Individualized care helps Laurels’ COPD patients breathe easier

T

he staff at The Laurels of Toledo, a skilled nursing and rehabilitation center located at 1011 Byrne Road, sees a wide range of diagnoses and conditions among the facility’s longterm residents and short-term rehab guests. One of the more common disease states they encounter and manage is chronic obstructive pulmonary disease, or COPD. COPD is a chronic inflammatory lung disease that makes breathing difficult and gets progressively worse over time. The most common cause of the disease is cigarette smoking, including secondhand smoke, but according to the National Heart, Lung, and Blood Institute, long-term exposure to other lung irritants, such as pollution, chemical fumes, or dusts, can also contribute to COPD, even in people who have never smoked.

Laurels certified nurse practitioner Jennifer Lockard explains that the symptomology of COPD can vary significantly from individual to individual and at different stages of the disease. In the earliest stage, COPD may cause no noticeable symptoms, but as it progresses, the individual

may experience shortness of breath, chest tightness, wheezing, coughing that produces mucus, fatigue, and other symptoms. In later stages, the disease can become debilitating. “It’s also important to be aware that COPD can affect people emotionally as well as physically. Even with relatively mild cases, the impact on quality of life—for example struggling more with day-to-day activities or being unable to go outside in certain temperatures or environments—can lead to feelings of depression or anxiety,” she says.

Lockard notes that in addition to experiencing a wide range of respiratory symptoms, people with COPD are at somewhat higher risk of developing secondary infections, which can make them even sicker. She also emphasizes that because their lung tissue is already compromised, it’s essential

for people with COPD to bolster their immune system with vaccines against flu and pneumonia at this time of year.

Fu r t h e rmore, various comorbidities (medical conditions occurring simultaneously with another), such as hypertension, heart disease, heart failure, diabetes, and others, can be associated with COPD, which can complicate treatment and management of the disease.

Interestingly, not only are people with COPD at increased risk of developing comorbidities, but having one or more comorbidities can also lead to exacerbation, or flare-ups, of the COPD. Though COPD cannot be reversed or cured, it can usually be managed effectively. Treatment can take many forms, both pharmaceutical and nonpharmaceutical, again depending on the individual and his or

her specific symptoms. “For all our residents or guests with COPD, we advise smoking cessation as well as exercise to increase their lung capacity so they’re better able to function and cope with flareups,” Lockard says. “Management might also involve the use of oxygen, inhalers and nebulizers, steroids to help control flareups, as well as physical therapy to improve the individual’s lung health and endurance.”

The vital educational component of this eight-month challenge will be provided by ProMedica physicians who will present seminars at Super Fitness on a variety of health-related topics such as nutrition and cardiovascular health.

Participants will also take part in various fun and competitive activities both at Super Fitness and at other locations around the community. “For example, we’ll go to the firefighters’ training center at Owens Community College where contestants will perform different activities while wearing heavy firefighting gear. We’ll also go to the Sky Zone trampoline park in Toledo and a track-and-field event in Ida, Michigan. One of the activities we’ll do at Super Fitness in December is the popular Case Race, which involves breaking up into two teams, picking up boxes of food, carrying them around the track, and then taking them out to a truck that will transport them to the SeaGate Food Bank to feed the hungry. All these events, both in and out of the club, are intended to help participants have a good time and build camaraderie so they can help motivate one another to stick with the program and succeed,” Hemelgarn says.

The Super Fitness Weight Loss Challenge is open to the general public, not just to Super Fitness members, because Hemelgarn wants the program to make an impact on the entire community. He also points out that anyone can win and there’s no predicting who that winner will be at the outset of competition. Last year’s first-place finisher was actually one of the last to join the competition, but she was so motivated and committed to the program that she managed to lose over 35 percent of her body weight (percentage of body weight lost—not pounds lost—is the important metric at weigh-ins), edging out the second-place finisher who shed over 32 percent of his body weight.

Carve a jack-o’-lantern to die for H

alloween without jack-o’-lanterns would be like Christmas without the tree, Easter without the eggs, or Thanksgiving without the turkey. Those delightful, leering gourds somehow convey the very essence of Halloween as they glow eerily from every window, porch, stoop, and stairway.

The best thing about jack-o’-lanterns is that you can carve them as simply or as creatively as your heart desires. But if you’re growing tired of the traditional design—you know, triangles for the eyes and nose, a gaping mouth with square teeth, etc.—why not try something different this year?

The right tools for the job If you plan to get creative with your pumpkin, you’ll want to have an assortment of knives at your disposal. One or two large, sharp knives will be necessary for making major cuts, while a short, sturdy knife will be useful for detail work. You’ll also need some sort of scraping tool if you intend to create any layered effects.

A sharp, serrated melon spoon is ideal for this purpose. If little hands will be involved in the project, it’s a good idea to purchase a child-safe pumpkin-carving kit (available at any craft store or supermarket). Of course, you’ll also need a long-handled spoon or an ice cream scoop in your carving arsenal for digging out the pumpkin “guts.”

Let your imagination run wild Instead of working from the same old boring patterns, why not inject a little originality into this year ’s jack-o’-lantern? Work from storebought stencils, or, if you have the artistic skill, draw your own unique stencil freehand and transfer it to the pumpkin’s face with ballpoint pen.

Michelangelo used to say that each slab of marble had a sculpture trapped inside it, and all he had to do was free it with his hammer and chisel—or some words to that effect. In some ways, the same is true of pumpkins. If you examine each pumpkin’s shape carefully, the most appropriate design seems to suggest itself. For example, tall, droopy pumpkins seem to suggest a dopey face, while lumpy or wary pumpkins are best for monster faces.

Start carving Once you’ve settled on the appropriate design, it’s time to start carving. Begin by cutting a star-shaped lid from the top. It should be tapered so that the inside is narrower than the rind. This will prevent the lid from falling into the pumpkin once it shrinks, which is likely to happen if a candle is placed inside. Next, scrape out the seeds and fibers with the spoon or ice cream scoop and discard them or set them aside in a bowl for use in recipes later on.

As you cut, move slowly and carefully around your sketched design, being especially cautious around curves and detailed areas. Taper the edges and remove all loose pieces or dangling strings of pumpkin innards that might throw off your design. You might also want to cut a ventilation hole in the back of your pumpkin so adequate oxygen can

reach the candle and sustain the flame when the lid is in place.

Carving in layers gives you the greatest opportunity for originality. This involves scraping away layers of the pumpkin without penetrating all the way into the hollowed-out center. By scraping to different depths, you can control how much light passes through each part of the design. This technique is ideal for creating eyebrows or facial hair, for outlining the face, or for writing words or slogans.

Play it safe Be sure to supervise your children closely if they will be carving their own jack-o’-lanterns, especially if they will be using knives. Better yet, dispense with sharp knives altogether for the little ones and go with a childproof pumpkin-carving knife.

While candles are commonly used to illuminate jack-o’-lanterns, there’s no getting around the fact that they can be a fire hazard. A safer option is to use a chemical glow stick instead of a flame to illuminate your jack-o’-lantern. ❦

The Gardens of St. Francis adds new memory-care suites

“T

here is a need in our community among families with loved ones who have Alzheimer’s and dementia-related illnesses. The facts are staggering. In Ohio alone, there will be an increase of 13.6% of Americans age 65 and older with Alzheimer’s dementia between the years 2018 and 2025. It was imperative that The Gardens of St. Francis take action to add memory-care suites for families and their loved ones for their need now and in the future.

The memory-care floor will open in February 2020, and already we have families selecting new, private, beautiful suites for their loved ones,” explains Curtis Woodward, Executive Director of The Gardens of St. Francis.

The 2018 Alzheimer’s Disease Facts and Figures Study, published by the Alzheimer’s Association, reports that one in ten people (10%) age 65 and older has Alzheimer’s dementia. The percentage of people with Alzheimer’s dementia increases with age: 3% of people 65-74, 17% of people 75-84, and 32% of people 85 and older.

“Our role is to provide the very best in senior care and help families understand seniors’ needs. We hosted dementia specialist and advocate Diana Waugh for our current families to assist them in addressing the devastation felt by their loved ones dealing with memory loss. Diana discussed how to provide support, encouragement and a better life for both the person suffering from memory loss and those talking to them.

We plan more community-education seminars on senior health needs,” explains Mr. Woodward.

The Gardens of St. Francis’ 19 new, private memory-care suites will be in their own secured area with more intensive personal care and closer supervision by staff specialists 24 hours a day. This area will have an open center floor plan in which many of the suites open up to include three living rooms, a music area, a library, a beauty/ barber salon, and a huge activity room.

