CEO'sMessage
This is a special time in our organisation’s history as we celebrate our successes and tremendous growth over the past 10 years. Tiara Haemophilia and Cancer Foundation began its journey in 2012, and in the years that followed, we have continued to build and expand our organisation. We are proud that the name Tiara has become synonymous with integrity and transparency, and our commitment to improving the health of children from underprivileged backgrounds.
THCF as we were previously known, is now Tiara. As we look forward to the future, we are focused on a broader vision - health equity for all children in India. Our new logo and brand identity embody this new mission.
We are bringing together key services, people, and organisations driven by the same goal: to provide strong local support for children growing up in underprivileged areas, giving them a better start in life. Our innovative, tailored efforts will tackle the cycle of health from the start to give all children equal opportunities to be healthy.
We are also excited to expand our medical aid projects to include disease prevention; and rehabilitation services for children with developmental delays. With new leadership and a committed team, we are confident in our ability to make positive changes in the communities where we live and work.
I am deeply grateful to each of our advisors, donors, community leaders, and nonprofits for supporting our mission in a remarkable way. Your generosity and commitment touched our hearts and helped us achieve more than we ever thought possible. We have been overwhelmed by the outpouring of support and inquiries on how you could help. We stand steadfast with our teams and network of partners, allies, and supporters to build a more equitable India where no child is left behind. The best f Ti i t t !

Chairman'sMessage
It gives me immense pleasure to connect with you through this message.
Children are the future of our country and the world. It is the responsibility of Society to see that in their developing years, they are healthy and fit. Any impact on the health of a child has a cascading effect not only on the child’s development, education, and emotional well-being but also has an impact on the caregivers.
The journey of Tiara Haemophilia and Cancer Foundation (THCF) began with a vision of Aparna Shyam to support children with Haemophilia and Childhood Cancer across the country. With over 700 children spread over 17 states supported, over a period of 10 years, THCF has indeed touched the lives of these children and held their tender hands, and helped them to smile.
Identifying the need and passion to bring equitable and holistic health care for children, THCF has transformed and rebranded as Tiara, which has a singleminded goal of achieving Health Equity for all children.
Over the years we have received tremendous support from donors and the industry which told us “We can dare to Dream”. We have made a small beginning, but we have miles to go and I don’t think we can ever sleep…
We are very grateful to all the donors who have been helping us and travelling this journey together with us. We seek your continued support and partnership in this journey. Together, we can do more. When we see a child smile, society smiles and the world becomes a better place! There is no greater satisfaction than seeing a healthy next generation.

MeetRiya
Riya is a 7-year-old cute girl who is eagerly expecting her mother to deliver a baby brother and is celebrated as a princess by her father. To be a princess doesn’t need immense wealth alone and in short of it, her father tries his best not to let her palpate the financial struggles that he is going through.
All was well till Riya was diagnosed with high grade B non Hodgkin’s lymphoma in December 2020. The family was shattered when the swelling in the face and abdomen was evaluated and told to be a malignancy. B NHL is highly curable and with scientific hopes, the family was motivated to initiate treatment. However, financial implications being huge, is not an easy task for a lower-middle-class family where father is working as a server in a hotel and the sole bread winner for the family. Thanks to the administration of Sri Ramachandra Hospital G block and the much more vital support in the form of Tiara, the family could tide through the crisis and see the girl improve.
Misfortunes often come single. After enjoying her disease-free period for a year, she had a disease relapse preventing her from walking due to spinal infiltration. The family just recovering mentally and financially would have been shattered again but for the huge help from good hearted donors and Mrs. Aparna Shyam from Tiara It is often painful to see someone deny treatment for want of money and we were at least able to help a few in the lots with cancer including Riya, with the huge hand holding from THCF. The timely financial support from Tiara has always been vital in the management of these kids who are waiting to see the light at the end of the tunnel.

