After 13 days of rehabilitation, they may be able to move around with little support, be able to eat on their own, and dress themselves with minimal assistance. Progress like this is life-changing. It allows patients to enjoy a better quality of life with more independence.

What Happens If You Go Straight Home?

Without the specialised care and support provided at our facility, patients may not make the same progress. Going straight home often means they don’t get the intensive therapy and assistance they need to improve quickly. This can lead to slower recovery, a higher risk of complications, and increased dependence on family members for everyday tasks.

At our facility, we focus on accelerating recovery and building independence, so patients don’t just go home—they go home stronger and more capable.

From Severe Dependence to Greater Independence in Just 13 Days!

At our sub-acute facility, we’re proud to help our patients make remarkable strides in their recovery journeys. Here’s what this means:

Starting Point - Severe Dependence: When patients first arrive, they often need a lot of help with everyday activities. This can include getting dressed, eating, or moving around. Patients will need assistance at the beginning of their stay at our facility.

• Transformation - Moderate Dependence: After just 13 days with our dedicated care and support, many patients become more independent. This means they need less help and can do more daily activities on their own. While they might still need some assistance, it is certainly much less than when they arrived at our facility.

Without rehabilitation after surgery, it is less likely that you will fully regain your functionality. Rehabilitation is vital for ensuring a successful recovery.

Faircape Health Tokai Rehabilitation provides restorative care and rehabilitation in a purpose-built facility for those recovering from injury or surgery.

Our team makes us different. Our integrated team of medical professionals, includes independent practices which collaborate seamlessly to deliver holistic healthcare.

Our facility includes a multidisciplinary team of on-site:

Dedicated practice of 6 full-time doctors, which includes 2 family physicians.

Nurses and Carers

12 Dedicated Physiotherapists and 7 Occupational Therapists

• Medication dispatched by our onsite Tokai Health Pharmacy

• Nutritional meals planned by our Group Dietitian

WHAT MAKES US DIFFERENT

Our rehabilitation mission is to drive goals and outcomes. We believe that setting clear goals and measuring outcomes are essential components of rehabilitation. This ensures the best possible care for our patients. Our rehabilitation programmes use the International Classification of Functioning, Disability, and Health (ICF) framework, Goal Attainment Scaling (GAS), and the Modified Barthel Index (MBI) to measure the outcomes of each patient.

The ICF Framework:

The framework was developed by the World Health Organisation and is a comprehensive model that helps us understand the impact of health conditions on an individual’s functioning and disability.

It encompasses three main components:

01 Body Functions and Structures:

We assess the physical and mental aspects of our patients’ health.

02 Activities and Participation:

We evaluate how these health conditions affect an individual’s daily life and participation in society.

03 Environmental Factors:

We consider the influence of the physical, social, and attitudinal environment on an individual’s functioning.

Goal Attainment Scaling (GAS):

Goal Attainment Scaling is a method used to set personalised rehabilitation goals with patients. Instead of relying on generic goals, GAS allows us to work with patients to define specific, achievable objectives that are meaningful to them. Each goal is assigned a numerical score, and we continually monitor progress. This approach ensures that our rehabilitation plans are patient-centred and outcome-driven.

Modified Barthel Index (MBI):

MBI is a widely recognised tool for measuring an individual’s level of functional independence in performing activities of daily living, such as bathing, dressing, and mobility. This index assists in assessing and tracking patients’ progress. Additionally, it can be used to quantify changes in a patient’s functional abilities over time.

While at Tokai Rehabilitation each patient;

• Is assessed by a Registered Nurse

Has a rehabilitation plan and goals developed by our multidisciplinary team

Has a discharge date and post discharge follow-up clinic date set

MEDICAL SERVICES

Faircape Medical is our aligned medical practice. The team of six doctors, including 2 family physicians, lead the interdisciplinary team, ensuring the best rehabilitation outcomes.

Our doctors play a crucial role in patient care, ensuring medical stability for effective rehabilitation. They provide a thorough consultation upon admission and oversee both chronic and acute medication management. Patients can expect daily doctor visits from Monday to Friday, with additional support over the weekend when necessary. Our medical team is available on-site from 8 am to 6 pm.*

*Any urgent after-hours calls to see patients will incur a fee.

Tokai Occupational Therapy is our aligned occupational therapy practice. The team promotes participation and independence in meaningful occupations through restoration of ability and environmental adaptations.*

Cognitive assessment and therapy

Visual perception and sensory rehabilitation

Recommendations on assistive devices

PHYSIOTHERAPY

Tokai Physiotherapy Inc is our dedicated physiotherapy practice. The team of physiotherapists approaches rehabilitation with the same commitment to excellence intrinsic to Faircape Health. Boasting a team of dynamic physiotherapists with special interests in different areas, they make a significant and positive impact on patient’s lives through care, healing, and empowerment.

