FROM THE PRESIDENT ADA NSW President Dr Kathleen Matthews explores what the term innovation means in the workplace and the impact it has had on her own professional career.

Innovation is here to stay Firstly, welcome to all the newly registered dentists who have graduated or qualified in 2020 to our profession.

Congratulations on completing the initial part of your career journey to now make the transition from student or candidate to registered dental professional. On behalf of the ADA NSW membership, the Board and Council and the ADA NSW team we wish you every success. We are here to support you all - we are only a phone call or an email away.

After a year of upheaval and change in 2020 we now commence the new year in 2021 and I suspect many of us, including myself, are hoping for some ‘ordinary’ times in this year ahead. This month’s NSW Dentist has a focus on ‘innovation’ as a theme; possibly a challenging notion for some, as some ‘normal’ would be comforting right now. Innovation represents change, and doing something differently. Professionally it involves us taking a calculated risk, challenging the status quo, developing new skills, and thinking and working in a different way.

For a ten-letter word it can certainly create a negative response in the workplace.

Only about 20% of the workforce within an organisation is ready to embrace innovation, while it remains in the top three priorities for CEOs, according to a Harvard Business Review article in February 2020 entitled ‘Stop Calling it Innovation’. Clearly there is a disconnect and maybe this word needs a makeover.

What have I changed or innovated in my practise of dentistry over the course of my professional life? A lot, and I suspect mostly it’s been small incremental changes that have occurred to drive the end result – the Monday morning idea I took away from a CPD presentation or adapted from a colleague.

Sometimes, I start a discussion with dental students or new graduates about the ‘first procedure’ I performed on my first day in my first job without any supervision. I was presented with a non-carious fracture of an upper premolar tooth. I placed a direct restoration – a pin retained amalgam - and was pretty happy with the result. Fast forward 30 years and now that is not my ‘go to’ design for that problem.

What else has altered? So, so much such as greatly improved infection control measures, including instrument reprocessing and mandatory instrument tracking. There is also x-ray technology (from film and chemicals to digital and direct capture technology), record keeping (paper, plaster/ stone and photographic film, to complete digitisation of records), tooth replacement (fixed or removable prosthetics through to implants) and orthodontics (fixed banding and now the added option of serial aligners).

These examples only represent some of the day to day processes we engage in as dentists; we’ve needed to adapt and embrace change as better solutions became available.

Not innovating is the bigger risk when I look back across my career. This last year

has driven a lot of innovation within the health system and concepts like telehealth and virtual appointments are now real world.

The foundation of our profession is still there – we take care of and help to repair and improve the oral health of our patients and communities. The ‘real-time’ portion of it has just had a re-invention. The changes we adopt and keep into the future will be called an ‘innovation’ when we look back. The year 2021 will undoubtedly bring with it a host of further new opportunities and challenges, and our ability to embrace new ideas; especially those which improve the process and experience of dentistry for both patients and its professionals, will play a vital role in shaping the future of our profession.

FROM THE CEO ADA NSW CEO Stes Stengos reflects on how the organisation is evolving to best meet the needs of members.

Embracing change is a two-way street Through my experience with ADA NSW, I have come to learn that dentists are hugely receptive and open-minded to new techniques, working practices and technological developments which can further improve patient care and oral health.

Although it has been an ordeal, the pandemic has also illustrated how dental practitioners are incredibly adaptable and flexible responding to a crisis while continuing to provide vital dental care for patients.

With vaccines commencing rollout across the globe, hopefully there will be a light at the end of the COVID-19 tunnel. ADA NSW recognises that the pandemic continues to present ongoing challenges for you and your practice teams. That’s why we are continually reviewing our Strategic Plan, systems and working methods to ensure we are providing you with the best possible service.

We’re always striving to improve because we know that’s what you do yourselves constantly as practitioners. We’re always looking at new and better ways of doing things and what you need from us to ensure you can carry out your roles most effectively.

Recent examples of this included shifting CPD courses online, introducing flexible membership payment options and further improving communications to members during the pandemic.

Whether it’s Practice Services, Membership, Advisory Services, CPD, Communications, Corporate Services or our Knowledge Centre, rest assured our team is committed to innovation and continuous improvement. Just as you as dental practitioners are always ready to embrace

innovation, we’ll do the same if it helps provide you with a better service.

Recently we were honoured to have our efforts recognised at the Association Forum national awards, with ADA NSW shortlisted as a finalist for the Association of the Year title by the Forum, which is the country’s leading organisation assisting associations and charities in governance, operations, membership and finances.

ADA NSW was also shortlisted as a finalist for the Achievement of the Year award, in recognition of our work through our Filling the Gap charity to provide voluntary pro bono dental treatment for vulnerable patients.

This achievement is a real testament to the terrific work of my colleagues, staff and everyone connected with ADA NSW, and it further highlights the vital importance oral health has in the community and why it must be promoted extensively as a key health issue in Australia.

At the time of writing, COVID-19 cases are unfortunately occurring in Greater Sydney and other areas of NSW. NSW and ACT dental practitioners continue to work in an ethical and COVID-safe manner, helping maintain patients’ oral health during the pandemic.

ADA NSW is also regularly informing members about significant COVID-19

outbreaks and news updates. Please watch out for these and keep monitoring the NSW Health and ACT Health websites for the latest information.

I’ll finish by thanking all ADA NSW staff for their hard work during this demanding time and the ADA NSW Board members for their ongoing support.

Future directions: where are we headed next?

A taste of what’s to come in 2021 as existing and emerging technologies continue to shape and impact on the profession:

Teledentistry Teledentistry has been used both formally and informally by many dentists for several years. COVID-19, however, accelerated its use, especially in areas of the globe badly hit by the pandemic. The TeleDent

by MouthWatch service, for example, is a platform offered by Henry Schein using video conferencing and even ‘virtual’ emergency triaging for patients needing treatment.

As recently reported by the dentistrytoday.com website, a recent survey in the US found that 75% of dentists who already provide teledentistry services expect the volume of remotely consulted patients to either stay steady or increase over the coming 12 months.

