Introduction

The Joint Commission of Taiwan (hereinafter referred to as “JCT”) is an organization established in 1999 funding by the Ministry of Health and Welfare, Taiwan Hospital Association, Taiwan Non-Government Hospitals and Clinics Association, and Taiwan Medical Association. With credit and trust earned from society for its honesty, integrity and neutrality in implementing accreditation, certification and onsite inspection activities over time, JCT has qualified to become a professional accrediting organization which is certified by the International Society for Quality in Health Care Ltd. (ISQua). JCT is also committed to promoting quality and patient safety concepts, the use of quality promotion management tools, and healthcare professionals training, establishment of resource sharing platform with medical partners in order to convey exceptional healthcare value, to stimulate institutions in continuous improvement and to jointly promote the health and welfare of the general public. In recent years, with consideration of the advancement of medical technologies, increase of complexity of types of diseases and the insufficient manpower in the medical industry, JCT values the concept of Smart Care currently implemented internationally and observes that various medical institutions have gradually started to utilize information technology to assist the performance of healthcare services in order to increase benefit and safety as well as to reduce errors. Since 2013, JCT has started to launch the “Smart Hospital Outstanding Project Campaign”. In 2014, for the “HQIC”, “Smart Healthcare Category” campaign has been officially established, seeking outstanding and successful projects from medical institutions utilizing information technology in medical practice. The campaign has designed with the concept of Solution, Service, and Smart Hospital, “Industrial Application Group” has further included in 2015 and “Smart Service Group” has also been included in 2018, in light of selecting outstanding healthcare intellectualization implementations from the campaign to provide the benchmarking opportunities for medical institutions. In addition, joint cooperation between technology industry and medical institutions can be encouraged in order to improve the medical performance and quality as well as to create industrial values. The campaign has been implemented for 6 years, and there are 71 institutions, 485 projects, and 4 services participating in the campaign (including medical institutions and information industry operators) up to the present day. The smart healthcare logo and prizes are awarded to outstanding projects and service, and according to the overall award winning performance of hospitals, the logo of “Smart Hospital” is also awarded to the entire institution in order to encourage hospitals in the overall development of intellectualization. Presently, there are 6 medical institutions in the nation that have obtained the logo of “Smart Hospital” for the entire institution. The purpose of the “Smart Healthcare Category” campaign is not only to select those with excellent smart healthcare services but also to demonstrate the MIT health service information solution to the world. In this collective presentation, winning projects of the Healthcare Institution Category of Smart Healthcare Campaign of JCT in 2018 are selected for the demonstration in order to allow industries to observe successful examples related to the integration of technology and innovative smart healthcare technology as well as the applications thereof in the current healthcare services in light of encouraging and stimulating greater outstanding information technology related operators to participate in future healthcare intellectualization. Healthcare services with assistance of information technology to increase efficiency, quality and healthcare value; Innovative technology applications to drive industrial development value.


