro cities. In Kerala, very active transplant centres, where allo BMT is being done on a regular basis are hardly a handful. These facilities are currently available in Thrissur at Amala hospital.
What are the complications in BMT?
The complications of this procedure starts after transplanting of the stem cells. This is a phase when the immunity of the body is going to be really low and patient runs a high risk of encountering the most serious infections ever occurred in humans. The immunity of the patient has to be kept low in the initial months to allow the foreign/donor stem cells to grow, otherwise the patients immunity will damage the donors stem cells leading to a process called rejection. Parallel to this there is a phenomenon where by the patient’s body tries to throw away or reject the donor stem cell, which is called as Graft Versus Host Disease (GVHD). This needs to be managed with medicines that reduce the immunity, which again increases the chances of serious infections. Overall this leads to a situation with only a narrow window through which the patient has to traverse through balancing low immunity status, risk of infection and other complications and chances of GVHD. On one side there is the risk of GVHD for which immunity needs to be kept low with medications while on the other side, there is the risk of infections due to very low immunity. On a third front there is also a risk of re-occurrence of the primary disease or GVHD and a balance has to be maintained between all these. Much of the advanced facilities that is required for BMT is to absolutely safeguard the patient from even the lightest of infections, while they are going through a phase of zero immunity.
What are the expected costs and benefits of BMT?
The cost of auto transplant in India would vary from hospitals. On an average for an auto transplant, it would be somewhere between INR 3,00,000 to INR 12,00,000. In some govt. hospitals where these are done, it could be done from fully free to around INR 4,00,000. For allo BMT, the cost primarily depends on the donor source. While for sibling matched donors, it might vary from INR 15,00,000 to INR 25,00,000 depending on hospitals, for Unrelated donors from registries, it could vary anywhere from between INR 25,00,000 to INR 50,00,000. For haplo related donors the cost would be around INR 15,00,000 to INR 30,00,000.
Why are the costs for BMT so high?
The cost of BMT is for various different aspects. Primarily, it is the cost of medicines required to avoid and treat infections of a patient with very low immunity, and for medicines required to keep the immunity at a very low level. Also cost is required for the advanced facilities to make the patient stay in an environment with zero chance of infections. These include facilities like HEPA filtered room, Positive pressure ventilations one to one nursing, separate kitchen and sterilization facilities etc. Then there is the cost for chemotherapy and radiotherapy, cost of blood transfusions etc. All these things will be needed for months to years. Another major cost involved in unrelated donor transplant is for procuring the the stem cells from national or international registries. Thus, BMT is a very complex procedure but has come a long way after its discovery in the 70s. Its becoming refined and complications though still present, are better managed now. Wherever indicated, this is a lifesaving procedure, having given a new life to hundreds of thousands of people.
BOTTOMLINE:
1. There is a definite increase in the number of cancer patients in India as well as in the world. Changes in lifestyle, habits, pollution etc. might all be reasons. The best way to reduce the number of cancers is certainly prevention and early detection strategies. So all efforts should be made towards these initiatives which are much less costly and more beneficial. High end treatments like BMT are discussed and provided only tp a small number of patients who will really benefit from them.
2. Some types of treatments for cancer are expensive. The benefits of these might be sometimes minimal. Unlike many developed countries where health expenses are paid by the government or insurance companies, most Indians spend for treatment from their pockets. So, it is important that in India before we embark on expensive treatments, we should know the real benefit. Where the benefit is to the tune of high chance of curing of disease or prolonging life for a good time, patients should benefit them.
