Welcome to Liverpool City Council’s new Adult Services and Health booklet. Social care is changing. It had to change. People are living longer, the number of people with caring responsibilities is increasing and, more importantly, people’s expectations have changed.

We hope that you find this booklet useful. As the social care landscape continues to change we will update it, so please tell us what you would like to see in future editions or what you have found particularly useful.

The time has long gone when people might have accepted being given a one-­size-­fits-­all care package. And the time has long gone when local authorities thought that this is acceptable.

Now, people want to be part of making choices around their care. They want to make sure that it enables them to stay part of their community and that it supports their independence. To do this they need to have as much information as possible. Also, as the number of older people in the city increases, more people will be involved in planning and choosing care for family members. In Liverpool, we have plenty of choice but this can sometimes be as overwhelming as it is helpful. We also know that people can find the social care system itself confusing with its assessments and bandings. That’s why we have produced this booklet -­ to give you an idea of some of the services available as well as tell you about what you can expect from the council if you or a family member is referred to us for an assessment. No matter what service you choose, whether it is provided by the council or from another organisation, in Liverpool you can be confident in the care you receive. The Clinical Commissioning Group and all organisations that provide services on our behalf have signed our Dignity in Care Charter that sets out how you can expect to be treated. Careline  &DUHOLQH LV $GXOW 6HUYLFHV DQG +HDOWK¶V contact  centre.  Careline  is  available  24  hours  a  day  seven  days  a  week  and  is  VWDIIHG E\ WUDLQHG DQG TXDOLILHG DGYLVHUV and  social  workers.  Careline  can  provide  information  and  advice  about  the  services  available  from  Adult  Services  and  can  refer  you  for  an  assessment  of  your  needs.  The  adviser  can  also  signpost  you  to  alternative  sources  of  support  in  your  local  area.  Â

Assessment  and  eligibility  Some  of  the  services  described  in  this  booklet  can  only  be  accessed  following  an  assessment  of  need.  If  you  contact  us  the  advisor  will  discuss  with  you  the  help  and  support  you  need  and  may  make  a  referral  to  Adult  Services  for  an  assessment  or  signpost  you  to  the  most  appropriate  service  to  meet  your  needs.  If  you  need  care  services  you  may  be  asked  to  complete  a  Supported  Self-­  Assessment  form,  and  a  meeting  will  be  arranged  with  a  social  worker  or  assessor  of  personalised  care  to  discuss  your  circumstances  and  what  support  you  think  you  need  to  help  you  live  at  home.   If  you  wish  you  can  be  supported  at  your  assessment  by  a  family  member,  friend,  or  carer.   You  can  also  be  supported  by  an  independent  advocate  if  you  prefer,  and  you  can  find  details  of  who  to  speak  to  about  advocacy  later  in  this  booklet. 6

During  your  assessment  we  will  talk  to  you  about  how  you  manage  your  day  to  day  life.   This  will  help  us  determine  if  you  are  eligible  IRU VHUYLFHV IURP XV +RZHYHU HYHQ LI you’re  not  eligible  for  council  funded  support  then  we  will  do  our  best  to  give  you  information  that  should  assist  you  to  improve  your  situation.   If  you  are  eligible  for  services  we  will  work  with  you  and  your  carer  to  develop  a  support  plan. If  you  are  referred  to  another  agency  they  will  explain  what  their  assessment  process  will  involve  and  what  criteria  you  need  to  meet.

Eligibility  for  social  care  services  Â

All  local  authority  social  care  departments  PXVW XVH WKH 'HSDUWPHQW RI +HDOWK guidance  Prioritising  Need  in  the  Context  of  3XWWLQJ 3HRSOH )LUVW 'HSDUWPHQW RI +HDOWK 2010),  when  deciding  who  is  eligible  for  a  service.   There  are  four  categories  of  need: Â

A  Support  Plan  describes  what  you  would  like  to  achieve,  for  example  living  independently  at  home  and  how  you  can  achieve  this.  These  are  called  outcomes  and  this  is  how  we  measure  whether  the  support  you  receive  is  helping  you.  Â

