PROVIDING CARE WHERE PEOPLE ARE

REPORT

Mission and Vision

The mission of Healthcare for the Homeless - Houston is to promote health, hope and dignity for those affected by homelessness through accessible and comprehensive quality care. Our vision is to provide pathways for moving beyond the cycle of homelessness through integrated healthcare and collaborative partnerships.

“The profound impact of our work is evident in the stories of recovery and transformation we witness every day.”

Leading with Purpose

As I reflect on the first year in my tenure as CEO, I am filled with gratitude and pride for the collective commitment of our staff, generous donors, and community partners to building a healthier Houston for all

This year with your support, HHH’s integrated healthcare services have touched the lives of thousands of individuals experiencing homelessness and housing instability, offering them not only medical care but also hope and dignity. We have strengthened our outreach programs, improved access to essential health services, and forged new partnerships to address the complex needs of those we serve. The profound impact of our work is evident in the stories of recovery and transformation we witness every day.

As we look ahead to the future, we are excited about the possibilities before us We are committed to deepening our impact, exploring new avenues for service delivery, and advocating for systemic changes that will ensure long-term solutions to homelessness. With your continued trust and support, we envision a community where everyone has access to the healthcare they deserve.

Building stability through health and well-being

We provided wholeperson, comprehensive medical care to 3,338 people from 59 zip codes through 22,840 visits across our ten service sites.

Healingbegins withhealth

Medical services include health screenings, preventative care, disease management, acute treatment, and specialty care.

“Every day we see how the lack of stable housing magnifies health challenges, turning manageable conditions into life-threatening crises. Healthcare is not a luxury, it’s a fundamental need.

COMMON CONDITIONS

Most patientsreceived 3 or more visits to address these chronic health issues. Services are not just focused on primary healthcare, but identifying barriers to chronic disease management and creative solutions tailored to each patient’s unique circumstances.

Empowering futures through hope

We provided twice as many visits to children and families through the Reed Family Clinic as the year before.

Healingbegins withhope

Mental and behavioral health services include crisis intervention, counseling and therapy, psychiatry and substance use treatment.

“Talking about recovery, it’s a much more complex area. When you get sober, all the stuff you’ve been running from and covering up is sitting right there. The work I do is recognizing that the full self has to be part of the process so there can be wholeness.”

COMMON CONDITIONS

other mental disorders 1,208 visits

psychiatry and tele-psychiatry appointments

98% of those who screened positive for depression reached remission within 12 months of treatment and care. Providing mental healthcare alongside primary care means people feel cared for and empowered in their journey toward health and housing stability.

Upholding dignity and restoring lives

On average, every patient had at least six visits with our multidisciplinary care team, including more than 4,508 wraparound support visits.

Healingbegins withdignity

Wraparound care encompasses all of the other areas of life, such as finding housing, food, transportation, applying for benefits or just someone to talk to.

Rasmus Temenos Place 403 visits

Obtaining a roof does not resolve

all of the challenges and barriers that cause housing instability. While securing permanent supportive housing is a critical milestone, continued care and support ensures people thrive in their new homes, breaking the cycle of homelessness for good.

“Having been there myself, I can’t stress enough how crucial it is to have services that treat us like people, not just a problem to solve. Wraparound support that addresses every aspect of our lives - from housing to healthcare - that’s what makes a real difference.”

Joseph “Cowboy” Benson, Community Health Worker

EXPENSES

"Having

a place like this to come and actually get the healthcare you need, the dental, just everything after being homeless - it makes a world of difference.”

“I

wake up every morning joyful and always got a smile on my face. When I come here, everybody’s smiling, they’re happy. They really make you feel like you’re at home, and you’re welcome. You’re not just a number, but you’re actually a patient, they really care about you.”
