ello, “Millennials”! Are the skin changes related to stress, pollution, smoking, and prior sun damage and tanning starting to show on your facial, neck, and chest skin? a group of non-smokers. When we consider the presence of age-related hearing loss and noise exposure, the smoking could complicate the hearing issue.

Nicotine and carbon monoxide reduce oxygen levels in the inner ear ’s auditory region. This lack of oxygen results in tissue damage.

Depending on how much a person smokes (i.e. packs per week), this gradual damage can result in hearing loss even at a young age, particularly if someone begins to smoke as a teen before his or her hearing nerves are fully developed.

The studies go further to show that the nicotine from cigarettes can also cause issues with the neurotransmitters of the auditory nerve.

When these neurotransmitters are no longer functioning, the signals

between the auditory nerve and the brain are affected, which results in hearing loss.

If you smoke, please stop. The increased risk of heart disease and cancer is reason enough to quit, but smoking can also lead to hearing loss, so kick the habit today. Experience a Real Polish-American Festival

n May 18th, 19th, and 20th, A Real Polish-American Festival will be celebrating Polish-American culture with food, dancing, music, art, and education. This fantastic festival celebration is going to be held at the 20+ acre Club 16 facility located on King Road just south of Hill Avenue in Springfield Township.

Vendors from all over the region will be displaying their Polish crafts and other specialty items. Polish food vendors from as far away as Hamtramck and Chicago will be at the festival serving everyone’s favorite Polish foods, including the award-winning Ski’s Sweet Sauerkraut Balls.

A specialty treat will be found at the Beer Pavilion where hops were imported directly from Poland by Patron Saints, a local brewery, who crafted two Polish beers exclusively for this festival. Each of the beers will commemorate two of Toledo’s oldest Polish parishes—Saint Anthony and Saint Hedwig. Folks will vote on their favorite brew.

Children will be entertained by Pattrick the Magician, Rita the balloon artist, face painters, plus two acres of rides, including dodgem cars, merry-

go-rounds, slides, and more. There will even be a train giving free rides all over the festival park to both kids and adults.

Highlights of the festival include: • “Dancing with the Stars – Polka Edition” being held Friday at 7:30 p.m. This parody on the famous TV show will include local celebrities dancing the polka with a professional polka partner.

Palliative Care Program demonstrates holistic focus of The Toledo Clinic Cancer Centers

T

he multidisciplinary team of providers at The Toledo Clinic Cancer Centers (TTCCC) embraces the philosophy that effective cancer care is about much more than fighting a disease using advanced therapies and technology—it is about caring for the whole person so he or she can experience the highest possible degree of comfort and quality of life throughout the course of treatment. One of the many ways TTCCC demonstrates this commitment is by offering comprehensive palliative-care services to patients in partnership with Sincera – Supportive Care and Symptom Relief.

What exactly is palliative care, and what role does it play in cancer treatment?

According to Laurie Long, Palliative Clinical Liaison at TTCCC, “Palliative care is a specialized form of medical care for people who have a serious illness and distressing symptoms, whether caused by the disease itself or its treatment.

The focus is on managing pain and other symptoms with the goal of reducing stress and maximizing quality of life. For example, cancer patients undergoing chemotherapy or other treatments can experience a wide range of side effects such as pain, nausea, fatigue, loss of appetite, insomnia, constipation, or diarrhea.

Palliative care can help relieve these and other physical symptoms as well as address any emotional and spiritual concerns the patient may have.”

Another important aspect of the palliative caregiver’s role is to have goals-of-care conversations with patients and their families. These discussions help them understand the complexities of the illness, what changes they can expect as the disease runs its course, and how various interventions may or may not benefit the patient. These discussions also allow them to make better-informed decisions with regard to treatment.

While palliative care certainly benefits cancer patients, its use is not limited to people with a cancer diagnosis. Long points out that, “Palliative care is also beneficial for people with advanced-stage heart disease, lung disease, kidney disease, neurological disorders, or any other

serious disease—or combination of diseases—that causes distressing symptoms, whether physical or emotional.”

Long also emphasizes that palliative care is not synonymous with hospice care. “Both have a similar focus, but hospice care is for people with a life-limiting illness who are expected to live six months or fewer and are no longer pursuing curative treatment. In contrast, palliative care is for anyone with a serious illness, whether they expect to recover fully or continue to decline, and is usually done while still pursuing a cure under the care of their doctor,” she says.

One factor that really sets TTCCC’s palliative caregivers apart is the amount of time they spend with patients. Long notes that, “Appointments typically last from an hour to 1½ hours, which not only gives the caregivers ample time to discuss what patients and their loved ones should expect, but also greatly reduces the likelihood that physical, emotional, or spiritual distress will go unnoticed and unaddressed.”

Another goal of the Palliative Care Program is to prevent unnecessary trips to the doctor or emergency room. Long states, “To help achieve that, we have a team of RNs that patients can call and speak with if they have health-related concerns.

This service ensures that they’ll get the timely advice they need while hopefully limiting unnecessary doctor or ER visits.”

“Palliative care makes a marked difference in patients’ quality of life, so the sooner it is incorporated into their care plan, the better,” says Long.

“Patients with serious illnesses who receive palliative care experience significantly lower levels of pain and discomfort, less depression, better spiritual well-being, and much greater satisfaction with their overall quality of care.” ❦ The Toledo Clinic Cancer Centers, located at 4126 N. n light of the horrific happenings over the past few weeks, I really didn’t know where to begin to write this article until I remembered the card one of my nieces sent me during my recuperation with the quote from Christopher Robin of Winnie the Pooh fame. I found these words of wisdom very consoling when I first read them, and I turn to them now to help me deal with the attacks in Parkland, Texas, the on-going wars in the Middle East and Europe, the world-wide plight of refugees and immigrants, and the tragedy in Saskatchewan, Canada. Where do we as human beings begin to comprehend all this sad news? Where do the families and friends of the victims go to begin to deal with all this grief and sorrow and loss? Where do I as a Catholic, Franciscan Sister turn to deal with all this sorrow?

Because of my age and stage in life, I certainly have experienced my own share of grief and sorrow and loss. Because of my background and training I find solace in prayer, the Scriptures, sharing with friends, the beauty of a sunrise and sunset, and seeing nature come to life again each spring. My religious belief in the death and resurrection of Jesus the Christ certainly consoles me as I try to comprehend so many deaths, so much suffering, and so much fear.

When I see so many young people working to really change the world, to lessen the violence done by guns in their schools, to make the world safer not just for themselves but for all of us, I am consoled and assured that the world will be better if they

succeed now and continue well into the future.

Remembering and reflecting on the lives of the real heroes and heroines of our time—people like Pope Francis, Martin Luther King, Jr., Rev. Billy Graham, St. (Mother) Teresa of Calcutta, and the Dali Lama—I am consoled that great people rise up among us and help save us from ourselves and others and give us much to say and do to make this world a better place for all people.

