Outcome

Immunization Records

A medical outcome can be good or bad.

A poor outcome such as death or severe illness from getting COVID-19 depends on many factors, including pre-existing medical conditions, age, and whether or not one has received any vaccinations against COVID-19.

A paper or digital record of your vaccination history.

Health Care System Capacity Can include human resources (healthcare workers), hospital beds, and medical equipment such as ventilators. In B.C., “capacity” usually refers to human resources and space. Since the COVID-19 pandemic, Canada’s hospitals and healthcare organizations have been operating at 100%-120% capacity. In addition to physical capacity issues, Canada also has a serious shortage of healthcare workers.1

Layers of Protection In a COVID-19 context, layers of protection include wearing a mask, maintaining a minimum 6-foot distance from others, not touching your face or nose, and washing or sanitizing your hands frequently.

Immunization Phases Times in the calendar year for different people to be eligible to be vaccinated. B.C. has four phases: Phases 1 and 2 (December 2020 - April 2021) for the “High Risk Population” and Phases 3 and 4 (April June 2021) for the “General Population” (over the age of 18).

Community (Herd) Immunity When a population is immune either through vaccination or immunity developed through previous infection.

The World Health Organization (WHO) “supports achieving ‘herd immunity’ through vaccination, not by allowing a disease to spread through the population, as this would result in unnecessary cases and deaths.”2

Antibodies Y-shaped cell proteins that attach themselves to a virus, for example, and signal the body to produce an immune response. COVID-19 vaccines give instructions to cells to make a protein that is unique to the virus that causes COVID-19.3

Clinically Extremely Vulnerable When someone has a medical condition that may lead to a high hospitalization and mortality (death) risk. This includes, but is not limited to, cancer, kidney disease, and other immunocompromised conditions.

Risk-Benefit Ratio (or Analysis) Balancing the potential harm (risks) of a choice or situation with the known benefits (positive effects). When considering COVID-19 vaccines and vaccination, the risks can be unique to the individual, their medical situation, and/or whether they are living in a high transmission or low transmission area.

Island Health The Health Authority region that includes all of Vancouver Island, the Gulf Islands, other surrounding islands, and Victoria.

During the current provincial restrictions, non-essential travel outside your Health Authority is prohibited.

Transmission When a virus is spread from one person to another. COVID-19 spreads from an infected person to others through respiratory droplets and aerosols (smaller droplets) created when an infected person talks, sings, shouts, coughs, or sneezes. COVID-19 can also spread by touching something that has the virus on it, then touching your mouth, nose or eyes with unwashed hands. You can transmit COVID-19 before you start showing symptoms or without ever developing symptoms.4

Health Gateway A website that provides a single place for B.C. residents to access their health records. After getting your COVID-19 vaccine, you will also be able to access a digital copy of your COVID-19 vaccine record by registering for B.C.’s Health Gateway5: www.healthgateway.gov.bc.ca

COVID-19 and COVID-19 Vaccines in British Columbia: an Interview with Dr. Srinivas Murthy Interview by Fiona Bramble

Dr. Srinivas Murthy is a clinical associate professor in the UBC department of pediatrics, an infectious disease specialist and investigator at BC Children’s Hospital, and Health Research Foundation of Innovative Medicines Canada Chair in Pandemic Preparedness Research. As part of a vaccine education project to reach newcomer and immigrant communities in partnership with REP Here In Canada, Masjid Al-Iman and the Victoria Foundation, Dr. Murthy spoke with Managing Editor Fiona Bramble about COVID-19 and COVID-19 vaccines in late April 2021.

I’m very sorry to hear about the recent loss [due to COVID-19] of such a young patient at BC Children’s Hospital. What kind of impact has that had on you? Any death is hard, and especially the death of a child is hard in general. It speaks to how nasty this virus is and how much we need to do to make everyone safe.

Do you think it’s had an impact on the wider community in terms of their behaviour?

I’ve gotten a lot of questions since it was announced, both from my friends and family, and from the media. Yes, it has affected people. It was a young patient.

We always presumed that this is a disease affecting older people. Not that that should change how we perceive the disease.

However, it definitely hits us in a different 4

way when we start to think that children are susceptible. How it affects people’s behaviour is difficult to say. Individual motivation is a tricky thing to predict and understand. We all have our own drivers.

What led you to this field of work? I have always been interested in emerging infections—things that come out of nowhere, and trying to figure out how to learn about them quickly, especially how to manage major outbreaks better. That’s always been my “thing,” both research and operational: Ebola, hemorrhagic fevers, and diphtheria—all these diseases that pop up in different countries. I would go with the WHO (World Health Organization) or MSF (Médecins Sans Frontières) so I could do research or clinically manage patients.

