Let’s face it...

Everyone wants to know more about the dentist they are considering. While personal recommendations are certainly valuable, most people want to know about a dentist’s credentials and experience as well. That’s were NJ Top Dentists comes in. “Patients essentially meet their dentists on our site NJTopDentists.com before they even schedule their appointments,” says Cristina Mendez, Director of Marketing & Client Relations. “Our website answers the most frequently asked patient questions, all in one convenient resource.” To be featured in the magazine and/or profiled on the site, each dentist must qualify and is extensively screened and approved. These providers were chosen based on a number of criteria, including but not limited to: • Verified dental licenses & credentials • Years in practice • Board certification review • Review of malpractice insurance • Continuing education requirements • Patient reviews

“Peer to peer selection is not used in our process. Each dentist featured has earned his or her approval based on merit,” says Ms. Mendez. NJ Top Docs & NJ Top Dentists have now expanded their total healthcare resource approach to include a series of 4 magazines (Top Dentists, Top Surgeons, Top Doctors & Living Well) that will serve as annual resource guides delivered to homes across the state. “For the last 7 years we have been featuring top healthcare providers in New Jersey online and we are ecstatic to be now delivering our total healthcare resources direct to homes,” says Ms. Mendez. So take a look for yourself. While all the dentists listed in the approved directory have passed NJ Top Dentists rigorous review, many of the providers also have an advertorial in the magazine and/or a full page profile online. Profiles detail their education, training, services provided, hours of operation, insurance information and more invaluable information. NJ Top Dentists is a division of NJ Top Docs, a comprehensive information resource of Top Doctors, Dentists & Hospitals that profiles over 900 healthcare providers in NJ and makes it convenient for you to find them. Aesthetic Smiles of New Jersey From the moment you meet him, Dr. Edward Romano’s enthusiasm and passion for what he does is undeniable.

T

he graduate of the Georgetown University School of Dentistry says that improved smiles are often the catalyst to other positive changes. He’s seen a number of his patients gain a renewed sense of confidence, shed excess weight and become more active and upbeat. Dr. Romano was at the forefront of cosmetic dentistry when it was first making inroads in the 1980s and has

been featured in numerous publications and television shows such as NBC’s Dateline for a story on the use of lasers. He’ s also a national speaker who travels coast to coast to prestigious events and learning institutions to lecture on topics related to aesthetic dentistry and methods for running an efficient office. Dr. Romano is an instructor at Morristown Memorial Hospital as well.

General Dentistry

He points out that cosmetic dental techniques have changed considerably since his early days. “In the 1980s, we had to be more aggressive, but now we can get incredible results with minor enamel smoothing of a tooth and super-thin veneers. I’m also not married to a particular product and choose what’s needed for each patient—sometimes that can vary even from tooth to tooth.” Aesthetic Smiles of New Jersey is a state-of-the-art, full-service general and cosmetic practice that prides itself on its high levels of customer service. The office team goes the extra mile to optimize insurance benefits and work out payment plans and other financial arrangements if needed. As part of his practice philosophy, Dr. Romano and his partner Dr. Vaccaro do not dictate treatment. Even if a patient has a preformed opinion about what they think they need,

“People often ask me why I chose to become a dentist. I believe that a smile is the gateway to many other aspects of life. There’s nothing more rewarding than seeing the response of patients after they’ve had an aesthetic dental makeover.” – DR. EDWARD ROMANO

he always explains and educates them about the various treatment options available so the patient has plausible choices and can make educated decisions about what treatment is best for them. Dr. Romano doesn’t like it when he’s kept waiting for an appointment

and doesn’t believe anyone should be kept waiting for him. If an emergency arises, the patient is called, emailed and/or texted to reschedule. Any such changes are accompanied by some sort of “compensation” such as a pass to the movies. Because he values his patients’ time, they do the same and call if they’re going to be any more than 15 minutes late (which can have an unpleasant trickle-down effect on later appointments if not managed properly). More than anything, it’s Dr. Romano’s unparalleled expertise that attracts so many of his patients. One of his trademark treatments is the combination of Invisalign and his “6-month Smile” program that achieves a beautiful, natural smile in the shortest amount of time. He also has a plastic surgeon and registered nurse on staff for those who ask to take the next step in the makeover process.