Memory Care has its own dining and serving areas for its culinary-prepared meals. Residents will enjoy the freedom to move about while remaining safe and secure. There is a secure garden area on the campus where memory-care residents can take advantage of the beautiful, serene grounds.

disease or other forms of memory impairment with a holistic and person-centered approach to care. Our specially trained caregivers will provide compassionate care in a safe and engaging therapeutic environment.

Mercy Health program identifies women at high risk of breast cancer T

he average woman’s lifetime risk of developing breast cancer is approximately 13 percent. However, a variety of different factors, both modifiable and nonmodifiable, can put women into the high-risk category, defined by the American Cancer Society as 20 percent or greater. These individuals commonly require a more frequent screening regimen as well as comprehensive education on preventive options. If genetics are determined to play a role, genetic counseling may be appropriate as well. For these reasons, it’s essential to identify women who are at high risk of breast cancer as early as possible.

Mercy Health is doing its part by offering a High-Risk Breast Program, a process that begins locally at the Mercy Health – Perrysburg

Cancer Center, Mercy Health – St. Charles Hospital and Mercy Health – St. Anne’s Hospital, as well as at Mercy Health – Defiance Hospital and Mercy Health – Tiffin Hospital.

“Our High-Risk Breast Program is a flexible, voluntary program that allows women who are undergoing a mammogram at any of our locations to receive a risk assessment, and if they are determined to be at elevated risk, we can then facilitate increased screening and other risk-reducing options,” states Michelle Hall, a licensed genetic counselor at Mercy Health – Perrysburg Cancer Center.

“Identifying women with high-risk profiles can lead to early detection of breast cancer that otherwise may have been missed or diagnosed at a later stage,” adds Dr. Suketu Patel, Mercy Health radiation oncologist.

The High-Risk Breast Program utilizes the newest version of the Tyrer-Cuzick risk-assessment tool, a validated model that estimates a woman’s breast cancer risk based on her age, body mass index, breast tissue density, obstetric history, family history of breast cancer, and a variety of other factors. Women who participate in the assessment simply answer a series of questions, many of which they are already asked during routine mammogram appointments.

Once this information is collected, the assessment tool uses it to calculate the patient’s lifetime risk of developing breast cancer. “The Tyrer-Cuzick assessment takes away the guesswork and makes it easy to quantify a woman’s risk. Instead of telling someone ‘We think you’re at

high risk,’ we can say, for example, ‘Your risk is 30 percent over the next 30 years,’” Hall explains.

If a woman is determined to be at high risk of breast cancer, the physician who ordered the mammogram will be notified of her actual risk score and provided options to discuss with the patient. These options might include increasing surveillance with annual breast MRIs staggered six months apart from the patient’s annual screening mammograms so her breast health is being monitored every six months instead of just annually. Consultation with a genetic counselor and a medical oncologist may also be recommended so the patient can get all her questions answered as well as discuss opportunities for genetic testing and other risk-reducing options, such as chemoprevention

“Treatment for early stage breast cancer is less intensive, has a much higher likelihood of cure, and has fewer side effects,” says Dr. Hassan Beydoun, Mercy Health radiation oncologist.

According to Hall, the purpose of genetic testing is to determine whether a patient has any hereditary risk factors for breast cancer beyond what can be identified with the Tyrer-Cuzick assessment tool.

“Women who carry hereditary risk factors can sometimes see a lifetime risk approaching 80 to 90 percent, so we offer these women even more advanced screening options,” she says.

Hall emphasizes that participation in breast cancer risk assessment is completely voluntary. Women who prefer not to know their lifetime cancer risk for one reason or another are free to opt out. However, she urges any woman who is concerned about her breast health or thinks she might be at high risk of developing breast cancer to talk to her healthcare provider about risk-reducing options and whether genetic counseling might be an appropriate next step.

Planning a safe and healthy Halloween

F

or youngsters all across our nation, the month of October means but one thing—Halloween is on its way. On this one special night of the year, kids get to fan out through their neighborhoods, dressed in outlandish costumes, with the goal of collecting a sack full of sugar-laden goodies. However, in their haste to accumulate as many treats as possible before the “witching hour” arrives (8:00—when the trick-or-treating ends in our community), kids are also quick to forget some basic rules of safety. That’s where parents can make all the difference with a little planning.

Safety starts with the costume Deciding what to be and putting the costume together is almost as much fun for kids as the actual trickor-treating. This is also the point at which parents should get involved to ensure their little ghouls get off on the right foot where safety is concerned.

Costumes should be bright, reflective, and short enough to prevent tripping, entanglement, or contact with flame (e.g., the candles in those porch jack-o’-lanterns). It’s a good idea to incorporate a flashlight and bands or strips of reflective tape into the costume’s design so your child will be clearly visible to motorists.

Law-enforcement authorities also suggest that emergency-identification information, including your child’s name, address, and phone number, should be discreetly concealed within each child’s costume or worn on a bracelet.

Home fire safety must also be considered. Make sure electrical circuits aren’t overloaded with Halloween lighting or decorations, and position jack-o’-lanterns at a safe distance from drapes, decorations, flammable materials, or any areas where costumed children will be walking. To eliminate the risk of fire from jack-o’-lanterns altogether, light them with chemical glow sticks instead of candles.

If your children are old enough to trick-or-treat without parental supervision, make sure you know the neighborhoods they will be visiting and the exact route they will take to get there. Agree upon a time for them to return, and equip them with a cell phone so they can call or text you in the event of any unforeseen delays or call 911 in the event of an emergency.

ctober has been designated National Physical Therapy Month to raise awareness of the benefits of physical therapy as well as to recognize the vital role professionals in this discipline play in improving patients’ health, function, and overall quality of life.

What exactly are physical therapists (PTs) and what do they do? According to the American Physical Therapy Association, PTs are “health care professionals who diagnose and treat individuals of all ages, from newborns to the very oldest, who have medical problems or other health-related conditions that limit their abilities to move and perform functional activities in their daily lives. PTs examine each individual and develop a plan using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability. In addition, PTs work with individuals to prevent the loss of mobility before it occurs by developing fitness- and wellness-oriented programs for healthier and more active lifestyles” (apta.org).

Elaborating further on this textbook definition, here are just some of the ways physical therapists help patients:

An extensive range of treatments and modalities PTs today have access to an extremely wide variety of therapeutic treatment options, ranging from simple heat or ice, to aquatic therapy, to more advanced modalities such as electrical stimulation, ultrasound, and laser or light therapy for reducing pain and accelerating healing. Some have even begun to utilize state-of-the-art technologies, such as specialized treadmills or robotic exoskeletons, that augment patients’ strengths and abilities while compensating for limitations to help them get back on their feet faster.

Of course, PTs also have extensive knowledge of the various exercises and movement strategies that can help improve patients’ strength,

rehab program because they’ve learned how to perform all the exercises and have already practiced them. In addition, they develop a high degree of comfort and familiarity with the rehab facility and its staff ahead of time.

Pain control

endurance, range of motion, balance, gait, and overall function and mobility, and they can provide expert training on the proper use of any assistive devices patients may require as they progress through treatment.

More innovative PT professionals are also drawing upon elements of disciplines such as tai chi and yoga to help improve deficits in balance, stability, coordination, or other areas.

Inpatient and outpatient rehab The role people most commonly associate with physical therapy is rehabilitation following a debilitating injury, surgery, illness, or prolonged period of immobilization/bed rest. In these circumstances, PT focuses on helping patients regain their prior level of function to the extent possible and getting them back to their daily lives, favorite sports, or other activities. Rehab may involve both inpatient and outpatient phases as well as a home-exercise component so patients can maintain and build upon the gains they achieved in formal therapy.

Prehab A relatively new but growing aspect of physical therapy is so-called prehab. Prehab is a form of proactive outpatient therapy that improves strength and function prior to surgery so patients are able to recover and return to normal activities and independence much more rapidly afterward. Patients who take part in a prehab program also have the advantage of knowing exactly what to expect from their post-surgical

For example, if a patient has an arthritic knee that is causing pain and discomfort when standing and walking, PT to strengthen the supporting musculature around the knee and to improve the patient’s gait mechanics may make it possible to postpone or (depending on the patient’s age, the degree of joint degeneration, and other factors) even avoid knee-replacement surgery.

Patient education Last but not least in this far-fromall-inclusive list is providing patient education. From the very beginning of a patient’s inpatient or outpatient rehab program through his or her transition home, this element of a PT’s job pervades all aspects of treatment and is critical to ensure proper compliance with exercises, prevent injury or reinjury, and promote the best possible long-term outcome. ❦

What PT really stands for

ctober is National Physical Therapy Month. Most of us have an idea of what physical therapy is and what physical therapists do, but do we really understand the full benefit?