More generally in your COVID-related work, what kind of questions are people struggling with? Usually, it’s a “should I in this X,

Y, Z situation get this vaccine?” A lot of “Can I do this? (which public health seems like it doesn’t want me to do) But can I do it anyway?” I get that question quite frequently. Like, “Can I go camping?” or “Can I go to the mall?” or have a birthday party or whatever. Day-to-day stuff, those are the common questions that emerge that relate to the public more broadly.

Obviously, clinical questions and diseasebased questions are part of my workday, but when I’m talking to friends or family what have you, it’s pretty much: “can I do this?” and “vaccine this?”

Many newcomers to Canada still rely on medical advice from their family physicians back in their countries of origin, and sometimes that advice challenges or contradicts medical or public health guidance here in Canada. How do you navigate this?

Obviously it’s complex. Different people have different perspectives, so it’s nice to

pick up a phone and have a conversation to assuage any concerns or questions that come up. I never want to say we know best in Canada and they don’t know anything over there, wherever there is, because that would be ridiculous. But it’s important to work through the process and find out where people are coming from.

It is being repeated that “any vaccine is better than no vaccine.” Is that absolutely true, always and in every case? Yes, it is, but everything depends on something.

If there is disease transmission in your community, then any vaccine is better than no vaccine. If there are very, very low rates of transmission, like, for example, in New Zealand right now, they are not in any hurry to vaccinate. They are taking their time. They have only Pfizer and are doing that across their population at a leisurely pace. They’re not saying any vaccine is better than no vaccine. They are saying we have one vaccine and will we vaccinate over a period of six months. We are nowhere near that [low rate of transmission]. We in B.C. are saying that, based on disease transmission risk, any vaccine is better than no vaccine. Newfoundland and P.E.I. could perhaps consider it from a New Zealand perspective as well. They have that flexibility and opportunity. We don’t.

Our community has many specific questions about vaccines, such as: are the vaccines safe for pregnant women and their unborn children, as well as for nursing mothers? As far as we know—and we have had 10s of 1000s of people vaccinated who are pregnant—they have no higher rate of adverse effects. We are saying, most societies are saying, even though Health Canada and the regulators can’t say it formally yet, it is an individualrisk decision. My recommendation is that it is safer to get the vaccine than not.

Does a vaccinated mother transfer COVID-19 immunity to her nursing infant?

We’ve seen a few studies looking at that and it actually seems that the infant does

have antibodies. Remember also that some types of antibodies pass from [a pregnant] mother, through the placenta, to the baby.

If mom has enough antibodies, they will protect the baby. Not that babies are at high risk of getting the disease, but it’s nice to get that protection.

And that is also true if the mother had had COVID, correct? Yes, but it’s a different risk profile. Pregnant women with COVID-19 tend to do poorly. On the balance of risk, getting the vaccine is much, much better than getting the disease if you want your neonates to be immune.

Teachers are considered essential workers?

Yes.

Babies are at low risk. Is that connected to why there doesn’t seem to be any urgency to ensure the current vaccines are safe for youth under 18? There will be vaccines available for children, young children. It’s just not a priority for anybody right now; everyone is focused on making sure the people most vulnerable from getting the disease and getting severe disease are protected. We won’t be able to get to “herd immunity” until we vaccinate children, but before we get there, we still have to protect people and prioritizing adults is totally rational.

That is because younger people’s outcomes are better when they contract COVID? Yes, there are very low rates of severe disease [in younger people].

Are essential workers being vaccinated through programs at their workplace?

Yes, many are being vaccinated through the workplace.

What percentage of the population will need to get vaccinated to achieve “herd” or “population” immunity from COVID-19?

Our definitions of what we need for “herd immunity” vary depending on what study you look at, how you define the herd and so on. It could range anywhere from 50-95 percent.

What do you mean by “def ine the herd?”

What is the population? Where are we drawing our lines? Is there a herd in the United States? A herd in Canada? A herd in British Columbia? In reality, you need a big herd. For example, we have measles “herd immunity” but we still have outbreaks and cases because people travel to places that do not have herd immunity. Vulnerable people travel back and forth and there will be outbreaks and problems. Saying that we have herd immunity in British Columbia isn’t particularly useful.

What vaccines are available in BC right now? Pfizer, Moderna, and AstraZeneca. Johnson & Johnson has not been rolled out here yet, but should be here shortly.

And we don’t know yet what that “herd immunity” rate is for COVID-19? There is a range of possibilities for what proportion of the population needs to be immune for it to be protective for everybody, and it depends on a huge host of things.

