With guidance and support from Assisted Living Locators, we propose an initial rollout with a select group of highperforming franchisees in targeted markets where facility partners have a demonstrated need. Assisted Living Locators will help identify these key franchisees by sharing relevant data and insights, and will facilitate warm introductions to ensure a smooth launch. Additionally, they will review and assist in distributing Jace Medical’s program materials and resources to support franchisee success.
Identify suitable facilities or communities and gather basic information regarding current wound care services and potential needs. Leverage existing facility relationships to facilitate warm introductions with key decision-makers.
Schedule and coordinate video or in-person discovery meetings between Jace Medical Solutions and facility leadership. Assist in securing facility participation by guiding them through the service sign-up process with a Jace Medical network provider. Maintain ongoing communication to nurture the relationship, gather success stories, and provide updates to support continued payment opportunities.
Provide Jace Medical with a list of approved franchise owners for limited, agreed-upon outreach. Communicate program details and opportunities directly to franchise owners. Collaborate with Jace Medical on necessary reporting requirements. Partner in ongoing training initiatives and continuous program optimization.
Lead all outreach efforts, providing support to franchise owners and engaging directly with facilities as needed. Train and equip franchise owners with handouts and resources to effectively engage facility decision-makers. Oversee documentation of initial engagements and manage ongoing reporting related to payments and care delivery activities.
Skilled nursing facilities - $10,000 per year
Assisted Living facilities - $6,000 per year
Rehab Facilities - $6,000 per year
