CHES'nursingsupportis availabletopatientswhowant self-directedservices(SDS). Theteamreviewshealthriskscreeningtools, developshealthcareprotocols,offerscommunity health-relatedresources,andestablisheslinesof communicationwithcareproviders.
CHESalsoappointsregisterednurses(RNs)whoconsultyouandyour
ThesepatientplansareregularlyreviewedbyCHES-theteam monitorsthequalityoftheservicesprovidedtodeliveroptimal care.