What are the priority populations for vaccinations? They include age-based, clinically-extremely vulnerable, and Indigenous populations, as well as essential workers. If you are vulnerable for a bad outcome, you should be vaccinated. If you are vulnerable for getting the disease because you can’t appropriately protect yourself because you have to work to

Does a single vaccine dose prevent transmission of COVID-19? Primary studies did not look for whether the vaccine prevented transmission. They showed that the vaccine prevented disease in the individual. Every vaccine has prevented transmission in that if you have less incidence of disease you can plausibly think that you will have less transmission 5

along with it. Since those initial studies of these vaccines, we’ve gotten some good studies that show that onward spread is slowed by people who are vaccinated. So I feel very confident that the vaccines likely do slow transmission. Do we know it proof-positive? No, we don’t know lots of things proof-positive. If it prevents disease after a single dose, then it should prevent transmission after a single dose. The amount of protection on spread we can’t say with a number, but we have to presume that it is there.

AstraZeneca was pulled from the vaccine rollout, and then reintroduced for certain age groups. Obviously, this created some confusion and mistrust of public health messaging. Given that the adverse outcomes are so rare and mainly affect women within a certain age group, would it not make sense to have a gender-based policy? It’s a tricky risk communication process as to how we say who is at risk and who is not at risk. It seems—and we’ve accumulated a ton of data so far—that you are right, that people of a certain age tend to be at higher risk. It’s still an exceptionally low risk in that population that we don’t want to say it’s dangerous for those people and it’s safe for these people because it’s safe for those people as well. It’s a good vaccine. It has a small rate of adverse events following it, which are serious, and something we need to pay attention to and have a conversation about how we balance risks and benefits. However, on the balance of risks and benefits right now, given our situation, the benefits hugely outweigh the risks.

You understand that different messaging about the safety of some of the vaccines is confusing to the public? I get asked that a lot. I think from a trust perspective and the hesitant folks perspective, in the long run, it’s actually going to be better that we did it this way. That we paused, the country paused, looked at the data carefully, looked at the accumulating data from around the world, and shared information. Then came 6

up with a pretty confident number of events per million vaccinated people and said this is the risk; it’s a very low risk. This is the problem and then balancing those risks to individuals. I think if we knew those things were happening—and the public knows— and we did nothing about it, then I think the outcry after a death or two in Canada directly related to the vaccine would be a big problem. These are all unknowns at this point and I’m making my best guesses how to communicate to people and it’s hard.

Do you have any concerns about people with autoimmune conditions? No, I don’t.The rates of clots are not higher in those with autoimmune conditions than those without autoimmune conditions.

Do you predict that once the vaccine supply is stabilized and we move into a post-pandemic state, that this will become a booster shot scenario with this particular virus? It’s likely. There are all sorts of variants that are emerging, so whether we’ll need a boost for the main disease, or whether we need “upping,” so to speak, for different variants that have emerged and become the dominant strains around the world is difficult to say. It’s an unknown. The PfizerModernas of the world are banking on the fact that there will be re-vaccinations happening. But we don’t know exactly what they’ll be for and why we’ll do them or how often we’ll do them, but I think it’s a high degree of probability that’s going to happen.

If we don’t vaccinate youth, around 15% of the population, and assuming an additional hesitant 10% are not vaccinated, that’s only 75% of the population vaccinated. Do you see that as a problem? Of course, yes. Every person who doesn’t get vaccinated is a problem, and it is worth a conversation to try to figure out why. It’s well worth the time and effort.

You mention that youth will be vaccinated eventually; they are just not a priority right now. Do you think youth will be vaccinated by

September? I look at the world, and I think who needs a vaccine more, and I can’t say the youth of British Columbia should be high on that list.

Does your second dose of vaccine need to be from the same manufacturer or just be the same type of vaccine? For now, we are saying same type, same manufacturer. That may change. Different places are doing different things. There might be a possibility of getting a second dose of a different vaccine as data emerges. Different places have been trying that. There’s no reason to think that it shouldn’t work.

Are there risks in delaying the second dose beyond the recommended timeline? I think we don’t know exactly what the risks are with that—like, does our immunity wane and we’ll not be protected for a period of time, or not have as robust a protection?

But in the balance of things, more people getting vaccinated means more people get protected and that is, in my mind, an easy trade-off to make.

Both are adenovirus vector vaccines, so why is the Johnson & Johnson a single dose and the AstraZeneca not? It’s the way they studied the vaccine mostly. One of them studied it to be a single dose and one of them studied it to be a 2-dose. They are similar vaccines but slightly different and it’s a matter of how they built the immunity system within it. Once Johnson & Johnson lands, it’s going to be great.

