Immunotherapy for Peanut Allergies Still a Work in Progress

DEAR DR. ROACH: Why can’t people see an allergist and get desensitized to a peanut allergy? As a child, I was allergic to dust, pet hair, milk and other things, but I received treatments. I am not bothered by any of these now. It would seem prudent to desensitize people who have allergies that are life-threatening, like peanut allergies. -- S.S.

ANSWER: Allergy immunotherapy continues to be a commonly used and valid treatment for many allergies, including environmental allergies, insect venom allergies and, more recently, food allergies. The treatment can be used for peanut allergies, but it is certainly not perfect. So, it isn’t used very often.

The goal of immunotherapy is to get a person to tolerate the allergen that they have an allergic response to. This involves giving very tiny amounts of the allergen below the level that would trigger an allergic response. Over time and under extremely close observation, the amount of the allergen is increased. For allergens like pollen, they are typically given by injection under the skin (or by drops under the tongue), but for food allergens, they are given orally. For peanut allergies in particular, reactions are common during oral immunotherapy. For about 1 dose per 1,000 under supervision in an allergist’s office, emergency treatment with epinephrine was required. Even during maintenance therapy at home, a reaction occurred 3.5% of the time, requiring treatment with 0.7% of all doses. Although oral immunotherapy is effective at getting people to tolerate peanuts, the effectiveness is not long-lived, with only 13% of people still being able to tolerate a peanut challenge three years after discontinuing maintenance therapy. (Thirty-seven percent of people who continued maintenance therapy were able to tolerate the peanut challenge.)

Until there are significant improvements in outcomes, I am unlikely to recommend oral immunotherapy for people with peanut allergies, unless they are unable to effectively avoid exposure to peanuts and they continue to have repeated reactions. Whether they’re on immunotherapy or not, patients are still advised to continue careful avoidance and have emergency treatment immediately available. A new treatment for food allergies was just approved by the Food and Drug Adminstration. Omalizumab works by blocking the immunoglobulin IgE, which is critically important in allergic diseases. The study showed that 47% to 80% of subjects who were treated with omalizumab for four months (given by injection every two to four weeks) were able to tolerate a standard dose of the foods that they were allergic to (peanuts, plus at least two other foods). However, it appears that the benefit of the treatment does not persist after the treatment is discontinued, and it is intended for use in conjunction with food allergen avoidance to protect against accidental exposures.

In the future, combining this new medication with oral immunotherapy could be superior to using either alone, and it can help prevent serious or fatal allergic reactions from inadvertently being exposed to peanuts. Further studies are addressing this issue.

CANNABIS AND CANCER-RELATED PAIN

The National Cancer Institute notes that, while more research is necessary, small studies have indicated the potential benefits of cannabis in helping cancer patients overcome the pain associated with their disease. According to the NCI, a small study of 21 patients with chronic pain who combined vaporized cannabis with morphine experienced improved pain relief compared to patients who took only morphine. However, combining vaporized cannabis with oxycodone, a narcotic pain reliever and cough suppressant that is similar to morphine, did not produce significantly greater pain relief. In addition, two small studies indicated that delta-9-THC, the main active cannabinoidinmarijuana,helped torelievepainaswellasnausea and vomiting. A second study indicated that delta-9-THC given in doses could provide pain relief similar to that provided by codeine, a pain-relieving drug derived from morphine. The NCI alsocitesastudythatindicateda cannabis plant extract medicine effectively relieved pain when sprayed under the tongue of advancedcancerpatientswhose pain was not relieved by strong opioids alone. That study also indicated that some patients were able to continue to control theircancer-relatedpainwithout needing higher doses of the cannabis spray or higher doses of other pain medications they weretaking.

Antoinette Brown Blackwell: Pioneer Minister and Champion of Women’s Rights

Afriend and colleague of Rochester’s Susan B. Anthony, Antoinette Brown Blackwell was a Henrietta native who became the first female ordained minister in the United States and was a prominent suffragist and women’s rights activist in the 19th century.

Born on May 20, 1825 in the family’s Henrietta double log home, when Antoinette was five, the family moved into a fieldstone house her father built on the property. e house, e Brown Blackwell Homestead at 1099 Pinnacle Rd., remains today and in 1988 received the Henrietta Architectural Heritage Award, a er owners, Dr. Elmar & Catherine Frangenberg, restored it.

Growing up amidst the rustic charm of Henrietta, Antoinette fondly remembered a childhood of family love and happy farm life. As the seventh of ten children, she displayed an early inclination towards intellectual pursuits over traditional domestic chores, fueled by her parents’ devout religious convictions. At age 9, she became a full member of the Congregational Church in Henrietta.

Antoinette studied at Monroe Academy, where she then taught, earning the funds necessary to enroll at Ohio’s Oberlin College. Oberlin was notably the first to award bachelor’s degrees to women in a co-ed program.

