The Leptin-Obesity Connection by meroe Rabieifar, PharmD my Community Pharmacy

eptin. yet another soldier in the battle of the bulge. Let me just preface by saying this was very confusing to me when i began researching the subject. i thought we want more leptin to aid in appetite control. Then i found that most obese people have an overabundance of leptin (which makes sense since fat cells secrete leptin). So... we want less leptin? But we need leptin in order to stop eating? Which is it? is leptin good or bad? The answer is complicated.

Leptin is the hunger and energy regulator. in times when the body has adequate adipose (fat) tissue, leptin is secreted from fat cells and binds to leptin receptors to signal the brain that there is plenty of energy and turns off the hunger response. Conversely, when fat storages are depleted, there is a lack of leptin and the brain’s hunger signals are switched on. That is exactly why obese people have copious amounts of circulating leptin in their bodies. But even though they shouldn’t be hungry, they still are. The problem is either the leptin is not reaching the receptors, or the number of effective leptin receptors have diminished. The latter is known as leptin resistance. Since, for most obese people, leptin supplementation does not help, the most likely scenario is dulled leptin receptors. The cause is probably our nutrient-poor, highly processed, far-removed from nature diet. When leptin levels are chronically high, the receptors stop reacting to them. We already knew a poor diet wrecks insulin balance; now another hormone is jumping on the bandwagon. There are several things you can do to get your leptin to the normal levels needed to maintain weight and energy. First, avoid fructose, especially high fructose corn syrup (HFCS). Fructose actually appears to desensitize leptin receptors so even when leptin is present, the receptors aren’t binding. in addition, fructose increases triglyceride levels and triglycerides physically prevent

leptin from crossing over to the brain. The result is the hunger mechanism stays switched on. Make sure you’re reading those nutrition labels!

Next, avoid grains. Besides being high in carbs and leading to insulin resistance and diabetes, grains contain high levels of peptides called lectins that are thought to contribute to leptin resistance. Some lectins actually bind to leptin receptors, thus preventing leptin from doing its job. Lectins are also present in most other foods, like beans, soy, dairy, eggs, and even meat. Soaking, fermenting and cooking appear to reduce the concentration of lectin dramatically. When possible, soak your beans for 24 hours prior to cooking, soak raw nuts for 12 hours (then refrigerate if not consumed right away) and stick to fermented dairy and soy products.

Get plenty of good sleep. Sleep appears to increase leptin levels significantly. Since stress produces cortisol, avoiding stress is key when trying to regulate sleep. High cortisol prevents the sleep hormone, melatonin, from doing its job. One way to beat cortisol is to take melatonin supplements. Melatonin supplementation has been shown to be safe in adults at hight doses. Generally, 1mg to 3mg taken before bedtime produces good results in most people, although higher doses are possible. if you have questions regarding melatonin supplementation, stop by my Community

One last thing to avoid is ultra-low-calorie diets. Just like eating too much throws off your balance, so does eating too little. eventually, your hormones will win out and you will eat. Constant dieting has been linked to lowered leptin production.

When battling weight or energy issues, it’s best keep the big picture in mind. Something that might produce immediate results (severe caloric restriction) may end up hurting you in the long run (decreased leptin levels). Be kind to your body by avoiding foods that are harmful, eating adequate amounts of nutritious foods and getting plenty of sleep. Love and respect your body and, trust me, your body will love you for it!

The co-dependency, the denial. the low selfesteem, the compliance and the control are some of the many patterns that result from being hyper-vigilant, which is a characteristic of a trauma survivor. Family members can carry on the cycles of dependency for generations. Most family members believe that treatment will cure them because this is a disease. They also believe that the only person who needs help is the client in treatment, and a lot of times they believe that they are responsible for the behavior and recovery of the person with the addiction. The family members are usually not aware that the recovery is a process, a path of self-discovery. As important as it is to help our clients, it is also important to provide support to

the families, since they are an extension of the client support, in most cases. Our ideal scenario is to facilitate the opportunity for the client to see that however the event took place, the existing situation for the caregiver was also an important piece in understanding the reasons why certain families operate in certain ways. What To Feed Your Pet Good food fuels our engines and keeps our system running smoothly. The same is true for your pet. Knowing what to feed your pet is as important as knowing what to feed yourself. We’re inundated by claims and promises. Most pet owners, and there are 164 million of you in America, don’t know what to believe.

ogs and cats are carnivores and more and more pet care professionals advocate a raw, natural diet to be healthy. Switching an animal with existing health problems to a raw diet can often improve their conditions and help avoid some illnesses.