However, only 34% of those surveyed actually see, or plan to see, patients via teledentistry platforms. This suggests that many dentists are either unaware of this type of technology or are unwilling to engage with it - for now.

Artificial Intelligence Artificial Intelligence (AI) is hugely impacting the provision of Australian healthcare, particularly in specialities such as radiology where machine learning and algorithms can be effectively applied. There are also several companies now promoting the use of AI in dentistry. These include dentistry.ai, whose founders have argued that AI will be “tightly woven” into dental practices over the coming decade.

This may encompass increasing use of computers to understand human speech, machine learning to analyse records and book patient appointments without human involvement, and using algorithms to interpret radiographs.

As Brad Race, CEO of dental laboratory Race Dental, highlights in this month’s article, AI currently has limitations for the way it can be applied to dentistry.

Robotic Dentistry Robotic-assisted surgeries have become a common part of most healthcare specialities across the globe. Their presence in dentistry is also starting to grow. Dental robotics company Neocis, for example, has said that about 1,000 implants were placed by their Yomi system during 2019.

Now in use at dental schools in the US, Yomi provides computerised navigation to assist in both the planning (pre-operative) and the surgical (intra-operative) phases of dental implantation surgery. Neocis claims that it allows a minimally invasive and more patient-friendly experience.

Further developments in technology and computer science will likely increase robotic surgery’s presence in dentistry.

Use of social media and apps Many dental practices in Australia are increasingly using their websites or social media accounts to highlight their services and better inform and educate patients about oral health. In a further step, mobile apps are now also becoming more prominent as a way of enhancing the practitioner-patient relationship.

Early last year US start-up company Dentulu launched an app connecting patients with dentists and provide ‘mobile and virtual dentistry on demand’. Billed as a form of ‘Uber for dentistry’, the service allows patients to contact and share information with dentists about oral health issues without breaching the country’s health privacy data laws.

Smart toothbrushes Just as dental practitioners are adopting new technologies, patients are also increasingly keen to try new dental products for their health – and cosmetic – benefits.

Last year Colgate and Oral-B both launched new Smart toothbrushes, equipped with biofilm detectors, pressure sensors to improve brushing and even displays to increase usage and motivation.

Other toothbrushes have been developed using radio frequency vibrations to loosen plaque and fight tooth decay.

Advocacy update

Members are reminded that they and their practice managers should continue to check updated NSW Health and ACT information daily for the latest details of updated COVID-19 cases.

COVID-19 cases are continuing in Greater Sydney and other areas of NSW at the time of writing. The NSW Health and ACT Health websites are updated continually with details of venues, dates and times of these cases, while ADA NSW is also continuing to contact members with latest COVID-19 updates through email and also via www.adansw.com.au The NSW Health website can be accessed via www.nsw.gov.au/covid-19.

The ACT Health information page is available via www.covid19act.gov.au The Federal Government has indicated final approvals and COVID-19 vaccine rollouts will progress this month.

While healthcare workers including dentists are expected to be among the first recipients of the vaccine, there has been no further confirmation of this at the time of writing.

In addition, it is not known yet what role – if any – Australian dentists will play in administering vaccines. In parts of the US and in the UK, where the pandemic is continuing to spread rapidly, dentists have already started receiving the vaccine. Further updates will be provided by ADA NSW when available.

ADA NSW has been continuing its strong advocacy of oral health awareness.

Just before Christmas, Engagement and Advocacy Executive Dr Sarah Raphael was interviewed as part of a Channel 7 News piece on the importance of spending outstanding private health insurance benefits on necessary dental work before expiration at the end of 2020.

The piece was featured on Channel 7’s national 6pm bulletins across the country and was arranged with assistance by ADA NSW’s Communications team.

It was one of about 300 media articles either arranged by the ADA NSW or featuring ADA NSW media representatives during 2020, all highlighting the importance of dental health.

Obesity crackdown in the UK Supermarkets in England are to be barred from displaying unhealthy food and drinks at checkouts or using them in buy one, get one free offers as part of a proposed government crackdown on obesity.

Free refills on soft drinks will also be banned under the new plans, which are scheduled to be introduced in April 2022.

The limits, which have been welcomed by campaign groups, will only apply to companies employing more than 50 people and stores spanning more than 2,000 square feet.

ADA NSW is continuing to advocate for stronger measures domestically to combat tooth decay and associated conditions such as obesity and type 2 diabetes.

These include supporting the Rethink Sugary Drinks campaign and calling for a levy on sugary drinks sold here in Australia.

What are some of challenges facing Australia’s largest dental laboratory? ADA NSW Media Advisor Stuart Turner discovers more.

Brad Race, CEO of Race Dental, still remembers the day he realised the world for Australian dental laboratories had changed forever. scanners, mouthguards, snoring / sleep apnea devices and clear aligners.

With a 100 plus-strong workforce at its Sydney base, Mr Race, who runs the business with his two brothers, Matt and Anthony, said working life was never dull.

‘Born and bred in teeth’

“I remember walking in to the office one morning about 15 years ago and saying, ‘where’s all the easy work gone’, Mr Race, CEO of family-owned Race Dental, said with a grimace.

“It was a period where a lot of work was going offshore, sourcing staff was problematic and we had significant innovation and training issues that we didn’t know how to fully address.

“I gave my brothers a block of zirconia and said, ‘we need to make this look like a porcelain-fused to metal crown for $120 or we will be out of business’.

“We’d been through huge technological changes over the years, but I knew we had to step up things like digitisation and automation while making sure we remained high quality, costeffective and efficient. It was a fight for survival.”

It was a fight that the organisation, which has now established itself as the largest dental laboratory in Australia, is clearly winning.

Based at Castle Hill in Sydney’s north-west, the company is a pioneer in the scanning, designing, milling and printing of dental restorations.

Established in 1936 by Mr Race’s grandfather, John, Race Dental makes and supplies crowns, bridges, implants, orthodontics and dentures to dental practices across Australia.

Now with additional laboratories in Malaysia, Singapore and New Zealand to support its export business from Australia, the company also provides dental equipment including intraoral

“There’s a few people walking around in lab coats working with Bunsen burners, but life inside the laboratory is a bit more than that,” he said.