SankaraNethralaya
Retinoblastoma is a childhood cancer of the eye Sankara Nethralaya has been a pioneer in this management of Retinoblastoma since 40 years and receives around 110 children a year.
This disease commonly affects families belonging to low socioeconomic status. More than 98 percent of these children receive cost less treatment through funding received from Sankara Nethralaya, Tiara, Sri Ramachandra Hospital and other philanthropists.
Treatment for retinoblastoma consists of chemotherapy via various routes Intraarterial chemotherapy (IAC) is a very sophisticated treatment given for advanced intraocular cancers and gives good results.
43 cycles of IAC has been completely funded by Tiara These children are fortunate to have received this support without which we would not have been able to save the eye and vision.
Mst. Kavinesh, 9 year old son of a driver from Chennai who presented to us when he was 8 months old with Retinoblastoma in both eyes. He received multiple cycles of chemotherapy through his veins along with laser. I had to perform a complicated retinal surgery which is very rarely done in children with retinoblastoma.
His surgery was done free of cost in Sankara Nethralaya and following this, he received 3 cycles of expensive IAC supported by Tiara which has helped Kavinesh to recover from his eye cancer.
The list of beneficiaries from Tiara is long. A few others who received IAC are Mst Phanindra from Vishakapatnam, 2-year-old son of a temple priest; and Miss Noori Jennet Riddi, 8year-old daughter of hotel steward from Bangladesh
Children with Retinoblastoma that has already spread to the brain is a huge challenge and needs a multidisciplinary approach Mst Pranav, the son of a street vendor had involvement of the eye and brain with retinoblastoma. He had his 12 cycles of chemotherapy done in SRIHER with the support received from Tiara followed by radiation. He now leads a normal life and is currently in 3rd standard in a Government school.
We have innumerable happy stories to share which have bonded Sankara Nethralaya and Tiara together in this noble deed. We and our patients will ever be grateful to Mrs Aparna Guhan Shyam, founder of Tiara for the unconditional support extended.

CustomisedOcular Prosthesis
Baby Thanavitha was ten months old when her parents, Kumari and Babu Rao Goddu of Srikakulam (north-eastern Andhra Pradesh), noticed a shining white gleam glimmering from inside her eye. It took another five months for them to be able to consult a doctor, who diagnosed it as possible retinoblastoma and referred them to the L V Prasad Eye Institute
Here, Dr. Swathi Kaliki, Head of the Operation Eyesight Universal Institute for Eye Cancer, examined the child under painless anesthesia and began treatment for saving the girl’s life, eyes, and vision. She reassured Kumari and Babu Rao that Thanavitha would have vision in at least one eye and started chemotherapy injections After the sixth chemotherapy injection, the cancer was still seen as aggressive in the child’s right eye. Dr. Kaliki was forced to inform Kumari that Thanavitha’s right eye needed to be removed. Although shocked and scared, Kumari agreed to save her daughter’s life.
Thanavitha’s family took the diagnosis very badly Her father and paternal grandparents had already faced a similar issue with Babu Rao, whose left eye was shrunken since childhood due to an injury. Thanavitha’s father had been teased all through school and became deeply depressed that his daughter might have a similar fate. Despite the support and encouragement his young wife extended to him, he died after a few weeks, having lost the will to live. Within a few days, Thanavitha underwent a procedure called evisceration
to remove the cancerous parts of her eye
The newly widowed Kumari faced multiple challenges – the agony of being alone, facing censure and criticism from her in-laws, financial issues due to the COVID-19 lockdown, and more. She shifted to her parent’s home, taking the child along, and made several trips for follow-up treatment – but was subjected to more hardship when the Indian Railways counter in Hyderabad denied her the subsidies due to her as the attendant of a child suffering from cancer. She appealed to Dr. Kaliki for help, who recommended her case to Tiara.
Tiara helped Thanavitha get fitted with a customised ocular prosthesis, an artificial eye. It helps the left eye look almost normal Kumari and Thanavitha are now trying to live as normal a life as possible. Between hospital visits for monitoring cancer, Kumari is at home with the child, taking her to the anganwadi crèche, watching her play, dance, sing, talk, and smile.
"The cancer was very aggressive in the right eye, and it had to be removed. My husband, having suffered an eye injury himself, took this very badly, and became so depressed that he lost the will to live. This was extremely difficult and left me as a widowed mother with a baby to save. Many more difficulties stood as obstacles in my way, till THCF stepped in. Their support enabled Thanavitha to be fitted with an artificial eye. She now smiles all the time, as she lives, sings, talks and plays. I am grateful for the support Thanavitha has receivedfromTHCF.Thankyou.”

