The mission of Healthcare for the Homeless - Houston is to promote health, hope and dignity for those affected by homelessness through accessible and comprehensive quality care. Our vision is to provide pathways for moving beyond the cycle of homelessness through integrated healthcare and collaborative partnerships.

This year with your support, HHH’s integrated healthcare services have touched the lives of thousands of individuals experiencing homelessness and housing instability, offering them not only medical care but also hope and dignity. We have strengthened our outreach programs, improved access to essential health services, and forged new partnerships to address the complex needs of those we serve. The profound impact of our work is evident in the stories of recovery and transformation we witness every day.
As we look ahead to the future, we are excited about the possibilities before us We are committed to deepening our impact, exploring new avenues for service delivery, and advocating for systemic changes that will ensure long-term solutions to homelessness. We provided wholeperson, comprehensive medical care to 3,338 people from 59 zip codes through 22,840 visits across our ten service sites.
Medical services include health screenings, preventative care, disease management, acute treatment, and specialty care.

Most patientsreceived 3 or more visits to address these chronic health issues. Services are not just focused on primary healthcare, but identifying barriers to chronic disease management and creative solutions tailored to each patient’s unique circumstances.
We provided twice as many visits to children and families through the Reed Family Clinic as the year before.
Mental and behavioral health services include crisis intervention, counseling and therapy, psychiatry and substance use treatment.

98% of those who screened positive for depression reached remission within 12 months of treatment and care. Providing mental healthcare alongside primary care means people feel cared for and empowered in their journey toward health and housing stability.
On average, every patient had at least six visits with our multidisciplinary care team, including more than 4,508 wraparound support visits.
Wraparound care encompasses all of the other areas of life, such as finding housing, food, transportation, applying for benefits or just someone to talk to.

all of the challenges and barriers that cause housing instability. While securing permanent supportive housing is a critical milestone, continued care and support ensures people thrive in their new homes, breaking the cycle of homelessness for good.

As we reflect on the past year, I am honored to share with you the strides Healthcare for the Homeless - Houston has made in serving our community. Despite the challenges presented by the ongoing aftermath of the pandemic, rising costs, and economic uncertainty, our commitment to providing essential health services to those experiencing homelessness and housing instability remains unwavering.
In 2023, our organization continued to expand its reach and impact, ensuring that individuals facing homelessness have access to comprehensive medical care, mental health support, substance abuse treatment, and vital resources to help them on their journey toward stability
Through innovative partnerships with local housing partners, outreach programs, and healthcare providers, we have been able to meet the evolving needs of our clients and adapt our services to best serve them in their unique circumstances Whether it's delivering street medicine, offering telehealth appointments for those unable to access traditional care, or providing case management services to help individuals navigate complex systems, we are committed to meeting our clients where they are and empowering them to reclaim their health and dignity
As we look ahead to the future, we remain steadfast in our commitment to advocating for systemic change, addressing the root causes of homelessness, and building a more equitable and compassionate healthcare system for all Healing begins with all of us, and together we can build a brighter, healthier future for our community
