If the market is down and seniors can withstand the work and cost a little longer, it may be best to wait until things bounce back in sellers’ favor.

According to the World Health Organization, standing water is a breeding ground for mosquitoes, which can breed in great numbers in pools of water. Mosquitoes are known to carry diseases like malaria, West Nile virus and Zika.

Furthermore, mosquitoes that bite pets can transmit heartworms, a serious problem if gone it goes undiscovered. Standing water also can be a haven for bacteria, mold and parasites that are dangerous to human health. If standing, stagnant water is a problem in your yard, remediation is necessary.

Directing downspouts away from the house can remediate standing water. Changing the grading of soil so that low spots are elevated is another way to reduce instances of standing water. This may be a project that requires the assistance of a drainage professional. In addition, homeowners can remove standing water from empty flower pots, pool covers, bird baths, and more to reduce the likelihood that mosquitoes will appear on their properties.

DEAR DR. ROACH: I am 75 and have had Waldenstrom macroglobulinemia for three years, which has been treated with ibrutinib (Imbruvica) and is now in remission. A side effect of Imbruvica is bleeding, and it requires the drug to be withheld before procedures.

I need a colonoscopy and a prostate biopsy now.

Ideally, how long before and after the procedures should Imbruvica be withheld? If the procedures cannot be performed a day apart, how long is too long an interval between them? How often can Imbruvica be withheld?

Also, at my age, how long is the recovery period required for anesthesia when undergoing two separate procedures? -- T. ANSWER: Waldenstrom macroglobulinemia is a rare disease -- a type of blood lymphoma and a cancer of the bone marrow. Ibrutinib is a new effective treatment for many people who have this condition and sustain remissions from the disease.

As you correctly say, bleeding can be a problem with this medicine, and it is generally held for three to seven days before and after surgery (depending on the type of procedure and the risk involved).

Because a prostate biopsy is a low-risk procedure and a colonoscopy has almost no risk (unless a biopsy is required, then making it a low-risk procedure), most experts would only hold the medication for three days before and after the procedure.

There have been reports of the disease flaring up when withholding the drug, so as little time as possible is the best answer of how long it should be withheld. If you can get the two procedures done within a few days of each other, that would seem reasonable to me, but it is your hematologist whose clinical judgment is required. BREAST CANCER TREATMENT Most patients with breast cancer have surgery to remove the cancer.

Sentinel lymph node biopsy is the removal of the sentinel lymph node during surgery. The sentinel lymph node is the first lymph node to receive lymphatic drainage from a tumor. It is the first lymph node where the cancer is likely to spread. A radioactive substance and/or blue dye is injected near the tumor. The substance or dye flows through the lymph ducts to the lymph nodes. The first lymph node to receive the substance or dye is removed. A pathologist views the tissue under a microscope to look for cancer cells. After the sentinel lymph node biopsy, the surgeon removes the tumor using breast-conserving surgery or mastectomy. If cancer cells were not found in the sentinel lymph node, it may not be necessary to remove more lymph nodes. If cancer cells were found, more lymph nodes will be removed through a separate incision. This is called a lymph node dissection.

Types of surgery include the following: • Breast-conserving surgery is an operation to remove the cancer and some normal tissue around it, but not the breast itself. Part of the chest wall lining may also be removed if the cancer is near it. This type of surgery may also be called lumpectomy, partial mastectomy, segmental mastectomy, quadrantectomy, or breast-sparing surgery.

• Total mastectomy: Surgery to remove the whole breast that has cancer. This procedure is also called a simple mastectomy. Some of the lymph nodes under the arm may be removed and checked for cancer. This may be done at the same time as the breast surgery or after. This is done through a separate incision.

• Modified radical mastectomy: Surgery to remove the whole breast that has cancer, many of the lymph nodes under the arm, the lining over the chest muscles, and sometimes, part of the chest wall muscles.

Chemotherapy may be given before surgery to remove the tumor. When given before surgery, chemotherapy will shrink the tumor and reduce the amount of tissue that needs to be removed during surgery. Treatment given before surgery is called preoperative therapy or neoadjuvant therapy.

Even if the doctor removes all the cancer that can be seen at the time of the surgery, some patients may be given radiation therapy, chemotherapy, or hormone therapy after surgery, to kill any cancer cells that are left. Treatment given after the surgery, to lower the risk that the cancer will come back, is called postoperative therapy or adjuvant therapy.

If a patient is going to have a mastectomy, breast reconstruction (surgery to rebuild a breast’s shape after a mastectomy) may be considered.

Breast reconstruction may be done at the time of the mastectomy or at some time after. The reconstructed breast may be made with the patient’s own (nonbreast) tissue or by using implants filled with saline or silicone gel.