“There’s lots of computers, banks of 3D printing machines manufacturing models of all kinds and big machines as well. A lot of our equipment is one-of-a-kind in Australia and we do massive testing and research and development before we release anything to our customers.

“We are a family born and bred in teeth so we have a feel for what will work and which technologies are worth pursuing.

“Before COVID-19, I spent about a third of my time travelling overseas, researching new technologies and ensuring we’re monitoring global developments.”

While the pandemic has put a (hopefully) temporary stop to his globetrotting activities, Mr Race said the virus had initiated other opportunities.

“Restrictions on international flights and freight has meant many dentists are supporting Australian labs again,” Mr Race said. “I don’t take that for granted but I think that will stay.

“We believe we are more competitively priced – the quality is there and the turnaround times are far better too.

“The virus has also impacted ‘dental tourism’.

Instead of going overseas, people are now staying here for dental procedures.

“I believe it will be 12-24 months before international travel starts again and, with Australia in a better position than most, we may see patients coming here to get treatment. The dental industry will have to work together and innovate to meet these new challenges.”

‘A different world’ Mr Race said the increasing desire for what he terms ‘Hollywood smiles’ among patients was driving product diversity.

“Years ago, a patient would see a dentist and ask for whatever’s available when it comes to things like crowns,” he said. “Now with things like social media, patients are more aware of what’s going on with their mouths. They want different things so we’ve had to factor that into our designs.”

With technology advancing so quickly, Mr Race said it was vital dentists kept abreast of new products.

“For dentists of a certain age, they were generally taught about porcelain-fused to metal but now the product range is changing so quickly,” he said. “I think dentists are generally interested in new technologies, but don’t have time to stay in touch with new developments. That’s fine, but if you don’t stay up-to-date, it’s like leaping from an old Nokia phone to an iPhone 12. When you eventually do make the leap it’s a different world and hard to adapt to.

“I’d encourage dentists to visit our laboratory and see what we do and how we do it, just as we encourage technicians to see how the dentist meets the challenge of fitting a crown. Dentists who take the time to visit us see the level of quality we are able to consistently produce as well as get a feel for how many of their peers have taken the successful step doing digital impressions.”

Mr Race also said the more ‘connected’ world and growth of ‘DIY dentistry’ posed challenges for his business.

“Patients are more educated about their teeth or are seeing cheaper options and without knowing the difference they think they’ve found the best option,” he said.

“We’ve seen the growth of offshore companies like Smile Direct Club and things like home impression kits hitting the market. Rather than focusing on trying to

shut them all down, we must work together as an industry in Australia to see how best we can educate the public and compete with these new competitors.

“No-one stepped in and helped the lab industry compete 15 years ago when the majority of work started to go to China. We had to innovate and learn to compete on a new landscape.

“We charge less for a crown now than we did when I first started working for my Dad over 30 years ago. To increase our margins, we no longer have the luxury of having a price rise – we have to innovate and improve processes alongside investing in the latest technology. The status quo no longer cuts it.

“I hear people complain about dental work going offshore all the time but rather than waste 10 years of my life trying to shut down the work going to China, I have spent those 10 years focusing on innovating to compete in a changing world. My passion now is to work with dentists so they compete with the very real threat of ‘non-dentist’ options and dental tourism which will come back post-COVID.”

‘One dentist is a Picassso, the next is a Rembrandt’ Mr Race said there were always exciting developments coming in dentistry.

“The biggest difference I’ve seen is the significant uptake of customers going with intraoral digital products,” he said. “About 60 per cent plus of our crown, bridge and implant work comes from intraoral scanners so we continually evaluate our work in this area.

“3D printing is the new ‘sexy’ toy. You need banks of machines though to make it efficient, so I think it’ll be a few years before we are making devices created in this manner that work long term in the mouth chairside.

“Artificial Intelligence is something I monitor but I wonder how it will apply to dentistry.

“As I see it, one dentist is a Picasso and the next is a Rembrandt. We aren’t cranking out the same crowns every time, for example. Every job is different.”

Mr Race said being CEO of a large organisation continually taught him valuable lessons about leadership which could be applied to dental teams.

“As ‘the boss’, even if you are unsure of the solutions – for example, as with a crisis such as COVID-19 - you have to be seen to have the answers,” he said. “You definitely need perseverance and also be constantly examining how you can progress.

“I’ve also never been afraid to work alongside people who are smarter and more capable than myself.

“You must invest in your staff too. I think if people feel they are learning and developing, they will be keener to stay with you.”

Mr Race said it was hugely rewarding helping people achieve better oral health care.

“We do hundreds of different jobs a day at Race Dental,” he said. “I love encountering a person who has had prosthetic work done and knowing there’s a chance our organisation has helped them.

“For myself, seeing the business continuing to grow constantly is the biggest buzz. Taking Australian-made products and exporting what we do here as opposed to the other way round is what I enjoy most.

The journey never stops.”

he COVID-19 pandemic has brought the quality of infection prevention and control (IPC) programs in all healthcare settings into the spotlight. While the pandemic has dominated the news headlines, ADA NSW Practice Services Manager Kylie Robb said it is ‘business as usual’ in dentistry when it comes to managing COVID-19 risk.

“Dental practitioners have been managing infection control risks and exercising their professional judgement when caring for patients long before COVID-19 came along,” Ms Robb said.

“In my view, the pandemic has provided an opportunity for all healthcare practitioners to critique their existing IPC processes and question the ‘status quo’. I am very happy the profession is looking for ways to quality improve their infection control programs – even when the literature indicates the evidence of transmission of pathological agents during dental treatment still remains low.”

“Screening your patients and considering what treatment you’re about to perform is a key element of your dynamic COVID-19 risk management strategy.

The outcome of this assessment would influence how transmission-based precautions are tailored or whether you can carry on with standard precautions as a minimum level of infection control.”

An innovative approach Kylie and her colleagues in the ADA NSW Practice Services team, Rebecca Jenkins and Vanessa Lane, are leading the charge on supporting practices across NSW and the ACT to quality improve their IPC programs.