Our physiotherapy special interest areas:

• Orthopaedic rehabilitation - Our orthopaedic physiotherapists have undergone rigorous training in orthopaedic-related surgeries and injuries. By working in close collaboration with orthopaedic surgeons, our patients receive a seamless continuum of care, resulting in a faster and more effective recovery.

• Neurological rehabilitation - Our neurology physio’s specialised knowledge allows them to provide cutting-edge care to individuals. By participating in prestigious training, including the Bobath course, they ensure that patients receive effective and personalised treatment.

• Cardiopulmonary rehabilitation - Our physio’s are trained through the American Association of Cardiovascular and Pulmonary Rehabilitation. They provide specialised care for heart and lung conditions. Advanced training equips our physio’s to assist patients who struggle with breathlessness and respiratory conditions. Providing specialised care for patients improves their overall quality of life and well-being.

• Amputee Rehabilitation - By working closely with prosthetists, our physio’s are able to maximise prosthetic rehabilitation after limb loss. Two of our physiotherapists specialise in amputee rehabilitation and work closely with our aligned practice, maximising prosthetic rehabilitation, and ensuring that our patients regain their mobility and independence.

• ICU Survivor Rehabilitation - Our ICU Survivor Rehabilitation strives to address physical impairments related to post-ICU care during rehabilitation. The team collaborates with key professionals, including Psychologists, Occupational Therapists, and Speech Therapists, to address the cognitive, emotional, and physical impairments that often follow an ICU stay.

Assessment Nurses

The first point of contact when a patient is scheduled to be transferred to our facility. They ensure that each patient’s journey is seamlessly guided to the next step of their medical care.

The Administrative Team

Managing medical aid claims and addressing inquiries, the team is a vital link connecting specialists, hospital, and patients, ensuring seamless operations.

Nurses

Providing care and assistance to patients, registered nurses ensure that needs are met with dignity and respect and that prescribed medications are managed appropriately.

Carers

Assisting patients with various activities of daily living, including hygiene and mobilisation, Carers promote and extend independence.

Pharmacists

The qualified team of pharmacists provide guidance on medication use and dosage, addressing complex patient needs.

Discharge Planner

A bridge between patient, family, and the healthcare team, our discharge planner promotes patient well-being and recovery while minimising the risk of readmission.

Rehabilitation Coordinator

Our Tokai Rehabilitation Coordinator facilitates seamless transitions after discharge. This includes running the follow up clinic and supporting patients on their recovery journey.

Dietitian

Contributing to achieving sustainable nutritional plans for rehabilitation patients, our dietitian implements healthy eating, weight management, nutrition, food service and menu planning. We use the Malnutrition Universal Screening Tool to assess new admissions for malnutrition risk, allowing early intervention and personalised support for your health journey.

Speech Therapist

Communication and swallowing techniques are attended to during rehabilitation with our speech therapists. They assist patients in improving their communication skills, articulation, and voice disorders.

Social Workers

Safeguarding and empowering patients in their daily lives by supporting them through changes, Social Workers ease the transition of care.

Psychologists

Perspectives Psychology Group is our aligned practice. They strengthen the mental health and well-being of those experiencing physical health challenges.

TOKAI HEALTH PHARMACY

Faircape Health Tokai is the only healthcare facility with a dedicated pharmacy onsite. Tokai Health Pharmacy strives to ensure that patients receive the correct medication in a secure and timely manner. Our pharmacy acts in accordance with ethical principles and standards of practice. The pharmacy team is committed to providing comprehensive patient care that goes beyond filling prescriptions.

Our pharmacy is registered as a Designated Service Provider with most medical aids. Medication is covered by the medical aid and billed under the hospital benefit while admitted to Faircape.

Please note, for safety reasons, the use of personal medication is prohibited. All patient medication will be issued by Faircape Health Pharmacy and administered by our staff. At Tokai Health Pharmacy we primarily use generic medications due to complying with medical aid formularies and creating a more cost-effective option for our patients.

What is generic medication?

Generic medications are copies of brand-name medication that are duly registered with the Medicines Control Council (MCC) of South Africa. Generics have exactly the same: dosage, intended use, effects, route of administration, and strength as the original medicine. The pharmacological effects of generics are therefore exactly the same as those of their brand-name counterparts. According to the Medicines and Related Substances Act, it is the responsibility of the pharmacist to supply a more cost-effective alternative when dispensing medication. On admission, all our patients are asked to consent to generic substitution of their medication during their hospital stay in order to provide a cost-effective service.

ACCOMMODATION & AMENITIES

Faircape Health Tokai Estate’s accommodation is specifically designed for comfort, and stress-free recovery. Patients are placed in a shared suite replete with amenities and features to create a healing environment. The cost of a private room is R1 500 per day.

From en-suite bathrooms, to assisted and wheelchair accessible showers. Faircape Health combines world-class healthcare provision with the comforts of premium hospitality.

In the event of an extension of stay at Tokai Healthcare, an additional cost of R300 will be charged to the patient’s account. A written motivation to the relevant medical aid will also need to be submitted.

