Seen supporting your hospital Highlights from our signature events 30th anniversary Markham Stouffville Hospital Foundation Golf Tournament On August 11, Markham Stouffville Hospital Golf Tournament celebrated its 30th anniversary.

Nearly 200 golfers enjoyed the private and prestigious York Downs Golf & Country Club course, lunch on the patio, live entertainment, cocktails, dinner and live and silent auctions.

This year we celebrated the many organizations and individuals who have generously supported the tournament over the years, including York Downs Golf and Country Club, which has hosted the event for 15 years, and Honeywell, returning for its ninth year as the presenting sponsor. Thanks to the efforts of all the sponsors, participants, supporters, committee members and volunteers, $220,000 was raised towards the completion of the Expansion Campaign.

Next year’s tournament is August 10 at York Downs Golf and Country Club.

4th annual MSH Leaders Night at the Races The MSH Leaders hosted their fourth annual Night at the Races, presented by Harrison-Langdon Financial Group-CIBC Wood Gundy, at Woodbine Racetrack on June 18. This year’s event hit record attendance of more than 330 guests, doubling last year’s count and raising more than $50,000.

Guests enjoyed a gourmet buffet dinner emceed by Cory Kimm of CHFI while raising funds for a new infant scale in MSH’s expanded centre for mothers and babies. With networking, photo booth fun, bidding on great auction items and taking in an amazing view of the races, it was truly a memorable evening.

CIBC Celebration of Hope presents ‘The Chocolate Sunday’ On Sunday, October 26, nearly 900 guests gathered at the Hilton Toronto Markham Suites Conference Centre and Spa as MSH Foundation presented the 26th Annual CIBC Celebration of Hope —“The Chocolate Sunday.”

With the support of numerous corporate and community sponsors, including CIBC as the returning title sponsor, the event raised close to $200,000. Highlights of the afternoon included an appearance by Christian LeBlanc, better known as Michael Baldwin on The Young and the Restless, delicious chocolate treats, Motown music, shopping at over 50 boutique vendors, as well as a runway show staged by Canadian fashion designers Brian Bailey and Wayne Clark and retailer Linea Intima. Guests also heard from this year’s Hope Award recipient, Dr. Alexandra Ginty.

Hosted again by CHFI’s Erin Davis, the day was filled with laughter, tears, inspiration and hope. Ms Davis was also one of the celebrity designers who artfully decorated a bra as part of the new Ta Ta Bra Design Auction. Other designers included Mr. LeBlanc, Frank Ferragine and Kevin Frankish of CityTV, Fashion TV host Jeanne Beker, Canadian women’s hockey Olympic gold medallist Natalie Spooner and NHL legends Darryl Sittler, Wendel Clark and Johnny Bower.

The afternoon was capped off with the awarding of a brand new 2014 MINI Cooper generously donated by MINI Markham.

A call to action for young professionals and entrepreneurs

Longtime Markham resident Meg Stokes is what you might call a dynamo. Active in the community, she was president of the Angus Glen Ratepayers Association and is a volunteer and advisor for a number of local groups and organizations. She’s outgoing, talks fast and has a ready laugh.

The marketing consultant specializes in the not-for-profit sector. She is married and has two daughters who were both born at Markham Stouffville Hospital.

In 2010, Allan Bell, director of community relations and corporate partnerships at the hospital foundation, contacted her to say they were thinking about starting up a young leaders group.

“If you know Allan, it’s very difficult to say no to him,” she says. “The hospital has a number of great fundraising events and large organizations in support and we saw an opportunity for something more specific in the community for young business professionals. So we recruited a volunteer committee specifically for them.”

Eddie Mariani, owner and operator of Mariani’s Custom Clothiers on Main Street Unionville, is Ms Stokes’ co-chair. MSH Leaders are business professionals and community leaders. All live, work or have family in Markham or Stouffville.

“We all want to help raise funds and awareness for our hospital,” Ms Stokes says.

The group currently has about 25 members aged 20 to 50 and they would love to welcome more.

“We always are looking for passionate people who want to make a difference and give back to their communities,” Ms Stokes says.

MSH Leaders run a host of fun activities throughout the year, including three big events.

Night at the Races, held at Woodbine Racetrack every June, is their premiere event.

This year all 350 tickets sold out quickly.

“It’s a unique event—not your typical fundraising gala. It’s a casual affair,” Ms Stokes says. “We’re right at the edge of the racetrack so the horses go thundering by. We’ve had Cory Kimm from CHFI emcee the event for the past two years. He is just so amazing and keeps it very fun and light. And you do a little betting on the side.”

In late August, the group supports Hockey Night in Stouffville, hosted by Paul Calandra, MP for Oak Ridges-Markham. NHL players, alumni and local personalities come together to play a game at this family-friendly evening.

In October, they organize a Markham Stouffville Hospital Foundation team in the Scotiabank Toronto Waterfront Marathon Charity Challenge.

“It’s probably selfish, but I want to take care of the hospital because I know it’s going to take care of us.”

who are avid runners. They want to get involved and raise money through sweat equity,” Ms Stokes says.

The group meets just three or four times a year—sometimes at the hospital foundation boardroom, but just as often at various locations throughout Markham and Stouffville.

“Because we’re the young leaders we’re a little more hip and fun. Is hip even a cool thing to say anymore?” Ms Stokes laughs. “We’ve met at restaurants and different businesses around the community. We always have fun.”

Members can get involved in as many or as few events as they like. Some run their own private events with friends and family, raising money they then donate through the MSH Leaders program. Some volunteer at specific events.