Background of Healthcare Quality Award

Since 2000, JCT has started to promote the “Healthcare Quality Circle Announcement and Campaign”. At the beginning, the “Healthcare Quality Circle” was the key of the promotion, and the purpose was to encourage the primary healthcare personnel to form teams, to focus on the topic of quality improvement and to use the PDCA management cycle as the basis such that self-inspiration and interactive inspiration among the team members, team works can be utilized along with the use of statistical data and quality management tools in order to perform continuous improvement activities. Based on the concept of incorporating diverse quality management methods for quality improvement, in 2002 (third year), the “Quality Improvement Group” was further included to seek quality improvement projects of non-healthcare quality circle to participate in the campaign. To encourage the medical industry to break through the self-departmentalism confined in its unit or professional field, in 2006 (seventh year), the participation aspects of the campaign were expanded, and the name of the campaign was also changed to “Overall Healthcare Quality Improvement Campaign” along with the promotion of relevant topics of “Integrated Holistic Healthcare’ and “Patient Safety”, “Evidence-Based Medicine”. Furthermore, in 2007 (eighth year), the campaign framework was restructured, and it was divided into three main campaign categories of “Subject Category”, “System Category” and “Evidence-Based Medicine Category”. In 2008 (ninth year), the name of the campaign was changed to “Healthcare Quality Award” with its full name as “Healthcare Quality Improvement Campaign (HQIC)” in light of extending the name of the original campaign and to emphasize the spirits of “continuous improvement”, “management cycle” and “team work” for the improvement of healthcare quality. To encourage small medical institutions, clinics, health centers and long-term care institutions to participate in the campaign, in 2011 (twelfth year), the Subject Category further included the “Community Healthcare Group”. JCT has started to host the “GrossStrait Healthcare Quality Promotional Exchange Campaign” Since 2012 (thirteenth year) in light of encoring greater number of institutions to perform quality improvement and benchmarking through such event in order to achieve the objective of improving the healthcare quality. In 2013 (fourteenth year), to encourage medical institutions in the utilization of simulation trainings to improve the cooperation and communication skills of healthcare team, the “Simulation Scenario Category” was further included. In 2014 (fifteenth year), to improve the utilization of information technology of medical institutions, the “Smart Medical Category” was included as the fifth category of campaign. To encourage new healthcare personnel during training to participate in the quality activities, the “Documentation Verification Newcomer Group” was further included in the EvidenceBased Medicine category, and “Emergency/Critical Care Newcomer Group” was included in the Simulation Scenario Category. To stimulate domestic institutions or units to improve the healthcare quality through innovative researches in order to develop outstanding characteristic healthcare services, since 2016(seventeenth year), the“System Category”has further included the“Outstanding Center Group” and “Outstanding Community Institution Group”. In 2018 (nineteenth year), the “Outstanding Institution Award” was awarded for the first time to medical institutions with outstanding performance in various campaigns in the year. Besides, the “Smart Healthcare Category” further included the “Smart Service Group” to identify the “Service” of utilizing information technology to assist in healthcare operations. -Joint Commission of Taiwan

Next generation genome sequencing (NGS) involves with huge amount of data volume. It is difficult to conduct manpower screening and verification as well as data processing and analyze with traditional interpretation. It takes long time and usually the conclusion is still difficult to be made after several weeks. The system is a self-developed program to assist physicians in interpreting genetic variation remark data after next generation genome sequencing. It can be used to screen data rapidly with pre-set or customized conditions to shorten the scope of data interpretation. The grogram can also look up the gene database to obtain the information whether the variation location is with pathogenicity. With reference of huge amount of literatures, BM25 value is used to represent the correlation between the candidate gene and clinical signs. A potential database of patient disease cause by gene is established so that physicians can screen out gene according to sequencing data with the support of inputting symptom of the disease or possible diagnosis. The system uses text mining comparison technology to list possible genes and diseases. Experience sharing and implementation performance

The program has become the conventional tool used for next generation genome sequencing interpretation by the Department of Medical Genetics in our hospital. With the assistance of the program, our team has successfully established the process of rapid next generation genetic diagnosis. Our team published the research outcome at 2018 Human Genetic Society Spring Conference and then at American College of Medical Genetics and Genomics (ACMG annual meeting) Taiwan Pediatric Association in 2018. Both received great attention and it proves the benefit of the program. Currently, the team still holds conference regularly to continue enhancing and improving the functions of software with the expectation to be able to meet the clinical demand even better. The project result is with highly clinical application value as well as the potential of being promoting it as commercial mode. In the future, we plan to develop the webpage version and convert the program into the online service pattern to provide it for the research units and medical clinics that have demand in genetic analysis as well as establish local genetic analysis database.

Before the program, it usually takes several weeks to decide whether the case is the rare genetic disease patient but still difficult to draw a conclusion. After using the program, the time spent to interpret genetic variation can be substantially reduced and the diagnostic rate of interpretation can be increased in order to enhance the quality of medical service. The positive rate of next generation genome sequencing result is 57% and it is not worse than that in overseas countries of 36-50%. Moreover, the report available time (including time needed for sequencing and interpretation) is 6.4 days and it is better than that done by overseas team of 13-73 days. Currently, the correctness rate of prediction by using text mining can achieve 0.97 and it is a very useful assistive tool clinically.

