Prostate Problems Most men may not think about their prostate until they begin experiencing problems.

Rodriguez But, given its location, changes in this gland can affect other important functions — like urination.

“Prostate problems can be significant,” said Dr. Gabriel Rodriguez, a urologist at Houston Methodist Clear Lake Hospital.

“You may have prostate issues if you have a frequent need to urinate, painful urination or incomplete emptying of the bladder.”

The three most common prostate problems men face include: ■ Prostatitis ■ Benign prostatic hyperplasia (BPH), or enlarged prostate ■ Prostate cancer

Although BPH is not cancerous, many of its symptoms are like those of prostate cancer. BPH can lead to life-disrupting and life-threatening problems, such as: ■ Bladder or kidney infections ■ Kidney failure ■ Urinary blockages ■ Voiding difficulty “You can lessen BPH symptoms by making a few lifestyle changes, such as limiting alcohol and caffeine,” Rodriguez said. “Sometimes medication or surgery may be recommended to ease discomfort and any urine flow problems.”

In some cases, noninvasive options may be considered: ■ Aquablation (see sidebar) ■ Button procedure ■ GreenLight™ laser procedures ■ Transurethral resection of the prostate (TURP)

HOW IS PROSTATITIS TREATED?

Prostatitis is prostate inflammation usually caused by an infection. A fairly common condition, prostatitis can lead to difficulty passing urine and sexual problems.

Prostatitis is typically treated with antibiotics, extending two weeks or more in some cases, depending on whether the infection is sudden or chronic. Antiinflammatory medication and medications that relax prostate muscle tissue may also be recommended to ease urine passage.

HOW IS BPH TREATED?

Many men experience prostate enlargement as they age. This condition can cause a squeezing of the urethra as well as bladder weakening, which can leave urine in the bladder.

HOW IS PROSTATE CANCER TREATED? Prostate cancer can slowly grow and never cause symptoms or threaten a man’s health.

Sometimes, however, it can aggressively attack and require treatment.

Treatment options for prostate cancer include: ■ Chemotherapy ■ High-intensity focused ultrasound (HIFU) treatment ■ Radiation therapy ■ Surgical removal of the prostate gland ■ Testosterone-blocking hormone therapy ■ Watchful waiting and symptom monitoring “How prostate cancer is treated depends on the tumor’s location, whether the cancer has spread, the man’s age and his overall health,” Rodriguez added. “Men should consult with a cancer care team — which consists of a urologist, radiation oncologist and medical oncologist — before choosing a course of therapy.”

MINIMALLY INVASIVE BPH TREATMENT Houston Methodist Clear Lake Hospital is the first in the Bay Area to offer robotic waterjet aquablation, a minimally invasive surgical treatment for BPH, also known as enlarged prostate.

“With aquablation, a urologist uses an image-guided robotic arm to hold a water jet that very precisely removes the prostate tissue causing symptoms,” Rodriguez said. “The prostate is reached through the urethra, so no incision is necessary. It also allows us to treat any size prostate with virtually no risk to sexual function or ejaculation.”

This procedure typically takes about 45 minutes, with less post-operative pain and faster recovery rates than traditional surgical approaches.

SIGNS IT’S TIME TO SEE A GYNECOLOGIST A trip to the gynecologist — a female reproductive health specialist — should be part of your annual well exams. But sometimes your body lets you know a visit is needed sooner.

“It’s important to call your gynecologist when you notice a difference in your reproductive health,” explained Dr. Chinemelum Orizu, OB-GYN at Houston Methodist Clear Lake Dr. Chinemelum Hospital. “Some issues can be quite serious Orizu and shouldn’t wait for an annual exam.”

Look for these signs that it’s time to schedule an appointment with a gynecologist.

■ Night sweats ■ Trouble sleeping or concentrating ■ Vaginal dryness “Menopause usually is diagnosed after 12 months without a period,” Orizu noted. “It’s important to visit your gynecologist to talk about what to expect during this phase of your life.”

PAINFUL, ABNORMAL PERIODS

Your gynecologist may be able to assess symptoms over the phone and prescribe medication if you’re prone to uncomfortable burning and strong urges that signal a simple urinary tract infection.

An abnormality could be to blame when your period is worse for several menstrual cycles. Look for symptoms such as: ■ Bleeding that lasts longer than a week ■ Heavy cramping before and during the cycle ■ Heavier-than-usual bleeding ■ Unexpected bleeding “Talk with a gynecologist if you have any signs of an abnormal period,” Orizu said. “Conditions like noncancerous uterine fibroids or endometriosis may be to blame.”