The so-called “ordinary people” who come to our rescue when we are in need during these terrible and tragic times help us begin to deal with it all. The pastor who preaches words of compassion at the funeral of a young addict who committed suicide, the policewoman who helps find a safe place for a battered mother and her children, the teacher who spends extra time and money in order to meet some special needs of her students—I could go on and on.

It seems to me that the place to begin to deal with all that happens in our world is with myself, with yourself, using the gifts and talents we have been given to share. We may need to remind ourselves from time to time, especially when things seem to be overwhelming, that we’re braver than we believe, stronger than we seem, and smarter than we think—and of course, pray! ❦ Sister Mary Thill is a Sylvania Franciscan Sister. A

ddiction destroys families; it destroys lives, generation after generation. Talbot Clinical Services of Ohio, LLC and Talbot Services DIP offer people struggling with alcoholism and drug addiction the opportunity to reclaim their life and their family.

For over 12 years, Talbot Services DIP has been providing alcohol and drug prevention, education, and driver intervention services for Northwest Ohio. With the 2015 opening of Talbot Clinical Services, the organization took the next step in their life-saving mission by providing outpatient chemical dependency treatment with a special emphasis on opiate and heroin addiction.

Matthew Talbot, the organization’s namesake, was born in Dublin, Ireland, in1856. He died in 1925. By the age of 13, he was, by his own report, an active alcoholic and addict. In his early 30s, he had a spiritual experience that changed his life. He embraced recovery and spent the rest of his days working the docks of Dublin helping others find and maintain an alcohol- and drug-free life.

Following the example of Matt Talbot, and sponsored by a local hospital, the Talbot Center opened its doors in 1986 as an alcohol and other drug treatment center. For 20 years, the Center provided care for those afflicted by the brain disease of addiction. The hospital ceased providing those services in 2006.

Fortunately the vital education and intervention services continued uninterrupted with the incorporation of Talbot Services DIP. This independent effort is committed to continuing Matt Talbot’s example of caring for addicts and alcoholics—saving lives.

Education and intervention provided by Talbot Services DIP include: The 72-hour Driver Intervention Program designed for individuals who have been convicted of a first time charge of OVI, or operating a vehicle impaired. Clients are usually required by a municipal court to attend. These services are held over a three-day weekend in either the Rossford or Holland Court Yard Marriott Hotel.

The 48-hour Non-Residential Educational Intervention Program was

developed to meet the needs of those clients who have alcohol or drug-related charges.

The 8-hour Remedial Defensive Driving Program is a one-day course open to anyone who may have experienced a 12-point driver’s license suspension through the Bureau of Motor Vehicles or desires to earn a two-point credit to be applied to their driver’s license.

Six (6) Day Education/Plus Program is recommended for those required by the Court to complete an extended Driver Intervention Program for possibly having multiple alcohol- or drug-related offenses or a Breath Alcohol Concentration (BAC) of .17 or higher.

Our mission of saving lives continues via the services provided

by Talbot Clinical Services of Ohio, including: • Comprehensive diagnostic assessments • Medically assisted recovery • Outpatient treatment • Intensive outpatient treatment • Group counseling • Individual counseling • Case management • On-site urinalysis • Relapse prevention and alcohol/drug education • Use of suboxone and other medications.

Talbot Clinical Services does require regular attendance and participation in group, family, and individual therapy sessions as well as attendance at 12-step recovery meetings. hen someone is diagnosed with a serious medical ailment, such as heart disease, diabetes, or cancer, the typical reaction from family, friends, and acquaintances is sympathy and support. However, a diagnosis of mental illness oftentimes elicits a very different type of response, ranging from wariness and suspicion to outright fear and even discrimination. The perception seems to be that mental illness is a flaw rooted in an individual’s character or personality rather than a neurobiological disease. As a result of this stigma, people with mental illness too often feel ashamed, suffer in silence, and avoid seeking vital treatment.

Mental illness is no small challenge in our nation. According to the National Alliance on Mental Illness (NAMI), in a given year in the US, approximately one in five adults experiences mental illness and one

in every 25 adults experiences a serious mental illness that substantially interferes with or limits one or more major life activities. With so many lives touched, why does our society still tend to stigmatize those who have a mental illness? The answer may lie in several persistent myths.

Taken as a group, people with mental illness are no more violent than people without mental illness. In fact, according to NAMI, people with mental illness are much more likely to be victims of violence than perpetrators of it—a rate four times that of the general public. This misconception is likely rooted less in objective reality than in unflattering or outrageous media portrayals of people with mental illness.

People with mental illness are weak and can’t handle the rigors of work or school Mental illness stems from a variety of biological and environmental factors, not individual weakness. On the contrary, many people with mental illness are quite capable of holding up under the pressures of work, school, and day-to-day life because they’ve learned certain skills and techniques that help them cope with and adapt to stress before it reaches an unmanageable level.

People with mental illness are strange and unpredictable Some forms of mental illness can manifest through odd or unpredictable behaviors, but many people with

mental illness lead active, productive lives without anyone around them knowing what they’re going through.

Also, those individuals who do exhibit unusual behaviors as a result of their illness can often learn to recognize and manage them effectively through proper treatment.

Mental illness will never affect me Nearly 44 million Americans experience mental illness each year, so it’s the rare individual whose life will never be touched by some form of it. Those who think no one close to them has mental illness might be surprise to learn that a friend, family member, coworker, or acquaintance lives and functions with mental illness every day. Whether it is experienced firsthand or not, mental illness is an issue that affects everyone.

ouble ARC, a local non-profit organization that has been serving children with Fetal Alcohol Spectrum Disorders (FASD) for 25 years, recently merged with A Renewed Mind.

The professionals at Double ARC meet with the parents or guardian to understand specific issues with the child. Then a comprehensive medical summary report is provided to the parents along with recommendations for additional help and resources. A diagnosis is important to qualify for State-funded services and to fully understand the cognitive strengths and deficits of their child.

A Renewed Mind, which specializes in mental-health treatment, will be involved in providing counseling to parents and the child. Most children with FASD have other disorders such as ADHD and will benefit from the counselors at A Renewed Mind. Sr. Suzette Fisher, co-founder of Double ARC and program director of the Double ARC Center for FASD, will also work with the child’s teachers and school on Individualized Education Plans so the child is successful in school.

Double ARC also is introducing a new video series called Triumph Today for parents. There are 16 separate topics that parents need to understand in parenting a child with FASD. The video series was made possible by a generous grant from ProMedica along with the Maumee Rotary, the Perrysburg Rotary, the Walter E. Terhune Fund through Key Bank, and the Anderson Foundation.

So we addicted a lot of people and then cut off their supply, but they still have a legitimate pain-management issue. As a result, a lot of these patients have turned to heroin, and many are now turning to fentanyl because heroin doesn’t work as well as oxycodone,” he says.

Recognizing the severity of the opioid crisis in our community—and the fact that substance abuse is a generational disease that puts children of abusers at greater risk—Harbor is taking a multipronged approach to addressing this problem and increasing access to care. Sheehan explains, “First, we need to make sure we target this next generation of kids who are at risk so they don’t follow the same path their parents took. Second, we need to provide treatment with the understanding that we can get people clean, but we won’t be able to keep them clean unless we address the underlying pain issue. Third, we need to ensure we provide a suffi-

cient period of long-term residential care to get them off drugs and keep them off with pain management.

