1. Störung des Stoffwechsels, die durch vermehrte Harnsäurebildung (Urat) insbesondere in den Füßen zu Gelenkentzündungen, Ablagerung von Harnsäuresalzen (Tophi) und akuten Schmerzattacken führen kann.

Der englische Begriff für die Gicht, “gout”, kommt vom lateinischen gutta, Tropfen. Dies spiegelt die humorale Theorie wider, nach der Tröpfchen eines morbiden Stoffes aus dem Blut fallen und sich auf den Gelenken ablagern.

noun 1. a disease in which defective metabolism of uric acid causes arthritis, especially in the smaller bones of the feet, deposition of chalkstones (tophi) and episodes of acute pain.

“Gout” derives from the Latin gutta (“a drop”), reflecting the humoral theory and the notion of the ‘dropping’ of a morbid material from the blood in and around the joints”.

Jill Tegan Doherty macht es sich schwer. Es reicht ihr nicht, leidende Kranke darzustellen, wie viele Künstler vor ihr. Es reicht ihr auch nicht, die äußeren Anzeichen oder inneren Zerstörungen einer Krankheit genau darzustellen, wie es Ärzte seit der Antike getan haben. Jill macht es sich zur Aufgabe, eine Krankheit selber und den Schmerz selber in ihrem Werden und Wirken zu erfassen. Also ganz Zeitgebundenes, Fließendes, Flüchtiges, Innerliches sichtbar und verstehbar zu machen in reglos vor uns stehenden Gemälden, Radierungen und Skulpturen.

Dem Betrachter macht Jill es leicht. Sie bringt alle ausgestellten Werke unter eine verbindende Überschrift, die umgekehrt in den Facetten ihrer Bedeutung durch die verschiedenen Werke erläutert wird. Der übergreifende Schlüssel zum Verständnis der einzelnen Werke ist etwas so Konkretes und – gerade den Ärzten und Studierenden der Medizin in unserem Haus natürlich - Bekanntes wie die Krankheit Gicht.

Die Werke selber sind gegenständlicher Art, sie verknüpfen sehr eindeutige Motive zu eindringlichen Vergleichen, und jedes Werk wird zusätzlich durch einen kurzen Begleittext erklärt.

Jills Kunst ist nicht fremd, vielschichtig, tiefgründig, schwer zugänglich, wie es gerade zeitgenössische Kunst häufig ist. Trotzdem sind Jills Werke – auch abgesehen von raffiniert kunstvollen formalen Aspekten – weder belanglos und oberflächlich noch lassen sie uns kalt und gleichgültig. Warum? Weil Jills Kunst nicht Selbstzweck ist, sondern ausdrücklich einer anderen, nicht belanglosen Sache dient.

Sie will das Verständnis der Gicht, die Jill in unserer Gesellschaft durch viel Unverständnis und Missverständnisse auch hinsichtlich ihrer Entstehung verkannt sieht, erleichtern und fördern.

Jill kommt dem Betrachter ihrer Werke also sehr entgegen – und dennoch: sind ihre Werke schön?

Sind sie eine Darstellung von Schönem? Oder sind sie eine schöne Darstellung von Unschönem, die zur Beschönigung tendiert? Wie ich persönlich sie erlebe, sind Jills Werke eine einigermaßen erträgliche und zugleich doch schonungslose, packende, verstörende Darstellung von etwas sehr Unschönem. Jills Werke sind nicht zur Verschönerung der Welt da, sondern verhelfen uns zu, ja halten uns an zu einer Verbesserung unseres Verständnisses der Welt, wie sie ist und wie sie sein könnte oder sogar sollte.

Ist es nicht das, was Kunst immer will?

Ich bin dankbar, dass Jill Kunst in unser Gebäude und in den Alltag unserer Medizinerausbildung bringt.

Jill Tegan Doherty walks a difficult path. She is not satisfied with depicting suffering patients as many artists have done before her. She is also not satisfied with a mere accurate representation of the external symptoms or the destructive effects of the disease, as doctors have done since antiquity.

Jill’s approach is a visualisation of the disease and the pain itself. In other words, she uses paintings, etchings and sculptures to expose and to help us to comprehend something that happens within the organism, and something that is transient and volatile.

Jill makes it easy for the beholder. The exhibited works are connected by a mutual theme, and each one of these works forms an explanation to one of the theme’s various facets. Therefore, the key to understanding these artworks becomes something tangible and – in particular to doctors and medical students at our facility – it becomes something familiar. The works on display are figurative in character – they connect very distinct imagery with powerful symbolism. A short explanatory text accompanies each piece of art.

