Letter from our CEO

Frances E. Isbell

As with people around the world, life at HHH was very different in 2020. When we heard news of COVID-19 entering the United States, we began preparing for the possibility of the pandemic reaching Houston. We worked quickly to adapt clinic operations to remain on the front lines, including expanding telemedicine, to continue to serve those in need of our care. Early in the pandemic, the City asked us to become the official testing site for people living on the streets and in shelters, and through surveillance testing, staff identified outbreaks so that the City could quickly respond. As vaccines became available, we became a vaccination site at the clinics, and we also took vaccines to shelters and supportive housing sites. All of these additional services were made possible through the generosity of foundations and people who recognized the need for our unique patient population. We saw an outpouring of contributions from people literally all over the world, and we do not have adequate words to express our appreciation except to say thank you from the bottom of our hearts.

OUR VISION is to provide pathways for moving beyond the cycle of homelessness through integrated healthcare and collaborative partnerships.

Foundations, individual donors and public grants made it possible for nearly 4,000 men, women and families to access comprehensive services at HHH at an average cost of $1,700/patient per year.

3,046 people accessed medical care via 19,474 visits.

853 people received psychiatry, mental health, behavioral health and addiction services via 3,659 visits.

1,297 people were provided with clinical case management via 4,656 visits.

Terry's Journey Terry was ready to start a new life, and HHH is helping him do that.

“Ms. Sharon has helped me get what I need to get my social security card, birth certificate and new ID. I like to work, and I’m ready to work. I need all of those things to start looking for a job.”

HHH is helping Terry move into Permanent Supportive Housing, and, “They also helped me get my medication. They’re helping me out a lot to get back on my feet so I can start helping people and get my life back.”

368 people with histories of chronic homelessness received on-site health and stabilization services within supportive housing from HHH care teams.

1 in 5 patients are over the age of 60.

Mental illnesses remain HHH top diagnoses, often with other chronic co-morbidities.

Responding to

COVID-19

HHH remains on the front lines.

In March 2020, we were deemed the official COVID-19 testing site by the City of Houston for those living on the streets and in shelters.

More than 1,200 people received COVID-19 tests at HHH in 2020.

Through these efforts, we were able to identify outbreaks within shelters and bunk houses and work with the City to quickly respond to help contain the spread of the virus.

On the Horizon

The Reed Family Clinic The Reed Family Clinic will expand HHH's care into pediatrics and whole-family services for a highly vulnerable families that face many barriers to healthcare.

In 2020, HHH relaunched efforts to open a new clinic within a supportive housing community that serves parents and children who have experienced chronic homelessness.

This unique endeavor represents only a few of its kind in the nation.

The Reed Family Clinic aims to break the cycle of multigenerational housing instability.

Opening June 2021, the clinic will serve the 185 parents and more than 450 children who reside within supportive housing at New Hope Housing Reed.

HHH providers are trained and familiar with the unique needs of serving those who are affected by homelessness.

Therefore, they are especially adept at providing early intervention and facilitating access to multi-disciplinary care. Homeless-specific providers are more likely to recognize the effects of trauma and implement early clinical interventions with behavioral health specialists. This type of culturally-sensitive care is shown to be highly effective in improving overall health and long-term stability.

2020 was a challenging and rewarding year for HHH, and as Board Chair, I could not be any prouder of the response and dedication that the staff, leadership and volunteers of HHH have exhibited. COVID-19 profoundly tested HHH, and the flexibility and resilience that has been displayed is phenomenal. We were able to address the effects of this pandemic while addressing the continuing needs of those who are impacted by homelessness. Looking forward, there are exciting programs in process to address the healthcare needs of children and families in transition.

We are thankful for all of the support both financial, and in time, that the dedicated donors and volunteers provide. For 2021 please join us in HHH’s efforts to promote the health, hope and dignity for those affected by homeless.