Others have the financial means to sponsor events and help obtain support by introducing fundraising initiatives to their networks.

“The nice thing about us is that we’re not demanding either of time or money and we’re really flexible on how members want to provide their support,” Ms Stokes says.

If you’re unsure about membership, they are always looking for volunteers to assist at events.

“Or, if you’re a runner or a walker, join us for the Scotiabank Toronto Waterfront Marathon next year,” Ms Stokes suggests.

In 2013, the group raised $20,000 at this single event alone. Ms Stokes and Mr. Mariani ran their first-ever half-marathon.

“It was very emotional,” she says. This year, her husband and children signed up for the 5K. Members often get their families involved.

In just a few short years, the group has raised more than $150,000 in support of Markham Stouffville Hospital’s $50 million Expansion Campaign. This is so valuable because the hospital relies on community donations to purchase the equipment that helps it offer stateof-the-art care.

“Don’t be afraid to get involved. It really doesn’t take a lot of time,” Ms Stokes says. “It’s so incredibly rewarding, no matter what you’re looking for—whether it’s self-satisfaction or some inspiration. It’s also a great way to meet new people in the community and network from a business point of view. We’re all going to need the hospital at some point in our lives, so it’s in our best interests to take care of it.”

Take your pick State-of-the-art technology supports patient safety

Shellyna Moledina remembers the first patient whose hospital identification bracelet included the bar code.

The barcode would allow the BoxPicker, a state-of-the-art automated medication storage and retrieval system, to find the medication the patient was ordered.

“Our first patient was so excited,” says Ms Moledina, director of pharmacy services at Markham Stouffville Hospital. “They were excited to have it scanned. It was nice to see that people are aware of the safety this technology brings to patients at the bedside “ The technology is world class and, frankly, kind of cool.

A robotic arm moves up and down aisles where more than 1,000 medications are stored in white boxes in proper dosages, each with its own barcode. The mechanical arm slides up and down its body, eventually bringing the newly found medication to the pharmacy staff.

A technician confirms the picker’s selection and then the medication is shipped via a pneumatic tube system to the appropriate

location in the hospital. A nurse on the other end will scan it and compare it to the patient’s eMAR (electronic medication record) and the patient’s armband to verify the right drug is being given to the right patient.

Ms Moledina says the implementation of the $300,000 BoxPicker has helped to reduce human error.

“We’ve seen a decrease in errors at the bedside because the nurses get an alert (on their computer screen) if they scan the wrong medication. It’s a huge double-check happening on both ends and therefore human factor errors have significantly decreased,” she explains. “Barcode technology is the latest thing that is available to help us make sure all our patients always receive the right drug at the right time.”

The technology’s success is two-fold, says Lisa Joyce, vice-president of corporate communications and engagement.

“It allows for the technology to be used to its fullest and our staff to the fullest,” she says. “It’s been great how we’ve been able to integrate technology into our approach and

improve safety.”

Patient safety is a core value at Markham Stouffville Hospital—and essential to ensuring patients receive the best possible treatment, Ms Moledina says.

“The community ultimately benefits when we have state-of-the-art technology,” she adds.

The recent expansion of the hospital provided the opportunity for the upgrade.

The new pharmacy unit was designed to incorporate the large piece of equipment. And, the $300,000 needed to buy it was included in the hospital foundation’s $50-million expansion campaign.

“As we engage community members to help equip their hospital, it’s crucial that we let them know what we need and why we need it,” Ms Joyce says. “The MSH Foundation has done an exceptional job of linking our need for support with the patient’s needs and I believe this is why we’ve had such great community support around the fundraising goal for the new building.”

Ms Strong hopes the striking new donor wall will inspire others to make a difference and help raise awareness about the importance of community giving in support of Markham Stouffville Hospital’s ongoing priorities and needs.

“The new and expanded Markham Stouffville Hospital is complete. It was made possible because of people’s generosity. In many ways, this end is only the beginning because we need continued community support to ensure our families, friends and neighbours get the firstclass health care they deserve,” she says.

And there is still space on the wall for more names....

Orchestrating a successful hospital expansion requires vision. A vision based on knowledge, including an understanding of the demographics of the population the hospital serves and the health care needs of that population now and into the future.

“What does a community hospital need?” asks Markham Stouffville Hospital president and CEO Janet Beed. “A vibrant emergency department; the ability to diagnose and treat; and a dynamic relationship with family medicine.”

Patients generally come to the hospital through a visit to the emergency department or a referral from their family physician for tests or treatment. As we look to the role of the hospital in the future, Ms Beed proposes that patients will either be very sick requiring 24-hour supervision with a hospital stay inclusive of intensive care or they will need ongoing support from an outpatient clinic with a specific specialty focus such 10 mshf.on.ca

as cardiac, respiratory, neurology, pediatrics, etc.

Shorter admissions for less intensive reasons will allow the patient to be supported at home post treatment. The expanded hospital was designed to meet all these needs: it tripled the size of the emergency department and doubled the size of the diagnostic imaging department; it boosted the number of beds in the ICU from eight to 24 and created substantial clinic space.

New staff will be recruited and beds opened to patients as need dictates and resources become available.

The expansion accomplished all these things while creating a space that is therapeutic and welcoming. Natural light floods in through the many windows; the walls are graced with art; and waiting areas include comfortable furniture and diversions such as magazines and television.

Throughout the planning and execution of the project, much thought and discussion were

given to how patients would use the space, says Lisa Joyce, vice-president of corporate communications and engagement.

