Professor Rob Moodie

Profits and Pandemics

Professor Moodie graduated from The University of Melbourne in 1976. He is Professor of Public Health and Director of Teaching and Learning at The University of Melbourne’s School of Population and Global Health. He teaches courses on public leadership and health promotion in Melbourne, Mozambique and South Asia.

Professor Moodie first worked in refugee health care in the Sudan with Save the Children Fund and Médecins Sans Frontières, and later for Congress, the Aboriginal communitycontrolled health service in Central Australia. He has worked for the Burnet Institute, the World Health Organisation and UNAIDS and was CEO of the Victorian Health Promotion Foundation (VicHealth) from 1998 to 2007. He was Chair of Melbourne Storm from 20062010, and of the National Preventative Health Task Force from 2008-2011.

Professor Moodie now chairs the Federal Minister’s Men’s Health Reference Group and the Technical Advisory Panel of the Gates Foundation’s HIV prevention program in India. He co-chairs the Australia-India Taskforce on Tobacco Control and is co-author of four books, his most recent being Recipes for a Great Life, written with Gabriel Gate.

Cancer, cardiovascular diseases, diabetes and chronic lung disease (non-communicable diseases) are now the leading causes of death and disability across the globe. The giant transnational tobacco, alcohol, and food and drink corporations have been called the ‘industrial vectors” of these diseases. In his presentation, Profits and Pandemics, Professor Moodie will posit Are these corporations to blame, or are they as they claim, part of the solution?

Twilight Lecture review Wednesday, 18th September, 2013

The Graduate Union was proud to host the Dean of the Melbourne Business School of The University of Melbourne, Professor Zeger Degraeve, on the 18th of September 2013. As the lecture is part of an ongoing course in leadership that Professor Degraeve offers at the Melbourne Business School, it cannot be replicated entirely in this summary review. Rest assured that the general ideology and energy that was conveyed to our attendees will be recounted to the best of our ability.

Professor Degrave is no stranger to leadership. A former Professor of Decision Sciences at London Business School, he is an expert in the art of decision making, a maestro in the forms of operations research and a professional in the management of professional portfolios. As a teacher, he has been awarded for his efforts in professional development programs in Europe, Asia, the Middle East and North America, notably in such companies as HSBC, Cadbury Schweppes, Rio Tinto, Carlsberg and IBM. This must have been the most energetic Twilight Lecture to date. Professor Degraeve moved about sporadically and intently, using his full vocal capacity to describe how we make good decisions. The energy expressed itself in the audience with all those attending engrossed particularly with the Professor’s use of a thumbtack, plate and coffee cup as the analogy for a ‘project’ and his invitation to the audience to contribute $10 to become the project leader, and thence gain authority to decide on the position of the thumbtack (by now shaken and hidden under the cup). If the person guessed the position of the thumbtack correctly, the business idea would be a success and whoever bought into the idea would gain a profit.

If incorrect, the buyer lost their $10 sunk cost. A courageous Mr Casey Tan stood up to the challenge and offered not $10, but $20 (largely due to the absence of a $10 note in his wallet). This move was the start of an eventful evening in which Professor Degraeve outlined whether Courageous Mr Casey had made a good, or a bad, decision.

Alongside Mr Tan, the audience learnt something that now seems quite simple. You can have good results alongside bad, and good decision making, just as you can have bad results formed from good or bad decision making. In an almost Kantian reference, the results of a project ought not to decide the efficacy of an employee, as their decision making should.

As Professor Degraeve posed: if a CEO gives their best employee a project that, regardless of making the correct decisions, results in the company losing $10million, and they then subsequently fire that employee, what happens the next time a project of that size comes around? The CEO has already fired the best employee, who made the correct decisions, so who remains to take on the project? The night resulted in success for Courageous Mr Casey! His $10 investment paid off and he received an actual $40 from Professor Degraeve which he promptly donated to The Graduate Union - thank you. Mr Tan’s success was not a gamble at any point in the night, as many of our attendees, when questioned, had suggested. Professor Degraeve explained quite thoroughly that there are statistical, algorithmic and logical decision-making processes that can be applied systematically in a business environment. As Mr Tan later put it: Aim, Alternatives, Attitude, Aptitude, Actions, Articulation, Audience and Achievements all govern the process of decision making and dictate the ability of a leader.

The Graduate Union would like to sincerely thank Professor Zeger Degraeve for spending the time to introduce us to the interesting process of decision making. His insights in leadership, together with his energetic presentation entertained all those present for this wonderful function..

October Monthly Luncheon review Wednesday, October 2nd, 2013

Professor Stephen Duckett Professor Duckett’s presentation at our October Monthly Luncheon was entitled Silver Tsumani or grey glacier? Next generation issues in health and health care in Australia.