Are there any other promising vaccines under review right now? They are all promising and exciting to see. Pfizer, Moderna, AstraZeneca, and Johnson & Johnson will be the backbone of our program here, but Novavax is probably going to roll out at some point in the near future as well, in Canada and elsewhere. They’re all great.

I’m excited by the Cuban vaccine. They’ve explicitly designed it to be made for export and made with open access, so anyone can make it. They’re designing it for the

developing world specifically, so I think that’s a cool way of getting their product out there.

against them are the same. Vaccinate and stay away from others who can give you the disease.

Side effects post-vaccinations seem varied.

Some people are down with aches and fever for a couple of days, others completely normal.

People who have had the same vaccine, but totally different after effects. Can this be explained? Why would one person have a stronger reaction than another? We have data from other vaccinations that show a fever day-one post-vaccine or more symptoms you have, you might have a slightly higher antibody level than those who didn’t have that level of reaction. Does it mean anything for these COVID vaccines? No one knows.

There are a thousand reasons why someone might have a stronger reaction than another: genetics, exposures in the past. Our immune system is a complex beast.

The curve in B.C. is finally trending down ever so slightly since the latest restrictions.

Does that give you some confidence that we are going to get ahead, buy some time to get the vaccinations rolling out? I have a marginal amount of hope that our health system won’t collapse. I’m still fairly anxious about the next few weeks and how that is going to look in our health system.

Are there any lesser known side effects or reactions post-vaccination that might surprise people? Fever and muscle aches are the main ones—general malaise for a day or two and then you get back up to speed.

Should transmission prevention best practices be changing at different stages of the vaccine process? For example, can I relax my “bubble” if I’ve been vaccinated? We are not at a place yet—only 25-35% of the population has had one dose—where we can comfortably say those people are protected enough to roam free and do whatever they want, even with each other, and that disease in the community is not going to affect them.

We’re not there yet.

Are there any other symptoms postvaccination, besides obviously, an anaphylactic reaction, that should trigger a visit to a doctor or Emergency? The AstraZeneca symptoms could happen within two or three weeks of getting vaccinated. It’s very rare. 1-3.2 out of a million. Basically, you might have a headache, you might feel swelling, you might feel short of breath. You should get checked out.

And that’s because a. even though the vaccine likely slows or prevents transmission, there aren’t enough people who have had the vaccine yet, but also b. that you can still contract COVID even though you’ve been vaccinated?

Yes, the vaccine is not perfect; you can still have the disease. It doesn’t mean that we will never be able to completely open up because that’s ridiculous. But what it does mean is that while we’re having widespread community transmission, having a small number of people be lax about things because they’ve gotten one vaccine and others can’t do those things is inequitable in a way, and also the actual protection you get from that first dose is unknown.

How do the current vaccines’ effectiveness against the prevailing variants in B.C. seem in terms of preventing infection and mitigating serious symptoms? Good. The vaccines work. Focusing on variants and things is important for us to control them and make sure we understand them, but the strategies

lands on the ground. “Airborne” means the virus itself can sit in the air for a prolonged period. As it sits in the air, it can travel on the current and then go somewhere in the room, in the building through the ventilation system, and so on. When you think about building design and where you are safe or not safe, droplet vs. airborne makes a substantial difference. There is also a conversation about the size of the droplet and the particles itself and what kind of mask works best to protect you. I think all of these situations are important to consider.

I’m not going to say it’s airborne and we all need to wear N95s in all circumstances and every building should have its ventilation upgraded, but I will say we should upgrade our ventilation.

Is there any current COVID-related research out there right now that excites you?

Obviously, the vaccine research. It’s coming fast and furious. Seeing how effective these vaccines are pretty much consistently in pretty much every population is obviously super-exciting to see—how quickly and effectively they were scaled up and able to do the things they have. Obviously, there is a long, long way to go to get to the place we want to be, but we have the tools to get there.

Are you seeing any race-based data in the vaccine research you are seeing? Yes, and no. Many studies don’t do a great job of collecting race-based data and different places in the world define things in different ways. So, race-based data oftentimes becomes challenging to interpret across regions and populations. We’re seeing consistent non-differential effects of the vaccine. I think that bad outcomes are more related to underlying socioeconomic factors and racism.•

Messenger RNA, or mRNA, is genetic material that tells your body how to make proteins.

Coronavirus, like the one that causes COVID-19, are named for their crown-like spikes on their surface, called spike proteins.

These spike proteins are ideal targets for vaccines.

