Arizona’s Medicaid agency, known as AHCCCS (Arizona Health Care Cost Containment System), provides the Arizona Long Term Care System (ALTCS) for individuals who are 65 or older, blind or disabled and need ongoing services at a nursing facility level of care. However, program participants do not have to reside in a nursing home. AHCCCS (pronounced “access”) uses several contractors to provide long-term care services under ALTCS. An ALTCS contractor works like a health maintenance organization (HMO). e program contractor works with doctors, nursing homes, assisted-living facilities, hospitals, pharmacies, specialists, etc., to provide care. Each enrollee is assigned a case manager who will coordinate their care. Many ALTCS participants live in their own homes or an assisted-living facility and receive needed in-home services. ere are several di erent meal programs available for seniors in the Valley. e Area Agency on Aging (602-264-4357) is an excellent resource to nd the meal-delivery service in your area. Several private-pay mealdelivery services are available as well. Many of the meal programs ask for a donation or a nominal fee.
In addition to a host of other services, some of Arizona’s Centers for Independent Living (CIL) provide non-emergency transportation to medical appointments, jobs, social activities and CIL-related events.
Some CILs also o er travel training, where seniors can learn the overall transit system and how to use public transportation so that they can be more independent in the community. Seniors may be required to pay for transportation services; however, travel vouchers might be available to help defray costs. e Arizona Statewide Independent Living Council lists the CILs in the state (including Arizona Bridge to Independent Living, see listing below) on its website, azsilc. org. Contact the CIL in your area about the availability of transportation services.

Continuing Care Retirement Communities (CCRC) o er residents the ability to “age
in place” with the knowledge that they won’t need to move to other facilities for health reasons. Whereas most retirement communities o er independent living and assisted living only, Continuing Care Retirement Communities provide a continuum of care that includes independent and assisted living, secured Alzheimer’s care and skilled-nursing care. Each level of care is provided in a separate building and/or area of a campus and nearly all residents move into the community while living independently (without the need for assisted-living or skilled-nursing services).
CCRCs may o er three basic types of contracts: life care (Type A), modi ed (Type B), and non-traditional (Type C). e primary di erence between the rst two is the amount of skilled-nursing care received, without a change in the monthly fee. In the third type, the resident is nancially responsible for all care.
Life care contracts include unlimited enriched-housing/assisted-living care (if o ered by the community) and unlimited skilled-nursing-facility services, along with independent housing and residential services and amenities. e resident’s monthly fee
cannot change due to a change in the level of covered health care needed by the resident (except for normal operating costs and in ation adjustments).
Modi ed contracts include independent housing and residential services but include only a limited number of days in a skilled-nursing facility. e number of skilled-nursing days covered in a modi ed contract may vary, but the contract must include at least 60 days of skilled-nursingfacility coverage, exclusive of Medicare days. As in the life care contract, the resident’s monthly fee cannot change due to a change in the level of covered health care needed during the skilled-nursing-facility bene t period speci ed in the contract. Modi ed contracts may also include an enrichedhousing/assisted-living bene t (if this service is o ered by the community). If included in the contract, the resident’s monthly fee cannot change due to a need for enrichedhousing/assisted-living services during this covered bene t period. When the contract’s health-care-facility days have been exhausted, the resident must then pay a market or per diem rate if additional skilled-nursing-care days (or enriched-housing/assisted-living days) are needed.
Non traditional, fee for service contracts (Type C)
Non-traditional, fee-for-service continuingcare contracts include independent housing, residential amenities and social activities, and access to a continuum of long-termcare services. e long-term-care services, enriched-housing/assisted-living and skilled-nursing-facility care are available on a fee-for-service or per diem basis. ere is no long-term-care bene t included in the contract; the resident pays for long-term care if and when needed.
Assisted living is an alternative to the CCRC independent-living concept. It is not appropriate for people who need 24-hour nursing care. Instead, assisted living is more of a philosophy of care that promotes the residents’ dignity. ese facilities provide supervision and assistance with activities of daily living, coordination of services by outside health-care providers and monitoring of resident activities that ensure their health, safety and well-being.

A nursing home is normally the highest level of care for older adults outside of a hospital. Nursing homes provide what is called custodial care and help with their activities of daily living, including getting in and out of bed, and providing assistance with feeding, bathing and dressing. However, nursing homes di er from other senior housing options in that they also provide a high level of medical care. A licensed physician supervises each patient’s care and a nurse or other medical professional is almost always on the premises. Skilled-nursing care is available on site, usually 24 hours a day. Other medical professionals, such as occupational or physical therapists, are also available. is allows the delivery of medical procedures and therapies on site that would not be possible in other housing. Skilled nursing is usually used to describe a Medicare stay for an acute medical condition, such as a broken hip or stroke. Once a resident has completed the therapy for standard recovery covered by Medicare, the resident either graduates and goes home or moves into long-term-care living at the nursing home.