People who become addicted at age 16 and get clean at age 23 are, for all intents and purposes, still 16 years old. They can’t function as adults.

It takes a considerable amount of time to get them to the point where they’re self-sufficient. If insurance only covers 20 or 30 days of residential care, that’s just not going to do it.”

Sheehan also emphasizes that the answer to the opioid crisis won’t be found in handing out Narcan (a drug that blocks the effects of opioids and can reverse an overdose) to police officers and other first responders.

“Our focus should be on intervening long before that point and, again, improving access to the quality care and services that are already available in our community. et’s take a quick ride down memory lane. Can you recall the first time you learned to ride a bike? Most of us have an interesting story to share.

Maybe you were the one percent that just hopped on and took off, or maybe you were the other 99 percent that found this newly introduced skill a bit more difficult to grasp.

Many of us can vividly recall falling over and over again, crashing into bushes and curbs, and remember every single battle wound endured.

However you learned, when you finally, miraculously, acquired the skills of balance, stopping, dismounting, and “look mom no hands,” you earned a bundle of happiness with a great sense of accomplishment, independence, and confidence. You were suddenly free to navigate and discover your neighborhood and explore places that weren’t part of

your universe before you learned to ride a bike.

Bicycling to school is more than just fun for kids! Like walking to school, cycling is a form of active transportation and is good for overall health and well-being. The Centers for Disease Control and Prevention (CDC) define active transportation as any self-propelled, human-powered mode of transportation. Like any kind of exercise, active transportation can aid in the prevention and management of disease; increase cardiovascular health, muscle strength, and flexibility; and strengthen bones.

Bicycling and walking to school isn’t just good for physical health, but it can also lead to improved academic performance. According to SRTS, when children are physically active before school, they arrive at school more alert, focused, and ready to learn.

Their ride also fosters independence and increases self-confidence—all of which can impact school achievement. Finally, bicycling to school can improve neighborhood connections and create a sense of community as young people build relationships and establish healthy habits that can last a lifetime!

May is National Bike Month, and many communities across the country are gearing up to celebrate by highlighting the many benefits of bicycling. Check www.tmacog.org/ bikemonth for lots of tips and activities coordinated by Toledo Metropolitan Area Council of Governments (TMACOG).

On May 9, schools will be celebrating National Bike to School Day. This one-day event builds momentum in schools, families, and communities to make a difference. The ride communicates to local leaders the public’s need for safe routes and the importance of investing in infrastructure projects like safer crossings and paths. Below are some tips and resources to use in preparing students for the big event.

Equipment • Give your child’s bike the ABC Safety Check (Air, Brakes, Chain).

• Make sure they are wearing a properly fitted helmet. It should be snug, not moving side to side or front to back, but not uncomfortable. Side straps should fit like the letter “V” below the ear, and you should

be able to slip two fingers between their chin and the chin strap.

• Make sure the bike is the right size. When sitting on the bike, your child should be able to comfortably reach the handle bars, place the balls of both feet firmly on the ground, and pedal clearing the handle bars.

• Wear bright-colored clothing and have bike reflectors and/or a bike light.

Safe riding tips • Identify a safe route together.

This may not be the shortest route. When possible, choose a route with a sidewalk, signaled crossings, slower traffic, and fewer cars.

• Practice riding the route with your child several times to and

from school, and make sure they know the traffic laws to be obeyed on their route.

• Whenever possible, your child should ride with an adult or a friend(s).

• They should not ride with a heavy backpack that could throw off their balance.

• If your child has a cell phone, it should be kept in their backpack and used only in case of an emergency. Don’t text and ride!

Every person has value and should receive the supports necessary to create opportunities for them to choose where they wish to live, to be gainfully employed, and to choose their relationships, hobbies and beliefs. That was the underlying theme for the Lucas County Board of Developmental Disabilities in 2017, even as we continued the transfer of direct services to providers in the private sector as mandated by federal rules. This change is requiring realignment of our staffing with the emphasis now on service coordination, ensuring a safe environment, and supporting the more than 400 certified Medicaid providers in Lucas County.

It is of primary importance to the Board of DD to help these providers maintain a high level of quality service that allows individuals being served the opportunity to live the life they choose. That said, we recognize the providers have a significant need for additional staff and have launched efforts, such as the Success Coach program and job fairs, to help with recruitment.

Supports provided to individuals with developmental disabilities by the Board of DD span a lifetime. More than 550 children were in Early Intervention. In the 3-21 age group, the Board will be supporting approximately 750 children and their families.

When children reach their mid-teens and get close to graduation our Employment Navigators help families prepare for their child’s adult life and the world of work.

While providing the help for the individuals and their families, the Board of DD continues its efforts to stretch your local tax dollars. Currently the vast majority of individuals served in the DD community has 60 percent of their services paid by Medicaid. The remaining 40 percent comes from local funds administered by the Board of DD. So for every local dollar the Board spends on serving others in our community it is able to secure $1.50 from Medicaid.

Among our goals for 2018: • The Board will work to expand the number of providers in the areas for people with autism, behavior concerns, dual diagnosis, and for those who are medically fragile.

• Further development of the FANS (Friends, Allies, and Neighbors) Network will address the goal of greater inclusion for people in community living.

• Public messaging through television, news releases and social media will be aimed at breaking down the barriers that stand in the way of employment, housing and social activities. The Board of DD will continue to support eligible individuals and their families in developing a vision for the future based on an individual’s strengths, interests, and choices.

Acupuncture for pain relief, smoking, and emotional well-being

A

cupuncture, which is a form of Alternative Medicine, is over three thousand years old, and its use predates recorded history. It had its original roots in China, but from there it spread across the Orient, so it is sometimes known as a form of “Oriental Medicine.”

The basic idea behind acupuncture is that the body is crisscrossed by a number of meridians that act like channels of energy circulating around the body—energy which keeps the body healthy.

This energy is called “qi” (pronounced “chi”) energy.

Physicians practicing acupuncture believe that along the meridians you can have blockages of the proper flow of qi, too much qi, or even too little. Changes in the qi flow can be made by stimulating certain points along the meridians.

Some points increase the flow of qi, other points balance the qi energy, while still more points may sedate the flow of qi. Treatments combine to restore “balance” to the flow of energy around the body.

Indeed, modern techniques now allow us to add energy with the needles to “cold” areas of the body—areas such as sites of non-union fractures or spinal arthritis or even to areas of failed spinal surgery. In fact, the stimulation for healing (which involves the regrowth of new blood vessels) has even been utilized in facial rejuvenation treatments such as acupuncture facelifts!

Today acupuncture is gaining wide acceptance in the West. It is now covered by Veterans Benefits, Ohio Medicaid, Workers Compensation, and some private insurance companies. With this kind of growth, soon acupuncture will be a standard covered expense in most policies.