‡ /RZ ‡ 0RGHUDWH ‡ 6XEVWDQWLDO ‡ &ULWLFDO

Your  assessor  will  work  with  you  to  create  your  support  plan  which  will  identify  how  you  will  achieve  the  outcomes  described  in  your  care  plan.   Your  support  plan  can  include  services  provided  by  us  as  well  as  support  you  get  from  family  and  friends.   Â

In  Liverpool  we  only  provide  services  to  people  who  are  assessed  as  having  a  substantial  or  critical  level  of  need.  You  can  find  more  information  on  the  eligibility  criteria  on  our  website  or  by  contacting  the  information  officer  0151  233  4437  for  Adult  Services  who  can  send  you  a  fact  sheet  and  a  copy  of  the  guidance  for  Liverpool. Some  of  the  services  we  commission  are  not  subject  to  the  council’s  eligibility  criteria,  and  more  information  on  these  services  on  how  to  access  these  services  can  be  found  on  the  city  council  website  www.liverpool.gov.uk Usually  these  services  will  have  their  own  criteria  for  who  they  can  help  and  Careline  can  advise  you  if  the  services  would  be  suitable  for  you.  Â

If  you  are  not  eligible  for  support  we  will  still  give  you  information  and  advice  and  can  signpost  you  to  alternative  services  and  groups  in  your  local  area. Â

Safeguarding  adults  Adults  of  all  ages  can  be  at  risk  of  abuse.  +RZHYHU EHFDXVH RI DJH RU GLVDELOLW\ VRPH people  may  be  at  an  increased  risk  of   abuse,  be  it  physical,  sexual,  emotional,  or  financial.  Because  of  increased  vulnerability  this  can  sometimes  make  it  difficult  for  someone  who  is  the  victim  of  abuse  to  report  it  themselves.   Additionally,  the  abuse  could  be  by  a  family  member,  a  friend,  a  care  worker  or  even  a  stranger.   If  you  are  concerned  that  someone  you  know  may  be  being  abused  or  is  at  risk  of  harm  please  contact  Careline  on:  0151  233  3800. You  can  make  the  referral  anonymously  if  you  wish. If  you  are  concerned  that  the  person  is  in  immediate  danger  you  should  call  999  and  ask  for  the  Police. Â

Working creatively, we have helped people design their own flats and have then supported them to move to new, purposebuilt, accessible accommodation. Turning dreams into reality! At our workshops, we offer a range of craft activities or the opportunity to work in the garden with the animals. Self-­directed  support  and  personalisation You  may  have  heard  self-­directed  support  or  personalisation  being  talked  about.   In  Adult  6HUYLFHV DQG +HDOWK WKLV PHDQV WKDW ZH ZLOO listen  to  you  and  the  people  who  know  you  well  when  deciding  what  support  would  help  you  live  the  way  you  wish.  We  will  do  our  best  to  help  you  to  find  suitable  support  that  meets  your  needs  and  achieve  the  goals  that  you  have  in  regard  to  your  health  and  wellbeing.  This   may  include  using  different  services  or  providing  support  to  help  you  use  the  right  services  for  you.   You  can  talk  to  your  assessor  about  the  way  for  you  to  get  the  services  that  best  suit  your  needs  and  are  right  for  you.   This  could  either  be  through  services  provided  by  the  council  or  by  using  a  direct  payment  to  purchase  your  own  care  and  support.

