He points out that cosmetic dental techniques have changed considerably since his early days. “In the 1980s, we had to be more aggressive, but now we can get incredible results with minor enamel smoothing of a tooth and super-thin veneers. Any such changes are accompanied by some sort of “compensation” such as a pass to the movies. Because he values his patients’ time, they do the same and call if they’re going to be any more than 15 minutes late (which can have an unpleasant trickle-down effect on later appointments if not managed properly). More than anything, it’s Dr. Romano’s unparalleled expertise that attracts so many of his patients. A

nice, bright smile is one thing, but the whitening craze may be getting a tad out of control. At first, it was just Hollywood celebrities that were flaunting big rows of blindingly white, ridiculously perfect teeth. But, as in the past, fads adopted by the stars tend to trickle down to the general public. By all accounts, teeth whitening has become the most popular, most requested cosmetic dental procedure around. Demand has driven the growth of self-proclaimed whitening spas and a barrage of advertising for whitening products such as special toothpastes, strips and gels, rinses, mouth guard-like tray systems and even chewing gum. Recently, MSNBC reported that Americans are now spending more than $1.4 billion on over-the-counter whitening products every year in their quest for the Holy Grail: Whiter teeth.

The professional versus the DIYS-er While all of these OTC whitening products work in varying degrees because they all contain some form of bleaching agent, inoffice whitening is still the quickest and safest way to get pearly whites. The dentist factors in the condition of your teeth and uses a whitener in combination with heat, light or a laser along with a protective gel for the gums and other areas of the mouth. If you go the professional route, the key is communication because everyone’s definition of “white” is different and you don’t want to end up with what many in the profession have coined “toilet-bowl” white teeth (not very flattering, I know) or being embarrassed to open your mouth like the character Ross (David Schwimmer) famously was in a classic episode of Friends when he tragically (and hilariously) way overwhitened.

If you opt for self-whitening, proceed with caution. There are an abundance of horror stories out there about people who become “bleachorexics” and do repeat applications or keep products on way beyond what the manufacturer recommends. Or worse, fall asleep with whitening strips on and wake up in excruciating pain. entistry is much more than “drill and fill” these days. According to a recent article published in Dental Economics, more

than half of the treatments achieved from dental procedures today didn’t even

exist 20 years ago. When you think about it, that’s pretty remarkable. The explosive rate of innovation in the dental industry is largely being driven by two factors: Advances in science and technology and the rising expectations from more educated and involved patients. With this trend only expected to continue at this pace for the foreseeable future, if you haven’t been to a dentist recently or think you may need a dental procedure in the near future, you should dismiss any and all preconceived notions of what may be involved. Your health is on the line. Keeping up with all of this change can be a bit challenging if you’re not a dentist yourself, and knowing which treatments are right for your specific needs is not always that easy. With this in mind, we’ve decided to provide insight into some cutting-edge advances that are becoming staples at dental practices throughout New Jersey.

No pain, all gain Dentists are keenly aware that plenty of patients still break out in a full-body sweat at the mere thought of going to the dentist, and that many would prefer to endure the agony of a toothache hoping that it will go away before seeking the attention of a dentist. However, there’s really no need for anyone to be fearful of a dentist; pain no longer needs to be a part of a visit. One of the most prominent solutions to the anxiety caused by going to the dentist or the pain associated with a visit is the addition of sedation dentistry as part of a dentist’s menu of services that can be used for procedures ranging from a routine cleaning to more extensive oral surgery. In addition to nitrous oxide—more commonly referred to as “laughing gas”—dentists have a number of oral and intravenous agents available to help their patients relax. Depending on the needs of the patient and the training of the dentist, there are various levels of sedation that can be used:

dentist’s office. Upon consumption, the medication makes the patient drowsy and sometimes causes he or she to drift off to sleep during the procedure, although you can usually be woken with a gentle shake. » IV SEDATION. Growing in popularity, IV sedation is made possible when medication is delivered directly into the blood stream via an IV. This method works very quickly and allows the dentist to continually adjust the level of sedation as needed. In New Jersey, it’s important to

note that dentists need to be certified to administer IV sedation. If you think this is what you need, check out your doctor’s credentials prior to your appointment. » GENERAL ANESTHESIA. Anyone placed under general anesthesia becomes completely or nearly unconscious. A patient who’s deeply asleep during a dental procedure will not wake up until the anesthesia wears off or a reversal drug is administered. Only a small number of dentists, usually those who