The only contraindications to the vaccines are a life threatening allergic reaction to any of the vaccine components.

The risks of COVID-19 are much greater than any risks from vaccines. COVID-19 can have serious, life-threatening complications, and there is no way to know how COVID-19 will affect you. And if you get sick, you could spread the disease to friends, family, and others around you.

COVID-19 vaccination will be an important tool to help stop the pandemic. Stopping a pandemic requires using all the tools we have available.

None of the COVID-19 vaccines can make you sick with COVID-19. None of the COVID-19 vaccines contain the live virus that causes COVID-19, so a COVID-19 vaccine cannot make you sick with COVID-19.

All pregnant people who are age 16+ are now eligible to receive the COVID-19 vaccines as a priority population through B.C.’s Immunization Plan. There is no need to wait until your age group is being vaccinated.

Please also register at gov.bc.ca/getvaccinated (if you haven’t already), to ensure you get a call back for your second dose.

Getting the COVID-19 vaccine if you are pregnant, planning to become pregnant, or breastfeed, is a safe and effective way to protect against COVID-19. Pregnant women and pregnant individuals who have COVID-19 appear more likely to develop severe complications requiring intensive care admission than women and individuals who are not pregnant.

The recommendation to get vaccinated during pregnancy is supported by the Society of Obstetricians and Gynaecologists of Canada (SOGC), the National Advisory Committee on Immunization (NACI) and public health experts in B.C.

Pregnant people are at increased risk for severe illness from COVID-19 and may be at risk for adverse pregnancy outcomes. If you are pregnant and your work or personal activities place you at a high risk for COVID-19 infection you should discuss the benefits and risks of the vaccine with your healthcare provider.

I am either pregnant, planning on getting pregnant, or breastfeeding. Is it safe for me to get my COVID-19 vaccinations?

Answer: Yes. The Canadian Society of Obstetrics and Gynecology (SOGC) has released the following consensus statement: “Pregnant individuals who are pregnant or breastfeeding should be offered vaccination at any time if they are eligible and no contraindications exist.” This decision is based on an understanding that the risk of infection and/ or morbidity from COVID-19 outweighs the theorized and undescribed risk of being vaccinated during pregnancy.

If I am vaccinated and breastfeeding, do I pass on COVID-19 immunity (antibodies) through breastfeeding?

Answer: Previous studies looked at mothers who were infected with COVID-19 themselves, but emerging data has also looked at whether vaccinated mothers may be able to pass along antibodies. Several recent small studies have shown that antibodies can be passed from mothers to their infants through breastfeeding. So, getting vaccinated while breastfeeding not only protects the mother, but could also protect the baby too. However, whether antibodies passed by a vaccinated or an infected parent give a child sufficient protection, and whether that protection lasts, are questions that still need to be answered.

The U.S. Food and Drug Administration has expanded the emergency use authorization* (EUA) for the PfizerBioNTech vaccine for the prevention of coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) to include adolescents 12 through 15 years of age. The FDA amended the EUA originally issued on Dec. 11, 2020 for administration in individuals 16 years of age and older.

People with a history of severe allergic reaction to any component of the vaccine should not take it.

The BC Centre for Disease Control states: “Health Canada has approved the use of the Pfizer vaccine for youth aged 12 to 17.” On May 20, 2021, B.C. Health Minister Adrian Dix announced that children aged 12-17 could now register for the COVID-19 vaccine in British Columbia.

Young people can register themselves and book their appointment. Parents, guardians and trusted adults are also able to register and book for young people, with booking invites sent to those who are older first. Most vaccine appointments for young people age 12 to 17 years will take place at the same immunization clinics used to vaccinate people in other age groups.

Household and group vaccination options are also available to make it easy for households to be immunized at the same time. Multiple young people in a household can attend one booked appointment and be vaccinated together.

Young people accompanying adults to the adult’s new vaccine appointment can register on site and do not need to be registered or booked in advance. Just let clinic staff know you have arrived.

*An Emergency Use Authorization (EUA) is a mechanism to facilitate the availability and use of medical countermeasures, including vaccines, during public health emergencies, such as the current COVID-19 pandemic. Under an EUA, FDA may allow the use of unapproved medical products, or unapproved uses of approved medical products in an emergency to diagnose, treat, or prevent serious or life-threatening diseases or conditions when certain statutory criteria have been met, including that there are no adequate, approved, and available alternatives.

Weakened Immune Systems COVID-19 vaccines are not live vaccines, and there are no significant concerns about safety for those with weakened immune systems. It is possible that the vaccine may not work as expected in people who have a weakened immune system. If you have questions and have a weakened immune system, speak to your healthcare provider about the COVID-19 vaccine.