“I think we really got it right in so many ways,” she says. “The care has always been good.

We’ve always had leading-edge diagnostics and the ability to do lab work quickly. But now by creating the broader environment, I think it is really right for the patient.”

Physicians and staff provided input into how their departments would look and function.

“Seeing the ideas of the staff get translated into a program and a service for patients has been wonderful,” Ms Beed says.

Ms Beed recalls other personal highlights of the construction of the new building: the installation of the first beams, which gave staff a real idea of just how big the new building would be (and why they’d been asked to park so far away); the “topping off” ceremony to enclose the structure and begin work inside;

The community stayed with us and the staff stayed with us. And I am very, very thankful that they did. I am thankful, at the end, that we still have our parade.

“They were enthusiastic and committed and they moved these patients without incident, they moved them with smiles, they moved them with enthusiasm,” she says of staff who arrived on site at 5 a.m. that day to transfer patients.

The renovation of the old space was perhaps more challenging. Some staff relocated three times during the 18 months of work. Detours were common as different sections of the hospital were closed for renovation.

“We were changing the tires on the car as it was driving,” Ms Beed says. “I don’t think any CEO could be prouder of how our staff stayed engaged and always kept an eye open for a family member or patient who had that ‘oh my gracious’ lost look on their face.”

That attention to patient care will continue to be a priority, she says.

“Now that I’m in my permanent home, how do I make the patient’s experience with me the best it can be? How do I receive them? Where do they wait? Why do they wait? How do we reduce the wait?” are all questions staff will be asking themselves over the coming months,” she says. “We’re really putting a big push on how we engage our patients in an experience that is very positive.”

The hospital will also consider the role it can play in improving the wider health care system—by boosting programming at the Cornell Community Centre, which is located on the hospital campus and already the site of some programs for stroke patients and youth with mental health issues, for example.

“We need to be part of the solution,” Ms Beed says. “How do we contribute to system solutions with new ideas and new health care models?”

A big ask, a bigger response When, in 2007, the Markham Stouffville Hospital Foundation launched the campaign to fund the hospital expansion, it was breaking new ground.

It was the first capital campaign in the hospital’s history and, at $50 million, it was the largest campaign of its kind in York Region.

The fundraising experts had recommended a smaller target of $35 million. But foundation CEO Suzette Strong says for the foundation to fulfill its mission to “enable the growth and innovation of the hospital,” it needed to stretch itself and the community.

The foundation recruited Markham Mayor Frank Scarpitti and longtime hospital supporter and foundation board member Peter Mason to establish and oversee a campaign cabinet that would champion fundraising efforts. These volunteers were “the engine” of the campaign, Ms Strong says. They spoke out, offered introductions, opened doors, made their own significant contributions and encouraged everyone in their circles to do the same.

The giving began at home, when the foundation launched its first staff pledge drive. Doctors, nurses, technicians, support staff and board members all responded. The physicians alone raised more than $1 million; total MSH family contributions were close to $2 million.

“The foundation knew it would be much more powerful for us to go out to the community at large and ask for its support if we could say that we had already given,” Ms Strong says.

Then, they took their message to the community.

Along the way, Ms Strong and her team heard many inspiring stories from residents whose lives had been impacted by their local hospital—and gratefully received their donations.

“The majority of our donors are individuals and families whose lives have been touched in a very deep and personal way,” she says.

Amazingly, the campaign received more than 20 donations of more than $1 million.

Among these donors were ethno-cultural groups representing the Pakistani and Chinese communities.

The donations illustrate the foundation’s success in raising awareness about the need of the hospital to expand to keep pace with our growing communities.

“It’s about helping people understand the problem (rapid community growth) and inspiring them to be a part of the solution (support the expansion).

The foundation also heard about how comfortable residents felt coming to MSH. “They said you didn’t feel like you were coming in to an institution, you didn’t feel like you were a patient number, you were a person,” Ms Strong recalls.

The new facility captures that same warmth— with art, natural light and comfortable waiting rooms for patients and families—while incorporating leading-edge technology and improved patient flow.

“The space and the programs have been designed around keeping the patient interest first,” she says. “Of course it’s one thing to have great space and a state-of-the-art hospital to support health and wellness, but what really makes our hospital special is the incredibly talented people who work here.”

Early on in the campaign, the company overseeing the construction and renovation announced what was an eight-year project could be accomplished in seven years. It was great news for the community, but an added challenge for the foundation.

A challenge it met and more—raising $56 million.

“We not only met our goal in a shorter period of time, we exceeded it, which, I think is a pure reflection of the commitment this community has and its desire to support excellent patient care,” Ms Strong says. “It’s rooted in the community’s pride and its understanding of how important it is to have a hospital close to home.”

To maintain that standard of excellent care will require the ongoing support of Markham and Whitchurch-Stouffville residents, community groups, organizations and businesses.

“It’s not over,” Ms Strong says. “We’re still in the business of fundraising and we still rely on the community to help us succeed.”

Bricks and mortar, space and technology It’s fair to say that every department, every staff member and every patient has been impacted by the expansion and renovation of Markham Stouffville Hospital. Here’s an in-depth look at four of those departments.

Diagnostic imaging A successful health care journey begins with the right diagnosis.

As such, expanding and updating the diagnostic imaging department was a priority of MSH’s expansion.

