ABOUT FAIRCAPE HEALTH TOKAI ESTATE Faircape Health Tokai Estate provides restorative care and rehabilitation for those recovering from injury or surgery. Our care effectively bridges the gap between hospital and home thanks to our comprehensive, multidisciplinary approach. The 70 bed, step down facility is located within 500m of the M3 motorway close to the Blue Route Mall. On arrival, each patient is individually assessed by the Multidisciplinary Team (MDT). MDT meetings are held a few times per week to discuss the status of each patient. Rehabilitation goals are set and the appropriate team members are assigned to the patient’s care plan. *COVID-19 control and the management of risk is a priority for us. In light of the coronavirus pandemic, Faircape Health Tokai has implemented strict COVID-19 control policies and procedures.


FACILITY SERVICES Faircape Health Tokai Estate’s accommodation is specifically designed for patient comfort, and stress-free recovery. Our rooms are replete with amenities and features to create a healing environment. Our in-house catering service works in partnership with our staff dietitian to develop nutritionally balanced meals. Special dietary requirements are accommodated The facility is proud to offer the following services and amenities: • • • • • •

FACILITY ADMISSIONS The admissions process is as follows: • • • • •

Doctor to complete and email our sub-acute authorisation/patient referral form to casemanager@faircape.co.za Referral document sent to medical aid for authorisation (may take up to 72 hours) Private paying patients are also accepted, they will be furnished with a quote that needs to be settled on or before admission. Once authorisation is granted, Faircape staff will liaise with the doctor to confirm the patient’s transfer. The patient will be seen by our dedicated Patient Assessment Sister before admission to the facility to ensure appropriate continuity of care.


MEDICAL SERVICES On arrival at Tokai Healthcare, the Nursing Services team will admit the patient to the facility. The doctor will immediately assess the patient and refer him or her to the relevant MDT members. Specialised Medical Services include: • • • • •

Dialysis Palliative care Oncology care Transplant care Blood transfusions

NGT and PEG feeding Stoma care IV fluids and antibiotics Wound care X-ray facilities

Our doctors are on site 8am - 6pm, leading the MDT to ensure best rehabilitation outcomes.


DOCTOR SERVICES ROLE OF THE DOCTORS’ AT TOKAI HEALTH We have a team of very passionate and dedicated Family Physicians and General Practitioners who will look after patients during their stay at Faircape Tokai. Our role is supportive and we aim to ensure that patients are medically stable and able to get maximum benefit from rehabilitation. We focus mainly on acute conditions that immediately affect the rehabilitation progress, and we are in contact with hospital specialists to ensure continuity of care. All medication given while at our facility will be provided by our pharmacy and claimed from the patient’s medical aid hospital benefit. We don’t allow patients to use their own medication whilst with us. We have X-Ray facilities and are able to give intravenous fluids and antibiotics should patients require them. Patients need to follow up with their GP or specialist for chronic medications and conditions after discharge from Subacute. We don’t routinely investigate or treat chronic conditions during Subacute stay.

FREQUENCY OF VISITS BY DOCTOR Patients will be assessed by a doctor on the day of their arrival and doctors will ensure that they receive all necessary chronic and acute medication while at Faircape. Following admission, the doctor will see the patient daily Monday-Friday and over the weekend if needed.

LENGTH OF VISITS BY DOCTOR The admission consultation is generally 20 mins. The daily consults that follow are mostly brief and aimed at treating acute problems and ensuring medical stability.



REHABILITATION SERVICES OCCUPATIONAL THERAPY Tokai OT is our aligned occupational therapy practise. This team promotes participation and independence in meaningful occupations through restoration of ability and environmental adaptations. Patients are seen on a referral basis. We assist patients in the following areas: • • • • •

Self-care training and recommendations Upper limb rehabilitation and splinting Cognitive assessment and therapy Visual perception and sensory rehabilitation Home assessments and adaptations

Carer and family training Return to work recommendations Return to driving recommendations Recommendations on assistive devices

PHYSIOTHERAPY Tokai Physiotherapy Inc approaches rehabilitation with the same commitment to excellence so intrinsic to Faircape Health. We assist patients in the following areas: • • • •

Orthopaedic rehabilitation Aquatherapy Neurological rehabilitation Cardiac rehabilitation

Pre/Post surgical rehabilitation Pulmonary rehabilitation Oncological rehabilitation Long COVID rehabilitation


SPEECH THERAPY Our aligned speech and language therapist helps residents to acquire or reacquire the ability to communicate and swallow to the best of their ability on a referral basis as needed. Our team assists patient in the following areas: • •

Patients with Neurological conditions (ie: stroke, head injury, tumor or dementia) Palliative or end of life patients

We provide treatment of: • • • •

Language disorders following fallouts Swallowing disorders Motor speech disorders

Implementation of Alternative and augmentative communication where needed

Voice disorders Cognitive linguistic rehabilitation Collaboration with the dietician to ensure patients receive the best food in the appropriate consistency in order to meet their needs


SUPPORT SERVICES SOCIAL WORKER Our Discharge Planner’s role is to facilitate the safe discharge of patients from the sub-acute environment. Where possible, this includes engaging directly with the patient regarding their future care needs and corresponding plan for either returning home with home-based care, or transitioning to a long-term healthcare/residential facility. The Discharge Planner also liaises with the next-of-kin to develop an appropriate care plan as per the recommendations of the treatment team. The Discharge Planner offers emotional support to the patients and family members alike, when necessary and facilitates family meetings to ensure that the patient’s care needs are communicated to and understood by both the patient and their next-of-kin so that optimal recovery is achieved post discharge.

PSYCHOLOGIST Our aligned psychologist, when referred by our MDT, provides therapy using a variety of methodologies to alleviate mood and cognition related symptoms, as well as adapt to changes and strengthen resilience on a referral basis as needed.

DIETICIAN Our Dietician when referred by our MDT ensures that patients receive optimal nutrition to assist in speedy recovery and lasting health. Patients are seen on a referral basis for input on the following: • • •

Enteral feeding Oral Nutritional supplementation regimes • Meal plan adjustments to accommodate food

allergies, intolerances and other feeding challenges Nutritional counselling, guidelines and meal plans
