PALLIATIVE AND END OF LIFE NEWSLETTER FOR CARE COMMUNITIES What does EoLP do.. ?

EoLP have been commissioned by Cheshire CCG to agree and deliver a programme of support to help improve palliative and end of life care. This support will be focused around care communities.

What are our aims and objectives? Improvements in one or more of the Strategic Collaborative Cheshire High Level Outcomes Increase in the Knowledge, skills and confidence of the workforce and community members in end of life care related topics including supporting people who are bereaved One or more of the following (to be chosen by each Care Community): Increase of people achieving their preferred place of care/death AND understand reasons why recorded preferences are not achieved Increase in people in the last 12 months of life that are identified & offered personalised care planning Decrease in proportion of people having 1 or more emergency admission in last 90 days of life Collection and anaylsis of feedback on end of life experience of patients, carers, professional and communities to inform practice and commissioning

Which Care Communities ? Using an equality and quality impact assessment process the following 4 Care Communities (2 in East, 2 in West Cheshire) have been chosen for the initial pilot project East : Bollington, Poynton & Disley and Nantwich & Rural West : Chester South and Winsford

Our Impact We'll be feeding back the impact made in areas such as dementia prevalence, palliative care registers, admissions in the last 90 days of life, care home engagement in end of life training and where people are dying
