THE PROGRAM LAYOUT
Step 1
Refer onto one of our programmes via the Refer-All platform. Some long-term health conditions allow for self-referral, other conditions may require referral by a medical practitioner.
Step 2
Referral requests are delivered to our Wellbeing Team, who will be in touch to book a face-to-face consultation.
Step 3
During the consultation, members of the Wellbeing Team will allocate 20 to 30 minutes to discuss the patient’s needs and goals, whilst collecting baseline data and measurements.
Step 4
An appointment will be booked for the patient to attend the first session of their program.
Step 5 – Sessions consist of:
• Program Length – 12 Weeks Tailored exercise plans to meet individual needs.
• Program Frequency – Twice a Week.
• Program Duration – Up to One Hour.
• Delivery - L4 Instructor qualified in the specific long-term condition.
• Delivery – Small groups of 10-12 patients, excluding family members or carers.
• Sessions held at Blackshots Leisure Centre in Grays, Corringham Leisure Centre in Corringham and Belhus Park Leisure Centre in South Ockendon.
Step 6
A final appointment will be arranged, where final data and measurements are taken.
Step 7
The Wellbeing Team will assess the findings from the patient’s final appointment.
The Wellbeing Team may refer a patient back into the program for a second term, offer a discounted membership to the leisure centre or refer the patient to an external community program.
LONG-TERM NEUROLOGICAL CONDITIONS
Our programme covers most common neurological conditions, acquired brain injury, spinal cord injury, cerebral palsy, multiple sclerosis, Parkinson’s disease, motor neurone disease and neurological conditions.
3% of Exercise programme referrals were for Parkinsons alone.
Referral Criteria:
• Must have a neurological condition.
• Referral by medical professional via our platform.
Features:
• Led by L4 Exercise for Long Term Neurological conditions instructor.
• Tailored exercise plans for individual’s needs.
• Small group sessions.
• Carers are welcome if necessary.
CARDIAC REHABILITATION
Designed for individuals’ post-cardiac events (heart attack, stent, bypass surgery).
7% of Exercise programme referrals were for Cardiac Rehabilitation.
Referral Criteria:
• Patients post-phase 3 cardiac rehab.
• Referral by medical professional via our platform.
Features:
• Led by L4 Cardiac Rehabilitation Instructor.
• Tailored exercise plans for individual’s needs.
• Small group sessions.
• Carers are welcome if necessary.

STROKE REHABILITATION
Our stroke rehabilitation programme focuses on improving mobility and strength for stroke survivors, accommodating varying stages of disability.
7% of Exercise programme referrals were for Stroke Rehabilitation.
Referral Criteria:
• If a patient is unable to transfer from their wheelchair, they will need to bring a carer to support with wheelchair transfer.
• Referral by medical professional via our platform.

• Led by L4 Neurological condition instructor.
• Tailored exercise plans for individual’s needs.
• Small group sessions.
• Carers are welcome if necessary.
PULMONARY, RESPIRATORY COPD AND LONG COVID REHABILITATION
Designed for individuals with pulmonary, respiratory COPD, long COVID conditions, our programme helps improve respiratory health through structured exercise.
Our Exercise programmes received 477 referrals. 240 of which were for Long Term Conditions such as Long COVID and 6% of Exercise programme referrals were for Respiratory COPD.
Referral Criteria:
• Referral by medical professional via our platform.
Features:
• Led by L4 Pulmonary Rehabilitation instructor.
Tailored exercise plans for individual’s needs. Small group sessions. Carers are welcome if necessary.
CANCER PREHABILITATION & REHABILITATION
Our community-based program sessions cater to various stages, focusing on both physical and emotional recovery.

Referral Criteria:
• Prehabilitation and Rehabilitation.
• Curative or palliative care.
• Referral by medical professional via our platform.
Features:
• Small group sessions led by Level 4 Cancer Rehab Instructors.
• Tailored exercise plans for individual’s needs.
• Small group sessions.
• Carers are welcome if necessary.
Patients accessing the program reported:
1. Increased energy levels.
2. Improved pain levels.
3. Better sleep.
4. Increased enjoyment in life.
5. Improved quality of life.
Satellite Programme:
• Macmillan Coffee Mornings: Held once a month and open to everyone in the community to share experiences, provide support and promote social interaction.
DIABETES MANAGEMENT
Our diabetes management programme supports individuals with Type 1* and Type 2** diabetes through structured exercise and education.
Referral Criteria:
• Type 1: Insulin-dependent individuals.
• Type 2: Diet or medication controlled.
• Referral by medical professional via our platform.
Features:
• Led by a L4 Obesity & Diabetes qualified instructor.
• Blood glucose monitoring before exercise.
• Tailored exercise plans to manage diabetes effectively.
• Small group sessions.
Features:
• Led by a L4 Obesity & Diabetes qualified instructor.

* For Type 1: Blood BGL must be below 15 mmo1 / L to exercise.
**For Type 2: Blood sugars below 7 should not exercise.
10% of Exercise programme referrals were for Diabetes Management.
WEIGHT MANAGEMENT
Our weight management programmes, support individuals in achieving healthy weight through structured exercise and education.
”The weight management classes at Blackshots centre help you lose weight and keep fit in a friendly environment. I feel better physically and mentally, I have more energy and sleep better.” - Chris.
Referral Criteria:
• Must have BMI of between 25 & 39.9.
• Referral by medical professional or selfreferral via our platform.
Features:
• Small group sessions led by Level 3 personal Trainer with L3 Exercise on Referral.
• Tailored exercise plans for individual’s needs.
• Small group sessions.
198 people lost weight on the program. 37 people experienced more than 5% body weight loss.
Satellite Programme:
• Weight Management – Aqua Aerobics: 12-week aqua instructor-led programme with healthy eating advice and weight monitoring.
50% of all Exercise programme referrals were for Weight Management.
HYPERTENSION MANAGEMENT
Our hypertension management programme supports individuals who have been diagnosed with hypertension and are controlling their blood pressure via medication, through structured exercise and education.
Referral Criteria:
• Blood Pressure must be controlled via medication.
• Referral by medical professional or selfreferral via our platform.
Features:
• Small group sessions led by Level 3 Personal Trainer with L3 Exercise on Referral.
• Tailored exercise plans for individual’s needs.
• Small group sessions.
MUSCULAR-SKELETAL MANAGEMENT / PAIN MANAGEMENT
Designed for all joint or conditions that effect mobility, including arthritis, back pain, tendonitis, osteoporosis, and fibromyalgia.
Referral Criteria:
• Referral by medical professional or self-referral via our platform.
Features:
• Session delivered by L3 Personal Trainer with L3 Exercise on Referral.
• Small group sessions.
• Tailored exercise plans to meet individual’s needs.
11% of Exercise programme referrals were for Back Care (MSK).

MENTAL HEALTH SUPPORT
Our mental health programmes, including a community garden at Belhus Park, support individuals with anxiety, low mood, and depression through structured activities and social interaction.

• Small group sessions focusing on mental health improvement.
• Social activities to promote well-being.
• Referral by medical professional via our platform.
Satellite Programme:
• Community Garden: created at Belhus Park, the garden is maintained and managed by local volunteers, promoting an active lifestyle and social interaction.
SUMMARY
Both the Exercise Referral and Cancer Pre/Rehabilitation programmes have demonstrated substantial results and a great need for improving participants’ health and overall well-being.
The continuation and expansion of these services are critical for maintaining the positive momentum experienced by the communities they serve.
