PALLIATIVE AND END OF LIFE NEWSLETTER FOR CARE COMMUNITIES What does EoLP do.. ?

EoLP have been commissioned by Cheshire CCG to agree and deliver a programme of support to help improve palliative and end of life care. This support will be focused around care communities.

What are our aims and objectives?

Improvements in one or more of the Strategic Collaborative Cheshire High Level Outcomes Increase in the Knowledge, skills and confidence of the workforce and community members in end of life care related topics including supporting people who are bereaved One or more of the following (to be chosen by each Care Community): Increase of people achieving their preferred place of care/death AND understand reasons why recorded preferences are not achieved Increase in people in the last 12 months of life that are identified & offered personalised care planning Decrease in proportion of people having 1 or more emergency admission in last 90 days of life Collection and anaylsis of feedback on end of life experience of patients, carers, professional and communities to inform practice and commissioning

Which Care Communities ?

Using an equality and quality impact assessment process the following 4 Care Communities (2 in East, 2 in West Cheshire) have been chosen for the initial pilot project East : Bollington, Poynton & Disley and Nantwich & Rural West : Chester South and Winsford

Our Impact We'll be feeding back the impact made in areas such as dementia prevalence, palliative care registers, admissions in the last 90 days of life, care home engagement in end of life training and where people are dying

Chester South

EoLP have joined two care community team meetings.

EoLP have met with the community nursing team manager, clinical lead and with one of the GPs from the pilot practice. They have identified their local outcome as : “a % increase in people in the last 12 months of life that are identified & offered personalised care planning”

The team have defined their local outcome measure as: "A % increase in people in the last 12 months of life who are identified & offered personalised care planning" Priority areas of work are starting to be defined to assist with the achievement of this outcome i.e. development of a joint MDT, dementia and compassionate communities support to local people through the ‘time to talk’ initiative and targeted care home support.

Nantwich & Rural EoLP have held productive initial conversations with key people from CCICP, the CCG and with the PCN clinical lead There will be a presentation from EoLP at the next care community meeting in October to discuss the launch of the pilot.

EoLP will be attending the PCN meeting in October to advise on our progress, share our work plan and gain agreement to proceed with the pilot site.

Oak Grange and The Willows have been identified as the care homes that will be completing the "Six Steps to Success End of Life Care Programme".

Winsford EoLP have met with the community nursing team manager & the care community service manager. They have identified their local outcome as “a % increase in people achieving their PPoC/PPoD and where recorded preferences are not achieved gain a better understanding of the reasons for this” We may look at a second outcome in this locality.

Overdene has been identified to complete the Six Steps Programme.

WORK ACROSS ALL CARE COMMUNITIES We've been....

developing key relationships agreeing local outcomes and how these will be evidenced sharing ideas about the type of activities we will work on together to achieve these outcomes formalising agreements between the partners that we will be working alongside

Training Needs Analysis & Education

We have surveyed each care community to allow us to understand where we can have the most impact through education. As well as the more practical courses, Verification of Expected Death or Syringe pump, we offer training on topics such as dementia, communication skills, EPACCS and Advance Care Planning. Delivered bespoke within the care community, or as part of EoLP's core education programme: see our website for all our offerings

Carers Pathway To help with earlier identification of unpaid "hidden" carers in the community. The pathway will provide a consistent approach to ensure all carer can access support and opportunities.

The Advanced Dementia Support Team will be working with Care Communities to ensure that dementia education and advice is available to both professionals and family carers.