Autoimmune People with autoimmune conditions may receive a COVID-19 vaccine. However, they should be aware that no data are currently available on the safety of COVID-19 vaccines for people with autoimmune conditions.

Post-COVID-19 Disease In general, it is safe to get the vaccine once you’ve fully recovered from COVID-19. Long haulers is a term given to people who have had COVID-19 disease, are no longer contagious, but still have symptoms.There is no evidence that a COVID-19 vaccine will make people who are long haulers sicker or at risk of more complications from the disease, and they should receive a COVID-19 vaccine whenever they are eligible.

People who are sick with COVID-19 should wait until they have recovered from acute illness and public health has told them they no longer need to isolate. It is important that you wait the full isolation period before getting the vaccine so that you do not expose people at your vaccination clinic to the virus.

If you had COVID-19, you should still get both doses (if it’s a two dose series) of the vaccine once you have recovered.

This is because you may not be immune to the virus that causes COVID-19 and you could get infected again. You do not need to get a COVID-19 disease or antibody test before getting your vaccine. However, as vaccine quantities are currently limited, those who tested positive for COVID-19 within the last three months are expected to have protection for the short term and can have their COVID-19 vaccination deferred.

Without a Personal Health Number or Documentation If you are living in B.C. during the pandemic, you are eligible to receive the COVID-19 vaccine. You do not need to be in the BC MSP health insurance program (a BC Services card or public health number) to get the vaccine. You do not need to be a Canadian citizen or have permanent residency to get the COVID-19 vaccine. However, you must have a public health number in order to register online. If you do not have a public health number, you need to register for your vaccine by calling the province-wide call centre (1-833-838-2323) or in-person at a ServiceBC office. The goal of the vaccination campaign is to vaccinate as many people as possible. If you have a unique circumstance and have concerns, please call in to discuss your situation.This means you can be a visitor from another province or country who is living in B.C. temporarily.

This includes international students or parents visiting from another country.

People with no documentation or photo ID: the priority is for anyone residing in B.C. to be vaccinated regardless of immigration status. Your privacy is protected and no personal information will be shared with other government agencies.

STEP 1: To get a Personal Health Number (PHN) in order to register for vaccination, call 1-833-838-2323. If you need to speak to someone in your first language, press #2. You only need to give your name and date of birth to get a PHN.

After Your Vaccination Common side effects may include Pain, redness, swelling and itchiness where the vaccine was given.

Some people experience local injection site reactions within 1-2 days after the vaccine, while some people experience local injection site reactions starting a week or more after they get the vaccine.

Reporting adverse events following immunization Pain or redness, or swelling that extends past the nearest joint (shoulder or elbow) or that persists for 10 days or more should be reported to the health care provider that gave the vaccine. It is important to always report unexpected or serious reactions to a health care provider so that they can be investigated further.

Local injection site reactions are a normal part of your body’s immune response to the vaccine and will resolve within a few days. A cool, damp cloth or wrapped ice pack where the vaccine was given may help. These local injection reactions will go away on their own; however you may feel unwell for a day or two. If you are unable to carry on with your regular activities because of these symptoms, you can take medication such as acetaminophen (Tylenol, Tempra) or ibuprofen (Advil, Motrin). Check with your health care provider if you need advice about medication.

If you have concerns about any symptoms you develop after receiving the vaccine, speak with your healthcare provider or call 8-1-1 for advice. To report a potential adverse event following immunization with the COVID-19 vaccine, contact the immunization provider that gave you the COVID-19 vaccine. Update on AstraZeneca First dose appointments in British Columbia • No more dose 1 appointments can be booked.

• If you already have an appointment scheduled, it will not be cancelled.

Getting a second dose in British Columbia • B.C. is holding all remaining AstraZeneca/COVISHIELD vaccines for dose 2 appointments.

• Everyone will get dose 2 within 16 weeks of their first vaccination appointment.

• If you received dose 1 prior to April 15, 2021, you must register with the Get Vaccinated provincial registration system to get an email, text message or phone call when you can book a dose 2 appointment: getvaccinated.gov.bc.ca.

Different vaccines for first dose and second dose At this time, it is recommended that both of your doses come from the same manufacturer. There were no studies looking at the use of different vaccines from different manufacturers and so at this time the recommendation is to receive the same vaccine from the same company.

Each manufacturer or company carried out their own studies with their individual vaccines. As there is no data on the use of different vaccines from different companies for the same person, we won’t know if people will mount a protective immune response. More research is being done on this topic, but for now, both of your doses will be from the same manufacturer.

