2017 ANNUAL REPORT

OUR MISSION Homage Senior Services’ mission is to promote independence, preserve dignity and enhance the quality of life through the provision of services for older adults and people with disabilties.

You Supported Our Clients When They Needed It Most

B

ecause of you, older adults in our community are able to live higher quality, independent lives. When you make a gift to Homage, you are supporting older adults and people with disabilities who may need more than one service. At Homage, we provide vital programs ranging from nutrition and transportation to home repair, wellness and social services. On a cold December afternoon, Mary was greeted at the hospital by Homage Senior Services. She explained to her case manager that she had recently gone through heart surgery and was getting ready to be discharged to a friend’s home. As the conversation continued, she expressed her concerns about leaving the hospital: transportation to appointments, rent and monthly

expenses, and keeping track of all her medical appointments and follow ups. Homage’s Care Coordination team assured her she was not alone and told her they would help walk her through the issues she faced. At the end of eight weeks, Homage staff helped Mary successfully connect with her primary care physician. She was being seen by her cardiologist regularly, and had begun attending cardiac rehabilitation sessions. She is now able to use Dial-a-Ride Transportation to get to and from her appointments. Mary has been approved for insurance through the Washington Health Exchange, reducing the cost barrier to afford her monthly medications. She received DSHS benefits while she recovered and is now able to go back to work. Homage

staff worked with Providence Home Health Services to help Mary apply for charity care to cover her medical debts. In addition, while working with Mary, her care coordinator recognized some common symptoms of depression and arranged a depression screening test. When the results came back, Mary was enrolled in short term counseling which is helping her to develop tools to build strength and move forward in her healing. She is now volunteering with other older adults and people with disabilities. Stories like Mary’s are not unusual here at Homage. Thank you for supporting vital programs and services that give our clients the independence and quality of life they deserve.