You might technically be “awake” but based on personal experience, there’s not much that can worry or concern you while you’re under the influence. Anyone who needs an extensive amount of dental work or simply can’t sit still in a dentist’s chair for prolonged periods of time may want to inquire about this route, too. Because there are risks to sedation— just as there are risks with the use of any medication—a patient must undergo a thorough evaluation and consultation to confirm that sedation is a safe and effective course of action. Even with this added step don’t be shy about asking about the dentist’s training and experience with sedation. To the delight of, well, everyone, the bloodcurdling whine of a drill has largely been replaced by digital technology that greatly reduces radiation exposure and allows the dentist to perform many procedures faster, with fewer visits and in a pain-free manner—even without sedation. A prime example is the adoption of CAD/CAM technology for metal-free crowns and other restorations that can now be made at the dentist’s office rather than being shipped to a laboratory. The patient no longer has to make multiple appointments or go to a specialist who they’re unfamiliar with. It’s not just the technology in the dental office that has been updated; the overall dental experience has changed considerably. The professionals in dental offices are now highly trained with a strong emphasis on customer service. It’s now often common for offices to be equipped with televisions, video games and other distractions to make patients feel at home. Many dentists have also embraced social media and digital communications such as emails, texts, Facebook, YouTube and

Twitter to keep their patients informed about appointments, scheduling, insurance issues, dental procedures and innovations. While it’s always nice to know that there’s someone looking out for your health, the movement to treat a patient’s overall health should not be confused with Holistic Dentistry. This is a loosely used term that includes alternative practices that don’t always agree with the recommendations of professional organizations such as the American Dental Association.

Start ’em young With all of the improved healthcare and diagnostic technology available, it may come as a surprise that The Centers for Disease Control and Prevention found a notable increase in the number of preschool children at all socioeconomic levels with a large number of cavities. The survey found that by the time children are brought to a dental appointment the level of decay is many times so severe and extensive—often six

to ten cavities—that these young patients sometimes require general anesthesia and lengthy and invasive procedures to correct the problems. In fact, dental decay is still the most common infectious and chronic childhood disease and is nine times more prevalent than childhood asthma. Poor oral health not only puts a child at the unnecessary risk of dental-related diseases and problems eating, speaking and sleeping, but also has been linked to emotional development problems, such as poor self-esteem. Studies have also shown that children suffering from chronic dental pain have trouble concentrating and, not surprisingly, often perform poorly in school as a result. The alarming increase in pediatric dental problems is often attributed to the endless consumption of sugary snacks and juice, sweet drinks at night and parents who prefer bottled water for their children instead of fluoridated tap water. The unfortunate truth is simply that many parents aren’t aware that they should take their children to visit a dentist even before all of their primary teeth have grown in. In fact, the American Academy of Pediatric Dentistry recommends that children make their first visits within six months of a primary tooth breaking through the gums and no later than a child’s first birthday. This recommendation comes after a great deal of research and for good reason. There are about 20 baby teeth in total, 12 of which should remain until a child is up to 12 years old. They’re important space holders for adult teeth and if a child loses one too early, it could create a crowding problem when the adult teeth are ready to grow in. And certainly no parent wants his or her child to go through unnecessary oral surgery if it can be avoided. Luckily, childhood dental decay is largely preventable with daily brushing, fluoride and regular visits to the dentist starting at an early age. Parents should also take into