Medical research has always been one of the important tasks in the medical center. Along with the emphasis on the medical quality and the proposing of the concept of learning health systems, medical and medical quality researches that are driven by data are becoming more and more important. It is quite challenging for medical personnel to balance the research with the busy work. The traditional approach requires browsing medical records and recording information one after another. With the implementation of Personal Information Protection Act, the traditional research method of reading through medical records will face more limitation. The electronic medical records used in our hospital have accumulated huge amount of digital data over a long period of time and new types of data continue to be added in. The utilization of big data not only can speed up medical research but also provide a new vision and new prospective for research. The system and service cover various processes in the clinical research from the estimation of the quantity of study case, screening, long-term tracking, and integration of subject’s clinical information; the system is able to provide assistance in these clinical research processes. Other than R&D on its own, the team also collaborates with Information Office, Medical Research Department and Ethics Committee to establish various big data repositories, system tools and working processes for more than a decade. SNOMED CT is used for all the disease diagnosis, allergy records, drug registration files, and pathological diagnosis in the electronic medical records at our hospital. 1.Limitation of clinical use of ICD9/10 CM classification: It is for classification purpose with limited vocabulary. For those are not commonly seen, they will be put into “Other specified diseases�. It is lack of precision and is insufficient for clinical use and research purpose. 2.SNOMED input is applicable to the diagnosis in outpatient, hospitalization and emergency appointment. It will also be automatically

corresponded to ICD-9/10 for the purpose of declaration. The system provides data for retrospective study, instrument interfacing, and data acquisition, such as respirators. It had been promoted after the successful research in quality control circle. Initial achievement in the integration of genetic precision medicine research and electronic medical records has been revealed. Various artificial intelligence analytical tools for big data are under exploration and development. There are still a lot of aspects for application in the future and it is anticipated to have further progress with dedicated organizations, tasks, and personnel in place. The R&D, management, and service provision done by the dedicated personnel with the interdisciplinary team is the most efficient method of providing and maintaining high-quality data. Meanwhile, it offers the excellent innovative environment in the hospital and it is very valuable for promoting it. The experience of big data repository establishment in our hospital can be provided to other hospitals for reference. Research driven by data is an important part of learning health systems. Even though the various tasks developed currently have strengthened the efficiency in providing and analyzing data, there are still a lot of important data to be collected and integrated in the future as well as there will be continuous sources of new data adding in, such as data related to gene, mobile health and internet of things. The continuous applying and developing various tools of big data, such as machine learning, natural language and AI analytical tools, and expanding big data repository are the efficient tools and key points of development for future researches. 1.Database inquiry application and achievement: The Clinical Information Research and Development Center has appointed dedicated personnel to be in charge of information-providing service since 2009. The total application numbers accumulated until December, 2018 were 3,021. There were 953 applications for academic research and accounted for 31.5% of total application numbers. The accumulated successful publication was 133 articles with SCI in 67 articles. 2.Research for quality control: It is to assist quality control circle and Symbol of National Quality (SNQ) and the number of cases assisted from 2013 to 2018 were 65. Among them, 18 cases were involved with awards and certification marks. 3.Special algorithm and key management scheme are used to process patients’ data that can be directly identified. Different coding will be generated for different users in order to comply with Human Subjects Research Act and Personal Information Protection Act. It will be provided to researchers to access for data identification in order to offer research data with higher quality and integration.