Feeding freeze dried raw (i feed this exclusively to my own pets!) can change the course of the life of your pet. i highly recommend a grain free diet. is it safe? Dogs and cats have very different digestive tracts and processes than we do. They’re shorter, so foods are processed quickly, before bacteria can multiply and cause problems. Also, they have a high level of acidity in their digestive systems which allows them to process raw meat and bones.

We can eat all the onion we want, but give onion to a dog, even a very small piece, and it can cause anemia. Dogs can die from a small amount of chocolate.

Get your pet food at a pet specialty store, rather than at the supermarket. each one has a list of ingredients on the package in order of the amounts contained. if corn is first, then that’s the primary ingredient. But corn has no redeemable nutritional value for pets. it’s useless filler and difficult to digest. All four of the top selling pet foods in the country today lists their primary ingredient as corn.

if substantial amounts of chicken, chicken livers and hearts, organic blueberries and cranberries, are included, then your pet is likely eating a healthy, protein-based diet. But if ingredients like these, as well as beef, fish, rabbit, duck, goose, venison or bison, rank lower on the list or not at all, it’s not a suitable diet.

Avoid foods containing meat-by-product and animal fat. Wheat can cause of allergies in dogs. Sugar cane molasses, corn syrup in any form, orbital, sucrose, fructose, glucose, and propylene glycol just add empty calories. Avoid Coloring (blue 2, Red 40, yellow 5, yellow 6, and Titanium Dioxide). your pet doesn’t care about the pretty colors and shapes and artificial coloring agents are known cancer causing agents, and also cause allergies.

Propylene Glycol is used in antifreeze and may be toxic and should definitely not be in a food an animal will eat daily for weeks, months or years. Hydrochloric Acid, used in kibbles n Bits, is a caustic chemical agent used in the production of chlorides, fertilizers, and dyes. Nutritional value should be your main guide when comparing pet foods. A higher quality of food will necessitate perhaps only ¾ of one cup to provide adequate nutrition. So, a 5lb bag of a higher quality food will take you further, and give you more fuel for your dollar.

Refusal to eat can be a serious concern in elderly or sick dogs. Once their fat reserves are

used up, their bodies will begin to burn protein for energy, metabolizing their own muscles. Many conditions, like kidney disease, heart disease, and cancer, cause decreased appetite. Prescription diets are available for certain health issues, but their bland taste and a pet’s lack of appetite is not a happy combination. Canned prescription-diet foods can be more appetizing if cut into slices and frozen or baked for about 10 minutes at 350 degrees F. you can even heat them briefly in the microwave. Nauseous dogs or cats may prefer cold food, which emits less scent. (Note: never feed a nauseous pet a diet you plan to continue in the future because your pet may associate it with feeling ill and consequently dislike it from then on!)

Some recommendations for finicky eaters are: Stella & Chewey’s Freeze Dried Raw, Primal Freeze Dried Raw. Canned food can be heated up for a few seconds in the microwave oven and the delicious warm aroma will make your pet a lot less finicky! Pets are compelled to eat as a direct result of their sense of smell. Grazing adds to the finicky nature. it’s a good habit to allow your pet access to food for 15 minutes or so, perhaps 30 minutes for senior pets. Then, pick up the uneaten meal, and put it away for the next meal, or dispose. Once your pet realize that the food not eaten is no longer available, they most likely will gobble it up when it is put down.

fter Olympic equestrian McLain Ward took a bad fall at the Surpass Grand Prix jumping competition, he learned that he had completely shattered his knee and would need orthopedic surgery. He was devastated. How could he recover in time to qualify to compete with the U.S. team at the London Olympics, just a few months away? Fortunately for him, he had been rushed to Wellington Regional Medical Center. While McLain was concerned about the future, his doctors were confident they could repair the damage and let McLain resume his career. After his surgery, McLain spent eight weeks on crutches and was restricted from putting any weight on his recovering knee. Although it seemed to be a very long eight weeks for him, McLain recovered and was back in the saddle by April.