Building on their extensive experience conducting hundreds of onsite infection control reviews and their engagement with over 2,500 participants through more than 60 education courses, the team is constantly looking for ways to improve member services based on the trends in data and their frontline experience.

“Innovation for us is about improving the quality of our members’ experience while focusing on service,” Ms Robb said.

“Everything we do is to benefit the member so they can be more confident in their practice when it comes to streamlining their IPC programs.”

Through their ongoing work, it became apparent to the team that shifting the mindset on IPC to a ‘whole of practice’ approach was key to empowering dental teams to maintain best practice in their workplaces.

Building on this sense of ownership, the ADA NSW Practice Services team recently launched an Infection Control Support Service to provide ADA NSW members

and their teams with access to confidential support for infection control. This new service can be used by any member of a member’s dental team, including dental assistants and hygienists. IPC support aligns with dental practice accreditation requirements and the expectations of the Dental Board of Australia.

A passion for dentistry Ms Robb thrives on responsibility and is passionate about the operational side of the profession. She started in a practice as a junior dental nurse 18 years ago and joined ADA NSW in 2013.

She knows that good systems and processes are paramount for a productive and effective dental team. Her primary focus has been IPC following a series of Sydney infection control breaches in 2015. In response to members requesting proactive IPC support, she compiled a risk-based IPC criteria framework that has since been applied to hundreds of dental practices and now standardises all IPC professional guidance for ADA NSW. “I have been feeling very popular lately as IPC support is such a priority at the moment,” Ms Robb said. “I am also a Board Director for the peak body for IPC across Australasia. So it has been a lot of pressure over the past 12 months, but I do love it.”

Ms Robb is a Board Director of the Australasian College for Infection Prevention and Control, has a Master of Health Services Management in Clinical Leadership, is a Credentialed Infection Control Professional, and Conjoint Lecturer for the University of Newcastle’s Faculty of Health and Medicine.

She has also presented for the World Health Organization’s Western Pacific Regional Office in April, 2020 – a career highlight.

Risk-based approach Identifying and analysing risks associated with the delivery of healthcare is an integral part of successful IPC. “We aim to support practitioners to adopt a risk-based, outcomes-focused approach when making decisions about what their IPC programs look like at a practice level,” Ms Robb said. “A key priority of the team at ADA NSW is understanding the settings that dentists work in. All of the information and resources we create are focused to help dentists and their teams to achieve best practice.

“I am very proud to be able to advocate for ADA NSW members and lift the profile of the profession in Australia at a local and international level. The ADA’s profile as a respected peak body has opened many doors for us to collaborate at the highest level to develop quality IPC professional guidance for members.”

All practitioners are required by the DBA to comply with the 2019 edition of the National Health and Medical Research Council’s Australian Guidelines for Prevention and Control of Infection in Healthcare document (view it online at www.nhmrc.gov.au).

The DBA also has continuing professional development guidelines that relate to a practitioner’s ability to demonstrate contemporary knowledge of IPC concepts.

Get Your Teeth Into Mouthguard Awareness Month With Mouthguard Awareness Month taking place once again next month, ADA NSW is asking members to help promote this vital initiative and help NSW / ACT patients avoid serious oral injuries during sporting activities.

With data showing only 36% of Australians wear a mouthguard while playing sport, ADA NSW is again highlighting how obtaining and wearing an appropriate, custom-fitted mouthguard from their dentist, rather than buying cheap alternatives from stockists, is essential for sports participants.

In a great initiative, Dapto Dentists recently linked with the local Avondale Junior Rugby Club, fitting and donating free custom-made mouthguards at the practice to some of the club’s players.

“A mother of some of the team members mentioned that the children weren’t wearing correct mouthguards while playing or training,” Dr Mohit Tolani, Associate Dentist at the Illawarra practice, said.

“It’s important to protect the mouth and prevent injuries in sports participants, especially those from low socio-economic backgrounds. Obviously, in a high-contact sport like rugby then the risk of tooth fracture and other more serious oral injuries is high.

“Anything we can do as dental practitioners to raise awareness of oral health, especially among young children, is terrific.

As well as helping protect their teeth and highlighting the important message about mouthguards, I also thought this was a great way to educate children about oral health knowledge more generally.

“We were delighted to help the kids and also hopefully reinforce the message of good oral health for a lifetime.”

ADA NSW has undertaken media advocacy in the past on this issue and will aim to do so again over the coming weeks.

From a Sri Lankan village to a lifetime helping others Dr Paul Gratiaen has lived an extraordinary life donating his time and resources to charity work across four corners of the globe.

As a child in a remote part of Sri Lanka (then called Ceylon), Dr Paul Gratiaen lived in a house in the middle of rice paddy fields owned and cultivated by his parents. There was no access to running water or power.

Wells were used to retrieve water that dropped down as far as 30 feet. Children played sport on fields just harvested and made their own toys.

It was a simple and happy upbringing that instilled in the recently-retired dentist the importance of community and in helping others.

“I learnt to value giving a helping hand,” he said. “Gratitude and values were ingrained in me from a young age and made me contribute in some way to make a difference.

“I learnt about the values of making a difference to others also from my uncle who was a Catholic priest in Sri Lanka and two aunties who were nuns.

“When I was eight years old, I fell down and fractured my upper central incisors.

I was a village boy in a remote part of Sri Lanka. I was able to get my teeth ‘fixed’ several years later when I was a student at the dental school.

“There are millions of kids and also adults in a similar plight to what I was facing seventy-one years ago.”

Dr Gratiaen, 79, originally aspired to do medicine before turning his efforts to dentistry. He started at Sri Lanka’s only dental school in 1964 where he was one of only 25 accepted in the school’s intake annually from a country that had 14 million people at the time.

After finishing his dental studies, he left for Liberia to work in a mission in 1974, travelling to the United Kingdom for a stint, before returning to work as a dentist again in Liberia which proved a life-changing experience.

“In some West African countries there are hardly any dentists and no dental training schools,” he said. “It was an honour and a privilege to volunteer in Africa and

help others. As dentists we often literally save people’s lives in poorer countries through our work.”

