Access to Benefits

Many part time and casual employees do not have access to benefits through the hospital and many employees lose benefits at age 65 or retirement.

Comprehensive Coverage

While employees at any hospital may join the plan, employees of endorsing hospitals receive more robust protection, guaranteed to them in a 60-day open window. Find out if your hospital is an endorsing hospital by searching under the resource tab on our website.

Package Eligibility & Benefits

You are eligible for the Signature Package. Your package eligibility is determined by your average hours worked and your employment status. All enrollees must be actively at work and not off on any type of leave such as disability or maternity.

$1000 OF EXCESS LTD COVERAGE AT NO COST Special Offer

Now, employees under age 35 who are working more than 18 hours per week are eligible to apply for up to an additional $1000 of long term disability coverage at no cost.

This additional coverage is subject to medical underwriting and is over and above the $1000 of LTD coverage already included with Signature and Supreme.

Why it’s important

Protecting one’s ability to earn an income is something that is often overlooked by many until it’s too late - especially by those who are young and healthy. At HCP, we believe in the importance of this coverage and know that life can change in an instant, which is why we developed a special long term disability offering to those under 35.

Premium Guide

Rates are effective November 1st, 2024 for Residents of Ontario

If you are not in an open window, you will not be billed a premium until/if your coverage is approved.

Additional long term disability and life insurance offerings (including employee, spouse & children) are available to Signature applicants.*

All rates listed are paid monthly and are inclusive of all taxes if applicable in your province of residence. Your coverage and your premium may be affected by changes in your weekly hours of work, your job status and/or your family status. It is your sole responsibility to make Health Care Providers aware of any such changes at the time of occurrence. Any premium paid towards coverage for which you were ineligible or which you no longer required as a result of non-contact at the time of the changes will not be refunded.

How to Apply

You can apply by completing our online enrollment form using the Buy Now button. A member of the HCP team will review your enrollment form and reach out if any additional information or forms are required.

You can apply using our forms as follows:

Form 1A: Group Enrollment Form to be completed by all enrollees (not required if applying through the Buy Now button)

Form 3: Plan member group health form to be completed by all enrollees applying outside of a 60-day open window or applying for any optional employee Life of LTD enhancements including the $1,000 special LTD offer.

Form 4: Optional coverage worksheet to be completed by those enrollees who wish to purchase any of the optional enhancements to the core Life, LTD, and ADD&D coverage. This form tells us how much coverage you are applying for. If you use the Buy Now button to enroll, this form is not required.

Form 5: To be completed by those applying for optional spousal life insurance.

Form 6: To be completed by those applying for optional child life insurance.

60 Day Offer: To be completed by HR for those enrolling in a 60-day open window. Contact your HR department to obtain the form. This form is not required if you have a copy of your job offer letter or loss of benefits letter.

If coverage for the optional enhancements to the core Life, LTD, and ADD&D is approved, you will be notified and any additional premiums will be automatically withdrawn from your account on a monthly basis.

Premium Payment

Deposit and First Month Premium

A deposit and first month premium payment are required with each enrollment. Both payments are equal to the monthly premium for the plan into which you are enrolling.

The deposit will be held in trust for the duration of the time you are covered under the HCP plan and may serve to ensure there is no disruption in your coverage should we not be able to collect payment from you in any given month - ie. - insufficient funds. If not used to cover premium, it will be returned to you at the time of termination.

The first month premium is used to cover the cost of your first month of coverage under the plan.

You can pay the deposit and first month premium by cheque or credit card.

If you choose to pay the deposit and first month premium by cheque, your deposit cheque is to be dated the same date as your enrollment forms and WILL be cashed when the enrollment forms are received. This cheque is NOT VOID and must be payable to HCP Group Insurance Plan.

Your first month premium cheque is to be dated for the first month in which your coverage will begin. This cheque is NOT VOID and must be payable to HCP Group Insurance Plan.

Ongoing Monthly Premium Payment

Following your enrollment, ongoing monthly premium payments are made by pre-authorized, automatic debit from the chequing account of your choice. The withdrawal will occur on your last pay day each month and are used to pay for your coverage for the following month. You must provide banking details with your enrollment (i.e.: a void cheque, pre-authorized debit form or banking details listed on the enrollment form).

Frequently Asked Questions

What is a 60-day Open Window?

The term 60-day open window refers to a one-time opportunity to receive the core Life, LTD, and ADD&D guaranteed. 60-day open window opportunities arise from the first of the following occurrences:

• Initial hire date at the hospital

• Losing group health coverage as a result of retirement from a full-time position

• Losing group health coverage as a result of a transfer from full-time to part-time

• FIRST offer from the hospital of non-temporary or non-contract employment

* 60-day open windows apply only to those employed at an endorsing hospital. Other specific circumstances may apply.

What does “actively at work” mean?

In order to be considered actively at work, you must be a hospital employee that is scheduled to work. For example, you are not considered to be actively at work if you are on a disability or maternity leave.

Do I have to cancel my coverage if I am no longer employed at a hospital?

Please contact our office to advise us of this change. Depending on your situation, you may be able to keep your coverage or convert the life insurance to an individual plan.

Frequently Asked Questions

If I enroll in a plan that includes basic life insurance, long term disability and accidental death, disease and dismemberment benefits, what happens when I reach age 65?

If you are enrolled in our plan that includes these benefits, your life insurance and ADD&D will terminate when you turn 65. Long term disability coverage will terminate 4 months prior to your 65th birthday.

How do I know if I work for an endorsing hospital?

You can confirm that you work in an endorsing hospital by viewing the list of endorsing hospitals. Or you may call our office at 1-866-768-1477 to speak to one of our Customer Service Associates.