He began by questioning the doom and gloom, and increasingly scaremongering and blaming, messages from the media, government and lobby groups about the effect of baby boomer and older generations on the sustainability of health spending and supposedly out-of-control health costs.

Using a quadrant graph to illustrate comparisons with other countries, he illustrated that Australia in 2004 was in the ‘good quadrant’ of lower ‘health expenditure per head’ and ‘potential years of life lost per 100,000 population (females) to age 69’; and performing well against the Organisation for Economic Co-operation and Development (OECD) average. Reminding the audience, however, of the considerable within-country variation, he showed a bar graph for Australia in 2010 of the number of people who reported access barriers due to cost in the last year according to remoteness. 28% to 32% of those in remote or very remote areas reported skipping a consult due or medication due to cost.

For the decade to 2010, Australian health expenditure per person shows a linear increase and only a recent increase to just under 10% of gross domestic product (GDP). Health is a major component of government spending and spending growth, with the per cent change against the Consumer Price Index (CPI) from 2002 to 2013 being >70% (and >85% for ageing and aged care services). The majority of this public health spending growth is in hospitals, which show a >85% change against CPI (and a 2012 cost of $20bn), as well as in pharmaceuticals and primary care and medical services (both near a 65% increase). Other than population growth, ageing and inflation (beyond CPI), almost two-thirds of real health spending increases came from other factors affecting public hospitals, medical services and medication. Professor Duckett feels that the ‘elephant in the room’ is thus changes in practices and by health professionals, rather than patient-driven changes.

Professor Duckett then introduced ideas for an alternative discourse. Yes, the population over 85 years of age may double but, using a ‘compression of morbidity’ hypothesis, it can be assumed that people in this age group will have 25% less morbidity. Yes, utilisation of public health services by this age group will increase by two thirds but when this message is portrayed as a per cent increase (that is, 2%) it is clear that these increases are approaching at a glacial, not a tsunami, speed. In Stephen’s view, such ‘voodoo (apocalyptic) demographics’ are demeaning to the elderly, ignore their past and current contributions, include no assumptions of ‘intergenerational interlinkages’ and neglect the effect of increased life expectancy on GDP. Given the sustainability panic approach, Professor Duckett suggests that it is tempting to adopt panic (non)solutions, which include shifting costs to consumers, ‘privatise or perish’ and reductions to services. However, Australia is already at the high end of consumer ‘out-of-pocket’ per cent health expenditure (around 10%); and low income households are already spending at their limit and a higher proportion (~7%) of income on health than high income households (~5%). www.graduatehouse.com.au | ABN: 55610 664 963 | Incorporated Association Registration No. A0023234B

Privatisation of the public health system is described by Professor Duckett as ‘risk transfer’ and the ‘name of the game’, illustrating this with reference to clauses from a recent KPMG consultancy recommendation for full outsourcing of services for Sunshine Coast University Hospital, for example: Significant transferred risks include operating cost risk, performance risk, industrial relations risk, and compliance with laws and standards. In addition, (the government) should be able to transfer a degree of demand risk through annual purchasing agreements …

Other cautions against privatisation were provided, such as the question of democratic accountability.

As pointed out by the South Australian Auditor General, public-private partnerships for health care: ... can extend for periods in excess of the life of a particular Parliament and, on the basis of historical experience, the Government of the day.

Contracting risks were also inherent, particularly in describing the product (so that the risk of role and responsibility gaming is reduced), in specifying the asset (so that the risk of its capture is reduced) and in managing the risk of failure, as has been demonstrated with such precedents as Robina, Modbury, La Trobe Regional Hospital and Port Macquarie. As also demonstrated, the longheld assumption that private hospitals are run more efficiently may be mislaid.

Solutions that Professor Duckett presented as sensible and in accordance with the careful and considered planning that can be undertaken given the grey glacial approach included reorientation of the health system, refocussing on efficiency - which he emphasised as not being the same as budget cuts, especially in the context of unmet demand - and the elimination of waste.

As he illustrated in this diagram, system reorientation would be with the patient as central to self-management, with support from family and caregiver networks in a ‘Medical Home’ comprising the family physician and an interprofessional team that is given appropriate and evidence-based system support for decision-making and access to records.

Professor Duckett felt that this should be possible given today’s technology, illustrating poignantly the difference in the capabilities of our Medicare card for information management when compared to our credit card!

With respect to hospital efficiency, Professor Duckett used the example of hip replacements to illustrate the significant cost variations for exactly the same operation within and between hospitals, and within and between States and Territories. In recommending a systematic costing approach, he thus put the questions: Should you reward less efficient hospitals by paying them more for treating patients than more efficient hospitals? and Should you reward poor quality by paying hospitals that have higher rates of adverse events more than hospitals with lower rates?