Life-threatening Disorder or Tracy Bartow, Wellington Regional Medical Center saved her life and the life of her child. Sevens months into her pregnancy, she was diagnosed with preeclampsia, a life-threatening disorder. it is a rapid rise in blood pressure that can lead to seizure, stroke, multiple organ failure and death of the mother or baby. Tracy and her unborn baby were in trouble, so she and her husband went to Wellington Regional Medical Center. There, she learned that the only available option to save her baby was to induce labor right away. When baby kaylee was delivered, she stopped breathing and had to be rushed to the Neonatal intensive Care Unit NiCU.

Unfortunately, each one of us, when we reach the approximate age of 40, will lose the ability to focus up close. Gradually, we will need more magnification and we will need the readers more often as years go by. This is all due to a simple change that occurs to the lens in our eye that is responsible for focusing.

each eye has a built-in magnification lens in it. This lens is attached to muscles, which when contracted, change the shape and focusing power of the lens. When we are younger and want to read a book, the muscles in our eyes contract and change the shape of the lenses, which then brings the text into focus. When we look away from our book to see at a distance, the muscles relax and the lenses return to their former shape, bringing the distance into focus. As we age, the muscles in the eyes still contract, but when they do, the lenses do not respond as well, as they stiffen with age. The further away you hold your book, the less focusing power (and smaller change in lens shape) is required to see it. This is reason why many people can get by without readers if they hold reading material at arms length….. until their arms are too short!

ertussis, commonly known as whooping cough is a highly contagious bacterial infection. World- wide it affects almost 50 million people per year and is responsible for 300,000 deaths per year. it’s incidence in the U.S. is alarmingly increasing. This is secondary to waning immunity after vaccination, lack of booster vaccinations or refusal of vaccinations. More worrisome is the fact that the most vulnerable individuals in our society are extremely young infants. The overwhelming majority of severe disease and death is seen in infants less than 3 months of age. Unfortunately most mothers’ immunity is insufficient to help the infants fight this infection until they reach the age of starting the vaccine series for themselves. This is not because it is physiologically not possible but because mothers’ have not been properly vaccinated.

Symptoms of whooping cough start as a mild cough that progresses into severe recurrent coughing episodes which leads to the typical “whoop” which is a high pitched inspiratory gasp for air following these attacks. The patients could also experience fainting, vomiting and possible rupture of one of the lobes of the lung. incubation can last 421 days followed by the early mild stage that lasts 1-2 weeks followed by the more serious paroxysmal stage that can last for 2-3 weeks before entering the convalescence stage.

The primary prevention is vaccination but as stated earlier starting the vaccine series at age 2-3 months leaves infants vulnerable. The Center for Disease Control as well as the American College of Obstetrics and Gynecology have made many recommendations in the past to help decrease this disease process with little positive effect. in February 2013 The Advisory Committee for immunization Practices strongly recommended the Tdap vaccination to be given to all pregnant women between 24 -36 weeks gestation. it is safe to be administered at any time during the pregnancy and at last resort should be given postpartum. Furthermore, it is recommended that all household members and caregivers be immunized, minimally 2 weeks before exposure to the new infant. These recommendations are irrespective of previous immunizations because rapid decrease in immunologic protection occurs soon after the vaccine is received. This most recent recommendation is supported by both the CDC and ACOG. Tdap is an acellular vaccine that is safe in pregnancy and has been shown to have a much lower side effect profile than the older versions. The Tdap is a vaccine that affords protection against Pertusssis, Diptheria and Tetanus. it is natural for expectant mothers to be weary of receiving any medications or vaccinations while they are pregnant. The safety profile of Tdap has been proven and the benefit of protecting the newborn is without question. During your pregnancy you should be made aware of these recommendations by your physician and a full discussion regarding the benefits will likely help the expectant mother be more comfortable with her final decision. n Saturday, August 17, 2013 Melia Morgan, inc in association with Terrible Mountain Productions and Borike Productions had the honor of celebrating “One Moment the Web Series” zumbathon Fundraiser which was sponsored by BANzAi Wellness Magazine, A Healthy Fit and The zoo Gym in Lake Worth FL. The event catered to a Palm Beach County web series production called “One Moment” that will be released towards November/December 2013. This production will help a select few of our South Florida Actors in becoming SAG eligible and will be donating all the generous gift cards from our sponsors to A Healthy Fit owned by Cynthia Smith who volunteers her time in participating and creating zumbathon events for a Cause. How do Phone and Skype Readings Work? i am often asked how phone and skype readings work compared to when the medium and client are physically in the same room. it is really quite simple. A medium's work is energy-based. it is the client's deceased loved one who is transmitting information to the medium. is this spirit physically present in any type of reading? No. The deceased is in spiritual energy form. Therefore, the spirit is not bound by time and space as we are in the physical world. This makes it possible for the medium to pick up the vibration of your departed's spirit anytime and anyplace after the client has made the request for the reading, provided the “spirits” are willing or able to lower their vibration to the medium's. in technical terms the medium raises her energetic vibration to a point where the two energy bodies connect. Understandably, this is a strange concept to most people; however, it is the best description of what takes place during any type of mediumistic experience.