Every year, the hospital’s 113 technologists in diagnostic imaging and cardio-respiratory services perform more than 190,000 tests including MRI (magnetic resonance imaging), CT scans (computed tomography), mammography, x-ray, ultrasound, nuclear medicine, bone mineral density, electromyography, electroencephalography, exercise stress tests, ECG (electrocardiogram)/ holter monitoring, echocardiography and pulmonary function and interventional procedures such as biopsies and joint injections. At 60,000 square feet in size and located in the hospital’s new building, the department boasts the latest equipment and technologies to provide accurate diagnoses.

“We are fortunate to have been able to design our new department with patients, families and referring physicians in mind,” says Lynne Campkin, director of diagnostic services. “The Markham Stouffville Hospital Foundation had a very successful fundraising campaign that supported the creation of a state-of-the-art diagnostic services department. The staff, radiologists and medical consultants in diagnostic imaging, cardio respiratory and electrodiagnostic services are showing their appreciation through the provision of excellent quality diagnostic services.”

The laboratory This year, MSH’s laboratory technologists and pathologists will perform more than 1 million tests, a number that has increased with the growth of the communities served by the hospital.

The tests are crucial to the diagnosis and ongoing care of patients.

“The lab plays a key supporting role in providing diagnostic information to our care providers,” says Jonathan Kwan, director of laboratory services. “Doctors rely on lab testing to provide diagnosis; 70 to 80 per cent of diagnosis is provided by lab results.”

To cope with the large volumes of specimens, hospitals in Ontario have adopted automated technologies to provide consistent quality and faster reporting times for test results. Part of MSH’s redevelopment was the purchase and installation of the latest generation automated track system for processing and testing blood specimens. The system streamlines the testing process, eliminates errors and allows for a more seamless transfer of specimens and results.

“This technology allows the staff to work to their full potential and makes technology a real partner in the lab and in serving our patients,” says senior technologist Mohamed Bhugun.

Surgery Surgical patients at MSH are seeing the results of new cutting-edge technology in improved wait times, says chief of surgery Dr. Patrick (Paddy) Whelan. Currently 97 per cent of patients have their surgery within the target wait times established by the province.

The new surgical department has seven spacious and state-of-theart operating suites currently in operation with ability to open up two more suites as volumes demand. All suites are equipped for minimally invasive surgery, which is done with small incisions, cameras and tools and means less pain and quicker recovery times for patients. Multiple screens allow every member of the surgical team to see and respond to what’s happening at all times during surgery.

Dr. Whelan says: “It’s the best technology available, so we are not limited as to what we can do. It’s given us a great ability to hire young innovative surgeons, nurses and support staff with new ideas and up-todate and innovative techniques.”

Attention to detail in the design and flow of the department has also improved the overall care experience for patients and their families, says Tracie Scott, operating room clinical manager. For example, waiting rooms are welcoming and comfortable and information boards with patients identified by numbers for privacy allow family members to know where their loved one is at all times.

Neonatal intensive care unit “Family-centred” is how MSH’s chief of pediatrics Dr. Deepa Grewal describes the new neonatal intensive care unit (NICU).

Each baby has its own distinct area, with enough room for parents to sit, a breast milk fridge and curtains that close for privacy—so parents can comfortably provide skin-to-skin contact, which has been shown to have a positive impact on baby’s well-being. The new design supports daily rounds, where the neonatologists, nurses, social worker, dietitian and other team members review the baby’s care and progress with the inclusion of parents.

These details provide comfort and reassurance, says Derick Cruz, whose son Tyler was in the NICU in March.

“The space allowed us to observe and participate in the activities the NICU team needed to perform for Tyler’s care without feeling that we were in their way,” Mr. Cruz says. “The assigned space for Tyler also gave us the privacy to bond as a family.”

Just as important, the unit is outfitted with they latest equipment—including state-of-the-art isolettes (enclosed beds that provide controlled temperature, humidity and oxygen supply to babies), new ventilators and the latest respiratory support equipment, says neonatologist Dr. Navneet Sharma.

“Of all the Level 2 NICUs we have in the Toronto area, this is one of the best built units, in terms of the working space and in terms of the equipment,” Dr. Sharma says.

Meet our MSHeroes

Patients of Markham Stouffville Hospital recognize excellent care by nominating physicians, nurses and other staff members as MSHeroes. Patients make a donation in honour of their heroes, who receive a card and gold pin from the hospital foundation. Here are four such heroes: Chris Burden registered nurse, intensive care unit A hero inside and outside the hospital walls, Chris Burden gained national attention this summer when he performed CPR on a man struck by lightning at a local golf course, saving his life.

As a nurse in the intensive care unit at Markham Stouffville Hospital, Mr. Burden treats critically ill patients mostly on life support. He is part of a team that can restore health to a large range of medical or surgical patients or help prepare a patient and family for end of life. Both, he says, are rewarding.

“I get the most satisfaction out of helping patients and their families during really difficult, critical times,” he says. “A big part of the job is communicating and connecting with families.”

A graduate of York University and local resident, Mr. Burden did some training at MSH during school and knew it was where he wanted to establish a career. The people, he says, are second to none and the continued growth of the community and the hospital will enable him to build his skills as a nurse.

Bonnie Jean-Baptiste patient relations representative A social worker by training, Bonnie Jean-Baptiste began working at Markham Stouffville Hospital on a casual basis as a member of the mental health crisis team.

Four years ago, she became a full-time social worker for the medicine, intensive care and surgery departments. In 2011, she moved to her current role as patient relations representative.

The focus of her job is to receive and resolve patient feedback—compliments, comments, suggestions, inquiries and complaints.

“It gives patients a voice,” Ms Jean-Baptiste says. “It’s an impartial, confidential and easily accessible process that ensures patients and their families have a mechanism to raise concerns about their experiences and provide feedback.”