Concluding his talk with other solutions that included consistency of Pharmaceutical Benefit Scheme (PBS) drug costs, disclosure of medication prices against benchmarks by pharmacy outlets and the undertaking of the 12 million per year ‘less complex’ primary care services by pharmacists and other allied health professionals, Professor Duckett called for a reduction in resistance from the medical profession, for governments to put in the ‘hard yards’ to achieve sustainability and for ‘keeping the baby when throwing out the bathwater’ - that is, acknowledging that Australia has a good health system in international terms but can always improve. www.graduatehouse.com.au | ABN: 55610 664 963 | Incorporated Association Registration No. A0023234B

Stephen has been appointed Dean of the Faculty of Medicine, Dentistry and Health Sciences at The University of Melbourne. Previously he was Vice-President (Research) and founding Dean of the Lee Kong Chian School of Medicine, a joint initiative of Nanyang Technological University and Imperial College London.

Professor Siriporn Chirawatkul resided with us in September, 2013. Professor and Director of the World Health Organisation Collaborating Centre at Khon Kaen University, Thailand, Siriporn specialises in public health management, medical anthropology, and in the fields of mental, women’s and community health. Professor Chirawatkul is a regular visitor to The Graduate Union and we hope to welcome her back soon and to speak about how our members can assist with global health.

Previous resident Maartje Luijten has taken up a new position at Radboud University Nijmegen, the Netherlands. Maartje resided at The Graduate Union in 2011 while a visiting researcher of the Cognitive Neuroimaging Laboratory where she used functional magnetic resonance imaging (fMRI) to investigate inhibitory control and reward processing in smokers. The Graduate Union extends best wishes in her new role and hopes to hear more about the job after she has settled in.

Professor Bryan Mercurio stayed here in August to teach the subject International Business Transactions at Melbourne Law School. Bryan is a Professor and Associate Dean (Research) in the Faculty of Law at The Chinese University of Hong Kong and is returning to Melbourne in 2014 for a mid-year teaching of this course.

Held from the 11th to 27th September 2013, at Union House the exhibition was on the behind the scenes work at the Ewing and George Paton Galleries in the 1970s to 1980s. It also highlighted the relationships between artists and directors through the displayed archival materials, such as correspondence and photographs. Established in the mid 1970s by experimental artists, students and academics, the George Paton Gallery exhibits artists with new ideas, and those who are pushing the boundaries and contexts of art.

The University of Melbourne’s 2013 Festival of Ideas, directed

by Professor Fiona Stanley AC and held 1st to 6th October, explored the science of healthy living faced with 21st century challenges. Each day of the festival had a different theme with Wednesday the 2nd being on the environment, Thursday on food and nutrition, Friday, families, Saturday, brains and minds and Sunday, democracy.

The week was a huge success, attracting large and interacting audiences on site and on-line, with talks being videoed and live-streamed, and plenty of opportunities to participate through mobile phone polls and social media. It was great to see Members and new visitors meet here before and after events during the week and to hear of their enjoyment in attending the many vibrant, informative, well-presented and inclusive presentations and debates by experts from across Australia and from all around the world.

San Francisco. The name conjures up such images as the Golden Gate Bridge, cable cars, Fisherman’s Wharf and Tony Bennett singing I left my heart in San Francisco. It is also within easy distance of the famous Napa Valley and Sonoma County wine country, Silicon Valley and Stanford University.

At 19 years of age, one might expect a typical person to be beginning their university experience, having the time of their lives in a gap year or working hard to move out of home.

Boyan Slat, a young inventor, is no typical nineteen year old.

He has invented a system to clean the garbage spirals that have formed in all of the major oceans in just five years.

The idea to remove the sea of its innumerable plastic waste was previously considered impossible, but Boyan seems confident that his system not only will remove the waste, but manage the whole process in five years.

Part of the problem is that the immense amount of waste that is currently in the ocean is dispersed over a huge area, so no images of the waste as a collective have ever been produced. Boyan’s system of floating booms forms an array that funnels in drifting plastic gradually, collecting a predicted 20 billion tons of plastic, making it entirely visible.

The array doesn’t just drag in plastic, it also rakes in a profit. The clean-up effort will bring in over $500 Million (US) in recyclable plastics each year. When Boyan and others realised that the concept would work they took a leap of faith and created a non-profit organisation he calls The Ocean Cleanup Foundation. This group will focus on the goal of developing his invention, raise funds for its manufacture and make it operational as soon as possible. His concept would save numerous aquatic species of fish and help reduce PCB and DDT contaniments that affect us all. Additionally, it operates on the power of the sun and by the oceans themselves.

Who Needs Sleep?