While contemporary art is often entirely strange and complex, profound and even standoffish, Jill’s artwork is not unapproachable. At the same time, her work – despite ingeniously artistic formal aspects – is neither trivial nor superficial and it does not leave us cold and indifferent. Why? Because Jill’s artwork is not an end in itself – it serves a highly significant purpose. She strives to promote the understanding of gout because she strongly believes that the topic is unrecognized within our society and that there are numerous 7

existing misconceptions regarding the disease and its causes.

Thus, Jill accommodates the viewer of her works – and yet: are her artworks aesthetically beautiful?

Are they a representation of beauty? Or are they a beautiful representation of something that is not beautiful per se, but often whitewashed? In my very personal view, I see Jill’s work as a ruthless and disturbing, but undoubtedly captivating and therefore bearable depiction of something very unpleasant. Her intentions are not to beautify the world, but instead to help us, or rather urge us to improve our understanding of the world as it is and how it could or even should be.

Isn’t this the eternal purpose of art?

Kristallisation des Denkens Kristallisation des Denkens ist ein kollaboratives Projekt zwischen der Künstlerin Jill Tegan Doherty und Prof. Michael Doherty. Die Gicht ist die gängigste, doch auch potenziell ‘heilbarste’ Form der entzündlichen Gelenkerkrankung Arthritis. Allerdings werden die meisten Patienten einer unzureichenden Behandlung unterzogen, was meistens am Irrglauben liegt, die Gicht sei nur eine kurzzeitige, unernste, ja fast lächerliche Krankheit, die man sich selbst durch zu hohen Alkoholkonsum und einen ungesunden Lebensstil zufüge.

Dieses Projekt zielt darauf ab, solche Barrieren zu durchbrechen, die im Wege einer wirksamen Behandlung stehen, und die Sicht der Menschen bezüglich der Gicht zu ändern. So soll Wissen durch künstlerische Darstellungen verschiedener Aspekte der Erkrankung vermittelt werden.

Das Projekt versucht ein neues Verständnis zu verbreiten und hofft dadurch, die Behandlungsqualität dieser gängigen und doch verkannten Krankheit positiv zu beeinflussen.

Crystallisation of Thought Crystallisation of Thought is a collaborative project between artist Jill Tegan Doherty and Professor Michael Doherty. Gout is the most common, but potentially “curable”, form of inflammatory arthritis. However, most patients receive suboptimal care, largely due to widespread misconceptions that gout is only an occasional, non-serious, almost laughable condition that is self-inflicted due to excess alcohol and lifestyle abuse.

This project aims to break down these barriers to care by changing people’s perceptions and knowledge of gout through artistic representations of various key aspects of the disease. It aims to engage a new understanding of gout, impacting on people’s existing perceptions and helping to improve the standards of care to this common, yet extremely misunderstood disease.

Die Gicht entsteht durch die Ablagerung von Harnsäurekristallen, sogenanntem Urat, in und auf den Gelenken – insbesondere den kühleren, peripheren Gelenken der Hände und Füße.

An Inescapable Pressure illustriert die psychische Belastung, die viele Gichtkranke empfinden.

Diese stammt oft von einer unzureichenden Behandlung und einem mangelnden Verständnis, was dazu führt, dass Menschen bisweilen über mehrere Jahre ihres Lebens unnötigerweise an der Erkrankung leiden. Der Titel suggeriert sowohl die psychische Belastung als auch die drückenden Gelenkschmerzen.

Gout is caused by sodium urate crystals that form in and around joints – especially cooler peripheral joints in the feet and hands.

An Inescapable Pressure aims to illustrate the psychological strain that can be caused by gout.

This is due to the suboptimal levels of care and understanding of gout, which means that people suffer unnecessarily from the disease, often for many years of their lives. The title suggests the psychological pressures, as well as the acute physical pressure gout has on the joints.

Die Hauptursache von erhÜhten Uratwerten und somit von Gicht ist eine fehlerhafte Nierenfunktion, bei der Harnsäure nicht effizient ausgeschieden wird. Dies ist meistens vererbt, kann aber auch durch die langfristige Einnahme von Medikamenten (z.B. gegen Bluthochdruck) oder durch eine chronische Nierenerkrankung bedingt sein.

The main cause of raised uric acid and gout is having kidneys that are inefficient at excreting uric acid. This is often inherited, but can also result from long-term medications (e.g. antihypertensive drugs) or chronic kidney disease.

Kristalle bilden sich langsam doch kontinuierlich über mehrere Jahre hinweg, ohne dass der Gichtkranke es merke.

Uratkristalle legen sich in und auf den Gelenken ab, wenn die Gicht nicht gezielt behandelt wird.

Dauerhaft erhöhte Uratwerte führen dazu, dass sich Kristalle bilden und zusammenwachsen.