One of the stereotypical comments that are heard regarding PT is that PT stands for “pain and torture.”

That really is the furthest thing from physical therapy and what physical therapists want to do! It’s true that individuals may experience some pain as they progress in therapy, but this is typically related to using muscles that haven’t been used or used correctly to improve movement and function.

Who are physical therapists?

Physical therapists are evidence-based healthcare professionals who diagnose and treat individuals of all ages who have medical problems or other health-related conditions that limit their abilities to move and perform functional activities in their daily lives. They offer cost-effective treatment that improves function and relieves pain, reduces the need for surgery and prescription drugs (especially opioids), improves mobility, and allows patients to participate in a recovery plan designed for their specific needs. In addition, physical therapists work with individuals to prevent the loss of mobility before it occurs by developing fitness and wellness programs for healthier and more active lifestyles.

Physical therapists are essential participants in the healthcare delivery system. They assume leadership roles in rehabilitation, prevention, health maintenance, and programs that promote health, wellness and fitness.

To establish diagnoses, prognoses, and plans of care, physical therapists perform evaluations/ consultations, as well as diagnose patients’ functions and movements by observing them stand, walk, or perform activities/tasks; performing various tests and measures; and listening to their concerns. Therapists then design individualized plans of care based on information obtained, incorporating techniques such as exercises, hands-on therapy, and equipment to ease patients’ pain, help them increase their mobility,

prevent further pain or injury, and facilitate health and wellness. Therapists evaluate patients’ progress and modify their plans of care, when necessary, to try new treatments.

Lastly, they educate patients and their families about what to expect and how best to cope with a recovery process.

A WALK IN THE PARK

the road are above treeline where there is nothing at all to obscure the view. Unless, that is, you share the opinion of Shirley’s flatland farmer father who claimed that “Mountains just get in the way of the view.”

For most people, though, the experience is literally breathtaking.

Not only because the visuals are stunning but because the air gets pretty thin up there. So, we spent a day acclimating at 6,000 feet or so before heading up the road and doing any strenuous hiking. Altitude sickness can leave visitors not only short of breath but headachy and prone to irritability as well. Some of us geezers are prone to crankiness all the time, so it is helpful to have the altitude to blame for it. Which I do even at sea level.

Fortunately, there is plenty to see at somewhat lower elevations.

One of our favorite walks is around Bear Lake and then out to Alberta Falls. In 1980, I just drove to the

end of the Bear Lake Road and parked the good ol’ Buick Estate Wagon. In recent years, however, park visitorship has increased so dramatically that the parking lot fills early and the rangers insist you take the shuttle buses.

But even catching the shuttle bus can be a challenge. We had a nice camp breakfast then went to the bus stop about 9:00. Found that a couple hundred people must have skipped breakfast to get there ahead of us. And the slugabeds continued to arrive after us. A steady stream of buses left as quickly as they could be loaded and took only about 15 minutes to reach the lake.

The trail around subalpine Bear Lake is justifiably popular because it is spectacularly scenic with Hallett Peak, Half Mountain, and Longs Peak rising above it. The shore is lined with spruce, fir, aspens, and lodgepole pines that smell wonderful.

Along the way, in the middle of July, Shirley and I met our daughter Sarah and her kids, Nick and Lizzy, for a week camping in Rocky Mountain National Park just north of Denver. This was important to us because Sarah had learned to love the Rockies on our first trip there in 1980. She wants

her own children to have the kind of opportunities to explore America that she did.

The central focus for most tourists in the park is Trail Ridge Road, the scenic route that climbs into the tundra for grand vistas, wildlife, and wildflowers. There are numerous pullouts along the way from which you can pause to enjoy all these features. It is the longest continuous road at that elevation in the US, topping out at 12,183 feet.

Alberta Falls where visitors were taking turns getting family pictures and selfies with the falls in the background. The falls were running heavy because of the greater-than-usual snow pack from last winter. As at Bear Lake, there was a greater-than-usual pack of people as well.

The connecting trails we walked that morning totaled about four miles, but much of what you want to see in Rocky Mountain requires surprisingly little effort. Along Trail

When I asked a park volunteer about its condition, he said it had only been open two days and there had been neither sufficient traffic nor foul weather thus far to cause any significant deterioration.

So, we set off in Sarah’s rented SUV. The speed limit is 15 mph, which, believe me, is plenty fast enough given that there are no guard rails

to prevent misadventures with the many sheer drop-offs. Stretches are also quite steep with grades of up to 16%. There are numerous hairpin turns, some with a radius of only 20 feet. Vehicles over 25 feet are forbidden because drivers could find it difficult or impossible to make the turns. Also, much of the road is only 14 feet wide—not enough room for cars to pass safely—so it is now one-way up bound.

Trail Ridge Road winds its way from the Estes Park entrance to the Alpine Visitor Center and then descends to the west. But it is no longer the only way to get up there.

heavy streams of snowmelt spilling down the steep mountainsides. Once above treeline where there were still sizeable snow banks, we stopped for the obligatory mid-July snowball fight just as staged by Sarah and her sisters back in the day.

Still, the highlight of the week in the Rockies for Sarah, Nick, and Lizzy was probably their trail ride.

They were particularly fortunate because they were the only three scheduled for their time slot so they got a private two-hour tour. Which turned out to be quite enough for riders whose thighs were unaccustomed to hugging a horse.

Back in camp, we were welcomed by a pair of wild turkey hens who were habitual visitors and totally

accustomed to sharing their space with campers. But not with other turkeys. A third hen made repeated efforts to join them but was ungraciously escorted away whenever she tried. They reminded me of the snooty, mean girls in the junior high cafeteria who refused to eat with anyone of lesser social rank.

Our most impressive camp visitor, though, had all the rank you could wish for. In early evening, a majestic bull elk, his rack still in velvet, nonchalantly ambled on through and disappeared into the lodgepole pines behind our site.

had met their share of campsite wildlife on earlier occasions. Elk, for example, made a daily practice of passing through when we were in Yellowstone two years ago. And last year in Shenandoah, there was a black bear that came to see if there were any unattended picnic baskets. If things continue at this rate, within a couple years they will be as jaded as their grandmother.

When Shirley sees a bear, she is likely to dismiss it with “Poo! He’s just a little one.”

With that said, the kids are perceptive enough to note the obvious joy we take in the mountains, and they reflect our enthusiasm. Sarah is already hinting that next year perhaps we can find a way to meet up in Yosemite. The logistics are a little more complicated, but our experience is that suitable motivation can reduce many obstacles to just a walk in the park.

Dr. Masroor further explains that after the advent of minimally invasive CryoMaze, a procedure called catheter ablation was introduced for treating AFib. As the name implies, this procedure involves threading a catheter through a blood vessel in the patient’s groin and up into the heart where heat or cold is used to ablate the tissue that is producing erratic signals. However, Dr. Masroor emphasizes that this technique has a much lower success rate than CryoMaze—only about 70 to 80 percent in the best hands. And because success is not guaranteed, patients who undergo catheter ablation are kept on blood thinners in case their AFib returns. “Many of my patients have had failed catheter ablations, but whether they underwent a prior catheter ablation that failed or they never had one, the success rate for CryoMaze is the same—90 to 95 percent,” he says.

Of course, not all patients with

AFib are good candidates for CryoMaze. Those who aren’t still have other good options available to them.

For example, AFib patients who are at risk of stroke but can’t take blood thinners and are too old to undergo Maze, can have an atrial clip, which closes off the left atrial appendage to reduce the risk of clot formation inside the heart. “Patients who undergo atrial clipping may not need to take blood thinners any longer even though their AFib has not changed,” states Dr. Masroor.

nobody’s perfect It’s a fair world after all!

“A

unt Karen, it’s fun to go places with you,” my nephew Matt told me when he was ten. “We get to go to the front of the ride lines!”

My sister lived in Los Angeles several years ago, and my nephew and I flew out there to do the “California” stuff—Grauman’s Chinese Theatre and the Hollywood stars, viewing the beautiful Pacific Ocean at Malibu and other beaches, and, of course, Disneyland! Disneyland is relatively small at about 500 acres compared to Florida’s Disneyworld at many thousand acres.

But there was something about being at the original park that Walt Disney began with over 50 years ago. So we went.