r. Walter graduated from Fairleigh Dickinson University with a dental degree and completed his residency at St. Joseph’s Regional Medical Center in Paterson where he gained considerable experience treating patients of all ages for general, traumatic and aesthetic issues. His pursuit of dental perfection and more than 500 hours of advanced clinical training earned him a fellowship with the American Academy of General Dentists, a prestigious designation not pursued by many in his profession. Dr. Walter understands what his patients want and is well known for his high patient satisfaction rates. “We respect people’s time and take care not to overbook; this isn’t a revolving door practice,” Dr. Walter says. “It’s important to me that my patients leave my office completely satisfied with the treatment and service they’ve received. My staff has been with me for a long time; they’re very personable and highly trained which also helps create a comfortable setting in the office.” All Smiles Family Dentistry provides most general and cosmetic services in-house at affordable rates for patients of all ages (including a few centenarians) with a focus on gentle, preventative and restorative dentistry. His state-ofthe-art office contains digital X-ray equipment, an intraoral

camera system to assist in the diagnosis of potential dental issues and the latest technology for greater quality and the most painless experience possible. The range of services provided in this Montville office run from cleanings and checkups to dental restoration, porcelain veneers, crowns and tooth-colored (non-metallic) fillings. Invisalign, Dr. Walter points out, is a popular alternative for adults and teenagers who don’t want the more visible traditional wire braces. On those rare occasions when he feels another doctor could provide more specialized treatment on a complex issue, Dr. Walter doesn’t hesitate to refer to a competent area specialist. These types of collaborative team relationships help him deliver the optimal result for each and every patient—which is really what his practice is all about. He and his hygienists spend considerable time educating his patients on preventative steps they can do on their own as well as applying

As an active member and past president of the Montville Rotary Club, he’s a familiar face around town and makes the time to participate in local community service events as much as possible.

Sunita Merriman, DDS

Dr. Sunita Merriman blends the practice of exceptional general and restorative dentistry with the recognition that patients’ smiles are critical to their overall physical and mental wellbeing. This philosophy, along with her attention to detail and passion of the pursuit of excellence has put her in the position of being one of dentistry’s thought leaders of today.

W

e’re a diagnosis-based practice, not a procedurebased practice. As a medical professional, it’s my responsibility to determine the cause of a problem, not just treat the symptoms.

Otherwise, I could be applying a Band-Aid to a potentially serious problem,” Dr. Merrriman says. Dr. Merriman and her team are specially trained to detect problems of TMJ, teeth, gums and muscles

in their early stages and take a proactive approach to treatment. On any given day, you may find her working on a complex case of reconstructing a broken-down smile, educating a child on how to brush and floss or making a splint for a patient with TMJ problems. Dr. Merriman says, “While technical skills are critically important, great dentistry is far more than just fixing teeth. New findings by top neuroscientists like Andrew Newberg, MD have shown that the mere act of smiling helps to interrupt mood disorders and strengthens the brain’s neural ability to maintain a positive outlook on life. Studies have shown that smiling is one of the best ways to exercise your brain.” After almost 20 years in practice focusing on providing exceptional dental care, Dr. Merriman has assembled what she calls her A Team. “This level of excellence doesn’t just happen. We’re constantly pursuing advanced training on new technology and developments in the field of dentistry,” Dr. Merriman says.“ I’m redefining what dental appointments are all about! My patients are congratulated on their commitment to their health, educated on how to effectively take care of their teeth and gums, and given the chance to

relax and mentally regroup while in the dental chair. Dr. Merriman points out that time in her office is often the only time during the day her patients have to themselves. The office was recently completely renovated to provide the latest in dental equipment and technology, as well as to create a relaxing, Zen-like environment to promote serenity, healing and a positive experience. She says that many people spend much of their lives unhappy with their smiles and ultimately end up having extensive work done years down the line. But it’s really not necessary to wait until your dental condition deteriorates to that point. Dr. Merriman is deeply committed to the education and engagement of her patients. In addition to

A continuous learner herself, she actively engages in the process of teaching other dentists and their teams about advanced dentistry, practice management and team building. She is also on staff at Overlook Health Care Center in Summit, NJ, where her work includes the mentoring of dental residents. Dr. Merriman values her patient relationships, which are based upon trust. She invites anyone to drop by her office and experience the difference of her unique and special practice.