Our hospital established “Veterans General Hospital- Taichung Branch” on July 1st, 1982 after being approved by Executive Yuan. Medical operation was unfolded on September 16th and was restructured and upgraded to “Taichung Veterans General Hospital” on July 11th, 1988. Since the establishment, the hospital was under the outstanding leadership and supervision by Mr. Chao Chu-Yu at the council and the predecessor and successor of chairman as well as the careful planning and full engagement from Mr. Lo Kuang-Jui and other superintendents. In 1984, it passed hospital and teaching hospital assessment and was rated “Tier 1 Teaching Hospital”. In 1988, it was rated as “Would-be Medical Center” and “Medical Center and Class I Teaching Hospital” in the next consecutive 5 assessments. The medical quality and technology are fully recognized. The hospital also passed “New System Hospital Accreditation Scheme and Teaching Hospital Assessment “ in 2007 and 2011. The hospitalization focus on acute and critical care to provide a safe, warm, and high-quality medical service to the public. The home exercise training with virtual coaching constructed by the combination with smart textile is the collaboration between our hospital and Taiwan Textile Research Institute. The application of human subject research for the project was approved in 2017, the reliability and validity of heart rate monitor sensor for the smart textile was then completed to confirm the safety and the availability of good reliability and validity, and the global pioneer smart medical system (APP) with the functions of home exercise coaching and heart rate monitoring is created. The clinical random test on patients with chronic diseases in the community was carried out between 2017 and 2018. Up to now, other than continuing recruit case subjects, the project is able to use the same pattern to the patient group with multiple chronic diseases all over the county through the preliminary feasible clinical test. Besides, in order to face the era of super-aged society in our country, the smart exercise training and monitoring pattern for the patients with smart multiple chronic diseases

In responding to the health care demand of the senior citizens, our institute works with Tri-Service General Hospital to develop cardiac rehabilitation supporting smart textile system. Fabric-based ECG electrode in medical equipment level is developed to combine with sport functional fabric and clothing pattern creating wearable ECG electrode smart clothing system. The integration of Bluetooth heart rate monitoring module and “health prescription” APP allows patients achieve the effectiveness of health promotion through home exercise. Health Prescription APP is an exercise training voice feedback system “health prescription” developed with Department of Physical Medicine and Rehabilitation in Tri-Service General Hospital. After evaluated by the physiatrist in the hospital, the patient can design the training procedure in APP. Through the dynamic monitoring of exercise strength on the smart textile and the verification and analysis of the exercise procedure, the feedback on training strength will be provided to the patient in real time to achieve the purpose of precision exercise.

1. Smart textile system almost has no difference in the tolerance of hospital system (standard), and it is with good validity. 2. The comparison between using smart textile and doing exercise at home reveals the physiological index of exercise- cardiovascular fitness (VO2), load wattage (work), pulmonary function index (FEV1/FVC), and the maximal heart rate of exercise test are significantly advanced. In addition, the walking activity levels every week as well as the medium strength and high strength physical activity levels all present outstanding improvement. Life quality is improved as well. 3. The real-time feedback and safety monitoring efficiency for the case management are enhanced to achieve seamless medical care as well as understand the insufficient strength of home exercise training implemented by patients with multiple chronic diseases in the real practice. 1. Tri-Service General Hospital is the only medical center among armed forces hospitals. The hospital includes Neihu main hospital, Tingzhou branch, Keelung branch, Penghu branch, and armed forces outpatient center. Except Penghu branch, the locations of each branch is in the capital in Taiwan, Taipei City and New Taipei City. It is with easy access link by land, sea and air as well as is close to Neihu and Nangang science and technology parks. In order to enhance the learning effects on students and meet the actual situation, scenario simulation teaching is used for students to experience the simulated clinical care situation through interactive learning during their studying. Through the interactive and instructor-aided response system applied to various procedures, it will be able to reach double results with only efforts. The main target in our hospital is to introduce the teaching material to other departments afterwards through the successful establishment of the teaching material so that the learning will be able to achieve horizontal expansion continuously. Our hospital selects “the first-aid treatment to patients with multiple traumas� as the main implementation area based on the consistence and availability. Through user-friendly interface and powerful VR editor, we developed a set of MAGIC VR editor. The editor is able to provide 3D basic model material database, storyboard editing function, 3D material editing function, and interactive setting function as well as to output and support VR head-mounted device. The project is implemented step by step through step 1: demand analysis of virtual reality and cloud service; step 2: MAGIC VR editor design and development as well as model establishment of 3D material database; step 3: demonstration and course presentation; step 4: launched formally and establishment of experience camp.