Personal  budgets  A  personal  budget  is  the  amount  of  money  that  is  allocated  to  you  to  meet  your  care  and  support  needs  identified  in  your  care  or  support  plan.   Your  personal  budget  can  either  be  in  the  form  of  a  direct  payment  or  as  services  that  are  arranged  by  us. Â

Direct  payments  A  direct  payment  is  a  different  way  of  getting  the  service  that  you  need.   Instead  of  providing  a  service,  Liverpool  City  Council  will  give  you  the  money  which  you  are  then  able  to  spend  on  getting  the  services  you  need  to  achieve  the  outcomes  identified  in  your  support  plan.   A  direct  payment  offers  more  flexibility  and  choice  and  control  in  the  way  your  services  are  provided  than  would  be  available  by  using  the  council  provided  care  and  support.   You  can  choose  the  services  you  use,  who  you  get  them  from  and  when  you  get  them. Â

Employing a personal assistant (PA) You may decide to use your direct payment to employ a personal assistant (PA). Employing your own PA means that you can have more control over when and how your care and support is provided. If you decide to employ a PA you will become an employer and will have some legal obligations. This might sound daunting, but the Self Directed Support Team can help you with all aspects of recruiting and employing a PA including help with payroll and setting up insurance. There are also independent organisations that can help you with the recruitment and the self directed support team can put you in touch with them. When you are choosing a PA, you cannot employ someone who lives in the same house as you. Choosing a care provider If you decide to use your direct payment to employ an agency here are some things that you need to ask before deciding which agency to choose. All agencies providing personal care must be registered with the Care Quality Commission (CQC) – the social care regulatory body for England.The CQC will also carry out inspections and you can check for any recent inspections on the CQC website. Help  to  live  at  home Â

Equipment  and  adaptations Â

Our  aim  is  to  support  you  to  live  as  independently  as  possible  in  your  own  home  for  as  long  as  possible  and  there  are  a  number  of  things  that  we  can  discuss  with  you  that  may  help  you  to  stay  at  home.

If  you,  or  a  member  of  your  family,  are  having  difficulty  getting  around  your  home  due  to  a  physical  disability,  sensory  LPSDLUPHQW RU ROG DJH \RX FDQ UHTXHVW DQ DVVHVVPHQW IRU HTXLSPHQW DQG DGDSWDWLRQV to  make  it  easier  for  you  to  live  independently.  Â

Home  care  +RPH FDUHUV FDQ SURYLGH SHUVRQDO FDUH DQG support  to  help  you  live  at  home,  this  can  include  helping  you  get  in  and  out  of  bed,  washing  and  dressing  and  supporting  you  to  make  a  meal. Â

Help with cooking

7HOHFDUH LV D UDQJH RI HTXLSPHQW ZKLFK FDQ help you live at home independently. This can be alarms or sensors that automatically activate to let someone know if you have had a fall for example.

Liverpool City council works in partnership with Wiltshire Farm foods to provide a frozen meals input service.

A Telecare base unit will be installed in your home which is connected to your phone line. This links to a contact centre which is available 24 hours a day 7 days a week. A series of sensors are linked wirelessly to the base unit and if one of the sensors is triggered for example if you have a fall, an alarm is activated. We can identify you from the alarm and speak to you via the base unit to find out what the problem is. We will then take appropriate action, for example, contact your carer or the emergency services. If you do not have a carer we will send out D FDUHU IURP RXU +RPH 5HVSRQVH 6HUYLFH If it is a false alarm, we can cancel the call. Managing common ailments and how to contact a GP in the out of hours period Liverpool Clinical Commissioning Group (CCG) is encouraging patients in the city to make more use of the GP ‘Out of Hours’ service after research showed there are misconceptions about what the service is, which has led to some patients either delaying treatment or going to Accident and Emergency (A&E). Did you know that many common ailments, like colds, coughs and flu, can be effectively treated by your pharmacist, saving an unnecessary trip to the GP? But for patients requiring medical attention then the next port of call should be their GP. Local practices are generally open between the hours of 8am-6.30pm on Mondays to Fridays and if the condition cannot wait until the surgery is open, then the Out of Hours service is there to help. It can be accessed 24 hours a day simply by calling your local practice number. Dr Jim Cuthbert is a local Liverpool GP and Urgent Care lead for Liverpool CCG, the NHS organisation which is responsible for commissioning community and hospital services. He explains: “We know that some of our patients are still unaware of the GP Out of Hours service in Liverpool and they don’t know that they can speak to a GP outside of normal surgery hours. Some patients believe that calling the surgery out of hours will result in disturbing their doctor, or they will only be able to see a GP who is unfamiliar with their medical history. “As a result we see many patients attending A&E as non-emergencies, or waiting in pain and discomfort until the next working day to get an appointment with their regular GP. “The Out of Hours service is run by local GPs, most of whom work in practices across Liverpool. Although they may not be your regular GP, in the majority of cases following patient consent, they can access a summary of your medical records and be aware of your main medical history. “The GP Out of Hours service is one of top rated services in the country and can provide advice over the phone and face to face appointments. Home consultations are available for those who are housebound or are too frail to come to a Centre for a review. You can even seek advice if you are away from home and become unwell. “We want to make it clear to patients across Liverpool, that local GPs are accessible 24 hours a day – even out of hours and on Christmas Day. So please don’t be worried about calling the GP Out of Hours service, it is there if you need it.”