It was the middle of summer and hot in LA. I used my electric cart and found that I was able to go to the back of the line that was designated with the universal handicapped logo (the little blue person in the wheelchair). The people running the rides would then let up to six people in a handicapped party go into the line in between regular visitors who lined up. It was very nice and I know that I, or other people with disabilities, could never have seen the park so comfortably if Disney had not allowed the privilege of more frequent entry.

I would have been withered with my MS from the heat.

So we rode on several rides and had a wonderful day. I was so grateful that Disney made allowances for persons with disabilities and, again, felt a little embarrassed that people without disabilities had to wait their turn. That is just my hangup.

Then, it came to my attention that many people without disabilities were getting the treatment allowed those who had physical problems.

It was similar to people abusing handicapped parking at stores and in parking lots.

Mickey and Minnie are pretty smart, and Disney wised up. Michelle Himmelburg reported in The Orange County Register that after years of issuing passes to people who say they couldn’t stand in the regular lines because of an ailment or disability, Disney is asking a lot more questions now before allowing anyone to move ahead to the front of the line. Himmelberg’s article continues, “The fact that they’re making it harder to get help only means that the people it was designed for in the first place are going to use it,” said John Kennerknecht of Fontanta, whose son has cerebral palsy. “Honestly, I’d rather stand in line for two hours with a totally healthy son, but it’s irritating when you see the abuse of the system.”

Many visitors to Disney’s Parks have had “special assistance passes” for so long that a sense of entitlement has developed. Some visitors are enraged about Disney’s tightening of special assistance passes and others are delighted. The system is tough, since there are many persons who have disabilities that are not that visible. Even Disney admits that gone are the days when a teenager would flip a coin to see who would rent a wheelchair that day and then use the vehicle to get a pass to the front of every line for their group of six.

The system of special assistance passes became so abused that “special assistance” lines sometimes became 30 to 40 minutes long. As those lines converged with fast pass or regular lines, bottlenecks backed up the boarding of rides, and then visitors from all three lines complained.

Disney acknowledges that the old system was too lenient, creating days when up to 20 percent of visitors had special passes. But even now Disney will not issue a written policy because it could open loopholes that n Nobody's perfect - continued on p42

Running tips for beginners

A

re you interested in running but afraid of the first steps? Here are a few tips to get you off on the right foot and on your way to enjoying the sport of running. Most important, consult your physician before starting any exercise program.

As a runner, the most important piece of equipment is your pair of running shoes. Before starting your running program, visit a shop that specializes in running shoes, such as Dave’s Performance Footgear, for an assessment of your needs and a personalized fit.

One of the most common complaints of a new runner is, “I can’t breathe.” Focus your energy on exhaling rather than inhaling. As your need for air increases, think about forcing the air out of your lungs rather than sucking it in. This will make you less tired than when you are huffing and puffing and will also help prevent hyperventilation.

In the beginning, take frequent

short runs after resting sufficiently.

Your runs should last long enough for your body to get a workout but still be able to recover in time for your next run. If you over exercise and your muscles do not recover to their original condition, you will not gain any benefit. Strength is gained during rest when your muscles repair the fibers that are broken down during exercise. But remember, every runner is different when it comes to the amount and frequency of exercise that is appropriate. Listen to your body.

As a novice runner, take quick steps rather than long ones. You will not tire as fast if you take short, fast steps rather than long, slow ones, especially when you are tackling hills.

When running downhill, lean your body forward in order to try to keep your body perpendicular to the hill.

Novice runners need motivation and a schedule. Join a beginner running group, or seek out a running partner. Some runners find that taking music along on their workouts helps make running fun. When

using headphones, though, play the music at a reasonable level so you can hear traffic noise and remain alert to potential hazards. Speaking of hazards, if you are running alone, carry identification and always run against traffic, preferably on sidewalks or running paths.

One of the best ways to improve and get more enjoyment out of running is to enter road races. The experience of finishing a running

race is exhilarating. Start with the goal of a 5K (3.1 miles) road race, “fun run,” or a relay race with your running friends. Races can provide the foundation for enjoying a long life of running.

EATING WELL Exploring the sugar-cancer connection

W

orking in a hospital environment, I’m often told by cancer patients that they don’t eat any sugar for fear that it will make their cancer worse. In fact, the idea that sugar fuels cancer cells has led some people to avoid carbohydrates altogether. Unfortunately, imposing a total ban on carbs is counterproductive to patients who are already struggling with the disease process of cancer and trying to maintain their weight.

So where did this misguided notion come from?

The sugar-cancer connection has been a hot topic for the past four to five years, though the idea that tumors thrive on sugar has been around since the 1920s. As you might expect, the media and internet continue to be sources of speculation and misinformation in this regard.

While researchers continue to investigate the controversial connection between sugar and cancer, this is what we do know: All our cells need glucose—sugar— for energy. It is the only fuel our brains like to use. Glucose is so important that the body has several backup methods to produce it if needed to maintain and save itself. Even without carbohydrate in your diet, your body will convert protein and fat into sugar.

Then there is the stress associated with shunning all sugar. You might not realize it, but reading labels, shopping, and meal planning with the intent of avoiding all carbohydrates demands a lot of work and worry. That stress can actually turn on your fight-or-flight mechanism, which can, in turn, raise your insulin level and decrease your immune function. In other words, the fear and anxiety associated with thinking that any carbohydrate intake will make cancer worse is very stressful and unhealthy in and of itself.

Of course, none of this means I’m

giving sugar a completely free pass.

There does seem to be a relationship between higher sugar intake and higher levels of insulin and related growth factors that may influence cancer cells. However, higher insulin levels can also be related to other risk factors such as genetics, inactivity, and obesity. Studies are being done on no-carb and very-low-carb diets like the Ketogenic diet, which limits carbohydrates to 20 gm per day, but more research is needed and nothing has been concluded yet. Always consult with your physician before starting this type of meal plan.

Another thing we know is that it’s important to be smart about meal planning if you want to reduce cancer risk or if you already have cancer.

The American Institute for Cancer Research promotes the MyPlate model, which can simplify meal selections and is most beneficial when combined with regular exercise. Of course, this model includes avoiding excessive intake of simple sugars (pop, cakes, cookies, pies, candy). The American Heart Association advises consuming no more than six teaspoons of sugar per day for women and no more than nine teaspoons per day for men. Be sure to include sources of natural sugars such as fruit and milk.

Choosing organic fruits and milk is optimal but not essential. Also, be sure to balance your diet with lean meats, fish, and whole grains, and try to eat routinely throughout the day, not just one or two big meals.

Aim for six smaller meals if possible.

This will help with hunger, lack of appetite (common in cancer patients), and insulin levels.

The bottom line is that eating natural sugars (fresh fruit, milk, whole grains) in moderation on a daily basis is encouraged. On the other hand, limiting simple sugars/concentrated sugars is recommended for everyone, not just people with cancer. So, eat fewer processed foods, choose more n Eating Well - continued on p46

ollowing a motorcycle accident in 1995, Melanie Rowland underwent an amputation of her lower leg. By all accounts, she adapted to her prosthesis. She was able to work full time and actively participate in the community.

Even so, she wasn’t living the life she wanted. What was troublesome for Melanie was that she had resigned herself to the fact that engaging in hiking or recreational activities came at a cost. Not only did she feel like the prosthesis held her back from more physically demanding activities, but when she tried, she found herself in increased pain.

For Melanie, the turning point came on a fall weekend. After shopping with her daughter and raking leaves, she developed a deep leg bruise, requiring her to use crutches until she could heal. After this incident, Melanie was determined to find a solution.

She was referred to Zach Weber at Reform Prosthetics. Reform’s mission is to work with patients in order to provide individualized solutions so they can reach their mobility and quality-of-life goals.

Melanie’s case is an excellent example of how that can work. The typical treatment for a hematoma is to remove the prosthesis until the area is fully healed—the exact course of action that put her on crutches.

Reform Prosthetics’ focus of discovering innovative ways to meet the goals of individual patients made it clear that this treatment was unacceptable for a full-time working mother of two. Reform refit Melanie with a custom solution that not only

allowed her wound to heal, but also allowed her to engage in activities she didn’t think were possible as an amputee. Since being refit, Melanie has completed a 5K and two half-marathons, and she is now participating in a cross-fit training program that has helped her lose over 30 pounds.

“It’s unfortunate to meet with patients who compromise on the activities they choose to be a part of because they wear a prosthesis.

Melanie is a common example where the patient is motivated to do well but needs to be fit with the right equipment. She clearly identified a list of her goals as an amputee, and we focused on a treatment plan that would complement those goals,” Weber says.

