Letter From The Publisher Greetings, Happy 2013! New Year’s Resolutions are back! January is a time for reflection on the past year and a desire to change in the new year. Community Cares Olivia Grace Armand Foundation
Olivia Grace Armand, a.k.a. “Cookie” was born with a very rare metabolic storage disorder called Mucolipidosis II or “I Cell”. She, as well as her brother, Mikey (age 6 1/2), were born missing the Lysosomal enzyme which is responsible for breaking down complex carbohydrates in the cells. It is a rare disorder with unfortunately an average life expectancy of 5 years. Throughout 2006, Trish and her family spent a lot of time at the Miami Children’s Hospital. Olivia received a bone marrow transplant while Trish was pregnant with Mikey, and twenty-three days later she welcomed her brother to the world! Only a week after his birth, Mikey was diagnosed with I-Cell as well and three months later he was back at hospital for his bone marrow transplant. While spending six months in a very confined area due to compromised immune systems, Olivia and Mikey’s mom, Trish Armand really got to know the nurses that cared for her children. The nurses not only provided medical care to her children, but they also provided emotional support to the family. Trish would often say “If I win Lotto, I am going to do a nursing scholarship in the kids’ names”. To this day, Trish is still in contact with the nurses that cared for Olivia and Mikey.

The new year is here and PB Parenting wants to make 2013 your healthiest! We are busy parents too and know how hard it is to keep up with the kids, run errands, and possibly work too. It is hard to fit exercise and healthy eating into the equation too. We are guilty of grabbing something quick through the drive-thru or putting off working out until tomorrow but tomorrow never comes. Here are some inspirational stories from local moms just like you and how they got fit and healthy.

Tina Marrs, 37 years old of Lake Worth Lost 42 lbs I’m a mom of 4 boys and I gained about 4060 lbs with each one and never got down to my “pre baby weight”. After my 4th son and the start of a new year I was DETERMINED to lose the weight. I also gave myself a goal of running my first 5K. I joined Weight Watchers and started to work out with Moms FIT 4 Fun 2-3 times a week. I also started to run about twice a week. With the help of Weight Watchers I learned portion control and to make better choices of what I was eating. I did whole wheat breads and pastas instead of white breads. During my pregnancy with my first child I had gained more weight than I had anticipated...almost 50lbs. I started jogging at 4 weeks postpartum. I didn’t change my diet at that point until my son started showing signs of food allergies around 2 months old (I was breastfeeding). I cut out all gluten, dairy, nuts and eggs, but decided instead of reading through every ingredients list I would start my own form of paleo diet/clean eating. I am now 60lbs lighter than I was at my last appointment at 40 weeks pregnant. I work out with Moms FIT 4 Fun 2-3 days/week along with running about 10-15 miles/week, and continue my clean eating. I have since re-introduced dairy, eggs and nuts, but still do not eat any processed foods, breads or pastas. One thing to keep in mind is to not see it as a diet...it is a lifestyle change!

This winter: What had happened to me? I remember the Tuesdays: Passport to the World day when I went to look for a toy under the ages 6 and up. When Should You Visit a Pediatric Urgent Care Center By Dr. Sandra Rivera-Luciano The evening’s homework is nearly done, when suddenly your son’s asthma begins to shift into high gear. Or a weekend trip to the park comes to a quick and unhappy end when your daughter tumbles from her bike, leaving her with “road burn” and a few deep cuts and bruises. Over time, most moms and dads have opportunities like these to earn their parenting stripes, arranging treatment for children with minor illnesses and injuries. Now parents have a trusted partner in meeting their child’s after-hours medical needs. Miami Children’s Hospital now offers a new facility at the Miami Children’s Hospital Nicklaus Outpatient Center in Palm Beach Gardens. When to Bring Your Child to an Urgent Care Center? Pediatric urgent care centers are designed to provide after-hours care for minor, nonlife-threatening illnesses and injuries for children and teens up to 21 years of age who require medical attention. If your child’s illness or injury is life-threatening, call 911 or proceed to the nearest hospital emergency room/department. Otherwise, here is a list of some conditions for which you may want to seek treatment in an urgent care setting: • • • • • • • •
Allergies Asthma attacks (minor) Burns (minor) Colds, coughs, and sore throats Cuts and bruises Dog/animal bites (mild to moderate severity) Earaches and ear infections Fever in children more than 2 months old