Undeterred by societal norms, Antoinette boldly voiced her aspiration to pursue ministry, a path traditionally only for men. Despite initial opposition from both Oberlin faculty and her family, Antoinette’s unwavering resolve led to a compromise allowing her to attend lectures and preach, albeit without the formal listing of her as a theology graduate and without official license to preach because she was female. Many years later, Oberlin awarded her an honorary Master of Arts degree and later an honorary Doctor of Divinity degree.

Returning to Henrietta, Antoinette embarked on a tireless journey, traveling the lecture circuits and pulpit engagements to champion causes such as women’s rights and temperance. Her impassioned speeches resonated across the nation, including at the first National Women’s Rights Convention in 1850.

Unfortunately, due solely to her gender and social unacceptance, her voice was sometimes silenced, such as at the 1853 World’s Temperance Convention in NYC, where a “hostile audience” shouted her down and she was expelled, which the ABB Society says was “a watershed for women’s battle for a public voice.”

In 1853, as well, Antoinette became the first woman ordained by a recognized denomination at the First Congregational Church in Butler and Savannah, NY in Wayne County. In 1856, at age 36, Antoinette married Samuel Blackwell, brother of Elizabeth Blackwell, the first female physician in the US, further underscoring her commitment to progressive ideals. ey raised five daughters, and (or but) Antoinette remained active in the ministry.

roughout her life, Antoinette remained steadfast in her advocacy, fostering friendships with prominent suffragists including Susan B.

Anthony and Elizabeth Cady Stanton, while also actively supporting the abolitionist cause. Antoinette’s literary legacy, comprising philosophical treatises and eight books, serves as a testament to her intellectual prowess and unwavering dedication.

In her twilight years, Antoinette continued to wield influence, advocating for suffrage before the Senate Committee and rallying support for the federal suffrage amendment. Her presence commanded reverence, symbolized for example by the standing ovation she received at the 1911 state convention and her prominent role in suffrage parades.

Antoinette was the last of the original leaders of women’s rights alive when the 19th Amendment finally passed in 1920, and she voted at the age of 95. She died in 1921, but her legacy endures. She was inducted into the National Women’s Hall of Fame in 1993, cementing her status as a pioneer and beacon of hope for generations to come.

e Antoinette Brown Blackwell Society is dedicated to inspiring the growth of every individual by promoting the legacy of this extraordinary woman. e Society awards an annual scholarship and selects a Henrietta Woman of the Year to honor in Antoinette’s name. On Saturday, April 6, the Society board announced Lynne Adamson Stewart as the 2024 Henrietta Woman of the Year, as a model of community engagement, a wonderful friend, and a woman centered in her effort to link us all for the betterment of the Henrietta community. Stewart supports local organizations including e Coffee Connection, Red Cross blood drives, FISH-RHAFT, Inc., mentors women with breast cancer, and promotes the work of Henrietta’s ReStore.

How long is too long to leave pets at home?

DEAR PAW’S CORNER: My neighbor across the hall confronted me yesterday, accusing me of mistreating my dog. She said that “Cookie” whines and barks for hours whenever I am gone, and that I need to take him with me rather than leave him alone in the apartment. I work fulltime at an office that doesn’t allow dogs. When I travel, I take Cookie to a sitter. But is 8 to 10 hours away too long for a dog? -- Gerald in Brooklyn

DEAR GERALD: Dogs aren’t always happy about their owners going away for 8 hours a day. ey can develop separation anxiety. is can manifest as nonstop barking, whining or even acting out (like destroying your slippers, pooping in the living room or worse).

Try counterconditioning, a training method recommended by the ASPCA in its article “Separation Anxiety.” Each time you leave home, give your dog a treat or a puzzle toy with treats inside (which will keep him occupied for several minutes). When you get home, put the toy out of sight. e idea is that Cookie will associate your leaving with an enjoyable treat.

You can also desensitize him to your leaving by getting Cookie used to your “departure cues.” For example, if you always put on a coat and pick up your keys when you leave, have a training session where you do the same thing, but then sit down and watch TV instead of leaving. You’ll need to do this (and probably other moves) for several weeks, but it could help.

Ronald L. Johnson, age 85 of Dansville, passed away peacefully surrounded by family on Sunday, May 5, 2024, at UR Noyes.

Ron was born on June 28, 1938, in Dansville to the late Alton & Eleanor (Norton) Johnson. After graduating from high school, Ron was drafted into the Navy and was honorably discharged in 1965. He went on to work as a trucker, starting at Shay’s Service and then AAA Freight until retiring after a successful career at ABF Freight where he was a proud Teamster.