Unison fights opioid addiction with whole-person approach and full care continuum

T

he crisis of opioid addiction and overdose has taken a heavy toll in communities all across our nation, including here in Northwest Ohio. Impacting every segment of the population, opioid addiction does not discriminate based on age, race, gender, income, education level, or any other factor. There is no “typical” opioid addict, and no one is immune.

The good news is, Unison Health is meeting this crisis head on in our community, drawing on a full continuum of care services to treat the entire person, not just the addiction.

Unison treats a wide range of substance-abuse and behavioral-health issues, but the organization is currently putting special focus on opioid addiction. “Cocaine, crack, and other drugs have not gone away, and we’re not ignoring them, but right now the overdose rate with opioids is so high in our community that we have to give it precedence,” says Tiffany Hairston, PhD, LPCC-S, LICDC-CS, NCC, Clinical Coordinator of Outpatient Alcohol and Drug Treatment Services for Unison.

Unison’s care continuum includes a 16-bed, medically monitored detox unit that helps both men and women safely rid their body of opioids or other substances. After detox, they can be stepped down to recovery housing

with treatment, where they continue to stabilize, and then to intensive outpatient care. Client’s programs might include medication-assisted treatment, or MAT, which involves the use of medications such as Suboxone, Vivitrol, or Sublocade in combination with counseling and behavioral therapies to wean them safely off opioids and help sustain recovery. “We provide all levels of care as well as various aftercare services if needed to give our clients everything they need to stay on the path to recovery and become productive members of society,” says Hairston.

Treating the whole person means addressing clients’ physical and mental health as well as their behavioral health, which is why Unison provides a wide range of services. Hairston explains, “When someone comes in for treatment, there’s usually a lot going on with them, not just their addiction.

In addition, Unison has case managers on staff who help clients overcome a wide variety of challenges and obstacles that might otherwise prevent them from complying with treatment or continuing in recovery.

If they’re lacking access to basic necessities such as food or clothing, the case managers can connect them with a food pantry or a source for donated clothing. They can also assist them in signing up for supplemental nutrition assistance, insurance, low-income housing, and other vital programs and resources, as well as assist them with issues related to children services or the criminal justice system. “We even have case managers who will help people fill out job applications if they don’t know how to do it themselves or how to do it well enough,” Hairston adds.

Though Unison stands ready to help people overcome addiction with a full continuum of care, the organization is working to get ahead of the opioid crisis by promoting prevention in the community. “For instance, we’re connecting with kids to teach them what drugs are and how they can affect their lives. We’re also trying to educate parents, teachers, and everyone else in the community on the potential warning signs of 1

substance abuse. A few examples include changes in appearance, dilated pupils, changes in weight, dental issues, poor personal hygiene, changes in behavior or mood, and secretive or manipulative behavior,” says Hairston.

Hairston notes that “It will never happen to me or one of my loved ones” is one of the biggest myths about addiction. Nor is there a face or characteristic that universally applies to addicts. “Addicts are not bad people, and they don’t deserve to be stigmatized,” she emphasizes. “Anyone can have an addiction, from any segment of society, who has made just one too many bad choices and is now spiraling out of control. It literally could be you or anyone you know.”

Hairston also cautions parents that they can’t assume their kids are too young to be exposed to opioids or other drugs. “Some kids today are being introduced to drugs at a very early age, so don’t assume you don’t have to worry about it or learn the warning signs because your child is only in fifth grade,” she says.

ctober is one of my favorite months of the year. It brings with it my favorite season of the year, autumn. Five members of my family were born in the later months of the year, and my grandparents and my dad died in the last months of the year. School began for me first as student and then as teacher in the autumn of the year. I love to see the leaves turn their brilliant and bold colors, and at the same time I enjoy the cool mornings and evenings and revel in the sounds of the cicadas and crickets as they replace the songs of the birds. As I recall why I love October, I am aware that it shows me that “everything we do matters.”

As you can see from the beginning of this article, this time of year has always been a time of joy and sorrow, peace and pain, life and death, celebrations and challenges over the years. I consider it a great grace that I am able to see at this point in my life that everything that has happened over my 75 years on the planet does matter in some way that I initially did not always appreciate.

Those who know me know that I would rather have been born in May, which I consider the perfect month. I do however appreciate being born in November because my mother and one of my best friends were born in that month as well as my soon-to-be-96-yearold uncle who is the last surviving member of his generation in the

Thill family. These births matter to me as I reflect on our lives and how much we have all contributed to life and living. It also amazes me that our lives matter to all the people we have touched in so many ways as spouse, parent, friend, sibling, teacher, nurse, chaplain, etc. At least I like to think that everything we have done matters to someone in some way.

When I think of Greta Sibley’s comment that, “There is no alternate utopia running parallel to this life.

This is it,” I’m struck by the truth in that statement. When I would like to escape from the difficulties in life, the violence, the pain, the fear, the poverty, the evil that sometimes seems to overwhelm us as a society, I am aware that most, if not all, of these challenges are part and parcel of life on planet Earth and that they eventually pass and we live one more day, one more week, one more month, one more year. We also often forget what has happened and move on realizing that this is it. My sister, Roberta, used to say, Get over it! And we do. This is part of LIFE! I think that is a blessing and hope to always remember that everything we do matters because it does.

Do you know the symptoms of Alzheimer’s?

A

s we get older, it’s normal to become more forgetful. Recollecting the name that goes with a familiar face, finding the right word in conversation, locating a misplaced set of car keys, or recalling the reason for entering a room can become more challenging with age. But it’s critical to distinguish between typical age-related forgetfulness and the symptoms of Alzheimer’s disease. Recognizing and heeding the early warning signs of Alzheimer’s can help those affected reap the greatest benefit from treatment, maintain their independence longer, and take a more active role in planning for their future care and support.

Here are some common warning signs to watch out for:

Persistent and progressive memory loss In contrast to normal age-related memory lapses, Alzheimer’s-related memory loss tends to be more persistent and troubling and becomes worse over time.

Some signs to watch for include forgetting recently acquired information; forgetting important dates, appointments, or events (and not recalling them later); repeating the same questions or comments over and over; misplacing possessions in peculiar locations; and forgetting the names of loved ones or everyday objects.

People with Alzheimer’s, however, may lose the ability to remember the day, month, season, year, or even the stage of life they’re in. Also, spatial and visual disorientation associated with the disease can lead to a sense of confusion or being lost in familiar surroundings.

Exercising poor judgment Alzheimer’s also has a negative influence on judgment and decision-making ability. For example, someone with the disease might go outside in the middle of winter with bare feet and wearing only pajamas, or someone who was once an astute money manager might begin to exercise poor financial judgment, such as giving away money that he or she needs to cover expenses.

Beyond simply struggling to find the right word from time to time, people with Alzheimer’s may develop more profound difficulty with vocabulary in speech or writing. They may have a hard time joining or continuing a conversation, identifying or giving the right name to common objects, and reading and writing. Similarly, they may develop problems with abstract concepts like numbers, which can lead to difficulty with balancing the checkbook, paying bills, and managing other aspects of finance.

The havoc that Alzheimer’s wreaks in the brain can cause people with the disease to experience a range of emotional issues, such as anxiety, depression, fearfulness, or mood swings.

They may also withdraw from social situations they once enjoyed and exhibit behavioral changes, such as increased irritability, suspiciousness, or aggressiveness.

Optometrists have seven to nine years of training after high school graduation. They have comprehensive training in optics and refraction (the art and science of determining

the glasses or contact lenses that give each eye its sharpest vision). In addition, optometrists can prescribe medications for treatment of glaucoma and eye infections. Optometrists counsel their patients regarding surgical and non-surgical options that meet their visual needs, and they often help in the post- operative management of patients who have had cataract surgery.

Good Grief of NW Ohio announces 7th annual community breakfast

ne in 12 young people in Ohio will experience the death of a parent or sibling before they reach 19. In Lucas County alone, that’s nearly 10,000 kids. Good Grief of Northwest Ohio is helping these children and teens find their way to a “new normal” through peer support groups in their Toledo, Ohio program space.

On Thursday, October 17, from 8:00 to 9:00 a.m., Good Grief will host over 350 people at The Pinnacle in Maumee for an hour-long program to enjoy breakfast and learn about the important resource Good Grief provides for this community, both in its Toledo program space and in four of the area’s school districts.

Current and past family participants, volunteers, staff, and community members will share their stories of how Good Grief has made a positive difference and helped them pick up the pieces of a life shattered by the death of someone they love. A

t times, individuals living with dementia are unpredictable in their ability to communicate their needs and the way in which they express themselves. As such, when someone displays behaviors or actions outside of the norm, they are often attributed to internal factors (e.g. their personality or their dementia).

