Rebate checks to be issued in USD currency and mailed to the individual person or company named on the form submitted, which must match the name of the purchaser of the DEXIS Intraoral Scanner. Only one check can be received per eligible scanner serial number.

Customers will receive a second email from DEXIS to schedule installation and training. After the install is completed, DEXIS is notified and will process the rebate claim
After customer receives their scanner, DEXIS will send an email with the product serial number and a link to fill out the rebate form - view page here
Once the status of install is verified and processed, the rebate check will be issued in USD and mailed to customer within approximately 60 days
