The benefits to establishing advanced healthcare directives Healthy adults may give little thought to injuries or illnesses. When the going is good, it is easy to forget about the less pleasant side effects of aging. However, putting off these conversations and decisions can lead to unnecessary obstacles in the years to come. Advanced healthcare directives can be invaluable resources for family members and friends who will be responsible for managing a person’s plans should they become unable to make their own decisions. Various organizations, including AARP, offer resources on advanced planning and the options available to adults looking to plan their estates. The National Healthcare Decisions Day movement, a group dedicated to providing clear, concise and consistent information on healthcare decision-making, defines advanced directives as establishing: a healthcare power of attorney or the person you select to be your voice for your healthcare decisions if you cannot speak for yourself. a living will to document which medical treatments you would or would not want at the end of life. While these are decisions that people often put off, it’s important to make them as early as possible. Not only will they dictate your wishes, they’ll take the pressure off of loved ones who would otherwise be tasked with making difficult decisions on their own. Healthcare proxy Appointing a healthcare proxy ensures that there will be someone there who has the legal authority to make healthcare decisions for you if you are no longer able to

speak for yourself. This may be a spouse, child, relative, or close friend. The Mayo Clinic suggests choosing a person who can be trusted to make decisions that adhere to your wishes and values and to be your advocate if there are disagreements about your care. Be sure to have a candid discussion with your healthcare proxy in which you go over the types of medical care you wish to receive and any ways you would or would not like your life prolonged. It helps to keep the proxy up-to-date on any medical conditions you may have so that he or she can make the most informed decisions on your behalf. Having a healthcare proxy does not mean you are giving

The benefits to establishing advanced healthcare directives (Continued from Page 3) up your right to make medical decisions. It’s a fail-safe in the event you are unconscious or cannot direct medical care. Living will An advanced healthcare directive enables you to create specific written instructions for future health care, known as a living will. The living will should include wishes regarding life-sustaining medical treatments and resuscitation if you are no longer able to speak on your own behalf. It also can spell out whether you want to remain in a hospital or receive palliative care at home for a terminal illness. A living

will can be changed if/when your wishes or circumstances change. Legal and medical advice Although legal advice is not required for an advanced directive, it can be helpful to iron out the legalities of your directives. Speak with your doctor about your desires and needs. A physician can help you form a coherent directive that is in line with your wishes. Remember, an advanced directive is not reserved for the elderly. Any consenting adult age 18 or older can benefit from a living will and other documentation.

Adopting a dog or cat later in life Companion animals bring great joy to their owners. The unconditional love cats and dogs provide appeals to people of all ages. While many people associate pets with kids who can’t wait to welcome the first cat or dog into their homes, pets can benefit aging men and women as well. It’s not uncommon for seniors to feel lonely or depressed when they retire, their children move away or they lose a spouse or close friend or friends. The American Humane Society states that studies show pets help seniors overcome loneliness and

depression by providing affection, company and entertainment. Pets also provide much-needed mental stimulation, and many pet owners find their pets help them become more physically active as well. Seniors who adopt pets may also feel a sense of purpose when helping animals who may not have anywhere to live. This is particularly true of older companion animals, which many young families are understandably hesi(Continued on Page 8)

tant to adopt. Mature pets might be an ideal fit for seniors. When seniors are looking to adopt a pet, there are various reasons why older pets or particular animals might be the perfect fit for them. • Adult pets may already be house trained, saving seniors the trouble and effort of training them. • Seniors may find cats fit their lifestyles more than dogs, as cats are less active and do not need to be walked or played with as much as dogs. Cats also are small and easily maneuverable, meaning even seniors who have arthritis or other physical limitations can easily care for cats. Many cats are also content to spend long periods of time sleeping on their owners’ laps. • Small dogs that can be active within the house might be a good idea as well, especially for seniors with mobility issues. They’re also easily transported to and from vet appointments. It’s important that seniors carefully weigh the benefits of adopting a pet against any limitations they may have. Having a backup plan for care is advantageous as well. Seniors should not adopt a pet if they anticipate frequent travel or medical care that requires they be away from home for long periods of time. 8 Life Af ter 50 2018