Dr Gratiaen emigrated to Australia in 1987 to complete a final year of dentistry at The University of Adelaide. He registered in 1990 to practise dentistry working in both the private and public sectors.

In a distinguished career, he worked 56 years as a dentist with nine years in clinical education. He retired as a Clinical Educator at the Westmead campus at The University of Sydney in November.

In the public sector, Dr Gratiaen made a significant contribution to public health at the Hunter New England Area Health Service, Central Coast Local Health District and Townsville Health Service District.

His overseas volunteer work has included short term positions in Vanuatu, Papua New Guinea and the Solomon Islands taking care of basic dental needs.

He has also worked in Cairns with Aboriginal communities before volunteering at the Australian Red Cross dental clinics four times annually from 2008.

Now re-named as Filling The Gap, Dr Gratiaen has continued to volunteer at these clinics up to his retirement, including at two last year.

“The patients at the FTG clinics feel we are there for them and that we care for them,” Dr Gratiaen said. “Patients often come in with an apprehensive demeanour at the start and are then treated and walk out with a smile.”

Innovative monitoring of tooth wear

Tooth wear is an ever-increasing problem for the dental profession with the growing consumption of acidic foods and drinks, dry mouth from polypharmacy and dehydration, as well as medical factors such as gastro-oesophageal reflux disease and eating disorders. There has been a trend towards consumption of more acidic foods, citrus fruits and vinegar dressings along with a worrying trend of patients consuming warm water with lemon juice in an effort to improve metabolism, aid weight loss and enhance their “wellness”.

The essential strategy in management is to determine the aetiology and provide a diagnosis. Identifying the cause can be difficult, and often multifactorial in origin so that acid erosion and parafunction may work synergistically to effect tooth wear with rapid progression. This may lead to visible changes with aesthetic consequences, tooth sensitivity, along with chipping, and dentine cupping of the tooth structures.

To treat or not to treat is a question many clinicians face in practice and is one of the dilemmas in managing these patients. The overriding objective of the practitioner is to ensure that the aetiological factors are managed so that further progression is avoided. The majority of tooth wear cases are slowly progressing, and unless

symptoms dictate intervention or patients want an aesthetic improvement, many cases simply require preventive strategies and subsequent monitoring.

Preventive strategies may include • Fluoride and desensitising agents • Beverage modification/dietary counselling • Habit changes – drinking through straw • Restorations – composite to replace chipped teeth and cover exposed dentinal surfaces • Stabilisation splint • Referral to medical practitioner

One of the difficult problems in management is to assess whether the wear is at normal physiological levels, is active or has ceased.

Often patients undergo sporadic bursts of wear activity with lifestyle changes and personal circumstances, which may have the potential to produce severe wear. It is the goal

of the practitioner to ensure patients are aware of the potential for tooth wear and how to minimise ongoing damage. Different techniques have been advocated to monitor the progression using tooth wear indices, however these can be tedious and difficult to assess each tooth correctly. Alternative techniques use analysis of photographs, or comparison of study casts which can be scanned with laboratory software and then compared digitally. Chairside, clinicians may use putty indexes/suckdowns that are placed onto the teeth providing a visual demonstration of any tooth surface loss (Figure 1). This demonstrates the problem clearly to clinicians and patients who may not be aware of the shortening of teeth due to the extended period that the tooth wear occurred.

More recently, the introduction of intra-oral scanners has allowed innovative monitoring of tooth wear with superimposition of the captured scans over time (Figure 2). The marked changes show precisely where the tooth wear is occurring as well as the extent of wear.

These graphic images educate patients so that they understand and ultimately take responsibility for their problem. Furthermore, it allows a decision to be made as to the need for continuing preventive strategies or restorative modalities.

How does this apply in practice?

Sometimes in our professional lives we need to take time away from direct patient care to reflect on how we are treating our patients.

It can be easy to forget what it is like to be on the ‘other side’, to be the recipient of healthcare. As a practitioner you are so familiar with the dental setting, it can be hard to anticipate the patient’s perceptions, fears, assumptions, expectations, prior experiences and so on.

The Australian Charter of Healthcare Rights (“the Charter”) provides a framework to consider when we think about how to deliver care. Although it seems self-explanatory, it is actually a really wonderful document to ‘reset’ your approach.

You would be surprised how frequently aggrieved patients refer to such documents when they make a complaint or claim about a practitioner.

This resource encourages you to take a step back and think about which aspect of your patient care could be improved or offered in a different way.

Here are some additional tips to consider.

• Consent is so important. Consent is not just a piece of paper or a signature, it is a process. It is usually a combination of both verbal and written information. Discussions need to be documented.

• You need to impart information to a patient in a way they understand such that they can make an informed choice. Think about different ways to describe things for people with different backgrounds. For example, a medical practitioner should have a different way of understanding disease processes to an engineer. A patient with limited English might benefit from pictorial depictions of treatment, and information sheets to take home and discuss with friends or family who can read them.

• A good way to document both the imparted information and the discussion would be to provide a paper information sheet about a treatment (e.g. an RCT information sheet), handwrite personalised discussion points on it as you talk through it with the patient, scan a copy for the file, and then give the original sheet to the patient to take home and think about prior to their next visit.

• Sometimes patients will want to dictate treatment. You still need to explain the pros and cons to them and discuss treatment alternatives. If a patient requests a treatment that you feel is absolutely not in their interests (i.e. a ‘bad idea’), you can and should decline to offer that option. You can always recommend that the patient seek a second opinion elsewhere. Likewise, if a patient pleads with you to complete treatment that is out of your scope (i.e. requires a specialist), you are absolutely within your rights to decline to provide that treatment and to recommend referral.

• Sometimes patients will want you to make the decision for them. Again, they need to make the choice themself, but you can help them by explaining the different options as simply as possible. Risks, benefits, limitations, costs, etc. need to be described for all the options.

Remember that patients need guidance but ultimately the choice must be theirs.

• What upsets patients? In the context of healthcare rights, it fundamentally comes down to lack of respect or care, either actual or perceived. Deficiencies in the consent process (both financial and treatment) are important too.