Having made that mistake with my eldest son in assuming our town’s water contained fluoride already, he had to endure having a half-dozen cavities filled after his first visit to the dentist. We immediately switched back to the prescription vitamins with fluoride than the over-the-counter, fluoride-less brands. Ultimately, it’s imperative that you choose your child’s dentist wisely. Not all general dentists are up-to-date on pediatric

recommendations or treatment options and shouldn’t be simply using adult guidelines in a scaled back format. Similarly, not every dentist has the skill set to treat a squirmy child either. I can still recall a personal experience of knocking out my front tooth in the second grade and having the dentist screaming at me to sit still when the nitrous oxide made me feel as though I was spinning backwards and causing me—in my drug induced state— to keep pulling myself forward to keep from falling. Needless to say, we never went back to that particular specialist.

No more tinsel teeth While the ultimate goal of using orthodontics to align teeth hasn’t changed, the means in which this is achieved and the type of patients seeking treatment has changed considerably. Bulky, metal braces are little more than a distant memory for most patients thanks to the strides made in improving orthodontic appliances. As a result, aligning one’s teeth is no longer just for the preteen generation.

The American Association of Orthodontists (AAO) recently estimated that one in five orthodontic patients is now an adult. The inventions of “invisible” braces and clear, removable aligners have made orthodontics mainstream among the more “mature” crowd. A growing number of adults who didn’t have the opportunity to have their teeth straightened when they were younger, or whose teeth have shifted in their later years, are now able to seek treatments that will properly align their teeth. Dentists and orthodontists appreciate adult patients, too, because they often times tend to be more motivated and self-disciplined, increasing the likelihood of successful treatment and satisfactory results. For children, the AAO also recommends that a child make his or her first visit to an orthodontist around the age of seven. Contrary to previous practice, braces are becoming more commonplace with younger children based on the philosophy that it is easier to treat an obvious orthodontic problem even before all of the permanent teeth have grown in. Besides, the fun and funky colors used for bands and retainers have made being treated with braces much more appealing to the younger crowd. Orthodontists also have a plethora of innovative tools at their disposal to streamline and improve the processes they utilize in treatments. Threedimensional scans of the head and face are being used more frequently to create digital impressions and circumvent any unpleasant molding materials. Digital technologies can also determine how a patient’s facial features will be influenced by the placement of braces before they’re attached. This gives the orthodontist the ability to focus on the overall aesthetics rather than just the positioning

of brackets. These machines provide for more precision placement by starting with the end result and then guiding the dentist to the exact location for placement of braces to achieve the desired effect. Self ligating braces are growing in popularity as well. This method of treatment removes the need for rubber bands so the wires slide comfortably in the brackets as teeth adjust. This form of braces often shortens treatment times and requires fewer trips to the dentist. Benefits that busy parents and children alike are sure to benefit from.

he days when a patient went to his or her dentist just for a check-up and cleaning are long gone. Curiosity of how an oral infection could possibly be related to the occurrence of disease in other parts of the body goes back centuries. In fact, Willoughby D. Miller published a report The Human Mouth as a Focus of Infection in 1891. Fast-forward more than a century to the year 2000 where Surgeon General Dr. David Satcher stressed in his report that bacteria in the mouth can enter systemic circulation and cause disease. Conversely, he noted that there are oral manifestations of systemic diseases and that medications can have adverse oral effects. And, often times, it’s your dentist who is the first to detect systemic diseases by noticing changes within the oral cavity. The investigation into oral-systemic disease connections is a rapidly advancing area of research. Ongoing research clearly shows an epidemiological association of periodontal disease and specific systemic diseases, including cardiovascular disease. What’s still unclear is whether periodontal disease is a risk factor for cardiovascular issues, including stroke, or does it contribute casually to heart disease and other systemic diseases such as diabetes, gestational development, kidney and pulmonary diseases? Periodontal disease is a progressive infection of the gums and their supporting structures caused by bacteria in the