n addition to graduating from the University of Michigan—one of the top three programs in the country—and building a successful general, implant, and cosmetic practice, Dr. Goldberg is one of the area’s most skilled implant practitioners. As part of the move to his new location, Dr. Goldberg installed a digital imaging machine with specialized software that further assists him in the planning and communication for any implant treatment plan. When asked what separates him from other implant offices, he states, “Oftentimes, I present treatment options that patients never were informed of. In addition, patients express frustration when other dentists tell them they have to see multiple doctors for their care: I provide all services myself. This also tends to bring down overall costs. The fact that I place and restore so many implants helps to keep the costs down, too. My staff and I have

streamlined many procedures.” Dr. Goldberg also took advantage of the move to equip the office with some of the latest, most effective dental technologies, including digital X-rays, intraoral cameras and computer-assisted cavity detection so that he could provide all of his patients with the best possible general, cosmetic and restorative dentistry. Dr. Goldberg’s list of services is comprehensive and includes exams, cleanings, periodontal and endodontic treatment, teeth whitening, crowns and bridges, veneers, complex restorative treatments, full mouth reconstruction, surgical and

He’s a Fellow, Associate Fellow or Master of the American Academy of Implant Dentistry, International Congress of Oral Implantologists, American Academy of General Dentistry, American Dental Association, NJ Dental Association, and the TriCounty Dental Society. A firm believer that there’s always more to learn, Dr. Goldberg

is constantly attending continuing education courses. He holds advanced degrees in the fields of general and implant dentistry, which place him in the top few percent of all dentists in the country. Additionally, the Consumers Research Council of America ranks Dr. Goldberg as one of the “Top Dentists in America”. Above all else, the compassion and quality of service that Morris County Dental Associates provides is unparalleled. “A good doctor surrounds himself with a great team. When this is accomplished,

it’s the patient who benefits. We are not a clinic; we develop personal relationships with our patients and treat them like family,” Dr. Goldberg says. Morris County Dental offers free consultations and second opinions, and will work with patients to maximize insurance benefits and create financial arrangements if needed. The practice’s website offers an abundance of information about the office, Dr. Goldberg and dental care in general. Take a look.

How White is Too White? By Robin Holleran

A

nice, bright smile is one thing, but the whitening craze may be getting a tad out of control. At first, it was just Hollywood celebrities that were flaunting big rows of blindingly white, ridiculously perfect teeth. But, as in the past, fads adopted by the stars tend to trickle down to the general public. By all accounts, teeth whitening has become the most popular, most requested cosmetic dental procedure around. Demand has driven the growth of self-proclaimed whitening spas and a barrage of advertising for whitening products such as special toothpastes, strips and gels, rinses, mouth guard-like tray systems and even chewing gum. Recently, MSNBC reported that Americans are now spending more than $1.4 billion on over-the-counter whitening products every year in their quest for the Holy Grail: Whiter teeth.

The professional versus the DIYS-er While all of these OTC whitening products work in varying degrees because they all contain some form of bleaching agent, inoffice whitening is still the quickest and safest way to get pearly whites. The dentist factors in the condition of your teeth and uses a whitener in combination with heat, light or a laser along with a protective gel for the gums and other areas of the mouth. If you go the professional route, the key is communication because everyone’s definition of “white” is different and you don’t want to end up with what many in the profession have coined “toilet-bowl” white teeth (not very flattering, I know) or being embarrassed to open your mouth like the character Ross (David Schwimmer) famously was in a classic episode of Friends when he tragically (and hilariously) way overwhitened.

If you opt for self-whitening, proceed with caution. There are an abundance of horror stories out there about people who become “bleachorexics” and do repeat applications or keep products on way beyond what the manufacturer recommends. Or worse, fall asleep with whitening strips on and wake up in excruciating pain. In fact, a study by the Journal of the American Dental Association found that half of those who bleached their teeth WHITE OUT The NBC hit series Friends featured a experienced some form of classic episode of Ross tragically (and hilariously) discomfort, from light tingling way over-whitening. and burning gums to shooting pain and sensitivity to sound or air, although an off-white crown or patches of usually these symptoms are temporary. bonding isn’t a great look. • Compromise. Dental 2000

At Dental 2000, Dr. Larry Kaplan and his staff of dedicated dental professionals dare to offer what other offices cannot. When you visit, you’re guaranteed that your treatment will be pain-free or your visit is free.