This concept is also known as Fundamental Attribution Error.

As explained by Sonya Barsness (2019), “Fundamental Attribution Error maintains that we attribute a person’s actions more to his or her internal factors rather than external factors. Internal can be our personality, disposition, motivation, etc.

External factors could be the situation, or how we are acting towards the person…[W]e overemphasize the influence of these internal factors on their [individual with dementia] behaviors, and underemphasize, or even ignore, the influence of external factors on their behavior.”1 According to Barsness (2019), we should pay more attention to external factors in order to better care for individuals with dementia.

This approach would include being cognizant of how an individual with

dementia perceives the way they are being treated.

Fundamental Attribution Error explores the idea that it is more important for “us” to attempt to understand and change the lens from which we view the people we care for, rather than placing the burden of behavior change on the individual with dementia. The approach to working with individuals with cognitive impairment has been a topic of discussion in caregiving for decades. It may be time to self-reflect and examine the way in which the paradigm of dementia care can shift.

Ultimately, if we are able to leave behind our own opinions, biases, and preconceived notions of dementia and how it impacts an individual, and instead meet them where they are, we have the potential to revolutionize the quality of care provided to individuals with dementia and older adults in general.

Our journey with a “butterfly child”

T

he birth of your child is normally a day of great joy and happiness.

For us it started out that way but quickly became a day of a thousand questions with no answers. In the very early morning hours of October 19, 2017, my family’s life changed.

I had a good third pregnancy, had a textbook delivery, all natural, until my husband along with the Labor and Deliver y team at St. Luke’s Hospital noticed some damage to Landon’s skin around his nose, lip, and a few other spots. Then they noticed the skin on his hand had completely torn away. This began to raise some flags for us.

The NICU team from ProMedica Toledo Children’s Hospital was there to see another baby, and they asked them to look at our little Landon. Once they examined him, they suggested he go to ProMedica Toledo Children’s Hospital for further observation, so they prepped transportation for him. He was not even at ProMedica Toledo Children’s Hospital an hour when they said he needed to go to the University of Michigan for further testing and treatment.

At this time, my husband got to the NICU at ProMedica Toledo Children’s Hospital and saw Landon and it took everything for him not to completely break down. He noticed additional unexplained damage with Landon’s skin. Landon’s fingers looked worse, and his feet now had some blistering.

The doctors at the ProMedica Toledo Children’s Hospital NICU came in to see my husband, and this wasn’t just a couple of doctors, but a team of doctors and staff. They explained to my husband what they believed Landon had, which is called epidermolysis bullosa (EB). This condition causes the skin to tear or blister from friction or pressure by touch.

Children with this condition are

called “butterfly children” because they are as fragile as a butterfly’s wings. This condition affects not only the top layer of skin, but it can also affect the organs, esophagus, and airways—basically anything with tissue (i.e. the whole body).

EB has been known to be fatal, but many kids go on to live long and normal lives (just a little more cautious). EB has various types and subtypes (yes, a child could have various types).

Also, each type and subtype can vary in terms of severity. Landon has been diagnosed with the subtype recessive dystrophic epidermolysis bullosa, one of the worst types of EB. This is a rare skin condition that only affects approximately 200 children a year. Unfortunately, there is no cure or treatment for this condition.

The only thing we can do is limit opportunities to cause issues as well as provide pain management and daily wound care.

Do you know what it means to have a child with epidermolysis bullosa?

For us, it means having to thoroughly inspect every inch of Landon's skin every day. It means taking a sterile needle to every new blister to lance and drain it so it doesn’t get larger and more painful. It means salt baths to prevent infection from many open wounds and covering him head to toe in ointments. It means wrapping his hands, feet, knees, and elbows with specially designed bandages otherwise intended for burn victims. If we don’t wrap them, his skin will blister and tear with the slightest friction.

It means going to countless doctor appointments on a regular basis. It means seeing all the good that comes with the bad—and knowing in my heart that we are incredibly lucky to have been blessed with a brave butterfly child.

We get asked all the time how we do it. The simple answer is because we have to; we are our son’s only hope against this disease. Honestly,

if it were not for his perseverance to not let this disease slow him down, I do not think we could do it. Early on, we would be minutes or seconds away from throwing in the towel. Thankfully we have each other (and Landon’s nurse) to support and build us back up when we hit a low because of a bad bandage change or we dwell on his future and the challenges that will come with it.

Most of our time and energy is spent monitoring and treating his wounds for infections or to prevent infections. He has had MRSA, strep, and staph infections already at his young age. Due to the constant state of healing our son is in, we have to monitor his nutrition to ensure he is getting the high level of calories and other key nutrients he needs more than most kids his age.

On the opposite side, we have had some victories we never thought we would—one of those being walking.

We talked to other EB families, and many said it is a constant struggle for their child to walk. Landon picked

up walking early on and by about 15 months was on the go-go-go.

The future for us and for Landon is very unknown. Unknown from the standpoint of how long he will live given the life expectancy of those with EB. Also if and when we will have to start using a feeding tube. Our goal for our future with Landon is to live every day in the moment with him, to enjoy that time with him and not dwell on all the unknowns and what ifs. For us it is hard to complain about our day when we come home to our son who is always smiling and happy given everything he is going through. We work every day to raise awareness for this disease with the hope and end goal being a cure or treatment for those with EB. C

hronic pain and inflammation feel like little forest fires breaking out all over your body. Old arthritis, failed back surgery syndrome, and hip and knee pain really slow us down as we get older. Most of this chronic pain stems from chronic inflammation.

Inflammation is the body's response to injury. Most acute inflammation comes from a new injury, such as a cut, burn, bruise, or infection. Acute inflammation usually dies down within a couple weeks once the injury is healed completely.

Chronic inflammation is different. As the name implies, chronic inflammation can last for years as the body finds it can’t completely heal whatever the problem is. Things like arthritis of the back, hips, or knees; scar tissue from previous surgeries; and even autoimmune disorders such as rheumatoid arthritis can leave lingering chronic inflammation that can be disabling.

Inflammation causes redness, swelling, and increased local supply of white blood cells, and is usually accompanied by pain. Think back to how your ankle swelled and hurt when you twisted it or how a skin cut healed with a red scab. All those are from acute inflammation.

Perhaps one of the largest sources of chronic inflammation and disabling pain is osteoarthritis of the neck, back, hips, and knees. This can arise from the wear and tear of a hard life or from scar tissue from an old surgery.

Over time, local inflammation will result in the buildup of acids and irritants in the local tissues, which causes the typical toothache-like deep, burning pain in the back and adjacent joints.

In the past, patients clamored for pain relief and were prescribed opioids for the chronic pain. The cycle of ever higher doses has led to the current opioid crisis in America.

Recently, the Veterans Health network, Ohio Worker’s Compensation, as well as Medicaid programs across the state have added a new weapon against the chronic pain for failed back surgery and osteoarthritis.

They have turned away from narcotics and enlisted acupuncture as a treatment modality.

Acupuncture works by inhibiting

The biggest hurdle to getting healthy and decreasing inflammation is the feeling that nothing can be done and you’ll be in pain forever. However, just taking the first few action steps by making slight modifications in diet and adding a small amount of exercise can return big and easily identifiable dividends in how you feel.

Acupuncture may also be beneficial in managing chronic pain.

Acupuncture and diet for chronic pain control pain pathways in the spinal cord and stimulating production of endorphins, the body’s natural pain killers, in the brain. A newer form of medical acupuncture, termed microstim acupuncture, can even provide extra healing energy into a specific spot of the body—like the scar tissue surrounding an old back surgery site or the degeneration within knees. This extra energy can be utilized by the body to assist in healing by attracting the regrowth of tiny blood vessels to help drain away irritating inflammatory byproducts and softening old, gristly connective tissue.

Sometimes global inflammation, such as that seen with fibromyalgia or chronic ulcerative colitis, can have dietary influences. It is now obvious how diet can negatively impact our health. Documentaries like Fast Food Nation showed that just a 30-day diet of fast food can lead to blood chemistry changes, liver enzyme abnormalities, and collapse of energy levels.

There are basically four major steps one can take to minimize the inflammatory effects of our diet.

1. Drink eight glasses of filtered water every day. Water should be carbon filtered. Make sure you change any faucet filter on your tap every 30 days or it will stop filtering. Using a reverseosmosis dispenser at your local grocery store is the cheapest route over the long term. Fivegallon jug/dispensers can be had for about $100 online or at your local hardware store, and jug refills at the grocery store are $2 for five gallons of water.

2. Avoid fast foods entirely, and

avoid processed foods due to added sugar and salt.