• What do we do when a patient feels aggrieved? Calmly listen to their concerns and find out what it is that upsets them. Only then can you address the issue. New research has underlined how dental practitioners must recognise the growing impact of Artificial Intelligence on their profession. ADA NSW Media Advisor Stuart Turner discovers more.

hen it comes to Artificial Intelligence (AI), dental practitioners shouldn’t fight – but flight isn’t an option either.

That’s according to NSW dentist Dr Hanna Pethani, author of a newly published research paper analysing AI’s potential impacts on practising dentistry and patient outcomes.

Promises and Perils of Artificial Intelligence also examines the ethical, legal and logistical considerations surrounding AI’s implementation in clinical practice.

“I think there’s a lot of fear about what AI can do,” Dr Pethani, who graduated from The University of Sydney’s dentistry school in 2012, said. “I also think there’s some unrealistic expectations about how it may affect dentistry.

“From my research, in clinical practice AI can have benefits for things like patient record-keeping and automated radiology reporting. I think it has potential to improve clinical outcomes and the patient experience, population health and dentists’ work flow.

“Dental practitioners shouldn’t be scared of AI, but they definitely need to understand it better.”

Dr Pethani, who started work on the study about a year ago, said AI’s impact on healthcare was already being felt.

“At the moment the research is new and mainly focused on things like dental imaging and interpreting,” she said. “In the US, however, there’s been approval in other areas of healthcare for AI systems to treat things like strokes and brain conditions.

“We’re also seeing universities now exploring subjects such as how digital health can enhance the patient experience.

“The people developing AI technology must work with clinicians to better master it most effectively. The IT guys may be great at developing systems, but not so much at understanding the clinical implications.

Government bodies must also work on

the legal and regulatory framework of developing AI technology. It’s a team effort.”

Dr Pethani also stressed that AI couldn’t – and shouldn’t – replace the ‘human face’ of practice teams.

“My own practice experience has shown me the ‘humane’ role all members of the dental team play,” she said. “Dental assistants, for example, play a vital role in calming anxious patients.

“AI is great in theory because it can absorb huge amounts of data and provide solutions, but it can’t replace the ‘human’ element.

“Ethics and privacy with AI are also huge issues. Patients may have reservations about their data being used in different ways and we’ve seen issues in Australia of data security breaches.

“There’s also the issue of ‘explainability’.

Dentists can rationalise why a clinical decision is being used, whereas an AI algorithm may just make the decision.

This has implications if something went wrong in the treatment being administered.”

Despite its limitations, however, Dr Pethani, who will shortly start an ‘Exploring Applications of AI in Dentistry’ PHD at The University of Sydney, said dental practitioners must accept its growing influence on the profession.

“AI is here to stay,” she said. “I think it will be many years before widespread implementation of AI is possible but I do think dental practitioners must understand the clinical use AI-based applications can have.

“Understanding its implementations and limitations is great, but I’d say don’t be scared. There’s nothing to fear in terms of job takeover.”

Dr Pethani’s study can be read in December’s Australian Dental Journal.

How could AI be applied to dentistry?

The English Oxford Living Dictionary defines Artificial Intelligence as “The theory and development of computer systems able to perform tasks normally requiring human intelligence, such as visual perception, speech recognition, decision-making, and translation between languages.”

Some ways in which AI could be used practically in dentistry (and which pilot studies or research have already been undertaken) are: • AI programs could help improve image interpretation in dental radiology by ‘learning’ to interpret pixelated images more clearly.

• AI algorithms have been used to process different x-ray modalities and help detect possible pathologic lesions.

• AI could assist decision-making treatment in orthodontics; an example is allowing patients to scan their teeth through their phones and orthodontists then analyse these scans.

Shaping the Future of Dental Training

In the era of globalisation of education and digitalisation of the profession, it is of paramount importance to prepare dental students for challenges they will face in their future careers.

Dr Tihana Divnic-Resnik is a senior lecturer and registered dentist at The University of Sydney School of Dentistry, teaching within the Doctor of Dental Medicine program (DMD) and is the course coordinator within the Discipline of Periodontics. She has extensive international experience in education.

Andrew Terry is a lecturer and registered as both a dental therapist and dental hygienist working at The University of Sydney School of Dentistry. He works with both the Bachelor of Oral Health (BOH) and the Doctor of Dental Medicine (DMD) programs since August 2015. His primary focus is in the education of periodontal instrumentation (hand and power driven instruments), oral hygiene instruction and ethical patient care.

With a turbulent year behind us, it is evident that traditional teaching approaches may have limitations when ‘face-to-face teaching’ is not feasible.

A simulated environment is less challenging for students; limiting their creativity to sole repetition of technical tasks.

Structured simulated training is necessary for novice dental students to achieve basic pre-clinical knowledge and a satisfactory level of proficiency in using various dental instruments and techniques prior to starting to treat patients in the clinic.

During previous years, our team endeavoured to implement an innovative concept to simulated training in periodontics to bridge the gaps of traditional teaching approaches.

Blended, Technology Supported Teaching in Simulated Environment To achieve this goal, teaching material was digitalised and packaged as a computer application (PerioPal) accessible to dental students on the tablets in the simulated learning environment.

The application was designed to complement traditional simulation training by encouraging self-directed learning with some guidance provided at the commencement of the session.

The students used the PerioPal app to practice a particular task and were subsequently provided constructive feedback on their understanding of the topic and practical performance.

Does it really work?

The impact of PerioPal application was tested on a cohort of second year dental students at The University of Sydney via an ethics approved study. The results of the study, published in the European Journal of Dental Education, showed a significant improvement in students’ motivation, engagement, and self-directed learning in periodontics1.

One of the most notable qualitative feedback themes from students was that they used the app for preparation for exams and emphasised that remote access would further enable them to review the content and practise at their own pace in their own time.

With the development of the COVID-19 pandemic and an inability to conduct in-classroom training, we faced a new challenge. One of the first questions that we had to answer was how to successfully organise distant teaching and training?

Our aim was to take the training out of the simulation clinic and make it available to all our students who were isolated and even scattered worldwide. The versatility of PerioPal allowed for its fast transformation to an online educational platform, iPerioPal.

