How to Use the Directory
Introduction
The Directory is in four parts: An Introduction Section showing how the information in the Directory is organized, a Reference Section of healthcare-related support organizations, a Physician Section of all practicing physicians in the state, and a Healthcare Resource Section of organizations delivering health care within the state.
INTRODUCTION SECTION The introduction section consists of a Listing Guide and Code Tables. Listing Guide describes the Physician and Healthcare Resource Sections of the Directory, including the format for individual listings. Code Tables are a listing of the various codes used in the Directory and the complete name associated with the code. There are six (6) Code Tables: Healthcare Facility Type, Language Spoken, Medical Specialty, Medical Schools, Hospitals, and HMOs. U.S. and Canadian medical schools appear at the beginning of the table (4 digit code) followed by other foreign medical schools alphabetically by country and school (5 digit code). REFERENCE Federal Health-Related Agencies-State and Regional Offices lists the addresses, telephone and fax numbers, and e-mail addresses of federal agencies in each state and regional office dealing with health-related issues. State Health-Related Agencies lists the addresses, telephone and fax numbers, e-mail addresses and websites of state agencies dealing with health-related issues. National Physician Organizations contains name, address, telephone and fax numbers, e-mail addresses and websites of physician organizations at the national level. State and Regional Physician Organizations lists name, address, telephone and fax numbers, e-mail addresses and websites of physician organizations at the state and regional level. National Medical Associations-State Offices lists name, address, telephone and fax numbers, e-mail addresses and websites of medical organizations and medical foundations at the state level. Medical Societies lists the name, address, telephone and fax numbers, e-mail addresses and websites for the state and regional/county medical society organization within the state (where applicable).
PHYSICIANS Physicians are listed in three Sections: Section 1, Alphabetically by Name. This section lists physicians in alphabetical order. Section 2, Alphabetically by Town. This section lists physicians alphabetically by town and then alphabetically by name for quick reference. The page number is then given for the complete physician listing in Section 1. Section 3, Alphabetically by Medical Specialty and Town. This section lists physicians first alphabetically by medical specialty, then by town and finally by name for easy referencing. The page number is then given for the complete physician listing in Section 1.
HEALTHCARE RESOURCES Healthcare Resources are listed in four Sections: Section 4, Hospitals lists the name, address, telephone, fax, chief operating officer, chief medical officer, bed size, description, website and e-mail address of each hospital in the state. Section 5, Nursing Homes lists the name, address, telephone, fax, administrator, bed size, website, and e-mail address of each nursing home in the state. Section 6, Group Practices and Clinics is an alphabetical listing of group practices and ambulatory care centers that also lists the physician roster at the location. Section 7, Healthcare Resources is a summary of all healthcare resources, including hospitals, nursing homes, managed care organizations, ambulatory care centers, and group practices listed alphabetically by town.