We ensure patient safety and security while monitoring carers and nursing staff with CCTV cameras in all rooms, bathrooms and common areas. These cameras are monitored exclusively by authorized healthcare professionals and will only be accessed in the event of a concern, incident, or when necessary to ensure your well-being. We are fully committed to respecting your privacy and dignity at all times. This measure is in place solely for your protection.

A beauty and hairdresser treatment price list can be provided upon request.

*Kindly be advised that all our healthcare centres operate as cashless facilities.

COMMUNICATION

POLICY

FAMILY COMMUNICATION:

On admission, a patient’s next of kin/contact person will receive an email providing contact details of the administrative team. The team will relay messages to the rehabilitation team, which includes the doctor, physiotherapists, occupational therapist, speech therapist, discharge planner or rehabilitation coordinator.

Patients capable of providing feedback on their rehabilitation are requested to do so. The family will be contacted upon admission and thereafter only if the patient becomes medically unstable or in the case of an emergency.

If a patient is unable to provide feedback to their family, the medical team involved will communicate with the family via email. Updates on the patient will be given to the next of kin/contact person as needed.

Please note, discharge times are between 10:00am and 11:00am.

Should there be an enquiry about a patient in the facility, we ask that the respective rehabilitation team be contacted.

VISITATION

POLICY

The visiting hours are from 10:00am-19:00pm. We understand that family and friends play a vital role in the recovery process of patients, and we encourage support during a patient’s stay.

We allow two visitors per patient, and each visitor can stay for an hour in the morning and an hour in the evening. We kindly ask that visitors adhere to the designated visiting hours to ensure a smooth and efficient rehabilitation process for our patients.

Furthermore, we request that visitors wait in the coffee shop should the rehabilitation team need to work with the patient. We understand that this can be inconvenient, but it is crucial to ensure the best possible care and rehabilitation outcomes for our patients.

Please note that the visiting hours are subject to change based on the patient’s condition, treatment plan, or any other unforeseen circumstances. We will notify visitors of any changes in visiting hours as soon as possible.

Please do not bring any food items or alcohol during the visit. Should visitors wish to bring alcohol, this needs to be authorised by the patient’s doctor in the facility.

STEP DOWN ADMISSIONS

Step down admissions take place on a referral basis from the hospital.

MEDICAL AID PATIENTS

1. At least 48 hours before discharge from Hospital: referring Doctor completes Subacute Authorisation & Patient Referral Form and sends to casemanager@faircape.co.za.

2. Faircape Health Tokai Case Manager submits to medical aid for approval.

3. Once authorisation has been received, F aircape Health Tokai staff liaises with the treating doctor to confirm when the patient will be ready for transfer.

4. Once discharge date is confirmed, the patient is seen for assessment within 48 hours before transfer is accepted.

5. Depending on the medical aid, Faircape Health or the hospital will arrange an ambulance to transfer the patient to Faircape Health Tokai Estate.

PRIVATE PATIENTS

1. At least 48 hours before discharge from Hospital: referring Doctor completes Subacute Authorisation & Patient Referral Form and sends to casemanager@faircape.co.za.

2. Case Manager and Nursing Services Manager confirm bed availability.

3. Patient Assessment Sister from Faircape Health visits patient in the hospital to assess the patient.

4. Quotation is provided by Faircape Health to the patient or next of kin.

5. Once quotation is accepted, referring Doctor discharges from hospital and Faircape Health Tokai can arrange ambulance transfer to Faircape Health. Ambulance transfer is for own account.

Please Note:

• Some medical aids do not cover the cost of nappies and consumables. If this is the case, nappies will be charged to your account and will be payable prior to discharge.

WHAT TO BRING

Below is a checklist of what should be brought to Faircape Health Tokai Estate during the stay:

Personal items can also be brought along, such as; a hairdryer, a book, stationery, mobile device and chargers. Valuables should not be brought to the facility.

• If you are in a shared room you may use the earphones provided by the health centre or your personal earphones.

Kindly note, if you are in a private room you will need to supply your own earphones.

Earphones can be purchased for R20.00

Gratuities: As per our company policy, no member of staff is allowed to accept gifts.

HOUSEKEEPING SERVICES

Our professional housekeepers ensure a sanitary environment. They carry out their duties with the highest levels of courtesy and discretion. Housekeeping services include: overall freshening up of rooms and daily linen changes.

KITCHEN SERVICES

Optimum nutrition and deliciously prepared meals are vital for health. Our in-house catering service works in partnership with our group’s dietitian to develop nutritionally balanced meals. Patients and residents are guaranteed inclusive, wholesome and healthy meal offerings.

Our kitchen at Tokai Estate Healthcare is Halaal certified. Kosher meals are subject to additional fees.

Included in the daily service fee:

• Three meals per day

• Special dietary requirements accommodated

Meal choice for every meal

Alcohol consumption is limited & monitored by a registered nurse

Tea and coffee in between meals

• Meal service in-room