What happens during an acupuncture treatment?

On your first visit, your acupuncturist will have you fill out a standard health questionnaire. Then you will have a consultation where you can

talk about your health concerns.

After that, your acupuncturist will perform an examination that may take several forms.

He or she may examine your tongue’s shape, color, and coating; feel your pulse; and possibly perform some additional physical examinations depending on your individual health needs. What he is doing is evaluating the quality of the qi energy flow around your body and how it is impacting various systems. From this examination your acupuncturist builds up a protocol of treatment points that is designed to bring the flow of qi energy back into proper balance. As an alternative, modern acupuncturists may perform an Electro Meridian Imaging (EMI) study. This is simply a diagnostic machine that records various qi levels around the body and helps the acupuncturist form a picture of the qi energy balance within the body.

Once the proper treatment protocol has been established for your body, the acupuncturist will lay you down on a comfortable table while precise acupuncture points are stimulated.

Most people feel no, or minimal, sensations as tiny needles are gently placed. The needles are very fine and push aside skin cells as they are emplaced instead of puncturing the skin as larger, painful hypodermic needles usually do. There is no bleeding and no need for band aids after treatment. The needles are left in place for 20 minutes.

During the treatment, one usually feels a sense of tranquility, as the treatment induces a relaxed state.

What conditions can acupuncture treat?

Since its introduction in the West, thousands of clinical studies have been performed detailing the benefits of acupuncture. Acupuncture is a “holistic” treatment and can be employed in the management of almost any health condition. Some

ost Toledo-area residents welcome the arrival of spring after our typically long and cold local winters. But for the orthopaedic physicians in our area, spring’s return signifies more than a pleasant change in weather conditions—it also means their practices will soon be inundated with patients requiring treatment for musculoskeletal injuries.

“In wintertime, when we tend to be less active, we see the occasional injury from wrestling or basketball, but as soon as the weather gets warm, the floodgates open, with June to August being our busiest months

Also, keep in mind that swimming pools and other shallow bodies of water can pose risks beyond that of accidental drowning. “Diving into shallow water headfirst and hitting the bottom or a hard object can cause

Keep an eye on the pollen count Pollen is a very fine yellow dust produced by different plants. You can see it occasionally as it is blown from plant to plant. Sometimes you’ll see it on your car, usually in the morning or afternoon.

Even if you can’t see pollen, it

Pollen count refers to the number of pollen grains landing on a given area during a specified time. Pollen counts of 50 or less are generally considered low, and very few people will be affected that day. Pollen counts of 1,000 or more are considered very high and would affect most spring seasonal allergy sufferers.

Limit your exposure to pollen Knowing the pollen count will help you take steps to limit your exposure.

Staying indoors is a good idea, especially between 5:00 a.m. and 10:00 a.m. when pollen counts are highest, and on warm and dry days—which can also increase the pollen count. You should also be aware that there are many ways pollen can find its way into your home. You can track pollen inside on your shoes, so take them off before coming in the house. It’s a good idea to change your clothes after you come inside as well. You may even want to shower after being outside to wash the pollen off your skin and hair.

Don’t forget about your pets.

If your dog or cat runs outside, it will likely get pollen on its fur, so wipe your pet off before letting it back inside. It’s also smart to use a HEPA filter on your furnace or air conditioner. This type of filter will help trap pollen, limiting how much gets into your home.

Talk to your doctor There are many types of over-thecounter medications you can try to help alleviate your symptoms. Some of these, however, can be habit forming, and long-term use may elevate your blood pressure. Your doctor can help determine which over-the-counter medications will work best for you.

If over-the-counter medications don’t provide enough relief or cause any side effects such as drowsiness or dizziness, your doctor can prescribe prescription drugs that don’t have these effects. You should also see your doctor if you think pollen is causing asthma attacks. Symptoms can include chest pain or tightness, wheezing, or coughing that wakes you from your sleep.

Because there are different pollens at different times during the season, you may find that your allergy symptoms subside earlier or later in the spring. But if you have a history of uncontrollable or severe symptoms or symptoms lasting all season long, your doctor may talk to you about allergy shots.

If patients develop skin cancer, Dr. Kouba who is fellowship trained in Mohs surgery and cosmetic surgery, is well poised to treat it using the most advanced techniques available.

The treatment of choice will depend on several factors such as the type, stage, and location of the cancer.

For basal cell and squamous cell carcinomas occurring in cosmetically sensitive areas such as the head, neck, hands, feet, or genitals, the ideal treatment is Mohs micrographic

surgery. With this technique, the surgeon removes one thin layer of tissue at a time and examines it under a microscope for the presence of cancer cells. When a layer is found to have no additional cancer cells, the surgery is complete. “The advantage of Mohs surgery is that it gives patients the greatest cure rate by leaving the surrounding healthy tissue unharmed while creating the smallest possible defect to reconstruct.

Since the surgery is completed all in one visit and the tissue layers are examined on site, patients don’t have to worry about being called back multiple times to have more tissue removed,” Dr. Kouba says.

That includes wearing a wide-brimmed hat and other sun-protective clothing as well as applying sunscreen to any exposed skin before spending time outdoors. “Also, be aware that sunscreen loses its effectiveness over time, so it’s important to reapply every two hours. If you apply sunscreen and then go out boating for eight hours without reapplying, you’re actually subjecting your skin to six hours of unprotected sun exposure,” he cautions.

As far as tanning is concerned, the best advice Dr. Kouba can give is simply don’t do it. A tan is the body’s response to DNA damage done by the sun’s harmful UV rays.

The tools for life: What sports can offer

T

his month, I am starting a series of articles that share how sports can provide your children tools for life, not just trophies and championships.

Now, winning games and championships is amazing; however, the lessons that one learns from both the good times and difficult times during those periods are what stick with a person and impact his or her overall growth and development. Many times this is overlooked.

Back in 2010 and 2011, the NCAA ran a series of ads where the main focus was that many college athletes will be going pro in something other than sports after college. The ads talked about the academic levels of athletes and how that knowledge was an asset to companies. In 2012, Enterprise Rent-A-Car ran an ad about how many of the people they hire are former athletes. Why is this? Well, let me start the series by sharing a few items learned and cultivated in sports that transcend into the working world:

bod It’s time for your spring body cleanup

pring is here, and all of us are getting ready for summer. We have started cleaning up outside as well as inside our homes. Have we started doing the same with our bodies? We all want to look and feel better as we approach summer. Unlike the spring cleanup of our homes, where we wait till the weather breaks and the snow is gone, we could have started our body cleanup sooner, especially if we want to be ready for swimsuit and shorts season. But no worries, we can start now and still be ready for the summer.