During the session by phone or by skype, the medium receives information from the spirits that are connected to the client by the bond of love. Therefore, the spirit communicator may be a blood relative, spouse, a close friend, or someone whose life was touched by the client who wants to come through and acknowledge that help or love that was given to him or her. 3D Mammography

“The addition of 3D Mammography (tomosynthesis) to conventional digital mammography results in a 30 percent reduction in the overall recall rate”

Breast tomosynthesis, also known as 3D mammography was approved by the FDA on February 11, 2011. Breast tomosynthesis minimizes the effect of overlapping breast tissue by having the X-ray tube and detector moving in tandem, acquiring the images at different angles. These images are then combined to create a three dimensional picture that can be manipulated by the radiologist when reviewing the mammograms. At the current time, the tomosynthesis images are acquired at the same time as the conventional 2D mammograms. Using conventional mammography, about 10 - 15% of women are called back for additional imaging. Tomosynthesis reduces the need for many of these call backs, because the initial studies with tomosythesis will be free of overlapping structures. Another benefit is the improved imaging in women with mammographically dense breasts, because of the elimination of viewing the overlying tissues above and below the areas of interest. Because of the 3D information included in obtaining the tomosynthesis images, the relative position of a suspicious area in the breast can be more precisely identified in all three planes. A recent large study of tomosynthesis was published online July 30, 2013 in Radiology by Brian M. Haas, M.D. and colleagues, from the yale University School of Medicine in New Haven, Connecticut. The addition of tomosynthesis to conventional digital mammography resulted in a 30 percent reduction in the overall recall rate. "All age groups and breast densities had reduced risk for recall in the tomosynthesis group," Dr. Haas said. "Women with dense breasts and those younger than age 50 particularly benefited from tomosynthesis.” Lower recall rates help reduce patient anxiety and also reduce costs from additional diagnostic examinations, Dr. Haas said. Tomosynthesis has one significant drawback: a radiation dose approximately double that of digital mammography alone. However, Dr. Haas noted that new technology approved by the U.S. Food and Drug Administration could reduce the dose.

A physical, chemical, or emotional factor that causes bodily or mental tension and may be a factor in disease causation.(This definition was taken from Merriam-Webster Dictionary). local Eight-Year-Old Stella martinelli to Debut Her Animated Short film at the l-Dub film festival - lake Worth Playhouse’s Stonzek Studio Theater Most participants in the festival will include adult-age directors and producers plus a handful of high school students. Stella Martinelli will be representing a younger iPhone savvy generation, as the only 3rd grader whose short film was selected. As with all of the participating directors and producers, she will participate in a brief question and answer session after the screening of her video.

“Stella amazes me,” said her mother krista Martinelli. “Recently our family was going through an emotionally rough time. We had just received the sad news of a death in the family. During that time, three out of four of us had bronchitis, and Stella was the only healthy one. Over a period of about a week, i noticed that Stella was taking a lot of photos of her American Girl dolls, but honestly i was quite consumed with other things and didn’t really know what she was up to.” Later, it was discovered that during this time Stella was taking hundreds of photos of her dolls and was on a mission to create a short film. Film Director for the Lake Worth Playhouse Charlie Birnbaum says, “These days when most animation is computer generated, it’s most refreshing to see filmmakers use the time-consuming, more personal techniques that stem from almost 100 years of great animation work. earlier this year we showed ‘Consuming Spirits’ a 2-hour animated feature with 250,000 hand drawn cells by artist Chris Sullivan - now we are excited to be able screen a new work by Stella Martinelli.” “This 8-year-old filmmaker, the youngest entrant in four years of LDub film festivals, took her American Girl dolls and used the old method of stop-motion animation. This involves photographing a scene, making a slight change and photographing again and continuing in this vein. The resulting frames, when shown on a screen, clearly give the illusion of movement to the story telling. The epitome of this technique can be found in the classic Battle of the Skeleton army in Ray Harryhausen’s ‘Jason and the Argonauts.’” “i made an American Girl Doll movie. it took 501 photos to make,” Stella wrote in an email to a few family members and friends. “i hope you like it.” She loaded the photos onto a computer and set them to a slideshow using a timer. All of this was done without any assistance from anyone. The only part of the whole process that Stella needed a