Ron had many joys in life - he was a devoted lifetime member of the Dansville Fish and Game Club and was an avid skeet shooter. He was also a master craftsman, showcasing a variety of hand-carved wood sculptures. Ron was a beloved husband, father, step-father, grandpa and great-grandfather and enjoyed nothing more than spending time with those he loved.

In addition to his parents, Ron was also predeceased by his first wife, Virginia (France) Johnson; his brother, Larry “Jake” Johnson; and many aunts & uncles.

Ron is survived by loving wife of 35 years, Linda (Wolf) Johnson; his children, Rodney (Sandy) Johnson, Renee (Rod) Green, Jeff (Kim) Englert & Paul Englert; his grandchildren: Lindsey (Justin) Backus, Emma Johnson, Caleb Johnson, Tyler Green, Megan (Matthew) Lowin, Alyssa (Chanel) Englert, Evan (Sola) Englert, Connor Englert, Alexis (Trey) Bailey, Jacob Englert, Mackenzie (Ryan) Bassett, Hannah (Matt) Williams, Zander Englert; his great-grandson, Parker Lowin; 7 great-grandchildren; and several nieces, nephews, cousins & friends.

Robin M. Jacobs

Wayland - Robin M. Jacobs, 68 years of age, passed away Wednesday, May 1, 2024, at Strong Memorial Hospital in Rochester, NY. Robin was born April 12, 1956, in Lodi, OH, the daughter of Harold and Elizabeth (Dickens) Owens. Robin grew up in Dansville, NY and resided in Wayland, NY for a number of years. She moved to Black Lake, NY in 1985 and returned to Wayland in 1998 where she lived until her passing. Robin and Gordie Sick met in 1998 and began their epic love story, spending 23 years together as best friends and partners. Gordie passed away January 2, 2021, and Robin never got over losing the love of her life. ere is comfort in knowing they are now reunited.

Robin worked as a Pharmacy Technician at Tops Markets in Dansville for over 20 years, retiring recently. She volunteered at the Vincent House in Wayland and was active within the Perkinsville Fire Department. Robin was a wonderful, giving person who was always willing to help out wherever she could. She was the most supportive mother that a daughter could ever ask for and she was an amazing grandmother to her cherished grandson, Blake. She loved her family, friends, and community; always offering a smile or cheerful word to pick people up. In her spare time, Robin enjoyed reading, cooking, giving treats to any pet she came across, and gathering with family and friends. She will be immensely missed by all.

Robin was predeceased by her parents, Harold and Elizabeth Owens; her stepmother, Beverly Owens; her brothers, David Kuhn and William Smith; her stepson, Kent Jacobs and her beloved companion and soul mate, Gordie Sick. She is survived by her daughter, Brook (Jeff) Peters of Colorado; her grandson, Blake; her stepson, Russ Jacobs of Arkport; her siblings, James (Paula) Smith of Conesus, Catherine Smith of PA, Robert (Karen) Owens of Arkport, Harold L. Owens Jr. of Nebraska; several nieces and nephews; her beloved Aunt Mary Richardson of Dansville; and her dear friends, Irene and Dean Kiefer.

Springwater - Norbert J. Lang, 91 years of age passed away Tuesday, April 16, 2024.

Norbert was born December 12, 1932 in Wayland, the son of Aloysius and Mildred (Loveland) Lang. He grew up in Wayland and graduated from Wayland Central School. After high school Norbert served his country in the United States Air Force during the Korean War. He entered service December 29, 1950 and was honorably discharged December 6, 1954 as Staff Sergeant. Prior to his discharge on April 3, 1954, Norbert and Beverly Miner were married in Albuquerque, NM. ey had dated for 5 years before and spent the last 75 years together.

After his honorable discharge, Norbert had a choice of either going to work at Kodak or Rochester Telephone. He chose Rochester Telephone because he loved being outdoors. He loved climbing the poles and even taught many people how to climb. As the years went on he finally went indoors as a communications technician. After a 41 year career he retired.

Norbert had many hobbies that included shooting sporting clays, hunting, fishing, NASCAR (especially Richard Petty), always joking around, and having a few beers with his buddies. He was a member and past officer of many organizations including one of the original members of the Springwater Ambulance Company, past president, treasurer and life member of the Springwater Rod-n-Gun Club and life member of the Wayland Rod-n-Gun Club, past chief, life member and fire commissioner of the Springwater Fire Department, Livingston County Fire Advisory Board, Livingston County Chiefs Association, Lakeville Firemen’s Exempt Club, Wayland American Legion and the Naples VFW and past steward of Local CWA 1170 (Communication Workers of America.)