Day  opportunities As  people  now  want  to  take  part  in  a  wider  range  of  activities  during  the  day,  the  council  needed  to  expand  the  way  it  offers  support  beyond  traditional  day  centre  services.  Now,  we  are  working  with  people  who  use  our  services  to  make  sure  that  the  support  they  receive  is  exactly  what  they  need  to  achieve  the  outcomes  on  their  support  plan.  So  that  we  can  do  this  to  the  highest  standard,  we  are  re-­modelling  some  of  our  existing  buildings  into  community  hubs  which  will  not  only  provide  the  very  best  environments  for  the  people  who  already  use  our  services  but  can  also  be  used  by  other  groups.  Work  has  already  started  on  transforming  the  former  Lime  Court  day  centre,  on  Upper  Baker  Street,  into  The  Lime.  This  hub  will  have  the  city’s  only  hydrotherapy  spa  and  will  also  feature  a  recording  studio  and  internet  café.  Meanwhile  at  Lancaster  'D\ &HQWUH 5LFH /DQH WKH EXLOGLQJ ZLOO be  improved  and  the  outside  space  will  also  be  extensively  developed. 7KH WKLUG KXE ZLOO EH DW 6SHNH 5HVRXUFH Centre,  Parklands,  and  satellite  day  services  also  operate  from  Alderwood  Day  Centre,  %ULGJH &KDSHO &HQWUH +HDWK 5RDG DQG WKH / &HQWUH 6HOERXUQH 6WUHHW

Re-­ablement  services As  the  population  continues  to  age,  there  will  be  more  and  more  demand  on  care  services.  In  Liverpool  we  decided  that  providing  services  that  supported  people  to  continue  living  independently,  rather  than  providing  long-­term  residential  care,  would  mean  that  we  could  help  more  people. With  this  in  mind,  our  three  former  care  KRPHV 6HGJHPRRU *UDQE\ DQG 9HQPRUH now  offer  short-­term  re-­ablement  services.  This  means  the  services  they  offer  will  help  people  to  get  back  on  their  feet  after  they  had  been  in  hospital  or  we  will  work  with  people  to  prevent  them  from  being  admitted  to  hospital. 9HQPRUH ZLOO FRQWLQXH WR RIIHU LWV VSHFLDOLVW service  helping  people  to  rebuild  their  skills  and  confidence  after  having  a  stroke  that  also  includes  a  bed-­based  service.  Generic  re-­ablement  care  is  offered  from  Granby,  Selbourne  Street,  which  will  support  people,  for  example  who  have  had  falls  or  who  might  need  support  to  socialise.  Building  work  on  all  hubs  is  expected  to  be  completed  by  the  end  of  2013.  The  first  of  our  re-­ablement  hubs  to  be  completed  is  Sedgemoor,  our  specialist  dementia  support  centre,  which  was  officially  opened  in  May  2013.  This  over-­£1m  facility  extends  the  existing  building  and  now  includes  a  4D  interactive  suite  to  help  with  immersive  therapy  and  a  sensory  garden.  The  day  services  at  9HQPRUH 0HZV DQG 1RUULV *UHHQ ZLOO now  also  be  provided  from  Sedgemoor.