Die marschierenden Ameisen illustrieren dieses unnachgiebige Wachstum. Oftmals glauben Patienten, fälschlich, dass der Vorgang aufhöre, wenn die Schmerzen einer akuten Attacke abklingen, und setzen dann die Medikamente ab. Zusätzlich zu Gichtattacken können sich die Kristalldepots in das Bindegewebe vorarbeiten und zu harten Steinen werden (sog. Tophi, lat. ‘kleiner Hügel’). Die Kristalle fühlen sich dann wie Klumpen unter der Haut an und können akute Schmerzen, Schwellungen und unheilbare Gelenkschäden hervorrufen. The Significance of Time (Der Wert der Zeit) bringt die Wichtigkeit einer zeitnahen Diagnose und frühen Behandlung zur Geltung.

Ameisen leben in Kolonien und agieren als Einheit.

Man weiß, dass sie kollektiv arbeiten, um das Nest mit Nahrung zu versorgen. Zudem machen sie vor nichts Halt, um die Kolonie zu unterstützen.

Das Gemälde der Ameise spiegelt den ähnlichen, unaufhaltsamen Zyklus der Kristalle wider und stellt dar, wie sich im Laufe der Zeit Klumpen bilden.

Die Kristalle wachsen solange die Harnsäurewerte nicht geregelt werden. Klinisch spürbar werden die Kristalle für den Gichtkranken jedoch erst, wenn sie über eine gewisse Zeit gewachsen und bereits in großen Mengen vorhanden sind.

Die gezeigten Ameisen sind rote Feuerameisen, eine invasive Spezies, die sich selbst in Gebieten etablieren kann, in denen sie nicht beheimatet ist. Feuerameisen sind auch für ihre besonders schmerzhaften Bisse bekannt.

Crystals form slowly but continuously over many years, without you knowing that this is happening.

Urate crystals can build up within the joints when gout is left untreated. Persistently raised levels of uric acid in the body cause sodium urate crystals to slowly but relentlessly form and eventually clump together. The marching ants illustrate a constant movement of crystal growth. Commonly people are unaware of this and often will cease taking medication once the pain of an acute attack dissipates. In addition to gout attacks, continuing crystal deposition can result in hard, slowly expanding subcutaneous crystal concretions (“tophus” – Latin, “a small hill or mound”). The crystals can appear as lumps under the skin and cause episodes of acute pain, swelling and irreversible chronic joint damage. The Significance of Time emphasises the importance of receiving a correct diagnosis and course of treatment early on.

Ants are part of a colony and operate as a unified entity; they are known to collectively work together to instinctively bring food back to the nest and will stop at nothing in order to support the colony.

The idea of the ants represents this cycle, creating a visual image, which shows that over time the

quantity of crystals is increasing and causing clumps to form. Crystals continue to form unless the level of uric acid is controlled. It is only once the crystals have been growing for some time and are present in large numbers that it becomes clinically apparent to the patient.

These particular ants are red imported fire ants, they are an invasive species and are able to establish themselves in environments where they have been accidentally introduced. The red ants are also known for their painful bites.

Über die Gicht hinaus können erhöhte Uratwerte auch Schäden innerhalb der Blutgefäße, sowie Herzleiden, Schlaganfälle, chronische Nierenerkrankungen und eine kürzere Lebenserwartung zur Folge haben.

The Monster Swims Within Our Blood ist eine moderne Interpretation des Gemäldes, das oft als Darstellung der akuten Schmerzen einer Gichtattacke verwendet wird. Ursprünglich von James Gillray 1799 gemalt, zeigt es die Pogara (lat. ‘Fußfessel’) des Künstlers als Dämon oder Drachen. Meine eigene Interpretation des Gichtmonsters beruht auf dem lebensbedrohlichsten Symptom der Gicht, dem Herzversagen. Der Titel deutet darauf hin, dass das Urat in unserem Blut das wahre Monster ist.

Apart from causing gout, elevated serum uric acid can injure the inside of blood vessels and cause heart disease, strokes, chronic kidney disease and shortened life-expectancy.

The Monster Swims Within Our Blood is a modern take on an image that is frequently reproduced to portray the pain of an acute attack of gout.

It was originally drawn by James Gillray in 1799, depicting the pain of the artist’s podagra (“seizing the foot”) as a demon or dragon. For my interpretation of this gout demon I have chosen the most life threatening symptom of Gout to represent, heart failure. The title suggests that it is the uric acid which runs in our blood that is the true monster.

Roots zeigt große Gicht-Tophi anstelle der Nieren und erinnert den Betrachter daran, die Ursache des Problems zu bedenken und zu behandeln, nicht nur die Symptome.

Crystals form when serum uric acid concentrations are persistently raised above the critical “saturation point”.

Roots visually depicts kidneys in the place of large gout tophi, leading the viewer to consider and treat the root of the problem and not simply the symptoms.