3. Protein should be fish and chicken and occasional red meat limited to no more than three weekly meals. Try to avoid a large meal of red meat before bedtime as that will trigger inflammation.

4. Make light exercise a daily habit. Every single day, ride a bike for 30 minutes or walk your dog around the block.

Now is the optimal window of opportunity for many cosmetic procedures

W

hen it comes to cosmetic procedures that require a certain amount of downtime for healing, timing can be a crucial consideration.

After all, who wants recovery time to cramp their summer fun or to have facial swelling or bruising just in time for that holiday family photoshoot? While different individuals may have different conceptions of the best time for them, right now is a perfect window of opportunity for many people seeking cosmetic procedures.

“A lot of minimally invasive and invasive procedures require a certain amount of downtime,” states Dr. Wade Banker of Luxe Laser Vein & Body Center. “For example, with procedures involving fat or skin removal, such as Smart Lipo or tummy tucks, even though patients typically return to normal daily activities the following day, they still need several weeks to heal fully. Fighting viruses: The Elizabeth Scott Community prepares its residents

T

he fall season is upon us, and with it comes the season of colds and flu. Residents of assisted-living and skilled-nursing facilities are particularly vulnerable to the flu based on their age, compromised immune systems, and close proximity to one another. The Elizabeth Scott Community is prepared to help protect their residents from viruses.

“Each year at this time we offer the current influenza vaccine to our residents and we educate them about the importance of proper hand washing,” says Brigitte Radabaugh, RN, Health Services Coordinator for Elizabeth Scott’s Independent Living/Assisted Living Level 1 facilities. “We also emphasize the importance of reporting any flu-like symptoms to our staff so we can isolate the affected resident, test them for the flu, and then treat them if necessary. There is usually no charge for the flu vaccine, but we advise our residents to check with their health insurance company to ensure that it is covered.”

“The flu vaccine is also voluntary for our staff members, and we encourage them to get it for their health and the protection of our residents.”

The Elizabeth Scott Community does not charge its staff for the flu vaccine. Radabaugh says typically 95% of residents and staff members agree to receive the flu vaccine. She reminds family members of residents to receive the flu vaccine as well. “It’s important that these family members are inoculated so they don’t bring the virus into our facility.”

The Elizabeth Scott Community also posts signs to warn visitors if a flu outbreak has occurred in the facility.

Visitors are then encouraged to stay away during that time to protect themselves and their families as well as their loved ones. It is also important for family members not to visit if they are sick in order to prevent the spread of the virus to residents.

ing,” says Radabaugh. “Our residents are typically over the age of 65, so we also offer them the pneumonia vaccine. But we tell our residents they need to check with their health insurance carrier to make sure the cost of the vaccine is covered. Pneumonia can be life-threatening for this demographic, so it is something that both the resident and family members should receive.”

cine is a third vaccine that residents and family members over the age of 50 should receive. “Shingles is an adult form of chicken pox, and it can be very uncomfortable. In some worst-case scenarios, people have lost sight in one of their eyes because of having shingles. Others have had long-term affects. We do not offer the shingles vaccine to our residents or staff members, but they can receive the vaccine from their physician. In the case of the pneumococcal vaccine or the shingles vaccine, residents and family members should always consult with their family physician for advice and guidance,” says Radabaugh.

E

arwax is one of those bodily substances that few of us like to discuss. However, about 12 million people a year in the United States seek medical care for impacted or excessive earwax. Earwax is only a problem if it becomes impacted in the ear canal causing pain, pressure, or decreased hearing ability. For most people, it does not have to be removed merely because it is present. In fact, earwax has a function for the ear and should be left alone unless it is occluding the ear canal or causing problems with hearing aids or ear plugs.

What is earwax?

Earwax, also known by the medical term cerumen, is a gray, orange, red, or yellowish waxy substance secreted in the ear canal of humans and other mammals. It protects the skin of the human outer ear or ear canal (the area between the outside of your ear and the eardrum), and it assists in cleaning and lubricating the ear canal. Earwax also provides protection against bacteria, fungi, insects, and water. It moisturizes the ear canal skin to help prevent itchy, dry ears. It tastes bad to insects and acts as a barrier to them going deeper in the ear canal.

The amount of earwax produced varies from person to person; however, adults generally produce significantly less earwax than children, and adult earwax tends to be darker in color and harder than that produced by children. Interestingly, there is a gene that determines whether your earwax is the "wet" or "dry" type, and the “wet” type is more prevalent. Also, you cannot change the amount of earwax produced by your ear with any type of medication or diet.

Excess earwax normally moves slowly out of the ear canal, with an extra boost from chewing and other jaw movements, carrying with it dirt, dust, and other small particles from the ear canal. Then, dried-up clumps of wax fall out of the ear opening.

People with narrow or curvy ear canals and people who wear hearing aids or ear plugs for noise reduction have more problems with earwax staying in their ears and accumulating to excessive amounts.

When earwax causes problems When this natural movement of earwax does not occur, or when individuals poke around in their ears with cottons swabs or other foreign objects such as bobby pins or matchsticks, earwax can build up and block part of the ear canal.

Older adults are more prone to earwax buildup than younger individuals. For older people, the wax gets much thicker and drier, and there is more hair in the ear canals, so the thick, dry wax can be trapped in the ear canals. Some children are prone to wax buildup because of the small size of their ear canals.

Unfortunately, we all feel the need to remove earwax manually from the ears. This can result in further impaction and other complications to the ear canal.

If you suspect that you have an earwax blockage, it is crucial that you speak with your healthcare provider.

If you have normal eardrums with no history of eardrum surgeries, holes in the eardrums, or other abnormalities, you may be advised to remove the obstruction using non-prescription wax-softening drops. The drops help loosen the earwax and allow it to move out of the ear canal slowly.

If the earwax-softening drops don't relieve your symptoms, or if you dislike drops but still have symptoms, it's time to see a doctor.

Three common techniques for earwax removal at the doctor's office work best, with no single method outshining the others. These include flushing the ear out with a water solution, manually removing the earwax using medical instruments, and sending the patient home with eardrops.

unless he or she experiences symptoms that may be associated with too much earwax, such as a plugged sensation. Cotton-tipped swabs or other objects should not be used to remove earwax. Oral jet irrigators and the alternative medicine technique called ear candling are also strongly advised against.

Ear candling involves making a hollow tube from fabric and soaking it in warm beeswax, which is allowed to cool and harden. Once cooled and hardened, the beeswax cone is stuck into the ear. The outer end of the tube is lit and allowed to burn for about 15 minutes, a process that supposedly draws the wax out of the ear. Studies have shown, however, that the drawn-out stuff is material from the candle itself. Doctors have also reported seeing patients who have burned the outer parts of their ears with this method, and it is always dangerous to hold a lit flame near the head.

Earwax, hearing aids, and hearing-protection ear plugs If you wear a hearing aid, you may have noticed ear wax on the earmold or hearing aid speaker cover.

A buildup of earwax in an earmold user can: • Damage a hearing aid • Cause a high-pitched squeal known as feedback • Block sound and reduce the volume of the aid • Cause a poor fit • Temporarily add to your hearing loss.

According to hearing aid manufacturers, 60 to 70 percent of hearing aid repairs are due to earwax buildup in the hearing aids. Wax can rest on the diaphragm of the speaker and reduce its function. Over time, the acids in the earwax can degrade the components of the hearing aid.

Hearing aid users are at especially high risk for earwax buildup. The presence of a foreign object in the ear seems to stimulate more wax production by the cerumen glands.

The ear is normally self-cleaning, but when there is an in-ear hearing aid, the earwax may not dry and slough from the ear as much as usual.

You will receive instructions on

how to clean and care for your hearing aids or earmolds to prevent wax buildup. Hearing aids need to be cleaned daily, allowed to dry overnight, and cleaned with a brush in the morning to remove wax and debris. If you don't perform this cleaning step consistently, you reintroduce wax and debris into the ear. The wax trap should be replaced as recommended by your audiologist or whenever the hearing aid isn't working.

Ear cleaning with hearing aids Hearing aid users face a dilemma when it comes to earwax. For some hearing aid users, a hearing aid or earmold prevents the ear from being able to clean itself out naturally.

People who wear ear plugs at work every day have the same problem.

However, earwax-softening drops can damage hearing aids and cause costly repairs. The hearing aid user and the audiologist must work together to find a safe way to remove earwax that will not damage the hearing aid components. No one should try to remove earwax by themselves. The best thing to do is leave the earwax alone and let the ear clean itself out naturally.