I have been writing about health and wellness for a number of years and continue to stay on top of the trends in the industry. This month I want to continue to share some helpful tips that I have written about over the years that do make a difference.

exercise to burn off this…”? Truth is, you can’t out-exercise a bad diet. The first step to any fat-loss plan has to be with improving your diet. Processed foods (many of those having a long shelf life), fake sugars, and refined carbs need to be drastically cut in our diets. The foods that the marketing gurus have done so well getting us to crave—cakes, deep-fried foods, candy—need to be reduced in our diets. Yes it may require reading ingredients on labels to better understand what has been added and what needs to be avoided. We need to add more foods that are low-energy-density—vegetables, whole grains, lean meat, fish, and fruit. This is not new and has been reinforced over and over, yet we still gravitate to the processed, sugar-filled foods.

Drink more water. It really is time to stop drinking the high-calorie beverages with little to no nutritional value. We may like the flavored lattes,

Meal pacing We have all heard many different ideas on how many meals to eat and how often. Some studies suggest grazing or small meals every three hours. Others talk about intermittent fasting or eating your daily meals in a shorter time span. Still others emphasize three meals around specific time periods and two small snacks.

I want to say that losing weight is not “one approach fits all.” You need

to find out what works for you and is best for your body. The key is to make sure the calories you eat don’t exceed the calories you burn each day. We need to understand this: One pound of fat is roughly 3,500 calories, and to lose weight, we need to have a calorie deficit.

Smarter indulging We all like to indulge and have those special foods. That’s not a problem if we are smart about it. Indulgences like alcohol, chocolate, or a special dessert can add calories quickly. If you indulge, don’t do it daily and go small on the portion size. As you may know, red wine and dark chocolate provide antioxidants, which are good for our overall health. Antioxidants help fight inflammation, but a certain kind of antioxidant, catechins, also helps burn fat. Find foods that have this antioxidant and be smarter when incorporating them into your meal plan.

Exercise What is a weight-loss program without moving? Getting up and moving (walking) and exercising (using weights to add and or tone muscle) on a regular basis will be a big benefit to your lifetime of health and wellness.

There are numerous benefits to cardio- and strength-training activities when done three to five times a week.

The best thing is you don’t have to perform at the level of a marathon runner or bodybuilder. Start slow if you haven’t done anything and gradually increase your activity. Remember, doing something is much better than doing nothing.

Again, for those who have done very little exercise in the past, always check with your primary care provider first. If you aren’t sure you can do an exercise program because you have some shoulder, back, hip, knee, or ankle/foot issues, see a physical therapist first. A physical therapist

Disability in today’s world “Disability should be as commonplace as a person who wears glasses.”

I

love that quote. I do not remember where I read it, but it represents that none of us is physically perfect.

As baby boomers age, they often become less agile, with a knee that might need replacement, or a hip that was injured in an accident. Their mobility, vision, hearing, or any other thing might weaken in their body.

Losing physical strength is just part of life, part of the aging process, and that is OK! Our bodies really seem to wear out after years of use.

I was intrigued to find that disability in today’s world was quite far reaching. Just recently, the National Organization on Disability (NOD) gathered some realistic statistics.

The United States Census Bureau says that approximately 57 million Americans have a disability. There are more people with disabilities living in America than the entire population of Canada or the Caribbean.

But what is considered a disability? How do you know if you are disabled? According to the Americans with Disabilities Act, an individual with a disability is a person who has a physical or mental impairment that substantially limits one or more major life activities, has a record of such an impairment, or is regarded as having such an impairment.

Some examples of physical or cognitive impairments include (but aren’t limited to) orthopedic, visual, speech, and hearing impairments; epilepsy; emotional illness; specific learning disabilities; and addictions (individuals who currently engage in the illegal use of drugs are not protected by the ADA).

Major life activities include functions such as performing manual tasks, walking, seeing, hearing, speaking, breathing, learning, and working.

It seems like the most obvious types of disability are the ones we can see: persons who have trouble walking, those with speech or hearing difficul-

ties, those with cognitive problems.

There are so many hidden disabilities that are not that obvious. Consider those who live with emotional illness, addictions, or visual and breathing challenges. Many of these are not immediately recognizable to others.

Students in the classroom deal with multiple disabilities. There are many disorders that present themselves as difficulties in processing information: in reading, writing, listening, speaking, organization, math calculation, problem solving, time management, or social skills.

Often these deficits are lumped into the category of learning disabilities.

Teachers and parents often deal with learning challenges such as visual, auditory, or spatial perception issues; memory difficulty; dyslexia; attention deficit disorder (ADD); and poor motor coordination.

To list disabilities can give us an idea of how complex the human anatomy is. The reality is, that we all have some type of physical or emotional challenge and we learn to live with it and compensate in some manner.

So, next time I see someone wearing reading glasses, I will remember that each of us belongs to the human race, and are going to face the human condition of our bodies with some physical impairment right now or a part wearing out a bit sometime in the future. That’s OK, though.

Laurels treats diverse array of rehab clients with help of AlterG

n the hands of an innovative rehabilitation team, the cutting-edge AlterG® Anti-Gravity Treadmill® can be put to use in treating clients with a surprisingly diverse range of diagnoses and functional limitations on both an inpatient and outpatient basis. And that combination of inventiveness and advanced technology is precisely what rehab guests encounter at The Laurels of Toledo—the only facility in our community to offer the AlterG as a therapeutic modality.

Developed by NASA engineers while studying the biomechanics of exercise in space, the AlterG uses differential air pressure technology to “unweight” patients as they walk, stand, or perform various exercises on the treadmill under the close supervision of a therapist. The unit can unweight patients as much as 80% while allowing normal gait mechanics.

To use the AlterG, patients put on special pants that zip into an inflatable chamber on the treadmill. The therapist then calibrates the machine and increases the air pressure in the chamber to the desired level. Video

Cage describes her workouts in the AlterG as “like floating in space” and comments, “I’m considered a fall risk, but I don’t worry about falling in the AlterG because I’m secured in the unit. Plus I don’t get tired when I’m using it, and the image on the video screen is large enough that I can see what my feet are doing.”

Inpatient Laurels client Eirgie Hughes would also seem to be an unlikely candidate for the AlterG. In both renal failure and heart failure and undergoing dialysis three times per week, Hughes’ activity tolerance is very low. Using a walker, she can travel only about 100 feet before getting exhausted. But in the AlterG, she is able to not only walk with a normalized gait, but she can also let go and swing her arms normally.

actually walked approximately a half mile. “The machine really builds your confidence by taking your weight off. I love it and would tell anyone to try it,” she says.

The rehab team at The Laurels has also found some very creative ways to work on Hughes’ dynamic balance using the AlterG. “For example, with Eirgie zipped into the machine backwards and the treadmill turned off, we’ve challenged her balance by having her perform exercises while standing on a special foam pad that simulates different surfaces, as well as activities that require her to extend her arms outside her base of support,” says Leidel.

Like Cage and Hughes, Kimberly Pressnall has a condition that wouldn’t normally seem conducive to therapy in the AlterG. Currently in outpatient rehab at The Laurels for back pain related to rheumatoid arthritis, Pressnall praises the AlterG

for helping build her muscle mass while improving her balance and control. “I just walked for 25 minutes, which is something I wasn’t able to do without the machine. I no longer use a wheelchair or walker, and I’m able to walk short distances without a cane. I’m even able to walk up steps with control,” she says.