Please note that the circumstances around COVID-19 vaccines in B.C. are changing regularly, so this information may change.

Canada’s chief public health officer Dr. Theresa Tam believes Canada could be mixing different kinds of COVID-19 vaccines in the next few months, as several provinces have already halted administering first doses of the AstraZeneca vaccine over concerns surrounding rare blood clotting.

In an interview during CTV News Channel’s “Get the Facts on the Vax” special, Dr. Theresa Tam was asked “whether Canadians who already received a dose of AstraZeneca would be given a second dose of another brand this summer.”

“That’s a possibility,” she said. “It will be really helpful, because then it makes subsequent doses easier to administer and predict, given the supply.”

Preliminary results from a United Kingdom-based study showed that receiving one dose of Pfizer’s COVID-19 vaccine and one dose of AstraZeneca’s vaccine is safe, but could result in more frequent mild side-effects. The early results found that there were more frequent minor side-effects, including fever and fatigue, compared to those who received two doses of the same vaccine.

On May 20, 2021, the Government of Canada announced funding for a new study to determine whether or not mixing and matching COVID-19 vaccines is safe and effective.

‫لقــد وســعت إدارة الغــذاء والــدواء األمريكيــة (‪ )FDA‬يف يف ‪ 10‬مايــو‬ ‫‪ 2021‬ترخيــص االســتخدام الطــارئ *(‪ )EUA‬للقــاح ‪Pfizer-BioNTech‬‬ ‫‪ 19-COVID‬للوقايــة مــن مــرض فــروس كورونــا ‪)19-COVID( 2019‬‬ ‫الناجــم عــن متالزمــة الجهــاز التنفــي الحــادة الوخيمــة فــروس كورونــا‬ ‫‪ )SARS-CoV-2( 2‬ليشــمل املراهقــن مــن ســن ‪ 12‬إىل ‪ 15‬عا ًمــا‪ .‬كــا‬ ‫قامــت إدارة الغــذاء والــدواء األمريكيــة بتعديــل ‪ EUA‬الصــادر أص ـاً يف‬ ‫‪ 11‬ديســمرب ‪ 2020‬لــإدارة يف األفـراد الذيــن تبلــغ أعامرهــم ‪ 16‬عا ًمــا فــا‬ ‫فــوق‪.‬‬

‫يجــب عــى األشــخاص الذيــن لديهــم تاريــخ مــع رد الفعــل التحســي الشــديد‬ ‫تجــاه أي ـاً مــن مكونــات اللقــاح عــدم أخــذه‪.‬‬

‫يقــول مركــز كولومبيــا الربيطانيــة ملكافحــة األمــراض‪« :‬وافقــت وزارة‬ ‫الصحــة الكنديــة عــى اســتخدام لقــاح فايــزر للشــباب الذيــن تــراوح‬ ‫أعامرهــم بــن ‪ 12‬و ‪ 17‬عا ًمــا»‪ .‬يف ‪ 20‬مايــو ‪ 2021‬قبــل امليــاد‪ .‬أعلــن وزيــر‬ ‫الصحــة أدريــان ديكــس أن األطفــال الذيــن تــراوح أعامرهــم بــن ‪ 12‬و‬ ‫‪ 17‬عا ًمــا ميكنهــم اآلن التســجيل للحصــول عــى لقــاح ‪ 19-COVID‬يف‬ ‫كولومبيــا الربيطانيــة‪.‬‬ ‫وميكــن للشــباب تســجيل أنفســهم وحجــز موعدهــم‪ .‬وميكــن أيضــا لآلبــاء‬ ‫واألوصيــاء والبالغــن املوثوقــن تســجيل الشــباب وحجزهــم ‪ ،‬مــع إرســال‬ ‫دعــوات الحجــز إىل مــن هــم أكــر ســنا أوال‪ .‬وســتجري معظــم مواعيــد‬ ‫اللقــاح للشــباب الذيــن تــراوح أعامرهــم بــن ‪ 12‬و ‪ 17‬ســنة يف نفــس‬ ‫عيــادات التحصــن املســتخدمة لتطعيــم األشــخاص يف الفئــات العمريــة‬ ‫األخــرى‪.‬‬ ‫وميكــن للصغــار املرافقــن للبالغــن يف املوعــد الجديــد للقــاح أن يســجلوا‬ ‫أنفســهم يف املوقــع وال يحتاجــون إىل تســجيلهم أو حجزهــم مســبقا‬ ‫ميكــن للعديــد مــن الشــباب يف األرسة أن يحــروا موع ـدًا محجــوزًا ويتــم‬ ‫تطعيمهــم م ًعــا‪.‬‬ ‫ميكــن للشــباب املرافقــن للبالغــن يف موعــد التطعيــم الجديــد للبالغــن‬ ‫التســجيل يف املوقــع وال يحتاجــون إىل التســجيل أو الحجــز مســبقًا‪ .‬فقــط‬ ‫دع موظفــي العيــادة يعرفــون أنــك وصلــت‪.‬‬

