The programme is made up of the following parts:

1. Discovery Phase: Get to know the HUB, staff, and other participants.You’ll explore your strengths and goals with a key worker. See image below:

KeyWorkerMeetings

A key worker is your identified supporter in the HUB programme Y support you to write your Personal Profile and person-centred plan you with the goals you set, for example, work, education, social we recreation, and community integration

Key workers do not work on all of your goals, however they will su other supporters and agencies/groups to achieve the goals you se plan and key worker meetings

You will have 1-1 time in your timetable to speak to your key worke concerned about anything

Your Key Worker will: meet you on a regular basis (how often varies depending on h the particular time) support you, with the help of your circle of support, to write up and review it regularly co-ordinate the delivery of the plan to help you achieve the go person-centred plan be present at various meetings/appointments, if requested answer your questions about the service and, if required work with your supporters if needed (with your permission)

Personal Profile

A Personal Profile provides information about and your likes and dislikes and your needs and wishes on one page This helps you to quickly understand what is important to you so that the right sort of support can be provided.

Your key worker will help you write up your Personal Profile which can be shared with your supporters, if needed. This will be done during the Discovery phase of the HUB programme when you start

Person-Centred Plan

Person-Centred Planning supports you to make choices about how you want to live your life The plan has information about your hopes and dreams - the things that are important to you.

Your key worker will support you to develop your Person-Centred Plan.You will lead this meeting and set goals You will tell your key worker what supports you need to reach your goals.You can also have other supporters attend this meeting. Just let your key worker know

Person-Centred Planning is a way to make plans for your future Your first plan will be developed during the Discovery phase of your HUB programme and will be reviewed every four months where you can see the progress you have made You can also check on your goals during your monthly key worker meetings.