HALLOW EEN CELEBRATIONS ACROSS THE GLOBE Candy, time spent w ith friends and colorful costumes make Hallow een a day to be enjoyed by children and adults alike. Hallow een also can be a lucrative time of year, as the National Retail Federation notes that Hallow een spending equals roughly $6.9 billion each year. But as popular as Hallow een seems, many countries do not celebrate the holiday.

Hallow een observances are limited mainly to North America and areas of Great Britain. Thanks to North American influence and trade, some elements of Hallow een have spread to other areas, including South America, Australia, New Zealand and parts of Asia. But Hallow een celebrations outside of North America may not be recognizable to those accustomed to celebrations stateside.

In Scotland and Ireland, traditional customs of Hallow een include dressing up in costume or “ guising.” Parties may be held, and some people have bonfires or set off firew ork displays. Filipinos meanw hile return to their hometow ns and purchase flow ers and candles in preparation for All Saints Day, w hich takes place the day after Hallow een.

In China, the Hallow een festival is know n as Teng Chieh. Food and w ater are placed in front of photographs of family members w ho have passed aw ay w hile bonfires and lanterns are lit in order to light paths for the spirits as they travel the planet on Hallow een night. In Latin America and M exico, Hallow een is know n as “ El Dia de los M uertos.” Translated as “ the day of the dead,” Dia de los M uertos is a three-day celebration that begins on the evening of October 31 and culminates on November 2. It is more a celebration of departed friends and family members than a chance to get hordes of candy.

In France, Hallow een w as relatively unheard of until the mid-1990s. It w as considered an American holiday and not celebrated. How ever, the draw of parties and costumed festivities led some French to embrace certain components of Hallow een.

Help Reduce Animal Overpopulation One of the easiest ways to prevent pet overpopulation is to spay and neuter animals. Cats can reproduce at very fast rates. According to the Cat Rescue, Adoption & Foster Team of Central Oregon, two uncontrolled breeding cats can create the following situation if they have two litters a year at a survival rate of 2.8 kittens per litter: 12 cats in the first year, 66 cats in the second year, and 2,201 cats in the third year. Cats reach puberty between 4 and 12 months of age. Female cats reproduce between January and September, and might come back into “heat” every 14 to 21 days until they have bred or daylight decreases considerably. Cats can give birth 60 days after they have bred.

Will Bells Help Stop Cats From Attacking Wildlife?

DEAR PAW’S CORNER: I’m writing about your recent article on the pack of outdoor cats that had started hunting in the person’s yard. This person was looking for ways to keep birds and other small creatures safe from these cats. You should have added a suggestion that the neighbors bell their cats so that chipmunks, birds and other creatures have a chance to escape.

-- Elizabeth L., via email DEAR ELIZABETH: That’s a good point, and thanks for suggesting it! One issue that the person had was that only a few of the cats roaming together actually had collars, making them wonder if some were housecats and others were strays or feral cats. They also weren’t sure which neighbors the cats with collars might belong to.

If they knew which neighbors had those cats, they could certainly speak with the neighbors about at least putting a bell on their collars to warn wildlife of their approach.

So, an indirect approach is sometimes the best or only way to address the problem in an area with no animal control department. Join the local community’s Facebook page and post about the problem that outdoor cats are causing for wildlife in the area. Find opportunities to speak to others about it at church, school board meetings and public forums.

DID YOU KNOW? A 2017 study published in the journal Cancer Epidemiology, Biomarkers and Prevention found that about one-third of women diagnosed with metastatic breast cancer in the United States live at least five years after diagnosis.

Also known as stage IV breast cancer, metastatic breast cancer is the most advanced stage of the disease.

Metastatic breast cancer refers to breast cancer that has spread beyond the breast and nearby lymph nodes to other parts of the body. Patients diagnosed with breast cancer also should know that improved treatments may further their chances of surviving a diagnosis, even a diagnosis of metastatic breast cancer. In fact, the American Cancer Society notes that survival rates are based on women who were diagnosed and treated at least five years earlier. In the time since those survival rates were documented, treatments could have advanced even further, potentially improving the five-year survival rates for metastatic breast cancer.

Here’s a Halloween tip

Keep all visitors outside. Stay inside when passing out candy. Do not invite strangers into your home or allow them to use the restroom or your phone. You can also place a bowl of candy on the front porch rather than handing it out. Include a sign that says: “Happy Halloween! One per person, please.”

Keep a clear pathway. Keep all walkways, both inside and out, clear from flame-lit jack-o-lanterns, oversized decorations, and home décor. Removing items that pose tripping hazards will not only help keep aging loved ones safe, but also the children coming to the door whose vision may be impaired by a mask.

Avoid using candles: Instead, use battery-powered lights to illuminate your jack-o-lanterns. Flames from candles can pose a fire hazard. Part of a costume could catch on fire or, if forgotten, it could cause a house fire.

