Can Sciatica Be Relieved Through Steriod Shots?

DEAR DR. ROACH: I am wondering what the best treatment is for sciatic pain. Would a steroid shot help? The pain is behind my knee, down the outside of my leg, and now on my foot. -- J.F.

ARE THERE GOOD AND BAD PROTEINS?

Determining the nutritional value of certain foods can be a tricky business. Many foods can be enjoyed in moderation, and labeling them as “good” or “bad” might lead to negative connotations. In addition, food labels change from time to time as nutritionists and doctors learn more about nutrition and revise their opinions on certain items.

One food type that has remained off the bad foods radar for quite some time is protein. Protein sources are largely touted as the be-all and endall in nutrition. But even seemingly infallible protein should be eaten in moderation, and even then only if the right sources of protein are selected. Protein is an essential building block of good nutrition that is found throughout the body and makes up the enzymes that power many chemical reactions. Protein helps fuel the hemoglobin in the blood that carries oxygen throughout the body.

The Institute of Medicine recommends that adults get a minimum of 0.8 grams of protein for every kilogram of body weight per day (or 8 grams of protein for every 20 pounds of body weight. Physicians in the United States recommend a daily protein allowance of 46 grams for women over the age of 19 and 56 grams for men. Too often, however, people are overloading on protein because they think it’s a better option than carbohydrates and other food sources. But not all protein is the same.

Protein that comes from animal sources offer all of the amino acids a body needs. Unfortunately, some animal sources are less healthy than others. That’s because animal-based protein sources also contain saturated fat. Consuming too much saturated fat may contribute to elevated levels of LDL, or “bad,” cholesterol in the blood. LDL may lead to the formation of plaque in arteries that limits blood flow and may be a risk factor for heart disease. Fatty red meats and whole-milk products tend to contain more saturated fat than other protein sources.

The key when consuming protein is to find the right balance in protein sources. Fruits, vegetables, grains, nuts, and seeds may offer many of the required essential amino acids. The rest can be obtained by choosing smarter animal-based protein sources. Salmon and other fatty fish are good sources of protein and omega-3 fatty acids (heart-healthy fats) and are generally low in sodium. Lentils offer 18 grams of protein and ample fiber. Plus, these legumes have virtually no saturated fat.

ANSWER: “Sciatica” is a general term for a process that compresses the nerve roots, which causes symptoms such as pain, numbness, tingling, and a loss of reflexes. The sciatic nerve supplies much of the innervation to the leg. The area that you describe is most likely the first sacral nerve (S1), and pain here is most frequently caused by disc herniation at the L5-S1 space. This can cause acute symptoms that are often quite severe.

Initial treatment is usually conservative, consisting of anti-inflammatories and avoiding activities that worsen pain. While bed rest was sometimes prescribed, it is seldom necessary for more than a day or two, and many people actually feel better when they are up and about. After a week, most people are able to do light activity, and I tell my patients neither to push themselves to do more than they can, nor force themselves to rest if they feel OK with light activity.

Steroids have certainly been used in this situation, and they are injected into the epidural space by an expert who uses imaging. Although there is a slight improvement in pain around three months among those who are treated with epidural steroids, there was no longer a benefit found around six months. This means that you get better only a little faster than normal at the cost of a small risk from the injection. If a person isn’t recovering well within a few weeks, I often refer them to physical therapy. If they aren’t getting benefit at all (or they are getting worse), then it is time to find out what the cause is with an MRI and a referral to an expert.

n a world o en dominated by metrics and profits, Joy Bergfalk stands as a beacon of compassion and resilience, channeling her life’s mission into Coffee Connection, a non-profit organization on South Ave in Rochester, and Marketplace Mall in Henrietta, dedicated to helping women reclaim their lives from the grips of addiction, trauma, and incarceration. I felt so blessed to chat with Joy at the Marketplace Mall location during a recent rainstorm.

As nature threatened to demolish all that humans created, I felt at home and at peace as we chatted about life, challenges no one should have to suffer, and the amazing women at Coffee Connection. Joy’s journey from a compassionate child rescuing traumatized animals to a pastor guiding women through their darkest times is nothing short of inspiring. All of our firstworld problems shrink when we consider the challenges others go through.