Of course, Pressnall doesn’t give all the credit for her rehab gains to the AlterG—she also recognizes that the rehab staff at The Laurels played a huge part. “Their skills are excellent,” she notes. “They work with you one on one, and the routine they put together for you is very effective.

hen an injury causes excessive blood loss, the victim can easily succumb while awaiting emergency medical care. His or her life may depend on prompt intervention from someone already on the scene. However, all too often, bystanders in this scenario lack the knowledge necessary to intervene and stop the bleeding. In an effort to prepare area residents to take effective action and save lives in bleeding emergencies, Mercy Health is offering a free hands-on training program called Stop the Bleed.

According to Mercy Health Trauma Education Coordinator and trauma nurse Nicole Knepper, uncontrolled bleeding is the number-one cause of preventable death from trauma, so Stop the Bleed was developed to teach everyday people how to stop

bleeding in the crucial moments before emergency help arrives. “Much like the campaigns to teach people how to perform CPR or the Heimlich maneuver, which have saved many lives, the focus of Stop the Bleed is what the immediate response should be—how to recognize life-threatening bleeding and how to stop it,” she explains.

Stop the Bleed was motivated by the 2012 Sandy Hook tragedy, when the trauma surgeon attending the victims observed that more could have survived their injuries if bystanders had been able to stop their bleeding at the scene. Of course, the active-shooter scenarios that continue to make headlines are only one way severe-bleeding emergencies can arise. People should also be prepared to deal with them in the wake of injury accidents on roadways, in the workplace, around the home, or in any other environment.

Stop the Bleed participants can expect to learn the acronym “ABC,” which stands for the following: A—Stay safe and Alert 911 B—Find the Bleeding C—Compress

They’ll also be taught how to recognize whether bleeding is life-threatening, what to do if the injury is to an arm or leg versus the neck or chest, how to apply pressure to a wound, how to apply a tourniquet, how to pack a wound, what to do if they have a trauma kit, and what to do if they don’t. They’ll even learn the various ways a shirt can be used to

save a life, including applying pressure to a wound, packing a wound, or tearing up the shirt to create a tourniquet. “Last week, we taught the class to a group of teachers at Central Catholic High School, and one of them commented afterward that she hopes she never has to use the knowledge but feels much more confident having gone through the

program,” Knepper says.

Collaborative families tend to discuss issues, generally have good listening skills, and deal directly with unpleasant things in an open way. They consider strong feelings normal and allow their expression.

Knox notes that families in this category tend to have the healthiest outcomes, the greatest marital and family satisfaction, and kids with better interpersonal functioning.

On the other hand, the aggressive approach to conflict resolution tends to yield very poor outcomes. Families in this category argue by attacking (either verbally or physically), do not allow the safe expression of feelings,

and usually use coercive tactics to get what they want. As a result, people get hurt and the stress level is high.

Children in these families also tend to experience chronic stress, which can actually impair their brain development.

Of course, spouses or partners sometimes bring different conflict-resolution styles to the same family, which can create tension. For example, Knox often treats behavioral problems in kids that arise because one parent is very permissive and the other very strict, leading to disagreements and frustration for both. “I usually tell these parents that they need to ‘take a page from each other’s book’ because the healthiest parenting style is authoritative, which sets firm limits for behavior but is also nurturing and focused on children’s emotions and experiences,” she says.

Knox also reassures parents who are struggling with children’s behavioral problems that all families disagree and that raising kids can be particularly challenging at certain times in their development, including the period from infancy to toddler-

hood and then again in adolescence.

She elaborates further, “There’s no question teenagers can push parents’ buttons, but it’s important to keep in mind that it’s normal and necessary at this age for kids to separate psychologically from their parents and start to develop their own identity. Sometimes in the process they speak and behave in ways that their parents do not understand or agree with. But if you use authoritative parenting, and remain consistent and nurturing, with patience, you’re going to get through it.”

To help families develop a healthier pattern of conflict resolution, Knox encourages them to practice “fair fighting,” which acknowledges that disagreements are going happen but can be constructive if handled properly. The basic principles of fair fighting include: • No insulting or degrading language.

• No “stonewalling” (refusing to speak to the other person).

• Be aware of your anger level and discuss issues only when you are calm.

• Take turns talking. This can involve the use of a timer so each person has a chance to speak uninterrupted for a predetermined period of time.

• Use “I” language, which takes responsibility for one’s feelings, rather than “you” language, which can sound accusatory.

• Recognize that kids are not always developmentally ready to understand or follow these principles.

“On a bigger scale, if parents model this peaceful and respectful behavior over and over again, they will raise kids who are non-aggressive and able to develop healthy relationships as adults,” says Knox.

or people with healthy lungs, it can be difficult to conceptualize the experience of living with a pulmonary disease that literally takes one’s breath away.

To help convey just how alarming breathing problems can be, Lori King, RCP, RT, Director of Respiratory Services for The Manor at Perrysburg, suggests trying to breathe in and out through a thin straw or coffee stirring stick for several seconds. She notes that once people perform this simple experiment, feel the panic of being unable to inhale and exhale freely, and then begin breathing normally again, their sense of relief is tremendous.

Now, thanks to the state-of-the-art, multidisciplinary pulmonary rehab program offered at The Manor at Perrysburg, this same sense of relief—and a greatly improved quality of life—is available to people whose breathing is compromised by conditions such as chronic obstructive pulmonary disease (COPD), congestive heart

failure (CHF), emphysema, or asthma.

“We can treat any type of pulmonary disease, even sleep apnea, and our approach is highly individualized, not just because every patient is unique, but also because the appropriate care is different depending on the disease,” King states.

Former and current smokers are among those encouraged to take advantage of this program. As King points out, lung damage occurs the very first time someone smokes. Of course, that damage is compounded significantly over years or decades of smoking, which, according to King, is why lung-transplant candidates are required to go through pulmonary rehab both before and after surgery.

Patients in the pulmonary rehab program receive one-on-one treatment in 60-minute sessions, five days a week. In addition to performing exercises that help keep the airways open and strengthen inspiratory and

expiratory function, patients can learn specialized techniques to help them breathe better despite their disease.

For example, patients with diseases such as multiple sclerosis (MS) or amyotrophic lateral sclerosis (ALS) can learn to build strength in their diaphragm by using a diaphragmatic breathing technique instead of breathing through their nose. Patients with COPD, who commonly experience CO2 retention, can be taught certain breathing techniques that help keep CO2 at a safe level in their body.

But the program goes far beyond breathing exercises. “In addition, we look at all the patient’s respiratory medications to make sure they’re appropriate and being used properly so they don’t cause other problems.

For instance, patients who use a bronchodilator too frequently can develop a false dry cough. Also, because people with pulmonary disorders tend to retain water very easily, which can impair breathing if it causes the lungs to fill up, our dietary team works closely with patients to ensure they’re eating the right foods, limiting sodium, and avoiding drinking too much water,” says King.