Therefore, hearing aid users need to make regular trips to a healthcare professional to have their ears checked for impacted earwax. The guidelines from the American Academy of Otolaryngology—Head and Neck Surgery Foundation recommend that people who wear hearing aids have their ear canals checked on each visit to a healthcare provider or every three or six months. Your healthcare provider can inspect your ears with an otoscope to see if you have any impaction that needs to be treated.

Do you know the dangers of distracted driving?

S afe driving is a skill that demands our complete concentration. Allowing one’s attention to drift from the road, even for a brief moment, can have tragic consequences. Unfortunately, all too many of us continue to tempt fate by driving while distracted.

What constitutes “distracted driving”? The NHTSA defines it as “any non-driving activity a person engages in that has the potential to distract him or her from the primary task of driving and increase the risk of crashing.” Essentially, doing anything that takes your eyes off the road (visual distraction), your hands off the steering wheel (manual distraction), or your mind off what’s going on around you (cognitive distraction) is considered distracted driving.

Chatting on a cell phone and texting while driving are certainly major forms of driver distraction that have justifiably garnered their share of attention lately, but they aren’t the only activities competing for our attention on the road. Other examples of common driver distractions include activities like changing the station or music selection on your car stereo, taking a bite of fast-food hamburger or a sip of coffee or soda, checking your makeup (or, worse, applying makeup) in the car mirror, reading a map, conversing with passengers, improperly secured pets moving around inside the vehicle, or, depending on the location of the viewing screen, even watching a video.

Texting while driving is particularly worrisome because it simultaneously involves visual, manual, and cognitive distraction.

Owing to their lack of experience, which can lead to poor judgment at crucial moments, the drivers most at risk of being involved in a fatal distraction-related vehicle crash are those under 20 years of age. Unfortunately, drivers in this age group are also the most likely to communicate through texting. Of course, teenage drivers aren’t the only ones who are being distracted by technology. More and

more drivers of all ages are using cell phones and other handheld devices while behind the wheel.

What about hands-free cell phones?

Aren’t they considerably safer than handheld phones? While that would seem to make sense, hands-free devices do little if anything to reduce distraction-related accidents. While they may address the issue of manual distraction, they do nothing to eliminate the cognitive distraction associated with cell phone use. In fact, based on the findings of a University of Utah study, using a cell phone while driving, whether it’s handheld or hands-free, delays a driver’s reactions as much as having a blood alcohol concentration at the legal limit of .08 percent.

Another myth is that some people deal better than others with driving distractions because they are simply better at multitasking. The truth is, good “multitaskers” have, through practice, become skilled at switching from one task to another rapidly. However, they are no better than the rest of us when it comes to performing more than one task simultaneously. Hence, whatever they might think, they are no less likely to be involved in a distraction-related motor vehicle accident.

While many states have enacted, or are soon to enact, legislation that prohibits certain driver distractions, such as talking or texting on cell phones, laws alone aren’t enough.

Distracted driving is a problem for everyone on the road, and we all have a stake in solving it. Each of us must do our part to set aside distractions when we drive. After all, no phone call, text message, song, or bite of food is so important that it’s worth putting yourself or other drivers at risk of serious injury or death. It’s especially important for parents of teenage drivers to discuss the dangers of texting, cell phone use, and other forms of distracted driving with their teens as well as to provide a positive example by avoiding distractions whenever they climb into the driver’s seat. ❦

ALS Association hosts Toledo Walk to Defeat ALS at Promenade Park October 6

T

he ALS Association Northern Ohio Chapter will host the Toledo Walk to Defeat ALS on Sunday, October 6th at Promenade Park. This is the Chapter’s 16th annual Walk in Toledo.

Registration opens at Promenade Park on Sunday, October 6 at 11:30 a.m., and the opening ceremony begins at 12:45 p.m. Participants will enjoy family activities, live music performances, and the camaraderie of those united in the fight to defeat ALS. Smash Dawgz Food Truck will be on site selling gourmet hot dogs. For more information, visit http://web.alsa. org/2019toledo. The Northern Ohio Chapter expects approximately 600 participants and hopes to raise $70,000 at the Toledo Walk to Defeat ALS. The Walk to Defeat ALS events are the Northern Ohio Chapter’s largest fundraiser, with a combined goal of $585,000 across the region. In addition to Toledo, events are held in Akron, Cleveland, and Tiffin. Proceeds from the Walk to Defeat ALS benefit the Chapter’s local care programs, including support groups, in home and phone/email support, a durable medical and communication device equipment loan closet, and financial assistance through the Chapter’s care and respite grant programs. Top-performing teams, as of this writing, include Walking Warriors, Jan’s Hope, Paul’s Crew, and Hike 4 Mike.

The 2019 Toledo Walk Planning Committee is co-chaired by Jenny Dorfmeyer and her husband Joe. Their family’s team, Charline’s Champions, is returning for their fifth year. “This is my third year on the Toledo Walk Committee,” says Mrs. Dorfmeyer. “I started walking in 2015 for my mom who passed on August 31, 2015 after being diagnosed just eight months prior. Since then, it has been my passion to raise awareness about this horrendous disease. In four years, I have raised over $70,000 for the Northern Ohio Chapter and will continue to do so tirelessly.”

There is no fee to participate in the Walk to Defeat ALS. Participants raising $75 or more will receive the official 2019 Walk to Defeat ALS t-shirt (while supplies last). The first 400 participants to arrive will receive a free Unlock ALS lanyard and Walk to Defeat ALS key. Toledo Museum of Art to feature four Great Performances in October

aculty members from the University of Toledo Department of Music will be among four Great Performances scheduled for October at the Toledo Museum of Art (TMA). The other three performers include singer Judy Dye, harpsichordist Kristian Nyquist, and pianist David Friend.

These Sunday afternoon concerts are free to the public.

• Recognize there are many kinds of thinking, such as big-picture thinking, creative thinking, reflective thinking, and strategic thinking.

• Find a place to think and set aside “think time” every day.

ow would you describe your life? Are you achieving what you desire? Do you consider yourself a success? The secret of your success is determined by your daily agenda.

If you make a few key decisions and then manage them in your daily agenda, you will succeed.

You will never change your life until you change something you do daily.

You see, success just doesn’t suddenly occur one day in someone’s life. For that matter, neither does failure. Each is a process. Every day of your life is merely preparation for the next. What you become is the result of what you do today. In other words, you are preparing for something. The way you live your life today is preparing you for your tomorrow.

You begin to build a better life by determining to make good decisions.

But that alone is not enough. You need to know what decisions to make. If you address the following 12 areas by making the right decision in each area, and then work to manage those decisions daily, you can be successful.

Attitude • Take responsibility for your attitude.

• Think, talk, act, and conduct yourself like the person you want to become.

• Eliminate these phrases: “I can’t,” “If only,” “I don’t know,” “I don’t believe.” Say these instead: “I can,” “I will,” “I know,” “I’m confident.”

Priorities • Determine and act upon important priorities daily.

• Evaluate priorities daily and plan your time carefully.

• Follow your plan.

Health • Have a purpose worth living for.

• Do work you enjoy.

• Eat right, exercise, handle stress effectively.

Family • Put your family on your calendar first.

• Create and maintain family traditions.

• Find ways to spend time together.

: Why does having hearing loss in one ear cause my child to have problems if he hears normally in the other ear?

A

: With infant hearing screenings becoming more and more common, hearing loss is being identified very early on. Out of every 1,000 births, two babies are identified with hearing loss in one ear, or unilateral hearing loss. Children with this form of hearing loss may have difficulties, and it becomes a concern for parents and teachers alike.

Hearing in noise, and understanding what is heard, becomes harder because the ambient noise masks the clarity of the speech sounds. As we all know, language development occurs early on and children will repeat what they hear whether they hear it clearly or not.

With the presence of a hearing loss, speech is not heard clearly, which makes it harder to develop clear speech. Also, auditory signals are a challenge when a speaker’s voice is soft or deep, making it a task for the child to hear and understand the speech.

Let us not forget the importance of localizing sounds. Try plugging one of your ears and closing your n

eyes. Can you identify where sound is coming from? Now imagine being a child in a typical classroom, which is usually not quiet, and being unable to hear the same in both ears. How frustrated would you be trying to identify where sound is coming from?

In this situation, it’s important to make listening easier by moving the child away from noises, positioning the normal-hearing ear toward the sound source, and minimizing the distance from the sound source.

It’s also a good recommendation to consult with an audiologist to determine the extent of hearing loss and possible treatment, which may include a hearing aid, surgical intervention, and/or aural rehabilitation.

Keep in mind, the early years are important for speech development, so it’s important to take unilateral hearing loss seriously.