System utilization analysis should be carried out after the completion of system establishment: (1)Lesson plan feasibility investigation: Its level of importance and testability should both achieve more than 4 points. (full mark: 5 points) (2)Experience satisfaction from lesson plan writer, students, and teachers: After the experience, the items of facility utilization, lesson plan design, and scene design must be responded by more than 80% of participants with the feedback of more than satisfied. (3)Learning satisfaction from students and teachers: According to the result of survey, the establishment of the system enables students to increase knowledge, enjoy learning, reduce learning difficulty, and be willing to share experience with other people. After being experienced by examiners, they all provide highly positive feedback towards the increase of the pleasure in teaching and the reduction of loading in teaching. (4)It not only can reduce the efforts and costs spent on producing the content of virtual reality but also be able to output a great deal of the VR content through rapid reproduction. 2. It eases teachers’ teaching pressure invisibly, reduces students’ mental stress in dealing with actual patients in the future, and enables continuous development on the system without limitation. 3. Our hospital was formally opened on October 31st, 1990 as “Taipei Veterans General Hospital, Kaohsiung Branch” and was restructured and upgraded as “Kaohsiung Veterans General Hospital” on July 1st, 1993. For a long time, IT information often leaves people an impression of cold and boring and the design angle usually is away from human nature in medical care. Chung Shan Medical University Hospital started to promote electronic medical records in 2009 to upgrade and renovate information and input human warmth. The cold and boring IT medical care is returned to human nature and concern by integrating cross-discipline talents with both medical and IT professionalism, and we developed “patient-focused method” (PFM) electronic medical record on our own. The so-called patient-centered is to think from patients’ points of view and re-build up electronic medical record system for patients’ diagnosis records; that is patients’ “diagnosis diary”. All the processes from the patient entering the hospital are actually recorded, including any critical examination, surgery, special treatment, and the illness explanation carried out to the patient or family members. It is guided by images to reduce physician-patient gaps. In the whole, it is established based on visualized electronic medical records and PFM concepts to allow new medical information system combining medical practice with human nature efficiently. Chung Shan Medical University Hospital establishes smart wards to integrate various information and communication platforms through the installation of clinical information system. It can also simplify operational process, advance working efficiency, and effectively improve the information communication among physicians, nurses and patients in order to provide an excellent care with the priority of patient safety. The Superintendent in our hospital invites Department of Medical Care, Information Office, and Department of Nursing to form a cross-functional smart ward task force to carry out regular meeting with information supplier. The system planning, establishment, discussion, and optimization are implemented according to the clinical demands. Smart ward system is positioned as equipment that supports clinical practice done by medical personnel, and each piece of equipment has its specific software function. Three key functions are innovated and developed, including exclusive nursing function, digital room card, and bed clearance upon discharged. Other than maintaining patient safety and enhancing quality of medical care, the purpose is to make the clinical practice process done by medical personnel simple, shorten working hours, and save tangible and intangible costs. Experience sharing and implementation performance

The software development for smart ward requires sustainable development. It relies on nursing and information teams to follow the new policies regulated by the hospital or government and the conventional process after empirical research to actively build up smart functions. For example, the physician-patient shared decision making (SDM) platform vigorously promoted by Ministry of Health and Welfare and we implement it by displaying digital information through patient information terminal system. Other than avoiding repeat transcription by medical and nursing personnel or the wrong transcription on patient information and medical information, the interfacing of medical information system in our hospital and smart ward information also innovates four items and eight categories of mobile nursing function, digital room card and bed clearance upon patients being discharged. The system can also automatically produce statistical reports for eight key items to advance management effectiveness. The specific effect after launching smart ward is the incidence of falls at six units reduced from 0.14% to 0.09%. The reporting function of care message shortens the resolving time of nursing demand from 10 minutes to 5.4 minutes and the time of contacting nursing personnel is shortened from average 6.6 minutes to 0 minute. Each unit can reduce around NT$10,200 in the cost of general affairs every month in average while the estimated cost saved on time and manpower is around NT$43,000. The satisfaction towards smart ward system optimization from nursing personnel is increased from 51% to 80.36% while the average satisfaction on hospitalization service from inpatients achieves 95.75%. The function of bed clearance upon discharged shortens the time needed for cleaning and arranging bed from 30 minutes to 22 minutes.

