Physicians Guide The Way to Our Heart

By Callie Curington Weber Bereavement Coordinator at Abbey Hospice The old adage goes, “The way to a man’s heart is through his stomach.” In many ways, this wisdom is true of many people and not just men. We often schedule our lives and our memories around food. Food provides comfort when we are lonely, memories of past celebrations, and holidays with tables full of signature dishes.

As we grieve people that have died in our lives, it is no surprise that we often struggle with memories of food and celebration. For many people the connection between heart and stomach is powerful and at times overwhelming. We remember sharing meals together, making special foods

for one another, and breaking bread even during leaner times.

The food that we eat connects us and keeps us safe and together. It warms our mouths, fills our bellies and our minds with memories. Some people have signature dishes that they make; others have favorite foods that they eat. My grandfather loved the large boxes of chocolate, the kind that came with the key to identify which piece was a particular flavor. I can still picture him sitting in his rocking chair surveying which piece was next. I still cannot pass by a chocolate display in the store and not think of him. I buy one from time to time to stay connected to him.

What were your loved ones favorite foods? Was there a dish that they prepared that you identify with them? How has

their food touched your heart?

As you remember your loved ones and their foods, remember them. Celebrate their life and their choices. Often it helps people to cook their food and tell stories. For others it is still too hard and too strong a connection and this is okay.

It is very normal in the grief process to have trouble sleeping or even sleep too much, to gain or lose weight, to have trouble focusing, and to cry at random and unpredictable times. It is also normal to feel some guilt, to wish that you would have said something important, and to find yourself struggling to return to your usual routine.

Although these feelings might seem uncontrolled they are very much part of the process.

However, prolonged feelings of grief, disabling feelings that last

for longer than 12 months should be addressed with a counselor or grief professional.

In journeying through grief, it is important to make the steps that feel right for you. There is not a right or wrong but only the direction that your heart needs to go. The grief process is a journey of the heart. Often we try to go on, force our tears back, and hide our emotions.

However, in order to heal and move forward, we must cry, remember, and process the love that we felt and the way that our loved ones marked our lives. It is only through working through our feelings that we can begin the next chapter.

Interventional Spine And Pain Management Offers New Mild® Procedure To Treat Common Cause Of Lower Back Pain If you are over age 50 and suffer from lower back pain that is relieved when bending over or sitting down, you may be experiencing the symptoms of lumbar spinal stenosis (LSS) – a degenerative, age-related narrowing of the lower spinal canal that causes pressure on the nerves and leads to pain and immobility. LSS is a common condition, with 1.2 million Americans diagnosed and treated each year. 1 The traditional treatment continuum for LSS includes conservative care, such as physical therapy, acupuncture, exercise and chiropractic, as well as symptom management with medications, epidural steroid injections (ESIs), pain pumps and/or neuromodulation. However, these treatments do not remove the source of the pain, and, in most cases, symptoms return. In the past, the next line of treatment for LSS included open surgical procedures such as laminotomy (partial removal of the lamina, a plate of bone in the vertebrae) or laminectomy (removal of the entire lamina and the ligaments that are attached to it), and/or fusion. Each of these has risk factors and leads to changes in the natural anatomy and structural stability of the spine.

But now Interventional Spine and Pain Management is offering LSS patients a new treatment alternative called mild*. Developed to remove the source of LSS symptoms earlier in the treatment continuum, mild is a groundbreaking new procedure that safely and therapeutically reduces pain and improves mobility while maintaining the spine’s structural stability. Many mild patients report immediate relief, and most go home the same day.

During the procedure, the physician uses mild devices to remove small pieces of bone and the tissue causing the pressure on the nerves. Performed under image guidance through a 5.1 mm mild Portal (about the diameter of a pencil), mild requires only an adhesive bandage for closure.

Approximately two-thirds of them survive and need rehabilitation.

According to the American Stroke Association, there are about 6.4 million stroke survivors alive today.

Rehabilitation plays a vital role in helping stroke victims regain their ability to take care of themselves and achieve the best possible quality of life.