Relaxation and meditation techniques are taught in the program, as well. These techniques are especially important for patients with obstructive pulmonary disease, meaning they can get air in but can’t get it out.

Directed by pulmonologist Dr. James Tita, the pulmonary rehab team consists of a respiratory therapist; physical, occupational, and speech therapists; nurses; social services; and dietary services—all of whom are focused on getting the patient back to the highest possible level of pulmonary function. “This multidisciplinary team working together produces phenomenal outcomes for patients,” King remarks. “However, she continues, “we also work closely with each patient’s doctor to keep him or her apprised of the patient’s condition and progress as well as to ensure any recommendations the doctor may have are incorporated into the treatment program.”

Effective treatment of any chronic breathing disorder requires a lifelong commitment on the patient’s part, so when patients complete rehab, they’re given a variety of educational materials so they can continue doing

everything they’ve learned at home.

“When patients leave here, we want them to have all the information they need to understand their disease, increase or maintain their stamina and lung volume, as well as reduce the frequency of their hospital and doctor visits. Furthermore, our philosophy is ‘Once a patient, always a patient,’ so we encourage our pulmonary patients to call us at any time with questions or concerns,” King remarks.

under the law. While this may sound like common sense, not all countries follow the same guidelines, so equal treatment isn’t guaranteed when traveling outside the US. Many people with disabilities find that traveling is truly a hassle, and at times, they are unfairly required to pay extra fees than those requiring no assistance.

What makes the problem even more challenging is that there are as many different types of disabilities as there are disabled folks. Each person’s needs are a little different, and traveling in cookie-cutter airline seats, hotel rooms, and rental cars can be very tricky.

Having a hearing loss shouldn’t stop anyone from enjoying the luxury of traveling. The following tips and resources will help hearing-compromised travelers and their companions anticipate some of the snags of accessible travel: 1. Whether you are traveling by plane, train or ship, make sure your reservation is flagged to indicate having a hearing loss and even what types of difficulties you have, what you can hear, and what you cannot hear. For instance, will you be able to hear the announcement to board the

2. Make sure you are able to receive alerts as a text or email to ensure you know about any changes to departure times, gate changes, or cancellations.

3. If you are reserving a hotel room, make sure it offers closed-captioning television and a vibrating/strobe-light alarm clock. It would also be great if they have a hearing-aid-compatible telephone. Some are now offering TTY systems for the profoundly hearing impaired.

4. Hearing in the airport may be challenging for some individuals, and TSA and/or immigration should be made aware of your hearing difficulties to avoid any misunderstanding of questions or instructions. Wearing your hearing aids or cochlear implant through TSA should not be an issue, but I would suggest any blue tooth device be treated as a mobile phone— take it off and place it through the screener as a safety precaution.

5. Do not put your hearing aids and/or supplies in checked luggage, but rather place them in your carryon bag to ensure they don’t get lost or delayed, ruining your first day on vacation. Treat everything as you would your medication, keeping it in your possession at all times.

6. Having extra batteries, wax filters, and cleaning tools is a must.

Some even take a backup hearing aid to ensure they have access to hearing at all times. If you are going to a humid vacation destination, pack your dehumidifier since all types of moisture will affect your hearing aids or cochlear implant.

7. Having an iPad or smart phone is great, so if you don’t speak the same language or you aren’t able to communicate in the presence of ambient noise, you would be able to communicate by writing or interpreting what is being communicated.

8.Try to take direct flights to relieve the stress of having to hear two or three sets of instructions for flight times and gates.

9. Do your homework prior to traveling and review the places you want to visit to see what services they offer for the hearing-impaired traveler. Are you going to the opera in Austria, do they have a looped system?

There are some travel agents who specialize in specific groups, so consider using a specialist travel agent. Some agents provide stellar niche services; one might be very experienced in working with hearing-impaired travelers, another with developmentally impaired travelers.

Helpful hints for heart-healthy eating

P

eople often ask me where I get the ideas I write about each month.

Truth is, I plan each month a year in advance, but my ideas come from you—my readers—as well as my family. This month, I’m going to talk about heart-healthy eating in response to a family member who recently underwent open-heart surgery at a relatively young age.

There are many diets out there that claim to promote heart health or reverse heart disease—Pritikin, Esselstyn, American Heart Association, vegan/plant-based, etc. And there’s no question that eating habits are pivotal before and after a cardiac UTMC 1247_2 COMMUNITY_HLN_HalfPg.pdf event, especially if you have a family

history of heart disease.

Leading heart scientists met recently at the American Heart Association Scientific Sessions to present their latest research on heart disease, much of it related to genetic research.

They found that multiple genes can be inherited for heart disease—but also that those predisposed can beat the odds by nearly 50% if they adopt and maintain healthy lifestyle habits.

Diet and nutrition were also discussed and presented at this conference.

Based on all this research, here are the latest recommendations from the American Heart Association: 1

A good starting point is to adopt a healthy lifestyle such as DASH, or Dietary Approaches to Stop Hypertension, which focuses on vegetables, fruits, low-fat dairy, whole grains, poultry, fish, and nuts while limiting red meats, saturated fat, sweets and sugars (including sugar-sweetened beverages). But the number-one thing you can do to limit your sodium/salt intake is to avoid processed foods.

2. Achieve and maintain a healthy weight I know, easier said than done, right? But keep in mind the impact excess body weight has on the heart as well as other organs. For example, overweight/obese people over 50 years of age commonly have “fatty liver,” which increases their chances

of developing insulin resistance, unhealthy blood sugar levels, lowgrade inflammation, and higher triglycerides. Losing just five to seven percent of body weight can help improve/decrease fatty liver, and losing 10 percent will have an even greater impact. Cutting 200 to 400 calories per day and adding a walk to your daily routine will accomplish this weight loss over time.

3. Limit added sugar. Excess sugar intake can increase your risk of type 2 diabetes and weight gain. Studies also show that excess sugar can raise blood pressure and increase the risk of stroke and heart disease.

So the American Heart Association recommends getting less than 200 calories a day from sugar, or about 25 grams for women and 36 grams for men. Skip the cakes, cookies, and pies on most days, choosing fruit, vegetables, and nuts as snacks. Also, keep in mind that sugar-sweetened beverages provide a lot of “empty calories,” meaning they deliver very few nutrients other than calories. Instead of soda and other sugar-laden beverages, choose water or other sugar-free, calorie-free drinks.

Primary risk factors for heart disease include an elevated LDL (“bad”) cholesterol level and elevated triglycerides. To lower triglycerides, the American Heart Association recommends limiting sweets as mentioned above, limiting alcohol consumption to no more than one drink per day for women and two for men, and limiting saturated fats, especially red meat. A good pattern of eating to adopt is Mediterranean style, which is rich in seafood, nuts, olive oil, and fresh fruits and vegetables.

Predisposition doesn’t equal destiny If you have a family history of heart disease, keep in mind that genetic predisposition doesn’t equal destiny. Recent studies show that leading a healthy lifestyle can make a significant difference in whether an inherited condition actually will lead to heart disease. Of course, it’s also important to schedule regular visits with your primary care physician and have your lipid profile checked on a routine basis to assess your level of risk.