Norbert was predeceased by his parents, Aloysius and Mildred Lang; and his siblings, Linda (Roger) Wolcott, Genevieve Cotton and Norma (John) Mark. He is survived by his wife, Beverly Lang of Springwater; his sons, Brett Lang of Springwater, Brady Lang (Liz Briscoe) of AZ, Scott Lang of AZ and Clint Lang of PA; his grandchildren, Danielle Lang of Wayland, Michael (Lacey) Lang of Wayland, Cody Lang of Rochester, Dakota Lang of Rochester, Daelyn (Matt) Rizzieri of Pittsford, Nikki Bill of Canandaigua, Nicholas Lang (Tanya Virgilio), and Jasmine Lang (Zackary Ryan) of Webster’s Crossing; his greatgrandchildren, Jax, Tucker, Colton, Luca and Noah; his brother and sister-in-law, Duane (Ellie) Miner of Rochester; along with many nieces and nephews.

Norbert’s family and friends called on Friday, April 26, 2024 from 4-7pm at St. George-Forsythe Funeral Home, 109 West Naples St., Wayland. His funeral service with Military Honors was Saturday, April 27, 2024 at 11am at the funeral home. Interment will take place at a later date at Mt. Vernon Evergreen Cemetery in Springwater. Contributions in memory of Norbert J. Lang may be made to a charity of your choice.

Richard John Lemen

April 28, 2024

Dansville- Richard John Lemen, age 89, passed away Sunday morning, April 28, 2024, at Elderwood Nursing Home in Hornell. All of his children were at his side in his last days. He was born in Dansville on July 15, 1934, the son of the late Tom C. and Martha Rose Holden Lemen. On June 26, 1954, he was married to Marilyn (Rectenwald) Lemen who predeceased him this year on January 31, 2024. He was also predeceased by his sister, Anita Burger, and his brothers, Lewis and Larry Lemen. Richard graduated from Dansville High School in June 1952. He was the Captain of the High School Band, playing various instruments such as the trombone, piano, and baritone. He had a lovely voice and performed in the musical e Mikado with Marilyn.

Following high school, he attended 1 year of college at Alfred State Technical School. He then enlisted in the Navy where he enrolled in the U.S. Navy School of Music. He was a trombone player in the band on the aircraft carrier Lake Champlain. He did several tours during the Korean War in the Mediterranean Sea. After his honorable discharge he came back to Dansville, started a family and worked many jobs in the area. He retired from Eastman Kodak as an electronics technician and educator in 1991.

His hobbies included wood working, clock repair, fixing anything and making furniture, and toys for the grandchildren. Richard was a member and Elder of the Dansville Presbyterian Church, where he also sang in the choir and played in the handbell choir with Marilyn.

Surviving Richard are two daughters, Barbara (Jim Minerd) Lemen, Caroline (Norbert Nielubowski) Lemen; two sons, Steven (Becky) Lemen and Peter (Wendy) Lemen. Also surviving are ten grandchildren and six great-grandchildren; sisters-in law, Lillian and Jean Lemen, and many nieces and nephews.

Friends may call at 10:00 am on Monday May 20, 2024. A memorial service to follow at 11:00 at the Hindle Funeral Home Inc., 271 Main St., Dansville. Interment, with full military honors will be in Greenmount Cemetery, Dansville. Online remembrances may be shared at HindleFuneralHome.com. Thank you for returning my purse

Last week after shopping at a local store I returned my cart to the cart corral, not realizing I had left my purse in the cart. While driving home and realizing (then panicking) I had left my purse in the cart I drove back to the store. My purse was not in the cart where I had left it so I went inside the store to the Customer Service desk. Yes! Someone had brought my purse in and everything inside was still there. At the time I didn’t think to ask if the person who did this had left a name. But I would like to let whoever it was know how very grateful I am and to thank them. It’s nice to know that there are still honest people out there. Thank you!

At times, during my husband’s recent hospital stay I felt invisible to some of the professional medical staff when they entered his room (thankfully most were fine). As a retired RN I feel it is crucial for professionals ( MD, RN, etc.) to also address caregivers in the room when they arrive to discuss treatment, surgery, or discharge care. At times a patient’s life may depend on it. My advice to the professionals is to take the blinders off and acknowledge and address the caregiver(s) in the room. My advice to the caregiver: don’t be ignored. Speak up with your questions until you get an answer. Find an advocate on the unit that you can work with. There is no need to go it alone or have unanswered questions. Fragmented care isn’t much better than no care.

Foolish Parenting

The surest sign of spring is back in our area. No, it’s not the returning Robins. It’s the Dad with a riding mower and the toddler balanced on his knee. For those of you who think a mower is an amusement park ride, it’s time to learn about all the children who have squirmed, bounced off, and ended up under the mower blades. About 9,000 children a year are injured in mower accidents. Of that number, roughly 15 are fatalities. Why bother strapping your kid into a car seat for a drive when you don’t value their lives enough to keep them away from a mower?