How senior donors can avoid scams Con artists masquerading as charities can victimize anyone, but seniors are especially vulnerable to such criminals. Men and women who perpetrate charitable fraud often target older men and women, feeling that seniors are more likely to have a significant amount of money in their bank accounts and less likely to report crimes for fear of embarrassment. But even if seniors are prime targets for charitable fraud, they still can take steps to safeguard themselves from scammers looking to exploit their generosity. Never donate over the phone. No reputable charity will insist on a donation over the phone. When a person calls and says they represent a charity, they should be willing to send you information in the mail so you can further explore their organization before making your final decision. If a caller attempts to pressure you into donating over the phone, hang up. Keep a list of organizations you have donated to in the past. Many people who perpetuate charitable fraud reference a past relationship or donation history between their victims and the charity the criminal is claiming to represent. Maintain a list of your donation history, including the organizations you have donated to and the amount of those donations. This can help you verify if the person calling, emailing or visiting you is telling the truth. Do not feel obligated to give. Many charities send gifts to their donors as a token of appreciation for their support. Some criminals masquerading as charities will send gifts to potential victims in an attempt to fool them into writing a check. Whether a charity is legitimate or not, seniors should not feel obligated to donate simply because they received a gift. If you receive a gift and want to donate, confirm that the organization that sent you the gift is legitimate and not an imposter organization that is baiting you with a gift. If the gift was accompanied by a donation form, do not fill out that form, choosing instead to make a direct donation via the organization’s website, a site you should find independent of the mailer/gift you received, or mail a check directly to the organization after you independently confirm its address, be it through your own research or by con-

tacting the Better Business Bureau (www.bbb.org). Insist that your personal information is not shared. Before making a charitable donation, donors are often notified that their contact information is likely to be sold to or traded with other charities. This can lead to an influx of charitable solicitations that elderly men and women may find overwhelming. So prior to making a donation, speak directly with the charity and insist that none of your information be shared with other charities or organizations. If this cannot be arranged, find an organization willing to make such a concession and contribute to that charity instead. Ask for help. If you are confused when it comes time to make your annual donations, ask a son, daughter or trusted confidante for help so you can rest easy knowing your donation goes where you intend for it to go.

Understanding, preventing and managing osteoarthritis The most common chronic condition of the joints in both the United States and Canada, osteoarthritis affects roughly 30 million people in just those two countries alone. While osteoarthritis, or OA, can affect people of all ages, it’s most common in men and women over the age of 65. Understanding osteoarthritis and how to prevent and manage the disease can help men and women over the age of 50 reduce their risk and live more comfortably even if they develop OA. What is osteoarthritis? According to the Arthritis Foundation, healthy joints are covered by cartilage, a flexible connective tissue that covers the end of each bone. Cartilage facilitates motion of the joints and serves as a cushion between the bones. When a person has OA, cartilage breaks down, causing swelling and pain and affecting the mobility of the joint. Over time, OA can worsen and

10 Life Af ter 50 2018

cause bones to break down and develop bone spurs, which form when bones meet each other in the joints. OA can even advance to a point where cartilage wears away and bone rubs against bone, creating even more pain while damaging the joints even further. What causes osteoarthritis? Once considered a byproduct of the wear and tear the human body naturally endures over a lifetime, OA is now viewed as a disease, notes the AF. The following are some potential causes of OA. • Genes: The AF notes that certain genetic traits can increase a person’s likelihood of developing OA. Collagen is a protein that makes up cartilage, and, while rare, a genetic defect that affects the body’s production of cartilage can lead to OA occurring in people as young as (Continued on Page 12)

strength training that focuses on building muscles around the joints, even if those joints are already affected by OA. Strong muscles around the joints can reduce the pain associated with OA, while range-of-motion exercises can improve flexibility of the joints and reduce stiffness. Aerobic exercise also helps men and women maintain healthy weights while facilitating weight loss for those who are already overweight. Those already diagnosed with OA should speak with their physicians before beginning an exercise regimen, and such conversations can also include discussions about the various medications that can be used to reduce symptoms of OA.

