PROVIDING QUALITY HEALTHCARE FOR ALL SINCE 1977 Dear Friends: As we close out our 41st year of caring for our community, I am proud to say that Healthcare Network of Southwest Florida continues to change the way healthcare is delivered to Collier County residents. Healthcare Network was started in 1977, with a very simple goal in a challenging healthcare framework: to provide quality medical and dental care, accessible to all. Thanks to dedicated employees, collaborations with local partners and continual support from our community, Healthcare Network is finding unique solutions necessary to increase access to primary medical, dental, behavioral healthcare, and now senior care for our community. With a staff of 434 employees now serving over 50,000 patients totaling 204,065 visits at 22 practices in Collier County, our dentists, doctors, nurses, psychologists and specialty providers are ensuring that better health is never more than a phone call away for you and your family. And as we continue to care for families throughout all stages of life, we have begun to address an increasing need: quality senior care for the 31% of Collier County’s aging population. For far too long, society has treated all seniors over the age of sixty-five in the same way, and it is time to develop programs that respond to what seniors at various stages in the life cycle want and need when it comes to their healthcare. A coordinated care model like that of Healthcare Network’s newly developed Senior Care practice not only works, but also reduces costly and preventable hospitalizations or emergency department visits whenever possible, by focusing on health management, disease management and medication management programs. With the healthcare system in a state of flux, it is becoming harder to find primary care doctors, especially those who practice senior care and accept Medicare. With the high cost of care, health centers like Healthcare Network are more important than ever as our model focuses on preventive and positive health outcomes, not just revenue. As we enter our new fiscal year in April, we are proud to announce that we are continuing with our mission to provide care to all by building a 50,000 square foot future center in Golden Gate City in 2020. The new Nichols Community Health Center will enable us to effectively serve the residents in Golden Gate including the many individuals who care for our community, the firefighters, the teachers, and the hospitality workers who keep our economy thriving. By providing these families and seniors with comprehensive care, our doctors will be able to provide access to an underserved community of 45,000 individuals and ensure they receive necessary treatment and provide preventive education that reduces long-term health care costs. We cannot do this alone. Your support is vital to ensure all members of our community have access to quality, comprehensive healthcare. Together, we can build a healthier community for all. Respectfully, Mike Ellis, President and CEO Healthcare Network of Southwest Florida
providing quality Healthcare Network of Southwest Florida is a private not-for-profit organization providing high-quality primary comprehensive healthcare to patients of all ages, regardless of their ability to pay. Founded in 1977 by community leaders in Immokalee, Healthcare Network now provides care to more than 50,723 residents in Collier County through over 200,000 visits at 22 practices including two mobile units. Primary care services offered include: children’s, women’s, family, senior, dental and integrated behavioral health. Healthcare Network welcomes all patients and provides the same high-quality standard of care to everyone. As more physicians choose not to accept Medicaid or uninsured patients, Healthcare Network has stepped in to care for these patients, offering an alternative to the emergency room or, even worse, no care at all.

giving kids the chance at a healthy life meet joshua Joshua had a terrible accident at school that knocked out his two adult front teeth. His mom rushed him to Healthcare Network’s Dental Care East for treatment. She was extremely worried that Joshua’s beautiful smile would never be the same and that the incident would impact his future dental health. Dr. Larios reassured mom that by bringing Joshua to the dental center so quickly the odds of saving his two adult teeth were greatly increased. Dr. Larios and his team worked quickly to slip and secure Joshua’s teeth back into place. When finished, everyone was literally all smiles.
meet susan Susan came to Healthcare Network of Southwest Florida in May 2018 after her primary care physician relocated out of state. “It was important for me to find a physician who was skilled in senior and mental health care who would also be genuinely interested in my wellbeing,” said Susan. “I found this level of care through Healthcare Network and Dr. Gloth. As a patient, I am most impressed with how incredibly attentive and attuned Dr. Gloth is with his patients. The professionalism, respect, and quality of services delivered by all the staff makes me feel truly blessed to have access to this care.”

collaborating to improve care It is primarily through collaborations with various organizations and institutions that Healthcare Network has been able to grow from two small trailers in 1977 to the vibrant organization that we are today. These collaborations allow up to provide our patients with improved access to care and much needed services.