The types and degrees of disability experienced after a stroke will depend on the area of the brain that is damaged.

Rehabilitation usually begins within a day or two after the stroke.

Rehabilitation typically involves three kinds of therapy. Physical therapy is designed to help patients deal with motor and sensory impairments. Physical therapists work with patients to improve strength, endurance, range of motion, gait abnormalities and sensory problems. They educate survivors how to regain use of stroke-impaired limbs and teach compensatory strategies to lessen the impact of remaining deficits.

Occupational therapy is geared to helping patients relearn skills necessary for daily living, such as get-

Occupational therapists may go to a patient’s home and suggest ways to make it more safe and accessible. For example, they may recommend installing grab bars in the bathroom or removing rugs that could cause a fall.

Speech therapy is done to reeducate stroke patients about all aspects relating to speech, including how to speak, understand, read, write, solve problems and even swallow. Depending on the extent of the language problem, speech therapists can use pictures or demonstrate how to perform tasks several times or in a different way to help the patient communicate.

It is common for patients to become depressed after having a stroke. Signs of depression include sleep disturbances, change in eating patterns, fatigue, irritability, lethargy, weight fluctuations and social withdrawal. Treating depression can include increased social activity, psychological counseling or antidepres-

sant medications.

The success of a rehabilitation program depends on patient cooperation and commitment. Although therapy sessions may be scheduled several times a week, it is important to do therapeutic exercises every day.

Because the risk of having a stroke is higher for people who have already had one, following a healthy lifestyle is important. Stroke patients should eat healthy, be as physically active as possible, control conditions that contribute to stroke or recurrent stroke such as high blood pressure and follow their doctor’s orders. Physicians Guide Winter Weather Doesn’t Cause Colds A myth still prevails that cold winter weather is the culprit behind seasonal colds and flu.

However, medical research indicates that those who reside in cold climates are no more likely to suffer a cold than someone who is basking in warm temperatures.

It’s not wet hair or the lack of a hat that causes common illnesses, it’s the flu or cold germs floating in the air that are responsible for these maladies.

The colder weather may contribute, though. When breathing in cold, dry air, blood vessels can constrict in the nasal passages and dry out in the process, producing less mucous. This can make the nose a more susceptible place for viruses to infiltrate.

The cold weather may also affect a person’s immune system

response. Some people’s immune systems actually strengthen when the weather is cold, while others’ systems weaken.

Therefore, it’s not exactly the cold weather that causes an illness, but it can play a role.

Similarly, when the weather outside is frightful people spend more time indoors, where airborne germs may be present.

Once more, cold weather plays a role in illness but doesn’t exactly cause it.

Whether it’s rain or shine, cold or hot, the best way to avoid colds and the flu is to wash hands thoroughly and regularly, eat a healthy diet that keeps the body in illness-fighting form and avoid others in close quarters when they are sick. ©Metro Creative Graphics

Physicians Guide That Pain in Your Back Could be Linked to Your Feet If your lower back has been hurting, and you don’t remember doing anything to injure it, the source of your pain could be your feet! Foot pain is something that many people try to ignore. After all, doesn’t everyone’s feet hurt now and then? But if foot pain is something that has been with you for quite awhile, it could be causing problems in your ankles, knees, hips and even your back.

That old song, “The leg bone’s connected to the thigh bone...The thigh bone’s connected to the hip bone…,” tells the whole story. Our bodies are like a chain, with one link–or bone–connecting at the joint to another link. Think about what would happen if the first link in the chain was out of position. The point at which it meets the next link would eventually overstress that link and adversely affect the entire chain.

That’s what happens when we have foot pain. If the normal way of walking is painful, we instinctively change our walking pattern. Say you have arthritis, and

your big toe joint hurts, so you change your gait to avoid bending the joint when you walk. Changing your gait changes the mechanics of your ankle joint, eventually causing ankle pain. This change in your walking pattern can also affect the whole chain of your lower body... from the ankle, to the knee, to the hip, and then to the lower back.