• Overuse: Overuse of joints, tendons and ligaments can accelerate the break down of cartilage and increase a person’s risk of developing OA. Cartilage also can break down more quickly in the bodies of athletes and people whose careers require them to stand for extended periods of time, bend over frequently and/or lift heavy items. • Preexisting conditions: Conditions such as rheumatoid arthritis, hemochromatosis and acromegaly may also contribute to the development of OA among people diagnosed with such disorders. Seniors becoming more tech-savvy Technology is the future, and digital communication has opened many doors for people around the world. Although younger generations have grown up with technology at their fingers, Baby Boomers and older adults did not. But in spite of that, studies show that growing numbers of seniors are open to the idea of technology and even seeking ways to further their use and knowledge. According to a 2014 study by Pew Research Center, 59 percent of seniors regularly use the internet Ñ a 6 percent increase from the previous study conducted in 2012. Today, 67 percent of adults age 65 and older say they go online. Pew also says that, although seniors consistently have lower rates of technology adoption than the general public, four in 10 seniors now own smartphones, which is more than double the amount that did in 2013. Seniors in Australia are especially tech savvy, as Deloittes mobile consumer survey found 78 percent of Australian seniors aged 65 to 75 own a smartphone, up from 69 percent in 2016. While stereotypes have long painted seniors as technologically inept, seniors are actually more socially and digitally engaged than ever before. Seniors use technology in many different ways. Some use mobile apps to manage medications and doctor’s appointments and monitor their fitness regimens. Some families employ 24/7 alert systems or smarthome technology to keep seniors comfortable and safe at home for as long as possible. Noninvasive, smart technology can analyze factors such as whether or not doors are left open, if there has been movement in a home, or whether appliances/lights are on or off. This represents a great way for families to stay informed and provide assistance even if they are not nearby. SilverSurfers, a senior-based information website, says other tech that seniors are embracing includes online dating; audio and digital books; online shopping, which is especially valuable to seniors who have mobility issues; and social media, which can keep seniors connected to others and feeling less lonely. A study conducted by researchers from the University of California, San Francisco found 18 percent of American seniors live alone, and 43 percent report feeling lonely on a regular basis. Loneliness can increase death risk. Social media and internet connectivity can be an important tool in helping seniors feel like active members of society. Technology is no longer just for teenagers or active

workers. Seniors are increasingly embracing technology and becoming a fast-growing demographic for tech usage. Although younger generations have grown up with technology at their fingers, Baby Boomers and older adults did not. But in spite of that, studies show that growing numbers of seniors are open to the idea of technology and even seeking ways to further their use and knowledge.

Power of attorney protects loved ones Life is full of the unexpected. But just because the future is unpredictable does not mean adults cannot prepare for what lies ahead. Estate planning is important, and establishing power of attorney can be essential for men and women looking to protect their financial resources and other assets. What is power of attorney? A power of attorney, or POA, is a document that enables an individual to appoint a person or organization to manage his or her affairs should this individual become unable to do so. According to the National Caregivers Library, POA is granted to an attorney-infac or agent to give a person the legal authority to make decisions for an incapacitated principal. Why is power of attorney needed? Many people believe their families will be able to step in if an event occurs that leaves them incapacitated and

16 Life Af ter 50 2018

unable to make decisions for themselves. Unfortunately, this is not always true. If a person is not named as an agent or granted legal access to financial, medical and other pertinent information, family members’ hands may be tied. In addition, the government may appoint someone to make certain decisions for an individual if no POA is named. Just about everyone can benefit from establishing an attorney-in-fact. Doing so does not mean men and women cannot live independently, but it will remove the legal barriers involved should a person no longer be physically or mentally capable of managing certain tasks. Power of attorney varies Power of attorney is a broad term that covers various aspects of decision-making. According to the legal resource Lectric Law Library, the main types of POA include general power of attorney, health care power of attorney, durable power of attorney, and special power of attorney. Many of the responsibilities overlap, but there

