WHAT ARE SOCIAL SUPPORT GROUPS?

Social Support Groups provide the opportunity to:

Social Support Groups provide regular socialisation and activities for eligible consumers. The program supports clients with wellness and reablement and focuses on a persons abilities and strengths.

Socialise and meet new friends

Enjoy games, quizzes, sing-a-longs and be entertained by guest speakers

Social Support Groups follow a designed program with input from all participants. Our staff are well trained, supportive and are there to assist with identified needs. Staff hold a minimum qualification of a Certificate III in Aged Care and/or Leisure and Lifestyle qualifications.

Participate in fun and tailored exercise programs

Enjoy bus trips and outings

A Social Support Group day consists of: 

Affordable fees are set in line with government guidelines with consideration to the ability to pay.

Morning tea, lunch and afternoon tea

Consumers 65 years and over (50 years and over if Aboriginal or Torres Straight people) are eligible. Those under 65 with a disability may also be eligible. (per eligibility guidelines) Referrals are received through My Aged Care (MAC) for those 65 and over. Anyone can phone MAC to refer to the Hesse PAG program (yourself or your GP/family member/friend, with your consent) Phone 1800 200 422. Hesse can also assist with enquiries by phoning 52671200. Referrals for those with disabilities (under 65 years) can be made directly to Hesse and include self referrals, referrals from GP’s and other health professionals and referrals from family /friends with consent from the consumer.