Common Pediatric Illness with Dr. Shannon Fox-Levine, M.D., F.A.A.P
While we take all of the necessary precautions, during the winter months, we see a peak of illnesses. Although the weather certainly is not as it is in other parts of the country, we need to be aware of the most common illnesses and how to proactively treat these symptoms. Life can be somewhat unexpected, so please always look to your pediatrician if you feel that something out of the ordinary is going on. One of Palms West Hospital staff pediatricians, Dr. Shannon Fox, M.D., F.A.A.P. is here to provide a guide on the most common things we are asked in the New Year. A fever…When should you worry? The important thing to remember is that your child’s brain is not going to “fry” from a fever that is in response to illness. Some viruses, like influenza, can even cause fever up to 106F. Serious illnesses can occur without very high temperatures. When should you look to the doctor or make an ER trip: • •

Any infant under 3 months of age with rectal temperature over 100.4F Any child 3 months to 3 years old with a temperature over 102.5F; or over 3 years of age, any temp higher than 103F Any child over 3 months with a fever for more than 48 hours with no other symptoms Any child with a temperature over 101F for greater than 5 days Any child with fever, headache, neck pain, rash, dry lips, decreased urine, and lethargy

The dreaded cough… How long is too long? Much like fever, a cough is often your child’s protective friend. Most coughs in children are caused by the drainage of mucous from behind the nose, down the back of the throat. This causes a cough in order to keep the mucous out of the lungs. Cold and cough medications are not approved for any child under the age of 4. In older children, a cough suppressant may be used at night to aid in sleeping. A cool mist vaporizer in the room and elevating the head are supportive techniques to make your child more comfortable. It is expected that a cough from a virus could last on average 7-10 days. It is a good rule of thumb to bring your child into the pediatrician if the cough lasts for more than two weeks, or if your child has fever, wheezing or difficulty breathing. Vomiting & diarrhea…What should you do for your child? Most gastroenteritis is caused by a virus; however, some is caused by food poisoning. For children, we do not recommend any over-the-counter medications for these symptoms. The concern with vomiting and diarrhea is dehydration. The overall goal is to get your child back into a regular diet as soon as possible. When vomiting begins, do not give your child anything by mouth for an hour or two. Begin to wean back with sips of water or an electrolyte replacement fluid such as Gatorade (for children over two years) or Pedialyte (under two) in small doses. As the vomiting decreases, you can increase the volume that you are giving.
Once your child is able to tolerate fluids without vomiting, you can advance to the BRAT diet: Bananas, Rice, Applesauce or Apples (NOT apple juice if diarrhea is present) and Toast. Don’t believe the myth to avoid dairy. Breast milk and formula are important to restart in children under the age of 1 since they need the calories to heal. Soy may be a better choice if diarrhea is present for more than 3 or 4 days. Yogurt is also an excellent choice, adding the good bacteria back into the intestine. Trust your instincts. If your child is exhibiting behavior that is not normal, watch and follow these guidelines. But always contact your pediatrician if in doubt. Our focus is to keep your children healthy, so feel comfortable to ask any questions to accomplish just that. Exercising Young Minds
BY Dr. Stephanie Lippman, Psy.D. Regular exercise not only has important benefits for the body, it has also been shown to have positive effects on the mind. In addition to helping people control weight and combat chronic diseases like high blood pressure and diabetes, exercise can improve stress management, alleviate symptoms of chronic mental illness, increase cognitive and academic functioning, improve self-esteem, and help with character development. While most studies have classically focused on exercise’s positive effects for adults, children face many of the same social and emotional challenges as adults. Together these factors provide strong evidence that children greatly benefit from regular exercise. Stress has become recognized as a significant problem for children with new issues like standardized school testing and the increased social difficulties that come with bullying in school as well as through social media. Studies have shown that 30 minutes of aerobic exercise can significantly reduce muscle tension associated with stress for up to six hours. It has been observed that children, aged 4 through 17, who exercised regularly showed greater abilities to handle stress and reduce anxiety than children who did