Kelly Smile Implant Dentistry Dental implants are a permanent and appealing solution to replace missing or extracted teeth. They are preferred above alternatives like bridges because no additional teeth need to be altered to place the new tooth. At Kelly Smile, our caring team is ready to give you implants for a natural smile. How Do Dental Implants Work? A normal tooth is typically one piece that features two parts: the crown and root. However, a dental implant involves installing these parts separately. First, the implant is custom-made to fit the shape of the natural crown. Then, an implant is put in that becomes the root. Implants are made of titanium alloy or titanium, which do not have any health implications. What Can You Expect Before and After the Procedure? The entire implant process is performed over the course of a few months. The first part of the process is to install the implant itself, where a screw is placed into the jawbone. Bone grafting may be necessary, which is important for patients who have a jawbone that is too soft. Next, an incision is made in the gum so that the implant can be inserted. Multiple implants can be placed

18 Life Af ter 50 2018

at once if necessary. After the implants are placed, the gums are sutured. The implant must be allowed about 3-6 months to heal, and during this time the jawbone will form around it in a process called osseointegration. During this healing time, temporary crowns can be installed so that eating and speaking can remain normal. After the implant has healed, it is time to place an abutment on the implant. The abutment serves as the base for the new tooth. Once this is placed, an impression of the abutment is taken and is used to create the final implant. A laboratory is utilized to create the crown. Once the recovery is complete, you can return to the office to attach the implant permanently. At each stage of the surgical procedure, the patient is expected to eat soft food to allow the sites to heal. If swelling or other problem gets worse the days following the surgery, contact us for an additional follow-up. With implant dentistry at Kelly Smile, your smile will look just like it used to. After a short period of getting used to the implant, it will feel just like one of your own teeth! 12120 Ridgecrest RoadSuite #203 Victorville, CA 92395 (760) 242-2338

What is a silent stroke? The brain is a complex organ responsible for controlling many different bodily functions. When working at optimal capacity, the brain is a wonder to behold. When illness or trauma affects the brain, various parts of the body may not work as they should. One of the more devastating things that can affect the brain is stroke. Stroke describes a sudden stoppage of blood from reaching the brain. Harvard Medical School states that if a large number of brain cells are starved of blood supply, they can die. With their demise, a person’s memory and ability to speak and move can be compromised. While many strokes come on suddenly, certain factors may indicate a person is at risk. Such factors may include prior heart attacks, genetics, high blood pressure, smoking, or a prior stroke. However, in a particular type of stroke a “silent stroke” symptoms are far more subtle and difficult to spot. Silent cerebral infarction, often

referred to as ÒSCIÓ or silent stroke, is a brain injury likely caused by a blood clot interrupting blood flow to the brain, offers the American Stroke Association. Silent strokes increase risk for other strokes and can be a sign of progressive brain damage. A silent stroke is typically only noticed as a side component of an MRI of the brain. Many times patients do not recall having a stroke and never felt any symptoms. Silent strokes should not be mistaken for mini-strokes. Mini-stroke is a brief but discrete and memorable event, with symptoms appearing for a few minutes or a few hours. According to a study on silent stroke titled Functional and Cognitive Consequences of Silent Stroke Discovered Using Brain Magnetic Resonance Imaging in an Elderly Population and published in the Journal of American Geriatrics Society, silent strokes are quite common and can have serious consequences. Researchers have found that silent stroke is associated with impairments in tests of cognitive function rather than movement-oriented performance tests like rising from a chair. Almost 50 percent of studied silent strokes affected frontal circuit components of the brain, such as the frontal cortex, basal ganglia and thalamus. Lesions in these brain structures compromised executive functions and were related to vascular dementia. Another study showed associations between silent stroke and visual field deficits, weakness in walking on heels, history of memory loss, migraines, and lower scores in (Continued on Page 22)

cognitive function tests. The silent part of a silent stroke also refers to the areas of the brain that the stroke affects. Experts at Harvard Medical School explain that, during a silent stroke, an interruption in blood flow destroys areas of cells in a part of the brain that is silent, meaning that it doesn’t control any vital functions. Researchers say that, over time, the damage from silent strokes can accumulate, leading to more and more problems with memory. Collectively, silent strokes become silent no longer. There are certain ways to reduce the risk of any type of stroke. These include: 22 Life Af ter 50 2018

• reducing the risk of diabetes and effectively treat the condition if it is present • losing weight to prevent obesity

• exercising and avoid a sedentary lifestyle