W

hen I entered the profession in 1970, I realized that people’s biggest objection to going to the dentist was fear of pain,” says Dr. Larry Kaplan. “Since then, delivering pain-free dental services has been my mission, and I teach others around the country techniques to make this happen.” He points out that relationship building should be an important part of any practice. As such, he spends a considerable amount of

time with new patients learning about their lives and passions. Using this information, Dr. Kaplan prioritizes their cosmetic, functional, comfort and longevity dental goals—and turns the five typical objections to dental care (fear, budget, time, no sense of urgency and lack of trust) into opportunities for improvement. “My role is to be an educator and coach for my patients so they can ultimately be responsible for their own dental health,” Dr. Kaplan says. “I love my practice and like to think I can make a difference in my patients’ lives.” To accommodate busy schedules, Dental 2000 is open weekends and four evenings per week. Each of the dentists in this practice brings their own unique talents and training so all services can be provided onsite. As if that’s not enough, they also offer oralconscious sedation and candlelight dentistry with heated neck pillows and paraffin treatments to further enhance their patients’ experiences.

He also provides comprehensive dental care for patients with special needs. “Children aren’t just little adults and need to be evaluated and treated differently,” he says. “For the same reason that pediatricians evaluate children very early in life, we endorse the position of both the American Academy of Pediatrics and the American Academy of Pediatric Dentistry that children should have their first dental

visit at around one year of age,” says Dr. Lam. “What parents often don’t realize is that many changes occur in their child’s teeth, diet and hygiene in the first years of life. In fact, one in four children between the ages of two and five have at least one cavity; by the age of five, over 50 percent of all children have at least one cavity.” Dr. Lam is also a member of a very select group of internationally board-certified pediatric dentists. He’s a Diplomat of the American Board of Pediatric Dentistry, a board-certified Fellow of the Royal College of Dentists of Canada and has hospital privileges at Morristown Memorial Hospital. Based on his extensive pediatric experience and reputation for providing the highest level of quality care in a calm and professional manner, many families choose Dr. Jay Felsenstein for their children’s dental needs.

P

art of Dr. Felsenstein’s appeal is his practice of looking his patients in the eyes and talking straight with them about dental hygiene and care of their teeth, which most everyone appreciates regardless of their age—although he’s known to sneak in a goofy noise or squeak from time to time, too. Kids Dental Kare is also decorated with bright colors, with televisions placed in all treatment rooms and video games in the reception area (as well as a coffee and tea station for parents). As an added touch, everyone in the office is trained to twist together

popular balloon figures for their departing guests. According to Dr. Felsenstein, there’s really no such thing as being too young to go to the dentist. People often don’t realize that neglected cavities in primary teeth or “baby teeth” can lead to problems in the development of permanent teeth, the jaw bone and muscles. Primary teeth also affect the development of speech and add to an attractive appearance. “Kids Dental Kare is most sensitive to the needs of children,” says Dr. Felsenstein. “Not everyone has the patience or is tuned into the psychology of treating young people.

r. Sparagna believes dentistry is a fusion of medicine and engineering based on the assessment of patient’s overall health. It is Shore Family Dental’s goal to resolve the pathologies and engineer dental restorations to reestablish functional and physiologic health. Identification of the etiology is critical for longterm, predictable dental health. “I evaluate, mount and generate a proper occlusal/orthognathic scheme prior to beginning a reconstruction. This is the cornerstone of any full reconstruction,” she says. Dr. Sparagna focuses on dental technologies that support her practice philosophy and continues

to expand this vision with extensive continuing education. Her focus for the future is the implementation of the digital impression, 3-D intraoral scanner. This scanner will allow for more detailed and real time sharing of the data, thus raising the collaborative bar in providing my patients with superior restorations.” “As a general dentist with a multiple discipline practice, treatment options are as diverse as our patients’ needs, such as conventional fixed bridge, implants, over-denture with attachments, etc.,” says Dr. Sparagna, “However, the final treatment is a fusion of the engineering demands of the patient’s occlusion and their wants and desires.”

