No Screening Tests for Bile Duct Cancer

DEAR DR. ROACH: A family friend has just been diagnosed with bile duct cancer. She is a breast cancer survivor and is 75 years old. The tumor is said to be the size of a quarter, and it was discovered after she complained of heartburn symptoms.

Are there screening tests that could have detected the condition before the heartburn symptoms? Are there things that she could have done to avoid the cancer? My family members are frightened and want to avoid her condition. -- Anon.

ANSWER: Cholangiocarcinoma, cancer of the bile ducts, is a rare cancer that is, unfortunately, associated with a high mortality rate. There are no symptoms in most people until it is advanced. Since the cancer is in the bile ducts, the most common symptoms are caused by obstruction of the bile ducts: jaundice (yellow in skin/eyes), generalized itching, dark urine and lightcolored stools. Abdominal pain, fever and weight loss are other symptoms.

Cholangiocarcinoma is not a disease that’s amenable for screening; it is rare in North America (it is more common in East Asia, where there are screening programs). Further, the available screening tests, such as blood tests and imaging studies, are not very sensitive, meaning the tests will miss cases. And they are not very specific -- a positive result on the screening test does not necessarily mean cancer. For these reasons, screening is not currently recommended for people at average risk, although it may be considered in people at high risk, such as people with primary sclerosing cholangitis, a chronic liver disease that puts people at high risk for cholangiocarcinoma.

Other risk factors for cholangiocarcinoma are mostly beyond a person’s control: other liver diseases, parasites and genetic conditions. There is some evidence that obesity and diabetes increase the risk of this cancer; however, it does no good for you or your friend to look backward. Never blame the victim.

She should concentrate on getting treated. I don’t have enough information to comment on her prognosis, but the fact that it was diagnosed early -- apparently before obstruction of the bile ducts -- and with a tumor only the size of a quarter, are favorable.

Treatment may include surgery and chemotherapy (before or after surgery).

DEAR DR. ROACH: I have a friend who is a longtime user of diet soda pop. A year ago, she was diagnosed with colon cancer and spots on her liver. She is taking chemo again now. She continues to consume diet soda, which concerns me because I have heard of possible adverse effects of artificial sweeteners.

How would you advise her? I hesitate to say anything, since she might be getting more unsolicited advice than she wants already.

-- M.K. ANSWER: I think that artificial sweeteners are unlikely to have had any role in causing her cancer in the first place, nor in her treatment or risk of recurrence. It’s more important for her to stay hydrated during chemotherapy, and she should have what is easiest for her -- and if that’s diet soda, that’s fine. Even if water is the best choice, I completely agree with your thoughtfulness: Now is not the time to give unsolicited health advice.

GRILLED CALIFORNIA SWEET POTATOES (NAPSI)—The next time you’re firing up the outdoor grill (or indoor grill pan, for that matter), throw on a sweet potato or two. An easy, healthy side dish with a huge wow factor, grilled sweet potatoes are nutrient rich, high in fiber and absolutely delicious. And they go great with burgers, steaks, chops, chicken or pretty much anything off the grill.

Start with this basic recipe, and if you’re feeling creative, try different shapes—sweet potato coins or steak fries, for example. Or add your favorite herbs and spices—such as chopped fresh rosemary, thyme, cinnamon or smoked paprika.

The world can thank Raffaele Esposito for creating the first pizza that resembled the pizza the world knows today. Esposito created the pizza for Italy’s Queen Margherita when she visited Naples in 1889. The crust was garnished with tomatoes, mozzarella and basil to represent the national colors of Italy: red, white and green.

PECAN-CRUSTED RACK OF LAM B W ITH ROSEM ARY BALSAM IC SAUCE 8 servings • Preparation time: 15 minutes • Cooking time: 30 minutes 2 American Lamb racks, frenched 2 tablespoons oil 1 teaspoon salt 3 tablespoons coarse grain mustard 2 cloves minced garlic 3/4 cup fresh bread crumbs 3/4 cup toasted and chopped pecans 4 tablespoons chopped fresh rosemary, divided 4 ounces semi-soft goat cheese, crumbled 1 cup balsamic vinegar Rub lamb w ith oil and salt. Place in a roasting pan, fat side dow n. Roast at 425 degrees F for 8 minutes; turn and roast an additional 8 minutes.

Remove from oven.

Combine mustard, garlic, bread crumbs, 2 tablespoons of the rosemary and cheese. Spread over lamb. Return to oven; cook 15 minutes, until crust is golden brow n. Remove from oven; cover w ith foil to keep w arm.

