A LETTER TO OUR COMMUNITY

"Supporting people with disabilities and empowering every ability" Since 1972, Foothills Gateway Inc. (FGI) has improved the lives of thousands of people with developmental disabilities and their families in Northern Colorado by helping to conquer challenges and achieve their maximum potential. In fiscal year 2011, nearly 1700 people with developmental disabilities and their families received service and support from FGI. FGI continues in the proud tradition of helping clients successfully contribute to their community at school, work, and home. In the not too distant past, many of our clients would have been warehoused in an institution, viewed as an economic and social burden, and lost to their families. Now, with the direct support of FGI, these same individuals are integral members of our community and their families. In 2011, individuals receiving services were productively employed with 71 community jobs by 62 local businesses. 707 infants and toddlers received critical therapies, evaluations, and service coordination to help them grow and learn. Two clients served as advocates on the Board of Directors of Foothills Gateway, providing valuable input into policy and the overall direction of the agency.

Sadly, the number of individuals who live in our community and are on the waiting list to receive services and currently receiving no services, other than intake case management, grew to 362 in 2011. We will continue to advocate with state officials to gain resources for individuals in need. Realizing State and Federal resources may be reduced in the future, the Foothills Gateway Foundation is working to increase the endowment to address the needs of our growing waiting list.

Fortunately, FGI continues to enjoy the generous support of Larimer County citizens. You make a difference through your contributions. As Federal and State budgets are cut, FGI will increasingly have to turn to volunteers and donors to help the individuals we serve. In 2011, $393,201 were received in private donations, $3,050,348 were received from the mill levy, and 61 volunteers donated 1,903 hours of their time and talents.

We thank you for your support and generosity. Found within these pages, you will discover how we used this support to benefit people with developmental disabilities and their families in Larimer County. We are proud of our accomplishments on behalf of the community, and we hope you will be too.

2011 FUNDRAISING & COMMUNITY OUTREACH

Foothills Gateway, Inc. has continued to be very fortunate to have the help of our support groups; the Family Support Services Council, the Foothills Gateway Rehabilitation Center Charitable Foundation, the Foothills Service League and the Fort Collins Service League; in helping to raise awareness of our programs and conduct local fundraisers on Foothills Gateway’s behalf. In addition to raising much needed additional funding for our programs, these groups also hosted several fun and social activities for the adult clients in our day and residential programs throughout 2011.

In 2011, Foothills Gateway, Inc. initiated monthly “Educational Luncheons”, inviting community members into our main facility to learn more about our services and programs. These luncheons provide community members with an opportunity to meet clients and see our activities in person.

Luncheons are geared toward community members understanding the wide variety of services and supports provided by Foothills Gateway because at Foothills Gateway we truly believe that by

We continue to keep in contact with our local legislators on issues of importance to the developmental disabilities field through our annual Legislative Forum and participating in several legislative functions throughout the year.

She always gets back to us and keeps in touch so we know what to expect during the year.” –FGI FAMILY SATISFACTION SURVEY Foothills Gateway, Inc. realizes that children show signs of special needs early on and that if those needs are attended to in the first few years of life, the outcome can be greatly improved. In 2011, over 700 families with infants and toddlers in Larimer County received Early Intervention services to help increase and support their full developmental potential. Early Intervention provides support and education to families of eligible infants and toddlers to help maximize their development. Families never experience a waiting list for Early Intervention and the program at Foothills Gateway, Inc. gets top marks in all state and federal compliance indicators.

FGI gives her a chance.” –FGI FAMILY SATISFACTION SURVEY In 2011, FGI’s Family Support Services Program (FSSP), through fundraising efforts of the FSSP Council, continued a collaborative effort with Respite Care, Inc. to host “Respitality” one to two afternoon/evenings per month September through May. “Respitality” is an afternoon/evening of free childcare for children (up to age 21) in FSSP (or on the waiting list for FSSP) and their siblings (up to age 12). Children who attend enjoy an afternoon or evening of fun filled activities while parents enjoy a much needed and deserved break in caregiving. Every child that attends is paired one-on-one with a volunteer buddy, for the entire afternoon or evening, who helps them complete each activity. Each group is led by a highly trained Respite Care, Inc. staff member.

Each “Respitality” session has a theme. During one session, “Dr. Suess Respitality” had activities centered around Dr. Suess books.

Attendance for each session ranged from 10-18 children in 2011. Last year, 120 different volunteers gave their time to meet the needs of “Respitality” totaling 458 hours of service! This amazing and low cost program is due to the community’s dedicated voluteerism and provides high impact to the families who attend. The FSSP Council plans to continue these fun and impactful events in 2012!

It’s all about me!” –GEOFF BAUER Foothills Gateway’s case management services continue to be an invaluable support to the nearly 1,700 individuals served through Foothills Gateway, Inc. Case management helps individuals served and family members put together the pieces of the complicated puzzle that make up services and supports for people in Larimer County. Case Managers know what resources are available and can connect people to the services they need. Foothills Gateway’s case management is a collaborative process of assessment, planning, facilitation, and advocacy for options to meet each individual’s needs. Every individual served by Foothills Gateway is assigned a Case Manager. Geoff Bauer (pictured right) meets with his Case Manager (Tim Priehs) on a regular basis and works with his Day Program Supervisor (Rowdy LeMasters) five days a week.