The hospital also benefits.

“The hospital gains valuable insight and can use this information to capture service excellence and identify opportunities for improvement,” she says.

Ms Jean-Baptiste was surprised to learn she has been nominated as an MSHero for her work.

“I don’t really see myself as a hero,” she says. “It takes the efforts of all of us to make the best possible experience for the patient and his family. I am just one small part of that.”

Gary Edney social worker, outpatient mental health Gary Edney came to Markham Stouffville Hospital seven years ago, after stints at other health care centres around the GTA. Now, he can’t imagine working anywhere else.

“It would be hard for me to find something at this point that I don’t love about being here,” he says. “This is a wonderful community to be a part of.”

As a social worker in the outpatient mental health department, Mr. Edney treats people struggling with a range of emotional and psychological problems. He sees patients for individual counselling and co-facilitates cognitive behavioral therapy, dialectical behavioural therapy and interpersonal psychotherapy groups.

Because he works with people over the course of many months, he is able to witness their improvements—and he finds it immensely satisfying to “make a depressed person smile, to help an anxious person take a deep breath and let go of something or help someone change an unhealthy to a healthy coping mechanism.”

His MSHero nomination shows his clients are benefitting from his care.

“I was quite humbled,” he says of the award. “I actually view the people we treat as heroes because these people are struggling with profound mental health issues and it’s a privilege to help them.”

Dr. Andrew Patterson family physician chief of rehabilitation, palliative, complex care As chief of rehabilitation, palliative and complex care, Dr. Andrew Patterson leads a team of family physicians caring for some 50 patients at Markham Stouffville Hospital. A family physician, Dr. Patterson began his career at MSH before it opened its doors as chief of family medicine and he has long been a champion of the role family doctors can play in the hospital setting, caring for patients with chronic health issues.

“I feel patient care is enhanced when more chronic patients are treated by family physicians whose skills are well honed in that area,” he says.

In recent years, palliative care has become an important focus of his practice.

“What we are able to achieve by working with both patients and families is helping patients deal with the symptoms of their disease and helping families come to terms with what they are going to have to cope with—and that is rewarding in itself,” he says.

Dr. Patterson says a true team approach to care that values the contributions of all has kept him at the hospital—and his MSHero award reflects on the whole team.

“It’s always pleasant to receive recognition for some of the things that you do,” he says. “I feel a great deal of pride in what we’ve achieved at Markham Stouffville Hospital over the years, so I’m more happy that patients and the community are identifying the hospital as a good place to come and receive medical care.” mshf.on.ca 17

Nurturing innovation

Dr. George Arnold, chief of obstetrics and gynecology at Markham Stouffville Hospital, has a clear passion for his work—as a physician and as a champion for innovation within the hospital.

“I love delivering babies,” he says. “There is nothing that gives me more joy. It never loses its appeal.

“Injecting new life into the way we do things and the way we practice is just as rewarding as bringing new life into the world,” he adds.

A graduate of the University of Toronto’s school of medicine, Dr. Arnold came to MSH shortly after he completed his studies in 1992.

He was named chief in 1999.

He was attracted to the hospital because of the growing community—lots of babies, lots of business—and because of the outstanding reputation of the department.

“There really wasn’t a similar group anywhere else,” he says. “As far as a working environment goes, you couldn’t ask for anything better and that’s something that I think we’ve been able to maintain…. We’ve been able to maintain a reputation for excellence throughout the province.”

That status has put the hospital in a position to hire the best possible candidates as the department has grown, he says.

In the past 20-plus years, the number of doctors in the department has jumped from six to 13 and the number of deliveries has more than tripled from about 1,000 to 3,500 annually.

The nursery can care for babies born as early as 30 weeks gestation, as well as twins and triplets, meaning very few patients must be transferred to another hospital.

On-site expertise in minimally invasive surgery and gynecological oncology means women can come to their local hospital for a wide range of gynecological procedures and surgeries, as well as obstetrical care.

As part of the extensive expansion of MSH, the department recently moved to new quarters.

At more than 85,000 square feet, it is one of the largest units in the GTA. “We offer a state-of-the-art experience for patients and state-of-the-art technology for providers,” Dr. Arnold says.

The department features 13 large private labour delivery recovery postpartum (LDRP) suites, designed so women can spend all their time at the hospital in one room, as well as additional postpartum rooms to handle

overflow when necessary. Two dedicated proximal operating rooms means women requiring emergency Caesarean section don’t have to be moved to another floor. An additional 13-room unit is on standby, ready to be opened when future growth demands it.

A collaborative, progressive approach to care is the standard in the department.

“There is an openness and willingness to adopt new ways of doing things,” Dr. Arnold says.

For example, in 1994, MSH was the first in Ontario to welcome midwives on staff. Now, 14 midwives provide care to women at the hospital.

More recently, the department implemented a number of strategies to reduce its Caesarean section rate. Caesarean deliveries mean increased risk and recovery times for patients in addition to doubling expenses for the hospital in comparison to vaginal births.

Initiatives such as revisiting policies on

inducing labour and educating mothers about the possibility for vaginal birth after a previous Caesarean have reduced MSH’s rate from nearly 30 per cent four years ago to 22 per cent today, one of the lowest in the GTA and across North America.

Active in the broader medical community, Dr. Arnold has sat on a number of committees of the Ontario College of Physicians and Surgeons as well as the Central Local Health Integration Network (LHIN).