D

entistry is much more than “drill and fill” these days. According to a recent article published in Dental Economics, more

than half of the treatments achieved from dental procedures today didn’t even

exist 20 years ago. When you think about it, that’s pretty remarkable. The explosive rate of innovation in the dental industry is largely being driven by two factors: Advances in science and technology and the rising expectations from more educated and involved patients. With this trend only expected to continue at this pace for the foreseeable future, if you haven’t been to a dentist recently or think you may need a dental procedure in the near future, you should dismiss any and all preconceived notions of what may be involved. Your health is on the line. Keeping up with all of this change can be a bit challenging if you’re not a dentist yourself, and knowing which treatments are right for your specific needs is not always that easy. With this in mind, we’ve decided to provide insight into some cutting-edge advances that are becoming staples at dental practices throughout New Jersey.

No pain, all gain Dentists are keenly aware that plenty of patients still break out in a full-body sweat at the mere thought of going to the dentist, and that many would prefer to endure the agony of a toothache hoping that it will go away before seeking the attention of a dentist. However, there’s really no need for anyone to be fearful of a dentist; pain no longer needs to be a part of a visit. One of the most prominent solutions to the anxiety caused by going to the dentist or the pain associated with a visit is the addition of sedation dentistry as part of a dentist’s menu of services that can be used for procedures ranging from a routine cleaning to more extensive oral surgery. In addition to nitrous oxide—more commonly referred to as “laughing gas”—dentists have a number of oral and intravenous agents available to help their patients relax. Depending on the needs of the patient and the training of the dentist, there are various levels of sedation that can be used:

dentist’s office. Upon consumption, the medication makes the patient drowsy and sometimes causes he or she to drift off to sleep during the procedure, although you can usually be woken with a gentle shake. » IV SEDATION. Growing in popularity, IV sedation is made possible when medication is delivered directly into the blood stream via an IV. This method works very quickly and allows the dentist to continually adjust the level of sedation as needed. In New Jersey, it’s important to

note that dentists need to be certified to administer IV sedation. If you think this is what you need, check out your doctor’s credentials prior to your appointment. » GENERAL ANESTHESIA. Anyone placed under general anesthesia becomes completely or nearly unconscious. A patient who’s deeply asleep during a dental procedure will not wake up until the anesthesia wears off or a reversal drug is administered. Only a small number of dentists, usually those who

» INHALED SEDATION. Nitrous oxide mixed with oxygen is on the minimal end of the sedation scale that has been around for years, but should not be underestimated. You might technically be “awake” but based on personal experience, there’s not much that can worry or concern you while you’re under the influence. It works like this: First, your dentist places a mask over your nose and controls the amount of sedation you receive as he or she works. The inhaled gas goes into effect almost instantaneously and wears off quickly. This is the only form of sedation patients can receive and be able to drive themselves home afterward. » ORAL SEDATION. Often, when administering oral sedation something from the Valium family is prescribed and, depending on the dosage, provides minimal to moderate levels of relief. This is a common form of sedation and is typically taken before arriving at the

specialize in oral or maxillofacial surgeries, are permitted to administer deep sedation and it’s typically reserved for extensive and invasive procedures. Those who are administered a form of sedation during their dental appointments tend to benefit tremendously. They’re far more comfortable than they ever would be otherwise and usually have little or no recollection of the treatment they receive. And being relaxed is not only an advantage for the patient; it also makes it a lot easier for a dentist to treat a nervous patient without having to worry about being bitten or having to anticipate sudden movements. In addition to being a solution to manage anxiety, sedation could be an option for those with a low pain threshold, highly sensitive teeth or a strong gag reflex. Anyone who needs an extensive amount of dental work or simply can’t sit still in a dentist’s chair for prolonged periods of time may want to inquire about this route, too. Because there are risks to sedation— just as there are risks with the use of any medication—a patient must undergo a thorough evaluation and consultation to confirm that sedation is a safe and effective course of action. Even with this added step don’t be shy about asking about the dentist’s training and experience with sedation. There’s no reason for a dentist to be taken aback about questions like these, and if they are, you might want to consider looking for a new practice.

Not your grandma’s dentist General dentists are usually the first line of oral care. And while they’re not expected to master every specialty, dentists in this area of practice are playing a greater role in other aspects of their patients’ wellbeing than ever before as scientists continue to uncover more links between oral and general health.