MeetSeshathri
Seshathri is a 16-year-old boy who had been well till October 2022. He developed breathlessness and pallor and was found to have severe anemia. A student of class XI with dreams of getting into higher education and hope of his parents was diagnosed with a life-threatening disorder of auto-immune hemolytic anemia (AIHA) To add to the woes and not fitting into the diagnosis of AIHA, he had profuse bleeding tendencies during hospital admission from the prick sites. A detailed evaluation suggested a diagnosis of ACQUIRED HAEMOPHILIA, one of the very rare disorders in children.
This has been a huge blow to the family. Mother being a librarian in a small village and father being a construction-site worker, couldn’t digest these hard facts He improved transiently with steroids and as his parents had significant social difficulties and financial constraints, went to his native place with ongoing medications.
He remained well for 6 weeks and had a second major blow in form of intracranial hemorrhage (bleeding into brain) requiring ICU care in December 2022. There was cyclone Mandous troubling Chennai at the same time and this was a bigger storm for parents. Being a haemophilia with Acquired inhibitors, the routine factor concentrates or blood products wouldn’t help him much.
He needed NOVOSEVEN for 72 hours to control the bleeds followed by RITUXIMAB (immunosuppression) and EMICIZUMAB (prophylaxis to prevent further bleeds) for the next 4 weeks
Making this medical plan was easy; however, executing this was the challenging part as the estimation for this was Rs. 5,50,000/- excluding the supportive measures required in PICU.
Miracles do happen The Implausible job was MADE POSSIBLE with immense support from TIARA. Not only approving the support but getting the medicines delivered despite the PHYSICAL STORM outside was highly valued.
Nothing can be equivalent to the joy of seeing a smile on parents’ faces to get their child discharged after such a stormy course.
On behalf of the Medical Team, we sincerely thank and appreciate the support of Tiara and Mrs. Aparna Shyam to have made this possible and thank on behalf of the parents who are left with no words to reciprocate their gratitude.

Multidisciplinary Clinicfor Haemophilia
Children with haemophilia need to be closely monitored with plenty of follow-up to ensure a good quality of life. Our multidisciplinary clinic provides doctors and parents with an opportunity to meet and update each other on the progress of our warriors.
These clinics have haematologists, orthopaedic specialists, and physiotherapists to provide all-round care. Parents can bring up concerns or questions and doctors can then help them understand and adapt better. The clinic is also open to our VWD warriors who are eligible for all support provided for Haemophilia.