not engage in regular physical activity. Exercise has the ability to act as a diversion from various factors in life that are associated with everyday life stressors. Rates of depression, anxiety, and attention deficit/hyperactivity disorder (ADHD) in children are dramatically on the rise. Five to eight times as many high school students are meeting the criteria for a diagnosis of major depression and/or an anxiety disorder than was true several decades ago. Additionally, the rates suggest that nearly 5.4 million children have been diagnosed with ADHD. Recent studies have shown that a regular exercise routine improved overall depression scores for children and adolescents with mood difficulties. While the exact reason exercise helps is not yet fully known, it is believed that exercise alters the levels of certain neurotransmitters including noradrenaline, serotonin, and dopamine, which are associated with depression and anxiety. A study focused on ADHD showed that a regular jogging routine established over 10-12 weeks can significantly reduce the need for stimulant medication in children diagnosed with the disorder. It is believed that the increase in motor activity stimulates the effect of the medicine, providing an alternative way to combat inattention and hyperactivity, while also promoting positive effects for the body.
While exercise does not necessarily have direct influences on intelligence, it does seem to improve academic performance. Researchers at Dartmouth University have suggested that exercising during periods of brain growth, like childhood and adolescence, changes the brain. Specifically, exercise appears to stimulate the birth of new neurons in areas of the brain responsible for supporting learning and memory. Forty minutes of exercise has been shown to improve executive functioning, which is the ability to pay attention, plan, and stay focused despite the presence of distractions. There have also been improvements reported in short-term memory, resulting in improved verbal reasoning and math skills. Healthy self-esteem is an important defense for all children against the challenges of the world. For obese children, however, the development of good self-esteem is even more important because of an increased likelihood of rejection by peers. The physical advantage of weight loss associated with exercise is certainly a good reason to encourage activity in children. However, other important lessons can be learned from developing and sticking to a regular exercise routine. These include the pride of reaching goals, feeling more competent at various activities, achieving mastery through practice, and increasing one’s social desirability. Remember, however, that exercise should be fun. It is important to find forms of exercise your child will really enjoy to encourage participation and limit rebellion. health and wellbeing. Athletic competition may become dangerous when the contest becomes linked to winning, self-worth, and personal integrity. Players with good self-esteem and higher performance expectations are less at risk for developing stress in competitive situations. It is vital to explore these traits with your children and seek help for them if you find they are lacking in self-confidence or set too many expectations for themselves. While structured activities like team sports and dance lessons are great options, other physical activities to consider are playing hide-and-seek, tag, taking nature walks, roller-skating, and bikeriding. Physical activity through play has the potential for personal growth in areas such as persistence, deeper selfreliance, commitment, and motivation. Because play is so important for the healthy development of children as well,
combining physical activity with play is a way to meet two of your children’s most important needs at the same time. December is a difficult month for Trish with the anniversary of Olivia’s passing, her birthday and then Christmas -- December will never be the same. But giving $2,500 scholarships in Olivia’s name to pediatric nurses has been helping Trish with her grief. Sadly, Mickey is not doing well, his health is deteriorating. He was born with no heart issues but now 6 years later he has 4 leaks in his valves. Trish tries not to think of what life will be without Mikey. It is hard to say if he has 2 days, 2 months or 2 years. She is grateful for the time with her children no matter how short. “My kids have taught me how strong I am, and taught many others not to take life for granted and live each day.” Trish is an advocate for bone marrow transplant as a method to enhance lives of I-Cell children and speaks to families affected all over the world. Besides working full time and caring for Mikey, her spare time is spent building the Foundation and bringing awareness of I-Cell. Her ultimate goal is for a cure for I-Cell to be found and to help families with medical costs relative to I-Cell that are not covered by insurance. How to Choose the Right Preschool for Your Child By Dr. Michele Borba Of course you want a great school for your child. How do you know which facility is the best one for your child? Observe a few and always observe when children are there. It will help you decide if it’s a place you want your child to spend part of their day. Here’s a few questions to ask yourself and the staff in making your final decision: 1. Does this seem like a place my child would like to be? Use your instinct on this one. Can you see your child fitting in and being comfortable in this environment? 2. What do you do when my child or other children are ill? Find out what the policy is when children are ill at the center. Is there a supervised location where they can be removed from the other children? 3. What is the daily schedule? There should be a consistent daily structure where children know what is expected. Is there a balance between physical activities and quieter ones. Watch the children. Are they doing the kinds of activities your child would enjoy doing? Make sure children are actively engaged in creative play. 4. What is the ratio between staff and children? It’s always best to have a smaller number of staff to children. You want to make sure your child is being closely watched. 5. Is the staff “child friendly?” Watch the interaction between the staff and children. Are they patient? Are they respectful

towards them? And most important do the children appear to enjoy the staff? The “child friendly” rule has always been the one I was the pickiest about when choosing a school for my own children. 6. How do you deal with aggressive children? Ask what their discipline approach is for inappropriate children’s behavior – especially for hitting or biting. Make sure they have a thought-out plan and you agree with their plan. 7. Are the children enjoying themselves? Do they appear to be happy and active? Is there a variety of activities that are ageappropriate for the children? 8. Will my child be safe here? Is it well gated? Are electrical sockets covered? Are fire extinguishers available? How well are they equipped to deal with accidents? Is the staff trained in CPR? Hopefully, there will never be a safety issue, but a good school makes sure that children’s safety is a primary focus. 9. Do the staff share the same values as I do? Their staff will be sharing their lives with your child, so you want them to hopefully share a few similar values. Watch to see if the staff models them.