When foot pain or a foot deformity causes you to change the way you walk, it changes the way the bones of all those other joints move with each other. Cartilage in the joints can wear down, ligaments and tendons can be stressed beyond their normal range, and arthritis can set in.

Assisted Living: An Enriching Option Assisted living is an alternative living arrangement available for seniors who, because of health and/or safety issues choose assistance in a supervised setting. The resident lives independently while benefiting from support services. The services promote dignity and prolonged independence.

Residents may have private accommodations, the luxury of much community space for socialization and recreation and the security of a safe environment with round the clock staff. Services include 24 hour staffing, assistance with bathing, dressing and grooming, supervision of medications and provision of three meals a day. Physicians Guide Exercises for Seniors With Limited Mobility

Exercising in a chair is an effective way for seniors to stay in shape.

Exercise is important for all people, including the elderly. Yet, seniors who have limited mobility or are confined to a wheelchair may think they couldn’t possibly exercise.

However there are many things such individuals can do to stay in shape — all while sitting down.

Doctors recommend at least 30 minutes of daily exercise for most people. Exercise is beneficial for seniors because it keeps muscles from atrophying, improves mental alertness, strengthens bones, and leads to a healthier metabolism, among other things.

For seniors who think they won’t be able to handle traditional exercise, chair exercises or modified pilates can be effective, even for those with trouble walking or standing for extended periods of time.

To get started, all one needs is some loose-fitting clothing and a chair. Begin by doing some stretching movements of the neck, arms and legs. Rotate the head left and right and in circles to stretch the neck and back muscles. Lift arms over the head and slowly drop down to the sides of the body to stretch arms and back muscles. Lift and lower the legs a few times to warm them up.

To begin exercises, start slowly and gradually build up repetitions.

Exercises to try include boxing or punching into the air, arm circles, and arm curls. For the legs, work the

back and front of the legs. Kick the legs out in front of the body several times. Hold legs parallel to the floor (as much as possible) and do leg crosses. Put feet flat on the floor and lift up the heels. Keep feet on the floor and push down to work the buttocks and the top of the thighs.

Individuals who have mastered these exercises and have been told by a doctor that it is OK to do something a bit more strenuous may want to add very light weights to the equation for more resistance. Two or five pound weights are all that’s needed to give muscles even more of a workout.

Those looking to change things up from day to day can think about doing yoga in a chair or even tai chi exercises. Deep breathing and meditation after workouts can be part of a cool down and stress-relief program.

If swimming is possible, or even simply entering a pool, water provides gentle resistance and could be a good way to work the body in a gradual manner. Buoyancy from the water will be easy on joints and this type of exercise is very low-impact.

Physicians Guide The In-Office Teeth Whitening Procedure While details may vary, a fairly standard routine is followed.

Typically, the steps involved are not painful or uncomfortable; in fact, many patients doze or watch a DVD or TV during the procedure.

• A cheek retractor is inserted into the mouth, exposing all the "esthetic zone" teeth (teeth that are visible when you smile).

• A liquid rubber dam or hardening resin is painted onto the gum tissue to protect against any irritation caused by the bleaching gel.

• A bleaching gel containing hydrogen peroxide is applied to the esthetic zone teeth and kept on for approximately 15 to 30 minutes.

• The bleaching gel is suctioned or washed off, and fresh gel is applied for one or more additional periods of 15 to 30 minutes.

• Some whitening treatments incorporate an intense light that is focused on the teeth and is said to activate or enhance the bleaching

process. Opinions vary as to whether this light improves the bleaching outcome.

• Between gel applications, the teeth are checked to see how well they have whitened, and whether more bleach needs to be applied.

• After the final gel application, the cheek retractors are removed, the patient rinses and the immediate post-treatment shade change is measured. The teeth may whiten by as few as two to three shades or as many as eight (out of a total of 16).

Part of the whitening effect is due to dehydration during the bleaching process, which makes the teeth look whiter than their true new color.

That color will emerge after a couple of days.