While this connection may seem farfetched at first, the Mayo Clinic points out that the condition of your mouth can offer valuable insight into potential health risks occurring in other areas of your body. For example, excessive oral bacteria that cause tooth decay and gum disease can be an early warning sign of a compromised immune system. Additionally, other conditions, such

Oral screenings are now routinely performed by dentists to detect a number of potential problems, including the presence of oral cancer, temporomandibular joint disorders (TMJ), sleep apnea (the involuntary cessation of breathing when asleep), loud snoring, teeth grinding and other health conditions that are closely related to the mouth. If any of these issues

Dentists are equipped with a number of new dental devices and appliances that are more affordable, less invasive and more effective in treating disorders than in the past. as cardiovascular disease, pregnancy and childbirth, diabetes, HIV/AIDS, osteoporosis, Alzheimer’s disease and eating disorders can directly impact one’s oral health. Conversely, dentists should be aware of any medications or medical treatments their patients receive outside of their offices. Some commonly prescribed medications are notorious for disrupting the natural balance and normal protective barriers in the mouth, allowing bacteria to enter the bloodstream and opening the door to serious infection.

are discovered, dentists are equipped with a number of new dental devices and appliances that are more affordable, less invasive and more effective in treating disorders than in the past. In recent years, the role of the dentist as a medical professional has changed considerably as well. Today, dentists focus a considerable amount of time on educating their patients about disease prevention and proper oral maintenance of the teeth, gums and maxillofacial area of the face (where the jaw meets the mouth).

In addition to the growing scope of dental care, the technology used to treat oral conditions has changed drastically. To the delight of, well, everyone, the bloodcurdling whine of a drill has largely been replaced by digital technology that greatly reduces radiation exposure and allows the dentist to perform many procedures faster, with fewer visits and in a pain-free manner—even without sedation. A prime example is the adoption of CAD/CAM technology for metal-free crowns and other restorations that can now be made at the dentist’s office rather than being shipped to a laboratory. The patient no longer has to make multiple appointments or go to a specialist who they’re unfamiliar with. It’s not just the technology in the dental office that has been updated; the overall dental experience has changed considerably. The professionals in dental offices are now highly trained with a strong emphasis on customer service. It’s now often common for offices to be equipped with televisions, video games and other distractions to make patients feel at home. Many dentists have also embraced social media and digital communications such as emails, texts, Facebook, YouTube and

44

New Jersey Top Dentists • 2013

Twitter to keep their patients informed about appointments, scheduling, insurance issues, dental procedures and innovations. While it’s always nice to know that there’s someone looking out for your health, the movement to treat a patient’s overall health should not be confused with Holistic Dentistry. This is a loosely used term that includes alternative practices that don’t always agree with the recommendations of professional organizations such as the American Dental Association.

Start ’em young With all of the improved healthcare and diagnostic technology available, it may come as a surprise that The Centers for Disease Control and Prevention found a notable increase in the number of preschool children at all socioeconomic levels with a large number of cavities. The survey found that by the time children are brought to a dental appointment the level of decay is many times so severe and extensive—often six

to ten cavities—that these young patients sometimes require general anesthesia and lengthy and invasive procedures to correct the problems. In fact, dental decay is still the most common infectious and chronic childhood disease and is nine times more prevalent than childhood asthma. Poor oral health not only puts a child at the unnecessary risk of dental-related diseases and problems eating, speaking and sleeping, but also has been linked to emotional development problems, such as poor self-esteem. Studies have also shown that children suffering from chronic dental pain have trouble concentrating and, not surprisingly, often perform poorly in school as a result. The alarming increase in pediatric dental problems is often attributed to the endless consumption of sugary snacks and juice, sweet drinks at night and parents who prefer bottled water for their children instead of fluoridated tap water. The unfortunate truth is simply that many parents aren’t aware that they should take their children to visit a dentist even before all of their primary teeth have grown in. In fact, the American Academy of Pediatric Dentistry recommends that children make their first visits within six months of a primary tooth breaking through the gums and no later than a child’s first birthday. This recommendation comes after a great deal of research and for good reason. There are about 20 baby teeth in total, 12 of which should remain until a child is up to 12 years old. They’re important space holders for adult teeth and if a child loses one too early, it could create a crowding problem when the adult teeth are ready to grow in. And certainly no parent wants his or her child to go through unnecessary oral surgery if it can be avoided. Luckily, childhood dental decay is largely preventable with daily brushing, fluoride and regular visits to the dentist starting at an early age. Parents should also take into