“This gives me the ability to be on top off new trends, new developments, new guidelines,” he says. “I can be sure my department and our community is benefitting from new developments within our profession.”

Connections beyond the province have also inspired Dr. Arnold.

On the recommendation of hospital CEO Janet Beed, he accompanied Markham Mayor Frank Scarpitti, local business owners and entrepreneurs on a trade mission to India in January 2013.

While there, Dr. Arnold visited hospital corporations and universities and spoke with health care workers, politicians and professors.

He came away with ideas for innovation and improvement at MSH and contacts for local entrepreneurs to partner with.

For example, working with a Markham entrepreneur, his team launched an online postpartum education resource this fall. It features evidence-based video and text that answer a wide range of questions about pregnancy, postpartum health and infant care.

There are future plans to enable patients to connect with caregivers for one-on-one advice.

‘This is a very exciting innovation,” Dr. Arnold says. “Nobody else is doing this.”

He is also working on several ideas, borrowed from hospitals in India, to improve patient flow in the hospital as a whole—such as introducing online registration or self-serve check-in kiosks similar to what you find at the airport and working with a local university to develop software to allow patients to schedule appointments with hospital clinics or for diagnostic tests online.

“I’ve been blessed to have been given opportunities to create and put forward new ideas and investigate putting ideas into practice,” he says.

The hospital and its patients are benefitting from Dr. Arnold’s drive to make the most of those opportunities.

Finding a way to recovery ATLAS helps teens manage anxiety and depression

For teens suffering from anxiety and depression, getting up to face another school day can seem an insurmountable task. Days away from school can turn into months and even years. Markham Stouffville Hospital’s Adolescent Treatment and Learning Alternative Service (ATLAS) program, now in its second year, provides a solution.

ATLAS is a day program for adolescents 12 to 19 years old struggling with depression or anxiety.

“These kids have a real hard time getting to school. They’re missing enormous amounts of time and losing academic standing. Parents are tearing their hair out, not knowing what to do,” says Paul Cappuccio, patient care director for mental health services and family medicine.

“It’s a great program. It’s set up exactly like a school class but slightly smaller.”

The program accommodates eight youth at a time and follows an eight-week cycle.

Participants earn a specialized high school credit.

“The ATLAS program is unique,” says child and adolescent psychiatrist Dr. David Ng. “We are not duplicating services in the community.

are mainly experiencing school avoidance due to their anxiety.”

The intensive program benefits from the services of a psychologist, a social worker, two child and youth counsellors, a psychiatrist and a teacher provided by the York Region District School Board (which also provides transportation).

Funded by the hospital, ATLAS was started last year because of an identified need in the community, Mr. Cappuccio says.

“There was a need from patients and their families coming to our regular child and adolescent program and we recognized it,” he says. “Often these kids have missed so much school that the school board has been alerted.”

The ATLAS classroom, built as part of the hospital’s expansion, is modelled on a regular school classroom “You need to bring them into a setting that’s therapeutic but not lose touch with the school system,” Mr. Cappuccio says. “Because of the skill set that our staff has, they understand how to engage a kid to get them back into the real world. By coming to us, to our smaller version of a school, it means they have the ability to go back.”

In its first year, the program posted a clear success rate. The first 12 students in the program returned to school.

The Canadian Mental Health Association estimates that 10 to 20 per cent of Canadian youth are affected by a mental illness or disorder.

About five per cent of males and 12 per cent of females aged 12 to 19 have experienced a major depressive episode. Once depression is recognized, help can make a difference for 80 per cent of those affected, allowing them to get back to regular activity.

“Early intervention is crucial,” Mr. Cappuccio says. “Being away from the classroom setting has lots of harmful effects as the years go by.

People who are absent from school chronically will also have absenteeism at work. The long-term academic and economic impact is obvious. ATLAS is a program that tries to lessen the burden of both future illness and current absenteeism.”

In some cases, it’s not just the children’s futures at risk.

“Parents spend many hours trying to coax their kids to go back to school,” Mr. Cappuccio says. “They’re yelling through doors. Some of the moms had to take leaves and were in

jeopardy of losing their jobs. ATLAS provides structure. It allows the parents to return to a normal life too.”

While many children try to avoid school occasionally, parents and teachers should look out for excessive worrying about day-today life, the future and world events. Children with severe anxiety may exhibit signs such as fidgeting, shakiness, talking about feeling scared, avoidance of activities (like going out with friends or doing homework), tiredness and difficulty sleeping.

Mr. Cappuccio says there are still too many sufferers not getting the help they need.

“Many people are embarrassed. They think this is what life is supposed to be like,” he says.

“When you intervene with kids early enough and they know that it’s an illness and not a character flaw, they succeed. Nobody tells you that depression is like diabetes. Nobody goes to a child with diabetes and says ‘Snap out of it.’ They have a responsibility to seek treatment and recover, but they have no fault in getting that illness.”

Because the ATLAS program is voluntary, participants must want to heal.

“We have to have willing participants and sometimes we know that they have the desire and they need a lot of help,” Dr. Ng says. “Some of them have not been in school for two years.

The transition to high school is especially hard.”

Dr. Ng points to the example of a boy who couldn’t get up in the morning to attend meetings. Team members felt that deep down he was able to make a change, so they worked with him to slowly overcome his anxiety, booking his first intake interview at 2 p.m. so he would have plenty of time to get out of bed.

“We really go out of our way to help them, however they also have to show us that they are willing,” Dr. Ng says. “He did, to the amazement of the school, and we were so proud of his progress.”