Complex carbohydrates are key to any athlete’s diet because they provide a steady, nutrient-rich energy supply. Immediately after your race, your body utilizes glucose in order to rebuild and recover from your intense effort. Fruits and vegetables are filled with vitamins, minerals, and fibers essential to a runner. Bananas, a good source of potassium, are believed to decrease cramping.

are probably not aware that mesquite trees produce bean-like pods that were a major food source in traditional diets. The pods are milled and sifted to remove wood chaff to produce a “flour” that has a sweet, earthy taste. (Nina had some for us to sample.) Because mesquite flour is the product of a legume, it is naturally gluten free. Also rich in soluble fiber and higher in protein, at 11-17%, than grain flours. More importantly, it is extremely effective in controlling blood sugar levels in people with type 2 diabetes. (More about that later.)

You can’t have a decent Thanksgiving celebration without some squash.

(Children may have a slightly different opinion.) The Tohono O’odham were into squash in a big way. Nina brought some samples that were about the size of basketballs, but she explained that, fully mature, they grow to 40 lbs. or so. These squashes have adapted to the desert by having leaves that fold up during the heat of the day to avoid loss of moisture and an extra-thick skin to further inhibit water loss.

The skin is so tough that thin strips of it are cut and woven together to make rope.

Shirley and I went to Mission San Xavier del Bac on the Tohono O’odham Reservation just a few miles south of Tucson. In addition to the spectacular mission church, we always enjoy some Indian fry bread. We ordered green chili Indian tacos for lunch. The flour dough taco shells are patted out by hand then fried for just a few seconds in hot oil. Then they are served with diced beef simmered with green chilies and topped with tomato, onion,

lettuce, and cheese. There you go. ”Indian” tacos.

I chatted up the woman who was making ours. She acknowledged that the recipe was “borrowed from other cultures.” Perhaps because the Ancient Ones were not familiar A "genuine" Indian taco with beef, green chilies, lettuce, tomato, and shredded cheese.

The whole concept of “Indian tacos” is symptomatic of what has happened to the Tohono O’odham.

They have grown accustomed to adopting from “other cultures” to the extent that they no longer know their own traditional foods. The results are physiological as well as sociological. According to the “thrifty gene” theory, the Tohono O’odham evolved the ability to live frugally in a desert environment where food was not abundant. It is the same effect that many dieters experience. Reduce your food intake dramatically and your metabolism slows way down.

Then, when you eat a single Dorito, you gain 7 lbs. No. That’s a lie. No one ever ate a single Dorito.

In 1938, the average Tohono O’odham weighed about 140 lbs.

Forty years later, the average was more than 200 lbs. and rising. According to Nina, in the second half of the 20th century, obesity among the Tohono O’odham contributed to the highest

rate of type 2 diabetes in the country at more than 50%. (The average for all Americans is bad enough at about 8%.) So, restoring their historic diet and active lifestyle is more than just a matter of recovering their heritage.

It is also a matter of recovering their health. It is not an easy process.

Consider how many times a day you consume something you know you really should not. Why would it be any easier for Native Americans to give up Twinkies and actually use that gym membership than it is for you?

Ok, I bought these cholla buds. Now what?

Shirley and I visit the reservation farm co-op store to buy products like cholla buds, tepary beans (Shirley won a chili cook-off using them and some prickly pear cactus), and mesquite flour. The challenge faced by the Tohono O’odham is indicated by the recipe pamphlets they distribute with what they sell. Ok, we bought this jar of cholla buds. Now what? The pamphlet says we could put some in a salad (evidently the Ancient Ones loved a nice green salad) or a quiche, or lasagna. There is just no end to “borrowing from other cultures.”

Still, there is increasing interest in growing and using traditional foods on the reservation and in towns like Ajo where there are experimental community gardens for traditional crops. Just imagine the day when Papa John’s offers Indian Pizza with a crust of mesquite flour topped with cholla buds and prickly pear. Until then, the Desert Rain Café in Sells, AZ serves cholla pico de gallo, prickly pear chicken wraps, and tepary bean & cheese quesadillas. Native-American food has a long way to go to catch up with Italian-American. The Tohono O’odham know it won’t be just a walk in the park, but they now have

arlington Nursing and Rehabilitation Center recently celebrated the grand opening of their Dialysis Den, with representatives from area hospitals, home health agencies, hospice, independent living, assisted living, skilled nursing facilities, retirement communities, rehab hospitals, and more in attendance. Downsizing for seniors is a difficult task

I

t’s complicated. Many seniors have lived in their homes for many, many years. In most cases, it is the house where they raised their family. With years comes “stuff.” When it’s time to downsize, the “stuff ” can be paralyzing and will actually prevent some seniors from shedding unneeded and unwanted items, causing them to stay in their homes. And while living independently is the goal for most people, that well-loved home might not be the safest place to be.

Stairs, stuff, and tripping hazards can be deadly. Simplifying and downsizing are excellent goals for older persons, but what are some tips for success?

1. Don’t bite off more than you can chew. Tackle one room at a time. Don’t expect to downsize in a matter of days. Try to set a goal to be done in a number of

months. Work a couple of hours each day and finish a room at a time.

2. Frame the “keep” or “get rid of ” questions with seniors as yes-or-no questions. Open-ended questions cause confusion and will slow you down. Here’s an example: “We will keep your everyday dishes and get rid of the fancy china that you never use. Is that ok?”

3. Use the new residence as the goal and the space reference. Work toward what will fit into the new

space. This creates a finite amount of items that can go.

Have two piles: the Keep and the Discard pile. Do not have a Maybe pile. The items in the Maybe pile will end up getting boxed and will never be looked at again.

Digitize photos, videos, and movies.

But don’t throw the old photos away, especially if there are points of interest, old vehicles, or other items in the photo that could have historical value. Your local historical society or college may want them. A family member or friend may want the others.

Items that have value should be gifted or handled by a selling professional. Take the time to do some research so that these items are not lost or discarded.

Check the references of selling professionals.

Learn about charity organizations that take donations, online garage sales, Craigslist, and liquidators/ estate sale organizers as possible methods to deal with the items that will not be kept. Check references of professionals that you bring in to help sell your items.

Important papers can be downsized.

You must keep originals for life of the following items: birth and death certificates, social security card, pension plan documents, ID cards and passports, marriage license, business license, any insurance policy (good to keep even if they have a digital copy in case problems come up), wills, living wills, powers of attorney, vehicle titles and loan documents, house deeds, and mortgage documents.

There are some papers that you keep for a little while: tax records and receipts (keep for 7 years); pay stubs and bank statements (keep for one year); home purchase, sale, or improvement documents (keep for 6 years after you sell); medical records and bills (keep at least a year after payment in case of disputes); and warranty documents and receipts (keep as long as you own them).

Finally, keep the most current social security statement, annual insurance policy statements, and retirement plan statement. Get a shredder or take old documents somewhere that will shred them for you. Staples and Office Max have this service. ❦