In saucepan, combine vinegar and remaining 2 tablespoons of rosemary; simmer for 6 to 8 minutes or until reduced to half.

Slice lamb into individual chops; drizzle sauce over chops. Serve w ith roasted butternut squash.

The Car Care Council recommends checking your tire pressure regularly during the winter to help keep the TPMS light off and your vehicle safe.

“It’s typical at this time of year for motorists to get TPMS warnings and then get worried about their tires,” said Rich White, executive director, Car Care Council. “Often drivers will see this in the morning when it’s coldest. If the temperature warms, the light could turn off but it’s likely that tires will still be a few pounds per square inch (PSI) underinflated. This is why it’s important to check tire pressure regularly.”

According to the U.S. Tire Manufacturers Association, for every 10 degree drop in temperature, tire pressure decreases one to two PSI. Checking the tire pressure is important for vehicle safety, tire life and gas mileage.

St. Agnes School to Celebrate Catholic Schools Week

n January 27th through January 31st, Catholic schools coastto-coast will celebrate National Catholic Schools Week, and St. Agnes School of Avon is no exception. For weeks, if not longer, the students at St. Agnes have been looking forward to the many activities the week will bring, including a special mass at the Sacred Heart Cathedral in Rochester. It is also a time of gratitude for this unique educational experience.

Catholic Schools Week will kick off on Monday with a 12:30pm prayer service at St. Agnes Church. On Tuesday, students will sport their pajamas and bring along a stuffed animal friend. They will also participate in reading with “Bigs and Littles,” something the St. Agnes School Principal, Elizabeth Jensen, says they especially enjoy.

“The kids love Tuesday,” she shared. “We pair 3rd-6th graders with the younger Pre-K-2nd graders throughout the entire school year. The older students [Bigs] read and work with the younger students [Littles]. Sometimes during this event, they pretend to be camping out as they read together. It is so special. My 6th graders have already started asking when that day is coming!”

On Wednesday, students will head to Sacred Heart Cathedral along with the 18 other Catholic schools in the Rochester diocese.

All students will be dressed in perfect uniform, and St. Agnes School will be honored to have two of its students altar serving with one student holding the Bishop’s miter (hat) and the other his Crosier (staff ) potentially.

“The Bishop’s mass is especially exciting for us this year,” said Jensen. “We will also have one of our 6th grade students doing her first reading; she is already practicing!” Jensen added that while the Bishop always gives “such a great homily,” the students participation in the mass makes it “extra special.”

Thursday will find students dressing down in red and navy to show their school pride, roller-skating in the gym and hosting a potluck dinner and talent show that evening. Finally, on Friday, students will enjoy a family fun day, either skiing and snowboarding at Swain or bowling at Livingston Lanes in Geneseo. “The community is so supportive of us,” said Jensen.

“During National Catholic Schools Week, we celebrate how lucky we are to have this opportunity thanks to the sacrifices of many, including parents, donors and teachers,” added Jensen, herself a former Catholic school student. “As the heart and soul of Avon, we view ourselves as an extension of a child’s family, continuing to teach the values our students are learning at home.”

Continuing a 144-year tradition of excellence, academics at St. Agnes remain strong, with 85% of St. Agnes students proficient in English Language Arts, and 85% of St. Agnes students proficient in math compared to New York State averages of 46% in English

Language Arts and 50% in math. One success factor is the school’s focus on teaching the whole child: spiritually, emotionally, socially and academically.

“State tests are just one measure of our students’ success,” explained Jensen. “Faith, how to be a good person, and interpersonal skills are taught along with academics so that students can be the best versions of themselves,” noting that students do not have to be Catholic to attend.

“We also encourage parents to partner with us, volunteer, attend events and use their expertise to make our school even better than it is.”

Still, Jensen reiterated, St. Agnes School is not lacking in academic resources, expertise and best practice. “We are more than Common Core,” stated Jensen. “We follow state standards, and we use the expertise of our highly-qualified, state certified teachers. We also offer academic intervention, tutoring, one-on-one instruction and follow research-based best practices.”

While Jensen recognizes Catholic school may not be the option for everyone, she would like those families interested in a Catholic school education to know that St. Agnes is able to offer a good amount of financial aid. “We offer scholarship opportunities through the St. Agnes School Foundation,” she said. “We will work with anyone who would like to come. We do not believe finances should stop anyone from attending who would like to attend, and I hope people will take advantage of the opportunities we provide.”

