Thank you for choosing Excelsior Wellness. Whether you are here for care, collaboration, or connection, we are honored to serve alongside you.
At Excelsior, we value our commitment to our community, ensuring access for all, regardless of ability to pay. We believe that health is more than the absence of illness; it’s about hope, dignity, access, and possibility. Excelsior offers three pillars of programs and services, creating a full continuum of care through the eight dimensions of wellness defined by the Substance Abuse and Mental Health Services Administration (SAMHSA). These pillars include engagement, integrated healthcare, education, and opportunities for success.
Inside this booklet, you’ll find key information about our services, care philosophy, and how we work together with you, our neighbors and community. We want you to feel informed, empowered, and supported in being a decision maker in your care. Excelsior Wellness is more than one place or one program. Through Excelsior Wellness Center, we offer comprehensive community behavioral health services for individuals and families. At Excelsior Family Medicine, we provide affordable, integrated healthcare rooted in dignity and compassion.
In addition, our Integrated Care Center (ICC) brings it all together, creating a one-stop experience where mental, emotional, and physical health are treated with equal care to support young adults successful transition to an independent, community-based living.

EQUITY AND ACCESSIBILITY:
We are dedicated to providing affordable, high-quality healthcare to all communities, with a focus on the underserved. By removing barriers to healthcare, we ensure equal access for everyone, regardless of race, ethnicity, gender identity, sexual orientation, religion, or socio-economic status.
Our healthcare providers are the foundation of our organization. We foster a supportive work environment that values diversity, encourages professional growth, and promotes collaboration, respect, and open communication. We prioritize work-life balance, reduce burnout, and empower our staff to deliver the best care possible.
We optimize resources to deliver highquality healthcare while minimizing waste and maximizing efficiency through evidence-based practices. We focus on programs and services that improve health outcomes and reduce healthcare costs.
Our ultimate goal is to achieve the best health outcomes for our patients. We provide high-quality, evidence-based healthcare services tailored to each patient's unique needs. By emphasizing preventive care, health education, and early intervention, we measure success through both clinical outcomes and patient satisfaction.
:
We are committed to enhancing the care experience by offering patient-centered, compassionate care that addresses physical, emotional, and spiritual needs. We engage patients by providing education and support to help them make informed decisions. Through collaboration with other healthcare providers, we ensure continuity of care and constantly work to improve the quality of our services.
Excelsior Family Medicine is a trauma informed health clinic with experience in integrated healthcare. Our approach brings together physical and mental health providers to collaborate on services that meet each patient’s needs and goals. Our team includes board certified physicians, nurse practitioners and counselors, dedicated to delivering personalized care. We offer life span health services co-located under one roof.
We focus on preventative care and chronic disease management across the lifespan. Primary Care Providers gather and analyze information regarding the patient’s medical history, mental health, chronic conditions, and health objectives. As the first point of contact for individuals seeking medical care, Primary Care Providers serve as the trusted partners who guide patients on their healthcare journey.
Our Medication Management program is designed to address broad diagnosis and treatment of moderate to severe mental and behavioral health needs. Providers use pharmaceutical medications to help address mental health symptoms, chronic conditions and improve quality of life. This program is designed to be short-term and patients transfer care to their primary care for ongoing medication monitoring.
EXIST is a voluntary, 16-week rolling enrollment offering 12 hours of weekly services, including 9 hours of group sessions and 3 hours of individual sessions for youth ages 14-17 with both substance use and mental health disorders.
Our collaborative care model started in 2024, and offers brief mental health support within the primary care setting. Patients are referred to this service by their primary care provider. A team of care givers, including a community health worker and/or a licensed social worker, partner with patients to set clear health goals, find local resources, and enroll in additional services that align with the express needs.
Our mental health counselors offer a variety of evidence-based therapies, and services are available for all ages. Participants work with a counselor to collaboratively establish personal goals, specifically deciding on what benefits they hope to gain over the course of treatment and what improved mental health and wellness looks like for them.