little help with was putting the animated film to music. She wanted her “stop action” animation video to be accompanied by one of her favorite Taylor Swift songs. “My friend and neighbor katie (13-years-old) showed me how to do that part,” said Stella. And voila – that was the finishing step. Her first animated film was complete, and her approximately 4-minute music video was launched. in the past month since the first film, Ms. Martinelli has completed five other short films, using either dolls or Playmobil® people. She also enjoys making music videos with her friends in the neighborhood, utilizing special effects and dancing. Stella Martinelli goes to a public school in Lake Worth and is quite fond of her teachers. in her off time, she enjoys playing ukulele, participating in dance classes and being a Girl Scout. evenings at home include time with younger brother Paul, and her parents Joe and krista Martinelli.

“it must be arthritis.” Many patients that begin experiencing discomfort and problems with their hands, often attribute their newfound problems to arthritis. Although this is often a cause of hand pain without any particular injury, there are many other potential causes that could be treated in addition to arthritis in the many joints of the wrist, hand, and fingers. Our hands are a complex machine made up of numerous parts that need to work in unison for a fully functioning, pain-free hand. When just one of the many moving parts is altered, we can definitely feel the difference, and oftentimes have pain. in our everyday lives, almost every activity we do requires the use of our hands, and whether our hands work well directly affects our quality of life. My goal as a hand surgeon is for my patients to carry on with their normal life and normal activities without having to even think about their hands or wonder if they will have pain or be unable to do certain activities because problems with their hands limits them. Depending on the location of your pain, whether in the fingers, at the bottom of the thumb, in the wrist, or in the hand itself, a hand surgeon, with a complete medical evaluation is often able to pinpoint what is causing your problem, and important tests such as X-ray and MRi are oftentimes used to further solidify the diagnosis. issues such as carpal tunnel syndrome, often presenting as numbness and tingling in the hand, as well as trigger fingers, often presenting as soreness at the base of the finger with or without the finger locking up, can both cause different types of hand pain. Repetitive activities, such as golfing, tennis, knitting, exercising, woodworking, or simply being on the computer or your smart phone a lot could cause inflammation of the tendons (tendonitis) that move your joints, or the joints themselves causing, pain, swelling, and dysfunction of the hand, wrist, or fingers. in addition to carpal tunnel syndrome, trigger fingers, and tendonitis, hand pain may also be coming from other treatable areas of the body such as the neck, shoulder, or elbow. A detailed evaluation of your concerns oftentimes sheds light on the source of pain, in order to customize treatment to get you back to enjoying life. injury, or other soft tissue injury can occur from minimal trauma that you may or may not remember. Minor falls, lifting something too heavy, over working your body in the gym or at home taking care of the kids, could cause an injury that can be effectively treated in a shorter time frame by being evaluated by a qualified hand specialist instead of letting nature take its course. Finally, your hand or wrist pain could also be caused by arthritis, and this is diagnosed by careful examination and X-ray. Arthritis in the hand could be caused by multiple factors including previous injury, immunologic condition such as rheumatoid or psoriasis, or years of wear and tear. Depending on the cause, there are multiple avenues of potential treatment that are often simple and noninvasive. Medications, lifestyle modifications, diet changes, creams, and splints are a few of the treatments that may be used prior to any more invasive treatments such as injections and/or surgery. Whatever the cause of your pain, do not believe it is something you have to live with the rest of your life. There are readily available treatments and options that can be tailored to your particular problem, in hopes of getting you back to doing what you love to do without worrying about your hands.