Joy’s affinity for aiding the vulnerable began in childhood, nurturing traumatized animals from the age of five. is early exposure to the healing process laid the foundation for her calling as clergy. Now a pastor in Rochester, Joy has dedicated her career to working with people who have endured trauma.

Her personal life mirrors her professional dedication. Joy and her first husband adopted five siblings from Guatemala, immersing themselves in the complexities of trauma and recovery within their own family. is experience deepened Joy’s empathy and commitment to her work, as she intimately understands the long and arduous journey of healing from profound personal challenges.

Coffee Connection, based out of South Avenue and in Marketplace Mall’s food court, transcends the typical coffee shop experience. It serves as a sanctuary where women, many with arrest records and histories of addiction, find employment as well as a supportive community as they heal. e organization’s mission is threefold: providing sober support, workplace support, and employment creation. is comprehensive approach ensures that women not only maintain sobriety but also acquire the skills and confidence needed to thrive independently.

Founded in 2001 by Nancy Sawyer-Molina, Coffee Connection was born out of a desire to support women in addiction recovery. When Joy joined in 2011, her merger with her own initiative, Project Empower, brought a renewed focus and energy to the organization. Today, Coffee Connection thrives as a non-profit roaster, offering stable employment and skill-building opportunities for women overcoming significant life challenges.

Noise Ordinances

To the person complaining about others mowing between 7am and 9pm, claiming it is unreasonable to mow during those times, most towns and cities have noise ordinances that allow those kinds of activities from those hours. When you factor in some blocks have 30+ houses that all need mowing, of course you’re going to hear mowing all of the time. It's a part of what summer is! Most towns also have a law about the height of grass, so mowing is necessary, and also helps keep the growing tick population at bay. So I'm sorry that people are following their local regulations.

They put in water lines throughout [local town]. They then "fixed" the huge crevices they made in the roads but didn't even make sure they were even with the existing road. There is one spot that is so bad that they have to keep filling it with gravel to make it easier to cross in your vehicle. Why not just fix them properly?! If you're going to mess the roads up for a project you should be held accountable to fix them properly when you're done.

DEAR PAW’S CORNER: My cat “Sticky” is 14 years old and still in great shape. He loves chasing a er a laser beam and rolling around with his toys. However, I’ve noticed that when he comes over to leap up on my lap, he hesitates and just puts his paws on my leg and meows. I have to encourage him to leap, and he doesn’t seem happy about it. DEAR JANINE: At age 14, Sticky is definitely a senior cat, and it sounds like he’s beginning to develop a common senior cat problem: arthritis. Just like us, cats can suffer from stiff, achy joints as they get older. You’re doing a great job of observing his behavior and spotting changes in the way he interacts. Arthritis (typically osteoarthritis, sometimes abbreviated as OA) is progressive, and sometimes not noticeable right away. ere are four key things to watch for:

Running -- Does your cat run with a fast, fluid motion, or in stops and starts?

Jumping -- Does your cat hesitate to take jumps that were effortless in the past?

Climbing -- Does your cat walk or run confidently on the stairs, or hesitate on each step?

Playing -- Does your cat keep up with the laser pointer, or start to lag behind?

Other noticeable behaviors, like excessive meowing, hiding away or even occasionally missing the litter box also signal that your cat is uncomfortable or in pain.

It’s important to consult with Sticky’s veterinarian at this point. Together, you can evaluate his pain level and function, and discuss options for improving his comfort. ere are treatments for cats with OA that will greatly improve his quality of life and get him back to his old self.

Serves 2

1/3 cup orzo

2 large tomatoes

1 cup crabmeat, picked over for pieces of shell

1/3 cup chopped black or green olives

2 tablespoons crumbled reducedfat feta cheese

2 tablespoons chopped fresh dill

2 teaspoons balsamic vinegar

1/8 teaspoon salt

1/8 teaspoon black pepper

1. Cook orzo according to package directions, omitting salt if desired. Drain and rinse under cold water; drain again.

2. Meanwhile, cut thin slice off tops of tomatoes; reserve tops. Using spoon, carefully scoop out seeds and pulp; reserve for another use.

3. Gently toss together crabmeat, olives, feta, dill, vinegar, salt, and pepper in medium bowl. Spoon crabmeat mixture evenly into tomato shells and cover with reserved tomato tops.

Cook’s note: Scoop the seeds and pulp from the tomatoes into a storage container. Cover and freeze up to 4 months and toss them into a soup or stew.