MeetHemavathy
Von willebrand disease (VWD) is a genetic disorder with an increased bleeding tendency. Type 3 VWD is very rare and a severe disorder with frequent bleeds and the need for significant supportive care Women empowerment is being talked about and motivated. Yet, in our patriarchal society, women face a lot of challenges to get what they need and especially when she is not backed by a man.
Hemavathy's family is one such family which is led by the mother who is the bread winner of the family. She is chronically ill and has 3 girls of the adolescent age group. Mother is a daily wage labourer who does household chores in 4-5 places to make the ends meet.
Poverty, managing girls without male member support, and blaming yet uncaring relatives are significant challenges for Hemavathy's mother. Yet she has lived her life with great hopes and has been striving hard to educate all 3 girls with great vision. Destiny struck her hard – Hemavathy who has been having recurrent bleeds requiring transfusion once or twice was admitted under us for hemarthrosis and gum bleeds.
She was diagnosed with VWD type 3 a very challenging disease not only for the doctors but also for the girl and her mother More so, when the girl was admitted with us on a Sunday afternoon with significant gastrointestinal bleed with shock in PICU.
"She was admitted with us on a Sunday afternoonwithsignificantgastrointestinalbleed withshockinPICU thesupportworthRs 4lac forhertreatmentandprophylaxisfurtherwould havebeenunimaginableevenforamiddleclass familytohavemadeitpossible."
Her hopes towards the universe were answered at an appropriate time She was admitted to the G block subsidised PICU ward at Sri Ramachandra Hospital and all the necessary factors and blood products she needed for the next 7 days were completely supported by THCF free of cost Not to exaggerate or measure, the support worth Rs. 4 lac for her treatment and prophylaxis further would have been unimaginable even for a middle-class family to have made it possible.
The girl with her ambitions to excel in class 12 to take her family to the next step was overwhelmed with this support without which she could have been crippled due to her complications The mother was speechless and the eyes spoke 1000 thank yous
We submit the gratitude of the family to the Tiara donors and Mrs. Aparna Shyam who have made these miracles possible. The thankful gestures would always stay in our memories to motivate us further to do better.

COMPLEXITY OF THE SURGERY
32
MeetKalaiselvi
Kalaiselvi, a 14-year-old girl, suffers from an extremely complex congenital cardiac condition. Basically, the heart normally has 4 chambers and 4 valves. This girl was born with a totally wrecked heart with the chambers all deformed and the valve supplying blood to the lungs narrowed and there were multiple holes in the heart. The baby had presented with cyanosis (blue-baby syndrome).
The options for surgery are limited in such a child. The holes cannot be closed as they keep the child alive in this case. It was like a bomb had been dropped in the middle of the heart at birth. The child had a procedure called Glenn as a child. This diverts the venous blood from the brain to vessels flowing towards the lungs thereby bypassing the block in the pathway to the lungs.
However, by the time the child came to us, the child was blue again The Glenn procedure was not sufficient to sustain the oxygen requirements of the child anymore.
A phone call was made to Ms Aparna of TIARA and the predicament was explained. We requested donations. This was sanctioned right away. We did the investigations.
We found an anomaly in the investigations. In a Fontan operation, we direct the venous return from the lower body to the lungs. However, there was a technical challenge - one of the veins coming back from the liver was separate.
The team came up with an innovative plan - a variant of Fontan with intra-extra cardiac Fontan and a separate Y-graft from the hepatic vein.
We were able to proceed with such an innovative plan due to the financial backing received from TIARA. They immediately came to our aid. These surgeries are very rare (Y- intraextra-cardiac Fontan)
Dr. Neville Solomon Head, Department of Pediatric Cardiac Surgery
Dr C S Muthukumaran Head, Department of Pediatric Cardiology Apollo Children's Hospital Chennai
"Kalaiselvi needed to undergo a complex second open heart surgery called Fontan. The father is a painter and the mother a homemaker. They could not even afford the investigations like angiography and CT scan let alone the complex openheartsurgery."

Regina Cassandra is an actor by profession but an activist at heart. Having worked in films when she was just 9 years old, Regina has been in the industry for almost 20 years now, taking up unique roles with an individualistic flair. Admirably, she has never shied away from standing up for difficult social issues, making her one of the most candid and outspoken actors in South India.
Tiara’s work is close to her heart due to the tragic fact that children afflicted with haemophilia and cancer are stripped of any chance of having a childhood. With her quick wit and passionate perspective, she hopes to spread awareness about haemophilia and childhood cancer as well as the impact that Tiara has on these young lives.
