Galleria Unico &_______________________________________________ (RENTAL PARTY) 1. It is agreed that Galleria Unico will rent their location to _______________________ (Rental Party) as determined by the event coordinator and the rental party. The fee paid by the rental party to rent Galleria Unico provides use of the building or specified floor for an eight-hour period. 2. The rental party agrees to discuss the arrival and departure times with the event coordinator. Galleria Unico agrees to allow the rental party to set up decorations the day before the event as long as the building is available. If the building is inaccessible the day prior to the event, set up and deliveries may be made as early as 8:00 am on day of the event with prior approval and appointment. The rental party agrees that events that run into the evening (such as weddings) will depart no later than 12:00 am unless arrangements are made to extend. Failure to leave at the agreed upon time will result in the retention by Galleria Unico of some or all of the security deposit. 3. The rental party agrees not to use any decoration that may or is likely to cause damage to venue, contents of venue, or property (i.e. tape, nails, screws, etc.). Extensive decorating must be discussed with event coordinator. The rental party agrees not to hang anything from the lights. They further agree that, No rice, confetti, or silly string will be used in the building or on the property; No birdseed will be thrown inside the venue and No bubble machines or bubbles will be used anywhere inside the building. 4. The rental party agrees that only candles that are enclosed in glass (i.e. hurricane or votives) will be used. Linens must cover the tables when candles are used. 5. The rental party agrees not to remove any works of art without permission from event coordinator. 6. The rental party agrees to use only those caterers that have been approved by Galleria Unico for use of the kitchen. Alcohol will only be allowed through approved, licensed caterers. 7. The rental party agrees to use one of the recommended caterers approved by Galleria Unico. If the chosen caterer is not a recommended caterer by Galleria Unico, a $300 fee will be added to the total charge. 8. Galleria Unico takes no responsibility for items left after a function. All items must be picked up at the end of the event or the following Monday morning by 11 am for Saturday events. 9. The rental party agrees to bag all garbage and dispose of waste in the rear alley dumpster outside the back door. 10. The rental party agrees not to enter the kitchen for any purpose unless agreed upon by management. 11. Galleria Unico is a non-smoking facility. Guests smoking outside the building must bring an outdoor ash urn. If the rental party wishes to provide additional urns they must first get the event planners approval. The rental party will be responsible for ensuring that the guests do not drop cigarettes, cigars, or tobacco, on property, and for the clean up of any tobacco products. Failure to do so will result in the loss of some or all of the security deposit. 12. Chairs & tables owned by Galleria Unico may be used only inside the building an additional fee will be added unless agreed upon by management.


13. All services, including, florists, bands, and photographers must have workman’s compensation and full liability coverage. 14.Galleria Unico can arrange for the rental of china, sliver, and glassware for up to 150 for an additional fee, otherwise, it is the responsibility of the rental party to make arrangements through one of the approved caterers. 15. Galleria Unico staff will set up for the event as long as the description of the floor plan is provided two weeks prior to the event. Tables may not be drug across the floor; they must be picked up when moved. Galleria Unico staff will set up initially for the event without an additional charge. Galleria Unico agrees to be responsible for ‘tear’ down of tables and chairs. 16. The rental party agrees that only decorations that have been approved by Galleria Unico will be installed. The rental party’s plans must be submitted at least two weeks prior to the event. 17. The rental party agrees to conduct all activities on the property in compliance with all applicable laws. Galleria Unico may designate certain areas as "off limits" on the event date. It is the rental party’s responsibility to insure their guests uphold these restrictions. 18. Galleria Unico will monitor volume of music played, and may require a reduction in volume if it will cause a disturbance to neighbors. 19. Galleria Unico can provide attendants to help with parking of the guests for an additional fee. 20. Galleria Unico will provide a host for the event. Galleria Unico’s host will check on the rental party periodically to replenish restroom paper supplies, check garbage, and assist with other requests within reason. 21. Galleria Unico makes no representation that security will be adequate to prevent injury, loss or theft during your event. 22. Catering cleanup shall be completed within one hour of the scheduled conclusion of the event. The property, including all floors, walls, equipment, and grounds, must be cleaned (tables and table legs wiped, chairs and chair legs wiped, garbage cans wiped and garbage taken to outside rear docking area, debris and garbage inside and outside picked up and thrown away) and restored to the condition in which they were given to the rental party. Galleria Unico maintains the right to charge for the cost of completing any inadequate or incomplete work. 23. The Rental Party agrees to be responsible for and pay for any repairs needed due to damage done to the property during or in connection to the event or to Galleria Unico’s personal property. 24. By signing this agreement, the Rental Party agrees to all rules and regulations between the Rental Party and Galleria Unico. By signing this agreement, you acknowledge that you have read the "Guidelines for Caterers, Musicians, and Vendors" attached to this contract. 25. A security deposit is refundable, provided all contract specifications are met and no damages have occurred. 26. All payments must be made in cash, certified check, credit card, or bank check payable to the order of Galleria Unico. The security deposit of $_________ is due at the time of signing the contract to hold ___________________, 20____ for the date of your event. The full payment of $_________is due 10 days prior to your event. The security deposit is separate from the total rental.


27. Cancellation of the event will result in 100% loss of the deposit unless cancelled 60 days prior to the event. Galleria Unico may amend or supplement the rules and regulations at any time without giving prior notice of such amendment or supplement. Enforcement of this Agreement This agreement shall become effective between the parties when signed by each party in the space indicated below. It shall be constructed under the laws of the state of Illinois. In the event of any dispute between the parties, including but not limited to civil litigation, the parties agree that the prevailing party shall be entitled all costs incurred to enforce this contract, including but not limited to reasonable attorney’s fees, expert witnesses fees, disbursements and court costs, including appeal. _______________________________________ ____________________ Signature of Rental Party Date _______________________________________ _____________________ Galleria Unico Management Date

Galleria Unico Guidelines for Caterers, Musicians, and Vendors 1. The rental fee provides use of the Galleria Unico for the event, which will not include cleanup. Only approved caterers may use the kitchen unless cleared by the establishment. 2. A parking space is allowed with approval from the Event Planner for Caterer's Vehicle. 3. Setting up catering with the approved caterers is the responsibility of the Rental Party. Galleria Unico will provide the tables and chairs. Tableware is available for an additional fee. When our tableware is used, Galleria Unico will provide personnel to wash tableware and a charge for personnel & tableware rental will be additional. Caterer will bring tableware to dish area and have dishes "dish-machine ready," i.e. emptying glasses, scraping food from plates. 4. Music, entertainment and flowers are the responsibility of the Rental Party. Musicians are expected to adhere to volume-level restrictions determined by Galleria Unico staff. 5. Galleria Unico's Event Coordinator must approve all decorating. 6. Breakdown should occur immediately following function. Caterers and Vendors must remove everything they bring on premises at the conclusion of the event. Any area used by guest/vendor must be left as clean as it was found. Garbage may be left on premises in the event that Galleria Unico's garbage dumpster is full; however, it must be sorted as follows: garbage bagged and placed behind the rear door on the ground level beneath the stairs with cardboard boxes broken down and placed separately.