Eight hours of sleep a night is supposedly the ideal amount of rest for an individual. After eight hours you are appraently better placed to feel replenished and ready for the coming day. However, most people cannot find the time to sustain such a healthy sleeping pattern. In the academic and professional working worlds, work or study often take up more time than they ought.

This means any person can be stuck up at 3.00am finishing that Board paper or assignment, only to have to be awake in four hours to repeat the work/study cycle. For some, less sleep isn’t such a bad thing. The occasional person (less than 5% of the entire world’s population) is what is known as a ‘short sleeper’ - that is, they can operate efficiently on just four hours a night.

Many short sleepers use the extra awake time to work, but it isn’t always prime productivity time.

Darren Witmer, the chief executive officer (CEO) of a North Carolina-based business consultancy admits that his late night hours — he often works between 21:00 and 2:00 — aren’t as productive as when he works between 8:00 and 17:00. “I’m not at my peak performance at 2.00am,” he says “but even 50% productivity is better than nothing.”

These short sleepers do make up for their sleep loss eventually. Witmer sleeps for a nine-hour stretch several times a month. Lawyer Richard Bobholz, another four-hour sleeper, often takes 45-minute naps in the afternoon. “When I wake up I’m ready to go again,” Bobholz said.

Here’s hoping that Members in work and end-of-year study madness either can get the required eight hours or are amongst the 5% who can carry-on, albeit with some reduced performance!

UNIVERSITY SECTOR NEWS The Man of Modern Money Plastic banknotes have existed in Australia for 25 years now - this country being the first internationally to adopt the plastic note system. Since then, the world has accepted plastic currency as a smarter, stronger and generally better way of printing money. Before this, we relied on paper notes which were easily destroyed and much easier to forge than the plastic notes we use today.

In 1968, a young David Solomon was invited to make a better banknote - a note that was harder to forge. It wasn’t an easy road from there. David was faced with diverting a culture away from the negative view of plastic notes to a positive outlook on a safer future. As he recalls, the banks in particular, were against the idea: The bank thought plastic was cheap and nasty. They said a banknote had to feel like quality.

To avoid further conflict (and possibly also to counter such above claims), David devised a clever little experiment. Puting large bundles of paper money and plastic money behind a plywood board which had cut-out holes, he asked his willing money-grabbing subjects to reach through the hole and grab a handful of money. Not a single person could pick out the plastic test notes. “I had no more trouble after that.” he said.

Once all the approvals were in order, research began in earnest in the mid-70s. One of CSIRO’s sheds at their Port Melbourne site was turned into a secret production line, and from there, the research team made over 50 million test notes. Testing involved tearing, scrunching, folding and drenching these notes in beer, water, lipstick and even synthetic sweat. Because of the difficulty in live-field testing a banknote, they had to think of all possible scenarioes in advance. Professor Solomon’s team also developed a series of test notes in seven and three dollar denominations; and would often joke about going to a shop and asking the shopkeeper for three and four dollars worth of change.

Seen at Graduate House On Tuesday, 17th September, Cai Shen (Chinese: 財神; literally “God of Wealth”) aka resident member Johnny Ji., was in the house.

Johnny had just come back from his performance for the Chinese Philosophy and Business Club, of which he is a member. The act was just a small part of the many Festival of Nations activities held to raise funds in aid of children in rural parts of China to support their education.

Cai Shen is the Chinese God of prosperity, often invoked during the Chinese New Year celebrations and depicted with an iron tool capable of turning stone and iron into gold. It is said that you will be blessed with great wealth and good luck should Cai Shen pay you a visit. Fingers crossed our current good luck is thus bolstered.

Conference Delegate Accommodation Options

Accommodation outside the academic year, either late November to December, or in the early part of next year before semester begins.

Many conferences occur at the end and beginning of each year while the campus is less populated, when academics around the world are generally free from heavy teaching schedules and because Melbourne, Victoria, Australia is an ideal place to be for the Northern Hemisphere winter.

If you are a conference organiser, please consider recommending The Graduate Union as a college stay in the heart of the graduate precinct of The University of Melbourne that can cater for various budgets - from student (rooms with shared bathrooms) to professorial (self-contained apartments).

Since advertising this out-of-academic year offer, we have had a tremendous response, so to avoid disappointment, advise conference delegates to book well in advance. Delegates who are not members but attending a conference in Melbourne, can choose to stay for a maximum of seven nights.

If you have graduated from any university in the world, you are eligible to become a member and may then enjoy lower accommodation rates and become part of The Graduate Union’s vibrant international collegium of graduates. The cost of becoming a member is recouped because members pay lower accommodation fees.

The Graduate Union welcomes delegates residing at our college while attending the 28th ACHPER International Conference.