If a satisfactory level of whitening hasn't been achieved, your dentist may recommend follow-up in-office bleaching at a future date, and/or a regimen of take-home bleaching trays.

Physicians Guide Can Diabetic Women Have Babies?

For years the norm was to tell women who were diabetic not to get pregnant. The bodily changes that occur during pregnancy, as well as the potential risks to the fetus and mother led many doctors to take this overly cautious position on pregnancy. Today, pregnancy is risky for diabetics, but can be successful if careful monitoring and attention to health is followed.

Good blood sugar control is essential for women thinking about becoming pregnant. Sugar levels should remain in the ideal range, which is 70 to 100 mg/dL before meals, less than 120 mg/dL two hours after eating, and 100-140 mg/dL before the bedtime snack.

Women considering pregnancy should work with a doctor to be sure glucose levels are in check. That’s because many times women become pregnant without knowing it and the early weeks of embryo formation are essential. High levels of sugar early in pregnancy can lead to birth defects, say doctors.

All women need more rest, an increase in nutrients and periodic monitoring to grow a healthy baby.

The same can be said for diabetics.

Women who have diabetes may find that being pregnant causes abrupt changes in their blood-glucose levels. In fact, non-diabetics often experience changes in blood sugar as well, which is why a blood-glucose test is recommended in the midpoint of a pregnancy. Some women develop gestational diabetes when they had no previous types of diabetes.

In addition to the normal problems high blood sugar could present in diabetics, pregnant women can be at risk for miscarriage or problems during labor. Too much sugar may contribute to an excessive amount of amniotic fluid forming, forcing a fetus into preterm labor. What’s more, infants born to diabetics tend to be larger in size, called “macrosomia,” meaning “large body.” This can present discomfort during pregnancy and lead to complications

during delivery and may necessitate a cesarean section. With macrosomia, the fetus receives too much sugar via the placenta. The fetus’ pancreas senses the high sugar levels and produces more insulin in an attempt to use up all the extra sugar.

That extra sugar is converted to fat, making a large baby.

An infant just born may experience very low blood-sugar levels from all the insulin being produced.

The baby may have to be given glucose intravenously to restore sugar levels.

During the entire pregnancy more insulin or oral drugs may be needed to control the diabetes, and diet will need to be more carefully monitored. An obstetrician also may require frequent check-ups to ensure both mother and baby are doing well.

While it is possible to deliver at term, many OBs prefer that diabetic women give birth a few weeks in advance of their due dates because of the larger size of the fetus. Also, dia-

It might lead to the baby being born early and also could cause seizures or a stroke (a blood clot or a bleeding in the brain that can lead to brain damage) in the woman during labor and delivery, according to the Centers For Disease Control and Prevention.

Winter Storm January 2011: The Home Care Perspective Client care is synonymous to SAFETY and the wellbeing of home care clients should motivate decisions taken in a Home care agency particularly in the event of an inclement weather. Management and Coordinators in collaboration with Case Management should work together to ensure the safety of all clients before any storm.

Agencies should follow the Policy and Procedure for Disaster Preparedness which should require that all clients be contacted and informed of the possibility of missed visits for some days and the reason why. Uninterrupted services would need to be approved and provided to level 1 clients and other clients who live alone and would have a problem caring for themselves if there was no assistance in a day or two.

In addition to the calls and extended services, the local Emergency Service Call Centers and Red Cross should be alerted with specific list of clients who might need a well person check as situation would permit. However, 24 hour telephone assistance should

also be available to all clients and daily phone calls should be made throughout the storm to ensure that all clients remain safe.

All staff should also be contacted to encourage them to make decisions that would ensure their safety for the duration of the inclement weather.

These would ensure that all clients are catered to accordingly with the least probability of calling for emergency because there has been a strategic plan and implementation of the P&P for Disaster Preparedness.

Exceptional dedication of the direct care staff and the team spirit of all would reduce healthcare cost and keep a smile on the faces of all clients and their families.

Even in the face of the storm, there is a need to keep faith with promoting a healthier community by generating solutions and delivering quality home care services safely to clients in the comfort of their homes.