Medical service has moved towards specialization due to the enhancement of quality and the electronization of medical records further spread out the care records done by the medical team and cause the failure of providing complete and continuous holistic health care. 1. Since the launch of the holistic medicine smart care system for the 2.5 years, holistic meetings has been held for 378 cases (around 10 cases/ month). Demands from patients are put as priority to provide holistic care. Among them, the holistic meetings with participants from at least 4 types of medical personnel were around 90%. Patients/ family members were also invited to the meeting for shared decision making (SDM) with participation rate of 74% and satisfaction rate of 93%. 2. With the joint discussion of personnel from multiple positions at the holistic meeting, the multiple concerns from patients or family members can be solved (83.7% medicine, 86.2% nutrition, 65% medication, 52.2% after care/ arrangement, 38.4% rehabilitation, 34% mental support, and 15.3% hospice discussion). It achieved the reduction of days of hospitalization and avoided medical dispute. 3. Convening rate of holistic meeting for acute stroke achieving the holistic assessment standard: Before introducing the system, there was no team meeting held. After introduction, the convening rate of the meeting was increased to 87.5%. The team uses the designed stoke PAC decision-making aided tool to explain to patients/ family members for whether to carry out SDM down referral and the hospital referred and its execution rate was increased to 94%. The down referral rate of patients meet PAC indications was increase from 42.2% to 65%.

The main target of the system is to construct “nursing home management system” that links the nursing records between medical care and nursing home. The integration care will be linked to the long-term care institution when the discharging planing in the medical system starts. Green channel is established between the hospital and the nursing home to link operation flow and information flow and to share the care record done in the interdisciplinary team (physician, nurse, pharmacist, dietician, physiatrist, social worker, and carer) in order to provide one-stop service and seamless connect front-end medical care and rear-end continuity of care together. Through the practical clinical application of the system, we found that “medical and care information need to be effectively integrated in order to actually provide continuity of care”. The residents in the nursing home usually have multiple diseases and complicated illness record, and they are often hospitalized due to the complication of multiple diseases and acute symptom during the care process. New demand of nursing often occurs during hospitalization due to function deterioration, and patients will have to travel between the hospital and the nursing home frequently. When patients visit the doctor in the hospital, physicians need to understand the care records and suggestions from other professional team (such as nurse, dietician, psychologist, physiatrist, social work, and pharmacist) in the nursing home. When patients receive care in the nursing home, healthcare information often needs to be integrated and obtains the care suggestions from physicians due to the complicated medical records. Therefore, the information of acute medical care and the institutional longterm care information in the nursing home must be seamlessly integrated and connected before the cross-institution continuity of care can be provided.

Due to the number of outpatients in our hospital every day, the public usually spend a lot of time finding a parking space and the location of the clinic during the huge parking demand at the peak hours and because of the factor of spreading-out buildings in the hospital. Therefore, the improvement on the above issues becomes the key matter that our hospital has to deal with. The project focuses on various services and approaches centered on patients to propose smart solutions. Smart car finding guidance system and 3D panoramic image indoor navigation system are established to introduce novel parking management system through the technology integration of combining smart phone and the facility of intuitive direction-giving machine. The traffic and parking problems in the hospital are improved by applying modern technology and implementing smart guidance in order to shorten the time needed for parking and looking for the clinic and provide higher quality service to the public. Smart guidance system includes : 1. S etting up the function of “panoramic image navigation inside the hospital� in World Wide Web : The public will be able to activate the webpage of VR guidance on the official website of the hospital to carry out the browse of the real scenes of indoor environment, passageway, floor and route in our hospital. 2. I ntroducing new license plate recognition and parking space detection system : Replacing the old system with license plate recognition system at each entrance and exit and establishing parking space detection and display system to help the public with quick accessing and parking. 3. Through simple operation, the public will be able to obtain the relevant information of route guidance to the destination to reduce the time wasted in way finding. 5. Through APP, the public will be guided to the destination in the hospital as well as inquire relevant services.