The unfortunate truth is simply that many parents aren’t aware that they should take their children to visit a dentist even before all of their primary teeth have grown in consideration whether their hometowns have fluoridated water, which plays a significant role in the strength of growing teeth. Having made that mistake with my eldest son in assuming our town’s water contained fluoride already, he had to endure having a half-dozen cavities filled after his first visit to the dentist. We immediately switched back to the prescription vitamins with fluoride than the over-the-counter, fluoride-less brands. Ultimately, it’s imperative that you choose your child’s dentist wisely. Not all general dentists are up-to-date on pediatric

recommendations or treatment options and shouldn’t be simply using adult guidelines in a scaled back format. Similarly, not every dentist has the skill set to treat a squirmy child either. I can still recall a personal experience of knocking out my front tooth in the second grade and having the dentist screaming at me to sit still when the nitrous oxide made me feel as though I was spinning backwards and causing me—in my drug induced state— to keep pulling myself forward to keep from falling. Needless to say, we never went back to that particular specialist.

No more tinsel teeth While the ultimate goal of using orthodontics to align teeth hasn’t changed, the means in which this is achieved and the type of patients seeking treatment has changed considerably. Bulky, metal braces are little more than a distant memory for most patients thanks to the strides made in improving orthodontic appliances. As a result, aligning one’s teeth is no longer just for the preteen generation.

The American Association of Orthodontists (AAO) recently estimated that one in five orthodontic patients is now an adult. The inventions of “invisible” braces and clear, removable aligners have made orthodontics mainstream among the more “mature” crowd. A growing number of adults who didn’t have the opportunity to have their teeth straightened when they were younger, or whose teeth have shifted in their later years, are now able to seek treatments that will properly align their teeth. Dentists and orthodontists appreciate adult patients, too, because they often times tend to be more motivated and self-disciplined, increasing the likelihood of successful treatment and satisfactory results. For children, the AAO also recommends that a child make his or her first visit to an orthodontist around the age of seven. Contrary to previous practice, braces are becoming more commonplace with younger children based on the philosophy that it is easier to treat an obvious orthodontic problem even before all of the permanent teeth have grown in. Besides, the fun and funky colors used for bands and retainers have made being treated with braces much more appealing to the younger crowd. Orthodontists also have a plethora of innovative tools at their disposal to streamline and improve the processes they utilize in treatments. Threedimensional scans of the head and face are being used more frequently to create digital impressions and circumvent any unpleasant molding materials. Digital technologies can also determine how a patient’s facial features will be influenced by the placement of braces before they’re attached. This gives the orthodontist the ability to focus on the overall aesthetics rather than just the positioning

of brackets. These machines provide for more precision placement by starting with the end result and then guiding the dentist to the exact location for placement of braces to achieve the desired effect. Self ligating braces are growing in popularity as well. This method of treatment removes the need for rubber bands so the wires slide comfortably in the brackets as teeth adjust. This form of braces often shortens treatment times and requires fewer trips to the dentist. Benefits that busy parents and children alike are sure to benefit from.

Smiles for life Often one of the first things anyone notices about other people when they meet are their teeth. In addition to first, positive impressions, a dazzling smile can go a long way in improving a person’s self-image. Smile makeovers are a fast-growing segment of the dental industry and many offices have plenty of before-and-after photographs on their websites and office walls to provide prospective patients with an idea of the dramatic difference that a makeover for their mouths can make. Combining professional training, an artistic eye and an arsenal of cosmetic procedures, including bonding, veneers, teeth whitening, braces and implants, dentists can truly have a life-changing impact on the way in which patients are perceived by others and how they feel about themselves. The foray into using more naturallooking and durable materials has made these techniques more predictable, affordable and achievable for those with less than desirable smiles. Many procedures with lasting impacts can be completed in one day at the same offices and are known to make the simple things in

In addition to first, positive impressions, a dazzling smile can go a long way in improving a person’s self-image life that many of us take for granted—think eating, chewing and smiling—much more comfortable. Despite what one may think—especially given the larger scope of their current capabilities—most dentists tend to be conservative in their approach to cosmetic dentistry. They try to preserve as much of the natural tooth structure as possible to avoid having to remove any teeth. If

missing teeth do become an issue, the use of dental implants is quickly replacing crowns and bridges as the best corrective solutions. The result is not only an improved smile, but also an overall more youthful appearance, as missing teeth can sometimes cause the face to sag and appear older. In less frequent, but more extreme cases, a cosmetic dentist may still recommend a complete mouth

reconstruction to correct problems with your bite, muscles and bone structure. Though this may seem daunting, many cosmetic dentists have converted their offices into spa-like settings, offering paraffin treatments, heated neck pillows and even massages during appointments to, again, reduce the anxiety associated with the dental office and set you on the path to perfecting your smile. ■

SPECIAL TO TOP DENTISTS

Your Dentist Can Save Your Life

It’s not just for a cleaning anymore. Learn about the oral-systemic health link and the impact of a smile on your brain function. Your dentist knows.