Three years ago, Morgan was living on the streets in Fort Collins. A year later she was incarcerated. The judge in Morgan’s case offered her one last opportunity to live in the community by receiving services through Foothills Gateway. Morgan agreed to the plea bargain and enrolled in services in June of 2010. Since that time, she has been successful in integrating back into the community and has even been giving back to her community by volunteering. With the coordination and support of her Case Manager, Morgan attends community AA meetings and receives support from her team including Larimer Center for Mental Health and The Otero Corporation. Morgan’s success in maintaining relationships is helping her work toward changing the outcome of her life and recognizing that there is a different path.

2011 FOOTHILLS GATEWAY, INC. SUPPORTED EMPLOYMENT

Not everyone excels at the same things. If we did, the world would be an incredibly boring place. Each individual has the ability to make amazing contributions – to the community, society, and the world. Some people need a little more encouragement and support to make the most of their abilities. For nearly 40 years, Foothills Gateway has empowered the abilities that make us unique.

In July of 2009, Tina Meyers was looking for employment with the help of Division for Vocational Rehabilitation (DVR). At that same time, the City of Fort Collins’ Transfort Bus System needed to fill an unusual job for a “Ride Checker”. With support from DVR and Supported Employment staff at Foothills Gateway, Inc., Tina demonstrated that she had the skills to perform the duties required as Ride Checker. The staff at Transfort offered her the job.

Tina has a one-of-a-kind job, gathering ridership information which estimates the number of riders on each route. She is responsible for checking eighteen routes with a varying schedule everyday. Her job is to keep track of everyone getting on and off the bus. Tina has mastered the use of the technology required to record the data which ultimately determines funding and how routes are scheduled.

When Tina started this job, it was apparent that an efficient method was needed to enable her to plan her own transportation to and from the start and end points of ride check routes. A computer was purchased with internet connectivity which allows for the creation of schedules that interact with support staff. With the help of her Job Coach (Jim Cox), she now uses her computer at home to plan how she will get from point A to point B. Tina admits, “Without my job coach, I wouldn’t know what to do!” Foothills Gateway’s Supported Employment program is proud to provide job support to Tina, as well as 62 other individuals, on a consistent basis because communities grow stronger when everyone lives up to their maximum potential!

Foothills Gateway’s Supported Employment Program provides clients and employers with customized job matching, on-site job coaching, and long term job support and is committed to empowering each individual’s unique abilities.

Foothills Gateway’s Residential Program provides a variety of housing opportunities and works with each client to provide the right living situation to promote a sense of community involvement and improve lives.

Foothills Gateway’s Adult Care Services Program provides short term relief to caregivers from daily care giving responsibilities in a home-like setting.

This service ensures that both family members and clients receive the support they need while strengthening families.

2011 FOOTHILLS GATEWAY, INC. DIRECT SERVICES

Foothills Gateway, Inc. has a long history of improving lives and independence through Residential and Specialized Habilitation. One example of just that type of success is Jenna Bishop. Jenna faced many significant hardships early on beginning with being removed from her family home at age 5 and subsequently living in several different foster homes, finally being institutionalized. Through these experiences and transitions, Jenna developed many highly challenging behaviors.

In 2006, a team of Foothills Gateway, Inc. staff was presented with Jenna’s story and immediately created a plan to successfully get Jenna out of the state institutional setting and into a local community setting. Foothills Gateway’s Residential Program provided Jenna with continual structured programming and stable living environments that met her current needs and were adjusted as those needs changed. With the constant support Foothills Gateway has provided, Jenna has overcome her challenging behaviors and enjoys a renewed sense of safety and stability. Jenna is now living in a Host Home setting with a family that has opened their home to her and she can truly avail herself of opportunities within the community.

Through the services and empowerment Foothills Gateway provides, Jenna has gained independence and self respect. Jenna participates in a multitude of community activities, has improved her health and continues to thrive as she reports that she is “the happiest she has ever been in her life”. Jenna has dreams of working in the community in the near future and Foothills Gateway will continue to support her in empowering the abilities that make Jenna unique; because everyone has unique abilities – and everyone is worth celebrating!

Foothills Gateway’s Early Intervention Services provides families with children birth through age 2 years a wide variety of therapies and specialized supports from which to choose.

These services help to increase and support their full developmental potential.

Foothills Gateway’s Day Habilitation programs focus on increasing self sufficiency and/or basic skills to enable individuals to achieve an increased level of independence. Individuals can receive these services as part of their Supported Living or Comprehensive Services plan.

Foothills Gateway’s Transportation services provide transportation between home and program or work settings and are developed to best fit the needs and abilities of each individual.

Whenever possible and feasible, existing community transportation options are used.

F I S C A L Y E A R F I N AN C I A L R E V I E W Foothills Gateway, Inc. has again expanded the provision of services to an increasing number of clients, both by delivering direct services and by contracting for services with 28 Program Approved Service Agencies, 2 Micro-boards, along with approximately 200 Independent Contractors and 50 other Vendors.

The year brought additional State funding decreases, with the largest percentage decrease affecting the Family Support Services Program, due to the economic conditions that continue to negatively affect Colorado State revenues. Despite this, the organization has worked to supplement these funds, enabling it to lessen the negative impact and to continue providing outstanding client services for all programs.