St. Agnes School also offers three-year-old Pre-school two days a week and four-year-old Pre-school three half days up to five full days a week. Affordable, afterschool care for Kindergarten through sixth grade students is offered, as well. “It is a special place,” said Jensen fondly. “We have fun, and we learn a lot, creating bonds that last forever.”

For a Christmas present last year, we received a gift card from a popular restaurant/bar in [local town]. Shortly thereafter, they closed for renovations.

We anxiously awaited their reopening. Finally, we were able to stop for lunch. The new owner refused to accept our gift card. We were not aware it was under new management. We do know Sherry would have done anything to retain us as a customer. Sadly, we will not darken the doorway of this establishment again. Hope the new owner brings all new clientele; he will need them if he is to remain in business.

On November 8th, the butcher called and said my deer was cut up. I picked it up and on the way home stopped at [local] grocery store to get a few things.

The meat and head were in the back of my truck.

When I came out of the store the head was gone!

Why would you steal it?

When you mount it, tell your friends that’s the one you stole from the grocery store parking lot. The tag was on the antlers. If you would like to return it, I’d appreciate it, but I doubt you will.

The bride spends all the holidays with her family and forgets his! This is very selfish and crosses the line to just plain mean when your husbands have only one surviving parent. What do you suppose that parent does for the day? Seriously, are the people who raised the man you love so awful? I won’t even go into how sad it is when grandchildren are involved. Take a minute to take stock of your holidays.

Are you respectful and inclusive of your husband’s families, or are you guilty of this behavior? Once you’re finished with all the excuses as to why you don’t go there, take time to put on your big girl pants and go act like a considerate adult.

You can set a good example for the kids. At least give your husband time to take the kids for a visit.

Attention Daughter-inLaw! Your husbands have families, too! I am seeing more and more one-sided holiday arrangements.

Enough is Enough about wheelchair rights! Please get off it. With all the horrific things happening in the world, move on about being an adult in a wheelchair and being treated like one. Be happy if someone is considerate and takes the time to try to help or assist someone they may think needs assistance. This could be anyone, and not necessarily someone in a wheelchair. If you don’t want the help, then tell them so.

Simple as that. Stop this obsessing about whether it is an assault or someone feeling “pity” for you. I thank God that my mother was a dignified and gracious woman (in a motorized wheelchair) who had enough common sense to know how to deal with someone trying to assist her. Get a life!

It simply takes a commitment to making small, sustainable changes that add up to impactful, healthy habits. Dr. Samara Sterling with The Peanut Institute says it’s best to begin by concentrating on the good habits you already follow.

“Set yourself up for success by building on the healthy things you like to do,” says Sterling. “For example, if you typically eat one serving of fruit a day, figure out a way to increase that to two servings.”

Over time, as you amp up your healthy habits, the detrimental ones tend to fade away more easily.

As part of a good health reset, Dr. Sterling recommends incorporating more protein, especially plant-based proteins, into a daily diet.

“Proteins and healthy fats make you feel fuller for longer and can help eliminate the urge to snack and consume empty calories,” explains Sterling.

Peanuts are an example of a powerful plant protein. Just one serving of peanuts delivers seven grams of protein, 19 vitamins and minerals and heart-healthy fats.

“Peanuts are categorized as a superfood because they’re nutrientdense and deliver superior health benefits in a very small serving,” says Dr. Sterling. “If you combine superfoods, like kale and peanuts or quinoa and peanuts, you’re really maximizing your nutritional intake.”

There’s more good news because peanut butter also packs protein.

“Research has shown that the consumption of small amounts of peanuts or peanut butter has been associated with a reduced risk of cancer, heart disease and diabetes,” says Sterling. “It’s easy to incorporate peanuts and peanut butter into breakfast, lunch or dinner.”

Mediterranean Grain Medley with Peanuts 3 Tbsp. peanut oil 2 garlic cloves, minced ½ tsp. hot pepper flakes ½ red bell pepper, diced 1 yellow squash, diced 8 oz. snap peas 2 cups beluga lentils or brown lentils, pre-cooked 1 cup jasmine rice, pre-cooked 2-inch piece of ginger, grated 1 cup peanuts, plain, roasted 1 bunch green onions, chopped Salt to taste, Lime juice to taste, Cilantro as garnish Heat a sauté pan and add peanut oil. Add garlic and pepper flakes, cook for 30 seconds. Add bell pepper and squash, sauté for 3–4 minutes.

Add snap peas, cook for an additional minute. Add lentils, rice, ginger, lime juice and salt. Mix well. Heat the grains for 1 minute then toss in the peanuts and green onions. Serve warm and enjoy!