Due to the huge number of the public visiting the hospital every day and the huge demand of parking and way finding guidance in our hospital, the project has generated improvement effectiveness towards the heavy traffic during the peak hours in the hospital and the public guidance through introduction of technology equipment and the utilization of big data. In the future, we anticipate in continuing improvement based on the current performance to fulfill the vision of smart parking lot in order to provide a quicker and more convenient service to the public. Saving the time of vehicle accessing to our hospital : The completion of recognition for each vehicle takes around 0.3 seconds (original old version of license plate recognition system requires around 2 to 3 seconds) and it is with 85% reduction. 2. Saving the time used for finding a parking space and parking : The average time needed for the public visiting our hospital at the peak hours from entering the hospital area to completing parking is around 40 minutes/ car. After introducing license recognition system, parking space detection system and parking lot smart guidance APP, the average time needed for the completion of parking at the peak hours is reduced to 15 to 20 minutes/ car with 50% reduction. Other than shortening the parking time, saving parking fee, and enhancing satisfaction, it also achieves energy conservation and carbon reduction.

Our hospital established “Veterans General Hospital- Taichung Branch” on July 1st, 1982 after being approved by Executive Yuan. Medical operation was unfolded on September 16th and was restructured and upgraded to “Taichung Veterans General Hospital” on July 11th, 1988. The hospital successfully developed the first dental technician fee order system in Taiwan and it added another application area to the smart healthcare. Through the technician fee order is launched on line, it links the hospital with the dental technology institute, chains the operation between dentists and dental physician as well as connects order and cashier systems. Dentists log in “physician fee order system” to create a new order after intra-oral impression on the patient’s mouth according to the patient’s chief complaint, oral hygiene and care demand and complete the selections of the prosthesis item, dental notation and material as well as select the expected delivery time before printing out the barcode and paste it on the patient’s tooth model. The system will automatically send an email to inform dental technology institute. The dental technology institute should log into the system from the dedicated area for supplier on the webpage of Kaohsiung Medical University and click confirmation to receive the order before visiting the hospital to collect the tooth model and complete the production of prosthesis and deliver it back to the hospital. Dentists should then verify whether it is consistent with the order and click receiving confirmation if there is no error. The status of the order will show the completion of goods-receiving of the order. The dental technology institute should add the prosthesis item and its price on the order. After being verified by dentists, the technician fee order will be transferred to cashier system to print out payment slip and give it to the patient. The order is completed and ended.

It was launched formally in January, 2018 and paper-based physician instruction sheet was fully suspended. 2. The progress of order can be monitored any time to facilitate appointment making in order to reduce patients’tiredness caused by long journey. Especial for patients with special demands, it often is involved with other people and the intangible manpower and society costs are increased comparatively. After confirming the prosthesis delivered by the dental technology institute is correct, patients will be able to try it and make necessary adjustment immediately when they visit the hospital. The number of time visiting hospital can be reduced and medical quality and patient satisfaction are enhanced. It is very important to use QR code to reinforce the identification of the identity of the prosthesis. In the future, the hospital will be able to recall and show the care to the patient’s status if there is any adverse reaction that the patient has towards the prosthesis. 3. Management efficiency optimization: The administrator should export order summary report to verify the prosthesis production progress for each dentist. The manual data verification one by one in the past took time and manpower and it saves time and manpower to change it to auditing on line. Compared the cashier and fee application 3 months before and after the physician fee order system, all orders are transferred to cashier to ensure no missing bill after the completion of prosthesis production when the system is fully launched on line. 4. Prosthesis traceability suggestion: Each prosthesis can be traced back for its production; for example, patient A’s dental notation 46 was produced by dental technology institute B with all ceramic dental crown, palladium was embedded on the occlusal surface and A1+A2 color matching. Kaohsiung Medical University Chung-Ho Memorial Hospital was established in 1957, and it was the first private medical university hospital. The REDCap system that is introduced to our hospital is with friendly interface and excellent flexibility. Users with no programing design background can design and establish the information project that meets demand after educational training. REDCap is generally used for case collection in the clinical research. Our hospital is innovative and applies it to clinical care, patient service and administrative management to reduce manpower operation and share real-time data in order to enhance the quality of medical care. After obtaining the licensing of REDCap, our hospital was the pioneer in the country to implement all Chinese character in the system and the instruction of more than 8,000 words were translated into Chinese. Through sound information security measures, the Medical Record Information Management Office invites the Department of Internal Medicine and the Information Room to conduct the comprehensive smart applications of the system as below: 1. Clinical care application: (1) Heart failure integrated care: The seamless service provided by the interdisciplinary team includes various evaluation, tracking, document forms, and medical record production. (2) Intensive care unit quality management: Cloud data collect facilitates realtime evaluation and quality control data sharing. 2. Patient service application: Establishing friendly interface for online application of medical record, strengthening management of operational procedure, and tracing the application are carried out to save the waiting time for patients. 3. Combining with daily medical equipment management operation at the unit, our hospital is the first to have online inquiry of back-up machine information and online registration, nursing handover list, instrument function test result uploading for file saving for future reference, automatic reminder of production suspension notice, automatic reminder of replacement suggestion, daily and monthly reports management, and contractor business maintenance record transmission. It significantly saves the time for personnel management. Before introducing the APP, the medical supplies routine count in the nursing unit, application for borrowing back-up machine, and the primary maintenance were still conducted by paper or phone call. It not only wastes paper and difficult to be stored but also takes a lot of time. On the other hand, the completion rate of online paperless application for repairing request has achieved nearly 100% but it is limited on the hardware of personal computer and the number of network causing the limitation of the convenience of the operational system. After introducing the APP, the function of mobile network was added. Clinical medical and nursing personnel use smart phone to scan QR code on the medical equipment to enter the operation mode, and they can select the service needed. It reduces the time used on administrative affairs and furthermore the records can be stored completely for future reference. It keeps complete records for paperless daily operation, real-time status of instrument, online emergency back-up request, common components inventory management, and medical equipment management.