‫ضعف جهاز املناعة‬ ‫إن لقاحــات ‪ 19-COVID‬ليســت لقاحــات حيــة‪ ،‬وال يوجــد مخــاوف كبــرة بشــأن‬ ‫ســامة أولئــك الذيــن يعانــون مــن ضعــف يف جهــاز املناعــة‪ .‬مــن املمكــن أال يعمــل‬ ‫اللقــاح كــا هــو متوقــع عنــد األشــخاص الذيــن يعانــون مــن ضعــف يف جهــاز‬ ‫املناعــة‪ .‬إذا كانــت لديــك أســئلة وتعــاين مــن ضعــف يف جهــاز املناعــة‪ ،‬فتحــدث إىل‬ ‫مقــدم الرعايــة الصحيــة الخــاص بــك عــن لقــاح ‪.19-COVID‬‬

‫املناعة الذاتية‬ ‫قــد يتلقــى األشــخاص املصابــون بأمــراض املناعــة الذاتيــة لقــاح ‪.19-COVID‬‬ ‫ومــع ذلــك‪ ،‬يجــب أن يدركــوا أنــه ال توجــد بيانــات متاحــة حاليًــا حــول ســامة‬ ‫لقاحــات ‪ 19-COVID‬لألشــخاص الذيــن يعانــون مــن أمــراض املناعــة الذاتيــة‪.‬‬

‫يؤســفني جــداً أن أســمع عــن الخســارة األخــرة‬ ‫التــي حدثــت بســبب ‪ 19-COVID‬ملريــض صغــر‬ ‫يف مستشــفى ‪ .BC Children’s Hospital‬مــا نــوع‬ ‫التأثــر الــذي تركــه هــذا الخــر عليــك؟ يُعتــر أي‬ ‫مــوت صعــب‪ ،‬خاصـ ًة إذا كان هــذا وفــاة طفــل‪ ،‬فإنــه‬ ‫صعــب بشــكلٍ عــام‪ .‬إنــه يتحــدث عــن مــدى ســوء‬ ‫هــذا الفــروس وإىل أي مــدى يتوجــب علينــا القيــام‬ ‫بــيء لجعــل الجميــع آمنــن‪.‬‬ ‫هــل تعتقــد أنــه كان لــه تأثــر عــى املجتمــع األوســع‬ ‫ُ‬ ‫تلقيــت الكثــر مــن‬ ‫حيــث ســلوكهم؟ لقــد‬ ‫مــن‬ ‫ُ‬ ‫األســئلة مــن أصدقــايئ وعائلتــي ووســائل اإلعــام منـ ُذ‬ ‫اإلعــان عنهــا‪ .‬اإلجابــة هــي نعــم‪ ،‬لقــد أثــرت عــى‬ ‫النــاس‪ .‬لقــد كان هــذا الطفــل مريضــاً صغــرا ً‪ .‬قــد‬

‫هي عبارة عن مرات يف السنة التقوميية ألشخاص مختلفني ليك‬ ‫يكونوا مؤهلني للتطعيم‪ .‬حيثُ يوجد يف كولومبيا الربيطانية‬ ‫أربع مراحل‪ :‬املرحلتان األوىل والثانية (ديسمرب ‪ – 2020‬أبريل‬ ‫‪ )2021‬وتكون موجهة للسكان املعرضني للخطر‪ ،‬واملرحلتني‬ ‫الثالثة والرابعة (أبريل – يونيو ‪ )2021‬وتكون موجهة للشكان‬ ‫بشكل عام الذين هم فوق سن الـ ‪.18‬‬

‫يعني عندما ينترش الفريوس من شخص إىل آخر‪ .‬حيثُ ينترش‬ ‫‪ COVID-19‬أيضاً من خالل ملس الشخص ليشء به الفريوس ثم‬ ‫يقوم بلمس فمه أو أنفه أو عينيه ِ‬ ‫بأيد غري مغسولة‪ .‬ميكنك نقل‬ ‫‪ COVID-19‬قبل أن تبدأ األعراض يف الظهور لديك أو دون‬ ‫‪1‬‬ ‫ظهور األعراض عىل اإلطالق‪.‬‬