Hang a sign: Sorry, out of candy. If you’re not handing out candy or if you run out of candy before trick-or-treating hours end, leave a sign on the front door or porch.

This will deter ghosts and goblins from walking up to your front door and ringing your doorbell. Don’t forget to leave your lights on.

What may be a safe environment during the day may no longer be so safe when darkness descends and children are unchaperoned. Rochester General Hospital Good Employees Keep Coming I’ve posted two articles this past year in Raves about encountering exemplary assistance from Rochester General Hospital (RGH) ER MD and a receptionist at the surgical waiting room. It happened again in the ER waiting room on October 16. It all started at 2PM in our home.

My wife started bleeding profusely from her right nostril. I could not stop the bleeding and called 911. Two guys came and drove her by ambulance to the back entrance of theER as my neighbor dropped me off in the front entrance of theER. I went to a side desk and inquired when I could see my wife back in the ER area. I was told awhile. After 10 minutes of nothing but the young woman behind the desk seeing me distraught said she would go and check on my wife. She smiled and jogged away. She came back in 10 minutes telling me she was going in a room that she arranged and an MD would see her soon. Then she got a uniformed guard to take me to the ER room. Yah!Yah! you will get flowers.

Since October is AntiBullying Month as per the GVPS, I want to state that, as a driver who drives safely, legally, and reasonably, I am tired of being tailgated too closely for comfort, of obscene hand gestures given my way, of loud blaring of the horn to get me to drive the way the bully desires, and of drivers recklessly speeding around me whether on the right side of the road or the left because they deem that I am too slow. Wake up! I am trying to preserve not only my life but yours as well! If you think your behavior is that of a mature adult, think again! I wish all of you could be arrested for reckless driving! Roads are there to transport us from one place to the next and NOT for driving as if we are INDY 500 racers.

Terror on Salem Circle: A Spooky Tradition Lives On

or three out of the past four years, the yard haunt at 5 Salem Circle, Victor, has become a beacon of Halloween spirit and a must-visit attraction for locals. Shana Smithers, proud homeowner and mastermind behind this haunting spectacle, shares her journey from childhood dreams of homeownership to becoming the heart of her town’s Halloween festivities.

Since she was eight, Shana dreamed of owning a home for the sole purpose of transforming it into her town’s legendary Halloween house, reminiscent of a house in town when she was young. She says, “There was a woman, Roberta, who created amazing Halloween displays in her yard. This was in Shortsville where I grew up. I had very little growing up and I was so grateful for having something like that to look forward to.”

Shana’s mission transcends the thrill of spooks and scares. She is determined to pass on the positive experiences and cherished memories she had at Roberta’s Halloween house. For her, it’s about creating an annual tradition that generations will look forward to, a testament to the enduring power of community and shared joy.

Terror on Salem Circle stands not only as a testament to Shana’s childhood dream come true, but as a beacon of Halloween joy that illuminates the hearts of all who visit. With each passing year, the haunt grows, not just in size, but in its power to unite a community and create lasting memories. Shana Smithers and her team prove that, sometimes, the scariest experiences can be the most heartwarming.

Since 2020, Shana and her boyfriend have invested their hearts, souls, and time into the creation of this annual neighborhood masterpiece.

They’ve become proficient in the art of constructing and repairing animatronics, constantly seeking ways to improve their displays. Balancing full-time jobs, their preparations begin as early as May, and Shana says they’ll be working on it right up until Halloween night.

In September 2022, their roof caved in, casting a shadow of uncertainty over that year’s haunt. Devastating as it was, the setback only served to strengthen their resolve. The outpouring of support from parents and children alike, who expressed their appreciation and yearning for the haunt, inspired Shana and her team to return this year.

The community support and appreciation are “definitely our annual inspiration,” she says.

Shana’s ambitious project hasn’t gone unnoticed by their neighbors.

Shana says, “Our neighbors are very understanding. We haven’t had any complaints [except that since we moved in], everyone runs out of candy faster and they have to buy more each year.” She notes the festive community atmosphere that has developed over the years.

Check out Terror on Salem Circle on Facebook and Instagram, and be sure to stop by Halloween night if you’re up for a fright. As Halloween night approaches, the excitement reaches fever pitch. The haunt officially comes to life each October night from 6:30pm-12:30am.

Shana looks forward to Halloween night, though, when she and her team generously distribute full-size candy bars and toys, ensuring that every visitor leaves with a smile and a bag full of treasured memories.

“We’ve noticed a few homes that used to have their lights off are now on with some Halloween decorations.” She adds that one family a few houses down the street with quite a bit of decor, and the father shared with Shana that their children love Shana’s house and help pick out decorations to be like “the Halloween house.”