T

Sunita Merriman, DDS

he days when a patient went to his or her dentist just for a check-up and cleaning are long gone. Curiosity of how an oral infection could possibly be related to the occurrence of disease in other parts of the body goes back centuries. In fact, Willoughby D. Miller published a report The Human Mouth as a Focus of Infection in 1891. Fast-forward more than a century to the year 2000 where Surgeon General Dr. David Satcher stressed in his report that bacteria in the mouth can enter systemic circulation and cause disease. Conversely, he noted that there are oral manifestations of systemic diseases and that medications can have adverse oral effects. And, often times, it’s your dentist who is the first to detect systemic diseases by noticing changes within the oral cavity. The investigation into oral-systemic disease connections is a rapidly advancing area of research. Ongoing research clearly shows an epidemiological association of periodontal disease and specific systemic diseases, including cardiovascular disease. What’s still unclear is whether periodontal disease is a risk factor for cardiovascular issues, including stroke, or does it contribute casually to heart disease and other systemic diseases such as diabetes, gestational development, kidney and pulmonary diseases? Periodontal disease is a progressive infection of the gums and their supporting structures caused by bacteria in the

mouth that infects tissue surrounding the tooth, causing inflammation. Usually asymptomatic, periodontal disease affects three out of four Americans in some form and can go undetected for many years if one doesn’t visit a dental professional regularly. Symptoms may include bad breath or bad taste that won’t go away, gums that are

in response to new situations or to changes in their environment. Researchers, including Andrew Newberg, MD and David Perlmutter, MD, have conclusively demonstrated that certain practices help exercise our brain for optimal neural functioning and repair. You may be asking the question: What does dentistry have to do with this exciting information? Well, the act of smiling is one such practice. Dr. Newberg states that smiling repetitively helps to interrupt mood disorders and strengthens the brain’s neural ability to maintain a positive outlook on life. It enhances the neural functioning in specific parts of the brain that are involved with lowering anxiety and depression, increases social awareness and empathy and improves cognitive and intellectual functioning. It has been shown that even if you fake a smile, other people respond to it with great generosity and kindness. To test this hypothesis, smile at a total stranger while waiting in a line and see how they respond. Then see how their response makes you feel. The results are often nothing short of amazing. So the next time see your dentist, we hope that you congratulate yourself on taking care of your teeth, gums, oral cavity, smile, systemic health and even your brain function in one important visit. As one can now clearly see, your dentist is way past going for a “cleaning.” Your dentist can now save your life. ■

As a result of the information being revealed by ongoing research, periodontal disease treatment helps control overall inflammatory burden, which has a positive impact on systemic health, especially if the patient is at an elevated risk for a more serious systemic problems. This has been a major paradigm shift from the earlier objective of dentists to treat periodontal disease solely to prevent tooth loss. Recent breakthrough findings from leading neurologists show that the brain has the ability to reorganize itself by forming new neural connections throughout life. This is called neuroplasticity. It allows the neurons (nerve cells) in the brain to compensate for injury and disease and to adjust their activities

“With passion, everything is possible.” Michael Maron, President/CEO, Holy Name Medical Center

“There’s something different about Holy Name. It’s why our patients insist on being treated here. It’s why our employees love working here. It’s why our doctors want to practice here. “It has to do with passion. A passion for the ministry of health care. At Holy Name, there is a collective mindset, a shared purpose, a common philosophy that healing can be found everywhere. Not just in medical technology and pharmaceuticals, but in the connections we make with the people we serve and with one another. “Amidst a revolution in our industry, surrounded by disruptive influences, Holy Name is prepared for the future and thriving on every level. I believe the blueprint for our success is the mission of our founding Sisters of St. Joseph of Peace. “The pursuit of quality, a willingness to change, sound stewardship, the embracing of plurality, honest value… these are enduring principles that are always relevant, and at the heart of everything we do. That’s Holy Name’s passion.”