For those who receive treatment for anxiety and depression, the outlook is positive.

“The good news is that most kids have resilience,” Mr. Cappuccio notes. “Many people with anxiety and depression have learned to live with their illness and recover.”

ATLAS harnesses the health-positive effects of fitness. The full-day program incorporates visits to the community centre, with opportunities for exercise, swimming, basketball, weight-training

and the possibility of earning a high school physical education credit. Combine this with rigorous study sessions, individual and group therapy and outings within the community and it makes for an intense short-term program designed for maximum results.

“I’m so grateful that we are able to run a program like this for our community. It’s innovative and not something hospitals usually do,” Mr. Cappuccio says. “And this would not have happened without the progressive thinking of the York Region District School Board.”

While the program is on a path to success, there’s a need for donors and sponsors to help with expenses related to outings and events that celebrate the youth’s achievements.

“We only have a certain amount of money and it’s for clinical programming,” Mr. Cappuccio says. “But there are a lot of little things you need,

especially with children, to make the program successful.”

With year one of the program behind them, ATLAS staff members are looking forward to helping even more kids in year two.

“To me, what’s great is what the kids say about the program,” Mr. Cappuccio says. At a first-year open house, youth in the program were encouraged to share their feelings about ATLAS on a white board. Notes included: “I’m relaxed dude.” “I’m not sad.” “I actually got up and left the house five days in a row.”

While someone without mental illness might take getting out the door in the morning for granted, it can be a strong indicator of success for those with anxiety and depression. “‘I’m not sad’ is an incredibly powerful statement given what they’ve gone through,” Mr. Cappuccio says.

Healthy holiday: Strategies for eating well

The holiday season is no time to begin a restrictive diet, says Yogeeta Sharma, a Markham-based registered dietitian and founder of Healthy Eating for Healthy Families nutrition consulting company.

By using a few simple strategies, you can navigate parties and potlucks, family dinners and work events without negating past good habits. Ms Sharma offers several tips for making your holiday meals as healthy as possible, while still enjoying the season.

Don’t skip meals.

Although passing on lunch when you have a party to attend that evening may seem like a good way to save calories, Ms Sharma says hunger may cause you to make up those calories and more at dinner. Regular meals will also keep you fuelled for the day’s activities.

Tweak your recipes.

Regardless of the holiday you’re celebrating, traditional dishes can be altered to reduce fat, salt and calories, Ms Sharma says. Lightly panfry your potato latkes and finish them in the oven. For Diwali favourites, make sauces 22 mshf.on.ca

with milk instead of cream, use less oil and salt and boost flavour with herbs and spices instead. Remove the skin from turkey before serving and choose whole-grain bread and add lots of vegetables and fruit when making stuffing.

Indulge in your favourites— within reason.

A good strategy for the buffet or dessert table, Ms Sharma says, is to take a small portion of those dishes you really love and round out your meal with vegetables or your dessert plate with fresh fruit.

“Half of a healthy plate should be reserved for vegetables,” she says.

Bring your own healthy dish.

Offer to contribute to the family meal or neighbourhood party—and bring a salad, vegetable tray or fresh fruit platter. You’ll know there will be a least one healthy option on your plate.

Eat mindfully.

Don’t stand at the buffet table, grazing and chatting. Instead, fill your plate and find a

place to sit and eat.

“You will enjoy the food more and be able to assess when you are feeling comfortably full and satisfied,” Ms Sharma says.

Limit the drinks.

The calories from alcohol and other holiday favourites such as eggnog and specialty coffees can easily add up—but our bodies don’t register the calories of drinks the same way they do food, says Ms Sharma. Indulge sparingly and drink lots of water to stay hydrated.

Add activity to your events.

A hike around the local tree farm to cut down a tree, skating at an outdoor rink or even a walk through the neighbourhood to admire the lights will help you burn a few calories and keep food from being the only focus of the celebration, Ms Sharma says.

Enjoy the holidays.

“At the end of the day, you should be realistic,” Ms Sharma says. “This isn’t the time of year you should be trying to lose weight.

Instead, aim to maintain your current weight— and enjoy the time with your loved ones.”

What’s new in the ever-evolving ED

It’s been 18 months since the emergency department (ED) moved into its brand new space at Markham Stouffville Hospital. Yet the move was just one piece of the puzzle for a department accustomed to near-constant change.

“The physical space and lack of capacity in our old ED created daily challenges,” admits Dr. Andrew Arcand, chief of emergency medicine. “It was modern compared to some, but it was still built in the times when we provided curtain-based privacy between all of our patients, even our sickest ones. Space was a big deal and so was patient privacy.”

The March 2013 move tripled the department’s footprint. It’s now expansive, bright and much more functional.

“Any emergency department is a busy and hectic place, irrespective of what that place looks like,” Dr. Arcand adds. “Ours is no different.”

Busy indeed. The department sees more than 200 patients a day and on exceptionally busy days can see as many as 245. It has approximately 80 nursing staff and, of its 30 physicians, nine are on duty each day, The department has seen an eight per cent increase in volume over the last several years.

“This year there’s been a spike,” says Clint Atendido, director of emergency and ambulatory services. “We’re up almost 10 per cent from the previous year.

“We are very fortunate to have new space and new equipment,” he continues. “But that

alone doesn’t make our department function well. Our job is to continually look at our processes, see how we are serving our patients and see what we can do better. The notion of constant improvement is what motivates us to continually fine-tune our processes.”

As part of this, the team utilizes the LEAN philosophy, a tool the hospital has used with success before.