IRREGULAR VAGINAL BLEEDING

Ovulation, a different or missed dose of birth control medication, or getting close to menopause can all cause small amounts of bleeding between periods. But seek care right away if you experience irregular bleeding that’s concerning.

These symptoms can mean a serious problem: ■ Unexplained bleeding between periods ■ Bleeding during pregnancy ■ Bleeding after menopause

PELVIC PAIN

See your gynecologist if pelvic pain occurs during exercise or intercourse or is accompanied by fever, vomiting, bloating or heaviness in your abdomen. An ovarian cyst or pelvic inflammatory disease — an infection that needs antibiotics — is sometimes the problem.

PERIMENOPAUSE OR MENOPAUSE

Perimenopause is the time leading up to menopause when hormone levels begin to decline. Common signs include: ■ Changes in your menstrual cycle or mood ■ Headaches or joint and muscle pain ■ Hot flashes

BREAST PAIN OR LUMPS

Your gynecologist can evaluate a breast lump or assess pain to help you determine next steps. Breast imagery such as a mammogram or ultrasound will likely be needed for a diagnosis.

It’s Time to Talk About

YOUR KNEE PAIN Knee pain is hard to ignore. You feel it with just about every move you make. And if you’re active, an achy knee can also make the first few minutes of a workout Dr. Myphuong pretty miserable — Phan or totally disrupt your exercise routine if the pain is bad enough.

It won’t be long before you’re looking to do something about it, but what’s the best way to get knee pain relief ? And should you see a doctor about it?

Knowing how to get rid of knee pain starts with trying to get to the bottom of why your knee hurts in the first place, as well as whether the pain warrants seeing a doctor or if there are things you can safely try at home first.

The most common causes of knee pain include: ■ Arthritis ■ Acute injury (to the meniscus, ligaments or cartilage) ■ O veruse (too much activity) ■ Growing pains “Knee injuries are extremely

common in recreational athletes, and they tend to be bothersome, as the knees are used for almost every daily activity, in addition to sports,” said Dr. Myphuong Phan, an orthopedics and sports medicine physician at Houston Methodist Clear Lake Hospital.

INJURIES TO TALK ABOUT

“In general, if you have an injury that causes new pain that exceeds normal soreness, results in the feeling that your joint is giving way, or leads to swelling, it is important to be evaluated sooner rather than later,” Phan explained.

The major signs you need evaluation for a new knee injury include: ■ Any trauma ■ Inability to fully straighten or bend ■ Instability (buckling or ‘giving way’) ■ Popping that causes pain ■ Swelling If you experienced an acute injury to the knee — felt or heard a pop, for instance — make an appointment with your doctor.

Pushing through injuries or pushing too hard can only make an existing injury worse or increase the risk of additional injuries.

“Even if your pain is minor and not improving, you need it evaluated,” Phan said. “If caught early, many times we can help guide a recovery process that allows you to get back to being active sooner.”

WEIGHT LOSS STRATEGY Every day we are bombarded with trendy diets, expensive programs and medications that claim rapid weight loss success. But do they work in the long run, and are they even good for you?

“Before taking any weight loss drug, seek the guidance of a bariatric specialist to help determine if a drug is the right choice for your weight loss goals and pre-existing health conditions. Medications do work for some, but you must have the right specialist prescribing them and monitoring you while taking them.”

SURGERY: A LIFELINE FOR CERTAIN INDIVIDUALS

“Surgery is the most powerful weight loss tool we have today and is a life-changing option for people who’ve struggled with obesity but have been unable to achieve sustainable weight loss through other methods,” Ali said. “To qualify for weight loss

surgery at Houston Methodist, patients must have a body mass index (BMI) of 35 or higher with an obesity-related condition such as diabetes, or 40 or higher without.”

Patients should get comprehensive intervention that extends more than a year after surgery. A team of dietitians, bariatric social workers and psychologists help make the weight loss results from surgery a lasting change.

Regardless of whether you choose a weight loss medication or surgery, your success is determined by long-term healthy lifestyle habits. Ali emphasizes that if you want to lose weight and maintain that loss, you should consult a bariatric specialist to determine the best course of action for you.

IS OZEMPIC A WEIGHT LOSS MIRACLE DRUG? Ozempic is a diabetes medication that helps patients control their blood sugar by curbing their appetite.

The brand name of the drug semaglutide, Ozempic, soared in popularity after a TikTok trend promised it as a weight loss hack, and celebrities extolled its success.

Now Ozempic is becoming harder to find for those diabetic patients who really need it. Because it’s a newer resource for weight loss, the long-term effects are still unknown. Some experts report gastrointestinal disorders such as nausea, diarrhea, vomiting and constipation as adverse effects.