Übergewicht und Fettleibigkeit können ebenfalls die Uratwerte im Blut erhöhen. Tatsächlich ist das Übergewicht ein größerer Risikofaktor als die exzessive Einnahme spezifischer Nahrungsmittel (z.B. rotes Fleisch, Meeresfrüchte) oder von Alkohol (Bier und Spirituosen).

Being overweight or obese can also raise serum uric acid levels, through an increased production of uric acid – being overweight is a more important risk factor than excessive intake of specific foods (e.g. red meat, shellfish) and alcohol (beer and spirits).

The Nest nimmt die großen Schmerzen zum Thema, die für die Krankheit typisch sind. Das Gemälde stellt die Stärke und Intensität der Schmerzen während einer akuten Attacke bildlich dar. Viele Patienten vergleichen den ersten Anfall mit einem Wespenstich, der plötzlich schmerzt und über Stunden hinweg schlimmer wird. Sie beschreiben ihn oft als unerträglich und nicht aufzuhalten – etwa wie wiederholte Wespenstiche.

The Nest focuses on the element of pain, which the disease typifies. It aims to visually represent the level and intensity of the pain during an acute attack. Numerous patients mentioned that the initial “attack” felt like a wasp sting, with sudden onset and then increasingly worsening within just a few hours. It was described as an unbearable, unstoppable sensation, perhaps that of repeated wasp stings.

Die unzureichende Behandlung der Gicht führt zu chronischen Schmerzen und zu einer verringerten Lebensqualität. Embodied zeigt die Depression, psychischen Traumas und Verzweiflung, die oftmals weitere Folgen der Gicht sind.

Die Verringerung der Uratwerte, sodass sie permanent unter dem Sättigungspunkt liegen, kann die Bildung neuer Kristalle eindämmen und langsam zur Auflösung existierender Kristalle führen. Wenn keine Kristalle mehr im Körper sind, verschwinden auch die Gichtsymptome.

Suboptimal care in the field of gout leads to long-term suffering and a reduced quality of life. Embodied illuminates the depression, psychological trauma and desperation that can be additional symptoms of gout.

Reducing and maintaining serum uric acid concentrations below the saturation point stops production of new crystals and slowly dissolves away existing crystals, so eventually there are no crystals and therefore no gout.

Die verstärkte Ablagerung von Uratkristallen in und auf den Gelenken führt zu kleinen, harten Klumpen (Tophi), welche Knorpel und Knochen dauerhaft beschädigen können.

Wenn sich zahlreiche Kristalle bilden, kann es vorkommen, dass sie vom Knorpel heraustreten und sich im Gelenk ablagern. Dies führt zu schmerzhaften Entzündungen des Bindegewebes, Flüssigkeitsbildung, Schwellungen und einem Druckgefühl im Gelenk (einer sog. Gichtattacke). [siehe Abb. 3 in Cross Section of a Joint Showing Urate Crystal Formation.]

In vielen Fällen befinden sich bei der ersten Attacke, sprich wenn die Gicht erstmals diagnostiziert wird, schon signifikante Kristalldepots in den Gelenken (häufig mit dem großen Zeh beginnend). Wird die Kristallbildung dann nicht verhindert, kann es zu wiederholten Attacken und zur Ausbreitung auf andere Gelenke kommen.

Increasing deposition of urate crystals in and around the joint leads to very small expanding hard lumps of crystals (tophi) that can damage the joint cartilage and bone. When a lot of crystals have formed, a few may spill out from the cartilage into the joint cavity, triggering severely painful inflammation of the soft joint lining with increased fluid production, swelling and greatly raised pressure within the joint (an “attack” of gout). [see figure 3 in Cross Section of a Joint Showing Urate Crystal Formation.]

Many joints will already have significant crystal deposits by the time the first attack occurs (often in a big toe) and gout is initially diagnosed. If crystal deposition is allowed to continue attacks are likely to get more frequent and spread to involve other joints.

An Unfortunate Gift basiert auf der vererblichen Natur der Krankheit. Hier wird die Gicht, in Form eines Ballons, nahezu wörtlich von einer Generation zur nächsten gereicht. Der Luftballon ist ein Symbol für die Schwellung der Gelenke und für den Druck, den die Tophi von innen ausüben. Gleichzeitig stellt der Ballon die Last der Eltern dar, die die Vererbung der Krankheit nicht verhindern können. Auch zeigt das Gemälde, dass die Mehrheit der Patienten ohne eigenes Zutun an Gicht leiden.

An Unfortunate Gift reveals gout as a hereditary disease. In this image gout is literally being handed down to the next generation, in the form of a balloon. The balloon is a representation of the physical swelling and feelings of pressure caused by the tophi, while simultaneously implying the pressure the parents must carry who are unable to prevent it. It highlights that the majority of people suffering from gout do so through no fault of their own.