Inventory management for common materials and it can be requested on line and carried out real-time inventory maintenance in order to reduce material waiting time, enhance the reliability of medical equipment, and increase utilization effectiveness. 5. The function of daily report and monthly report provides managers daily and monthly medical equipment maintenance and repairing costs at each unit, utilization rate, status of machine lending and backing up, production suspension of the asset, and replacement suggestion in order to easily control the current status of medical equipment. 6. Contract maintenance and custody management for medical equipment in the acute and critical units. Our hospital is the only Medical Center in New Taipei City and is responsible for acute and critical care. Currently, there are 64 divisions and 2 centers in our hospital, including Cardiovascular Center, Division of General Medicine, Division of General Surgery, Department of Gynecology, Department of Pediatrics, and other medical divisions. We have 1,383 beds, more than 6,500 outpatients for daily services, around 400 patients are treated in the emergency room that is ranked fourth in the country, and the hospital bed occupation rate is maintained over 85%. Each operation has its standard operational procedure, and ISO9001 quality system had been introduced to the hospital early back to 2004. We set up quality target each year and have passed five certifications for disease care quality. We are the leading hospital in north part of Taiwan that has the most certification number. Other than medical service, our hospital encourages research and we have established 3 collaborated research rooms, 1 animal laboratory, and 24-hour e-library. 2019 Appalication Of Smart Medical Technology

Given that the golden window from the arrival of ambulance to the diagnosis and treatment after arriving at the hospital for patients with severe cardiovascular blockage is precious and every second counts, the system aims to assist first-aid team to transmit the patient’s 12-led ECG and relevant physiological data to the emergency room in the evacuation hospital before arriving at the hospital. The emergency medicine specialist will be able to judge the status of the patient, and the cardiac team will be activated before the arrival if the diagnosis is confirmed. It can effectively reduce the death rate of patients with myocardial infarction and further enhance the prognosis quality. It integrates first-aid systems at disaster command center in Fire Department, Taipei City Government, 12-lead ECG in the ambulance, and emergency room in the evacuation hospital. 1. App can transmit/receive patient’s basic information and the 12-lead ECG operated by manual external defibrillator. 2. It can transmit the specific physiological data to the emergency room in the evacuation hospital, and it can be operated and used through computer or mobile phone. 3. P hysiological signal data should be able to be transmitted to current electronic medical care record as the attachment and be consolidated for saving. It is centered with Fire Department to integrate the real-time emergency bed occupancy and availability in many evacuation hospitals to enhance the time efficiency.