What is iPerioPal and How to Access it? iPerioPal is an open access website that can be accessed on www.iperiopal.com. It offers structured training guidance in basic periodontics and periodontal instrumentation. Periodontal instrumentation training concepts are broken down into several categories under two main subheadings for Right- and Lefthanded operators.

The sub-headings encompass the following: 1. Basics of instrumentation principles 2. Operator seating guidelines 3. Hand instrumentation 4. Ultrasonic instrumentation 5. Air abrasive/polishing systems 6. Basic screening and diagnostic principles in periodontics.

The interactive multimedia teaching resource contains diagrams/ images of instruments, explanatory notes related to their design and clinical indications as well as short benchmark videos featuring technique/s for their use.

In addition, quizzes featuring real clinical cases allow for a gradually increasing learning curve leading from basic theoretical and practical concepts of periodontal instrumentation to complex real case scenario-based tasks.

The distinctive attribute that iPerioPal presents is the material customised for practical training of left-handed students who may be sometimes disadvantaged due to lack of customised resources for left-handed clinicians as well as lack of left-handed educators.

An end of year survey conducted at the Sydney Dental School following 2020’s teaching under COVID-19 restrictions indicated that during times of isolation, as an independent resource, iPerioPal enabled continuous learning and preparation for exams.

Students felt like they were part of a worldwide open classroom which reduced stress and improved confidence and performance.

The Road Ahead We are proud that iPerioPal was shortlisted for the Best Educational App Award 2020 by the QS Reimagine Education global competition and conference for worldwide educational innovators.

By now the iPerioPal platform has been accessed by users from more than 100 countries.

Its true potential lies in its adaptability and versatility. Dental students are not the only group that may benefit from its implementation to their learning armamentarium: it can also be used for calibration of educators, continuous education, and training of dental professionals as well as companies that are vendors of products used for dental training.

It is our vision to keep expanding the platform to other dental disciplines and pair it up with specially designed cameras for education that are planned to be tested this year.

I

n a new feature called Meet the Dentist, we chat to the personalities in our profession and learn what makes them tick. This month we speak to Sam Martin, Lead Dentist at Nambucca Dental Surgery, who followed his passion to start a craft brewery. Juggling work commitments with raising a young family, Sam is Director of Great Hops Brewing Family Co. He is loving living in the New England region and splitting time between dentistry, fatherhood and the craft brewery.

What is your career background?

There was not much of a career between finishing my University of New England degree and starting at The University of Sydney - I worked with Lockheed Martin in Uralla due to there being few jobs for an undergraduate science graduate. This was a neat job and I was able to train new ground station operators in my dental school holidays.

You are a biochemist and dentist, how did you become interested in brewing beer?

Graduating in 2007 and travelling up Victoria Road every day from Bondi to Hunters Hill I would pass a home brew store. I kept passing it and then one day went in and bought six small kegs and a stove top brewing kit. I had every Wednesday off back in the day and it turned into brew day.

Brewing almost weekly I had friends over and there was always the dream of doing it.

Part of me wishes I’d just leased a place, filled it full of tanks and started the brewery earlier than I did. My wife Kate and I also travelled to the USA to look at an old brewery I wanted to purchase. I’m glad the brewery is on 40 acres with a rural outlook

now. Moving from our Bondi flat (which had kegs hidden in every cupboard and every bed I could store them) and I wanted space.

I love the New England region and have loved my time growing up here.

How do you juggle time between dentistry, working at the brewery and other commitments such as raising a family?

It’s difficult. I really have been reducing my dentistry hours of late to spend time with the kids. Starting any business, including a dental surgery is always hard on family. I get up early on brew days for a 4.30am start.

Otherwise the brew-day goes on forever. I’ve got great staff. I didn’t build this brewery on my own, a lot of good people came together to help and most of them stayed on.

Which is more demanding: being a dentist or running a brewery?

Both are equally challenging. Beer is an interesting one, there are so many different tastes out there. I’ve been a dentist for longer than a brewer so I’m a lot better at dentistry.

I love the creativity and art of brewing as well as the need for science. There’s a lot of water science behind trying to replicate really good beer. I love not working on someone or having a deadline to the day (other than being home for dinner).

The brewery opened its own tap room in Armidale. How has the town embraced the brewery since it opened?

Armidale loves the brewery. They love the location and the rural atmosphere there.

I think all towns take ownership of their brewery and call it their own. The brewery even features an International Harvester fire engine. My wife bought the fire engine from eBay in 2009 and it came from Tasmania.

The owners were going to get rid of it and I couldn’t let that happen. I drove it on the Spirit of Tasmania and brought it home to NSW and it’s now in the brewery.

What area of dentistry are you most passionate about?

I like all general dentistry (except root canal therapy). I hope in my lifetime there is a radical improvement in RCT technology. I hope I haven’t upset too many specialists there!

How do you strive to be innovative in both your brewery but also as a dentist at your practice?

I think we strive to be efficient in both the dental practice and brewery. I am always looking at new technologies that are backed by evidence.

What does the concept of innovation mean to you in your profession?

Innovation to me is finding something and improving it or doing it as efficiently as we can. I think almost every flavour of craft beers have been done already.

We are not reinventing the wheel – just making great beer.

Variation to the Health Professionals and Support Services Award 2020

A

s part of the 4-yearly review of modern awards, the Fair Work Commission (FWC) has recently made a number of further significant variations to many awards including the Health Professionals and Support Services Award 2020 (HPSS Award).

These variations include the confirmation of casual overtime and changes to the list of common health professionals listed within the HPSS Award.

Please note that the changes referred to above are in addition to the annual increase to award minimum rates on 1 July 2020.

Changes to Casual Overtime Historically, under the HPSS, overtime for casual employees was calculated in substitution of the casual loading. However, a decision at the FWC in November this year clarified the application of overtime rates for casual employees across a large number of awards.

From the first full pay period on or after 20 November 2020 overtime for casual employees is calculated according to the compounding method. That is, overtime is now calculated on the employee’s hourly rate of pay, inclusive of casual loading.