“It’s great for assessing quality and looking for improvement,” Mr. Atendido says. “You map out processes so you understand what you do each step of the way.”

The methodology, borrowed from Toyota, helps find waste, uncovers opportunities to streamline, find efficiencies and increases value for the patient.

We take them very seriously. If people aren’t happy with the care they received, we encourage them to share that with us. We always look at it as a way for us to improve—it’s not punitive by any means. Whether good or bad, give us the feedback. Let us know how we’re doing.

In looking for improvement opportunities, many of the people who make the ED work were engaged: physicians, nurses, assistants, clerical staff, porters and housekeepers as well as staff from the lab, diagnostic imaging and information technology.

“We made sure it was inclusive so we had different perspectives informing us,” Mr. Atendido says.

“We always have to be in a change state, to keep adapting. It’s not just about being more efficient, but also patient satisfaction. How do we make our patients happier?”

The department recently began using “just in time” surveys, an average of 10 a day, asking patients about their experience. The unique opportunity allows the team to gauge patient satisfaction results in real time.

While the increased space means more beds and more access, the hospital must make certain it has the resources to support that space, he says.

The province has an incentive program called Pay for Results (P4R), whereby hospitals receive funding based on how they stack up performance-wise against the other 73 hospitals in the program.

“Out of 74 hospitals in the incentive program, we’re in the top 10 to see a physician in a timely manner, he says. “We endeavour to have patients seen within two hours.”

Fifteen new initiatives were funded through P4R this year, including additional nursing and portering staff, lab technicians in the ED and opening a transitional bed unit (TBU).

The relocation of admitted patients out of the ED to the TBU creates needed capacity for patients waiting to be seen by an ED physician.

Mr. Atendido recently received the latest results and says MSH continues to make gains in the rankings, and the investment and changes are paying off.

He believes in sharing performance metrics with everyone in the hospital. Change management and quality improvement is a collective effort, not just for a select few, he says.

“Traditionally, P4R funding and the data have not been shared as broadly as it is now; it was very ED-focused,” he says.

“But we’re transparent now about our data and how well we’re doing. We’re trying to be more inclusive with our decisionmaking, grassroots and bottom-up rather than top-down, to encourage support and accountability to improve access and flow and the patient experience.”

To improve flow in individual units, for example, they asked staff what would help.

The result was more corporate investment in housekeeping, registration and support services. They increased physiotherapy hours on weekends and brought in a day assistant flow manager.

One of the biggest complaints they still see in surveys is the length of the wait.

“We actually do a really good job getting patients to see our physicians. From the time you walk in to the time you see a doctor, our data shows you’re going to be seen in about two hours. Twenty minutes of that is administrative activity: triage and registration,” Mr. Atendido says.

That figure is for 90 per cent of all patients.

Depending on the reason for your visit, you could be seen very quickly, if not immediately, based on the Canadian Triage Assessment Score (CTAS).

Staff has also chosen to focus on patient comfort and managing pain. This summer, the ED brought in flow clerks to help with logistics such as ensuring patients in any area are ready to be seen by a physician, preparing equipment, booking follow-up appointments, entering orders and preparing reassessments.

They also see to the softer side of care: asking patients in the waiting room how they’re doing, communicating wait times and providing a blanket or a drink.

“In the emergency department, we see most of what comes into the hospital, so we set the tone for the visit for the entire organization. It’s our goal to make sure you start with a great experience,” Mr. Atendido says. “Processes improve but at the end of the day it’s people who deliver the care.

Patients aren’t going to know about all the improvements we’ve done behind the scenes to make things better. They want to know that when they come through our door, they’ll be greeted with compassion and excellent care in a timely manner.”

With the colder, darker weather here now, what can I do to avoid being affected by SAD?

Seasonal affective disorder or SAD affects three per cent of people living in the Toronto area. The symptoms appear in October and disappear in May. Symptoms include depressed mood, loss of interest and pleasure, increased appetite (especially for carbohydrates) and weight gain.

The need for sleep is also increased. The “winter blues”, a much milder form of SAD, affects 20 per cent and is characterized by decreased enthusiasm for life, with loss of energy, creativity and productivity. Once again, there is increased appetite and sleepiness. The conditions, due to imbalance of serotonin and melatonin, are more common in women. Treatment of both conditions includes getting outside in the sunshine as often as possible. Take a walk at lunch time if you can.

Light therapy with a special light box is effective, often bringing relief within a week. Severe cases of SAD require antidepressant therapy.

ASK A NEUROLOGIST

My doctor ordered an EMG for me. What is this test and why do I need one?

An EMG/nerve conduction study is a test that provides information about the health and function of the nerves and muscles in the arms and legs. The test consists of two parts, the nerve conduction study and EMG (electromyography). Patients may require one or both parts of the test. During a nerve conduction study, the doctor will tape small electrodes to your hand, arm, foot or leg and then stimulate the nerves with small electric impulses. During an EMG test, the doctor places a small, thin electrode just under the skin into certain muscles to tell if there is evidence of nerve or muscle damage.

This test gives important diagnostic and treatment information for problems such as carpal tunnel syndrome, peripheral neuropathy, muscle disease and pinched nerve roots (radiculopathy).

It can also be used to diagnose and manage more rare conditions such as myasthenia gravis and ALS. Sometimes, a normal test can provide evidence that the symptoms are not due to impairment in the peripheral nerves and muscles.

The EMG/nerve conduction study is a valuable tool used by neurologists to diagnosis the type of nerve or muscle disease and can therefore help establish the appropriate treatment.
