The new transgingival implant design is featured with a conically convergent component of 1,9 mm in height in continuity with the one-piece implant body (‘preabutment’). This extended Vertical Neck® due to its peculiar shape and length is supposed to provide a suitable space for the establishment, accomodation, and coronal maturation of periimplant soft tissues (Biologic Width). Furthermore, the Microgap positioned more than 2 mm coronal to the crest of the bone, may justify why no crestal bone resorption was noted as it appears in the presented case documentation assuming that the bacterial load is at least in part responsible for the bone crest resorption. The conical design of the preabutment/abutment assembly allows to define and adjust an anatomically customized and scalloped crown margin/emergence profile (Guided Tissue Shaping [GTS]) within the periimplant sulcus according to the most pleasing esthetics at tissue maturation stage through a provisional and stabilized through the definitive crown thereafter. Thus, a metal margin and/or abutment exposure, being one of the major esthetic disadvantages of conventional implant approaches can be prevented/corrected. An appropriate prosthetic emergence profile in combination with a peculiar transgingival conical implant component seem to have the potential to ideally shape and maintain periimplant soft tissue contours (Guided Tissue Shaping [GTS]) resulting in a stable “close-to-ideal” esthetics.