This has resulted in an overall increase in overtime rates for casual employees: • Overtime for the first two hours (time and a half) will increase from 150% to 187.5% of the base rate of pay inclusive of casual loading; • Overtime after two hours (double time) will increase from 200% to 250% of the base rate of pay inclusive of casual loading; and • Overtime worked on a Public Holiday (double time and a half) will increase from 250% to 312.5% of the base rate of pay inclusive of casual loading.

It is very important to note that the HPSS Award prescribes a minimum rate of pay which is to be afforded to casual employees.

Where a casual employee’s contracted rate of pay already exceeds the HPSS overtime rate, then no overtime penalty is payable unless specifically agreed.

Detailed Pay Guides are available through the HR Hub. Given the recent amendments to casual overtime, it is important that practices understand when and how overtime rates will be payable. Overtime penalties will apply in the following circumstances: • Firstly, and most importantly overtime penalty rates only apply to employees who are covered by the HPSS Award.

Award-free employees have no entitlement to overtime penalties; • Any work performed in excess of 10 hours per shift; • Any work performed in excess of 38 hours per week or 76 hours in a fortnight (depending on the roster structure adopted); and • If, on the instruction of the employer, an employee does not receive a minimum break of 10 consecutive hours off duty between shifts when they work overtime. In this situation a casual employee must be paid at a rate of 250% of the minimum hourly rate applicable to their classification and pay point until they are released from duty.

This update to casual wages also serves as a timely reminder that practices should review their roster structures to minimise the circumstances and frequency in which overtime penalties might be payable, as well as to ensure that rates of pay comply with the HPSS Award.

Changes to the list of Common Health Professionals Earlier in December, the FWC announced that it had finalised its review into the list of common health professionals covered by the HPSS. Practices will now need to be aware that the following occupations will be included in Schedule B - List of Common Health Professionals: • Dental Hygienist; • Dental Prosthetist; and • Oral Health Therapist.

These variations will come into effect on 1 July 2021 and will apply to individual employees from the start of the employee’s first full pay period on or after 1 July 2021.

Although is common practice that the above occupations are generally paid in excess of the HPSS Award, the effect of having these occupations now fall under the coverage of the HPSS Award means that practices must be aware of award entitlements and rostering practices which may give rise to the obligation to pay additional penalties/entitlements.

In addition, now that such occupations attract other award monetary entitlements such as weekend penalty rates and annual leave loading, practices must ensure that an employee’s salary adequately compensates for all such entitlements.

HUMAN RESOURCES

We recommend that these remuneration arrangements should be reviewed at least annually to ensure compliance with the award minimum wage increases that typically occur on 1 July. In 2020, the FWC annual wage review decision applied a 1.75% increase to the award minimum rates however another announcement will be made in 2021 on the rate increase.

As always, a bespoke contract of employment is highly recommended and the changes to the HPSS Award presents an excellent opportunity for employees in those professions listed above to be reviewed to ensure that the rostering and salary structures are compliant, but also that a suitable contract of employment is in place to ensure that the remuneration paid is compliant with the HPSS Award or clearly reflective as incorporating all such entitlements.

DR KATE AMOS (NSW) Outstanding Young Dentist Award Dr Amos graduated from the University of Queensland with first class honours before working as a private practitioner in Coffs Harbour. Kate has made a significant contribution to the Association in various roles at both a State and Federal level, and was awarded Australian Young Professional of the Year by Professions Australia in 2015.

Kate has pursued further study to receive a Master of Health Professional Education, and has successfully developed dental education programs for schools in NSW. A great ambassador and leader for the profession, we congratulate Kate on this exciting recognition.

DR DOMINIC AOUAD (NSW) Outstanding Service Award Since graduating in 2009, Dr Aouad has made continuous efforts to contribute to the profession. Dominic is an ADA NSW Board Member and has served as Chair of the Recent Graduates Committee. With Dr Kate Amos he founded and developed online record keeping course Dental Compliance Australia, and is Professional Officer for the Dental Council of NSW. Throughout the COVID-19 pandemic, Dominic facilitated numerous ADA Webinars which have helped guide and navigate the profession through unprecedented challenges. Congratulations Dominic on your well-deserved award.

DR CARMELO BONANNO (ACT) Honorary Life Membership Dr Bonanno has rendered continuous and outstanding service to dentistry and the Association for close to 30 years. Carmelo is a longstanding Federal Councillor and has served on numerous Committees at both State and Federal levels, including ADA NSW President from 2001-2002 and Federal President from 2018-2020. Flexible online professional development tailored to our members As we head into a new year, the NSW CPD team are ready to help members with their professional development needs whatever the future may hold.

Last year ensured innovation was at the forefront of most businesses.

When the concept of face-to-face meetings and events was seriously questioned, and sometimes legislated against, businesses needed to look for new and inventive ways to ensure meetings could be held.

The need to innovate last year prompted many businesses worldwide to devise new ways to engage with their customers, and ADA NSW was no different.

Take the 2019 CPD schedule for example.

Consider the fact that the team held over 190 courses and study group meetings and hosted more than 50 participants at the St Leonards lecture theatre for any given course.

Suddenly in early-2020, the COVID-19 pandemic arrived and it was necessary to reassess. With three months of face-to-face events cancelled, it was important to be able to continue to offer quality CPD to members – but how?

Video conferencing technology has come a long way in the last few years, and now was the time to take advantage.

The CPD team planned a range of informative and entertaining dental presentations to be made available to ADA NSW members and their staff through the local Division and Study Groups.

Last year saw over 60 online lectures with more than 2,300 attending. A great success!

Complaint management, oral hygiene, endodontic challenges, orthodontic treatments, management of dental trauma and anterior composite restorations were just some of the informative topics covered during the run of online lectures.

The presentations were an incredible success and a valuable component in allowing participants to continue their CPD under challenging circumstances.

The overwhelmingly positive responses received from these online lectures made it an

easy decision to look at ways to continue to provide valuable CPD content online into 2021.

The CPD team are proud to announce we will continue to offer a program of online lectures for NSW Divisions and Study Groups.
